4 The expression and significance of Tim-3/Galectin-9 pathway and Foxp3 in the peripheral blood mononuclear cells of oral lichen planus patients

Objective To study the correlation between Foxp3 and T cell immunoglobulin domain mucin domain protein-3 (Tim-3) and Galactosin -9 (Gal-9) in order to discover the role of Tim-3/Gal-9 pathway in the pathogenesis of oral lichen planus (OLP). Methods From September 2015 to September 2016, 58 OLP patients admitted to the specialized Outpatient Department of Oral Mucosal Disease of the Affiliated Hospital of Guizhou Medical University were selected as the OLP group (36 cases of non-erosive type and 22 cases of hyperemia type). And 17 healthy subjects admitted to the Affiliated Hospital of Guizhou Medical University during the same period were selected as the normal group. The expression levels of Tim-3, Gal-9 and Foxp3 mRNA in peripheral blood mononuclear cells of the two groups were measured by real-time quantitative polymerase chain reaction, and the correlation of the three factors was analyzed. Results The relative expression levels of Tim-3 mRNA and Foxp3 mRNA in non-erosive OLP and hyperemic OLP were higher than those in the normal group (P < 0.05). The relative expression levels of Gal-9 mRNA in all OLP types were not significantly different from those in the normal group (P > 0.05). Foxp3 was positively correlated with Tim-3 and Gal-9 (r = 0.738, P = 0.000; r = 0.443, P = 0.010); there was a positive correlation between Tim-3 and Gal-9 (r = 0.453, P = 0.018). Conclusion Tim-3/Gal-9 signaling mediated Th1 cell apoptosis may be one of the mechanisms by which OLP induces Th2 immune migration, and may be involved in the immune regulation of OLP cells.

8 Effect of Allgower-Donati addition method on the initial healing of calcaneal fracture

Objective To analyze the effects of two different suture methods on the healing of incisions after calcaneal fracture surgery. Methods A total of 62 patients with calcaneal fractures who underwent open reduction and internal fixation at Jieshou City People Hospital from January 2015 to December 2018 were selected. According to different incision suture methods, they were divided into experimental group (31 cases) and control group (31 cases). The experimental group was used Allgower-Donati suture, while the control group was used intermittent vertical mattress suture. The operation time, incision suture time, inflammatory reaction area of the skin around the incision, the differences in incision skin temperature and the healing of the incision were compared between the two groups. Results Both groups of patients successfully completed the operation. The incision suture time of the experimental group was shorter than that of the control group, and the difference was statistically significant (P < 0.05). The inflammation reaction area of the skin around the incision in the experimental group was significantly smaller than that in the control group, and the difference was statistically significant (P < 0.05). The differences in incision skin temperature between the two groups were statistically significant in different time and between groups (P < 0.05). Among them, the differences in incision skin temperature between the experimental group and the control group on the third day after surgery were lower than those on the first and second days after surgery, while the difference on the second day after surgery was lower than that on the first day after surgery, and the differences were statistically significant (P < 0.05); the differences in skin temperature of the experimental group were lower than those of the control group on the second and third day after surgery, and the differences were statistically significant (P < 0.05). There was no significant difference in the incidence of complications between the two groups (P > 0.05). Conclusion Compared with the traditional vertical mattress suture method, the Allgower-Donati suture method is used to treat the “L”-shaped incision in patients with calcaneal fractures. The incision suture time is shorter and the area of skin inflammation around the incision is smaller. It is worthy of promotion.

12 Influencing factors of autonomous learning ability of freshmen in Jilin Province medical colleges

Objective To understand the current situation of the autonomous learning ability of the freshman students in medical colleges and universities in Jilin Province, and to analyze the factors affecting the autonomous learning ability of the freshmen. Methods Using the cluster sampling method, a total of 900 questionnaires were distributed to three medical colleges and universities in Jilin Province. A total of 300 freshmen students as the research objects were randomly selected in each college. The general information questionnaire, autonomous learning ability scale, college student psychological self-reliance questionnaire, Chinese version California critical thinking tendency questionnaire, educational environment rating scale, self-efficacy scale, and social support scale were used as data collection tools, and SPSS 22.0 software was used for principal component analysis. Results The Cronbach′s α coefficient of the questionnaire with 9 indicators was 0.920, the KMO was 0.934, and the significance of Bartlett′s test was 23 308.544. Taking the first six principal components with eigenvalues> 1, the total variance explanation rate was reached 79.491%, which were social support, self-efficacy, curiosity, cognitive maturity, educational environment, and self-management. The main factors affecting freshman students′ autonomous learning ability were social support, personal factors and education platform. Conclusion Social support has a significant impact on freshman students′ autonomous learning ability, personal factors have a general impact, and education platform have relatively little impact.

16 Effect study on MTDH mediated baicalin inhibiting proliferation, migration and invasion of 4T1 breast cancer cells

Objective To investigate the effects of baicalin on proliferation, migration and invasion of 4T1 cells based on metadherin (MTDH) gene. Methods 4T1 cells in logarithmic growth stage were divided into blank control group and baicalin group with different doses of 25, 50, 100, 150 mg/L and 200 mg/L, and then treated with corresponding drugs. Cell activity after baicalin treatment was detected by MTT method. Migration was measured by scratch assay. Invasion was measured by transwell assay and MTDH expression in cells was measured by Western blot. Results The effect of baicalin at 25 mg/L on cell migration was not obvious. As the concentration increased to 50 mg/L, the inhibition of cell migration gradually became obvious. When the concentration increased to 100 mg/L and 150 mg/L, the inhibition of cell migration showed obvious effects. The inhibition rate of cell invasion of baicalin groups with different concentrations was statistically significant (P < 0.05). The inhibition rate of cell invasion in the group of 150 mg/L baicalin was higher than that in the group of 100 mg/L baicalin (P < 0.05). The inhibition rate of cell invasion in the 100 mg/L and 150 mg/L baicalin group was higher than that in the 50 mg/L baicalin group, and the differences were highly statistically significant (P < 0.01). Compared with the control group, MTDH expression was decreased in the 50, 100 mg/L  and 150 mg/L baicalin groups (P < 0.05 or P < 0.01). Conclusion Baicalin can inhibit the growth and proliferation of 4T1 breast cancer cells, inhibit the migration and invasion ability of breast cancer cells, and its mechanism may be related to the down-regulation of MTDH expression.

20 Determination of 12 plasticizers residues in pharmaceutical composite films by gas chromatography-mass spectrometry

Objective To establish a gas chromatography-mass spectrometry (GC-MS) method for the determination of 12 plasticizers residues in pharmaceutical composite films. Methods The samples were extracted with n-hexane ultrasound and detected by GC-MS. The gas chromatography was performed on a HP-5 MS capillary column (30 m×0.25 mm×0.25 μm). The program was heated up, the inlet temperature was 290℃, and an electron bombardment (EI) ion source was used in the mass spectrometry to detect under the selected ion monitoring mode. The ion source temperature was 280℃ and the quadrupole temperature was 150℃. Results The values of correlation coefficient r of the 12 plasticizer standard curves were greater than 0.998, the average recovery was 78.6%-110.9%, RSD ≤ 4.0% (n = 9). Conclusion This method has good accuracy and high sensitivity, which can be used for the detection of plasticizers residues in pharmaceutical composite films.

