4 Effect of Danshen Injection on expression of BMP-2 and TGF-β1 during healing of rat mandibular fracture

Objective To investigate the effect of Danshen Injection on mandibular fracture of healing in rat and observe the changes of bone morphogenetic protein-2(BMP-2), transforming growth factor-β1(TGF-β1). Methods Forty-eight SD rats were operated for incomplete fracture of hibateral mandible penetrated from buccal side to lingual side. The rats were divided into experimental group and control group according to random number table, with 24 rats in each group. The experiment group was administered with Danshen Injection by intraperitoneal injection, and the control group was administered with physiologic saline by intraperitoneal injection. The right healing of fracture were used to detect fracture healing by X-ray and to measure the average bone density of the callus area, the percent trabecular and trabecular width were analyzed by bone histomorphometry and the serum levels of BMP-2, TGF-β1 was determined by ELISA at 1, 2, 3, 4 weeks after the operation. The right healing of fracture were comminuted, and the expression of BMP-2 and TGF-β1 were analyzed by RT-qPCR. Results The X-ray results showed that the fracture healing in the control group was obviously delayed in the experimental group. After 2 weeks, compared with the control group, the callus bone mineral density (BMD) in the experimental group was increased (P < 0.05 or P < 0.01). The trabecular area ratio and trabecular width in the callus area were markedly increased (P < 0.05 or P < 0.01), and the difference was even more increased along with the increasing time. ELISA results showed the serum levels of BMP-2 and TGF-β1 in the experimental group were higher than those in the control group (P < 0.05). RT-qPCR results showed the expression of BMP-2 and TGF-β1 in the experimental group were higher than those in the control group from the 2nd week after operation (P < 0.01). Conclusion Danshen Injection can promote the healing of mandibular fracture, which may be related to the transcription and protein expression of BMP-2 and TGF-β1 gene during the process of fracture healing.

9 Effects of bone marrow stem cells transplanted via cisterna magna on injured spinal cord white matter

Objective To investigate the effect of bone marrow stem cells (BMSCs) transplanted via cisterna magna on the rehabilitation of damaged spinal cord white matter. Methods The spinal cord injury (SCI) model was carried out by a modified Allen method, assigned into the control group and BMSCs group randomly. The BMSCs group and control group were given 10 μL BMSCs suspension and 10 μL physiological saline via cistern magna respectively. BBB scale was utilized to estimate the neurological recovery on 1, 7, 14, 21, 28 d postoperatively after SCI. The white matter changes in the injured spinal cord were tested using 4.7 T MRI scanner. HE staining was applied to access the rehabilitation condition. Flourescence microscope was performed to identify the signal level of Hoechst 33342 marked BMSCs in the injured spinal cord. Results The BBB scores in the BMSCs group were significantly higher than that of the control group  on 7, 14, 21, 28 d after SCI (P < 0.05). There was no statistically significant difference between the two groups regard to the longitudinal diffusion gradients of the damaged spinal cord white matter (P > 0.05). The average size of the lesion center in the BMSCs group was significantly smaller than that of the control group (P < 0.05). HE staining revealed that the recovery of the BMSCs group was better than the control group at 14, 28 d after SCI. The counts of Hoechst 33342 labelled cells were less in the control group than that in the BMSCs group at 7 d. Subsequently, they continued to decrease from 7 d to 4 weeks later, with highly statistically significant difference (P < 0.01). Conclusion Bone marrow mesenchymal stem cells transplanted via cisterna magna has a positive role in promoting the rehabilitation of damaged spinal cord white matter, which is a novel way of treating SCI.

14 Pharmacological activity of Qufeng Jiejing Decoction in treating cough variant asthma

Objective To preliminarily investigate the efficacy of Qufeng Jiejing Decoction on airway inflammation reaction in guinea pigs with cough variant asthma (CVA). Methods Forty SPF female guinea pigs were divided into five groups with eight Digs in each group by random number table, including control group, model group, positive drug group, low-dose Qufeng Jiejing Decoction group and high-dose Qufeng Jiejing Decoction group. The OVA-induced method, by injecting OVA and aluminum hydroxide (ALP) intraperitoneally, was applied to make guinea pig CVA model. On the date of nebulization, the positive drug group, high-dose and low-dose Qufeng Jiejing Decoction groups were respectively treated with prednisone acetate at 3 mg/kg, Qufeng Jiejing Decoction at doses of 2.88 g/kg and 8.22 g/kg by gavage. The control group and model group were treated with normal saline at the same time. The cough response sensitivity (CRS), eosinophil (EOS) percentage in bronchoalveolar lavage fluid (BLAF), as well as levels of serum interleukin -4 (IL-4), IL-12, IL-13, tumor necrosis factor β (TNF- β) and interferon-γ (IFN-γ) of each group were detected. Results Compared with the control group, the CRS, EOS percentage, and the expression levels of IL-4, IL-12, IL-13, TNF-β and INF-γ in serum of the model group were increased significantly (P < 0.05). Compared with the model group, the CRS and EOS percentage were significantly decreased in the positive drug group, high-dose and low-dose Qufeng Jiejing Decoction group, while the levels of serum IL-4, IL-12, IL-13, TNF-β and INF-γ were significantly reduced (P < 0.05). Compared with the low-dose Qufeng Jiejing Decoction group, the CRS, EOS percentage, and the levels of TNF-β and INF-γ in high-dose Qufeng Jiejing Decoction group were reduced significantly (P < 0.05). Conclusion Qufeng Jiejing Decoction can evidently inhibit the CRS and the EOS percentage of CVA guinea pigs, and also reduce the expression of inflammatory cytokines in their serum to some extent. However, the relevant mechanism is worth investigating and needs further exploration.

18 Prediction of therapeutic drugs for diabetic nephropathy based on disease gene expression profiling

Objective To explore the application effects of gene expression profiling in screening the effective therapeutic drugs for diabetic nephropathy. Methods The gene chips related to diabetic nephropathy in GEO databases were searched and the differential gene expression was analyzed by d-chip software. Enrichment analysis of the relevant signaling pathways was carried out by GSEA, Cmap was then used for the screening of candidate drugs of diabetic nephropathy and pharmacodynamic validation was evaluated in animal experiments. Results Through the analysis of differential genes in qualified samples from the GSE31022 chip, a total of 540 genes were obtained, of which, 150 genes were up-regulated and 390 genes were down-regulated. The main enrichment to NF-κB signaling pathway by GSEA was most closely related to diabetic nephropathy (P < 0.05), and screening for Thiamphenicol may have a therapeutic effect on diabetic nephropathy (score=1). The results of animal experiments showed that compared with model group, the mesangial expansion of the diabetic nephropathy mice treated by Thiamphenicol was significantly improved and almost reached the normal level (P < 0.05). In addition, the levels of urine protein and the mRNA levels of CD68, transforming growth factor β, plasminogen activator inhibitor 1 and interleukin 6 in the Thiamphenicol group were significantly lower than those in model group (P < 0.05). Conclusion As an effective means of predicting effective therapeutic drugs for diabetic nephropathy, the gene expression profiling based on diabetic nephropathy provides a new idea for the discovery of therapeutic drugs for the disease.

