8 Exploration on the innovation of “new master-apprentice” practical teaching mode under the background of omnimedia
ZHU Shu1 WANG Chunyan2 WANG Shijun1 REN Jian1 WANG Lin3 LYU Yingyu1 WANG Jin1
Master-apprentice is the main way of inheriting education of ancient Chinese medicine, but the large-scale cultivation and standardized development of higher education of traditional Chinese medicine have affected master-apprentice education to a certain extent. Many colleges and universities of traditional Chinese medicine try to realize teacher training again through various ways. Although some progress has been made, there are still some problems. Under the background of omnimedia, Shandong University of Traditional Chinese Medicine tries to create the innovative teaching mode of “new master-apprentice”. So-called “new”, the first is to innovate the form of inheritance, change the previous “one-to-one” single form of teacher-student inheritance, create the project of inheriting students, build the inheritance team of teachers and students, realize various forms of “one to many, many to one, many to many”, and promote the large-scale development of teacher apprenticeship training. The second is to innovate the content of inheritance, change the previous inheritance of “academic view of one’s own”, summarize the excellent medical ethics, academic ideas and characteristic skills of famous doctors in the past, organically integrate them into the inheritance and training, and highlight the local characteristics while realizing the standardized inheritance. The third is to innovate the inheritance media. Under the background of omnimedia, we should promote the full integration of the new and old inheritance media, and thus start the innovative exploration of the “new master-apprentice” teaching mode.

WANG Lifang HAN Han SHI Bin
Objective To investigate the evaluation value of red blood cell distribution width (RDW) combined with neutrophil/lymphocyte ratio (NLR) in prognosis of patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD). Methods One hundred and sixty patients with AECOPD admitted to Suqian Hospital Affiliated to Xuzhou Medical University from January 2017 to December 2019 were selected and divided into survival group (120 cases) and death group (40 cases) according to their prognosis and survival. The treatment methods, complications, RDW and NLR levels of two groups were compared. The effects of RDW and NLR on the prognosis of AECOPD were analyzed by multivariate logistic regression. The diagnostic value of serum RDW and NLR in the prognosis of AECOPD were evaluated by the area under ROC curve. Results The proportion of mechanical ventilation in survival group was significantly lower than that in death group (P < 0.05). There were no significant differences in the proportion of β2 receptor agonist and inhaled hormone between two groups (P > 0.05). The incidence rate of pulmonary heart disease in survival group was significantly lower than that in death group (P < 0.05). There were no significant differences in the incidence of hypertension, hyperlipidemia, diabetes and atrial fibrillation between two groups (P > 0.05). The levels of RDW and NLR in survival group were significantly lower than those in death group (P < 0.05). Multivariate logistic regression analysis showed that mechanical ventilation (OR = 23.050, 95%CI: 1.688-314.701, P < 0.05), pulmonary heart disease (OR = 15.223, 95%CI: 1.248-185.706, P < 0.05), RDW (OR = 171.288, 95%CI: 6.075-4829.307, P < 0.05) and NLR (OR = 8.410, 95%CI: 1.414-50.022, P < 0.05) were independent risk factors for the prognosis of AECOPD (P < 0.05). The area under the curve of combined detection was significantly higher than that of RDW and NLR (P < 0.05). Conclusion The elevated levels of RDW and NLR are risk factors for the prognosis of AECOPD patients, early combined detection can be used as an important biochemical marker to diagnose the prognosis of patients.

