4 The protective effect of Cangai Volatile Oil on upper respiratory tract in the mice of smoking model

Objective To investigate the safety of Cangai Volatile Oil (CAVO) by inhaled administration and the protective effect. Methods A total of 60 SPF Kunming mice with half male and half female were selected, and 20 of them were randomly divided into blank group and drug group (CAVO 2%) by random number table method, with 10 mice in each group for study the safety evaluation of CAVO. The other 40 were divided into the passive smoking model group (mode group), the CAVO high (3%), medium (2%) and low (1%) dose groups according to the random number table method, with 10 mice in each group for the study of protective effect of CAVO on upper respiratory tract. The model group were replicated in all groups except the blank group. Light 3 cigarettes in the homemade smoke room, once in the morning and once in the evening, for 20 days in total. The high, medium and low dose CAVO groups were molded and inhaled for 20 days at the same time, once in the morning and once in the evening, each lasting for 30 min. Secretory immunoglo-bulin A (SIgA) content in saliva of mice was determined by enzyme-linked immunosorbent assay on the 21st day of model administration. White blood cell count and percentage of neutrophils in mice were measured by eyeball extraction. The tissue of posterior pharyngeal wall was taken out for observation. Results There was no statistically significant difference in the white blood cell count and percentage of neutrophils, content of SIgA and ocular tissues between the drug group and the blank group (P > 0.05). Compared with the blank group, the content of SIgA in the model group was significantly decreased, and the white blood cell count and the percentage of neutrophils were significantly increased (P < 0.01), and the score of lesion degree in the pathological sections of pharyngeal tissues was increased (P < 0.01). Compared with the model group, the content of SIgA increased and the white blood cell count and percentage of neutrophils decreased in the CAVO high, medium and low dose groups (P < 0.01). The score of lesion degree in pharyngeal histopathological sections decreased in CAVO medium and high dose groups (P < 0.01). Conclusion CAVO inhalation administration can reduce the damage of the upper respiratory tract caused by passive smoking, repair the mucosal barrier of the upper respiratory tract, protect the mucosal immune function of the upper respiratory tract of mice, and has no stimulation to the upper respiratory tract of mice.

8 Application of cervical atypical gland cells in cervical cytology

Objective To investigate the application of atypical cervical gland cells (AGC) in cervical cytology. Methods From January 2012 to August 2019, a total of 34 patients with AGC were diagnosed by liquid-based thin-layer cell test in the Department of Pathology of the Second Affiliated Hospital of Nanjing Medical University, and the histological results were compared and analyzed. Results There were 7 cases (20.59%) of cervical glandular epithelial dysplasia, 2 cases (5.88%) of cervical adenocarcinoma, 4 cases (11.76%) of endometrial adenocarcinoma, and 9 cases of highly squamous intraepithelial lesions involving glands ( 26.47%), 2 cases (5.88%) of poorly differentiated squamous cell carcinoma, 7 cases (20.59%) of chronic endometrial chronic inflammation, 2 cases (5.88%) of endometrial polyps, and cervical glandular microepithelial type Hyperplasia was 1 case (2.94%). According to the type of lesion, they were divided into glandular epithelial lesion group (13 cases) and squamous epithelial lesion group (11 cases). Compared the age composition of the two groups, the difference was statically significant (P < 0.05). Conclusion In the cervical histological control of AGC, if it is a benign or precancerous lesion, most are cervical dysplasia of the glandular epithelium and a large proportion of highly squamous intraepithelial lesions that need to be identified involve the glands; if it is malignant, most are cervical adenocarcinoma or endometrial adenocarcinoma and poorly differentiated squamous cell carcinoma to be identified. In addition, different types of lesions may related to age, and over 40 years of age are the high incidence age of glandular epithelial lesions.

11 Curcumin antagonized CCl4-induced liver fibrosis in mice by activating SIRT1

Objctive To investigate the activity of silencing information regulator 1 (SIRT1) in the liver of carbon tetrachloride (CCl4) induced liver fibrosis in mice, and to investigate the effect of curcumin on liver fibrosis and its molecular mechanism. Methods Twenty-four C57BL/6 mice were randomly divided into normal group, model group and Curcumin treatment group, with 8 mice in each group. Hepatic fibrosis model was prepared by intraperitoneal injection of CCl4. After the modeling, the curcumin treatment group was given 0.5% Carboxymethyl Cellulose Sodium Curcumin Suspension by gavage. The model group and the normal group were given 0.5% Sodium Carboxymethyl Cellulose by gavage, the mice were killed after 6 weeks. HE staining was used to detect the pathological changes of mouse liver. The levels of serum alanine aminotransferase (ALT), aspartate aminotransferase (AST), interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α) and activity of SIRT1 were detected. Transforming growth factor β1 (TGF -β1), α-smooth muscle actin (α-SMA), collagen typeⅠ (Coll Ⅰ) mRNA expression, and Cleaved - caspase3, Coll Ⅰ protein expression. Results The expression levels of ALT, AST, IL-6 and TNF-α in serum of the model group were all significant higher than those of the normal group (all P < 0.01). The expression levels of ALT, AST, IL-6 and TNF-α in the treatment group were all lower than those in the model group, and the differences were highly statistically significant (all P < 0.01). The enzyme activity of SIRT1 in the liver cells of the model group was lower than that of the normal group, and the difference was highly statistically significant (P < 0.01). The enzyme activity of SIRT1 in the treatment group was significantly higher than that in the model group, and the difference was highly statistically significant (P < 0.01). Model group in the liver tissue, TGF-β1, α-SMA mRNA and Coll Ⅰ expression of amount of expression were significantly higher than those of normal group, Cleaved-caspase3 was significantly lower than that of normal group, with highly statistically significant differences (all P < 0.01). The mRNA expression of TGF-β1, α-SMA and Coll Ⅰ expression quantity in the treatment group were lower than those in the model group, Cleaved-caspase3 was significantly higher than that of model group, with highly statistically significant differences (all P < 0.01). Conclusion Curcumin has an inhibitory effect on CCl4-induced liver fibrosis, and the mechanism may be to up-regulate the activity of SIRT1, thereby reducing the inflammatory response, inhibiting the activation of hepatic stellate cells, and antagonizing liver fibrosis.

16 Protective effect of berberine on in vitro blood-brain barrier injury induced by oxygen glucose deprivation/reoxygenation in rats

Objective To investigate the protective effect of berberine on oxygen glucose deprivation/reoxygenation (OGD/R) induced blood-brain barrier injury in vitro in rats and its mechanism. Methods Primary cultured rat brain microvascular endothelial cells (rBMECs) were divided into the control group, the model group and the berberine administration group. Fresh medium was added to the control group. The model group and the berberine administration group were preincubated with berberine at different concentrations (0, 1, 3, 10, 30, 100 μmol/L) for 24 h, then the low glucose medium was replaced and the culture medium was placed in a three-gas incubator without oxygen (95%N2/5%CO2) for 2 h (oxygen glucose deprivation, OGD). then replaced with fresh medium, at 37℃ and 5%CO2 incubator continue to develop 2 h (Reoxygenation, R). Cell survival was determined by MTT assay, cell tightness was determined by transendothelial electrical resistance, protein expression levels of tight junction protein ZO-1, occludin and nuclear factor-κB p65 (NF-κB p65) were detected by Western blot assay, and levels of inflammatory factors tumor necrosis factor-α (TNF-α) and interleukin-1β (IL-1β) were detected by ELISA assay. Results Compared with the the control group, rBMECs cell survival rate, ZO-1 expression level and occludin expression level in the model group were significantly reduced, transendothelial electrical resistance showed a decreasing trend, and NF-κB p65 expression level, IL-1β, and TNF-α levels were significantly increased in the model group, the differences were highly statistically significant (all P < 0.01). Compared with the model group, the survival rate of rBMECs cells in the berberine administration group increased to different degrees, the ZO-1 expression, occludin expression, the transmembrane electrical resistance increased significantly in the berberine 10 and 30 μmol/L concentration groups, with statistically significant differences (P < 0.05 or P < 0.01). Meanwhile, the NF-κB p65 expression and the levels of IL-1β and TNF-α were significantly decreased in the berberine 10 and 30 μmol/L concentration groups, the differences were highly statistically significant (all P < 0.01). Conclusion Berberine plays a protective role of blood brain barrier on OGD/R injury by inhibiting NF-κB entry into the nucleus, reducing the release of inflammatory factors, and improving the damage of OGD/R-induced rBMECs.