22 Effect of Metformin on tubulointerstitial fibrosis following renal ischemia-reperfusion injury in rats

Objective To assess the potential effects of Metformin on the development of tubulointerstitial fibrosis after acute renal ischemia-reperfusion. Methods Twenty-four male Sprague Dawley rats were randomly assigned to three groups: Sham group, I/R group and Metformin group (n = 8). Twelve weeks after reperfusion, levels of serum urea nitrogen (BUN) and creatinine (Cr) were assessed; Hematoxylin-eosin (H&E) and Masson′s trichrome staining were used to assess renal fibrosis; the expression spot and protein levels of α-smooth muscle actin (α-SMA) and transforming growth factor-β1 (TGF-β1) were analyzed by immunohistochemistry (IHC) and Western blot. Results There was no statistically significant difference in levels of Cr and BUN among Sham group, I/R group and Metformin group (P > 0.05). Compared with the Sham group, tubulointerstitial fibrosis was more obvious in the I/R group and the Metformin group; however, the level of tubulointerstitial fibrosis in the Metformin group was less than that of the I/R group. Compared with the Sham group, the levels of α-SMA and TGF-β1 were significantly increased in the I/R group and the Metformin group, the expression of α-SMA and TGF-β1 in the Metformin group were significantly decreased compared with those in the I/R group; the differences were statistically significant (P < 0.01). Conclusion Administration of metformin significantly attenuated tubuleinterstitial fibrosis following I/R in rats, potentially via down-regulating expressions of α-SMA and TGF-β1.

27 Study on HPLC-DAD dual-wavelength chromatographic fingerprint of Yinyanjing Lotion

Objective To establish fingerprint methods in Yinyanjing Lotion by high performance liquid chromatography (HPLC). Methods A Hypersil ODS2 column (4.6 mm×250 mm, 5 μm) was used, the flow rate was 1.0 mL/min. The mobile phase was acetonitrile and 0.1% phosphoric acid solution with gradient elution at 25℃. The flow-rate was 1.0 mL/min and the detection wavelength was set at 210 nm and 280 nm. Results The common modes for the HPLC fingerprints were set up by ensuring 33 common peaks in the chromatograms. 5 chemical constituents were analyzed qualitatively. Conclusion Dual-wavelength chromatographic fingerprint can realize the analysis of a formula as a whole. This method is sensitive, accurate and simple, and can be used for the quality control of Yinyanjing Lotion.

31 Simultaneous determination of two triterpenic acids in root of Kadsura coccinea by HPLC

Objective To establish a method for the determination of 12-acetoxycoccinic acid and seco-neokadsuranic acid A in root of Kadsura coccinea. Methods Ultrasonic method was used to extract the herbs. HPLC chromatographic conditions included the Atlantis T3-C18 (4.6 mm×250 mm, 5 μm) column, using acetonitrile-0.1% phosphoric acid solution (80∶20) as the mobile phase at a flow rate of 1.0 mL/min. The detection wavelength was set at 310 nm and the column temperature was maintained at 30℃. Results The calibration curve of 12-acetoxycoccinic acid was in good linearity over the range of 0.0157-0.3140 μg (r = 0.9998). The average recovery was 100.4% with RSD of 2.58% (n = 6). And the calibration curve of seco-neokadsuranic acid A was in good linearity over the range of 0.0848-1.6960 μg (r = 0.9999). The average recovery was 101.1% with RSD of 1.21% (n = 6). Conclusion The method is accurate, simple, reproducible, and can be used to determine the content of 12-acetoxycoccinic acid and seco-neokadsuranic acid A in the root of Kadsura coccinea.

35 Research progress of targeting hepatitis B virus core protein in treatment of chronichepatitis B

Hepatitis B virus (HBV) core protein is an integral part of HBV nucleocapsid, which is involved in viral packaging, viral maturation and viral releasing to the outside of cells. Core protein participates in regulation of cccDNA function and the induction of immune responses. This paper summarizes the research progress of targeting hepatitis B virus core protein in the treatment of chronic hepatitis B in recent years, starting from the structure and function of HBV core protein and the progress in the treatment of HBV. All of these indicate that core protein allosteric modulators (CpAMs) can directly interfere with the activity of HBV core protein as anti-HBV drugs, and the use of HBV core protein as a target antigen to induce HBV-specific immune responses has anti-HBV and immunomodulatory effects.These provide a theoretical reference for the development of treatment for chronic hepatitis B.

This therapy is mainly suitable for patients with early insignificant symptoms. 57 A new mode for the transition of college English teachers from EGP to ESP in medical universities

Against the backdrop of increasing difficulties in college English teaching and of the “Double First-Class” university project in China, it is inevitable to shift from English for general purposes (EGP)-oriented teaching to English for specific purposes (ESP)-oriented teaching. Faced with various pain spots, the college English teachers in medical universities must adapt themselves to the shift. Through an analysis of their role and knowledge and based on an ESP program in a medical university, a new transition mode for college English teachers to be ESP-ready is proposed. Featuring “mutually beneficial cooperation”, the mode is carried out in cooperation between EGP teachers and medical teachers, and benefits both of them. Of high feasibility, the“mutually beneficial cooperation mode” can effectively assist with college English teachers′ transition from EGP to ESP.

"Internet plus teaching" mode brings opportunities and challenges to the teaching of pharmacology. According to the training goals of innovative medical talents and the characteristics of pharmacology teaching, this article focuses on three aspects of teaching staff, teaching mode and teaching content to actively explore ideas and methods to optimize pharmacology teaching. It includes strengthening teachers′ quality from the aspects of professional ability, information awareness and bilingual teaching ability, adopting the "flipped classroom" teaching mode as a reflection of the "student-oriented" teaching philosophy and "flipped classroom" teaching model both online and offline which can be conducive to improving students′ thinking mode. Besides, to optimize the teaching content, it should ensure the smooth development of the new teaching mode and highlight the cultivation of humanistic quality.

64 Investigation of quality of life in lung cancer malignant pleural effusion patients with catheter drainage and analysis of influencing factors

Objective To investigate the quality of life (QOL) of lung cancer malignant pleural effusion patients with catheter drainage and explore the main factors affecting their quality of life. Methods From January  to October in 2017, in Department of Respiratory Medicine, Chinese PLA General Hospital, 64 cases of lung cancer with malignant pleural effusion were selected as observation group, and at the same time, 66 cases of advanced lung cancer without malignant pleural effusion were selected as control group, the quality of life of patients were evaluated by using lung cancer quality of life scale (Fact-l 4.0). Multivariate analysis was used to analyze the factors that may affect the life quality of patients with malignant pleural effusion and drainage of lung cancer. Results The Physical well-being, functional well-being, lung cancer subscale and FACT-L total score in the observation group were significantly lower than those in the control group, the differences were statistically significant (P < 0.05). Pathologic typing, PS score, number of indwelling pleural catheters, education level, source of income, informed status were the factors of quality of life in lung cancer malignant pleural effusion patients with catheter drainage (P < 0.05). PS score, informed status, and number of indwelling pleural catheters were independent factors affecting the quality of life of lung cancer malignant pleural effusion patients with catheter drainage (P < 0.05). Conclusion The quality of life of lung cancer malignant pleural effusion patients with catheter drainage is poor, PS score, informed state and number of indwelling pleural catheters are independent influencing factors.