ZHANG Duobin CHEN Houqin CAI Hongxing ZUO Jian
Objective To study the risk factors of cerebral microbleeds (CMBs) in patients with ischemic stroke and to analyze the relationship between serum cystatin C (CysC), neutrophil/lymphocyte ratio (NLR) and CMBs. Methods A total of 80 patients with ischemic stroke admitted to Wuhu NO.1 People’s Hospital, Anhui Province from January 2017 to February 2020 were selected and divided into CMBs group (33 cases) and non-CMBs group (47 cases) according to the occurrence of CMBs. The differences of baseline data and biochemical indexes were compared between the two groups. The risk factors of CMBs and predictive value of CysC and NLR for CMBs were analyzed. Results The age and hypertension history rate in CMBs group were significantly higher than non-CMBs group (all P < 0.05). The fasting blood-glucose (FBG), NLR and CysC levels of the CMBs group were higher than those of the non-CMBs group (all P < 0.05). High age, with history of hypertension, high NLR, and high CysC levels were all independent risk factors for CMBs (all P < 0.05). The receiver operating characteristic (ROC) curve analysis for the prediction and diagnosis of CMBs showed that the theoretical threshold of NLR was 2.60, the sensitivity was 80.2%, the specificity was 73.9%, and the area under curve (AUC) was 0.727 (P < 0.05). The theoretical threshold of serum CysC level was 1.19 mg/L, the sensitivity was 82.6%, the specificity was 71.8%, and the AUC was 0.758 (P < 0.05). The combined application of the two indexes had higher diagnostic efficiency: the sensitivity was 84.2%, the specificity was 85.0%, and the AUC was 0.845 (P < 0.05). Conclusion Elderly patients with ischemic stroke who have a history of hypertension, high serum CysC level, and high NLR may be at higher risk of CMBs. Serum CysC level and NLR can be used as sensitive indicators for CMBs occurrence.

LIU Qiwei1 WU Qiong2 WANG Zhiwu2
Objective To evaluate the effect of serum bone morphogenetic protein-2 (BMP-2) level on the prognosis of patients with non-small cell lung cancer (NSCLC). Methods A total of 57 NSCLC patients admitted to Tangshan People’s Hospital, Hebei Province from January to December 2017 were selected. Clinicopathological features and overall survival data of the patients were collected and the serum BMP-2 level of the patients were detected. The relationship between serum BMP-2 level and clinicopathological characteristics and the relationship between serum BMP-2 level and prognosis in patients with lung cancer were analyzed, and the influence of BMP-2 level and other clinicopathological factors on patients with NSCLC was discussed by univariate and multivariate survival analysis. Results The difference of serum BMP-2 level in patients with different pathological stages and metastatic sites was statistically significant (P < 0.05). There was no significant difference in serum BMP-2 level among patients with different age, sex, histological type and degree of tumor cell differentiation (P > 0.05). The median survival time of all patients was 9.8 months (95%CI: 7.3-11.5). The median survival time of NSCLC patients with high BMP-2 level was 6.8 months (95%CI: 4.5- 9.3), shorter than that of NSCLC patients with low BMP-2 level was 12.1 months (95%CI: 9.4-14.8), the difference was highly statistically significant (χ2 = 9.15, P < 0.001). Single factor survival analysis found that the pathological stage Ⅳ (HR = 1.539, 95%CI: 1.127-3.228, P = 0.013), number of metastatic sites (≥three sites) (HR = 1.738, 95%CI: 1.194-4.380, P = 0.001) and BMP-2 levels (≥75 pg/mL) (HR = 2.019, 95%CI: 1.233-6.229, P = 0.000) were the adverse factors affecting the prognosis of patients with NSCLC. Multivariate survival analysis showed that serum BMP-2 level (≥75 pg/mL) (HR = 1.837, 95%CI: 1.146-3.473, P = 0.007) and number of metastatic sites (≥three sites) (HR = 1.199, 95%CI: 1.028-2.337, P = 0.037) were independent adverse factors affecting the prognosis of NSCLC patients. Conclusion High serum BMP-2 level and more than three metastatic sites are independent adverse prognostic factors for NSCLC patients, which can predict the prognosis of NSCLC.

49 Research progress of traditional Chinese medicine in preventing and treating hypertensive renal damage
LIU Shufang LIU Xiangying ZHANG Lin
Hypertension is a widespread primary chronic disease in China. Long-term poorly controlled hypertension may cause intimal thickening of renal arteries and arterioles and lumen stenosis, glomerular sclerosis, interstitial fibrosis, and renal tubular atrophy. Kidney damage caused by high blood pressure is a multifactorial process that plays a vital role in the development of chronic kidney disease. According to the kidney database, hypertensive renal damage is the main cause of end-stage renal failure, second only to diabetic nephropathy. Hypertension and chronic kidney disease are serious public health problems related to each other. Hypertension can cause cardiovascular and cerebrovascular diseases and kidney damage. However, chronic kidney disease can also cause cardiovascular events, cerebrovascular events and all-cause death. The increase in the prevalence of hypertensive renal damage, the high cost of treatment and poor prognosis are an important health problem, which to a certain extent causes social and economic burdens and waste of medical resources. The concept of “preventive treatment of disease” in Chinese medicine has a long history. This article explains the specific measures of traditional Chinese medicine prevention and treatment of hypertensive renal damage from the two aspects of “prevention before disease” and “prevention after disease”.

