9 Effects of Yigan Jiedu Decoction and its preparation on UCP-2 in non-alcoholic steatohepatitis rats

Objective To establish the rat model of non-alcoholic steatohepatitis (NASH) by high-fat diet, and to observe the effect of Yigan Jiedu Decoction and its preparation on UCP-2 in rats with NASH. Methods According to the random number table method, 60 Wistar rats were divided into normal group, model group, whole formula group, phlegm elimination group, stasis removal group and detoxification group, 10 rats in each group were given high-fat diet for 16 weeks to replicate the NASH model. Rats in the normal group (1.575 mL/100 g normal saline), model group (1.575 ml/100 g normal saline), whole formnla group (1.730 g/100 g of traditional Chinese medicine), phlegm elimination group (1.730 g/100 g of traditional Chinese medicine), stasis removal group (1.730 g/100 g of traditional Chinese medicine), and detoxification group (1.730 g/100 g of traditional Chinese medicine) were put to death after 16 weeks of intervention, and specimens were collected. Serum alanine aminotransferase (ALT), aspartate aminotransferase (AST), glutamyl transferase (GGT), triglyceride (TG), total cholesterol (TC), high density lipoprotein (HDL), low density lipoprotein (LDL) and reactive oxygen species (ROS) were determined by Reitman colorimetry. UCP-2 mRNA was detected by RT-PCR. UCP-2 protein was detected by Western blot. Results NASH model of rats was successfully established. The mean values of ALT, AST, GGT, TC, TG, HDL, ROS and UCP-2 mRNA in the whole formula group and its preparation groups were significantly lower than those in the model group, while LDL was higher than that in the model group, with statistically significant differences (P < 0.05). The mean values of ALT, AST, GGT, TC, TG, HDL, ROS and UCP-2 mRNA in preparation groups were higher than those in the whole formula group, with statistically significant differences (P < 0.05). The levels of ALT, AST, GGT, TC, TG and HDL in the stasis removal group and the detoxification group were lower than those in the phlegm elimination group, and the differences were statistically significant (P < 0.05). The LDL level of the stasis removal group was higher than that of other groups, and the difference was statistically significant (P < 0.05). The results of Western blot showed that compared with normal group, the expression of UCP-2 protein in the model group was increased; compared with model group, the expression of UCP-2 protein in the whole formula was decreased; while the expression of UCP-2 protein among all preparation groups was similar. Conclusion The whole formula group has the best effect, which shows that the TCM compound is the result of the synergistic effect of its components. Yigan Jiedu Decoction can inhibit the production of ROS, reduce the high expression of UCP-2 and the levels of serum ALT, AST, GGT, TC, TG, HDL, and improve the level of LDL.

13 Effects of 17-methoxyl-7-hydroxyl-benzofuran chalcone on inflammatory responses in myocardial ischemia reperfusion injury in rats

Objective To observe the effects of 17-methoxyl-7-hydroxyl-benzofuran chalcone (YLSC) on inflammatory responses in myocardial ischemia reperfusion injury in rats. Methods Sixty 12-month-old SPF Sprague-Dawley (SD) rats were randomly divided into sham group (equal volume of normal saline), model group (equal volume of normal saline), positive control group (Puerarin Injection, 12.00 mg/kg) and YLSC low, medium and high-dose groups (2.50, 5.00, 10.00 mg/kg) via random number table method. The anterior descending coronary artery was ligated for 1 h then reperfused for 3 h to establish the ischemia reperfusion (I/R) model. Rats in sham group were applied stitches under the coronary artery without ligation. Levels of serum troponin I (cTnI), creatine kinase-MB (CK-MB), interleukin-6 (IL-6), interleukin-1β (IL-1β) and tumor necrosis factor-α (TNF-α) were examined by enzyme-linked immunosorbent assay. The infarct sizes were measured by evans blue and 2, 3, 5-triphenyltetrazolium chloride staining. Protein levels of high mobility group box-1(HMGB1), toll-like receptors 4 (TLR4) and nuclear factor-kappa B (NF-κB) in myocardiums were evaluated using Western blot. Results Compared with model group, YLSC reduced the leakage of myocardial enzyme CK-MB and cTnI (P < 0.05 or P < 0.01), decreased myocardial infarction size (P < 0.05 or P < 0.01), lower the release of serum inflammatory cytokines such as IL-6, IL-1β and TNF-α (P < 0.05 or P < 0.01), and inhibited the protein expression of myocardial HMGB1, TLR4, NF-κB (P < 0.05 or P < 0.01). Conclusion YLSC can reduce ischemia-reperfusion injury in rats, the myocardial protective effect may be related to reducing of inflammatory reaction, inhibiting the expression of myocardial related protein such as HMGB1, TLR4 and NF-κB.

17 Anti-inflammatory effect of the total flavonoids of Fructus Kochiae in DNBC-induced allergic contact dermatitis rats

Objective To investigate the anti-inflammatory effect of the total flavonoids of Fructus Kochiae (FFK) on DNBC-induced allergic contact dermatitis (ACD) rats. Methods According to random number table method, 40 half male and half female SPF grade SD rats were randomly divided into 5 groups, with 8 rats in each group, including blank control group, ACD model group, positive control group (Splash Methylprednisolone 2.5 mg/kg), FFK low-dose group (75 mg/kg), FFK high-dose group (150 mg/kg). After ACD model were established by treatment of DNCB on right ear for 10 days, rats were treated with FFK in 10 d and the allergic reaction and swelling degree of ears were observed, the content of IL-6, IL-10 and IFN-γ in serum and the level of NF-κB p65 in ears were determined. Results Compared with the blank control group, the swelling of ears in ACD model group was significantly increased, the content of IL-6, IFN-γ in serum and the level of NF-κB p65 in ears were up-regulated, while the content of IL-10 was down-regulated in serum (P < 0.01). Compared with the ACD model group, the ears swelling degree of rats in FFK low-dose group and FFK high-dose group was significantly decreased, the content of IL-6, IFN-γ in serum and the level of NF-κB p65 in ears were significantly down-regulated, while the content of IL-10 in serum was significantly up-regulated (P < 0.01). Conclusion FFK has anti-inflammatory effect on DNBC-induced ADC rats and its mechanism involved in down-regulate the NF-κB p65.

21 Effect of Shugan Jieyu Capsules on brain-derived neurotrophic factor in model rats with depression

Objective To investigate the effect of Shugan Jieyu (SGJY) Capsules on brain-derived neurotrophic factor (BDNF) in model rats with depression. Methods Thirty SD rats were randomly divided into five groups (n = 6) by random number table method: control group, chronic unpredictable mild stress (CUMS) group, Fluoxetine (FLU) group (FLU 2 mg/kg), SGJY group (SGJY Suspension Liquid 150 mg/kg) and SGJY+U0126 group (SGJY Suspension Liquid 150 mg/kg and 0.01% U0126 5 mL/kg). The CUMS was performed to induce depressive-like animal model. Each group was given corresponding drugs for 21 d. The control group and CUMS group were given normal saline 5 mL/kg by gavage. All animals were taken behavior tests. The expression of BDNF mRNA was detected by real-time PCR, the expression of pERK1/2 protein was determined by Western blot. Results Compared with control group, the times of passing through grille, the upright times and sugar preference in the rest groups were all reduced before administration (P < 0.01). After administration, compared with CUMS group, the times of passing through grille, the upright times and sugar preference in FLU group and SGJY group were increased significantly (P < 0.01 or P < 0.05), the expression of BDNF mRNA, pERK1/2 was improved significantly (P < 0.01); compared with SGJY group, the times of passing through grille, the upright times and sugar preference in SGJY+U0126 group were decreased signfiicantly (P < 0.01 or P < 0.05), the expression of BDNF mRNA, pERK1/2 was reduced significantly (P < 0.01 or P < 0.05). Conclusion SGJY Capsules may play a role in improving the depressive symptoms by regulating the ERK1/2 pathway to increase the expression of BDNF.