69 Meta-analysis of Butylphthalide in treatment of delayed encephalopathy after acute carbon monoxide poisoning

Objective To compare the efficacy and safety of Butylphthalide in treatment of delayed encephalopathy after acute carbon monoxide poisoning. Methods Up to January 2018, PubMed, EMBase, the Cochrane Library, Wed of Science CBM, CNKI, VIP, Wanfang Data were searched to collect randomized controlled trials (RCTs) about Butylphthalide in treatment of delayed encephalopathy after acute carbon monoxide poisoning. The RCTs selected, study quality assessed, and data extracted were carried by two researchers independently. According to the evaluation system of Cochrane Handbook, the included RCTs were analyzed by using RevMan 5.3 software. Results Eleven studies were included and contained total 678 patients. Control group: treated with Butylphthalide; test group: routine/other treatment. The total effective rate of test group were higher than that of control group (OR = 5.62, 95%CI: 3.31-9.56, P < 0.01). The MMSE scale scores of test group were better than control group (SMD = 5.18, 95%CI: 3.72-6.64, Z = 6.96, P < 0.01). The BI scale scores of test group were better than control group (SMD = 16.39, 95%CI: 11.78-21.01, P < 0.01). Conclusion Butylphthalide is safe and effective to treat delayed encephalopathy after carbon monoxide poisoning.

74 Value of procalcitonin, high-sensitivity C-reactive protein and D-dimer in diagnosis and prognosis of acute respiratory distress syndrome caused by sepsis

Objective To investigate the value of diagnosis and prognostic evaluation on acute respiratory distress syndrome (ARDS) caused by sepsis and procalcitonin (PCT), high-sensitivity C-reactive protein (hs-CRP), D-dimer. Methods From January 2015 to January 2017, in the First Hospital Affiliated to Hainan Medical College, 82 sepsis-causing ARDS patients were selected. According to the severity of patients, they were divided into severe group (33 cases) and non-severe group (49 cases), according to the ending of patients, they were divided into non-death group (69 cases) and death group (13 cases). At the same period, 100 healthy physical examinations were selected as healthy control group. Serum PCT, hs-CRP, and D-dimer levels in the group were evaluated and compared. Results Serum PCT, hs-CRP, and D-dimer levels in Severe group > non-severe group > healthy control group, the differences were statistically significant (P < 0.05); serum PCT, hs-CRP and D-dimer levels in death group > non-death group > healthy control group, the differences were statistically significant (P < 0.05). Correlation analysis revealed that serum PCT, hs-CRP and D-dimer levels were positively correlated with patient outcome (r = 0.634, 0.518, 0.537, P < 0.05). Conclusion Serum PCT, hs-CRP, and D-dimer levels are associated with sepsis-induced ARDS disease and prognosis. The more severe the patient′s condition and the worse the prognosis, the higher the above-mentioned serum factor levels.

77 Effects of propylene ammonia acyl-glutamine on nutrition and immune function in patients with severe cerebral hemorrhage

Objective To investigate the effects of propylene ammonia acyl-glutamine on nutrition and immune function in patients with severe cerebral hemorrhage. Methods Sixty-eight patients of severe cerebral hemorrhage admitted to Maanshan Shiqiye Hospital from May 2015 to June 2016 were randomly divided into routine group (32 cases) and AlaGln group (36 cases). The routine group received conventional treatment, while the AlaGln group received extra 10 g of AlaGln in 400 mL of compound amino acids, ivgtt, qd, for two weeks. Bowel function, the immune indices of liver function and renal function, infections score before and after treatment between the two groups were compared. Results The improvement of intestinal function score in AlaGln group was significantly better than that in the routine group, and the difference was statistically significant (P < 0.05). The levels of serum albumin, immunoglobulin G (IgG), immunoglobulin A (IgA) and immunoglobulin M (IgM) of patients in the two groups before treatment were compared, there was no statistically significant difference (P > 0.05). The levels of serum albumin and IgG at day 7, IgG, IgA and IgM at day 14 in AlaGln group were compared with those in the routine group at day 7, and the differences were statistically significant (P < 0.05). After two weeks of treatment, the incidence of hospital infection in AlaGln group was significantly lower than that in the routine group, and the difference was statistically significant (P < 0.05). There was no statistically significant difference in liver and kidney function between the two groups before treatment and at day 7 and 14 after treatment (P > 0.05). Conclusion For patients with severe cerebral hemorrhage, propylene ammonia acyl-glutamine supplements can improve bowel function and nutrition; protect the intestinal mucosal barrier and immune function while reduce the nosocomial infection rate and maintain normal liver and renal function.

81 Effect of Valsartan on coronary restenosis after coronary intervention

Objective To investigate the effect of Valsartan on coronary restenosis after coronary intervention. Methods A total of 98 patients who underwent coronary intervention and stent placement at the Second Affiliated Hospital of Guangzhou Medical University from January 2016 to November 2017 were randomly divided into experimental group (n = 49) and control group (n = 49) according to the principle of randomly selected case numbers. The control group received routine treatment. The experimental group were treated with Valsartan Capsules on the basis of routine treatment. At the same time, 30 patients who underwent coronary angiography without stenting were selected as negative control group. The echocardiographic EF values, the occurrence of adverse cardiovascular events and coronary restenosis, and the contents of endothelin-1 (ET-1), high-sensitivity C-reactive protein (hs-CRP), interleukin-6 (IL-6), lipoprotein-related phospholipids and enzyme A2 (LP-PLA2) were analyzed in the three groups of patients. Results In the experimental group, there were 5 cases with coronary restenosis, with an incidence of 10.20%. In the control group, coronary restenosis occurred in 13 cases with an incidence of 26.53%, and the difference between the two groups was statistically significant (P < 0.05). The ET-1, hs-CRP, IL-6, LP-PLA2 and EF values of the three groups 4 weeks after surgery were compared with those before surgery, and the differences were statistically significant (P < 0.05). Four weeks after surgery, the differences of ET-1, hs-CRP, IL-6, LP-PLA2 and EF values in the experimental group, the control group and the negative control group were statistically significant (P < 0.05). Four weeks after surgery, experimental ET - 1, the hs-CRP, IL-6, LP-PLA2 were compared with control group, negative control group, and the differences were statistically significant (P < 0.05), while the control group compared with negative control group, there was no statistically significant difference (P > 0.05). Echocardiographic EF values of the experimental group, the control group and the negative control group were compared with those of the same group 4 weeks after surgery, and the differences were statistically significant (P < 0.05). EF values of the experimental group compared to control group, the difference was statistically significant (P < 0.05). The incidence of adverse cardiovascular events in the experimental group were less than that in the control group, and the difference was statistically significant (P < 0.05). Conclusion After coronary intervention, valsartan can effectively reduce the incidence of coronary restenosis, reduce the content of endothelin and inflammatory cytokines, and significantly improve the occurrence of adverse cardiovascular events and echocardiographic EF values.