24 In vitro study on the effect of tooth brushing on enamel wear under different oral pH

Objective To study the dynamic changes of salivary pH and the effect of tooth brushing on enamel wear in different pH environments. Methods From March 10 to 15, 2019, the dynamic changes of saliva pH in 180 medical volunteers in General Hospital at different time points after morning rise were detected for five days (T1: immediately after morning rise, T2:five minutes after morning rise, T3: 10 min after morning rise). From June to September 2019, the isolated teeth collected from the Department of Dental Surgery, Aviation General Hospital were prepared into enamel bases, and after screening, they were divided into two groups: the control group (n = 9) and the experimental group (n = 10), which were soaked with pH=6.8 and 5.8 respectively. The brushing wear experiment was carried out every eight hours. After repeating the experiment for 14 days, the wear depth and surface morphology of the samples were detected. Results There were statistically significant differences in salivary pH values at T1, T2 and T3 after morning rise in 180 healthy subjects (P = 0.01). Compared with T1, saliva pH value increased at T2 and T3, with statistically significant differences (P < 0.05). Compared with T2, salivary pH value increased at T3, with statistically significant difference (P < 0.05). After brushing, the wear depth of both groups increased compared with that before brushing, and the experimental group was higher than the control group, with statistically significant differences (P < 0.05). Scanning electron microscope illustration showed that the surface morphology of the experimental group was coarser. Conclusion Immediately after morning rise, the pH of oral cavity is low and becomes weakly acidic. Brushing in this environment results in increased enamel surface wear. Saliva pH remained stable and close to neutral 10 min after morning rise. In this environment, tooth brushing resulted in less enamel surface wear.

28 Research progress of traditional Chinese and Western medicine in the treatment of chronic heart failure combined with anemia

In clinic, anemia is associated with a high incidence rate in chronic heart failure, and patients with chronic heart failure combined with anemia have high mortality and poor prognosis, thus, anemia correction is becoming one of the new promising targets for the treatment of patients with chronic heart failure. On this basis, this paper summarizes conventional Western medicine treatment of chronic heart failure combined with anemia in clinic. In addition, with the enrichment of the theoretical knowledge of Chinese medicine on chronic heart failure with anemia, this paper also summarizes and discusses the research on traditional Chinese medicine treatment of chronic heart failure from the aspects of treatment based on syndrome differentiation and clinical efficacy.

32 Exploration of the relationship between intestinal flora and mucosal immunity and its research progress under pneumonia based on “lung and large intestine”

Today, pneumonia is a global public problem and the major cause of death. The theory of “lung and large intestine inside and outside” summarizes from the long-term clinical practice of traditional Chinese medicine still plays a guiding role. Current physiological and pathological studies suggest that the lung and the large intestine interact through mutual mucosal immunity. This paper mainly explores the immune response of the body under the condition of pneumonia, which leads to the interaction between mucosal immunity and intestinal flora. By summarizing the interaction between intestinal flora and immune system, enriched the new thinking on the pathogenesis and treatment of pneumonia, new ideas for drug development are provided, and new research on the theory of “lung and large intestine” is enriched.

36 Research progress on the correlation between comedo mastitis and body immunity and the immunomodulatory effect of traditional Chinese medicine

Comedo mastitis, includes plasma cell mastitis, granulomatous mastitis, and some rare inflammatory breast diseases, such as IgG4-related sclerosing mastitis and sclerosing lymphocytic lobular mastitis. The cause is still controversial, however, based on domestic and foreign literature study, this disease may be an immune-related disease. This paper reviews the correlation between comedo mastitis and organic immunity and the immunomodulatory effects of traditional Chinese medicine on this disease in recent ten years were reviewed. It was found that T cell mediated cellular immunity, B cell mediated humoral immunity and secretion of various cytokines, immune proteins and complement all participated in the pathological process with certain regularity. By giving full play to the advantages and characteristics of the combination of internal and external treatments and focusing on treatments by stages and syndrome differentiation, traditional Chinese medicine can significantly improve the immune status, shorten the course of the disease and improve the prognosis. This paper reviews the correlation between comedo mastitis and body immunity and the immunomodulatory effects of traditional Chinese medicine. Therefore, the immunological mechanism can be used as the research direction of comedo mastitis, and the traditional Chinese medicine is an effective method for this disease.

40 Research progress of self-management interventions for lower urinary tract symptoms secondary to benign prostatic hyperplasia

Lower urinary tract symptoms secondary to benign prostatic hyperplasia (LUTS/BPH) is a chronic disease that affects the health and quality of life of the elderly. Self-management intervention, as a self-treatment measure in which patients actively participate, can be introduced into the treatment of male LUTS/BPH to help relieve symptoms and improve patients′ quality of life. This paper reviews the self management connotation of LUTS/BPH patients, treatment plan and related adverse events, and lifestyle (liquid management, drug management, etc.), behavioral interventions (secondary urination, bladder training, etc.), exercise training, and puts forward the existing problems, in order to optimize the self-management intervention plans of future clinical nursing workers reference.

44 Research progress on the application of hospital based continuous nursing in cardiac rehabilitation of patients with coronary heart disease

As an important part of the clinical pathway of coronary heart disease treatment, cardiac rehabilitation can not meet the rehabilitation needs of patients at present. Hospital based continuous nursing is the most researched and developed continuous nursing mode, which can promote the patients with coronary heart disease to participate in the heart rehabilitation process, improve the effect of heart rehabilitation, improve the quality of life of patients, and reduce the incidence of cardiovascular adverse events. In this paper, the concept of cardiac rehabilitation and extended nursing, the background and application of extended nursing based on hospital in cardiac rehabilitation were reviewed, in order to provide a reference for the clinical practice of coronary heart disease rehabilitation.

47 Management of glycated hemoglobin A and influencing factors of type 2 diabetes patients at a grade Ⅲ class A hospital in Urumqi

Objective To realize the management of glycated hemoglobin A (HbA1c) in hospitalized patients with type 2 diabetes at a grade Ⅲ class A hospital in Urumqi, and to explore the influencing factors of HbA1c. Methods From January to May 2019, 300 hospitalized patients were selected in a grade Ⅲ class A hospital in Urumqi by convenience method, according to the level of HbA1c, they were divided into standard group (HbA1c<7.0%, 55 cases) and non-standard group (HbA1c≥7.0%, 245 cases), the effects of ethnicity, severity degree of non-alcoholic fatty liver disease (NAFLD), life style and lipid metabolic indictors on HbA1c in type 2 diabetes patients were analgzed. Results There were significant differences among ethnicity, severity degree of NAFLD, regular exercise, sleep quality, triglycerides (TG) and total cholesterol (TC) between two groups (P < 0.05); Spearman rank correlation analysis showed that HbA1c was positively correlated with ethnicity, severity degree of NAFLD, TG, TC (P < 0.01), and negatively correlated with sleep quality and regular exercise (P < 0.01). Conclusion Attention should be paid more to ethinc minorities groups, strengthening the management of NAFLD, cultivating healthy lifestyle, improving lipid disorder, so that to enhance the management level of HbA1c.

51 Study on the relationship between PON1rs662 gene polymorphism and hypertension and its related factors in Shicheng Island of Dalian

Objective To investigate the prevalence and related factors of hypertension in the elderly in Shicheng Island of Dalian, and the relationship between serum P-oxophosphase 1(rs662) gene(PON1rs662) and hypertension. Methods The epidemiological survey on hypertension and related factors was conducted among 654 residents aged more than 65 from June to August 2016. The adjusted WHO-MONICA research questionnaire was used for questionnaire survey, biochemical and echocardiographic examinations were carried out. DNA was extracted by centrifugation column, and gene typing of PON1rs662 was carried out by TaqMan PCR. All the studies were determined whether the genotype distribution of PONIrs662 accord with Hardy-Weinberg Law, and the relationship between PON1rs662 gene and hypertension was investigated. Results The prevalence of hypertension was 73.1%, 77.5% in women and 68.3% in men. There were statistically significant differences in heart rate, body mass index (BMI), waist circumference, uric acid (UA), triglyceride (TG) and left atrial anteropostery (LA) between hypertensive and non-hypertensive subjects (P < 0.05). The genotype distribution of PON1rs662 was in line with Hardy-Weinberg Law (P > 0.05). Both the allele model and the additive moded showed no correlation between the PON1rs662 SNP and hypertension (P > 0.05). Conclusion The prevalence rate of hypertension in the old residents of Shicheng Island was 73.1%. Heart rate, BMI, waist circumference, UA and TG were associated with the prevalence of hypertension on the island. PON1rs662 gene may not be correlated with the prevalence of hypertension in these study objects.

