4 Effects of dehydrocorydaline on pain behavior and spinal cord inflammatory factor expression in chronic inflammatory pain mice

Objective To observe the effects of dehydrocorinine (DHC) on hyperalgesia and spinal cord inflammatory cytokine expression in chronic inflammatory pain mice. Methods According to the random number table method, 30 male ICR mice were divided into three groups: Sham group, complete Freund’s adjuvant (CFA) group and CFA+DHC group, with ten mice in each group. Mice in Sham group were subcutaneously injected with 25 μL normal saline in the middle of the right hind paw. Mice in CFA group and CFA+DHC group were subcutaneously injected with 25 μL CFA into the middle part of right hind paw. From one to seven days after modeling, mice in the CFA+DHC group were intraperitoneally injected with 10 mg/kg DHC every day, and mice in the CFA group were intraperitoneally injected with solvent, and the paw withdrawal mechanical threshold (PWMT) and paw withdrawal latency (PWL) of the mice were determined 30 min after drug administration. On the seventh day, lumbar enlargement was taken, and the protein expressions of the proinflammatory cytokines tumor necrosis factor-α (TNF-α), interleukin (IL)-1β, IL-6 and anti-inflammatory cytokines IL-10 were detected by Western blot. Results Overall analysis showed that there were statistically significant differences in the inter-group comparison, time point comparison and interaction between PWMT and PWL (all P < 0.05). Further comparisons were made in pairs. Intra-group comparison: comparison of PWMT and PWL before and after modeling in Sham group showed no statistically significant difference (P > 0.05). After modeling, the PWMT of CFA group and CFA+DHC group were lower than those before modeling, and the PWL was shorter than those before modeling, with statistically significant differences (all P < 0.05). Comparison between groups: before modeling, the basic values of PWMT and PWL of the three groups showed no statistically significant difference (P > 0.05). From one to seven days after modeling, compared with CFA group, PWMT and PWL in CFA+DHC group were increased and prolonged, and the differences were statistically significant (all P < 0.05). Compared with the CFA group, the expression levels of proinflammatory cytokines TNF-α, IL-1β and IL-6 in the CFA+DHC group were decreased, while the expression levels of anti-inflammatory cytokines IL-10 were increased, with statistically significant differences (P < 0.05). Conclusion DHC can effectively relieve hyperalgesia of chronic inflammatory pain induced by CFA in mice, and the mechanism may be related to inhibition of the expression of proinflammatory cytokines TNF-α, IL-1β, and IL-6 and increase of the expression of anti-inflammatory cytokines IL-10.

8 Inhibitory effect of deep-sea fish oil on stress gastric ulcer

Objective To explore the inhibitory effect of deep-sea fish oil on stress gastric ulcer. Methods According to the random number table method, 24 SD rats were divided into control group, model group and fish oil group, with 8 rats in each group, half male and half female. Rats in fish oil group were gavaged with 0.84 mL/kg of deep-sea fish oil for 5 d consecutive, and rats in the other two groups were gavaged with 0.84 mL/kg of 0.9% sodium chloride solution for 5 d consecutive. The rats in three groups fasted for 24 h before the experiment. The rats in model group and fish oil group were treated with the shackled water immersion stress to make rat gastric ulcer model. After the end of the experiment, the stomach ulcer index (UI) was measured, and HE staining was performed to observe the damage of gastric mucosa. Meanwhile, the activity of superoxide dismutase (SOD) in serum was measured. Results UI of model group was higher than that of control group; UI of fish oil group was lower than that of model group (all P < 0.01). SOD level in model group was lower than that in control group; SOD level of fish oil group was higher than that of model group (P < 0.05 or P < 0.01). The gastric mucosa of rats in the control group was intact. Defects were found in the gastric mucosa of rats in both model group and fish oil group, but the defect occurred only in the superficial mucosa of fish oil group, and the degree of damage was less than that of model group. Conclusion Deep-sea fish oil has a certain inhibitory effect on stress gastric ulcer, this effect may be mediated by enhancing SOD activity and consequently promoting the ability to scavenge oxygen free radicals.

12 Application value of carbon nanoparticle in the prevention of central area lymph node dissection of papillary thyroid microcarcinoma at cN0 stage

Objective To investigate the application value of carbon nanoparticle in the prevention of central area lymph node dissection of papillary thyroid microcarcinoma (PTMC) at cN0 stage. Methods From January 2017 to June 2019, a total of 260 PTMC patients with clinical cervical lymph node metastasis negative (cN0) who admitted to Daping Hospital, Army Medical University were selected as study objects. They were divided into experimental group (133 cases) and control group (127 cases), according to whether carbon nanoparticle were used during the operation. All patients underwent hemithyroidectomy and isthmus excision, and preventive central area lymph node dissection was performed. Carbon nanoparticle was used in the experimental group and not in the control group. Lymph node detection and metastasis were compared between the two groups, and the rate of postoperative parathyroid injury and complications such as temporary hypocalcemia and permanent parathyroid hypoparathyroidism were further analyzed. Results The average number of detected lymph node and metastasis in experimental group were higher than those in the control group (P < 0.05 or P < 0.01). The rate of parathyroid mistranslation and incidence of temporary hypocalcemia in the experimental group were lower than those in control group (P < 0.01). There was no permanent hypoparathyroidism or recurrent laryngeal nerve injury in either group. Conclusion In the prevention central area lymph node dissection of cN0 PTMC patients, the application of carbon nanoparticle can improve the detection rate of positive lymph nodes, and help reduce the damage of parathyroid and the occurrence of postoperative complications.

16 Changes of serum sTM levels and its prognostic value in patients with acute respiratory distress syndrome

Objective To study the changes of serum soluble thrombomodulin (sTM) levels and its prognostic value in patients with acute respiratory distress syndrome (ARDS). Methods A total of 96 patients with ARDS admitted to Wuhan Third Hospital from December 2018 to December 2019 were selected as the observation group, they were divided into mild group (25 cases) and moderate group (53 cases) and severe group (18 cases) according to the oxygenation index (OI). Patients were divided into survival group (46 cases) and death group (50 cases) according to their survival status after 28 days. Another 48 healthy persons who were admitted to the hospital during the same period were selected as the control group. The difference in serum sTM level and acute physiology and chronic health score Ⅱ (APACHE Ⅱ) between the observation group and the control group, ARDS patients of different degrees, and different outcome groups were compared, while the correlation between serum sTM level and APACHE Ⅱ score were analyzed. ROC survival curve was drew and to analyze the predictive value of serum sTM level and APACHE Ⅱ score on the prognosis of ARDS patients. Results The serum sTM level and APACHE Ⅱ score of the observation group were higher than those of the control group, and the differences were statistically significant (P < 0.05). Comparison of serum sTM levels and APACHE Ⅱ scores of ARDS patients in different degree groups showed statistically significant differences (P < 0.05), and the levels of indexes in the severe group were higher than those in the moderate and mild groups, while the levels in the moderate group were higher than those in the mild group, and the differences were statistically significant (P < 0.05). Serum sTM level was positively correlated with APACHE Ⅱ score (r = 0.691, P < 0.05). The serum sTM level and APACHE Ⅱ score of the survival group were significantly lower than those of the death group, and the differences were statistically significant (P < 0.05). ROC curve results showed that the predictive power of serum sTM level on the prognosis of ARDS patients was higher than the APACHE Ⅱ score (P < 0.05). Conclusion sTM may be involved in the progression of ARDS, and its level changes can be used as an early biomarker to judge the severity and prognosis of ARDS patients.