85 Study on the effect of glycosylated hemoglobin in the pathogenesis of atherosclerosis

Objective To explore the significance of glycosylated hemoglobin (HbA1c) in the occurrence and development of atherosclerosis. Methods Totally 120 patients with coronary heart disease, including 40 patients with acute myocardial infarction (AMI group), 40 patients with unstable angina (UAP group) and 40 patients with stable angina (SAP group), were enrolled in Shanghai Ninth People′s Hospital, Shanghai Jiaotong University School of Medicine from January 2015 to December 2016, and compared with those of 40 healthy subjects in the control group, grouped by the results of selective coronary angiography. Color Doppler ultrasound diasonograph was used to detect carotid intima media thickness (IMT). Digital subtraction angiography was used to perform the selective coronary angiography, and Gensini coronary disease score method was used to evaluate the severity of the coronary artery lesion. Enzymic method was used to detect the level of glycosylated hemoglobin and blood lipid. Results The levels of HbA1c and IMT in AMI group, UAP group and SAP group were significantly higher than those in control group (P < 0.01), and the levels of HbA1c in AMI group and UAP group were significantly higher than those in the SAP group (P < 0.01). IMT in AMI group and UAP group were significantly higher than those in the SAP group (P < 0.05). Both the levels of HbA1c and IMT had no significant difference between AMI group and UAP group (P > 0.05). The correlation between the level of HbA1c and the degree of coronary artery stenosis was positive in patients with coronary heart disease (r = 0.516, P < 0.05). Conclusion The level of HbAlc is independently associated with the severity of the coronary stenosis, and further accelerates the development of atherosclerosis. HbA1c plays an important role in the occurrence and development of coronary atherosclerosis. By Monitoring the level change of HbA1c, there will be important significance to the condition judgment and prognosis evaluation in patients with coronary heart disease.

88 The value of the Helicobacter pylori in saliva in the occurrence and development of gastric cancer

Objective To investigate the correlation between salivary Helicobacter pylori (Hp) and various stages of gastric cancer development. Methods Saliva was collected from 201 patients with pathologically diagnosed superficial gastritis, atrophic gastritis, dysplasia, and gastric cancer in Beijing Luhe Hospital, Capital Medical University from January 2016 to October 2017. The HPS test was used to detect Hp in saliva. The incidence and correlation of Hp infection were analyzed according to age, sex, periodontal status, and different gastric lesions. Results Among all patients included in the trial, the male infection rate (75.5%) was higher than that of female (54.5%) (P < 0.05). The Hp infection rates of middle-aged and elderly patients were 69.9% and 75.4% respectively, which were significantly different from the younger ones (42.9%) (P < 0.05). The positive rates of oral Hp in the atrophic gastritis group, atypical hyperplasia group, and gastric cancer group were 64.5%, 76.0%, and 75.0%, respectively, which were significantly higher than superficial gastritis group (44.4%), and the differences were statistically significant (P < 0.05). The infection rates of gingivitis group and periodontitis group were higher than periodontal health group, and the differences were statistically significant (P < 0.05). Conclusion Oral Hp infection rate is higher in the oral cavity, and related to age, gender and periodontal disease. The oral Hp infection rate is related to the precancerous lesions and gastric cancer. The presence of oral Hp may be an important risk factor for gastric precancerous lesions and gastric cancer.

92 A study of the clinical efficacy and safety of Ticagrelor in  the treatment of acute coronary syndrome  in the basic level hospital

Objective To analyze the clinical efficacy and safety of Ticagrelor in  the treatment of patients with acute cornoary syndrome(ACS) in the basic level hospital. Methods A total of 97 patients with acute coronary syndrome was analyzed retrospectively in Beijing Yanqing Hospital from January 2015 to May 2017. According to dual antiplatelet therapy, they were divided into Ticagrelor group (n = 49, treated with Ticagrelor combined with Aspirin) and Clopidogrel group (n = 48, treated with Clopidogrel combined with Aspirin). Major adverse cardiac events (MACE), the risk of bleeding and adverse drug reactions(ADR) were compared after a follow-up of 6 months. Results Compared with the Clopidogrel group(14.58%), the incidence of MACE of Ticagrelor group (2.04%) was significant decreased and there was significant difference (P < 0.05). But there was no significant difference in the incidence of bleeding event rates between the two groups (P > 0.05). Compared with the clopidogrel group (6.25%), the ADR of ticagrelor group (18.4%) was increased, but there was no statistically significant difference(P > 0.05). Conclusion Ticagrelor combined with Aspirin can decrease the MACE incidence of ACS and do not increase the risk of bleeding. It is safe, effective and worth popularizing in the basic level hospital.

Objective To determine the median effective dosage of 0.447% Methansulfonic Ropivacaine for ultrasound-guided infraclavicular brachial plexus nerve block. Methods From December 2015 to December 2017, 100 cases carried out upper limb surgery in Anesthesia Department of the Fifth People's Hospital of Hengshui City were selected, ASA Ⅰ-Ⅲ level. Subclavian brachial plexus nerve block guided by ultrasound was carried out, in ultrasonic axillary arteriovenous screen real-time imaging, at brachial plexus inside (outside, behind) lateral band, ribs, pleura, chest muscle groups such as size, 0.447% Methansulfonic Ropivacaine was injected after successfull positioning. Dixon sequential method (Up-and-Down) experiment, with 0.447% Methansulfonic Ropivacaine, and 20 mL solution for the initial capacity, if the block effect was complete, reduce 2 mL to the next patient; if the block effect was incomplete, increase 2 mL to the next patient, and so on. Dixon sequential formula was used to calculate the median effective dosage of 0.447% Methansulfonic Ropivacaine and 95% confidence interval (95%CI) in subclavian brachial plexus nerve block guided by ultrasound. Results Upper limb in patients with subclavian brachial plexus nerve block guided by ultrasound, the median effective dosage of 0.447% Methansulfonic Ropivacaine and 95%CI of 17.5 (17.0-18.0) mL. Conclusion 0.447% Methansulfonic Ropivacaine, for patients with upper limb subclavian brachial plexus nerve block guided by ultrasound, median effective dosage is 17.5 mL.

99 Analgesic and sedative effects of Fentanyl Citrate combined with Parecoxib Sodium anesthesia in procedure of intervertebral foraminoscopy

Objective To probe into the influence of Fentanyl Citrate combined with Parecoxib Sodium anesthesia for percutaneous transforaminal endoscopic discectomy (PTED) in patients with restlessness, pain and sedative effects. Methods A total of 78 patients treated with percutaneous transforaminal endoscopic surgery in Department of Pain of Dongguan Fifth People′s Hospital from January to December in 2016 were selected as study objects, and they were divided into two groups by random method, 39 patients of the control group were taken Fentanyl Citrate 50 μg diluted with normal saline to 5 mL in preoperative intravenous infusion of about 20 min, while  39 patients of the observation group were taken preoperative intramuscular injection of Parecoxib Sodium 40 mg, and were taken the Fentanyl Citrate (50 g) diluted to 5 mL spinal injection anesthesia when the puncture needle reaches the fiber ring during the operation, and the changes of blood pressure and heart rate (HR) index of patients of two groups were recorded before anesthesia (T0), guided needle puncture (T1), annulus fibrosus fenestration (T2), nucleus pulposus removal (T3) and radiofrequency ablation (T4). And the degree of pain and the state of sedation during each period during the operation of patients in the two groups were evaluated by the Ramsay sedation rating scale and visual analogue scale (VAS). Results There was no significant difference in the blood pressure and HR before anesthesia between the two groups (P > 0.05). At T1-T4, the blood pressure and HR in the observation group were fluctuated with more than T0, but closed to the normal value, the blood pressure and HR in the control group significantly increased, and the difference between the two groups was statistically significant (P < 0.05). In each period except T0, VAS of patients in the observation group was lower, and Ramsay was higher than that of the control group, and the differences were statistically significant (P < 0.05). Conclusion The Fentanyl Citrate combined with Parecoxib Sodium anesthesia in application of percutaneous transforaminal endoscopic discectomy which can reduce the blood pressure and HR and other indicators, and the postoperative analgesia and sedation effect are significant, thus it is worth promoting and using.