25 Regulation of Sophora japonica extract on Th17/Treg cell balance in rheumatoid arthritis rats

Objective To investigate the effect of Sophora japonica extract on the balance of helper T cells/regulatory T cells (Th17/Treg) in rheumatoid arthritis rats. Methods Fifty male SD rats were fed adaptively for 7 days. Qualified rats were selected and randomly divided into normal group, model group and Sophora japonica group, with 15 rats in each group. A rat model of type Ⅱ collagen induced arthritis was established. Sophora japonica group was given 300 mg/(kg·d) of Sophora japonica extract by gastric perfusion on the 21st day of model establishment, once a day. After 4 weeks of treatment, serum levels of tumor necrosis factor-α (TNF-α), interleukin (IL)-6, IL-10, IL-17, IL-21, IL-23 and IL-27 were measured by ELISA. The ratio of Th17/Treg in peripheral blood was detected by flow cytometry. The pathological changes of ankle joint were observed by pathological section. Results Compared with the model group, the levels of TNF-α, IL-6, IL-17, IL-21 and IL-23 in the serum of rats in Sophora japonica group decreased significantly, the levels of IL-10 and IL-27 increased significantly, and the ratio of Th17/Treg in peripheral blood decreased significantly (P < 0.01). Conclusion Sophora japonica can induce cytopoiesis of Tregs while interfering with the differentiation of Th17 by regulating the related immunocytokines, which improves Th17/Treg imbalance, thus exerting therapeutic effect for rheumatoid arthritis.

29 Research progress of new drug Opicapone for Parkinson′s disease

Parkinson′s disease (PD) is the second most common neurodegenerative disease after Alzheimer′s disease. At present, the main way to treat PD is still Levodopa therapy. However, long-term use of Levodopa can lead to a series of side effects, such as dyskinesia, decreased efficacy, "on-off" phenomena, end of dose phenomena, and soon. The catechol-O-methyltransferase (COMT) inhibitor can inhibit the metabolism of surrounding Levodopa and increase the transmission amount of Levodopa, so it is used in Levodopa additional therapy to enhance the efficacy of Levodopa. As a new third-generation COMT inhibitor, Opicapone can be used in Levodopa addition therapy to improve the motor fluctuations and end of dose phenomena. Opicapone has sustained activity in vivo for a long time. The current recommended dose is 50 mg per day taken orally before bedtime. The half-life of opicapone-induced COMT inhibitors is longer than that of the previous two generations of Opicapone inhibitors, and can significantly increase Levodopa systemic exposure, with more effective, safer and more lasting properties.

34 Research progress of long noncoding RNA in primary liver cancer

Hepatocellular carcinoma (HCC) is the most common primary liver cancer in clinic. The prevalence of HCC in China is higher around the world. HCC is highly malignant, difficult to treat, prone to recurrence, with poor prognosis. Therefore, more and more studies focus on finding new molecular markers for the diagnosis, treatment and prognosis evaluation of HCC. Long noncoding RNA (lncRNA) is one of the non-coding RNA. The imbalanced expression of lncRNA acts as oncogene or tumor suppressor gene and participates in the occurrence and development of malignant tumors. Recent studies have found that the imbalance of lncRNA expression is involved in many biological processes of HCC. This paper mainly summarizes recent studies on the role of lncRNA in the occurrence and development of HCC tumors, and provides theoretical basis for the use of lncRNA in the diagnosis, prognosis or treatment of HCC.

38 Research progress on the status of nosocomial infection in neurosurgical inpatients

Neurosurgery is a department with high incidence of nosocomial infection, and nosocomial infection has different clinical characteristics in different times, regions or hospitals. This paper mainly analyzes the characteristics, risk factors, location, common pathogenic bacteria and drug resistance of nosocomial infection in neurosurgical patients, as well as countermeasures, so as to provide reference for clinical prevention and control of nosocomial infection in neurosurgery, and thus reduce the adverse effects of infection on patients.

41 Research progress on the relationship between chronic heart failure and lymphocytes and cytokines

Chronic heart failure (CHF) is a complex clinical symptom group. As the ultimate outcome of most cardiovascular diseases, it is one of the main causes of death of heart disease. The pathogenesis of CHF is complex. In recent years, with the deepening of research, it has been found that immune factors play an important role in the disease process. B lymphocyte and T lymphocyte damage myocardium through different ways of action, induce and aggravate pathological ventricular remodeling, and ultimately accelerate the occurrence and development of CHF. Tumor necrosis factor-α and interleukin-6, as the representatives of proinflammatory cytokines, can accelerate the development of CHF. Cytokines such as IL-33 can protect the heart and delay the progression of CHF. This article reviews the recent research progress of lymphocytes and cytokines related to the occurrence and development of CHF in order to provide new ideas for its clinical treatment in the future.

45 Advances in research on cell-cell interactions in oral squamous cell carcinoma tumor microenvironment

Oral squamous cell carcinoma (OSCC) tumor microenvironment (TME) includes tumor cells, tumor-associated macrophages (TAMs), tumor-associated fibroblasts (CAFs), and endothelial cells. During the occurrence and development of OSCC, tumor cells produce certain substances that affect the recruitment, differentiation and activation of TAMs and CAFs. TAMs and CAFs secrete cytokines to promote tumor cell proliferation, invasion, metastasis and other activities. The interaction between them forms a favorable TME, which plays an important role in the various stages of tumorigenesis and development. This article reviews the recent advances in the interaction of tumor cells with TAMs and CAFs and the role of CAFs in TAMs.

49 Research advances of adipose-derived stem cells on bone repair

Bone tissue engineering consists of three main factors: stem cells for osteogenesis, growth factors for bone induction and biodegradable scaffolds for bone conduction. Mesenchymal stem cells have achieved significant success in repairing bone defects, and their safety has been demonstrated in a number of preclinical trials. Adipose-derived stem cell (ADSCs) can also differentiate into osteoblasts, chondrocytes, myocytes and adipocytes, and they have similar differentiation potential to BMSCs with a minimally invasive approach. Meanwhile, their yield is also higher than BMSCs, which has a broad clinical application prospect.

53 Treatment status of lobular hyperplasia of mammary gland

Lobular hyperplasia of mammary gland is a common hyperplasia of mammary glands of women in reproductive age. Western medicine mainly uses hormone therapy. The treatment of traditional Chinese medicine (TCM) starts from the concept of holism, and adopts treatment based on syndrome differentiation, mostly from liver, and TCM takes dispersing stagnated liver qi and regulating qi-flowing, dissipating phlegm and resolving masses, promoting blood circulation for removing blood stasis as therapeutic principle, and takes care of spleen and kidney as well, at the same time, TCM pay attention to both whole and partial, makes combination of endotherapy and external treatment, takes the methods of Chinese herb decoction for oral administration, Tuina, acupuncture and moxibustion, scrapping therapy, moving cupping, and so on, the effect is significant. At present, the combination of traditional Chinese and Western medicine with psychological counseling, as well as standardized life diet intervention treatment has obvious advantages. Therefore, it is suggested that clinical practice should be tailored according to the actual situation of patients and based on health care and clinical decisions, which is the individualized treatment plan based on healthy lifestyle under the guidance of precise medicine, it is in line with modern bio-psycho-social medical model.

57 Research status on the operating efficiency in traditional Chinese medicine hospitals

Traditional Chinese medicine (TCM) hospital is an essential component in the whole healthcare system in China, the operating efficiency of which has great impact on the utilization of TCM resources. This article summarizes the research status from following aspects: models for evaluating efficiency of TCM hospitals, input-output indicators, current efficiency status of TCM hospitals, and its influential factors, and then provide some suggestions for to promoting TCM hospitals′ efficiency and making full use of TCM resources.

60 Retrospective study on the epidemiological characteristics of colorectal cancer

Objective To explore the epidemiological characteristics of colorectal cancer admitted and treated by the First Affiliated Hospital of Hebei North University ("our hospital" for short) in recent 5 years, and to provide decision basis for the government to formulate cancer prevention and control strategies. Methods A total of 696 cases of colorectal cancer patients admitted to our hospital from January 2013 to December 2017 were selected as research objects, and data including age, gender, regional distribution, lesion site, differentiation degree, pathological type and main symptoms were analyzed and summarized. Results A total of 111, 119, 130 cases, 147 cases and 189 cases were counted from 2013 to 2017. Among them, 414 were male and 282 were female. The age of hair is 50 - 70 years old, especially 60 - 70 years old. There were 427 cases in urban areas and 269 cases in rural areas. There were 457 cases of rectal cancer and 239 cases of colon cancer. The pathological type was mainly papillary adenocarcinoma, and signet-ring cell carcinoma was mostly female. Symptoms were mainly bloody stool and abdominal discomfort. Conclusion In the past 5 years, the number of colorectal cancer patients admitted to our hospital has been increasing year by year, more men than women, more urban patients than rural patients, the incidence of rectal cancer is much higher than the colon nearly 2 times, mainly papillary and median-differentiated adenocarcinoma, female patients should pay attention to the occurrence of signet-ring cell carcinoma; Therefore, it is necessary to gradually improve the three-level prevention measures of colorectal cancer, strengthen colonoscopy screening and education, and improve the early diagnosis rate.