21 Progress of experimental research on bone marrow suppression after chemotherapy assisted by traditional Chinese medicine adjuvant treatment

Bone marrow suppression after chemotherapy for malignant tumors in traditional Chinese medicine belongs to the categories of “blood deficiency” “consumptive disease” and “blood syndrome”. The principle for bone marrow suppression treatment of single traditional Chinese medicine and classical prescriptions is to invigorate qi and nourish blood, it can promote the recovery of hematopoietic function of bone marrow by improving peripheral blood picture, enhancing immunity, improving hematopoietic microenvironment and promoting proliferation and differentiation of hematopoietic stem cells and other ways. The traditional Chinese medicines play a significant role in the treatment of bone marrow depression, including ginseng, milkvetch root, Chinese angelica, Sichuan lovage rhizome, Siwu Decoction, Bazhen Decoction and so on. This article reviews the experimental research progress of traditional Chinese medicine in adjuvant treatment of bone marrow suppression after tumor chemotherapy in recent years, in order to provide references for the research and development of new anti-bone marrow suppression drugs of traditional Chinese medicine.

25 Research progress of animal model of necrosis of femoral head

Necrosis of the femoral head is a common disease in orthopedics. All kinds of animals have their own advantages and disadvantages in loboratory animal modeling. This article summarizes and analyzes the different modeling methods of necrosis of femoral head that has recently appeared at home and abroad. The necrosis of femoral head animal models are divided into a quadruped animal model and a biped animal model, which include rat, rabbit, dog, sheep, pig, and other quadruped animal models, as well as chicken, ostrich, and other biped animal models. In order to discuss the key issues and the direction of improvement to be considered when building animal models. It is hoped that through this review, more scientific modeling methods and more advanced assessment methods will be found, which will provide some references for the selection and establishment of animal models.

29 Advances in oxidative stress signaling pathways in osteoarthritis

Osteoarthritis is a common joint disease in orthopedics, and its prevalence is increasing with the aging of population. The pathogenesis of osteoarthritis is influenced by multiple factors, including oxidative stress and excessive production of reactive oxygen species, which regulate intracellular signal transduction, chondrocyte aging and apoptosis, extracellular matrix synthesis and degradation, synovial inflammation and subchondral bone function. Due to relatively few drugs for osteoarthritis, this paper targeting complex oxidative stress signaling pathways could provides strategies for treating osteoarthritis.

33 Review of relevant molecular mechanism of lymphatic metastasis in gallbladder cancer

Gallbladder cancer is a common clinical malignant tumor of the biliary system with a high degree of malignancy. For most patients, symptoms are not typical in early stage and the diagnosis has been in the late stage. So radical resection is always needed. However, the postoperative high recurrence rate still remains. Factors influencing the prognosis of surgery for gallbladder cancer include the depth of invasion, histological grade, lymphatic and peripheral nerve invasion and lymph node metastasis. Though lymphatic metastasis is an important factor which affects the prognosis of gallbladder cancer, the molecular mechanism is still unclear. Therefore, it is necessary to clinically analyze the mechanism of lymphatic metastasis in gallbladder cancer. This paper reviews the current situation of molecular mechanism on lymphatic metastasis of gallbladder cancer at home and abroad in order to provide reference for relevant practice and research.

37 Research progress of amino acids in the treatment of inflammatory bowel disease

Inflammatory bowel disease (IBD) is a chronic non-specific inflammatory disease of the intestinal mucosa with unknown etiology, including crohn′s disease and ulcerative colitis. The etiology and pathogenesis are not completely clear, and it is generally believed to be related to environment, heredity, infection and immunity. Amino acids are important nutrients in living organisms, which are essential substances for promoting intestinal growth, maintaining intestinal mucosal integrity and barrier function. Amino acids are also energy substrates for intestinal cells and the material basis for wound healing of intestinal mucosa. A large number of studies have confirmed the abnormal amino acid metabolism in the course of IBD disease, including glutamine, glutamate, arginine, tryptophan, cysteine, glycine, etc., and the supplementation of the above amino acids has a protective effect on the intestinal damage of IBD. The mechanism involves stimulating intestinal epithelial cell proliferation, inhibiting oxidative stress damage, reducing the release of pro-inflammatory cytokines, and increasing the migration capacity of epithelial cells. This article aims to explore the potential value of amino acids in the treatment of IBD by collecting relevant literature at home and abroad.

41 Research progress on influencing factors related to stroke recurrence

Stroke is the leading cause of death in adults in China, with and high recurrence rate. Compared to the first stroke patients, recurrent stroke patients prone to be with worse prognosis and severer disease burden. The influencing factors of stroke recurrence are various and the pathogenesis is complex. We should clarify the potential risk factors and underlying mechanisms to make a better secondary prevention for stroke patients. This paper reviews the research progress of the related factors and pathogenesis of stroke recurrence, which provides clues for the establishment of standardized prevention and control strategy system, and provides reference for more effective prevention and control of stroke recurrence.

45 Application progress of multimode CT in assessment of collateral circulation in acute ischemic stroke

Acute ischemic stroke is one of the most common causes of death in the world and the leading cause of disability. In patients with ischemic stroke, the better the collateral circulation is, the smaller the core infarction volume is, and the more ischemic penumbra can be saved. When intravenous thrombolysis and endovascular treatment are performed, the better the collateral circulation is, the smaller the risk of intracranial hemorrhage is and the better the clinical prognosis is. Multimode CT can be used to evaluate collateral circulation. Different CT angiography has different specificity and sensitivity. Reasonable use of CT angiography can quickly and effectively evaluate collateral circulation. Without delaying the time of treatment, the evaluation of collateral circulation should be perfected as far as possible, which is helpful for clinical decision-making and judgment of clinical prognosis. It is of great significance to explore the evaluation of multimode CT collateral circulation in patients with acute ischemic stroke. This paper reviews the research progress of multimode CT in evaluating collateral circulation in patients with acute ischemic stroke at home and abroad.

49 Reliability and validity of Chinese version of preparation decision making scale among patients with visual disturbance

Objective To translate and debug decision preparation scale cross the cultural, and to test the reliability and validity of the Chinese version of preparation decision making scale (PreDM) among patients with visual disturbance. Methods Based on the scale introduction principles, a Chinese version of the PreDM was formed through translation, back translation and cross-cultural debugging. The convenience sampling method was used to select a total of 105 visually impaired patients who visited the Ophthalmology Department of the First Hospital Affiliated to Army Medical University from April to October 2018 were selected by the convenience sampling method. The reliability and validity of the results were investigated. Results The Chinese version of the PreDM had a content validity of 0.95, the retest reliability of the scale was 0.634, and the Cronbach′s α coefficient was 0.965. The exploratory factor analysis extracted a total of 1 common factor and the cumulative variance contribution rate was 76.506%. Confirmatory factor analysis showed that each index was fitted well. Conclusion The Chinese version of PreDM has good reliability and validity in visually impaired patients, and can be used to evaluate decision-making readiness of visually impaired patients.