103 The relationship between menstrual cycle and recovery quality in breast cancer patients undergoing radical mastectomy

Objective To observe the influence of menstrual cycle on recovery quality in breast cancer patients undergoing radical mastectomy. Methods The 168 patients with breast cancer undergoing radical mastectomy from June 2015 to June 2016 in Beijing Friendship Hospital, Capital Medical University were selected, and all cases were divided into follicular phase group (n = 55), ovulatory phase group (n = 53) and luteal phase group (n = 60) according to the different phases of menstrual cycle at the time of surgeries, and the recovery quality were observed and compared among three groups, including time of recovery, OAA/S score, time of stay in PACU and the incidence of postoperative anesthetic. Results The time of recovery and stay in PACU of the luteal phase group were shorter, and the modified observer′s assessment of alertness/sedation scale (OAA/S) score on 5, 15 and 30 min were higher in follicular phase group than those in the ovulatory phase group and luteal phase group, the differences were statistically significant (P < 0.05 or P < 0.01). The time of recovery and stay in PACU were lower, OAA/S score on 5 min and 15 min were higher in ovulatory phase group than those in the follicular phase group, the differences were statistically significant (P < 0.01). The incidence of postoperative nausea and vomiting was statistically significant among three groups (P < 0.01), and the incidence of postoperative nausea and vomiting in luteal phase group were lower than those in follicular phase group and ovulatory phase group, the differences were statistically significant (P < 0.01). Conclusion The patients undergoing breast cancer surgery during the luteal phase are better than those who are follicular phase and ovulatory phase, and the incidence of postoperative nausea and vomiting are also statistically reduced.

107 The effect of thunder-fire moxibustion in the treatment of chronic fatigue syndrome and its influence on T lymphocyte subsets in peripheral blood

Objective To evaluate the efficacy and safety of thunder-fire moxibustion in the treatment of chronic fatigue syndrome (CFS) on Shenque acupoint (CV8) comparing with common moxa-stick moxibustion therapy, and to investigate its influence on CD3+, CD4+, CD8+ T lymphocyte subsets in peripheral blood. Methods From October 2016 to June 2017, 85 cases of patients diagnosed as CFS in Center of Traditional Remedies, the Second Clinical Medical College, Guangzhou University of Chinese Medicine were selected as research objects and divided into the experimental group and the control group. The experimental group was treated with thunder-fire moxibustoin, while the control group was treated with common moxa-stick moxibustion. Both groups chose CV8, once every other day, three times were as one course, for 4 courses. Finally, 38 cases in the experimental group and 37 cases in the control group finished the study. The changes of CFS clinical syndrome scale and the levels of T lymphocyte subsets (CD3+, CD4+, CD8+) in peripheral blood before and after treatment were observed. Results Both groups could decrease the scores of clinical syndrome scale, and the experimental group was better than that of the control group, the difference was statistically significant (P < 0.05). Both groups could increase the number of CD3+, and the effective rate in the experimental group and control group was 65.79%, 43.24% respectively, the difference was statistically significant (P < 0.05). Both groups could two-way regulate the ratio of CD4+/CD8+, and the effective rate in the experimental group and control group was 50.00%, 21.62% respectively, the difference was statistically significant (P < 0.05). There were no obvious adverse events in both groups. Conclusion Both thunder-fire moxibustion and common moxa-stick moxibustion can effectively improve the clinical symptoms of CFS, increase the number of CD3+ and restore the balance of CD4+/CD8+ ratio, but the effect of thunder-fire moxibustion is better. Both are safe, easy to operate, and valuable for clinical application.

This article focuses on the interaction of "deficiency", "blood stasis" and "toxin" in the process of occurrence and development of the disease, and exploration of the corresponding effective treatment and prescriptions. It is significant to recognize and stress the interaction of them in pathogenicity so as to enrich TCM etiology and improve TCM clinical efficacy in the treatment of chronic obstructive pulmonary disease combined with sleep apnea hypopnea syndrome.

Ke-nang theory is developed and formed on the basis of the phlegm and blood stasis. Chronic phlegm and static blood are cemented as toxic, stagnant and stale, thus formed ke-nang, which has its own characteristics of disease position, disease nature, disease status, etc. The treatment period of malignant tumor is long and difficult. Most of them are long-term and difficult to be cured. According to the ke-nang theory, treating malignant tumor from "phlegm" and "stasis" is helpful to deeply understand the pathogenesis of malignant tumor, and it provides theoretical basis for developing new treatment ideas and methods. In this paper, the formation and development, clinical features and symptoms, therapeutic methods and commonly used drugs of ke-nang are described combined with typical cases.

119 Clinical observation of Guipi Decoction in the treatment of arrhythmia

Objective To observe the clinical effect of modified Guipi Decoction in the treatment of arrhythmia. Methods Sixty patients with arrhythmia in Outpatient Department of TCM, Beijing Xiaotangshan Hospital from October 2014 to June 2017 were collected, and 60 patients were divided into treatment group and control group according to different treatment methods, with 30 cases in each group. The treatment group was treated with basic western medicine + Guipi Decoction, the control group was treated with basic western medicine, the course was 4 weeks. The clinical efficacy, the 24-hour dynamic electrocardiogram efficacy, the changes of TCM symptom scores before and after treatment and the TCM symptom score efficacy between the two groups were compared, and the drug safety of the two groups was observed. Results In the clinical effect, the total effective rate was 93.3% in the treatment group and 73.3% in the control group, the difference of the total effective rate between the two groups was statistically significant (P < 0.05). In the 24-hour dynamic electrocardiogram efficacy, the total effective rate was 90.0% in the treatment group and 66.7% in the control group, the difference of the total effective rate between the two groups was statistically significant (P < 0.05). In the TCM symptom scores, the TCM symptom scores in the treatment group after treatment were lower than those before treatment (P < 0.05), which were lower than those of control group, the difference between the two groups was statistically significant (P < 0.05). In the TCM symptom scores efficacy, the total effective rate was 96.7% in the treatment group and 76.7% in the control group, the difference of the total effective rate between the two groups was statistically significant (P < 0.05). There were no significant differences in liver function, renal function, blood routine, urine routine, regular routine before and after treatment in both groups. Conclusion Guipi Decoction can effectively relieve myocardial ischemia in patients with arrhythmia and improve clinical symptoms.