64 Analysis of influencing factors of red blood cell distribution width in patients with chronic heart failure

Objective To investigate the influencing factors of red blood cell distribution width (RDW) in patients with chronic heart failure (CHF). Methods From September 2016 to May 2018, 136 patients with CHF who were hospitalized in the Department of Cardiovascular Medicine, Fenyang Hospital Affiliated to Shanxi Medical University were enrolled. According to the RDW level, patients were divided into low RDW group (RDW < 13.2%, 43 cases), medium RDW group (RDW = 13.2% - 14.5%, 47 cases) and high RDW group (RDW > 14.5%, 46 cases). The basic clinical data, laboratory data and activities of Na+/K+-ATPase and Ca2+/Mg2+-ATPase in erythrocyte membranes of the three groups were compared, and the factors affecting the changes of RDW in CHF patients were analyzed by multiple linear regression. Results There were significant differences in hemoglobin, albumin, direct bilirubin, left ventricular ejection fraction (LVEF), log N-terminal brain natriuretic peptide (NT-proBNP), Na+/K+-ATPase and Ca2+/Mg2+-ATPase activities in erythrocyte membrane among the groups (P < 0.05). RDW in patients with CHF was associated with LVEF, albumin, direct bilirubin, Na+/K+-ATPase, Ca2+/Mg2+-ATPase activity in erythrocyte membrane, and the regression coefficients were -0.104, -0.133, 0.111, -0.577, -0.263, respectively (P < 0.05). Conclusion LVEF, albumin, direct bilirubin, Na+/K+-ATPase and Ca2+/Mg2+-ATPase activity in erythrocyte membrane are the factors affecting RDW in patients with CHF. RDW has certain value in evaluating the condition, therapeutic effect and prognosis of CHF.

68 Correlation between serum S100B protein expression and RAGE levels in patients with chronic heart failure and its clinical significance

Objective To investigate the correlation between serum calcium binding protein B (S100B) expression and glycation end product receptor (RAGE) level in patients with chronic heart failure. Methods A total of 151 patients with chronic heart failure (observation group) and 150 healthy normal volunteers (control group) from the Second Affiliated Hospital of Hainan Medical University from January 2016 to February 2018 were selected. The levels of S100B, esRAGE and cRAGE were compared between the two groups and the correlation between S100B and esRAGE and cRAGE was analyzed. Results S100B and cRAGE in the observation group were significantly higher than those in the control group (P < 0.05), esRAGE was significantly lower than that in the control group (P < 0.05). S100B was negatively correlated with esRAGE (r = -0.876, P = 0.032), and S100B was positively correlated with cRAGE (r = 0.987, P = 0.028). Conclusion The severity of chronic heart failure is related to S100B and RAGE. The elevation of S100B in patients is correlated with the level of RAGE. The above indexes are abnormal, which has a good clinical value in the diagnosis and judgment of the disease.

72 Analysis of influencing factors of carotid plaque in patients with type 2 diabetes mellitus

Objective To investigate the risk factors of carotid plaque in patients with type 2 diabetes mellitus. Methods A total of 192 patients with type 2 diabetes mellitus who were admitted and hospitalized in Affiliated Hospital of Chengde Medical University from January 2016 to January 2018 were selected for the study. They were divided into plaque group (n = 101) and non-plaque group (n = 91) according to intima-media thickness (IMT). Clinical data and laboratory indicators were collected and compared between the two groups. Risk factors of carotid plaque were analyzed by binary multivariate Logistic analysis. Results The age, waist circumference, duration of diabetes mellitus, number of smokers, glycosylated hemoglobin (HbA1c), triglyceride (TG), low density lipoprotein cholesterol (LDL-C) levels in plaque group were significantly higher than those in non-plaque group (P < 0.05), while 25-hydroxyvitamin D3 [25 (OH) D3] levels and the number of patients taking statins were significantly lower than those in non-plaque group (P < 0.05), the differences were statistically significant (P < 0.05). Multivariate Logistic analysis showed that the duration of diabetes mellitus (OR: 1.06, 95%CI: 1.03-1.08, P = 0.021), smoking (OR: 1.13, 95%CI: 1.10-1.16, P = 0.005), HbA1c (OR: 1.46, 95%CI: 1.24-1.54, P = 0.010) and LDL-C (OR: 1.28, 95%CI: 1.16-1.42, P = 0.014) were the risk factors for carotid artery plaque, and elevated level of 25(OH)D3(OR: 0.84,95%CI: 0.82-0.87, P = 0.000) and taking statins(OR: 0.78,95%CI: 0.72-0.82, P = 0.039)were the protective factors. Conclusion With the prolongation of the course of disease, the risk of carotid plaque in type 2 diabetes mellitus increased. Smoking, elevated levels of HBA1c and LDL-C all can increase the risk of carotid plaque, while high level of 25 (OH) D3 and taking statins may delay the development of carotid plaque.

76 Expression and correlation of serum sPD-L1 and VEGF in patients with acute myeloid leukemia

Objective To investigate the expression and correlation of serum soluble programmed death ligand 1 (sPD-L1) and vascular endothelial growth factor (VEGF) of patients with acute myeloid leukemia. Methods Eighty-nine patients with acute myeloid leukemia admitted to Dongguan Kanghua Hospital (“our hospital” for short) from January 2015 to January 2018 were selected as research objects and enrolled into observation group, and 50 healthy subjects who underwent health examination in our hospital during the same period were selected as the control group. The sPD-L1 and VEGF levels of the two groups were detected by enzyme-linked immunosorbent assay (ELISA) and compared between the two groups. The patients in the observation group were treated with standardized therapy for 2 months, according to the criteria of curative effect, the patients were divided into complete remission (CR) group (36 cases), partial remission (PR) group (40 cases) and proceed (PD) group (13 cases), and the levels of sPD-L1 and VEGF in the three groups were detected and compared. The correlation between sPD-L1 and VEGF was also analyzed. Results Before treatment, the levels of sPD-L1 and VEGF in the observation group were significantly higher than those in the control group, the differences were statistically significant (P < 0.05). After treatment, the levels of sPD-L1 and VEGF in CR group and PR group were significantly lower than those in PD group, and the levels of sPD-L1 and VEGF in CR group were lower than those in PR group, the differences were statistically significant (P < 0.05). Pearson correlation test showed that sPD-L1 was positively correlated with VEGF (r = 0.447, 0.327, all P < 0.05). Conclusion The expression of sPD-L1 and VEGF in acute myeloid leukemia patients is high, and the levels of sPD-L1 and VEGF are decreased with the improvement of prognosis. Therefore, the detection of sPD-L1 and VEGF has certain assistant effects and clinical value for the diagnosis, treatment and prognosis evaluation of acute myeloid leukemia patients.

80 Effects of Shugan Jieyu Capsules combined with antidepressant in the treatment of post-stroke depression

Objective To observe the effect of Shugan Jieyu Capsules combined with antidepressant in the treatment of post-stroke depression. Methods From July 2016 to July 2018, 128 patients with post-stroke depression (PSD) treated in the Inner Mongolia Mental Health Center were selected and divided into control group and observation group according to random number table method, with 64 patients in each group. The control group received conventional antidepressant treatment, and the observation group was treated with Shugan Jieyu Capsules on the basis of antidepressant. Both groups were treated for 6 weeks. Hamilton depression rating scale (HAMD) was used to evaluate the efficacy and depression degree. Neurological function was assessed using the National Institutes of health stroke scale (NIHSS). Quality of life was assessed by referring to the nurses′ observation scale for inpatient evaluation (NOSIE). Adverse reactions were observed in the two groups. Results The total effective rate of observation group was significantly higher than control group (P < 0.01). After 6 weeks, the NIHSS score of the observation group was lower than that of the control group, and the difference was highly statistically significant (P < 0.01). HAMD scores in the observation group after 6 weeks were lower than those in the control group (P < 0.01). There was no statistically significant difference in the rate of adverse reactions between the two groups (P > 0.05). Both groups showed mild nausea, headache, constipation and other reactions, which spontaneously subsided after 2-3 days. The scores of depression, personal hygiene, social interest and social ability of the observation group were lower than those of the control group (P < 0.01). Conclusion The combined treatment of Shugan Jieyu Capsules and antidepressant for PSD has a definite effect, which can relieve the symptoms of depression, improve neurological function and the quality of life. Only mild headache, nausea and other reactions occur, with no serious adverse reactions, it is safe and effective.