20 Mutation analysis of 21 STR loci in 587 cases of paternity testing

Objective To observe and analyze the mutation phenomenon of 21 short tandem repeats STR loci in paternity testing cases, and explore the application value of the multiple amplification typing system in paternity testing. Methods The 1628 samples from 587 paternity test cases were collected from January 2016 to Decomber 2019. The template DNA was extracted by Chelex-100 method, and amplification of 21 STR loci was performed using STRtyper-21-G kit. PCR products were separated and detected by capillary electrophoresis in an ABI 3500 genetic analyzer. Genotyping was done using allelic ladders provided with the kit and the GeneMapperID-X1.3 software. Results Among the 587 cases of paternity testing, 443 were triplets and 114 were doublets; 30 cases (5.11%) were excluded; 557 cases (94.89%) were identified. A total of 20 mutations were observed in 19 mutation cases in 557 cases, including 19 mutations in one step and one mutation in two steps; the ratio of paternal mutation to maternal mutation was 8∶1. Conclusion The 21 STR loci review amplification typing system has high non-paternal exclusion performance, which can meet the needs of daily paternity testing. At the same time, STR locus mutation is a relatively common phenomenon. If necessary, supplement detection of STR locus and the use of sequencing to ensure the accuracy of the identification results.

23 Establishment and application of a model of taxonomy of educational goals in clinical medicine

The cognitive process classification of the classification table in Bloom’s taxonomy of educational goals is arranged from bottom to top, then a virtual two-dimensional coordinate system is added to the classification table, and the table is divided into four quadrants, thus establishing the taxonomical model of clinical medical education goals. The lower cognitive process of clinical medical theoretical knowledge is based on the lower order thinking skills, starting from “memory/recollection” and transferring to “understanding” and “application”. The higher cognitive activities for “diagnosis, treatment and clinical practice” correspond to cognitive processes such as “analysis, evaluation and creation” based on higher-order thinking skills. The application of the established taxonomical model shows that it is helpful to guide the design of clinical teaching goals, teaching activities and evaluation.

31 Reasearch progress of Zengshengping in the prevention and treatment of tumor

Zengshengping is a pure traditional Chinese medicine preparation which was developed by Cancer Institute of Chinese Academy of Medical Sciences. It is mainly used to block the precancerous lesion of the esophagus, improve the postoperative survival time of patients with esophageal and cardiac cancer, and gradually apply in the treatment of oral leukoplakia, reverse the chronic atrophic gastritis intestinal metaplasia and reduce the recurrence rate of postoperative of giant cell tumor of bone in extremities. The anti-tumor mechanism of Zengshengping is mainly related to regulation of Notch signaling pathway, promotion of tumor cell apoptosis, and inhibition of tumor cell proliferation. The active components of the medicinal components, such asfraxinellone, dictamnine, maackiain, obacunone and matrine in Sophorae Tonkinensis Radix Et Rhizoma and Dictamni Cortex, may play the main anti-tumor activity. At present, the relevant basic research of Zengshengping is still limited to exploring its influence on the expression of relevant molecules in tumor cells, and its role in tumor microenvironment remains to be further explained. At the same time, most of the relevant clinical studies have problems such as a small number of patients and a lack of rigor in the study design, so the multi-center, large-sample randomized controlled trials need to be further carried out.

35 Research progress of new marker serum exosomal miRNA in the diagnosis and prognosis monitoring of traumatic brain injury

Traumatic brain injury (TBI) is one of the main causes of death and disability among young and middle-aged adults, and can have a significant impact on patients’ physiology and psychology. Neuroinflammation runs through the entire process of TBI. Although the diagnosis and treatment plan of TBI has improved in recent years, there is still a lack of biomarkers with timely diagnosis and accurate prediction value, and the treatment results are not satisfactory. Exosomes (EXO) are formed after being shed from various types of cell membranes under physiological or pathological conditions, and transport specific signals such as functional proteins, mRNA transcripts, and microRNA (miRNA) carried to the recipient cell. Biological nanoparticles that transmit information to each other have great potential in disease diagnosis and treatment. Quantitative detection of its contents can assist in assessing the degree of brain damage, monitoring disease, predicting prognosis, and providing new targets for diagnosis and treatment. This article aims to review the domestic and foreign studies on miRNAs derived from exosomes, a new biomarker with potential value for assessing the degree of injury and predicting the prognosis of TBI patients.

39 Investigation on professional identity and learning motivation of medical device majors in higher vocational colleges

Objective To study the professional identity and learning motivation status of medical device students in higher vocational colleges, and to provide references for improving the quality of talent cultivation. Methods From September 2015 to December 2016, the stratified random sampling method and self-made questionnaires were used. A survey was conducted among vocational students in the first to third grades of three majors: precision medical device technology, medical device maintenance and management, and medical device operation and management in Zhejiang Pharmaceutical College. The professional identity and learning motivation of students and their impact on learning effectiveness, and based on the analysis results were proposed countermeasures. Results A total of 389 students were surveyed in this study, and 379 questionnaires were valid, with an effective rate of 97.43%. The results showed that the family background and environment of most medical device majors in higher vocational colleges were relatively disadvantaged. There was a certain degree of blindness in professional choice, and it was not easy to produce professional identification and self-confidence. The learning motivations emphasize material pursuits of students and their learning attitudes were relatively negative, which affected the effectiveness of learning and the quality of talent training. Conclusion Talents cultivation should pay attention to the combination of “selection” and “education”, prepare in advance and take various measures with industry-wide participation, enhance major identity, and improve the quality of medical devices majors in higher vocational colleges.

44 Analysis of the mental health status and its influencing factors of the elderly in Beijing

Objective To discuss the mental health status of the elderly in Wanshoulu Street, Beijing, and to analyze the factors that affect their mental health. Methods A cluster sampling method was used to select 435 elderly people (≥80 years old) in Wanshoulu Community, Beijing from February 2019 to February 2020 as the survey subjects, and the symptom check list 90 (SCL-90) was used to assess the psychological status of the elderly, while the influencing factors of the mental health of the elderly were analyzed. Results Among the 435 cases of elderly people, 85 cases (with mental health problems) were detected by SCL-90, accounting for 19.54%. Among the 10 factors, the highest positive detection rate was somatization (40.92%), and the positive detection rates of others (reflecting sleep and diet), obsessive-compulsive symptoms and depression were 25.52%, 23.91% and 23.45%, respectively. Single factor analysis showed that the differences in gender, body mass index (BMI), education level, family per capita annual income, self-evaluation of family economic status, living situation, family relations, and combined with chronic underlying disease between different mental health status of the elderly with chronic underlying diseases were statistically significant (P < 0.05). The multivariate binary logistic stepwise regression model showed that BMI>24.0 kg/m2 (OR = 0.619), low education level (OR = 10.262), average family economy (OR = 0.401), average family relationship (OR = 0.418) and combined the types of chronic underlying diseases>2 (OR = 0.899) were risk factors affecting the mental health of the elderly (P < 0.05). Conclusion The mental health of the elderly is mainly affected by BMI, education level, family economic status, family relationship and combined chronic underlying diseases.

48 Investigation on the medical experience of patients with chronic diseases under the hierarchical diagnosis and treatment model of outpatients based on information

Objective To observe the experience of patients with chronic diseases before and after the development of the information-based outpatient hierarchical diagnosis and treatment model, aiming to provide a reference for the establishment of a reasonable diagnosis and treatment model for chronic diseases in the future. Methods The Second Affiliated Hospital of Chengdu Medical College (hereinafter referred to as “our hospital”) had been implementing a hierarchical diagnosis and treatment model based on information-based outpatient clinics on a trial basis in June 2018, and 450 patients with chronic diseases who had been in regular outpatient clinics of our hospital from June 2017 to December 2019 were selected as research objects, the scores of patients’ satisfaction and the experience were compared before and after the implementation of the information-based outpatient grading diagnosis and treatment model. Results After the implementation of the hierarchical diagnosis and treatment model of informationized outpatient clinics, the overall satisfaction of patients was higher than before the implementation, and the difference was statistically significant (P < 0.05). The scores of experience in diagnosis and treatment services, registration, nursing and other services of patients were higher than those before implementation, and the differences were statistically significant (P < 0.05). Conclusion The hierarchical diagnosis and treatment model of outpatient clinics based on informatization is conducive to improving the experience of patients with chronic diseases, increasing satisfaction with outpatient visits, and improving the efficiency of outpatient services.