123 The value of color Doppler ultrasonography in the evaluation of diabetic lower extremity vascular disease and its relationship with serum Lp-PLA2,25 (OH) D3 levels

Objective To study the value of color Doppler ultrasonography in the evaluation of diabetic lower extremity vascular disease and its relationship with serum lipoprotein related phospholipase A2 (Lp-PLA2) and 25-hydroxyvitamin D3 [25(OH)D3] levels. Methods From February 2015 to February 2016, 200 patients with diabetes in the First Hospital of Shijiazhuang were divided into group A with 43 cases of diabetic lower extremity vascular disease, group B with 157 cases of no diabetic lower extremity vascular disease according to color Doppler ultrasonography examination results. And 40 cases of healthy check-up were selected as healthy control group. The arterial diameter and arterial blood flow in each group was examined by color Doppler ultrasonography, and serum Lp-PLA2, 25(OH)D3 levels were detected by enzyme-linked immunosorbent assay, Pearson correlation analysis was used to analyze the association between artery diameter and blood flow, serum Lp-PLA2, 25 (OH)D3. Results The femoral artery, popliteal artery, dorsalis pedis artery diameter of group A were significantly lower than those of group B and healthy control group, and femoral artery, popliteal artery, dorsalis pedis artery blood flow were significantly lower than those of group B and healthy control group (P < 0.05), the above indexes were not statistically different between the healthy control group and the group B (P > 0.05). The level of serum Lp-PLA2 in group A was significantly higher than that of group B and control group, 25 (OH)D3 level was significantly lower than group B and healthy control group (P < 0.05), while there was no significant difference in levels of Lp-PLA2 and 25 (OH)D3 between the healthy control group and group B (P > 0.05). The vascular diameter and blood flow of patients with lower extremity arterial disease related to diabetic were positively correlated with the level of serum Lp-PLA2 (r = 0.562, 0.621, P < 0.05), and was negatively correlated with the level of 25 (OH)D3 (r = -0.663, -0.582, P < 0.05). Conclusion Patients with diabetic lower extremity vascular disease show the presence of lower extremity arterial stenosis, slow flow of blood, and abnormal changes in serum Lp-PLA2 and 25 (OH)D3 levels. The intra arterial diameter and arterial blood flow are closely related to the levels of serum Lp-PLA2 and 25 (OH)D3.

Objective To investigate the clinical diagnostic value of ultrasound, digital mammography (DM), MRI and combined diagnosis of benign and malignant breast lesions. Methods The breast ultrasound, DM, MRI features and clinical data of 56 lesions in 53 patients in Dongguan Eighth People′s Hospital and Affiliated Mindong Hospital of Fujian Medical University were retrospectively analyzed form April 2015 to March 2018. Based on the BI-RADS, benign and malignant lesions were classified and compared with pathology. The ROCs were plotted and the area under the curve (AUC) were calculated, and the diagnostic efficacy of each and combined examinations were analyzed. Results There were 56 lesions in 53 patients, 38 located in benign and 18 located in malignant. In the 38 benign lesions group, according to the ultrasound dignosis, there were 3 cases of type Ⅱ, 23 cases of type Ⅲ, 9 cases of type Ⅳa and 3 cases of type Ⅳb; according to the DM dignosis, there were 5 cases of type 0, 8 cases of type Ⅱ, 12 cases of type Ⅲ, 7 cases of type Ⅳa and 4 cases of type Ⅳb; according to the MRI dignosis, there were 7 cases of type Ⅱ, 17 cases of type Ⅲ, 11 cases of type Ⅳa, 2 cases of type Ⅳb, and 1 case of type Ⅳc. In the 18 malignant lesions group, according to the ultrasound dignosis, there were 2 cases of type 0, 1 case of type Ⅲ, 6 cases of type Ⅳa, 4 cases of type Ⅳb, 4 cases of type Ⅳc and 1 case of type Ⅴ; according to the DM dignosis, there were 2 cases of type 0, 2 cases of type Ⅲ, 3 cases of type Ⅳa, 2 cases of type Ⅳb, 2 cases of type Ⅳc and 7 cases of type Ⅴ; according to the MRI dignosis, there were 1 case of type Ⅳa, 4 cases of type Ⅳb and 13 cases of type Ⅴ. The AUC of combination of three techniques and MRI, DM, ultrasound were 0.987, 0.976, 0.809, 0.779 respectively, the three combined examination and MRI diagnostic efficacy superior to ultrasound and DM (P < 0.05), and the combined examination and MRI no significant difference(P = 0.308). Conclusion There have some ultrasound, DM and MRI had features in benign and malignant breast disease, a variety of imaging methods of comprehensive utilization can improve the diagnostic performance.

131 High frequency ultrasonography for detecting blood flow parameters of fingertip and toe artery in patients with type 2 diabetes mellitus and its screening value in early vascular diseases

Objective To analyze the detection of high frequency ultrasonography in blood flow parameters of finger, toe tip artery and screening value of early vascular disease in patients with type 2 diabetes mellitus (T2DM). Methods From March 2016 to March 2017, 90 cases of T2DM in the Workers′ Hospital of Tangshan City ("our hospital" for short) were selected as the T2DM group, among them, there were 48 cases with the disease duration of 1-3 years, 22 cases with > 3-9 years, 11 cases with >9-<16 years, and 9 cases with ≥ 16 years. Ninety healthy subjects in our hospital were selected as the control group at the same period. The blood flow parameters of finger and toe tip artery were detected by high frequency ultrasonography in the each group. Results The resistance index of finger tip artery in patients with the disease duration of >3-9 years, >9-<16 years and ≥ 16 years in the T2DM group were significantly higher than those in the control group, and the blood flow were significantly lower than those in the control group, the differences were statistically significant (P < 0.05). The finger tip peripheral arterial vascular diameter for patients with the disease duration of >9-<16 years and ≥ 16 years in the T2DM group were significantly lower than those in the control group, the differences were statistically significant (P < 0.05). The resistance index of toe tip artery for patients with the disease duration >3-9 years, >9-<16 years and ≥ 16 years in the T2DM group were significantly higher than those in the control group, and the vascular diameter and blood flow were significantly lower than those in the control group, the differences were statistically significant (P < 0.05). Conclusion Using high frequency ultrasonography can effectively detect the blood flow parameters of finger and toe tip artery in patients with T2DM, which can help to screen the early vascular lesion, so it has important assessment value.

135 MRI findings of ovarian sex cord-stromal tumor and analysis of clinical pathological features

Objective To investigate the value of magnetic resonance imaging (MRI) diagnosis for ovarian sex cord-stromal tumor, and the correlation of its MRI findings with pathological features. Methods The MRI findings of 30 patients of ovarian sex cord-stromal tumor confirmed by pathological features were retrospectively analyzed in the Third Affiliated Hospital of Guangzhou Medical University from 2012 July to 2017 June. The correlation of imaging features with clinical pathological structural features was analyzed. Results Among the 30 patients of ovarian sex cord-stromal tumor, 22 (73.33%) were thecoma-fibroma folliculi tumor, including 3 patients of fibroma, 8 patients of theca cell tumor, 11 patients of thec-fibroma. The lesions were isointesity or hypointense on T1WI and equal or higher signal on T2WI. Fiber components showed lower signal on T1WI and T2WI, with slight enhancement in 18 cases and moderate enhancement in 3 cases after contrast medium injection. There were 8 (26.67%) cases of granulosa-cell tumors. The lesions were isointesity or hypointense on T1WI and higher signal on T2WI. Cystic walls and septation showed obvious enhancement on contrast-enhanced images. Conclusion MRI has some characteristics to assess and diagnosis ovarian sex cord-stromal tumor, and MRI features of each type of tumor have a certain internal correlation with clinical pathological structural features.