53 The mediating role of professional values and professional efficacy in the influence of professional values on professional efficacy of medical students in the cultivation of localized talents in primary medical and health institution

Objective To explore the mediating role of professional values and professional efficacy in the influence of career cognition on career intention of medical students for the local talent training in grass-roots medical and health institutions. Methods A self-made scale questionnaire “Questionnaire for the professional identification and influencing factors of cultivating clinical medical students for the local talent training in primary medical and health institutions in poor areas” was used for investigation. A total of 393 questionnaires were recovered with an effective recovery rate of 97.3%. The mediation effect of professional values and professional efficacy in the impact of professional values and professional efficacy were tested by using the mediation effect test. Results There were significant positive correlations between professional cognition, professional effectiveness, professional values, and professional identity (r > 0, P < 0.01). The analysis of the mediation effects showed that occupational cognition could not only affect professional identity, but also affected the professional identity through the indirect effects of the three paths, that was, the individual mediation effect of professional values and professional effectiveness, and the chain mediation effect of professional values and professional effectiveness. The related mediation effects were 31.37%, 33.89% and 7.45%, respectively. Conclusion The professional cognition of medical students for the local talent training can directly affect professional identity, and can also indirectly affect professional identity through professional values and professional effectiveness.

57 Effect of special training in health management literacy of grassroots medical staff and hypertension patients in Hotan region of Xinjiang Uygur Autonomous Region

Objective To explore the effect of special training mode based on independent factors on improving hypertension health management in grassroots medical staff and hypertension patients in Hotan region of Xinjiang Uygur Autonomous Region. Methods From January to August 2019, a total of 110 primary health care workers and 120 hypertensive patients from 8 primary health care units in Hotan region were selected as the research objects, while special training based on influencing factors on hypertension management was conducted for the health care workers. There were 60 patients were enrolled before training, and 60 patients were enrolled after training of 120 patients. Hypertension management ability of the workers, while the satisfaction, disease cognition and blood pressure levels of hypertension patients before and after the training were investigated by the questionnaire survey. Results The location of the unit and whether it has participated in training were influential factors for the management knowledge score of medical staff, and the education level and whether it has participated in training were the influential factors for the management behavior score of medical staff (P < 0.05). The scores of hypertension health literacy knowledge and behavior of medical staff after training were significantly higher than before training, and the differences were statistically significant (P < 0.05). The scores of disease awareness and satisfaction of trained hypertensive patients were higher than those of untrained patients, and the differences were statistically significant (P < 0.05). Conclusion The special training on hypertension based on multi-factor analysis can significantly improve the health management literacy of primary medical staff in Hotan tegion, Xinjiang Uygur Autonomous Region. At the same time, it can improve disease awareness and satisfaction of patients, which has great promotion value.

61 Research on influencing factors of elderly′s willingness to purchase intelligent monitoring equipment in different gender

Objective To analyze the factors in the willingness of the elderly to purchase intelligent monitoring equipment and the relationship between the factors, while to explore the main influencing factors in the willingness to purchase. Methods Based on planned behavior theory and technology acceptance model, a model of influencing factors of the elderly′s willingness to purchase intelligent monitoring equipment was constructed, and a total of 251 elderly people from 4 communities in a city of Guangdong Province from December 2017 to December 2018 were surveyed. Results Subjective norms, behavioral control, perceived usefulness, and perceived ease of use all had a positive impact on the willingness of older people to buy. Multi-group analysis showed that the subjective norms, perceived usefulness and perceived ease of use had a greater impact on the male group, while the behavioral control had a greater impact on the female group (P < 0.05). Conclusion The analysis of the influencing factors of the willingness of the elderly to purchase intelligent monitoring equipment based on the structural equation model can provide a reference for the further promotion of intelligent monitoring equipment in the elderly.

65 Clinical study on detection of inflammatory factors and surgical treatment of obstruction of pelvic ureteral junction

Objective To retrospectively analyze the effects of different surgical methods for treating the ureteropelvic junction obstruction (UPJO) on the detection indexes of various inflammatory factors. Methods From January 2014 to June 2019, a total of 89 patients with unilateral lesions of UPJO in Department of Urology of the First People′s Hospital of Naning were selected. Grouped according to the surgical method, retroperitoneal laparoscopy was used as the experimental group (45 cases), while open surgery was used as the control group (44 cases). Detection indexes of inflammatory factors such as C-reactive protein (CRP), procalcitonin (PCT), and white blood cell count of patients were compared between the two groups. Results There was no significant difference in CRP, PCT, and white blood cell count of patients between the two groups before surgery (P > 0.05). CRP, PCT, and white blood cell counts in the two groups after surgery were higher than those before surgery, while those in the experimental group were lower than those in the control group, and the differences were statistically significant (P < 0.05 or P < 0.01). There was no significant difference in the number of positive urine cultures of patients between the two groups after surgery (P > 0.05). Conclusion UPJO surgical trauma can cause stress inflammatory response, causing increased inflammatory factors. However, the increase of inflammatory factors caused by retroperitoneal laparoscopy is significantly lower than that of open surgery, which has advantages over open surgery. It can provide a certain reference for the selection of clinical surgery plan.

69 Meta-analysis of tourniquet in total knee arthroplasty

Objective To study the effect of balloon tourniquet in total knee arthroplasty (TKA) on operation time, total blood loss and knee range of motion 7 days after operation. Methods Chinese Biomedical Literature Database, Viper Database, CNKI, WanFang Data Knowledge Service Platform, PubMed, Cochrane Library and EMbase were searched by computer. The retrieval time was from database construction to December 2018. All the randomized controlled trials conducted during the operation were collected that the experimental group using tourniquets as the tourniquet group and none using tourniquets as the control group. Differences in operation time, total blood loss and knee range of motion 7 days after operation were compared between the two groups, and meta analysis was performed using RevMan5.3.0 software. Results A total of 14 randomized controlled studies including 924 patients were included in the study. The results of meta-analysis showed that the operation time of the tourniquet group was shorter than that of the control group, and the difference was statistically significant [WMD = -3.66, 95%CI (-5.94, -1.37), P = 0.002]. There was no statistically significant difference between the two groups in total blood loss and knee range of motion 7 days after operation (P > 0.05). Conclusion The use of tourniquet during TKA can reduce the operation time, but it has no effect on the total blood loss and knee range of motion 7 days after operation.