52 Relationship between left atrial volume index and adverse cardiac events in patients with acute coronary syndrome and its diagnostic value

Objective To explore the relationship between left atrial volume index (LAVI) and major adverse cardiac events (MACE) in patients with acute coronary syndrome (ACS). Methods A total of 198 ACS patients who were followed up in Xianyang Central Hospital in Shaanxi Province from July 2018 to January 2019 were selected as the research objects. The follow-up time was six months. According to whether MACE occurred, they were divided into good prognosis group (115 cases) and poor prognosis group (83 cases). The differences in general information, LAVI, left atrial inner diameter (LAD), left ventricular end-diastolic diameter (LVEDD), left ventricular end-systolic diameter (LVESD) and left ventricular ejection fraction (LVEF) between the two groups were compared. The influencing factors of MACE were analyzed and the diagnostic efficacy of LAVI for MACE in ACS patients were tested. Results During the 6-month follow-up, the incidence of MACE in ACS patients was 41.92% (83/198). The age and Killp grade in the poor prognosis group were significantly higher than those in the good prognosis group, and the differences were statistically significant (P < 0.05). The LAVI, LAD, LVEDD and LVESD levels in the poor prognosis group were significantly higher than those in the good prognosis group, while the LVEF levels were significantly lower than those in the good prognosis group, and the differences were statistically significant (P < 0.05). Logistic regression analysis showed that age, Killp grade, and LAVI were risk factors for MACE in ACS patients (OR > 1, P < 0.05), while LVEF was a protective factor (OR < 1, P < 0.05). The area under the ROC curve of LAVI predicting MACE in ACS patients was 0.874 (95%CI: 0.819-0.929, P < 0.001), the cut-off value was 27.00, the accuracy was 0.671, the sensitivity and specificity were 81.90%, 85.20%, respectively. Conclusion LAVI is an independent risk factor for MACE in ACS patients, and it has a high diagnostic efficiency for MACE. According to this, early intervention can improve the prognosis of patients.

56 Changes of plasma pro-brain natriuretic peptide before and after operation in children with congenital heart disease with different degrees of pulmonary hypertension

Objective To evaluate the changes of pro-brain natriuretic peptide (NT-proBNP) in children with congenital heart disease combined with different degrees of pulmonary hypertension (PAH) at different times before and after surgery. Methods A total of 60 children with congenital heart disease who were diagnosed and treated in the Affiliated Hospital of Inner Mongolia Medical University were collected from January 2015 to January 2018. According to the pulmonary artery pressure, they were divided into four groups, including 16 cases in the no PAH group, 16 cases in the mild PAH group, 14 cases in the moderate PAH group, and 14 cases in the severe PAH group. Preoperative transthoracic echocardiography was used to estimate the pulmonary artery systolic blood pressure, and the changes in plasma NT-proBNP concentrations at different times before and after the operation in children with different degrees of PAH were compared. Results The overall comparison of NT-proBNP levels before and after in the four groups was statistically significant (P < 0.05). Among them, there was no significant difference in plasma NT-proBNP levels between the no-PAH group and the mild PAH group (P > 0.05); the plasma NT-proBNP levels in the severe and moderate PAH groups were higher than those in the no PAH group and the mild PAH group, and the differences were statistically significant (P < 0.05); the plasma NT-proBNP level in the severe PAH group was lower than that in the moderate PAH group, and the difference was statistically significant (P < 0.05). The level of NT-proBNP in the mild PAH group after operation was higher than that before operation, and the difference was statistically significant (P < 0.05). Conclusion In clinical studies, the plasma NT-proBNP level before and after surgery in children with congenital heart disease with left-to-right shunt gradually increases with the severity of PAH, which can be used as an indicator to evaluate the degree of PAH and help protect the health of patients, it is worth applying in practice.

61 Logistic regression analysis of early neurological deterioration in patients with acute cerebral infarction

Objective To investigate the risk factors of early neurological deterioration (END) after intravenous thrombolysis for acute cerebral infarction. Methods A total of 152 patients with acute cerebral infarction treated by intravenous thrombolysis with Alteplase from March 2016 to December 2019 in the Department of Neurology, Baoding Second Central Hospital were selected as the research objects. According to whether the patients presented END after thrombolytic therapy, they were divided into END group (28 cases) and no-END group (124 cases). Basic data of each study object were collected, and risk factors of END were analyzed by univariate and multivariate logistic regression analysis. Results The proportions of diabetes history and ischemic stroke history in the END group were higher than those in the no-END group (P < 0.05). There were significant differences in infarct site, degree of responsible vessel stenosis and trial of Org10172 in acute stroke treatment (TOAST) type between the two groups (P < 0.05). Diabetes history, ischemic stroke history, cardiogenic embolism and large artery atherosclerosis were all independent risk factors of END in patients with acute cerebral infarction after thrombolytic therapy (OR = 4.832, 2.542, 1.854, 1.679, P < 0.05). 65 Effect and prognosis analysis of hypertensive posterior basal ganglia intracerebral hemorrhage treated by the small bone window through the lateral fissure Rolandic point-insular lobe approach

Objective To study the effect and prognosis of hypertensive posterior basal ganglia intracerebral hemorrhage treated by the small bone window through the lateral fissure Rolandic point-insular lobe approach. Methods A total of 90 patients with hypertensive posterior basal ganglia intracerebral hemorrhage admitted to the Department of Neurosurgery, the First People’s Hospital of Anqing, Anhui Province from January 2016 to October 2019 were selected and divided into observation group and control group according to the random number table method, with 45 patients in each group. The observation group was hematoma removal by the small bone window through the lateral fissure Rolandic point-insular lobe approach, while the control group was hematoma removal by the traditional small bone window through the temporal cortical approach. The perioperative situation, the changes of the Glasgow coma score (GCS), Glasgow prognosis score (GOS) were compared between the two groups, and the postoperative complications were recorded. Results In the observation group, the postoperative automatic eye opening time was shorter than that in the control group, and the hematoma removal rate 24 h after the surgery was higher than that in the control group, with statistically significant differences (all P < 0.05). The GCS at one week after surgery in both groups were increased compared with that before surgery, and the observation group was higher than that in the control group, with statistically significant differences (all P < 0.05). Follow-up showed that the rate of good prognosis in the observation group was higher than that in the control group, and the difference was statistically significant (P < 0.05). The total incidence of complications in the observation group was lower than that in the control group, and the difference was statistically significant (P < 0.05). Conclusion The small bone window through the lateral fissure Rolandic point-insular lobe approach is well for hypertensive posterior basal ganglia intracerebral hemorrhage, which have the characteristics of high hematoma removal rate, fast postoperative recovery, less complications and so on, and have a good prognosis, it’s worth popularizing.