139 Analysis on clinical safety and efficacy of Reduning Injection combined with Ceftazidime Sodium in treatment for patients with Mycoplasma Pneumoniae pneumonia

Objective To analyze the clinical safety and efficacy of Reduning Injection combined with Ceftazidime Sodium in treatment for Mycoplasma Pneumoniae pneumonia. Methods A total of 110 patients from April 2016 to July 2017 treated in the First Affiliated Hospital of Harbin Medical University with Mycoplasma Pneumoniae pneumonia were selected as research objects, and they were randomly divided into experimental group and control group via blind method, with 55 patients in each group. The experimental group was treated by Reduning Injection combined with Ceftazidime Sodium, while the control group was treated by Ceftazidime Sodium. Improvement time of symptoms, serum C-reactive protein (CRP), effective rate and adverse reactions between the two groups were compared. Results The disappearing time of pulmonary moist rales, chest X-ray shadows, fever, cough and wheezing in experimental group were significantly shorten than those in control group, the differences were statistically significant (P < 0.05). The hospitalization time in experimental group was significantly less than that in control group, the difference was statistically significant (P < 0.05). After treatment, the levels of CRP in the two groups were lower than those of the before treatment, and the experimental group was lower than that in control group, the difference was statistically significant (P < 0.05). The total effective rate in experimental group was 96.36%, which was significantly higher than that in control group (83.64%), the difference was statistically significant (P < 0.05). No severe adverse reactions occurred in the two groups. The incidence rate of adverse reactions in experimental group was significantly lower than that in control group, the difference was statistically significant (P < 0.05). Conclusion The effect of Reduning Injection combined with Ceftazidime Sodium in treatment for Mycoplasma Pneumoniae pneumonia is better, that can alleviate the shortterm inflammatory reaction, improve the symptoms, inhibit aggravation of the disease, and reduce adverse reactions. The safety of Reduning Injection combined with Ceftazidime Sodium is good.

143 Surveillance and analysis of multi-drug resistant bacteria infections in AIDS patients in traditional Chinese medicine hospitals

Objective To investigate the differences of composition and distribution of multidrug-resistant bacteria in patients with HIV/AIDS between Hubei Provincial Hospital of Traditional Chinese Medicine ("our hospital" for short) and western hospitals, and to explore the existing problems in the prevention of multi-drug resistant bacteria hospital infection in traditional Chinese medicine hospitals and put forward effective measures for reference. Methods A total of 852 samples of the AIDS patients diagnosed with hospital infection in our hospital from January 2008 to December 2017 were isolated and cultured, bacterial identification and drug sensitivity analysis were conducted, and personal data were collected. Results The detection rate of multidrug-resistant bacteria in our hospital was 51.17%. The lower respiratory tract (60.09%) was the main infected part, followed by blood system (19.95%) and urinary system (14.91%). HIV/AIDS infection in patients with multiple drug-resistant bacteria were mainly Pseudomonas aeruginosa (25.00%), Escherichia coli (20.18%), coagulase negative staphylococcus (17.89%), Klebsiellapneumoniae (10.09%), Acinetobacterbaumannii (8.95%), Staphylococcus aureus (8.03%), Candida albicans (6.88%). Conclusion Compared with Western hospitals, the detection rate of multidrug-resistant bacteria is lower in our hospital. However, it is necessary to strengthen the monitoring of multidrug-resistant bacteria spectrum, attach importance to the combination therapy of Chinese and Western medicine, implement the classification management system of antibacterial drugs, so as to effectively control the occurrence of multiple drug-resistant bacteria hospital infection of AIDS patients.

146 Diagnostic evaluation of three methods for detecting Helicobacter pylori in clinical digestive tract related diseases

Objective To evaluate three diagnostic methods for detecting Helicobacter pylori (Hp) infection in clinical digestive tract related diseases. Methods A total of 177 cases treated in Beijing Ditan Hospital from January 2017 to December 2017 were collected, the Hp was detected via C-urea breath test (13C-UBT), rapid colloidal gold and Western blot methods. 13C-UBT method was used as golden standard and rapid colloidal gold and Western blot methods were compared in the sensitivity and specificity. Results Of the 177 cases, 106 cases were positive in 13C-UBT test, 71 cases were negative. Among 106 cases of 13C-UBT positive patients, 100 cases were positive in colloidal gold assay method and 6 cases were negative. The sensitivity rate was 94.3% and the specificity rate was 83.1%; the positive predictive value was 89.3% and the negative predictive value was 90.8%. Among 106 cases of 13C-UBT positive patients, 102 cases were positive and 4 cases were negative by the method of Western blot method. The sensitivity rate was 96.2%, specificity rate was 84.5%; the positive predictive value was 90.3% and the negative predictive value was 93.8%. There was no significant difference between the two methods (P > 0.05). There were 147 cases of different gastrointestinal diseases were detected by the method of Western blot, of which 66 cases were type Ⅰ, accounting for 44.9%; type Ⅱ 20 cases, accounting for 13.61%. There was a significant difference between the two subtypes (χ2 = 34.778, P = 0.000). The subtypes distribution of Hp, type Ⅰ and Ⅱ had a significant difference between gastroduodenal ulcer and chronic gastritis (P < 0.05), while the distribution in reflux esophagitis had no difference (P > 0.05). Conclusion The rapid colloidal gold method is highly correlated with the infection of Hp and is suitable for the preliminary screening of Hp infection. However, the method of Western blot can be used for toxicity typing and is more suitable for the choice of clinical drug treatment.

A retrospective analysis on the problems existing in drug dismantling and management in outpatient pharmacy in the Third People′s Hospital from January to December in 2017, to improve the situation and evaluate the effect. The problems in drug dismantling and management outpatient can be attributed to the difficulty in the management of the drug′s effective period, pollution and loss during the process of drug unpacking, the difficulty in correctly identifying the drug, and the quality change after the drug dismantled. By the establishment of a perfect drug management system and operational procedures, a clear division of labor and regular training for the personnel, strict control over the drug environment after the removal of the zero, and more humanized methods for the use of drug for patients, it ensures that the unpacked drug safety in management, dispensing and quality.

157 application effect of high-quality nursing at perioperative period in treatment of paroxysmal atrial fibrillation by radiofrequency ablation

Objective To investigate the clinical effect of high-quality nursing at perioperative period in treatment of patients with paroxysmal atrial fibrillation by radiofrequency catheter ablation. Methods From January 2016 to December 2017, in Department of Cardiology, West China Hospital, Sichuan University, 98 patients  with paroxysmal atrial fibrillation treating with radiofrequency catheter ablation. According to the random digital table, 98 patients with paroxysmal atrial fibrillation were divided into observation group and control group, with 49 cases in each group. The observation group was given high-quality nursing at perioperative period, while the control group was given routine nursing at perioperative period. The total effective rate, complication rate and the patients′ satisfaction in two groups were compared. Results The total effective rate, satisfaction rate in the observation group were higher than those in the control group, the differences were statistically significant (P < 0.05), the complication rate in the observation group was lower than that in the control group, the difference was statistically significant (P < 0.05). Conclusion For patients with paroxysmal atrial fibrillation, high-quality nursing at perioperative period in treatment of paroxysmal atrial fibrillation is performed, which can significantly improve the total effective rate and reduce the incidence of related complications, improve the relationship between nurses and patients, and improve the quality of nursing and patient satisfaction degree.

