77 The role and significance of the Youth Forum in the standardized training of general practitioners
ZHANG Mingxin LU Ning CUI Manli WANG Jia DU Zhaozhao ZHU Lin▲
The generalized resident standardization training plays an important role in the training and re-education of general practitioners, but it also has certain deficiencies. In the long-term practice of general practitioners, the Department of Gastroenterology in the First Affiliated Hospital of Xi′an Medical University (“our hospital” for short) has gradually explores a characteristic training mode in the long-term standardized training practice of general practice residents, among which the most significant one is “youth forum”. The specific practice method is to take 1 month as a cycle and the time is every Monday. In the first week, a general practitioner and a young doctor in the Department of Gastroenterology report and discuss the current hot spots and difficulties in diagnosis and treatment. In the second week, young doctors from other departments in our hospital are invited to give lectures. The multidisciplinary communication is more conducive to the cultivation of clinical thinking of general practitioners. In the third week, young doctors from key departments of other hospitals in Shaanxi Province are invited to the department of gastroenterology for exchange and study, so that the rotation of specialties and general practice learning can be interchanged, so that the general practitioners can see their own shortcomings and receive better teaching resources, so as to improve their interest in learning and communication. In the fourth week, domestic experts and professors are invited to give academic lectures, so that general practitioners can broaden their vision, understand and learn the latest medical knowledge and improve medical literacy during the training period. The review is conducted every 3 months and is reported by the general practitioner. Through the development of the youth forum, not only has the general practitioners′ medical skills been improved, but the most important thing is to enhance their sense of belonging in our hospital, facilitate centralized management, stimulate their independent learning, and comprehensively improve the quality of teaching.

132 Differentiation and analysis of the differences between acupuncture and internal medicine of traditional Chinese medicine
JIN Yiting1 XU Xinyin1,2 WANG Xuefeng1 YANG Tingting1
Acupuncture and internal medicine of traditional Chinese medicine are both important parts of traditional Chinese medicine. Based on the theory of meridians and collaterals, acupuncture and moxibustion is characterized by differentiation and treatment of meridians. It can dredge meridians and regulate qi and blood by selecting appropriate shu points, operating measures and techniques, stimulating amount, etc. The theory of internal medicine of traditional Chinese medicine includes five elements of yin and yang, five movements and six qi, qi, blood and body fluid, etc. It has the characteristics of syndrome differentiation and treatment. Based on traditional Chinese medicine, it can play the role of strengthening the body and removing pathogenic factors and harmonizing yin and yang by applying appropriate prescription, processing and decoction. At present, the system of acupuncture and moxibustion is gradually converged in the internal medicine of traditional Chinese medicine, which mistakenly confuses syndrome differentiation with syndrome differentiation, and rigidly selects the prescription of acupuncture and moxibustion. Making clear the difference between acupuncture and internal medicine of traditional Chinese medicine, it is convenient to give full play to the thinking of acupuncture diagnosis and treatment, show the characteristics and advantages of acupuncture, and better serve the clinical diagnosis and treatment.

YUAN Hongmei1 PU Jingsong2 YUE Wensheng1 CHEN Tianwu3 ZHOU Haiying3 GU Peng1 LEI Huilan1
Objective To evaluate the clinical value of ultrasound (US) and magnetic resonance imaging (MRI) in rotator cuff injury. Methods From January 2016 to February 2018, 38 cases of rotator cuff injury who confirmed by arthroscopy were collected in Affiliated Hospital of North Sichuan Medical College. US and MRI were performed before operation. Shoulder arthroscopy was the gold standard. McNemer test was performed on US and MRI results of rotator cuff injury and Kappa test were used to evaluate the consistency of US and MRI in the diagnosis of rotator cuff injury and common complications. Results Arthroscopic examination showed that there were 26 full-thickness tears and 23 partial-thickness tears. The sensitivity, specificity, and accuracy of US in diagnosis of full-thickness tear were 92.31%, 100.00% and 98.24% respectively, while MRI was 96.15%, 100.00%, and 99.12% respectively. The sensitivity, specificity, and accuracy in US of the diagnosis of partial-thickness tear were 78.26%, 97.80% and 93.85% respectively, while 86.96%, 96.70% and 94.74% respectively in MRI. There was no statistically significant difference between the two methods in the diagnosis of rotator full-thickness tear and partial-thickness tear (P > 0.05). There was a good consistency between US and MRI in the diagnosis of rotator cuff full-thickness tears and partial-thickness tears (κ= 0.922, 0.811), but the consistency of US and MRI in the diagnosis of complications after rotator cuff tears was poor (κ = -0.068). Conclusion US can be used as the first screening diagnosis of clinical rotator cuff injury and the first choice for routine follow-up. When the diagnosis of rotator cuff injury is not clear or more detailed complications information is needed before surgery, MRI can be further performed.