84 Analysis on the clinical characteristics of early and late onset epilepsy after acute ischemic stroke

Objective To investigate the clinical characteristics of early and late onset epilepsy after acute ischemic stroke. Methods Clinical data of 3108 patients with acute ischemic stroke in the Department of Neurology of Fujian Provincial Hospital from June 2012 to June 2016 was retrospectively analyzed, with long-term follow-up (2-6 years). A total of 108 patients with epilepsy after stroke were divided into early-onset epilepsy group (41 cases) and late-onset epilepsy group (67 cases) depends on the time of attack. The clinical and imaging data, classification of seizures, occurrence rate of epileptic status and eeg results of the two groups were analyzed. Results The proportion of males in the late-onset epilepsy group was higher than that in the early-onset epilepsy group (P < 0.05). The mean age of later-onset epilepsy group was lower than that of early-onset epilepsy group (P < 0.05). The seizure types in the early onset epilepsy group were mainly simple or complex partial seizure, while those in the late onset epilepsy group were mainly partial secondary comprehensive seizure (P < 0.05). Cerebrovascular disease risk factors, National Institutes of health stroke scale score, occurrence rate of epileptic status, abnormal occurrence rate of electroencephalogram, cortical involvement rate, occurrence rate of hemorrhagic cerebral infarction, etc. were compared between the two groups, and the differences were not statistically significant (P > 0.05). Conclusion The patients with delayed epilepsy after acute ischemic stroke are younger and more male. The main attack form of early onset patients is simple or complex partial attacks, and the main attack form of late onset patients is partial secondary comprehensive attacks.

88 Analysis of risk factors for relapse of lacunar cerebral infarction in elderly men with alcohol addiction

Objective To investigate the risk factors of lacunar cerebral infarction recurrence in elderly men with alcohol addiction. Methods Clinical data of 97 elderly male patients with alcohol addiction with lacunar cerebral infarction admitted to the Department of Neurology of Beijing Daxing District Hospital of Integrated Chinese and Western Medicine was retrospectively analyzed. According to their medical history, they were divided into the initial group (50 patients with the initial onset of a cavity obstruction) and the recurrence group (47 patients with ≥2 times recurrence of a cavity obstruction). The patient′s age, body mass index, amount of alcohol dependence, smoking history, and medical history were compared between the two groups, the method of multi-factor Logistic regression was used to analyze the related risk factors of lacunar cerebral infarction recurrence in elderly men with alcohol addiction, the Cochran-Armitage trend test was used to determine whether there was a linear trend between the daily alcohol dependence and the recurrence of lacunar cerebral infarction. Results The proportion of patients with hypertension, smoking history, diabetes history, hyperlipidemia history, atrial fibrillation history, transient ischemic attack history and daily alcohol dependence in the relapse group were significantly higher than those in the primary group (all P < 0.05); the results of multivariate Logistic regression analysis showed that the history of diabetes, history of atrial fibrillation, history of transient cerebral ischemia and daily alcohol dependence were all risk factors for the recurrence of lacunar cerebral infarction in elderly males, with regression coefficients of 1.80, 1.06, 1.60 and 2.11, respectively. The corresponding P values were 0.03, 0.04, 0.04 and 0.01 respectively, analysis of Cochran-Armitage trend showed that there was a linear trend between the daily alcohol dependence of elderly men and the recurrence of lacunar cerebral infarction, and the correlation coefficient was r = 0.28, P = 0.01. Conclusion Clinical intervention should be strengthened for risk factors such as diabetes, atrial fibrillation, transient ischemic attack and daily alcohol dependence, so as to reduce the recurrence rate of lacunar cerebral infarction in elderly men.

91 Correlation between lymphovascular space invasion and prognosis of endometrial carcinoma

Objective To study the clinicopathological features and prognosis of endometrial carcinoma with lymphovascular space invasion (LVSI). Methods The data of 671 cases of endometrial carcinoma treated in Nanjing Drum Tower Hospital from January 2007 to December 2017 was retrospectively analyzed, including 120 cases in LVSI positive group and 551 cases in LVSI negative group. The clinicopathological information of patients was analyzed, and Kaplan-Meier curves were established. Univariate and multivariate analyses were performed. Results Compared with LVSI negative group, LVSI positive group was significantly higher in proporation of patients with menopause, late stage of operation and pathology, infiltration of deep muscle layer, poor tissue differentiation, lymph node metastasis, recurrence and death, the differences were statistically significant (P < 0.05). There was no statistical difference between the two groups in the proporation of patients with medical complications of hypertension, diabetes and uterine leiomyoma (P > 0.05). Multivariate analysis showed that cervical stroma involvement (OR = 0.342, 95%CI: 0.134-0.873, P < 0.05) and LVSI (OR = 0.110, 95%CI: 0.051-0.238, P < 0.01) were significantly correlated with lymph node metastasis. Survival rate analysis showed that both OS and RFS were significantly decreased in LVSI positive patients (P < 0.05). The results of COX regression showed that the survival time and outcome of endometrial carcinoma were affected by the late stage of operation and pathology, the nonendometrial carcinoma and LVSI (P < 0. 05). Conclusion LVSI is closely related to lymph node metastasis and may be an important predictor of lymph node metastasis. LVSI is an important risk factor for poor prognosis of endometrial carcinoma.

95 Effect of different delivery modes on pelvic floor electromyography and muscle strength in early postpartum period

Objective To investigate the effect of different delivery methods on pelvic floor electromyography and muscle strength in early postpartum period. Methods Total of 200 parturients who were examined and delivered in Liaocheng Dongchangfu District Maternal and Child Health Hospital of Shandong Province from June 2015 to October 2018 were selected as the research objects. They were divided into cesarean section group (n = 98) and vaginal delivery group (n = 102), vaginal delivery group was divided into lateral section group (n = 53) and non-lateral section group (n = 49) according to the different modes of delivery. The pelvic floor myoelectricity and muscle strength were compared between cesarean section group and vaginal delivery group, lateral incision group and the non-lateral incision group before delivery, 6, 12 and 20 weeks postpartum. Results The peak value, activity value and velocity value of pelvic floor potential in cesarean section group and vaginal delivery group decreased first and then increased at 6,12 and 20 weeks postpartum (P < 0.05). There was no significant difference in pelvic floor potential activity between cesarean section group and vaginal delivery group at 6 weeks postpartum (P > 0.05). The peak value of pelvic floor muscle potential, pelvic floor muscle activity and pelvic floor velocity at other time points were higher than those in vaginal delivery group (P < 0.05). There was no statistical difference in peak pelvic floor potential, pelvic floor activity and pelvic floor velocity between the two groups (P > 0.05), and the peak pelvic floor potential, pelvic floor activity and pelvic floor velocity of the two groups was lower 6 weeks postpartum than those before delivery (P < 0.05). There was no statistical difference in the decrease rate of muscular strength of type Ⅰ and Ⅱ muscles between cesarean section group, vaginal delivery group and lateral section group and non-lateral section group (P > 0.05). Conclusion The influence of cesarean section and vaginal delivery on the decrease of pelvic floor muscle strength is not different, and lateral incision during vaginal delivery can not reduce the influence of pelvic floor muscle strength after delivery. The effect of cesarean section on pelvic floor EMG in early postpartum period is slight.

100 Diagnostic values of serum MiR-155 combined with HE4 and CA199 in endometrial cancer

Objective To investigate the diagnostic values of serum miR-155 combined with human epididymal protein 4 (HE4) and carbohydrate antigen 199 (CA199) in endometrial cancer (EC). Methods Thirty-right patients with EC admitted to Oncology Department of Handan First Hospital in Hebei Province from June 2016 to June 2018 were selected as the EC group, 60 cases of endometrial hyperplasia patients were selected as the endometrial hyperplasia group, 60 healthy persons were taken as control group, the levels of serum microRNA-155, HE4 and CA199 were detected and compared among the three groups, and their diagnostic value for EC patients was analyzed. Results The levels of serum miR-155, HE4, and CA199 in EC group and endometrial hyperplasia group were higher than those in control group (P < 0.05), and those in EC group were higher than those in endometrial hyperplasia group (P < 0.05); the levels of serum miR-155, HE4, and CA199 in EC patients with pathological grades G3 and G2 were higher than those in patients with grade G1 (P < 0.05), while those in patients with grade G3 were higher than those in patients with grade G2 (P < 0.05); the levels of serum miR-155, HE4, and CA199 in EC patients with stage Ⅱ, Ⅲ, and Ⅳ of FIGO were higher than those in patients with stage Ⅲ (P < 0.05), those in patients with stage Ⅲ and Ⅳ were higher than those in patients with stage Ⅱ (P < 0.05), and those in patients with stage Ⅳ were higher than those in patients with stage Ⅲ (P < 0.05). The ROC curve showed that the AUC, sensitivity and specificity of the combined detection were higher than those of the single detection. Conclusion Serum miR-155, HE4, and CA199 are highly expressed in EC patients, and with the increases of pathological grade and FIGO stage, the sensitivity and specificity of the combined diagnosis are high, which has high clinical value for early diagnosis of EC patients.