69 Analysis of characteristics and pregnancy outcomes in advanced maternal age with heart disease

Objective To investigate the characteristics and pregnancy outcomes in advanced maternal age with heart disease. Methods A retrospective analysis was carried out on clinical data of patients with heart disease who delivered in the First Affiliated Hospital of Xinjiang Medical University from January 2008 to December 2017. According to the age, they were divided into 216 cases in the right age group (>18 - <35 years old) and 398 cases in the advanced age group (≥35 years old). The kinds of heart disease and pregnancy outcomes were compared between the two groups. Results The incidence of hypertensive heart disease during pregnancy in the advanced age group was higher than that in the right age group, and the incidence of arrhythmia was lower than that in the right age group, with statistically significant differences (P < 0.05). The incidence of fetal growth restriction and iatrogenic prematurity in the advanced age group were higher than those in the right age group, and the differences were statistically significant (P < 0.05). Conclusion The incidence of advanced maternal age with heart disease was higher than right maternal age. Cardiopathy induced by hypertensive disorders was higher in advanced maternal age, while arrhythmia was higher in right maternal age. Advanced maternal age with heart disease would induce poorer perinatal outcome.

73 Meta-analysis of GnRH-a combined with Levonorgestrel intrauterine sustained release system in the treatment of adenomyosis

Objective To systematically review the safety and efficacy of gonadotropin-releasing hormone agonist (GnRH-a) combined with Levonorgestrel intrauterine sustained release system (LNG-IUS) in the treatment of adenomyosis (AM). Methods Randomized controlled trials were searched in PubMed, The Cochrane Library, EMBase, Web of Science, CNKI, WanFang, VIP and CBM databases. The retrieval time was from inception to April, 2020. The experimental group was given GnRH-a combined with LNG-IUS, and the control group was given LNG-IUS. The uterine volume, decyclification rate, irregular vaginal bleeding rate and total effective rate after treatment were compared between the two groups. Results A total of 11 literatures involving 845 patients. The results of meta-analysis showed that: the uterine volume of the experimental group was smaller than that of the control group (MD = -34.35, 95%CI: -48.47, -20.23, P < 0.000 01). The decyclification rate in the experimental group was lower than that in the control group (RR = 0.21, 95%CI: 0.11, 0.40, P < 0.000 01). The rate of irregular vaginal bleeding in the experimental group was lower than that in the control group (RR = 0.23, 95%CI: 0.14, 0.40, P < 0.000 01). The total effective rate of the experimental group was higher than that of the control group (RR = 1.23, 95%CI: 1.13, 1.34, P < 0.000 01). Conclusion GnRH-a combined with LNG-IUS can significantly improve the clinical efficacy and safety.

78 Analysis of influencing factors of adverse pregnancy outcome in early-onset preeclampsia

Objective To explore the related influencing factors of adverse pregnancy outcome in early-onset preeclampsia. Methods The clinical data of 161 pregnant women with early-onset preeclampsia who hospitalized and terminated their pregnancy in the People’s Hospital of Xinjiang Uygur Autonomous Region from January 2017 to December 2019. According to the adverse pregnancy outcome, they were divided into adverse pregnancy outcome group(52 cases) and non-adverse pregnancy outcome group(109 cases). The influencing factors of adverse pregnancy outcomes in two groups were analyzed. Results There were significant differences between the two groups in prepregnancy weight, prepregnancy body mass index, serum creatinine, serum uric acid, albumin, alanine aminotransferase, aspartate amino transferase, total bilirubin, neonatal weight (P < 0.05). Albumin was a protective factor for adverse pregnancy outcome in patients with early-onset preeclampsia (OR < 1, P < 0.05), while prepregnancy body mass index and alanine aminotransferase were risk factors for adverse pregnancy outcome in patients with early-onset preeclampsia (OR > 1, P < 0.05). Conclusion For overweight women planning to have a pregnancy, it is necessary to strengthen dietary guidance and weight management before pregnancy, closely monitor changes in liver and kidney function, especially changes in albumin, during pregnancy, and reasonably guide treatment decisions in the course of disease, so as to reduce the occurrence of adverse pregnancy outcomes.

82 Clinical effect of Siji Antiviral Decoction combined with Oseltamivir in the treatment of influenza A in children

Objective To investigate the clinical effect of Siji Antiviral Decoction combined with Oseltamivir in the treatment of influenza A in children. Methods Atotal of 82 children with influenza A who admitted to the First Affiliated Hospital of Shandong University of Traditional Chinese Medicine from October 2019 to January 2020 were selected as study objects. According to the different treatment plan, they were divided into control group (treated with Oseltamivir Phosphate Granules, 40 cases) and observation group (Siji Antivirus Decoction combined with Oseltamivir Phosphate Granules treatment, 42 cases), both groups were treated for five days. The curative effect, clinical symptom improvement time and adverse reactions of two groups were compared. The changes of traditional Chinese medicine syndrome score, serum C-reactive protein (CRP), interleukin-6 (IL-6) levels in two groups were observed before and after treatment. Results The total effective rate of observation group was higher than that of control group , and the difference was statistically significant (P < 0.05). The time of fever reduction, cough disappearance and general fatigue disappearance in observation group were shorter than those of control group, and the differences were highly statistically significant (all P < 0.01). After treatment, traditional Chinese medicine syndrome score, CRP and IL-6 levels in two groups were lower than those of control group; and observation group was lower than control group, the differences were highly statistically significant(all P < 0.01). There was no difference  in the incidence of adverse reactions between the two groups(P > 0.05). Conclusion Siji Antiviral Decoction combined with Oseltamivir can rapidly improve the clinical symptoms and reduce the inflammatory reaction of children with influenza A, which has significant curative effect.

86 Analysis of clinical features and risk factors of 220 children with primary intussusception

Objective To analyze the clinical features and risk factors of 220 children of primary intussusception. Methods A total of 220 cases with primary intussusception in Fuyang women’s and children’s Hospital of Anhui Province were retrospectively selected from January 2018 to January 2020. All children were treated with air enema for intussusception. According to the success of reduction, they were divided into observation group (200 cases) and control group (20 cases). The factors influencing reduction of primary intussusception air enema in children were analyzed. Results There were statistically significant differences in course of disease, fever, abdominal mass, jam like stool, abdominal distension, abdominal effusion, mesenteric lymphomegaly, intussusception diameter and intussusception length between the two groups (P < 0.05). The course of disease, abdominal mass, jam like stool, abdominal distension, intussusception diameter and intussusception length were independent risk factors affecting the reduction of primary intussusception air enema (OR > 1, P < 0.05). Conclusion Primary intussusception in children is temporary, and the reduction success rate can be improved obviously by air enema. The course of disease, abdominal mass, jam like stools, abdominal distension, diameter and length of entrapment were independent risk factors for air enema reduction.

90 Effect of fast track surgery concept on postoperative rehabilitation and perioperative stress index in patients undergoing laparoscopic radical gastrectomy

Objective To explore and analyze the effect of fast track surgery (FTS) concept on postoperative rehabilitation and perioperative stress indicators in patients undergoing laparoscopic radical gastrectomy. Methods A total of 82 patients undergoing laparoscopic radical gastrectomy admitted to the First Affiliated Hospital of Harbin Medical University from June 2017 to June 2019 were selected as the research objects. A random number table method was used to divide them into the control group and observation group, each with 41 cases. The control group was given routine perioperative management, and the observation group was giren group perioperative management based on the concept of FTS on the basis of the control group. The surgical indicators (operating time, intraoperative blood loss, number of lymph node dissections, number of indwelling drainage tubes and intraoperative fluid replenishment), postoperative rehabilitation related indicators (anal exhaust time, urinary tube removal time, oral feeding time and the length of hospital stay), perioperative stress indicators [C-reactive protein (CRP) and interleukin-6 (IL-6) before operation and on the first, fourth, and seventh days after operation] and complication rate were compared between the two groups. Results There were no significant differences in the operation time, intraoperative blood loss, and number of lymph node dissection between the two groups (all P > 0.05). Compared with the control group, the number of indwelling drainage tubes and the amount of intraoperative fluid replacement in the observation group were less (all P < 0.01). In the observation group, the time of anal exhaust, the time of removing the urinary tube, the time of oral feeding and the length of hospital stay were shorter (all P < 0.01). Compared with preoperation, the levels of CRP and IL-6 in the two groups increased significantly on the first, fourth, and seventh days after operation (all P < 0.05). The levels of CRP and IL-6 in the observation group were significantly lower than those in the control group (all P < 0.05). Compared with the control group, the observation group had a lower incidence of anastomotic leakage, infection, gastric retention, intestinal obstruction, and postoperative bleeding complications (all P < 0.05). Conclusion Implementing the concept of FTS based on conventional perioperative management and applying the concept of laparoscopic radical gastrectomy can effectively promote the recovery of patients after surgery, improve perioperative stress indicators, and reduce the incidence of complications, ensure safety better.