55 Surgical cost analysis and management recommendations for gastric cancer patients in a third-level general hospital in Southwest China

Objective To analyze the cost, cost composition and influencing factors of gastric cancer surgery patients in a third-level general hospital in Southwest China, and to provide a reference for the management and decision-making of medical institutions, medical insurance institutions and health regulatory agencies. Methods Descriptive statistics, single factor correlation analysis and multiple linear stepwise regression were used to analyze the average cost of hospitalization for gastric cancer surgery patients (5868 person-times) and influencing factors from January 2011 to December 2017 in a large tertiary general hospital in Southwest China. Results From 2011 to 2017, the average cost of hospitalization for gastric cancer patients in the top three hospitals was 59 186.46 yuan. The examination expenses, surgical expenses, and nursing expenses per time of gastric cancer surgery patients were constituted an increasing trend. Old age, long hospital stay, type of healing, male, presence of comorbidities and complications, total gastrectomy and year were risk factors for the cost of gastric cancer surgery (OR > 1, P < 0.05), while the type of incision and other types of surgery were the protective factors for the cost of gastric cancer surgery (OR < 1, P < 0.05). Conclusion Medical expenses are increasing rapidly, and the status of expenses reflecting the labor value of medical workers has improved, howere, it is still at a low level. The relatively controllable factors affecting medical expenses are mainly the length of hospital stay.

59 A qualitative study of team building and cooperation status of family doctor contract service in five districts in Beijing

Objective To understand the current situation of the formation and cooperation of the family doctors′ contracted service team in the five districts of Beijing, and to put forward reasonable suggestions to provide references for improving the service capabilities of the family doctor team. Methods From March to May 2019, conduct personal in-depth interviews of community health service management centers (one director per district) in Dongcheng, Xicheng, Chaoyang, Fengtai, and Haidian District in Beijing. The interviews were conducted in the conference rooms of each center and lasted about 60 to 90 minutes. The audio recording was used to record information, while the content analysis was applied to extract topics. Results The topics were extracted from family doctor team combinations, information platforms, incentives measures, existing problems. Team combination: based on the classic composition of medical care and defense, the team combination of each district has its own characteristics; information platform: all districts rely on information platforms and mobile APPs to enhance family doctor team cooperation; incentive measures: performance rewards are implemented to the family doctor team and individuals; existing problems: increased workload, insufficient staffing, unscientific incentive distribution, low income, lack of internal team assessment standards, internal team cooperation problems, low enthusiasm for team members, and weak basic medical service capabilities of general practitioners. Conclusion Strengthening the introduction of talents, establishing a scientific performance appraisal and reward mechanism, increasing the income of family doctors team members, and improving team members′ cooperation ability and basic medical service capabilities through training are the main measures to improve the quality of family doctor team services.

63 Analysis on the influencing factors of the effect of management undergraduates′ graduation practice in Yunnan University of Chinese Medicine

Objective To understand the influencing factors of the effect of management undergraduates′ graduation practice. Methods From March to May 2019, three four-year management undergraduate majors in Yunnan University of Chinese Medicine, including marketing, logistics management, and public utility management were used as the survey objects. A self-made questionnaire was used to survey a total of 142 management majors and exploratory factor analysis were used to extract common factors. Results A total of four common factors were extracted, namely “work attitude” “work ability” “internship performance and management” “internship environment”. Conclusion It is necessary to increase the importance of students on graduation internships. It is also necessary to focus on professional skills and learning ability, and strengthen the training of basic management professional capabilities in the personnel training system. Efforts should be made to improve the standardization level of practice bases and strengthen the evaluation and assessment of graduation practice. As far as possible, we should choose an institution with a better working environment, organizational culture and brand image as the graduation internship unit.

67 Effects of Exenatide Microspheres on insulin resistance and inflammatory factors in patients with type 2 diabetes

Objective To explore the effects of Exenatide Microspheres on insulin resistance and inflammatory factors in patients with type 2 diabetes. Methods A total of 90 patients with type 2 diabetes treated in the Department of Endocrinology of the Second Hospital of Jilin University from September 2018 to June 2019 were collected. According to the random number table method, the patients were randomly divided into the control group (Metformin Hydrochloride) and the observation group (Exenatide Microspheres), with 45 cases in each group, treated for 24 weeks. The levels of clinical biochemical indexes and inflammation markers were compared between the two groups before and after treatment. Results A total of 10 cases withdrew from the study due to side effects or loss of follow-up, and 80 cases entered the statistical analysis, with 40 cases each. After 24 weeks of treatment, the weight, body mass index (BMI), fasting blood glucose, 2-hour postprandial blood glucose, glycated hemoglobin, triglycerides, total cholesterol (TC), low density lipoprotein-cholesterol, fasting insulin (FINS), homeostasis model assessment of insulin resistance index (HOMA-IR), interleukin-6, tumor necrosis factor-α (TNF-α) and high mobility group box-1 protein (HMGB1) levels of the two groups were lower than those before treatment, and the differences were statistically significant (P < 0.05), and there was no significant difference in the high density lipoprotein-cholesterol (HDL-C) level compared with that before treatment (P > 0.05). 71 Effect of Insulin Glargine combined with Repaglinide on hyperglycemia in patients with first-onset type 2 diabetes and its effect on islet β cell function

Objective To investigate the effect of Insulin Glargine combined with Repaglinide on hyperglycemia in patients with type 2 diabetes (T2DM). Methods Seventy-two patients with first-onset T2DM who were admitted to the First Affiliated Hospital of Hebei North University from February 2017 to December 2018 were selected and divided into combined group (Insulin Glargine + Repaglinide) and control group (premixed Insulin) according to random number table method, with 36 cases in each group. After 3 months of treatment, the blood glucose (fasting blood glucose [FBG], 2-hour postprandial blood glucose [2hPBG], glycated hemoglobin [HbA1c]), blood lipid (total cholesterol [TC], triglycerides [TG], and low density lipoprotein-cholesterol [LDL-C], high density lipoprotein-cholesterol [HDL-C]), inflammatory factors (interleukin-6 [IL-6], monocyte chemoattractant protein-1 [MCP-1], serum amyloid A [SAA]), islet β cell function indicators (fasting insulin [FINS], HOMA islet β cell function index [HOMA-β], HOMA insulin resistance index [HOMA-IR]) were compared between the two groups, and the occurrence of hypoglycemia was evaluated. Results After treatment, TC, TG, LDL-C levels decreased and HDL-C levels increased in the two groups, TC, TG, LDL-C levels in the combined group were lower than those in the control group, and HDL-C level was higher than the control group (all P < 0.05). After treatment, serum IL-6, MCP-1, SAA levels decreased in the two groups, which in the combined group were lower than those in the control group (all P < 0.05). After treatment, FBG, 2hPBG, and HbA1c levels were decreased in the two groups, which in the combined group were lower than those in the control group (all P < 0.05). After treatment, FINS and HOMA-β increased with HOMA-IR decreased in the two groups, the combined group improved better than the control group (all P < 0.05), and there was no significant difference in HOMA-IR between the two groups after treatment (P > 0.05). The incidence of hypoglycemia in the combined group was lower than that in the control group (P < 0.05), and there was no significant difference between the two groups in the rate of blood glucose reaching the standard (P > 0.05). Conclusion For patients with first-onset T2DM with hyperglycemia, Insulin Glargine combined with Repaglinide has obvious effects on blood glucose and lipids, and can effectively protect islet β-cell function, inhibit inflammation, and reduce the incidence of hypoglycemia.