104 Research on comparison of internal quality control data among prenatal eugenics laboratories for clinical chemistry

Objective To analyze internal quality control data among prenatal eugenics laboratories for clinical chemistry, to understand the status of clinical chemical quality control in Jiangsu Province, and to explore methods and evaluation indicators for indoor quality control data comparison. Methods The indoor quality control data of 3 routine biochemical test items in 10 laboratories using the same quality control from November 2017 to April 2018 were collected and analyzed. And the monthly average (X), standard deviation (SD), coefficient of variation (CV), and the standard deviation index (SDI) and coefficient of variation index (CVI) of each objective were calculated, the consistency of the analysis results was compared simultaneously. Results High-value control materials: CV satisfaction laboratory proportion of three testing items: 1/3 TEa standard and provincial control standard were both > 90%, while laboratories satisfying 1/4TEa accounted for >80%; low value quality control: the proportion of laboratories satisfying CV of 3 test items: 1/3 TEa standard >90%, provincial control standard >85%, and 1/4TEa standard >75%. The total qualified rate of SDI and CVI was 87% and 77% respectively. Conclusion SDI and CVI can be used to evaluate the quality of analysis in clinical laboratories, which is conducive to timely detection of problems in the laboratory, improve and improve the detection level.

108 Effect of Dexamethasone intrauterine perfusion on E2R, TGF- beta 1 and VEGF in endometrium with moderate and severe intrauterine adhesion

Objective To investigate the effect of intrauterine perfusion of Dexamethasone on the estradiol receptor (E2R), transfotruing growth factor-beta 1 (TGF-β1), vescular endothelial growth factor (VEGF) of endometrium with moderate and severe intrauterine adhesion. Methods From October 2016 to June 2017, 120 patients with moderate and severe intrauterine adhesion in Nanshan District People′s Hospital of Shenzhen City, Guangdong Province were selected as the research objects, and they were divided into research group and control group by the random number group, with 60 cases in each group. The research group were given Dexamethasone 5 mg + normal saline 10 mL of perfusion palace 10 min; and the control group was perfused with the same amount of normal saline. The level changes of E2R, TGF-β1 and VEGF before and after artificial treatment were observed. Results There were statistically significant differences in the total effective treatment for 1 and 3 months after artificial treatment between the two groups (P < 0.05); the total effective rate of research group 3 months after artificial treatment was higher than that 1 month after artificial treatment, the difference was statistically significant (P < 0.05). The E2R, TGF-β1 and VEGF of the two groups 1 and 3 months of after artificial treatment were all lower than those before treatment of the same group, and those 3 months after artificial treatment were lower than 1 month after artificial treatment, the differences were statistically significant (P < 0.05). The E2R, TGF-β1 and VEGF of the research group 1 and 3 months after artificial treatment were all lower than those of the control group, the differences were statistically significant (P < 0.05). Conclusion Dexamethasone can alter the expression of E2R, TGF-β1 and VEGF in endometrium samples, and can assist in the post-hysteroscopic treatment of patients with moderate and severe intrauterine adhesion by intrauterine perfusion.

112 Effect of nasal rinse frequency on adverse reactions associated with radical radiotherapy for nasopharyngeal carcinoma

Objective To study the therapeutic effect of different nasal flushing frequency on radiotherapy related adverse reactions in patients with nasopharyngeal carcinoma. Methods A total of 108 patients received radical radiotherapy for nasopharyngeal carcinoma in the Oncology Department of Cancer Center, Renmin Hospital of Wuhan University from January 2013 to December 2016 were selected. According to the random number table method, the patients were divided into two groups, group A and group B, who received nasal irrigation with nasal cola irrigator. The frequency of irrigation in group A was 3 times/d, and the frequency of irrigation in group B was 5 times/d, and the irrigation time was from the day of radiotherapy to 1 year after the end of radiotherapy. The incidence and efficacy of mucosal reactions, pain and anxiety in the two groups were recorded. Results The incidence of mucosa reaction in group B was significantly better than that of group A, the incidence of moderate or above degree of pain and anxiety scores were lower than those of group A, the difference was statistically significant (P < 0.05), However, there was no statistically significant difference in the short-term efficacy between the two groups (P > 0.05). Conclusion In patients with nasopharyngeal carcinoma receiving radical radiotherapy, nasal flushing for 5 times/d can further reduce local mucosal adverse reactions, relieve pain and anxiety, and improve patients′ treatment experience and quality of life compared with 3 times/d.

116 Relationship between the ratio of peripheral blood neutrophils and platelet to lymphocytes in patients with gastric cancer and the pathological characteristics of gastric cancer

Objective To explore the correlation of serum neutrophil-to-lymphocyte ratio (NLR) and platelet-to-lymphocyte ratio (PLR) with pathological features of gastric cancer in patients with gastric cancer. Methods A total of 457 patients with gastric cancer who were admitted to the Affiliated Hospital of Xuzhou Medical University (“our hospital” for short) from January 2015 to August 2018 were enrolled in the study. The 200 healthy people taken physical examination in our hospital during the same period were used as the control group. The differences of NLR and PLR between the two groups were analyzed retrospectively. And the correlation between tumor invasion depth or lymph node metastasis or TNM staging and NLR or PLR was analyzed. Results NLR and PLR in the gastric cancer group were significantly higher than those in the control group, and the difference was statistically significant (P < 0.05). The patients in NLR elevation group (NLR≥2.44) were more likely to have larger tumor diameters, worse differentiation, deeper infiltration depth, more lymph node metastasis, and later TNM than those in the NLR reduction group (NLR<2.44), as well as PLR elevation group (PLR≥176.59) compared with PLR reduction group (PLR<176.59) (P < 0.05). The proportion of lymph node metastasis in patients with NLR elevation + PLR elevation group was significantly higher than that in NLR reduction + PLR elevation, NLR elevation + PLR reduction and NLR reduction + PLR reduction groups (P < 0.05). The lymph node metastasis rate of NLR reduction + PLR elevation group and NLR elevation + PLR reduction group was significantly higher than NLR reduction + PLR reduction group (P < 0.05), there was no significant difference in lymph node metastasis rate among the other groups (P > 0.05). There was a significant positive correlation between NLR and PLR (r = 0.648, P < 0.05). Conclusion The higher the NLR and PLR of gastric cancer patients, the deeper the degree of tumor infiltration, the lower the degree of differentiation, the greater the number of lymph node metastases, the larger the tumor diameter and the later the TNM stage, the worse the prognosis.

121 Expression and clinical significance of UCA1 and miR-141 in bladder cancer tissue

Objective To study the expression and clinical significance of urothelial carcinoma antigen 1 (UCA1) and micoRNA-141 (miR-141) in bladder cancer tissue. Methods A total of 83 patients with bladder cancer admitted to Linyi Cancer Hospital from February 2013 to February 2015 were taken as research objects. The expression of UCA1 and miR-141 were detectd by real-time fluorescent quantitative polymerase chain reaction and the relationship between the expression of UCA1 and miR-141 and the clinicopathological characteristics of the patients was analyzed. In addition, all patients were followed up for 3 years and divided into survival group (n = 39) and death group (n = 44) according to different survival conditions. The expression of UCA1 and miR-141 was compared between the two groups. Results The positive rate of UCA1 and miR-141 in bladder cancer tissues was higher than those in adjacent normal tissues (all P < 0.05). The positive rate of UCA1 in bladder cancer with pathological grade G1, clinical stage T1-T2 and no lymph node metastasis was lower than that in bladder cancer with pathological grade G2-G3, clinical stage T3-T4 and lymph node metastasis (all P < 0.05). The positive rate of miR-141 in bladder cancer tissues with clinical stage T1-T2 and no lymph node metastasis was lower than that in bladder cancer tissues with clinical stage T3-T4 and lymph node metastasis (all P < 0.05). The positive rate of UCA1 and miR-141 in survival group was lower than those in death group (all P < 0.05). Conclusion UCA1 and miR-141 is highly expressed in bladder cancer tissues. The two indicators mentioned above can be used in clinical work to evaluate the condition and prognosis of patients with bladder cancer.