94 Relationship between chemotherapy efficacy and neutrophil to lymphocyte ratio in advanced non-small cell lung cancer

Objective To investigate the relationship between the neutrophil to lymphocyte ratio (NLR) and chemotherapy efficacy in advanced non-small cell lung cancer (NSCLC). Methods A retrospective analysis of 80 NSCLC inpatients with inoperable stage ⅢB and Ⅳ who met the enrollment criteria in the People’s Hospital of Guangxi Zhuang Autonomous Region from January 2016 to January 2020. NLR was collected before treatment, and the relationship between NLR and chemotherapy efficacy was analyzed. According to the treatment effect, the patients were divided into the curative effect response group and the curative effect non-response group. Results Univariate analysis showed that there was a statistically significant difference in NLR between the two groups (P < 0.05). Multi-factor regression analysis showed that NLR was an independent risk factor affecting the effect of chemotherapy (OR > 1, P < 0.05). Conclusion The efficacy of chemotherapy is related to pre-treatment NLR, and the chemotherapy efficacy is better in patients with lower NLR. NLR may be an index to predict the efficacy of first-line chemotherapy in advanced NSCLC.

98 Effect of high-demand blood transfusion on postoperative recovery of esophageal cancer and changes in related biochemical indicators

Objective To investigate the causes of high-demand blood transfusion during perioperative period of patients with esophagus cancer, and to analyze the effect of high-demand blood transfusion on postoperative recovery and related biochemical indicators. Methods A stratified cluster sampling method was used to select 58 patients with esophageal cancer admitted to the Department of Thoracic Surgery of the First Hospital of Jiaxing from May 2017 to December 2018, with 29 patients with blood transfusion volume ≥1000 mL during the perioperative period of esophageal cancer resection were divided into the observation group, and 29 patients with blood transfusion volume <1000 mL were divided into the control group, the causes and effects of high-demand blood transfusion (blood transfusion volume ≥1000 mL) were analyzed, with the length of stay in the intensive care unit, length of hospital stay, complication rate after esophageal cancer resection, platelet count and blood potassium and calcium levels included. Results The complication rate of the observation group (62.1%) was higher than that of the control group (31.0%), and the difference was statistically significant (P < 0.05). The blood loss during resection of esophageal cancer in the observation group was higher than that in the control group, while the length of stay in the intensive care unit and hospital stay were both longer than those in the control group, and the differences were all statistically significant (all P < 0.05). After blood transfusion, the platelet count of the observation group was lower than that of the control group, and the difference was statistically significant (P < 0.05). After blood transfusion, the blood potassium and calcium levels of the observation group were lower than those of the control group, and the differences were all statistically significant (all P < 0.05). Conclusion The main reason for the high-demand blood transfusion during perioperative period of esophageal cancer resection in patients with esophageal cancer is intraoperative bleeding. High-demand blood transfusion can lead to enhanced postoperative coagulation function and the incidence of complications after esophageal cancer resection will increase.

102 Effect and prognosis of 131Ⅰ combined operation in the treatment of differentiated thyroid cancer in the elderly

Objective To study the effect and prognosis of 131Iodine (131Ⅰ) combined operation for elderly differentiated thyroid cancer. Methods A total of 82 elderly patients with differentiated thyroid cancer treated in the 2nd Affiliated Hospital of Chengdu Medical College from January 2016 to October 2017 were selected for this study. All patients received total thyroidectomy or near total thyroidectomy, and neck lymph node dissection, and then treated with 131Ⅰ. The therapeutic effect, changes of thyroglobulin and thyroglobulin antibody before and after operation, adverse reactions, recurrence and survival situation were observed. Results Among 82 elderly patients with differentiated thyroid cancer, 53 cases were successfully treated after the first 131Ⅰ treatment, and 19 cases were successfully treated after the second 131Ⅰ treatment, with a total success rate of 87.80%. Six months after operation, the level of thyroglobulin and the positive rate of anti-thyroglobulin antibody were significantly lower than those before treatment, the differences were statistically significant (all P < 0.05). During 131Ⅰ treatment, a few patients had short-term adverse reactions such as throat discomfort, neck swelling, abdominal discomfort, and no serious pulmonary fibrosis, bone marrow suppression, liver and kidney function damage. At the end of follow-up, four cases relapsed and three cases died. Conclusion 131Ⅰ combined with total thyroidectomy or near total thyroidectomy is safe, effective and has a good prognosis in the treatment of differentiated thyroid cancer in the elderly, which is worthy of clinical application.

106 Effect comparison of laparoscopic cholecystectomy and laparotomy in the treatment of gallstones

Objective To compare the effect of laparoscopic cholecystectomy and laparotomy in the treatment of gallstones. Methods One hundred cases of gallstone patients in 962 Hospital, Joint Logistics Support Force of PLA, from January 2019 to April 2020 were selected and divided into control group and observation group according to Random number table method, 50 cases in each group. Control group received conventional laparotomy, while observation group received laparoscopic cholecystectomy. Clinical efficacy and prognosis of two groups were compared. Results Total effective rate of observation group was higher than that of control group, the differences was statistically significant (P < 0.05). The intraoperative blood loss of observation group was less than that of control group, operation time, postoperative exhaust time and hospitalization time of observation group were shorter than those of control group, and postoperative visual analog score of observation group was lower than that of control group, the differences were statistically significant (P < 0.05). The incidence rate of postoperative intestinal adhesion and total incidence rate of complications in observation group were lower than those in control group, the differences were statistically significant  (P < 0.05). Conclusion Laparoscopic cholecystectomy and laparotomy can both effectively treat gallstones. However, clinical effect of laparoscopic cholecystectomy is superior to that of laparotomy. It has faster recovery, less trauma, less postoperative complication, and therefore, it is worthy of promotion clinically.

110 Influnce of myoelectric biofeedback and suspension exercise training on walking function of patients with lower limb spasm after spinal cord injury

Objective To explore the effects of myoelectric biofeedback and suspension exercise training on walking function of patients with lower limb spasm after spinal cord injury. Methods One hundred and twenty patients with lower limb spasticity after spinal cord injury admitted to Rehabilitation Medical Center of West China Hospital of Sichuan University from June 2018 to April 2019 were selected and divided into group A, group B and group C, with 40 cases in each group. All the three groups were given routine rehabilitation training, on this basis, group A received myoelectric biofeedback combined with suspension exercise training, group B received myoelectric biofeedback training, group C received suspension training, all patients were intervened for two months. The scores of functional walking ability scale (FAC), modified Ashworth spasticity scale (MAS), comprehensive spasticity scale (CSS), Berg balance scale (BBS) and Fugl-Meyer motor function score were compared among three groups after intervention. Results Two months after intervention, FAC grading of group A was better than that of group B and group C (P < 0.05). Two months after intervention, the scores of MAS and CSS in three groups were lower than those before intervention, and the scores of group A were lower than those of group B and group C, the differences were statistically significant (P < 0.05). Two months after intervention, the scores of BBS and Fugl-Meyer motor function in three groups were higher than those before intervention, and the scores of group A were higher than those of group B and group C, the differences were statistically significant (P < 0.05). Conclusion Myoelectric biofeedback and suspension exercise training can improve the spasm of patients with lower limb spasm after spinal cord injury, promote the recovery of balance function and motor function of patients, and be more conducive to improving the walking function of patients with lower limb spasm.