75 Clinical effect of Sitagliptin combined with Insulin Aspart 30 in the treatment of obese type 2 diabetes

Objective To study clinical effect of Sitagliptin combined with Insulin Aspart 30 in the treatment of obese type 2 diabetes. Methods Eighty cases of obese type 2 diabetes patients who were admitted to the First People′s Hospital of Hefei from March 2018 to September 2019 were selected for this study. They were divided into the control group and the observation group by random number table method, with 40 cases in each group. The control group was given Insulin Aspart 30, and the observation group was treated with Sitagliptin on the basis of the control group, and both groups were treated continuously for three months. The clinical efficacy, the changes in blood lipids(total cholesterol [TC], triacylglycerol [TG], high density lipoprotein-cholesterol [HDL-C], low density lipoprotein-cholesterol [LDL-C]), blood glucose (fasting blood glucose [FBG], 2-hour postprandial blood glucose [2hPBG], glycated hemoglobin [HbA1c]), body mass index (BMI), uric acid (UA), hypersensitive C-reactive protein (hs-CRP) before and after treatment and adverse reactions were compared between the two groups. Results After treatment, the total effective rate of the observation group (95.00%) was higher than that of the control group (80.00%), the difference was statistically significant (P < 0.05). After treatment, TC, TG and LDL-C levels decreased with HDL-C levels increased in both groups compared with those before treatment, and TC, TG and LDL-C levels in the observation group after treatment were lower than those in the control group, HDL-C level was higher than that in the control group, the differences were statistically significant (P < 0.05). After treatment, FBG, 2hPBG and HbA1c levels decreased in both groups compared with those before treatment, and FBG, 2hPBG and HbA1c levels in the observation group were lower than those in the control group, the differences were statistically significant (P < 0.05). After treatment, BMI, UA, hs-CRP levels decreased in both groups compared with those before treatment, and BMI, UA, hs-CRP levels of the observation group were lower than those of the control group, the differences were statistically significant (P < 0.05). There was no significant difference in the adverse reactions between the two groups (P > 0.05). Conclusion The clinical efficacy of Sitagliptin combined with Insulin Aspart 30 in the treatment of obese type 2 diabetes is outstanding, and the patient′s blood lipid and blood glucose levels are significantly improved, which can effectively reduce the BMI, and it is worthy of clinical promotion.

79 Clinical effect of warm acupuncture combined with exercise therapy on sports knee injury in convalescent patients

Objective To observe the clinical effect of warm acupuncture combined with exercise therapy in the treatment of sports knee injury in convalescent. Methods A total of 90 patients with sports knee injuries in convalescent diagnosed in the Department of Rehabilitation, Hubei Provincial Hospital of Traditional Chinese Medicine from January to December 2018 were selected and divided into three groups according to the random number table method. Group A received conventional acupuncture treatment, group B received warm acupuncture treatment, and group C received warm acupuncture combined with exercise therapy, with 30 patients in each group. The curative effect was observed after one month. Efficacy was evaluated comprehensively by Lysholm knee score (LKS score), knee injury and osteoarthritis score (KOOS score), American knee association score (AKS score), and knee joint evaluation form of the international knee documentation committee (IKDC evaluation form). Results There was no significant difference in LKS scores between group A before and after treatment (P > 0.05). LKS scores of group B and C increased after treatment compared with those before treatment, and the differences were statistically significant (all P < 0.05). After treatment, LKS scores in group B and C were higher than those in group A, and LKS score in group C was higher than that in group B, with statistically significant differences (all P < 0.05). Compared with before treatment, KOOS scores in groups A, B and C increased after treatment, and the differences were statistically significant (all P < 0.05). After treatment, KOOS scores of groups B and C were higher than those of group A, and group C was higher than that of group B, with statistically significant differences (all P < 0.05). Compared with those before treatment, AKS scores of group A, B and C were increased after treatment, and the differences were statistically significant (all P < 0.05). After treatment, AKS scores in group C were higher than those in group A and B, with statistically significant differences (all P < 0.05). There was no significant difference in IKDC score between group A before and after treatment (P > 0.05). IKDC scores of group B and C increased after treatment compared with those before treatment, and the differences were statistically significant (all P < 0.05). After treatment, IKDC scores of group B and C were higher than those of group A, and IKDC score of group C was higher than that of group B, with statistically significant differences (all P < 0.05). Conclusion The therapeutic effect of warm acupuncture  combined with exercise therapy on sports knee injury is definite and worthy of clinical application and promotion.

84 Changes of IL-3, IL-6 and TNF-α levels before and after arthroscopic knee joint lateral support belt release and their prognostic value

Objective To analyze the changes of interleukin-3 (IL-3), IL-6 and tumor necrosis factor α (TNF-α) levels before and after arthroscopic knee joint lateral support belt release and their value in predicting the prognosis of patients. Methods Retrospective analysis was performed on the clinical data of 123 patients with knee osteoarthritis (KOA) who underwent arthroscopic knee joint lateral support belt release in the Affiliated Zhoupu Hospital of Shanghai University of Medicine & Health Sciences from August 2016 to August 2018. Serum levels of IL-3, IL-6 and TNF-α were compared before and 14 days after surgery. Patients were followed up for one year after surgery and divided into poor prognosis group (30 cases) and good prognosis group(93 cases) according to their prognosis. The preoperative general clinical data and serum levels of IL-3, IL-6 and TNF-α were compared between the two groups, and the risk factors leading to poor prognosis were analyzed by logistic regression analysis. The receiver operating characteristic curve (ROC) was drawn to analyze the predictive value of risk factors for poor prognosis. Results The levels of IL-3, IL-6, and TNF-α were higher in the 14 days after surgery than before and the differences were statistically significant (all P < 0.05). There was no significant difference in gender, body mass index, family history, course of disease, hypertension, diabetes, cartilage injury, ligament injury, free body and preoperative VAS score between the two groups (all P > 0.05). The age of the poor prognosis group was older than that of the good prognosis group, the number of affected compartments was more than that of the good prognosis group, the preoperative Kellgren-Lawrence> 2 grade ratio and preoperative serum IL-3, IL-6 and TNF-α levels were higher than those of the good prognosis group, and the differences were statistically significant (all P < 0.05). Logistic regression analysis showed that age, number of affected compartments, preoperative Kellgren-Lawrence> 2 grade, preoperative serum IL-3, IL-6 and TNF-α levels were all risk factors leading to poor prognosis (P < 0.05). ROC results indicated that the optimal cutoff value for IL-3, IL-6 and TNF-α for poor prognosis were 248.35 pg/mL, 174.61 pg/mL and 1.96 ng/mL, and the AUC was 0.752, 0.774 and 0.704, respectively. Conclusion The levels of serum IL-3, IL-6 and TNF-α increase in KOA patients after arthroscopic knee joint lateral support belt release, and the preoperative serum levels of these three factors have certain value in predicting the prognosis of the disease.

Objective To observe the effect of arthroscopic Fiber Tape circular fixation in the treatment of avulsion fracture at the inferior stop of anterior cruciate ligament (ACL). Methods A total of 80 patients with avulsion fracture at the inferior stop of ACL treated in the First Affiliated Hospital of Anhui Medical University from August 2017 to September 2019 were selected and divided into group A (39 cases, open surgical reduction and fixation) and group B (41 cases, arthroscopic Fiber Tape circular fixation) according to different surgical procedures. Perioperative indicators, knee function score, knee range of motion and complications were recorded and compared between the two groups. Results The operative time of group B was shorter than that of group A, and the intraoperative blood loss was less than that of group A, with statistically significant differences (all P < 0.05). International knee documentation committee (IKDC) and Lysholm scores of the two groups at one and six months after operation were higher than those before operation, and group B was higher than group A, with statistically significant differences (all P < 0.05). Knee range of motion of the two groups at one and six months after operation were higher than those before operation, and group B was higher than group A, with statistically significant differences (all P < 0.05). Conclusion Arthroscopic Fiber Tape circular fixation in the treatment of avulsion fracture at the inferior stop of ACL has a short operation time, less intraoperative blood loss, and can promote the recovery of the knee joint of patients, with fewer postoperative complications.