125 Clinicopathological features and prognosis of patients with gastric stromal tumors

Objective To explore the clinicopathological features and prognostic factors of gastric stromal tumors further. Methods Clinical data, pathological characteristics and survival status of 108 patients with gastric stromal tumors admitted to the First Affiliated Hospital of Kunming Medical University from February 2009 to November 2012 were analyzed retrospectively. Survival analysis was used Kplan-Meier method to calculate 1, 3, 5-year survival rates. Log-rank test was used for univariate analysis and COX proportional hazards model was used for prognostic multivariate analysis. Results One hundred and eight patients were enrolled in the study. The male was 57 cases, the female was 51 cases, the age was (57.54±12.03) years, the operation time was (155.53±82.86) min, the intraoperative bleeding volume was (165.46±329.74) mL, the postoperative feeding time was (7.71±2.62) days, and the hospitalization days after operation were (11.80±7.54) days. The overall survival rates for 1, 3, 5 years were 100.0%, 91.6% and 76.3%. Univariate prognostic analysis showed that tumor size, mitotic numbe, modified NIH classification were closely related to prognosis of gastric stromal tumors (P < 0.05). Multivariate prognostic analysis showed that modified NIH risk classification (HR = 6.177, 95%CI: 1.348-28.299, P = 0.019) was an independent prognostic risk factor for patients with gastric stromal tumors. Conclusion Gastric stromal tumors are the most common digestive tract stromal tumors. The overall prognosis is good. Surgical resection is the main treatment. The modified NIH risk classification is the main risk factor affecting prognosis. The higher the risk, the worse the prognosis.

129 Expression and significance of SATB2 in ovarian mucinous tumors

Objective To determine the expression of special AT-rich sequence binding protein 2 (SATB2) in primary ovarian mucinous carcinoma and metastatic ovarian mucinous adenocarcinoma from colon and rectum, and analyze its significance for clinicopathological diagnosis. Methods Forty-seven cases of patients with primary ovarian mucinous tumors and 48 cases of patients with metastatic ovarian mucinous adenocarcinoma from colon and rectum treated in Cangzhou People′s Hospital of Hebei Province from January 2010 to June 2017 was selected. The expression of SATB2, cytokeratin 7 (CK7), cytokeratin 20 (CK20), caudal-regulated homeobox transcription factor 2 (CDX2) in paraffin specimens of both tumor tissues and ascites was determined by the immunohistochemical method. The expression of SATB2, CK7, CK20 and CDX2 in tumor tissues and ascites was analyzed and compared. The sensitivity and specificity of SATB2 combined with CK7, CK20 and CDX2 for the diagnosis of the origin of ovarian mucinous tumors was analyzed. Results The expression of SATB2 was not related to patient′s age and tumor diameter (P > 0.05), but related to the degree of tissue differentiation, pathological classification, ovarian involvement and clinical stage (P < 0.05). The expression of SATB2, CK7, CK20 and CDX2 was related to the origin of ovarian mucinous tumors (P < 0.05). The sensitivity and specificity of SATB2 combined with CK7 in determining the origin of primary ovarian mucinous tumors and metastatic colorectal mucinous adenocarcinoma were significantly higher than CK20 and CDX2. Conclusion SATB2 is specific in distinguishing tumor origin, it is valuable for clinical pathological diagnosis.

133 Effects of Sofren Injection on quality of life and immune function in patients with advanced breast cancer

Objective To observe the effects of Sofren Injection on quality of life and immune function in patients with advanced breast cancer. Methods Form June 2014 to June 2016, 56 cases of breast cancer patients with Ⅳ stage who received chemotherapy treatment in Hengshui Harrison International Peace Hospital were selected and divided into observation group and control group by the random number table method, with 28 cases in each group. Both groups selected Paclitaxel combined with Cisplatin chemotherapy regiments, and the observation group was given Sofren Injection by intravenous drip on the basis of the chemotherapy regiments. After 2 cycles of chemotherapy, the clinical efficacy was monitored and evaluated. The indexes related to cell immunology in the two groups were observed: CD3+, CD4+, CD4+/CD8+, CD8+ and NK values were measured and calculated before and after chemical drug treatment in the two groups. The blood routine, liver and kidney function and cardiac function of the patients were measured, and the adverse drug reactions and grades during the hospitalization were recorded. Results After 2 cycles of treatment, CD3+, CD4+, CD4+/CD8+ and NK cells in the two groups were all higher than those before chemotherapy, and CD8+ was lower than before treatment, with statistically significant differences (P < 0.05). CD3+, CD4+, CD4+/CD8+ and NK cells in the observation group were significantly higher than those in the control group after treatment, CD8+ was lower than that of the control group, and the difference was statistically significant (P < 0.05). The incidence of cardiotoxicity, nausea and vomiting in the observation group was significantly lower than that in the control group during the combined treatment, with statistically significant differences (P < 0.05). The improvement rate of life quality in the observation group was better than that in the control group, and the difference was statistically significant (P < 0.05). There was no statistically significant difference in efficacy between the two groups (P > 0.05). Conclusion Large strains of Sofren Injection combined with chemotherapy drugs can significantly improve the cellular immunity and quality of life of patients with advanced breast cancer.

137 Expression and clinical significance of IL-27 in adults with bronchial asthma during acute attack stage

Objective To study expression and clinical significance of IL-27 in adults with bronchial asthma ("asthma" for short) during acute attack stage. Methods Ninety-seven cases of adults with asthma were selected from the Third People′s Hospital of Dongguan City in Guangdong Province from January 2016 to January 2017. They were divided into seizure group (54 cases) and remission group (43 cases) according to symptoms and signs, another 40 cases healthy adults were selected as control group at the same time. The serum level of immunoglobulin E (IgE), interferon-γ (IFN-γ), interleukin (IL)-12, IL-27 and transforming growth factor-β (TGF-β) was measured and compared in all the researchers. Canon AS-505 pulmonary function instrument was used to measure the percentage of forced expiratory volume in the first second (FEV1% pred) and the maximum expiratory rate in the first second (FEV1/FVC). The correlation of the above indicators were analyzed. Results IgE and TGF-β in remission group and seizure group were significantly higher than those in control group (all P < 0.05), and seizure group was higher than remission group (all P < 0.05). The levels of IFN-γ, IL-12 and IL-27 in remission group and seizure group were significantly lower than those in control group (all P < 0.05), and IL-12 and IL-27 in seizure group were lower than those in remission group (all P < 0.05). FEV1% pred and FEV1/FVC in seizure group were lower than those in remission group and control group (all P < 0.05), FEV1% pred and FEV1/FVC in remission group were also lower than those in control group (all P < 0.05). IgE and TGF-β were negatively correlated with FEV1% pred and FEV1/FVC (P < 0.05). IFN-γ, IL-12 and IL-27 were positively correlated with FEV1% pred and FEV1/FVC (P < 0.05). Conclusion Serum IL-27 in adults with asthma during acute attack stage is elevated, and it is positively correlated with pulmonary function status. It is important to evaluate the severity of asthma.

141 Value of detection of VEGF,EGFR and CD40 in differential diagnosis of benign and malignant pleural effusion

Objective To explore the value of detecting vascular endothelial growth factor (VEGF), epidermal growth factor (EGFR) and CD40 in differential diagnosis of benign and malignant pleural effusion. Methods Sixty patients with lung cancer and pleural effusion hospitalized and diagnosed in the First Affiliated Hospital of Gannan Medical University from June 2015 to June 2017 were selected as observation group. 60 tuberculous pleurisy patients with pleural effusion in the same period were selected as control group. The expression levels of VEGF, EGFR and CD40 in pleural effusion samples of all patients were detected and compared between two groups. The diagnostic value of each index for malignant pleural effusion was analyzed by receiver operating characteristic curve (ROC). Results The expression level of VEGF, EGFR and CD40 in pleural effusion of observation group was significantly higher than those of control group, the difference was statistically significant (P < 0.05). ROC curve analysis showed that the area under the curve of VEGF in the diagnosis of malignant pleural effusion was 0.840, with 893.50 pg/mL as the cut-off point, the sensitivity was 70.00%, and the specificity was 91.70%. The area under the curve of EGFR in the diagnosis of malignant pleural effusion was 0.839, and the cut-off point was 29.50 mg/L. The sensitivity was 80.00% and specificity was 88.30%. The area under the curve of CD40 in the diagnosis of malignant pleural effusion was 0.911, and the cut-off point was 41.50 pg/mL. The sensitivity was 85.00% and the specificity was 93.30%. The diagnostic efficiency of CD40 in benign and malignant pleural effusion was better than that of VEGF and EGFR. Conclusion The expression of VEGF, EGFR and CD40 in malignant pleural effusion is significantly increased, the three methods have certain diagnostic value in differentiating benign and malignant pleural effusion. However, the sensitivity and specificity of CD40 in the diagnosis of malignant pleural effusion is higher than that of VEGF and EGFR.