ZHAO Liping1 ZHOU Chenming2 LI Jingjing3 XU Yannan4▲
Myocardial infarction is the most common critical disease in clinic, which is a serious threat to human health. Bone marrow mesenchymal stem cells are ideal seed cells for the treatment of cardiovascular diseases due to their advantages of multidifferentiation, easy access, rapid proliferation, low immunogenicity and strong transplantability. The mechanism of bone marrow mesenchymal stem cells therapy in myocardial infarction includes anti-inflammatory and anti-fibrosis, promoting angiogenesis, and immune regulation. However, in the process of transplantation, the survival of transplanted cells is affected by the activity, migration, homing and other factors of transplanted cells. In this paper, the characteristics of bone marrow mesenchymal stem cells and the mechanism of treating myocardial infarction are summarized as follows.

Objective To investigate the current status of vitamin D levels of Shanghai shooters in winter and to compare them with local healthy young people of the same age, and to analyze its influencing factors. Methods From February to March 2018, 46 shooting athletes from the Shanghai shooting team were selected as the athlete group, and 50 healthy young people of the same age who had a physical examination at the Shanghai Ninth People’s Hospital during the same period were selected as the control group. The serum 25-hydroxyvitamin D[25(OH)D] level, and relevant information was collected. The difference in vitamin D levels between athletes and people of the same age were analyzed, as well as the possible influencing factors that lead to the difference. Results The level of 25(OH)D and serum parathyroid hormone (PTH) of the athlete group was significantly higher than that of the control group, the blood calcium level was significantly lower than that of the control group, while the sun exposure time was longer than that of the control group, and the differences were statistically significant (P < 0.05). There was a statistically significant difference in the distribution of serum 25(OH)D levels between the two groups (P < 0.05). The difference in exposure time of athletes with different serum 25(OH)D levels was statistically significant (P < 0.05). Among them, the vitamin-deficient athletes had significantly longer exposure time than other levels (P < 0.05). There was a positive correlation between serum 25(OH)D levels and sun exposure time in all subjects (r = 0.685, P < 0.05). The level of serum 25(OH)D in the athlete group was correlated with the exposure time (r = 0.498, P < 0.05). Sun exposure time was a protective factor that affects the serum 25(OH)D level in the athlete group (OR = 0.095,95%CI: 0.018-0.493, P < 0.05). Conclusion In winter, Shanghai shooting athletes have insufficient or lack of vitamin D. Sun exposure time is the protective factor of serum 25(OH)D level in athletes. A reasonable assessment of the nutritional status of athletes’ vitamin D and the formulation of a reasonable vitamin D supplementation plan will help maintain athletes’ health and improve their competitive status.

FENG Gong1* LI Rong2* LIU Junlin3* HAO Shuai2 XU Meiqi2 WU Ge2 MI Man1 YAN Qinqin1▲
Objective To explore the training mode of general medical students from the perspective of professional identity. Methods A cluster sampling method was adopted to select 600 general medical students from Xi’an Medical University as the research objects, and a nested case-control study with gender as a matching factor (matching according to 1∶1) was adopted. Starting from September 2018, the first survey of occupational recognition would be conducted; the second survey would be conducted in December 2019. The students whose occupational identity had changed in the two surveys were taken as the case group (the score changes from high to low for the case-control group 1 and the score from low to high for the case-control group 2), and students whose occupational identity unchanged as the control group. The influencing factors of professional identity were analyzed. Results In the case-control group 1, there were statistically significant differences in the occupational identity scores of students of different grades, ages, and majors (P < 0.05). Grade (OR = 0.589) and major (OR = 0.546) were independent factors influencing the professional identity of general medicine students (P < 0.05). In the case-control 2 group, there was a statistically significant difference in the professional identity scores of students who participated in club activities and whether they had part-time experience (P < 0.05). Part-time experience (OR = 0.648) was an independent factor influencing the professional identity of general medicine students (P < 0.05). Conclusion Grade, major, and part-time experience are important factors that affect students’ professional identity. Based on this, a plan for optimizing the training model of general medical students can be further explored.