73 Comparison of clinical effect of minimally invasive percutaneous and transmuscular approach space with pedicle screw internal fixation in the treatment of thoracolumbar fractures

Objective To compare the clinical effect of minimally invasive percutaneous and transmuscular space approach pedicle screw internal fixation in the treatment of thoracolumbar fractures. Methods The clinical data of 100 patients with thoracolumbar fractures admitted to the First Affiliated Hospital of Shantou University Medical College from February 2017 to August 2018 were retrospectively analyzed. The selected patients were divided into minimally invasive percutaneous group (51 cases) and transmuscular space group (49 cases) according to the surgical methods. Minimally invasive percutaneous therapy was used in the minimally invasive percutaneous group, and transmuscular space therapy was used in the transmuscular space group. Functional improvement, pain, clinical efficacy, incidence of complications and imaging indexes were compared between the two groups. Results There was no statistically significant difference between the two groups in the clinical total effective rate at 6 months after surgery (P > 0.05). Compared with the minimally invasive percutaneous group, the transmuscular space group had shorter operative time and fewer intraoperative fluoroscopy times. However, the intraoperative blood loss was large and the hospital stay was long, the differences were statistically significant (all P < 0.05). There was no significant difference in incision length between the two groups (P > 0.05). The differences in time points and interaction between the two groups were statistically significant (all P < 0.05), while the differences between the two groups were not statistically significant (P > 0.05). Compared with the preoperative results, visual analogue scale (VAS) and Oswestry dysfunction index (ODI) scores of the two groups were decreased at 1 week, 3 months and 6 months after surgery, and compared with 1 week after surgery, VAS and ODI scores of the two groups decreased at 3 months and 6 months after surgery, moreover, VAS and ODI scores of the two groups at 6 months after surgery were lower than those at 3 months after surgery, the differences were statistically significant (all P < 0.05). There was no statistically significant difference in VAS and ODI scores between the two groups at 1 week, 3 months and 6 months after surgery (all P > 0.05). Compared with the preoperative results, the anterior edge height of the injured vertebral body increased and the Cobb angle of kyphosis decreased in the two groups at 1 week and 6 months after surgery, with statistically significant differences (all P < 0.05). There was no statistically significant difference in the anterior edge height of the injured vertebral body and the Cobb angle of kyphosis between the two groups at 1 week and 6 months after surgery (P > 0.05). There was no statistically significant difference in the incidence of complications between the two groups (P > 0.05). Conclusion In the treatment of thoracolumbar fracture, minimally invasive percutaneous and transmuscular space approach with pedicle screw internal fixation has the same therapeutic effect. Minimally invasive percutaneous surgery has less blood loss and shorter hospital stay, but transmuscular space surgery has shorter time and less intraoperative fluoroscopy.

78 Comparison of the effect of two different surgical methods in treatment of Rüedi-Allg?觟ver Ⅲ closed Pilon fractures with fibula fractures

Objective To compare the clinical efficacy of staging operation internal fixation and one stage operation internal fixation in treatment of Rüedi-Allg?觟ver Ⅲ closed Pilon fractures with fibula fractures. Methods From January 2014 to December 2017, the clinical data of 61 patients with Rüedi-Allg?觟ver Ⅲ closed Pilon fractures with fibula fractures hospitalized in the Department of Orthopaedics and Traumatology, National Hospital of Enshi Autonomous Prefecture were analyzed retrospectively. According to the different treatment methods, they were divided into 29 cases in the treatment group and 32 cases in the control group. In the treatment group, internal fixation was performed by stages, fibula was fixed at stage one, tibia was fixed at stage two. The control group received simultaneous surgical internal fixation, and the tibia and fibula were fixed in one operation. Patients in both groups were followed up for 12 months. Length of stay, cost, complications, fracture healing and ankle function recovery were compared between the two groups. Results At 12 months follow-up, most of the fractures in both groups were completely healed. There was no statistically significant difference in length of stay between the two groups (P > 0.05). The cost of treatment group was higher than that of the control group, the incidence of complications was lower than that of the control group, and the American orthopaedic foot and ankle society (AOFAS) score at the last follow-up was higher than that of the control group, with statistically significant differences (all P < 0.05). The differences between the two groups at time points and between the two groups were statistically significant (all P < 0.05), indicating that the Lane-Sandhu score had a trend of changing with time, and the grouping factors could play a role. There was no statistically significant difference in the interaction between the two groups (P > 0.05), indicating that the effect of time factor was not affected by the grouping factor. Compared with this group 1 month after the operation, the Lane-Sandhu score in the two groups increased at 3 months and 6 months after operation, and the Lane-Sandhu score at 6 months after operation was higher than that at 3 months after operation, the difference was statistically significant (all P < 0.05). Lane-Sandhu score in the treatment group was higher than that in the control group at 3 and 6 months after operation, with statistically significant differences (P < 0.05). Conclusion Staging operation in the treatment of Rüedi-Allgöver Ⅲ closed Pilon fractures with fibula fractures has higher safety, rapid fracture healing, greater functional recovery. It is worthy to spread in clinic.

82 A randomized controlled study of the efficiency of different preventive methods among patients with moderate and highrisk deep venous thrombosis after gynecological pelvic surgery

Objective To explore the preventive effect of different preventive methods among patients with moderate and highrisk deep venous thrombosis (DVT) after gynecological pelvic surgery. Methods From January to August 2019, prospective inclusion of 156 cases with moderate and highrisk DVT patients after gynecological pelvic surgery who admitted Beijing Luhe Hospital, Capital Medical University were selected. According to the random number table method, they were divided into IPC+GCS group (78 cases) and IPC+GCS+LMWH group (78 cases). The incidence of postoperative DVT and pulmonary embolism (PE) and the occurrence of complications were compared between two groups. Results The incidence of DVT in IPC+GCS group was significantly higher than that in IPC+GCS+LMWH group, and the difference was statistically significant (P < 0.05). There was no significant difference in the incidence of PE between two groups (P > 0.05). There was no significant difference in the incidence of DVT in moderate risk patients between two groups (P > 0.05). The incidence of DVT in high-risk patients of IPC+GCS+LMWH group was lower than that of IPC+GCS group, and the difference was statistically significant (P < 0.05). There was no significant difference in PE incidence between two groups of highrisk patients (P > 0.05). The incidence of vaginal stump bleeding in IPC+GCS+LMWH group was significantly higher than that in IPC+GCS group, and the difference was statistically significant (P < 0.05). Conclusion Both IPC+GCS and IPC+GCS+LMWH are safe and effective combined preventive measures for moderate and highrisk patients with DVT after gynecologic pelvic surgery. The combination of IPC+GCS+LMWH is more effective in preventing DVT and PE, especially for high-risk patients.

87 Meta-analysis and sequential analysis of the efficacy of vitamin B1 injection in treating hyperemesis gravidarum nèiguān point (P6)

Objective To study the efficacy of vitamin B1 injection at nèiguān point (P6) for hyperemesis gravidarum was evaluated by meta-analysis and sequential analysis. Methods PubMed, Embase, Cochrane Library, CMB, CNKI, WanFang and VIP datebases were searched. The retrieval time was from the database establishment to October 2019. The retrieval strategy was research object (P) + intervention measures (I) + research design (S), with the combination of subject words and free words. The literature was screened by 2 trained researchers according to the inclusion and exclusion criteria. The basic characteristics and publication deviation of the literature were observed and recorded. The meta-analysis and sequential analysis of the experiment were carried out with RevMan 5.3 software and Stata12.0 software. Results A total of 12 articles and 1247 cases were included. Meta-analysis showed that effective rate of hyperemesis gravidarum in the experimental group was higher than that in the control group, and the difference was highly statistically significant (OR = 9.21,95%CI:4.87-17.43,P < 0.01). The effect of injection of vitamin B1 in nèiguān point (P6) was confirmed by sequential-analysis. Conclusion Injection of vitamin B1 into nèigu?ān point (P6) has a more obvious effect on the treatment of hyperemesis gravidarum and is easy to operate. However, due to the limitation of this study, the results may be biased, so it is necessary to be cautious in clinical application.