145 Effects of primary thrombocytosis on platelet function and thrombus formation and the clinical effect of Sodium Ozagrel in preventing thrombus

Objective To study the changes of platelet function and its effect on thrombosis in patients with essential thrombocytosis (ET), and to analyze the effect of Sodium Ozagrel on thrombosis prevention. Methods Thirty-two patients with ET admitted to Shaanxi Provincial People′s Hospital ("our hospital" for short) from September 2014 to September 2017 were enrolled as the study object, and they were divided into two groups by random number table, 16 cases in each group. Patients in routine group were treated with routine therapy, while patients in study group were treated with Ozagrel Sodium on the basis of routine treatment. Another 20 healthy volunteers who received physical examination in our hospital during the same period were selected as control group. The levels of P-selectin (CD62P), anti-fibrinogen receptor monoclonal antibody (PAC-1), thromboxane B2 (TXB2), 6-keto-prostaglandin F1α (6-keto-PGF1α) and thrombosis rate of three groups were observed. Results Before treatment, there were no differences in levels of CD62P, PAC-1, TXB2 and 6-keto-PGF1α between the routine group and study group (P > 0.05), but they were significantly higher than those of the control group, the differences were statistically significant (P < 0.05). After treatment, the levels of CD62P, PAC-1, TXB2 and 6-keto-PGF1α in the study group were significantly lower than those in the routine group, but still higher than those in the control group, the differences were statistically significant (P < 0.05). The thrombosis rate in the study group was only 6.25%, which was significantly lower than 18.75% in the control group, the difference was statistically significant (P < 0.05). Conclusion The indexes of platelet function and activity in ET patients are significantly different from those in healthy person, and the incidence of thrombosis is significantly increased. Sodium Ozagrel can significantly improve the platelet status of ET patients, promote the therapeutic effect and reduce the incidence of thrombosis, which is worthy of clinical application and promotion.

148 Application of serum procalcitonin and high-sensitivity C-reactive protein in early diagnosis of acute exacerbation of chronic obstructive pulmonary disease

Objective To investigate the diagnostic value of serum procalcitonin (PCT) and hypersensitive C-reactive protein (hs-CRP) in acute exacerbation of chronic obstructive pulmonary disease (AECOPD). Methods From January 2016 to September 2018, according to whether the patient had fever, cough, yellow phlegm and acute attack and the increase of white blood cells and neutrophils (common manifestations of bacterial infections), AECOPD patients from Hepingli Hospital of Dongcheng District in Beijing City were divided into infection group (52 cases) and non-infection group (48 cases). Another 100 chronic obstructive pulmonary disease (COPD) patients were collected as control group. The changes of serum PCT and hs-CRP levels before and after AECOPD treatment were compared. Results The levels of serum PCT and hs-CRP in AECOPD group were significantly higher than those in COPD group, and the difference was statistically significant (P < 0.05). Before treatment, the levels of serum PCT and hs-CRP in infection group were significantly higher than those in non-infection group, and the difference was statistically significant (P < 0.05). After treatment, the levels of serum PCT and hs-CRP in infection group and non-infection group were significantly lower than those before treatment, and the level of indicators in infection group was significantly higher than that in non-infection group, the difference was statistically significant (P < 0.05). Conclusion Serum PCT and hs-CRP are valuable for the early diagnosis of AECOPD and whether the patient suffer infection, and can be used to evaluate the efficacy of AECOPD.

152 Comparison of inflammatory response in the perioperative period of acute appendicitis with different pathological types

Objective To investigate the changes of perioperative inflammatory response in patients with different pathological types of acute appendicitis and its clinical significance for preoperative diagnosis. Methods The perioperative clinical data of 126 patients with acute appendicitis in the Aerospace Center Hospital from June 2015 to January 2017 was analyzed retrospectively. The inflammatory response indicators of patients with acute simple appendicitis (ASA, group A), acute purulent appendicitis (APA, group B) and acute gangrenous appendicitis (AGA, group C) were compared and analyzed before operation and 1, 3, 5 d after operation. The inflammatory response indicators included the highest body temperature (HBT), peripheral white blood count (WBC), neutrophil percentage (NE%), peripheral blood neutrophil / lymphatic ratio (NLR), high sensitivity C reactive protein (hs-CRP) and procalcitonin (PCT). Results The indicators of postoperative inflammatory reaction of the three groups decreased gradually with the passage of time. The inflammatory indicators at 1, 3, 5 d after operation were significantly lower than those before operation, the inflammatory indicators at 3, 5 d after operation were lower than those at 1 d after operation, which at 5 d after operation were lower than those at 3 d after operation, the differences were statistically significant (P < 0.05). Before operation, HBT, WBC, hs-CRP, PCT of group C were higher than those of group A and group B (P < 0.05). The inflammatory indicators had no statistically significant differences between group A and group B at 1, 3, 5 d after operation (P > 0.05). The WBC, NE%, hs-CRP, PCT and NLR in group C were significantly higher than those in group A and group B at 1, 5 d after operation, with statistically significant differences (P < 0.05); the WBC, NLR, hs-CRP and PCT in group C were significantly higher than those in group A and group B at 3 d after operation, with statistically significant differences (P < 0.05). Conclusion The indicators of inflammatory response play a very important role in the diagnosis of acute appendicitis and it is helpful for diagnosing AGA before operation, so as to determine the conditions of acute appendicitis and deal with the condition promptly. The inflammatory response of patients with AGA has a relatively slow recovery and a higher incidence of infection-related complications.

156 Clinical effect of recombinant human interferon α-1b atomization therapy in herpes angina in infants

Objective To explore the effect of recombinant human interferon α-1b atomization therapy on herpes angina in infants. Methods From January 2017 to January 2018, 300 infants with herpetic angina in the Jiaozuo Hospital of 988th Hospital in Chinese People′s Liberation Army Joint Logistics Support Force were divided into observation group and control group according to random number table. Children in the control group were given the Ribavirin atomization therapy, and the observation group was given recombinant human interferon α-1b atomization therapy. The effect and drug safety of two groups were analyzed. Results After treatment, the fever abatement time, salivation relief time, herpes subsidence time and hospitalization time of the observation group were significantly shorter than those of the control group (P < 0.01). After treatment, the total effective rate of the observation group was significantly higher than that of the control group (P < 0.05). No obvious adverse reactions were found in the two groups. Conclusion The recombinant human interferon α-1b is effective and safe in the treatment of infantile herpetic angina, and there is significant clinical application value with it.

159 Correlation between drug resistance of Klebsiella pneumoniae and antibiotic use

Objective To explore the effect of antibiotics on drug resistance of Klebsiella pneumoniae and to provide theoretical basis for rational use of antibiotics in clinic. Methods The annual dosage of antibiotics and the trend of drug resistance of Klebsiella pneumoniae in patients with community acquired respiratory tract infection from 2015 to 2017 in Xiaolan Hospital Affiliated to Southern Medical University were retrospectively analyzed. Pearson was used to analyze the correlation between drug resistance of common anti-Gram-negative bacteria and Klebsiella pneumoniae. Results From 2015 to 2017, the detection rate of Klebsiella pneumoniae producing extended-spectrum β-lactamase [ESBLs (+)] showed an upward trend, with no significant difference (P > 0.05). The resistance rates of Klebsiella pneumoniae strains ESBL (+) to Ceftazidime and fourth-generation cephalosporins were less than 30%, while the resistance rates to the first generation cephalosporins and Ceftriaxone were higher than 90%, and the resistance rates to aminoglycosides and Compound sinomenin were higher than 40%, and the resistance rates to carbapenems were lower than 1%. The second and third generation of cephalosporins, β-lactamase inhibitors and quinolones were the most frequently used drugs. The resistance rate of Klebsiella pneumoniae to Piperacillin/Tazobactam and Ceftazidime was positively correlated with the frequency of antibiotics (P < 0.05). Conclusion There is a certain correlation between the frequency of antibiotics used and the drug resistance rate of Klebsiella pneumoniae. We should continue to strengthen the management of clinical application of antibiotics, optimize the selection of antibiotics and control the use of antibiotics.