114 Analysis of influencing factors of weight loss in elderly patients with depression

Objective To observe the weight loss of elderly depression patients, and to explore the factors influencing weight loss in elderly depression patients, so as to guide future weight intervention for elderly depression patients. Methods One hundred and ten elderly depression patients treated in Department of Gerontology, Mental Health Center, Shanghai Jiaotong University School of Medicine from January 2018 to December 2019 were prospectively selected. According to article 16 of HAMD-24 version, the weight loss was assessed and divided into weight loss group (weight loss of at least 0.5 kg within one week) and no weight loss group, and a self-made general information questionnaire was used to collect patient data. The influencing factors of weight loss in elderly patients with depression were analyzed by single-factor and multi-factor analysis. Results Among 110 elderly patients with depression, 46 cases lost weight, with an incidence of 41.82%. After a single logistic regression analysis and a multiple regression model were established, the results of multivariate analysis showed that anxiety-type depression, diabetes, high level of CRP, high level of IL-6 were the influencing factors of weight loss in elderly depression patients (OR = 11.528, 9.135, 5.141, 2.809, P < 0.05). Conclusion The incidence of weight loss in elderly patients with depression is high, which may be affected by factors such as anxiety-type depression, diabetes, and high levels of inflammatory factors. The clinic should pay attention to the early intervention of patients with the above risk factors.

118 Systematic review and meta-analysis of Simiao Yong’an Decoction in the treatment of diabetic foot with blazing heat-toxin pattern

Objective To systematically evaluate the clinical efficacy of Simiao Yong’an Decoction in the treatment of diabetic foot with heat-toxin pattern. Methods CNKI, Wanfang Data, VIP, PubMed, Embase, Web of Science and The Cochrane Library databases were searched by computer. The retrieval time was from the establishment time of database to February 15, 2020. The randomized controlled trials of Simiao Yong’an Decoction for the treatment of diabetic foot with blazing heat-toxin pattern were searched, on the basis of strict quality evaluation, the RevMan5.3 software was used for meta-analysis. Results Nineteen literatures were finally included, including 1380 patients. There were 694 cases in the treatment group and 686 cases in the control group. The results of meta-analysis showed that total effective rate (RR = 1.26, 95%CI [1.20, 1.33], P < 0.000 01) in the treatment group were significantly higher than that in the control group. Ankle-brachial index (ABI) (MD = 0.09, 95%CI [0.07, 0.12], P < 0.000 01), blood flow of dorsalis pedis artery (MD = 7.46, 95%CI [5.48, 9.43], P < 0.000 01), motor nerve conduction velocity (MCV) (MD = 7.44, 95%CI [6.32, 8.5], P < 0.000 01), sensory nerve conduction velocity (SCV) (MD = 5.05, 95%CI [4.05, 6.05], P < 0.000 01) in the treatment group were significantly better than those in the control group. There were no significant differences in both whole plasma viscosity (MD = -0.06, 95%CI [-0.16, 0.03], P = 0.19) and fasting blood glucose (GLU) (MD = -0.20, 95%CI [-0.66, -0.27], P = 0.41) between the two groups. Conclusion The current evidence shows that Simiao Yong’an Decoction can effectively improve the clinical symptoms of diabetic foot, increase ABI, blood flow of dorsalis pedis artery, MCV and SCV, decrease the level of GLU. Limited by the quantity and quality of the included studies, the above conclusions need to be verified by more high-quality studies.

Methods Sixty patients with essential hypertension with syndrome of upper hyperactivity of liver yang were selected from January to December 2019 in the Department of Cardiology, Nanjing Pukou Hospital of TCM. All patients were randomly divided into experimental group and control group with 30 cases in each group. The control group was treated with Captopril Tablets and the experimental group was treated with TCM acupoint sealing on the basis of the control group. The total effective rate of clinical treatment after one course of treatment was compared between the two groups. The ambulatory blood pressure value and variation in blood pressure (24 h mean systolic blood pressure [mSBP], 24 h mean diastolic blood pressure [mDBP], daytime mean systolic blood pressure [dSBP], daytime mean diastolic blood pressure [dDBP], nighttime mean systolic blood pressure [nSBP], nighttime mean diastolic blood pressure [nDBP], blood pressure fluctuations [BPF] of systolic blood pressure) before and after treatment were compared between the two groups. The incidence of adverse reactions in two groups were compared during treatment. Results The total effective rate of the experimental group was significantly higher than that in the control group, the difference was statistically significant (P < 0.05). After treatment, mSBP, mDBP, dSBP, dDBP, nSBP, nDBP and BPF of systolic blood pressure in the two groups were significantly lower than those before treatment, which in the experimental group were lower than those in the control group (P < 0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P > 0.05). Conclusion TCM acupoint sealing treatment of patients with essential hypertension with syndrome of upper hyperactivity of liver yang has good clinical efficacy, which can effectively control blood pressure, and it has good safety and high clinical value.

128 Exploration on the theoretical mechanism of Banxia Houpu Decoction in the treatment of reflux esophagitis

Reflux esophagitis (RE) is one of the common diseases of alimentary system. At present, the relapse rate of RE treated by modern medical is still high, while the traditional Chinese medicine treatment of RE gradually shows some advantages. This paper analyzes and discusses the theoretical basis of Banxia Houpu Decoction in the treatment of RE, the legislative framework of Banxia Houpu Decoction and the pharmacological effects of the drugs. It is considered that the deficiency of the spleen and stomach is the basis of the disease, and the key pathogenesis is the ascending and descending of qi movement of middle jiao. Banxia Houpu Decoction has the effects of resolving stagnation and dissipating phlegm, descending qi and harmonizing stomach. Its prescription medicine can promote gastric emptying, improve inflammatory reaction, reduce reflux and alleviate depression, and play a role based on the theoretical basis of “corresponding formula and evidence”. This paper provides a theoretical basis for Banxia Houpu Decoction to play a therapeutic effect on RE.

Diabetic foot is one of the common chronic complications of diabetes mellitus. It is located in the veins of the limbs. It has the characteristics of severe disease, rapid development, high disability and mortality rate. It belongs to the category of “collateral disease-vascular system disease”. The pathological basis and basic pathogenesis of diabetic foot is deficiency of qi and yin and the syndrome of intermingled phlegm and blood stasis, which cause heat-toxicity and necrotic pathogen blocking collaterals. The method of removing blood stasis and unblocking collaterals based on the principle of “using collaterals for circulation” is widely used in the treatment of diabetic foot. In clinical practice, the effect of self-made Huayu Tongluo Decoction on diabetic foot is significant, which can significantly improve the clinical symptoms of patients, and the drug is simple, safe, and treated with both manifestion and root cause, which is worthy of clinical promotion. This article introduces the experience of treating diabetic foot with self-made Huayu Tongluo Decoction.