161 Clinical effect of high-quanlity nursing care in advanced hepatocellular carcinoma patients with massive ascites

Objective To explore the clinical effect of high-quality nursing care in advanced hepatocellular carcinoma patients with massive ascites. Methods From January 2015 to June 2017, in Nanchong Central Hospital, 92 patients with advanced hepatocellular carcinoma with massive ascites were selected, according to nursing method, they were divided into control group and observation group, with 46 cases in each group. The observation group was given high-quanlity nursing care, the control group was given conventional nursing care. The score of quanlity of life, pain and satisfaction degree for nursing were observed. Results After nursing, the scores of quanlity of life, satisfaction degree in observation group were higher than control group, the differences were statistically significant (P < 0.05), scores of numeric rating scale in observation group was lower than control group, the difference was statistically significant (P < 0.05). Conclusion Advanced hepatocellular carcinoma patients with massive ascites nursed with high-quanlity nursing care can improve quanlity of life, reduce pain, improve satisfaction degree, the clinical effect is remarkble.

164 Study on pathogenic factors of ischemic stroke patients and primary prevention method

Objective To explore the risk factors of patients with stroke and the primary prevention methods, in order to provide references for the clinical treatment and nursing. Methods From February 2016 to February 2017, in Department of Neurology, Chinese PLA General Hospital ("our hospital" for short), 80 stroke patients were selected as cases group, at the same period, 100 non-stroke patients were selected as control group. Through literature review made possible factors associated with the disease: routine physical examination, smoking, excessive alcohol consumption, improper diet, excessive waist-to-hip ratio, physical exercise, mental stress and depression, normal communication with children, normal social interaction, hypertension, diabetes, basic heart disease, hyperlipidemia. The case-control study was conducted on the occurrence of related factors in the two groups, and the difference comparison was made using the odds ratio (OR) and chi-square test; combined with the results of case-control analysis, further exposure factors and Logistic regression analysis were performed. Results There was no statistically significant difference between the two groups in routine physical examination, normal social intercourse, diabetes and hyperlipidemia (P < 0.05), while there were significant differences in other factors (P < 0.05). The results of Logistic regression analysis showed that physical exercise, normal communication with their children were protective factors (P < 0.05), and smoking, excessive alcohol consumption, improper diet, excessive waist-to-hip ratio, mental stress and depression, hypertension, and underlying heart disease were risk factors (P < 0.05). Conclusion Smoking, excessive alcohol consumption, improper diet, excessive waist-to-hip ratio, mental stress and depression, hypertension, and underlying heart disease are risk factors for stroke. The primary prevention method for stroke patients can be found according to the cause of the disease.

168 Effects of evidence-based nursing intervention on deep vein thrombosis of lower extremity after multiple trauma

Objective To explore and analyze the effect of evidence-based nursing intervention on postoperative deep vein thrombosis in patients with multiple trauma. Methods From June 2015 to June 2017, in Nuclear Industry 215 Hospital, 90 patients with multiple traumatic injuries were selected, according to random number table method, they were divided into control group and observation group, with 45 cases in each group. The control group was given routine nursing, and the observation group was given evidence-based nursing intervention based on the control group, the incidence of postoperative lower extremity swelling, tenderness, and deep venous thrombosis after operation, Porter score and patient satisfaction after treatment in two groups were compared. Results After operatiom, the incidence of lower extremity swelling, tenderness of gastrocnemius, and the deep venous thrombosis in the observation group were lower than those in the control group, the differences were statistically significant (P < 0.05). After treatment, the Porter scores in the two groups were lower than before treatment, and this in observation group was lower than control group, the differences were statistically significant (P < 0.05). The nursing satisfaction of the observation group was significantly higher than that of the control group, the difference was statistically significant (P < 0.05). Conclusion Evidence-based nursing intervention can effectively prevent postoperative deep venous thrombosis and improve nursing satisfaction compared with routine nursing.

171 Application effect of clinical nursing pathway in rehablitation nursing for patients with ischemic stroke

Objective To analyze the effect of clinical nursing pathway in the implementation of rehabilitation nursing for patients with ischemic stroke. Methods From April to September in 2017, in Department of Neurology, Affiliated Hospital of Xuzhou Medical College ("our hospital" for short), 120 patients with ischemic stroke were selected, according to random number table method, they were divided into two groups, with 60 cases in each group. The control group was treated with routine nursing intervention, the observation group was treated clinical pathway rehabilitation nursing intervention based on control group. The Bathel index (BI) was used to analyze the daily living ability of the patients, the Vada test score was used to evaluate the swallowing function before and after treatment, the Montreal Cognitive Assessment Scale (MoCA) was used to analyze the cognitive status of the patients after treatment. The HAMA and HAMD were used to analyze the depression and anxiety before and after nursing. The self-made nursing satisfaction scale of our hospital was used to investigate the nursing satisfaction of patients after nursing, stroke neurological deficits (CNS) and clinical dementia scores (CDR) before and after nursing were analyzed. Results After nursing, the scores of BI, Vada test score, MoCA, HAMA, HAMD and CNS, CDR of two groups were better than before nusing, and these of observation group were better than those of the control group, the differences were statistically significant (P < 0.05); after nursing, the nursing satisfaction rate of observation group was higher than that of the control group, the difference was statistically significant (P < 0.05). Conclusion For patients with ischemic stroke, in their rehabilitation care work, the application of clinical pathways contributes to cure of the disease, improve the quality of life of patients, it is worth further use.

176 Application of fast track surgery concept combined with clinical nursing pathway in patients undergoing laparoscopic cholecystectomy

Objective To explore the application of fast track surgery (FTS) concept combined with clinical nursing pathway in patients undergoing laparoscopic cholecystectomy (LC). Methods From June 2015 to June 2017, in Hospital of Ningshan County, 108 patients with gallbladder disease treated with LC were selected, according to different nursing methods, they were divided into two groups, with 54 cases in each group. The convention group was given perioperative standardized clinical nursing pathway intervention, while FTS group was given perioperative FTS combined with clinical nursing pathway intervention. Preoperative negative emotions, postoperative pain, sleep quality and recovery in the two groups were observed, levels of inflammatory stress and blood glucose, postoperative complications and nursing satisfaction in two groups were compared. Results The positive and negative affect schedule (PANAS) score in FTS group at 1 d before operation, visual analogue scale (VAS) and electronic Pittsburgh sleep quality index (ePSQI) scores in FTS group at 1 d after operation were significantly lower than convention group, the differences were statistically significant (P < 0.05). The postoperative initial anal exhaust time, recovery time of bowel sound, the time of taking food for the first time, initial ambulation time and hospitalization time in FTS group were significantly shorter than convention group, the differences were statistically significant (P < 0.05). The white blood cell count (WBC), percentage of neutrophils (GRA), C reactive protein (CRP) and blood glucose levels in the two groups at 1, 3 d after operation were significantly higher than 1 d before operation, and 1 d after operation was significantly greater than 3 d after operation, the improvement of above-mentioned indexes in FTS group at different time points after operation were greater than convention group, the differences were statistically significant (P < 0.05). The incidence rates of complications in FTS group was lower than convention group, the difference was statistically significant (P < 0.05). The nursing satisfaction in FTS group was higher than convention group, the difference was statistically significant (P < 0.05). Conclusion The application of FTS concept combined with clinical nursing pathway in the perioperative nursing for patients undergoing LC can effectively relieve the psychological and physiological stress response, promote the postoperative revcovery and reduce complications, the nursing satisfaction of patients is high.