164 Characteristics and treatment of Adefovir Dipivoxil combined with Lamivudine in the treatment of renal damage caused by chronic hepatitis B and cirrhosis

Objective To observe the clinical characteristics of Adfovir Dipivoxil (ADV) combined with Lamivudine (LAM) in the treatment of chronic hepatitis B (CHB) and liver cirrhosis (LC) patients with renal hypophosphatemia and other adverse reactions, and to explore the efficacy of Entecavir (ETV) treatment. Methods From January 2012 to March 2018, patients with CHB and LC (Child-Pugh A grade) who were treated with LAM and ADV combined therapy (add-on scheme) after drug resistance in Zibo Central Hospital of Shandong Province were selected for routine examination. If adverse renal reactions such as hypophosphatemia were found, ADV and LAM were discontinued in time. Patients with mild or moderate CHB were selected as group A and replaced with ETV 0.5. Mg/d. Patients with severe CH B and LC were treated with ETV 1.0 mg/d as group B. Wilcoxon sign rank sum test of paired data was used for statistical analysis. Results Renal damage such as hypophosphatemia occurred after 63 (49, 71) months of add-on regimen treatment, and the biochemical indexes and symptoms gradually improved after ETV treatment. There were significant differences in blood scales and eGFR between the two groups after 6 months of drug change (P < 0.05). There was no significant difference in blood phosphorus and eGFR between the two groups before and after drug change (P > 0.05). Blood HBV DNA was still below the detection limit in all patients after dressing change. Conclusion Patients with CHB and HBV-related liver cirrhosis may develop hypophosphatemia and other adverse reactions after a long-term add-on therapy. Switching to ETV can reduce adverse effects and maintain the efficacy in inhibiting HBV replication.

168 Effects of rational emotive therapy combined with music intervention on postoperative fear, self-esteem and sleep quality in patients with abdominal aortic aneurysm

Objective To explore and analyze the effects of rational emotive therapy combined with music intervention on postoperative fear mood, self-esteem status and sleep quality of patients with abdominal aortic aneurysm. Methods Eighty cases of abdominal aortic aneurysm surgery patients admitted to the First Affiliated Hospital of Harbin Medical University (“our hospital” for short) from July 2014 to April 2018 were selected, all the patients were divided into control group and observation group by random number table method, with 40 cases in each group. The control group was given the routine medical intervention after operation, and the observation group was given the rational emotional therapy combined with music intervention on the basic of the control group. The postoperative fear, self-esteem, sleep quality changes and patients′ satisfaction were compared between the two groups. Results Compared with before intervention, the scores of fear visual analogue scale (FAVS) in the two groups were decreased after intervention, and the FAVS scores in the observation group were lower than those of the control group after intervention, the differences were statistically significant (P < 0.05). Compared with before intervention, the scores of behavioral self-esteem, social self-esteem and external self-esteem in the observation group were all increased after intervention, and the scores of indicators above in the observation group were higher than those of control group, the differences were statistically significant (P < 0.05). After intervention, the scores of sleep time, sleep disorder, time of falling asleep, sleep efficiency, daytime function and sleeping drugs in the two groups were lower than those before intervention, and the scores of the observation group were lower than those of the control group after intervention, the differences were statistically significant (P < 0.05). The degree of satisfaction in the observation group was higher than that of the control group, the difference was statistically significant (P < 0.05). Conclusion Rational emotive therapy combined with music intervention can improve the postoperative fear and self-esteem of patients with abdominal aortic aneurysm, and enhance the sleep quality and satisfaction of patients.

172 Effect of whole-process systemic nursing on clinical efficacy, psychological state and self-management efficacy for patients after laparoscopic radical nephrectomy

Objective To observe the effect of whole-process systemic nursing on clinical efficacy, psychological state and self-management efficacy of patients after laparoscopic radical nephrectomy. Methods Total 98 patients underwent laparoscopic radical nephrectomy in Affiliated Hospital of Inner Mongolia Medical University from July 2016 to July 2018 were divided into two groups according to the different nursing options, with 49 cases in each group, by retrospective analysis. The control group received routine nursing, while the observation group received whole-process systemic nursing. The recoveries, complications, psychological status and self-management efficacy of patients in two groups were compared. Results After nursing, the exhaust time, indwelling time of urinary catheter, getting out of bed activity time and hospitalization time of observation group were shorter than those of control group, and the sleep time of observation group was longer than that of control group, with highly statistically significant difference (P < 0.01). The incidence of postoperative complications in observation group was lower than that in control group, with highly statistically significant difference(P < 0.01). The scores of self-rating anxiety scale and self-rating depression scale in two groups after nursing were lower than those before nursing, and those in observation group were lower than those in control group, with highly statistically significant difference (P < 0.01). The total scores of positive attitude, self-decision-making, stress relief and self-management efficacy of observation group after nursing were higher than those of control group, and the difference was highly statistically significant (P < 0.01). Conclusion For patients undergoing laparoscopic radical nephrectomy, the application of whole-process systemic nursing can significantly accelerate their postoperative rehabilitation and reduce the risk of postoperative complications, alleviate their negative psychological emotions and enhance their self-management efficacy.

177 Role of medical humanistic spirit in medical service and medical education under the concept of sustainable development

With the progress of science and technology and the development of the times, medical technology has made great progress, and greatly improved the level of human health care. Modern medicine has developed into a comprehensive system with rich contents, which covers many aspects, such as exploring the mystery of life, diagnosing and treating diseases, improving health, alleviating pain and so on. However, the constant exposure of doctor-patient conflicts in the media makes people re-examine and question the purpose and value of medicine while enjoying modern medical and health care services, and look forward to the return and revival of the traditional medical humanistic care. By consulting relevant documents and materials, this paper explains the connotation of medical humanistic spirit with the concept of sustainable development, analyzes the great significance of medical humanistic spirit and the value of the times, and finally discusses how to carry forward medical humanistic spirit in an all-round way from college training to system reform. Advising medical college education and hospitals at all levels achieving true sustainable development and promoing the development of medical humanistic spirit in China.

181 Clinical application effect of PBL combined with LBL teaching mode in clinical practice teaching of thyroid surgery

Objective To explore the clinical application effect of problem-based learning (PBL) combined with lecture-based learning (LBL) in clinical practice teaching of thyroid surgery. Methods One hundred clinical undergraduates who came to West China Hospital of Sichuan University for thyroid surgery practice in March 2018 were selected as the subjects of study. They were divided into control group (n = 50, LBL teaching method) and experimental group (n = 50, PBL combined with LBL teaching mode) according to random number table method, the internship lasted for 3 months. At the end of the study, the students′ theoretical achievements and operational skills were assessed to evaluate the teaching effect. The subjective evaluation of students′ satisfaction with teaching and experts′ evaluation of teaching effect was compared. Results The scores of professional theory knowledge, clinical operation skills, medical record writing and medical record analysis and discussion of undergraduates in experimental group were higher than those in control group, and the difference was statistically significant (P < 0.05). The experimental group undergraduates′ interest in learning, clinical thinking ability, mastery of knowledge points, learning efficiency, curriculum arrangement, self-study ability and innovation scores were higher than those of control group, with statistical significance (P < 0.05). Experts′ evaluation results on teacher′s teaching plan design, medical record writing and analysis, problem discussion, clinical practice and teaching effect in experimental group were better than those in control group, and the difference was statistically significant (P < 0.05). Conclusion PBL combined with LBL teaching mode has a good clinical application effect in clinical practice teaching of thyroid surgery. It can improve students′ examination results, cultivate students′ ability to discuss and solve problems collectively and students′ comprehensive quality. It has obtained higher satisfaction from experts and students. It is worthy of further promotion in clinical practice teaching.

185 Significance of normalization of critical value of inspection items in Physical Examination Center

The primary responsibility of Physical Examination Center is disease prevention. Some examiner may have the urgent situation during physical examination. The physical examination center should establish the critical value of inspection items and reporting mechanism. When doctors get the report, they should rescue patient immediately. This system is propitious to the prevention of critical disease. Physical Examination Center of Dongfang Hospital of Beijing University of Chinese Medicine relies on the Laboratory and Inspection Department of top three hospital. Technical strength is strong. Over the years, we have summarized a series of critical value indicators for health examination, including high-risk values or reports of laboratory, imaging, electrocardiogram and other inspection items, which can discover major diseases in time, highlight the disease prevention advantages of physical examination, and also reflect the professional ability and high-quality service of Physical Examination Center.