objective To observe the clinical efficacy and safety of Phloroglucinol in the process of terminating midtrimester pregnancy in scar uterus. Methods From January 2017 to June 2019, a total of 78 cases with scar uterus  who desiring to terminate midtrimesting pregnancy were selected from the Seventh People′s Hospital of Hebei Province. They were divided into observation group and control group according to the random number table method, with 39 cases in each group. In observation group, Mifepristone combined with Misoprostol were used to induce labor, and 5% glucose 250 mL plus Resorcinol 160 mg was given by intravenous infusion when the uterine orifice was 1 cm. Control group was induced by Mifepristone combined with Misoprostol. The time of induced labor, the degree of patiens pain, the laceration of soft birth canal, the success rate of terminating pregnancy, the bleeding of induced labor, and the adverse reactions were observed and recorded. Results The time of induced labor in observation group was shorter than  that of control group , the degree of patiens pain was less severe than that of control group, the proportion of patients with soft birth canal laceration was lower than that of control group, the success rate of labor induction was higher than that of control group, and the differences were statistically significant (all P < 0.05). There was no statistically significant difference between the two groups in the bleeding of induced labor and the incidence of adverse reactions (P > 0.05). Conclusion When Mifepristone combined with Misoprostol are used to induce labor in terminating midtrimester pregnancy in scar uterus, the addition of Phloroglucinol is safe and effective, and can improve the success rate of labor induction, shorten labor induction time, reduce the pain of patients, and not increase the amount of blood loss, which is worthy of clinical application.

Objective To investigate the effect of quantitative assessment nursing of temperament dimension on high-flow oxygen inhalation in children with bronchiolitis Methods From January 2018 to February 2019, 98 children with bronchiolitis who admitted to the Children′s Hospital of Shanghai Jiaotong University were selected as subjects. They were devided into the control group and the study group according to the random number table method, with 49 children in each group. Both groups were given high-flow oxygen inhalation, and the control group was given routine care, the study group was treated with evaluation nursing based on temperament dimension. The degree of coordination, the time of improvement of clinical symptoms and signs and the length of hospital stay of the two groups were observed and the changes of temperament dimension before and after nursing were compared. Results The degree of cooperation in the study group was better than that of the control group (P < 0.05). The duration of cough, wheezing, duration of wheezing and the length of hospital stay in the study group were shorter than those in the control group (P < 0.05). There was no statistically significant difference in temperament dimension between the two groups before nursing (P > 0.05). After nursing, the activity level, adaptability, attention span and persistence, response threshold and mood scores of the study group were all higher than before nursing, and the attention dispersion, avoidance and response intensity scores were lower than those before nursing. Attention span and persistence, mood scores of the control group were higher than those before nursing, attention dispersion and response intensity scores were lower than those before nursing. Moreover, the scores of adaptability, attention span and persistence, response threshold and mood sceres in the study group were higher than those in the control group, and the scores of attention dispersion, avoidance and response intensity were lower than those in the control group (all P < 0.05). Conclusion The quantitative evaluation nursing of temperament dimension can significantly improve the adaptability of children in the process of high-throughput oxygen inhalation, reduce the children′s attention dispersion, avoidance and response intensity, promote the cooperation in the treatment of children, so as to improve the clinical treatment effect.

Objective To investigate the clinical efficacy of mild hypothermia combined with cattle encephalon glycoside and ignotin in the treatment of neonatal asphyxia brain injury and its effect on blood NO. Methods A retrospective analysis of 62 neonates with moderate and severe asphyxia brain injury who admitted to Department of Pediatrics, Huabei Petroleum General Hospital from May 2018 to October 2019. According to the different treatment methods, they were divided into mild hypothermia combined with cattle encephalon glycoside and ignotin treatment group (31 cases) and the routine treatment group (31 cases). Symptomatic supportive treatment such as maintaining electrolyte balance, lowering intracranial pressure, anti-infection and nutritional support was given to children in routine treatment group. The children in mild hypothermia combined with cattle encephalon glycoside and ignotin treatment group were treated with mild hypothermia combined with cattle encephalon glycoside and ignotin on the basis of conventional treatment. The changes of neonatal behavioral neuropathy assessment (NBNA) were compared between two groups on 7, 14 d after treatment, and the dynamic changes of blood NO were monitored. Results There was no significant difference in NBNA score between two groups before treatment (P > 0.05). NBNA scores on 7, 14 d after treatment of two groups were higher than those before treatment, and mild hypothermia combined with cattle encephalon glycoside and ignotin treatment group was higher than routine treatment group, with statistically significant differences (all P < 0.05). There was no statistically significant difference in blood NO level between two groups before treatment (P > 0.05). Blood NO level on 7, 14 d after treatment of two groups were lower than those before treatment, and mild hypothermia combined with cattle encephalon glycoside and ignotin treatment group was lower than routine treatment group, with statistically significant differences (all P < 0.05). Conclusion Mild hypothermia combined with cattle encephalon glycoside and ignotin treatment can significantly improve NBNA score. The detection of serum NO is helpful to the judgment of brain injury after asphyxia.

103 Clinical observation for point-injection at the back of internal sphere of high-dose Houttuyniae Herba in the treatment of herpes simplex keratitis

Objective To investigate clinical efficacy and safety of point-injection at the back of internal sphere of high-dose Houttuyniae Herba in the treatment of herpes simplex keratitis (HSK). Methods A prospective, randomized, controlled, single-blind study was conducted from October 2016 to September 2017 in Department of Ophthalmology, Hubei Provincial Hospital of Traditional Chinese Medicine. 120 patients (123 eyes) diagnosed with HSK were selected and divided into two groups according to the random envelope method, 60 patients (61 eyes) in control group and 60 patients (62 eyes) in experimental group. Control group was treated with Ganciclovir Eye Drops and Ganciclovir Eye Gel, while experimental group was added with point-injection of high-dose Houttuyniae Herba. All patients were treated for 15 days. Clinical efficacy, total scores of ocular symptoms and signs, the recovery of corneal sensitivity were compared, and the recurrence rate was compared with the follow-up for one year. Results After treatment, total clinical effective rate of experimental group was higher than that of control group, the difference was highly statistically significant (P < 0.01). After treatment, total score of ocular symptoms and signs in two groups was significantly lower than that before treatment (P < 0.01), and that in experimental group was significantly lower than that in control group (P < 0.01). After treatment, the corneal perception of two groups recovered significantly (P < 0.01), and corneal perception of experimental group recovered significantly (P < 0.01). The recurrence rate of HSK in experimental group was significantly lower than that in control group (P < 0.05). There was no obvious discomfort and side effects in both groups. Conclusion The double effects of Houttuyniae Herba combined with medicine and acupoint stimulation at the back of internal sphere in the treatment of HSK is significant. It can obviously improve the ocular symptoms and signs of patients, at the same time, reduce the recurrence, and the adverse reactions are slight. It is worthy of clinical application.

107 Optimization of preparation process parameters and pilot production of Yinshen Capsules

Objective To study the optimization of the technological parameters in preparation process of Yinshen Capsules and 3 batches of samples were produced. Methods Single factor method was used to screen suitable filler, lubricant and adhesive. The mixing uniformity and water content were used as the investigation indexes, the mixing time, baking temperature, baking time and other factors were optimized, and the optimal preparation technology of Yinshen Capsules was obtained, which was verified by three batches of pilot scale production. Results When 32% microcrystalline cellulose was selected as the filler, 0.9% magnesium stearate as the lubricant, and 95% ethanol as the binder, the particles had good fluidity. The total mixing time was 15 min, the baking index was 55℃ and 1.5 h, the particle flow performance of the prepared particles met the requirements of the pilot production, and the three batches of the prepared products all met the requirements. Conclusion The preparation process is simple, suitable for industrialization, the appearance of Yinshen Capsules is bright, clean and beautiful, and the quality meets the requirements.