93 Analysis of the recent complications and long-term follow-up of type 31-A2 intertrochanteric fracture patients with osteoporosis treated by PFNA implantation by two postural methods

Objective To investigate the proximal femoral nail anti-rotation (PFNA) for treatment of type 31-A2 intertrochanteric fractures patients with osteoporosis by two postural approaches. Methods A total of 92 patients with type 31-A2 intertrochanteric fracture and osteoporosis admitted to the Fouth Department of Orthopedics and Traumatology, Lu′an Hospital of Traditional Chinese Medicine from December 2014 to May 2019 were selected for study, and they were divided into the experimental group and the control group according to the random number table method, with 46 patients in each group. In the experimental group, PFNA was placed in the healthy lateral position, while in the control group, PFNA was placed in the supine position. The two groups were compared for general information, surgical conditions, incidence of recent complications, length of hospital stay, hip function, and long-term follow-up. Results There was no significant difference in operation time between the two groups (P > 0.05). The experimental group had less intraoperative blood loss, postoperative drainage volume and total incidence of recent complications, and shorter the length of hospital stay than the control group, with statistically significant differences (all P < 0.05). During follow-up, three of the 46 cases in the experimental group fell off, leaving 43 cases, and four of the 46 cases in the control group fell off, leaving 42 cases. Harris function score was higher in the two groups at each time point after surgery than before surgery, and the differences were statistically significant (all P < 0.05). There was no significant difference in Harris function scores between the two groups before and 12 months after surgery (P > 0.05). Harris function scores one and six months after surgery in the experimental group were higher than those in the control group, with statistically significant differences (all P < 0.05). The weight bearing time and fracture healing time of the experimental group were shorter than those of the control group, and the incidence of fractures around internal fixation was lower than that of the control group, with statistically significant differences (all P < 0.05). There was no significant difference in the incidence of surgical failure and secondary surgery between the two groups (P > 0.05). Conclusion PFNA placement in lateral lying uninjured position for treatment of type 31-A2 intertrochanteric fracture with osteoporosis can reduce the length of hospital stay and the incidence of recent complications, restore hip function earlier, and shorten the time of underground loading and fracture healing.

98 Meta analysis on the treatment of perimenopausal syndrome in obese women with acupoint catgut embedding

Objective To conduct a meta analysis of the efficacy of acupoint embedding in the treatment of obese women with perimenopausal syndrome, and to provide a basis for clinical treatment. Methods Wanfang, VIP, CNKI, PubMed and Embase databases were searched. The retrieval time limit was the database construction to July 2019. The relevant randomized controlled trials that met the inclusion criteria were searched and analysised by RevMan 5.3. Results Five randomized controlled trials were enrolled and 344 patients were included. Results show that the effective rate of obesity was compared between the two groups, and the difference was statistically significant (OR = 14.04, 95%CI: 5.64-34.94, P < 0.000 01). The effective rate of perimenopausal syndrome was compared between the two groups, and the difference was statistically significant (OR = 8.35, 95%CI: 3.71-18.81, P < 0.000 01). Subgroup results showed that KI score of acupoint catgut embedding therapy was better than those of simple diet control and oral Chinese medicine, body mass index and waist circumference were better than those of simple diet control, oral Chinese medicine and conventional comprehensive therapy (all P < 0.05). Conclusion Acupoint catgut embedding in the treatment of obese patients with perimenopausal syndrome has a significant effect on reducing fat and has a good effect on the improvement of perimenopausal symptoms. However, limited by the quantity and quality of the included literature, large sample, multi-center, high-quality randomized controlled trials are still needed for further verification.

Objective To evaluate the effect of intracavitary electrocardiogram position on peripherally inserted central venous catheters (PICC) in neonates. Methods CNKI, Wanfang, VIP, Cochrane Library, PubMed, Web of Science databases were searched by computer. The retrieval time was from database construction to August 2019. Randomised controlled trials that met the inclusion criteria were searched, and meta analysis was performed using RevMan5.3 software. Results A total of 16 references were included in this study, including 2052 neonates meta analysis showed that the incidence of complications in the observation group was lower than that in the control group (OR = 0.37, 95%CI: 0.24-0.58, P < 0.000 01). The tip positioning accuracy of the observation group was higher than that of the control group (OR = 5.65, 95%CI: 2.72-11.77, P < 0.000 01). The one-time arrival rate in the observation group was higher than that in the control group (OR = 4.85, 95%CI: 3.20-7.36, P < 0.000 01). Conclusion Intracavitary electrocardiogram positioning method is better than body surface measurement method, which can improve the one-time arrival rate, tip positioning accuracy and reduce the incidence of complications.

107 Effect of pyeloureteroplasty combined with internal drainage in the treatment of severe hydronephrosis caused by UPJO in children

Objective To investigate the effect of pyeloureteroplasty combined with internal drainage in the treatment of severe hydronephrosis caused by UPJO in children. Methods From October 2017 to April 2019, 140 children with severe hydronephrosis who admitted to Department of Urology, Hebei Children′s Hospital were selected as study objects. According to the surgical method, they were divided into group A (72 cases) and Group B (68 cases). Group A was treated with double J tube drainage after pyeloureteroplasty, and group B was treated with ureteral stent + renal fistulas after pyeloureteroplasty. Duration of operation, extubation and hospital stay were compared between two groups, and occurrence of complications within six months after operation and recovery were compared between two groups. Results The duration of extubation and hospital stay in group A were shorter than those in group B (P < 0.05). People of postoperative complications in group A was less than that in group B (P < 0.05). There were no significant differences in operation time and the incidence of hydronephrosis between two groups (P > 0.05). Conclusion Pyeloureteroplasty combined with internal drainage in the treatment of severe hydronephrosis caused by UPJO in children can shorten the extubation and hospital stay time, more safety and have similar therapeutic effect with external drainage.

111 Effect of transvaginal ultrasonography combined with anti-Mullerian hormone on ovarian reserve function

Objective To investigate the effect of transvaginal ultrasonography combined serum anti-Miillerian hormone (AMH) on ovarian reserve function. Methods From January 2018 to June 2019, 129 infertile women who received in vitro fertilization embryo transfer treatment in the Reproductive Center of the Third Affiliated Hospital of Guangzhou Medical University were selected as study objects. According to age, they were divided into younger age group (<35 years old, 68 cases) and older age group (≥35 years old, 61 cases); and according to the number of eggs (N), they were divided into normal ovarian response group (89 cases) and low ovarian response group (40 cases). The ultrasound indexes, AMH level and N of patients in different age groups, and age, ultrasound indexes and AMH level of different ovarian response groups were observed. The correlation of age, ultrasound indexes, N and AMH among different ovarian reactive groups were observed. Results The sum of three uterine diameter lines (D) in younger age group was greater than that in older age group, AMH level was higher than that in older age group, and the number of antralfolliclecount (AFC) and N were better than those in older age group, with statistically significant differences (all P < 0.05). D and endometrial thickness (EN) in normal ovarian response group were greater than those in low ovarian response group, AMH level was higher than that in low ovarian response group, and AFC level was better than that in low ovarian response group, with statistically significant differences (all P < 0.05). Age was negatively correlated with AMH level (r = -0.265, P < 0.05). EN, AFC and N were positively correlated with AMH level (r = 0.256, 0.313, 0.799, P < 0.05). Conclusion Transvaginal ultrasonography combined with AMH have a better predictive value for ovarian reserve function, while EN and AFC combined with AMH are effective indexes.