Objective To investigate clinical effect of point-through-point acupuncture combined with traditional Chinese medicine in the treatment of intractable hiccup after radiotherapy and chemotherapy in patients with malignant tumor. Methods Sixty patients with intractable hiccup after radiotherapy and chemotherapy and traditional Chinese medicine syndrome differentiation belongs to the syndrome of intermingled heat and cold from May 2018 to September 2019 admitted to Sanhuan Cancer Hospital of Chaoyang District of Beijing City were selected. They were divided into control group (30 cases) and treatment group (30 cases) according to random number table method. Control group was treated with modified Huanglian Decoction. Treatment group was treated with acupuncturing cuánzhú through yúyāo, nèiguān through wàiguān, zúsānlǐ through yīnlíngquán combined with modified Huanglian Decoction. Seven days as a course of treatment, followed up for one week. Clinical efficacy, onset time and relapse rate of followed up for one week between two groups after treatment were observed. Results Total effective rate of treatment group after treatment was higher than that of control group, the difference was statistically significant (P < 0.05). The average onset time of treatment group was shorter than that of control group, the difference was statistically significant (P < 0.05). The relapse rate of treatment group was lower than that of control group after one week follow-up, the difference was statistically significant (P < 0.05). Conclusion Point-through-point acupuncture combined with traditional Chinese medicine can effectively improve the intractable hiccup with syndrome of intermingled heat and cold after radiotherapy and chemotherapy in patients with malignant tumor, with rapid onset of action and low replase rate.

Starting from ancient literatures and modern research, this paper analyzes and classifies the meanings of dampness-heat in different categories, expounds the theoretical origin of dampness-heat, the nature and characteristics of the etiologies of dampness-heat, the occurrence of dampness-heat pathogenesis, the cause of dampness-heat constitution and the relationship with syndrome, so as to be beneficial to the related theoretical research and disease treatment.

143 Application of continuous quality improvement in reducing the unqualified rate of traditional Chinese medicine preparations

Objective To improve the quality of traditional Chinese medicine preparations in medical institutions through continuous quality improvement. Methods By using PDCA method, the causes of unqualified traditional Chinese medicine preparations in medical institutions were analyzed, and the improvement countermeasures were put forward and implemented. The unqualified proportion of traditional Chinese medicine preparations before the implementation of quality improvement (from January to December 2018) and after the implementation of quality improvement (from January to December 2019) was compared, and the effect of the improvement measures was evaluated. Results After improvement, the unqualified rate of traditional Chinese medicine preparation in 2019 was lower than that in 2018, and the difference was highly statistically significant (P < 0.01). The unqualified rate of microorganism limit examination decreased by 78.69%, and the unqualified rate of content decreased by 84.89%. Conclusion Through continuous quality improvement, the unqualified rate of traditional Chinese medicine preparation is effectively reduced, the work efficiency is improved, the quality consciousness is promoted, and the safety and effectiveness of the patient medication is ensured.

Objective To sort out the international research status, hotspots and frontiers on anxiety in women with infertility, and provide references for the development of infertility-related clinical care in our country. Methods The Web of Science core database was extracted from January 2000 to June 2020 on research themed on Female infertility anxiety, and Citespace software was used to analyze country, institution, cited literature, keywords and their emergent words. Results Finally, 433 articles were included, and the number of published articles showed a rising trend. The United States, China, Iran, and the United Kingdom had greater influence in international academic circles. The research focuses on the Iranian Educational and cultural Research Academic Center and Cardiff University. Research hotspots included research on sources of anxiety in female infertility (assisted reproductive therapy effects, endometriosis, sleep quality, negative emotions, marital satisfaction, equity, etc.) and anxiety intervention studies (partner support, cognitive behavioral therapy, traditional Chinese medicine, etc.). The research frontier is to assess the fertility quality of life and develop related scale tools for female infertility anxiety patients. Conclusion The research on female infertility anxiety has attracted extensive attention from scholars at home and abroad, and has formed a certain scale of research in space. The research hotspots and frontiers have a guiding value to enrich the comprehensive nursing program for female infertility anxiety patients in our country.

152 Application of health education in elderly patients with knee osteoarthritis under the guidance of empowerment theory

Objective To explore the application effect of health education based on the theory of empowerment in elderly patients with knee osteoarthritis. Methods A total of 86 elderly patients with knee osteoarthritis who admitted to the General Hospital of Eastern Theater Command from July 2018 to June 2019 were selected. According to the length of hospital stay, they were divided into treatment group (January to June 2019) with 44 cases and control group (July to December 2018) with 42 cases. The control group was given regular health education, and the treatment group jointly applied health education based on the theory of empowerment. After six months of follow-up, the functional exercise self-efficacy, quality of life were compared. Results The functional exercise self-efficacy of the two groups was longer than before education (P < 0.01), the total time of physical exercise, endurance exercise and functional exercise in the treatment group was longer than that in the control group (P < 0.05); the quality of life scores of the two groups were improved compared with that before education (P < 0.01), the total scores of physical function, symptoms, social contact, work and quality of life were higher than those of the control group (P < 0.05). Conclusion Health education guided by the empowerment theory is more easily accepted and recognized by elderly patients with knee osteoarthritis, which helps to promote the development of their functional exercise self-efficacy and improve the quality of life of patients.

156 Effect of chronic illness trajectory intervention mode on self-management efficiency and living quality of lung cancer patients

Objective To investigate the effect of chronic illness trajectory intervention mode on self-management efficiency and living quality of lung cancer patients. Methods A total of 98 lung cancer patients received treatment to Jiangsu Cancer Hospital from January 2017 to December 2018 were selected. According to the admission time, the patients were divided into experimental group (January to December 2018, 52 cases) and control group (January to December 2017, 46 cases). Patients in control group were performed routine nursing intervention, experimental group were performed nursing intervention based on chronic illness trajectory intervention mode. The Chinese version of the cancer self-management efficacy scale (SUPPH) and the Chinese version of the cancer quality of life core scale (EROTC-QLQ-C30) were used to evaluate the self-management efficacy and quality of life of the two groups. Results After follow-up for six months, the score of self-management efficacy of the two groups was higher than that before intervention, and the total score of self-decision-making, positive attitude, self-decompression and self-management efficacy of the experimental group was higher than that of the control group, with statistically significant differences (P < 0.05); the quality of life scores of the two groups were higher than before the intervention, and the scores of physical function, role function, emotional function, cognitive function and social function of the experimental group were higher than those of the control group, with statistically significant differences (P < 0.05). Conclusion The nursing intervention based on chronic illness trajectory intervention mode can promote the development of self-management efficiency of lung cancer patients and improve their living quality.

160 Application effect of WeChat platform health management in patients with peripherally inserted central catheters during the outbreak of corona virus disease 2019

Objective To investigate the application effect of WeChat platform health management in patients with peripherally inserted central catheters (PICC) during the outbreak of corona virus disease 2019 (COVID-19). Methods A total of 108 cases of PICC in-home patients managed by Jiangsu Provincial People’s Hospital during the outbreak of the COVID-19 from January to February 2020 were selected as the research objects. The patients were divided into experimental group (56 cases) and control group (56 cases, 52 cases were actually included) by random number table method. The control group was given discharge guidance and telephone follow-up management, and the treatment group used WeChat platform health management. The home treatment compliance, PICC catheterization self-management ability, and PICC catheterization-related complications were compared between the two groups. Results The patients in the experimental group had higher compliance with healthy life, self-protection and physical monitoring than in the control group (P < 0.05); daily catheter observation, maintenance compliance, catheter management confidence, abnormal situation handling, information acquisition, tube exercise, tube daily life, and PICC management total scores were higher than those of the control group (P < 0.01); the incidence of PICC-related complications was lower than that of the control group (P < 0.05). Conclusion The health management of the WeChat platform can help improve the compliance behavior of PICC intubation patients at home during the COVID-19 epidemic, promote the development of PICC intubation self-management ability, and reduce the occurrence of PICC intubation-related complications.