Objective To establish the quality standard for Linzhitiaozhi Tea. Methods Thin layer chromatography (TLC)  was adopted in the qualitative identification of Ganoderma. High performance liquid chromatography was used for the content determination of chlorogenic acid. Results The spots of TLC were distinctive without negative interference. The response of chlorogenic acid was linear in the range of 25.40-253.97 μg/mL (r2 = 0.9991), the average recoverie was 100.14%, RSD was 1.6%. Conclusion This reasonable and feasible method can be used for the quality control of Linzhitiaozhi Tea.

114 Effects of Jiawei Tingli Dazao Xiefei Decoction combined with Urokinase in the treatment of encapsulated tuberculous pleurisy

Objective To observe the effect of modified Jiawei Tingli Dazao Xiefei Decoction combined with Urokinase in the treatment of wrapped tuberculous pleurisy. Methods A total of 46 patients with encapsulated tuberculous pleurisy in the Department of Infectious Diseases, Xinjiang Shache People′s Hospital, Xinjiang Uygur Autonomous Region from August 2018 to June 2019 were selected. According to the random number table method, they were divided into the treatment group (32 cases) and the control group (23 cases). The control group received chest drainage and Urokinase injection and 2HRZE/7HRE antituberculosis treatment based on a standardized antituberculosis regimen, and on this basis, the treatment group was treated with Jiawei Tingli Dazao Xiefei Decoction. The course of treatment was 1 week, followed by standard anti-tuberculosis treatment. After the course of treatment, the disappearance time of pleural effusion, pleural thickness, B-ultrasonic effusion depth, length of hospital stay and traditional Chinese medicine syndrome score of the two groups were observed. Results After treatment, the effusion disappearance time in the treatment group was shorter than that in the control group, and the difference was statistically significant (P < 0.05); the depth of effusion measured by B-ultrasound in the treatment group was significantly lower than that in the control group, and the difference was highly statistically significant (P < 0.01); The pleural thickness of the treatment group was significantly lower than that of the control group, and the difference was highly statistically significant (P < 0.01); the length of hospital stay in the treatment group was shorterer than that of the control group, and the difference was statistically significant (P < 0.05). The scores of various traditional Chinese medicine symptoms in the two groups were lower after treatment than before treatment, and the scores of fever, cough, sputum and chest pain in the treatment group were lower than those in the control group, with statistically significant differences (all P < 0.05). There was no significant difference in the scores of hemoptysis and shortness of breath between the two groups after treatment (P > 0.05). Patients of the two groups had not obvious bleeding, pain, itching and other adverse reactions. Conclusion Jiawei Tingli Dazao Xiefei Decoction can improve the absorption of pleural effusion, improve the degree of pleural thickening, shorten the length of hospital stay and improve the symptoms of traditional Chinese medicine.

118 Clinical application of Jushuang Lotion Fumigation and washing combined with Hemorrhoid Suppository in wound healing after mixed hemorrhoids operation

Objective To explore the clinical effect of Jushuang Lotion Fumigation and washing combined with Hemorrhoid Suppository in wound healing after mixed hemorrhoids operation. Methods From January 2015 to June 2019, a total of 90 patients with mixed hemorrhoids after operation admitted to the Second Affiliated Hospital of Anhui University of Chinese Medicine were selected. According to the random number table method, they were divided into the control group (n = 44, treated with musk Hemorrhoid Suppository of Mayinglong) and the research group (n = 46, treated with Jushuang Lotion + Mayinglong Suppository), with a course of 2 weeks. The wound healing time, pain degree, clinical efficacy, anal function and life quality of the two groups were compared, and the adverse reactions and postoperative complications occurred during treatment were compared. Results The total clinical effective rate of the study group was significantly higher than that of the control group (P < 0.05). After treatment, the diastolic pressure, maximum systolic pressure and resting pressure of anal canal in the two groups were higher than those before treatment, and those in the study group were higher than those in the control group (P < 0.05). The length of anorectal high-pressure band was shorter than before treatment, and the study group was shorter than the control group (P < 0.05). After treatment, the visual analogue scale (VAS) of the two groups was lower than that before treatment, and that of the study group was lower than the control group (P < 0.05). The quality of life of the two groups was higher than that before treatment, and that of the study group was higher than that of the control group (P < 0.05). The wound healing time of the study group after treatment was shorter than that of the control group (P < 0.05). There was no adverse drug reactions during treatment in the two groups. The incidence of complications in the study group was lower than that in the control group (P < 0.05). Conclusion Jushuang Lotion Fumigation and washing combined with Hemorrhoid Suppository can promote anal function recovery and wound healing, relieve pain and reduce postoperative complications in patients with mixed hemorrhoids after operation. This method can be further promoted in clinical treatment.

In recent years, the incidence of alcoholic liver disease (ALD) in China is increasing at an alarming rate, which has become one of the focuses of domestic and foreign scholars. Many studies have shown that habitual long-term drinking can cause intestinal microecological imbalance. It has been confirmed that intestinal microecological imbalance plays a central role in the occurrence and development of liver diseases. Treatment of intestinal microecological imbalance is expected to be a new treatment opportunity for ALD. “Regulating liver and spleen” therapy for ALD has been proved to be effective in relieving alcohol-induced liver injury, protecting hepatocytes and alleviating liver fibrosis in long-term clinical practice and previous studies, but its mechanism has not been fully elucidated. Therefore, from the perspective of intestinal microecology, this paper systematically expounds the modern theoretical basis of “regulating liver and spleen” in the treatment of ALD, and on the basis of previous studies, explores the effective mechanism of “regulating liver and spleen” in the treatment of ALD, with a view to find new ideas and research directions for the treatment of ALD.

In Jingui Yaolüe·On Pulse Syndrome Complex and Treatment of Blood Impediment, Zhang Zhongjing devises blood impediment into two different syndromes, light syndrome and heavy syndrome using different treatments, acupuncture and herb decoction. He points out that the main pathogenesis is ying wei qi and blood deficiency. Blood impediment is the dysfunction of limbs, but it is very different from other somato-joint diseases or deficiency diseases. Huangqi Guizhi Wuwu Decoction is a kind of Guizhi Decoction, which represents the level of medicine theory at that era and is a special prescription for blood impediment now widely used in clinical practice. Based on other physicians′ discussion and the driginal text of Jingui Yaolüe, this article discusses the pathological mechanism of blood impediment and the principle of formulating prescription of Huangqi Guizhi Wuwu Decoction, so as to provide gudance for clinic.