Objective To observe the effect of midwife simulated delivery education on the selection of delivery methods, labor process and initial lactation in primiparas, to provide new education methods for primiparas. Methods From January 2017 to December 2019, 120 primiparas in Department of Obstetrics, Wanbei Coal and Electricity Company General Hospital were selected as research objects. They were divided into the control group and the observation group according to the random number table method, with 60 primiparas in each group. The control group received routine delivery education; the observation group received midwife simulated delivery education on the basis of control group. The delivery methods, labor process, initiation lactation time, Apgar score 1 min after birth, postpartum hemorrhage, and maternal total satisfaction were recorded and compared between the two groups. Results There were no significant differences in delivery method, postpartum hemorrhage between the two groups (P > 0.05). Apgar score 1 min after birth and maternal total satisfaction rate of the observation group were higher than those of the control group (all P < 0.05). The first, second and third labor process, total labor process and initiation lactation time in the observation group were shorter than those in the control group (all P < 0.05). Conclusion The midwife simulated delivery education has a higher application value for the primiparas. Compared with traditional delivery education, the labor process and initiation lactation time are shorter, which is conducive to improving nursing satisfaction.

119 Breast conserving breast cancer resection and modified radical mastectomy for premenopausal breast cancer

Objective To evaluate the effect of breast conserving breast cancer resection and modified radical mastectomy for premenopausal breast cancer. Methods From May 2016 to May 2017, 115 cases with premenopausal early breast cancer who treated in Penglai People′s Hospital were selected as study objects. They were divided into the observation group (58 cases) and the control group (57 cases) according to surgical methods. The clinical effect of the two groups were compared. Operative time, intraoperative blood loss, drainage volume, hospital stay and quality of life score were compared between the two groups. The rate of tumor recurrence and distant metastasis on six months and two years after operation were observed. Results The total effective rate in the observation group was higher than that in the control group, and the difference was statistically significant (P < 0.05). The operative time and hospital stay of the observation group were shorter than those of the control group, and the drainage volume was less than that of the control group, with statistically significant differences (all P < 0.05). There was no significant difference in intraoperative blood loss between the two groups (P > 0.05). There were no statistically significant differences in scores of physical function and physiological function between the two groups before surgery (P > 0.05). After surgery, the scores of physical function and physiological function in two groups were higher than those before surgery, and the observation group was higher than the control group, with statistically significant differences (all P < 0.05). There were no significant differences between the two groups in tumor recurrence rate and distant metastasis rate on six months and two years after surgery (P > 0.05). Conclusion Breast conserving breast cancer resection for premenopausal early breast cancer patients has a significant clinical effect. It can shorten the hospital stay and improve the quality of life of patients. It is worthy of clinical application.

Methods A total of 80 patients with gingivitis admitted to Liaoning Health Industry Group Fuxin Mine General Hospital from October 2018 to October 2019 were selected and they were divided into control group (treatment with Cetylpyridinium Chloride Gargle) and study group (treatment with Duyiwei Capsules+Cetylpyridinium Chloride Gargle) according to the random number table method, with 40 cases in each group. Both groups were continuously treated for 15 days. The clinical efficacy, gingival recovery, changes of inflammatory factors and adverse reactions were compared between the two groups. Results After treatment, the total clinical effective rate of the study group was higher than that of the control group (P < 0.05). After treatment, gingival index, sulcus bleeding index and plaque index in the two groups were lower than those before treatment, and the study group were lower than those of the control group (P < 0.05). After treatment, the levels of interleukin-1β, tumor necrosis factor-α, prostaglandin E2, soluble adhesion molecule 1 and C-reactive protein were lower in the two groups than before treatment, and the study group were lower than those of the control group (P < 0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P > 0.05). Conclusion The treatment of gingivitis patients with Duyiwei Capsules combined with Cetylpyridinium Chloride Gargle can achieve a better clinical effect. The mechanism may be improve the gingival crevicular hemorrhage and the number of dental plaque and other clinical manifestations by reducing the inflammatory response of patients, so as to play a therapeutic role, which can be widely used in clinical.

130 Effect of quadruple therapy combined with Weifuchun on serum osteopontin and gastrin in patients with chronic atrophic gastritis

Objective To explore the effect of quadruple therapy combined with Weifuchun on serum osteopontin (OPN) and gastrin (GAS) in patients with chronic atrophic gastritis. Methods A total of 65 patients with chronic atrophic gastritis treated in o Penglai People′s Hospital, Shandong Province, from March 2017 to March 2018 were divided into observation group (33 cases) and control group (32 cases) by the ordinal number table method. The control group was treated with quadruple therapy to remove Helicobacter pylori(Hp), while the observation group was treated with Weifuchun on the basis of the control group. The clinical efficacy, OPN and GAS, incidence of adverse reactions and eradication rate of Hp in the two groups were eradicated. Results The total effective rate of the observation group was significantly higher than that of the control group, with statistically significant difference (P < 0.05). There were no significant differences in OPN and GAS between the two groups before treatment (P > 0.05), after treatment, OPN in both groups was lower than that before treatment, and OPN in the observation group was lower than that in the control group, with statistically significant differences (P < 0.05). After treatment, GAS in both groups was higher than that before treatment, and GAS in the observation group was higher than that in the control group, with statistically significant differences (P < 0.05). The total incidence of adverse reactions in the observation group was lower than that in the control group, and the difference was statistically significant (P < 0.05). The eradication rate of the observation group was higher than that of the control group, with statistically significant difference (P < 0.05). Conclusion Quadruple therapy combined with Weifuchun has a significant effect on chronic atrophic gastritis. It can reduce the level of OPN in serum and increase gastrin. It is worthy of clinical application.

134 Analysis of acupoint selection rules in acupuncture treatment of anal prolapse in Chinese Medical Canon

Objective To analyze and summarize the rules of acupoint selection in the treatment of anal prolapse in ancient books. Methods The search scope was set as “text”, the works were set as “acupuncture and massage”. With “anal prolapse” as the key word in Chinese Medical Canon, the relevant acupoint items were collected, and their frequency, meridian tropism and location were sorted out, and the rules of acupoint selection were analyzed. Results A total of 195 items related to the treatment of anal prolapse were obtained, involving 30 ancient Chinese medicine books and 28 acupoints. Among the ancient Chinese medicine books, Puji Fang·Acupuncture and Zhenjiu Dacheng had the most relevant items, which had the greatest guiding significance for acupuncture treatment of anal prolapse. High frequency acupoints included bǎihuì, chángqiáng, shénquē, nèiguān, jǐzhōng and mìngmén, which were commonly used in acupuncture treatment of anal prolapse. Most of the acupoints used were Du and Ren meridians, and the head and lumbosacral part were the main parts. The most frequently used specific acupoints were chángqiáng, nèiguān and gōngsūn. In the co-occurrence analysis of selected points, “bǎihuì- chángqiáng” was a common combination of acupuncture and moxibustion in the treatment of anal prolapse without dialectics, “dàchángshù- jiānjǐng-hégǔ-qìchōng” could be used for anal prolapse caused by cold diarrhea, “mìngmén-chéngshān” was often used for anal prolapse with syndrome of deficiency complicated with excess, and “zhīgōu-zhàohǎi” could be used for anal prolapse caused by heat constipation. Conclusion Modern big data mining technology is used to sort out and analyze the related items of acupuncture and moxibustion treatment of anal prolapse in the ancient Chinese medicine books in this study, and the acupoint selection compatibility rules are analyzed, which have certain guiding significance for modern clinical application.

In the Treatise on Cold Pathogenic Diseases, prescriptions related to Chaihu mainly include Xiaochaihu Decoction, Dachaihu decoction, Chaihu Guizhi Decoction, Chaihu Guizhi Longgu Muli Decoction, Sini Powder, and so on. Taking the main syndrome as the main principle, formulating the prescription, skillfully using the matched pair of medicine, combining with external treatment, Professor Li Xia is good at applying the prescription related to Chaihu from the perspective of treatment in accordance with patient′s individuality and treatment based on syndrome differentiation, showing unique curative effect in the treatment of depression. This article intends to summarize professor Li Xia′s experience in using prescription related to Chaihu to treat depression, with a view to help the clinic.