164 Effect of preventing pressure sores after operation by pearl powder unite zinc oxide

Objective To discuss the result of preventing pressure sores after operation by pearl powder unite zinc oxide. Methods A total of 70 patients receiving surgical treatment from February to May 2020 in Guangdong Hospital of Traditional Chinese Medicine were selected as the study subjects, 35 cases in zono oxide group and 35 cases in observation group (pearl powder combined with zinc oxide) were divided by random number table method, the amount of blood perfusion, skin temperature and oxygen saturation of the skin under pressure before and after intervention were observed, the stage and satisfaction degree of pressure ulcer were observed in the two groups. Results After intervention, the amount of blood perfusion and oxygen saturation of the skin of the two groups increased compared with that before treatment, and the skin temperature decreased compared with that before treatment (P < 0.05). Moreover, the amount of blood perfusion and oxygen saturation of the skin of the observation group was higher than that of the control group, and the skin temperature was lower than that of the control group (P < 0.05). The incidence of pressure ulcers and different stages of pressure ulcers in the observation group were lower than those in the control group (P < 0.05). The nursing satisfaction of patients in the observation group was higher than that of the control group, and the difference was statistically significant (P < 0.05). Conclusion The application of pearl powder combined with zinc oxide surgery can improve the amount of skin blood perfusion and blood oxygen saturation, reduce the skin temperature and the incidence of pressure ulcers, and improve the nursing satisfaction, which is worthy of popularization and application.

168 Influence of “pregnant school+WeChat official account” on fear of childbirth and delivery mode of primipara

Objective To explore the effect of “pregnant school+WeChat official account” on the fear of childbirth and delivery mode in primipara. Methods From January 2019 to June 2019, 200 primiparas in the First Affiliated Hospital of Hebei North University were selected as the research objects, and they were divided into control group and observation group by digital random method, 100 cases in each group, all primiparas received relevant knowledge of pregnancy and childbirth health care. The control group was given routine outpatient education, while the observation group was filing with the system of “pregnant school+WeChat official account” to carry out a comprehensive and systematic health education in pregnancy. The methods of delivery and the time of delivery were compared between the two groups; the anxiety of the two groups were evaluated by anxiety self-rating scale (SAS); the knowledge of the two groups were analyzed and summarized by questionnaire. Results The natural delivery rate of the observation group was higher than that of the control group (P < 0.05). The time of each birth process of primipara in the observation group was shorter than that in the control group, the difference was statistically significant (P < 0.05). The midwifery rate of the observation group was lower than that of the control group, and the difference was statistically significant (P < 0.05). The cesarean section rate in the observation group was lower than that in the control group, and the difference was statistically significant (P < 0.05). The SAS score of the observation group was lower than that of the control group, and the incidence of anxiety in the observation group was lower than that of the control group, with statistical significance (P < 0.05). In the observation group, the knowledge of pregnancy, childbirth, puerperium and neonatal nursing were higher than that of the control group, the difference was statistically significant (P < 0.05). Conclusion The mode of “pregnant school+WeChat official account” can improve the natural childbirth rate of primiparas and reduce the rate of cesarean section. Therefore, the model of pregnant school+WeChat official account is worthy of clinical reference and has the value of clinical promotion.

172 Effect of supportive psychotherapy on self-perceived burden and health-related behaviors in patients with chronic hepatitis B

Objective To investigate the effects of supportive psychotherapy on self-perceived burden and health-related behaviors in patients with chronic hepatitis B. Methods A total of 115 patients with chronic hepatitis B hospitalized to the Huai’an Fourth People’s Hospital from July 2016 to June 2018 were selected. According to the length of hospital stay, the patients were divided into observation group (from July 2017 to June 2018) and control group (from July 2016 to June 2017) with 56 cases and 54 cases. The control group was given routine health education and psychological intervention for patients with chronic hepatitis B, and the observation group was combined with supportive psychological therapy. After six months of follow-up, the self-feeling burden, health-related behavior, and patient satisfaction rate were compared between the two groups. Results The scores of each dimension of the self-perceived burden of the two groups were significantly lower than before the intervention of the same group (P < 0.01), the total scores of the physical burden, emotional burden, and self-perceived burden scales of the treatment group were significantly lower than those of the control group (P < 0.01); the compliance rate of health-related behaviors of the two groups was significantly higher than before the intervention of the same group (P < 0.05 or P < 0.01), the rate of compliance with medical drugs, standardized diet, sleep rest, and appropriate exercise was significantly higher than that of the control group (P < 0.05); the satisfaction of patients in the observation group was higher than that in the control group (P < 0.05). Conclusion Supportive psychotherapy can help relieve the self-feeling burden of patients with chronic hepatitis B, promote the development of relevant health behaviors, and improve patient satisfaction.

Cancer has a high incidence in the elderly, and pain is one of the main symptoms that plague elderly cancer patients. Cancerous pain management is a standardized process of pain control and has important clinical significance. This article summarizes the common problems in the pain management process of elderly cancer patients, the deviation of elderly patients’ perception of pain and the degradation of their physiological levels, insufficient attention of medical staff and insufficient knowledge reserves, imperfect hospital regulations and lagging social cognition, etc. All have negative impacts on the management of elderly cancerous pain. With the stepping into an aging society, the cancerous pain problem of elderly patients urgently needs to be solved by mature clinical management solutions. This article puts forward suggestions for the pain management and evaluation process. Genitourinary fistula is an abnormal channel formed between urinary organs and reproductive system. Fallopian tube bladder fistula is one of the rare types, and congenital cases have not been reported. This patient went to Liuzhou Maternity and Child Healthcare Hospital, Guangxi Zhuang Autonomous Region for hysterosalpingo-contrast sonography (HyCoSy) examination due to “infertility” and found fallopian tube bladder fistula. After hospitalization, she underwent hysteroscopy combined with cystoscopy surgery. During operation, she was diagnosed with the right fallopian tube bladder fistula, and the left double-cavity fallopian tube. HyCoSy can observe the internal and external structures of female internal and external genitalia and the adjacent relationship of surrounding organs, evaluate the patency of the fallopian tubes, and find the fallopian tube bladder fistula, which is a better method for diagnosing female genital genitourinary malformations.

Objective To explore the effect of the full appointment system on outpatient medical treatment efficiency and service quality under the clinical assistant intervention mode. Methods A total of 1000 outpatients from Shanghai Proton and Heavy Ion Center from July to December 2019 were selected for the study. According to different pre-examination and triage modes, they were divided into two groups: the intervention group (500 cases, full appointment consultation under the clinical assistant intervention mode) and the conventional group (500 cases, appointment appointment on the online registration platform). The efficiency of pre-examination and subdiagnosis treatment, quality of outpatient service and satisfaction of the two groups were statistically analyzed. Results The outpatients in the intervention group belonged to the proportion of indications were lower than those in the conventional group, the time required for outpatient visits and the waiting time for outpatient visits were longer than those in the conventional group, and the cost of outpatient costs was higher than that in the conventional group, with statistically significant differences (all P < 0.05). The scores of basic service, service attitude, service skill, and nursing care disputes in the intervention group were all higher than those in the conventional group, with statistically significant differences (all P < 0.05). There was no statistically significant difference between the two groups in satisfaction with outpatient service (P > 0.05). Conclusion The full appointment system under the clinical assistant intervention model can improve the medical treatment efficiency and service quality of outpatient pre-examination and subdiagnosis treatment, improve the efficiency of medical treatment and the total cost of social care, and effectively ensure the satisfaction of outpatients. Therefore, it is worth promoting in the full appointment system.