130 Clinical effect of Cinobufacin Injection in the treatment of malignant pleural effusion with “damp heat toxin syndrome”

Objective To explore clinical effect of Cinobufacin Injection in the treatment of malignant pleural effusion with “damp heat toxin syndrome”. Methods Thirty patients with malignant pleural effusion of “damp heat toxin syndrome” admitted to Dongfang Hospital of Beijing University of Chinese Medicine from September 2017 to March 2019 were randomly divided into treatment group and control group, with 15 cases in each group. All patients were treated with closed thoracic puncture and drainage to drain the pleural effusion. Treatment group was given Cinobufacin Injection, while control group was given IL-2 perfusion, three times a week, two weeks as a course of treatment. The quantity of pleural effusion, the quality of pleural effusion (RBC, LDH, TM), the symptom score of Chinese Medicine, KPS score and VEGF level were compared before and after treatment between two groups. Results There was no significant difference in the amount of pleural effusion between two groups (P > 0.05). The differences of RBC and TM after treatment between two groups were statistically significant (P < 0.05); the difference of LDH after treatment between two groups was not statistically significant (P > 0.05). After treatment, the symptom scores of two groups were lower than before treatment, the differences were statistically significant (P < 0.05). There was no significant difference in KPS between two groups (P > 0.05). There was no significant difference in VEGF level between two groups before and after treatment (P > 0.05). Conclusion Treatment of malignant pleural effusion with Cinobufacin Injection is related to the decrease of RBC and TM in pleural effusion, and its possible mechanism is to reduce the expression of VEGF in pleural effusion.

134 Experience of angle method acupuncture combined with muscle regions row sting acupuncture in the treatment of refractory facial paralysis

138 Effect of electroacupuncture with different retention time on trigeminal neuralgia

Objective To investigate the effect of electroacupuncture with different retention time on trigeminal neuralgia. Methods From December 2017 to March 2019, 90 patients with trigeminal neuralgia admitted to Outpatient Department of Acupuncture in the First Affiliated Hospital of Heilongjiang University of Chinese Medicine were selected as research objects. According to the random number table method, patients were divided into 10 minutes retention group (30 cases), 30 minutes retention group (30 cases) and 50 minutes retention group (30 cases). All patients were treated with electroacupuncture combined deep needling at Xiaguan (ST 7) once a day, a month for one course of treatment. The efficacy and pain indexes of three groups were compared. Results After treatment, total effective rate of 30 minutes group was higher than that of 10 minutes group and 50 minutes group the differences were statistically significant (P < 0.05). After treatment, VAS score of three groups was lower than that before treatment, and VAS score of 30 minutes group was lower than that of 10 minutes group and 50 minutes group, the differences were highly statistically significant (P < 0.01). Conclusion Therapeutic effect of electroacupuncture with 30 minutes retention on trigeminal neuralgia is better than that of 10 minutes and 50 minutes, and there is no significant difference between 10 minutes and 50 minutes electroacupuncture retention on trigeminal neuralgia.

145 Effect of traditional Chinese medicine combined with emotional therapy on patients with threatened abortion after in vitro fertilization-embryo transfer

Objective To discuss the effect of traditional Chinese medicine combined with emotional therapy on patients with early threatened abortion after in vitro fertilization-embryo transfer (IVF-ET) with kidney deficiency. Methods From October 2017 to July 2019, 90 patients with early threatened abortion and kidney deficiency treated with IVF-ET in Shenzhen Maternity & Child Healthcare Hospital Affiliated to Southern Medical University were randomly divided into control group and treatment group by random number table method, with 45 cases in each group. Control group was given luteal support and oral treatment of traditional Chinese medicine. Treatment group added emotional therapy on the basis of control group. The anxiety self-rating scale (SAS) score, Pittsburgh sleep quality index scale (PSQI) score, and traditional Chinese medicine syndrome score were compared between two groups before and after treatment. The value changes of serum human chorionic gonadotropin (β-HCG), estradiol (E2) and progesterone (P) before and after treatment, as well as clinical efficacy of two groups were compared. Results After treatment, the scores of SAS, PSQI and traditional Chinese medicine syndromes in two groups were lower than those before treatment (P < 0.05), and those in treatment group were lower than those in control group (P < 0.05). The levels of serum β-HCG and E2 in two groups after treatment were higher than those before treatment (P < 0.05), and those in treatment group were higher than those in control group (P < 0.05). There was no significant difference in P level between two groups before and after treatment (P > 0.05). Total clinical effective rate of treatment group was higher than that of control group (P < 0.05). Conclusion Traditional Chinese medicine combined with emotional therapy can alleviate the anxiety degree of patients with early threatened abortion and prevent fetal growth after IVF-ET with kidney deficiency, improve the sleep quality and the effect of embryo preservation.

153 Effect of acupuncture needle and round sharp needle on β-endorphin and prostaglandin E2 in rats with knee osteoarthritis

Objective To explore effect of acupuncture needle and round sharp needle on β-endorphin and prostaglandin E2 (PGE2) in rats with knee osteoarthritis (KOA). Methods KOA model of rats was established by injecting papain into the joint cavity. 40 male Wistar rats were randomly divided into normal group, model group, acupuncture needle group and round sharp needle group, with 10 rats in each group. Needle knife intervention was used in needle knife group, and needle knife intervention with the same diameter was used in the round needle knife group. Both interventions were treated with needle knife technique once a week for 4 weeks. Behavior of rat in each group before and after intervention was evaluated. Serum levels of β-endorphin and PGE2 were measured after sampling. Results Before intervention, behavioral scores of Lequesne MG in model group, acupuncture needle group and round sharp needle group were higher than those in normal group (P < 0.01). After intervention, behavioral scores of Lequesne MG in model group, acupuncture needle group and round sharp needle group were higher than those in normal group (P < 0.01). The Lequesne MG behavioral scores after intervention in model group, acupuncture needle group and round sharp needle group were lower than those before intervention, there were highly statistically significant differences (P < 0.01). The levels of β-endorphin in model group, needle knife group and round needling group were lower than those in normal group, levels of PGE2 in model group, needle knife group and round needling group were higher than those in normal group, and the differences were statistically significant (P < 0.05 or P < 0.01). The levels of β-endorphin in needle knife group and round needling group were higher than those in model group, levels of PGE2 in needle knife group and round needling group were lower than those in model group, and the differences were highly statistically significant (P < 0.01). Conclusion The needle-knife loosening method is more effective than round needles in improving behavioral scores, and promoting the synthesis of β-endorphin and inhibiting the synthesis of PGE2 may be one of its mechanisms.

156 Clinical effect of acupotomy in the treatment of genu valgus deformity of cerebral palsy

Objective To evaluate clinical efficacy of acupotomy in the treatment of genu valgus deformity of cerebral palsy. Methods From March 2016 to October 2019, 12 patients with genu valgus of cerebral palsy were selected from the Department of Acupotomology of Huangjiahu Hospital in Hubei University of Chinese Medicine to use acupotomology to release the adhesion, scar and contracture of the muscles, ligaments and fascia around the knee joint. Valgus angle of femoral tibial angle (FTA), range of flexion and extension of knee joint and effective rate before and after treatment were evaluated. Results FTA valgus angle of patients after treatment was significantly smaller than that before treatment, the difference was statistically significant (P < 0.05). After treatment, the range of flexion and extension of knee joint was significantly larger than that before treatment, the difference was statistically significant (P < 0.05), and total effective rate was 91%. Conclusion The principle of acupotomotor in the treatment of genu valgus is to loosen the soft tissue around the knee joint, change the FTA valgus angle, correct the deformity; to increase the range of flexion and extension of the knee joint, so as to significantly improve the function of the knee joint of the patient. The curative effect is significant, it should give priority to acupotomotor conservative treatment in clinic.