146 Effect comparison of comprehensive auricular point therapy and acupuncture in the treatment of migraine

Objective To compare the effect of comprehensive auricular point therapy and acupuncture in the treatment of migraine. Methods From March 2018 to March 2019, 80 patients with migraine admitted to Department of Acupuncture and Moxibustion of the Affiliated Hospital of Shandong University of Traditional Chinese Medicine were selected and divided into treatment group and control group according to random number table method, with 40 cases in each group. Treatment group was treated with comprehensive auricular point therapy, while control group was treated with conventional acupuncture. Thirty days was a course of treatment. After two courses of treatment, the curative effect was evaluated and headache score was evaluated. Results The headache scores of two groups after treatment were lower than those before treatment, and treatment group was lower than that control group, the differences were statistically significant (P < 0.05). Clinical efficacy of treatment group was better than that of control group (P < 0.05). Conclusion Comprehensive auricular point therapy has certain advantages over acupuncture in treating migraine, which is worthy of active promotion.

According to her long-term clinical practice, professor Lin Yi has found that patients with breast cancer-related insomnia usually have signs of liver stagnation and spleen deficiency. She summarized its etiology and pathogenesis as liver not freely distributing as well as spleen qi deficiency. The primary syndromes of insomnia include liver depression and qi stagnation pattern, liver blood deficiency pattern and spleen deficiency pattern. The main treatment should be soothing liver, tonifying blood and invigorating spleen. In prescription and medication, professor Lin Yi attaches importance to the connection of disease differentiation and syndrome differentiation and emphasizes the molecular classification and the characteristic of staged treatment, which are divided into perioperative period, peri-chemotherapy period, peri-radiotherapy period and consolidation period. It is advocated to develop mind and medication simultaneously, coordinating with life and mental adjustment. The therapeutic effect of breast cancer-related insomnia is significant.

It is concluded that the connotation of “shaoyang as shu” lies in that shaoyang is the pivot of air conditioning, the pivot of body fluid transport and the pivot of phase fire transport. 157 Exploration on qigong suitable diseases based on modern literature

Objective To summarize the application of qigong in diseases prevention, treatments and rehabilitations, discuss the scope of suitable diseases of qigong and provide a literature basis for clinical application of qigong intervention in the future. Methods CBM, CNKI, VIP and Wanfang database from January 1949 to October 2018 were searched. According to the inclusion and exclusion criteria, the general information, research objects, intervention measures and outcome indicators were statistically analyzed. Results A total of 1707 articles were included in the study, involving 21 chapters of international classification of diseases 11th revision (ICD-11), with 486 articles involving musculoskeletal system and connective tissue diseases. The second was circulatory diseases, endocrine, nutritional or metabolic diseases, respiratory diseases, mental, behavioral or neurodevelopmental disorders, involving 247 kinds of diseases. The highest frequency was chronic obstructive pulmonary disease, a total of 143 articles, followed by cervical spondylosis, diabetes, hypertension, lumbar disc herniation, insomnia disorders. There were 142 kinds of skills involved, the highest frequency was baduanjin, 620 times mentioned, followed by yijinjing, wuqinxi and liuzijue. Conclusion Qigong is widely used in disease pre-treatment, rehabilitation and other fields. It has certain advantages in the intervention of chronic diseases such as chronic obstructive pulmonary disease, psychological and physical and mental diseases. These high-frequency diseases may be the appropriate diseases of modern qigong, which is worthy of in-depth study. In addition, the relationship between gongfa and diseases is worth further exploration, so as to provide more accurate basis for the research and clinical application of qigong dominant diseases.

161 Application effect of Qige Huayu Decoction in the treatment of gastroesophageal reflux after esophageal cancer stent implantation

Objective To investigate application effect of Qige Huayu Decoction in the treatment of gastroesophageal reflux after esophageal cancer stent implantation. Methods Forty-six patients with gastroesophageal reflux after stent implantation in Hebei Hospital of Traditional Chinese Medicine from June 2017 to August 2018 were selected and divided into two groups according to random number table method. Control group (23 cases) was given Rabeprazole Sodium Enteric-coated Capsules orally, while treatment group (23 cases) was treated with Qige Huayu Decoction. Both groups were treated for four weeks. The symptom score of gastroesophageal reflux, clinical efficacy and gastroscopy efficacy of two groups were observed. Medical outcomes study short form-36 (SF-36) was used to evaluate the quality of life in patients. Results The scores of gastroesophageal reflux symptoms in two groups after treatment were lower than those before treatment, and treatment group was lower than that in control group, the differences were statistically significant (P < 0.05). There was no significant difference in total effective rate between two groups (P > 0.05). There was no significant difference in total effective rate of gastroscopy between two groups (P > 0.05). The physical pain and mental health scores of two groups after treatment were higher than those before treatment, and treatment group was higher than that in control group, the differences were statistically significant (P < 0.05). Conclusion Qige Huayu Decoction can effectively alleviate the symptoms, improve the degree of esophageal mucosal injury and improve the quality of life in patients with gastroesophageal reflux after stent implantation of esophageal cancer. Its clinical efficacy is better than that of Western medicine alone.

Renal osteopathy is one of the common complications of uremia, the course of disease progresses slowly. Pathological fractures and calcification of blood vessels and soft tissues can occur in the later stage, which seriously affects the quality of life and life cycle of patients with uremia. Early identification of abnormal bone metabolism is the key to prevent and treat renal osteopathy. Bone turnover markers, mianly parathyroid hormone (PTH), can early judge the type of bone metabolism, guide drugs and predict the risk of long-term fractures risks. In this paper, the changes of calcium, phosphorus and PTH in a patient with tumor-like ectopic calcium deposition and pathological fracture hemodialysis during 5 years were reviewed, and to review the literature of other bone transformation markers, and discuss the application value of bone transformation markers in the diagnosis, classification and treatment of renal osteopathy.

169 Pectoralis minor muscle flap axillary sleeve surgery + total axillary lymph node dissection for the recovery of upper limb function after sentinel lymph node positive breast cancer

Objective To observe the effect of pectoralis minor muscle flap axillary sleeve surgery combined with total axillary lymph node dissection on postoperative upper limb function recovery in patients with sentinel lymph node positive breast cancer. Methods From January 2017 to December 2019, a total of 80 patients with sentinel lymph node-positive breast cancer admitted to Fuyang Tumor Hospital and the First Affiliated Hospital of Anhui Medical University were selected as the research objects, and they were divided into the observation group (implementation of pectoralis minor muscle flap axillary sleeve surgery combined with total axillary lymph node dissection) and the control group (total axillary lymph node dissection), with 40 cases in each group. The perioperative basic conditions of the two groups were counted. The wolf motor function test (WFMT) was used to evaluate the upper limb function of the two groups at one, two, and three months after the operation. The quality of life measurement scale for breast cancer patients (FACT-B) was used to assess the postoperative quality of life of the two groups, while the complications of the two groups were compared. Results The length of hospital stay in the observation group was significantly shorter than that in the control group, and the difference was statistically significant (P < 0.05). Comparing the patient time and interaction between the two groups, and the difference was statistically significant (P < 0.05). Further pairwise comparisons, intra-group comparison: the WMFT scores of the two groups at two and three months after the operation were higher than one month after the operation, and the differences were statistically significant (P < 0.05); comparison between groups: the WMFT scores of the observation group at two and three months after operation were significantly higher than those of the control group, and the differences were statistically significant (P < 0.05). The postoperative physiological status, social/family status, emotional status, functional status, additional attention scores and total scores of the observation group were significantly higher than those of the control group, and the differences were statistically significant (P < 0.05). The incidence of concurrency in the observation group was significantly lower than that in the control group, and the difference was statistically significant (P < 0.05). Conclusion Pectoralis minor muscle flap axillary sleeve surgery combined with total axillary lymph node dissection can promote the recovery of upper limb function in patients with early sentinel lymph node positive breast cancer, reduce the incidence of complications, shorten the patients′ hospital stay, and improve the quality of life of patients after the operation.

173 Discussion on the construction of research hospitals from the perspective of medical technology subject evaluation

Objective To discuss the supporting role of medical technology subject evaluation in the construction of research hospitals. Methods Through the establishment of an evaluation index system for medical technology disciplines, three consecutive discipline evaluation data of nine medical technology disciplines of a tertiary hospital were collected from 2006 to 2014, while the distribution and changes of scores were analyzed. Results There was no statistically significant difference in the total scores of the three subject assessments (P > 0.05). In the first, second, and third support condition score comparison, and the differences were highly statistically significant (P < 0.01); and the difference in the other dimensions of the score was not statistically significant (P > 0.05). The score rate of scientific research work and teaching work were lower than those of medical work, subject team, and supporting conditions, and the differences were statistically significant (P < 0.05). The cluster analysis results showed that the hospital′s medical technology disciplines could be divided into three categories. The department of radiology and laboratory were listed as the first echelon, the department of ultrasound, pharmacy, nursing and clinical medical research laboratory were the second echelon, while the department of hyperbaric oxygen, blood transfusion and pathology were the third echelon. The score distribution of each dimension of the first echelon was balanced and reasonable; the score distribution of each dimension of the second echelon was unbalanced; and the score distribution of each dimension of the third echelon showed a more serious skew distribution. Conclusion The discipline evaluation can reflect the level and trend of the development of the disciplines. Scientific research and teaching are the short board of the development of medical technology. Discipline evaluation can be used as a vane to guide the construction of research hospital.

177 Influence of the concept of accelerated rehabilitation surgery on the rehabilitation of elderly patients after total knee arthroplasty

Objective To investigate the effect of the concept of accelerated rehabilitation surgery on the rehabilitation of elderly patients with osteoarthritis after total knee arthroplasty. Methods From January 2018 to January 2020, 86 elderly patients undergoing knee arthroplasty in the First People′s Hospital of Dongcheng District, Beijing were selected. Group E and Group T were divided according to the random number table method. Group E (n = 43) was guided by the concept of accelerated rehabilitation surgery during perioperative management, while group T (n = 43) was treated with routine therapy. Pittsburgh sleep quality score (PQSI) was recorded before surgery and 24 h after surgery and nausea and vomiting score (PONV) was recorded 24 h after surgery. Fasting blood glucose levels on the second day of admission, the morning of surgery, 24 h and 48 h after surgery, visual analogue scale (VAS) at 24 h and 48 h after surgery, the Amercian knee society (AKS) before and seven days after surgery, preoperative duration of abstinence, intraoperative fluid inflow and outflow, time of first getting out of bed and length of stay were recorded. Results PQSI score 24 h after surgery in group T was higher than that before surgery, and PQSI score at 24 h after surgery in group E was lower than that in group T, with statistically significant differences (P < 0.05); Compared with group T, fasting blood glucose in group E on the morning of operation, 24 h and 48 h after operation showed statistically significant differences (P < 0.05). Comparison of fasting blood glucose in T group at each time point showed statistically significant difference (P < 0.05). There was no significant difference in fasting blood glucose and on the second day of admission and the morning of operation in group E (P > 0.05). The differences between the remaining time points were statistically significant (all P < 0.05); VAS score at 24 h and 48 h after surgery in group E were lower than those in group T, and 48 h after surgery in group T was lower than that 24 h after surgery, with statistically significant differences (all P < 0.05); AKS scores on seven days after surgery showed statistically significant differences between and within the two groups (P < 0.05 or P < 0.01). There were statistically significant differences between the two groups in the duration of abstinence, time of first getting out of bed, length of hospital stay, total amount of fluid input, blood loss and urine output (all P < 0.05 or P < 0.01). Conclusion The concept of accelerated rehabilitation surgery has a significant impact on the perioperative rapid rehabilitation of elderly patients with osteoarthritis, which can significantly improve the perioperative discomfort of patients, maintain a stable internal environment, and is conducive to postoperative rehabilitation and reduce the length of hospital stay.

Objective To understand the status quo of the medical translation abilities of nursing teachers, explore their channels to improve medical translation abilities, and provide references and evidence for better translation of nursing textbooks and bibliographies in the future. Methods From December, 2019 to January, 2020, by using the method of focus group interview, a semi-structured interview was conducted in ten nursing teachers from Shaanxi Provincial People′s Hospital and Xi′an Medical University. The interview covered the abilities, difficulties, how to overcome difficulties, how to better carry out the task and other influencing factors in the nursing teachers′ medical translation. The original data were collected and analyzed by using the seven-step analysis method of Colaizzi. Results Four themes were extracted for the medical translation abilities of nursing teachers: the components and shortcomings of the medical translation abilities of nursing teachers, the strategies adopted by nursing teachers in the process of medical translation, the better model for medical translation of nursing teachers and other factors influencing the medical translation abilities of nursing teachers. Conclusion Nursing teachers should give full play to their advantage of medical background, establish channels to compensate for or improve their English translation abilities, and emphasize the attitude towards medical translation as well as appeal for more incentives for medical translation works.

In recent years, traditional Chinese medicine tourism has gradually emerged as another important way to promote Chinese culture. Therefore, the accurate, standardized and professional translation of traditional Chinese medicine tourism texts are urgently needed. Based on the skopos theory and field research, this study aims to reveal the existing problems in the translation of traditional Chinese medicine tourism texts and propose strategies. Our findings have shown that the common mistakes in English translations of traditional Chinese medicine tourism texts mainly include misspellings, grammatical errors, inconsistent translations, mistranslations, translation pretermission, and so on. Finally it′s suggested in the paper that the English translation of traditional Chinese medicine tourism texts should be revised in line with the principles so as to improve the overall quality of text translation and promote the healthy development of traditional Chinese medicine tourism as well.

189 Study on the influence of Tibetan traditional culture on the utilization of women and children health services

Like other works, there are regional imbalances in women and children health in China, especially in Tibet. The core indicators of women′s and children′s health in Tibet are lower than the national average, which is related to the weak work foundation, poor natural environment, lack of human resources, lack of professional technology and poor service quality, as well as the influence of traditional culture. This paper introduces the characteristics of Tibetan traditional culture with religious culture as its core, and the cognition on health, disease, life and death, and also analyzes the influence of Tibetan diet culture, marriage and childbirth culture on women and children health and service utilization. Based on the characteristics of Tibetan diet culture and marriage and childbearing culture, this paper also puts forward some suggestions on how to improve the utilization rate of Tibetan women and children health services, so as to improve the situation of Tibetan women and children health.

193 Practice and exploration of Party construction in public hospitals in the new era

Public hospitals undertake the tasks of medical treatment, teaching, scientific research management, practice base construction and other personnel training tasks as well as scientific research. To strengthen the practice and exploration of party building in public hospitals in the new era is of great significance to improve the management system of hospitals and promote the medical treatment and medical personnel training in hospitals. In recent years, the Party Committee of the Second Affiliated Hospital of Guilin Medical College attaches great importance to party construction. Through the implementation of the “Party construction +” leading project, the common development of party construction and business work of the hospital has been realized in the aspects of ideology, system, technology, service, league construction, poverty alleviation and scientific research. Taking the second Affiliated Hospital of Guilin Medical College as an example, this paper introduces the experience of party construction in this hospital and explores the development path of party construction in public hospitals in the new era.