160 An analysis of 2 cases of children with spontaneous spinal epidural hematoma treated by modified single-door laminoplasty

Spontaneous spinal epidural hematoma (SSEH) is an unexplained abnormal accumulation of blood in the spinal epidural space, and the disease has a lower incidence in children. Delayed surgical decompression can lead to severe neurological impairment or even lifelong paralysis. Decompression hematoma drainage by laminectomy is the preferred treatment for SSEH. However, the application of this method in children cases is still controversial because it is highly invasive and damages the stability of posterior column. Two cases of SSEH children with initial symptoms of neck pain or limb numbness and weakness were reported. The hematoma was confirmed by MRI. After complete examination at admission, hematoma drainage with modified single open door laminoplasty was performed in the emergency department. Postoperative symptoms of neck pain disappeared. Recovery of neurological impairment. No obvious kyphosis deformity was found during 3 months of follow-up. This article aims to highlight the potential advantages of improved single-door laminoplasty in the treatment of SSEH in children.

168 Changes and clinical significance of serum miR-1-3p and H-FABP levels in patients with acute ST-segment elevation myocardial infarction

Objective To investigate the expression of serum miR-1-3p and Heart-type fatty acid binding protein (H-FABP) in patients with acute ST-segment elevation myocardial infarction (STEMI), and to analyze the diagnostic value of both to STEMI. Methods A total of 120 STEMI patients treated in the First Affiliated Hospital of Dalian Medical University (hereinafter referred to as “our hospital”) from December 2016 to June 2019 were selected as the STEMI group, and another 120 patients with normal cardiac function in our hospital during the same period were selected as the non-STEMI group. The expression of serum miR-1-3p was detected by real-time fluorescence quantitative PCR. Serum levels of H-FABP and N-terminal pro B-type natriuretic peptide (NT-proBNP) were determined by enzyme-linked immunosorbent assay. The levels of total cholesterol (TC), low density lipoprotein cholesterol (LDL-C) and triacylglycerol (TG) were measured by automatic biochemical analyzer. Left ventricular ejection fraction (LVEF) and left ventricular end-diastolic diameter (LVEDD) were measured by color ultrasound diagnostic instrument. Pearson correlation was used to analyze the correlation of serum miR-1-3p and H-FABP with lipid metabolism index and cardiac function index in STEMI patients. The diagnostic value of serum miR-1-3p, H-FABP and combined detection of STEMI was analyzed by using the receiver operating characteristic curve (ROC). Results Serum miR-1-3p expression and LVEF in the STEMI group were lower than those in the non-STEMI group, and the levels of serum H-FABP, TC, LDL-C, NT-proBNP and LVEDD in STEMI group were higher than those in the non-STEMI group, with statistically significant differences (all P < 0.05). There was no significant difference in serum TG between the two groups (P > 0.05). Serum miR-1-3p in STEMI group was negatively correlated with TC, LDL-C, NT-proBNP, LVEDD, and H-FABP (r < 0, P < 0.05), and positively correlated with LVEF (r > 0, P < 0.05). Serum H-FABP was positively correlated with TC, LDL-C, NT-proBNP and LVEDD (r > 0, P < 0.05), and negatively correlated with LVEF and miR-1-3p (r < 0, P < 0.05). There was no significant correlation between them and TG (P > 0.05). ROC analysis showed that serum miR-1-3p and H-FABP had certain diagnostic value for STEMI, and the diagnostic value of the combined detection was higher than that of the single detection [AUC=0.815, 95%CI (0.728-0.846)]. Conclusion Serum miR-1-3p was low expressed and H-FABP was high expressed in STEMI patients, both of which were correlated with lipid metabolism index and cardiac function index of the patients to some extent. The combined detection of the two could further improve the diagnostic value of STEMI.

173 Changes and clinical significance of serum carbohydrate response element binding protein in patients with diabetic nephropathy

Objective To study the changes of serum carbohydrate response element binding protein(ChREBP) level in patients with type 2 diabetes mellitus (T2DM) and its relationship with diabetic nephropathy (DN). Methods From February 2016 to December 2018, 193 cases with T2DM patients who admitted to the Second People′s Hospital and Gynecology and Obstetrics Hospital of Fuxin City of Liaoning Province (“our hospital” for short) were selected. According to the results of urinary albumin creatinine ratio (UACR), the patients whose UACR≤30 were in simple T2DM group (123 cases), and the patients with UACR>30 were in DN group (70 cases), and 50 healthy people receiving physical examination in our hospital during the same period were selected as control group. Clinical data in three group were compared. The serum ChREBP level was detected by enzyme-linked immunosorbent assay and logistic regression analysis was conducted DN influencing factors. The energy efficiency of serum ChREBP in the diagnosis of DN was analyzed by the receiver operating characteristic curve of subjects. Results The duration of diabetes in DN group was longer than that of simple T2DM group (P < 0.05). The levels of fasting blood-glucose (FBG), glycosylated hemoglobin (HbA1c), total cholesterol (TC),triglyceride (TG) and low density lipoprotein cholesterin (LDL-C) in simple T2DM group and DN group were higher than those of control group, while the level of high density lipoprotein cholesterol (HDL-C) was lower than that of control group (all P < 0.05). The 24 h urinary albumin excretion rate (24 h UAER) and ChREBP levels of DN group were higher than those of control group and simple T2DM group, and the glomerular filtration rate (eGFR) was lower than that of control group and simple T2DM group (all P < 0.05). The levels of 24 h UAER and ChREBP insimple T2DM group were higher than those in control group, and the eGFR was lower than that in control group (all P < 0.05). Duration of diabetes, 24 h UAER and ChREBP levels were independent risk factors for DN. The cut-off value was 154.36 ng/mL, the sensitivity was 0.872, and the specificity was 0.834. Conclusion Serum ChREBP level is abnormally highly expressed in DN patients, and clinical detection of serum ChREBP level can be used for early diagnosis of DN.

177 Study of alertness function resting-state magnetic resonance imaging in patients with Parkinson′s disease

Objective To investigate the function of alert network in patients with Parkinson′s disease (PD). Methods From October 2016 to April 2018, 40 PD patients diagnosed in the Nanxishan Hospital of Guangxi Zhuang Autonomous Region (“our hospital” for short) were selected as the PD group, and 45 healthy adults who passed the physical examination in our hospital at the same time were selected as the control group. Attention network test software and local consistency analysis method of resting state functional magnetic resonance imaging were used to explore the alert function and resting state changes of brain function in PD patients. Results Phase alert reaction time (RT) and intrinsic alert RT in the PD group were significantly longer than those in the control group, with statistically significant differences (P < 0.05). However, there was no significant difference in alert network efficiency between the two groups (all P > 0.05). The results of functional magnetic resonance imaging showed that the regional homogeneity values of left cingulate gyrus and right parahippocampal gyrus in the PD group were significantly lower than those in the control group, with statistically significant differences (P < 0.05). The correlation analysis found that the alert network efficiency was negatively correlated with the course of disease (r = -0.373, P < 0.05), but not with educational level (r = 0.217, P > 0.05) and intelligence (r = 0.113, P > 0.05). Conclusion PD patients have abnormal brain function and the damage of alert behavior, which is aggravated with the progress of the course of disease. This study can lay a foundation for further exploration of cognitive function damage in PD patients in the future.

181 Research and management of hypoglycemia in neonates

Neonatal hypoglycemia is one of the common complications in the early stage of newborn. The current diagnostic standard is that blood glucose < 2.2 mmol/L. However, some studies have pointed out that the more significant intervention level of blood glucose is 2.6 mmol/L. The clinical manifestations of neonatal hypoglycemia are weak response, poor sucking, excessive sleep, sweating, blood oxygen fluctuation, increased tremor, etc. Asymptomatic neonatal hypoglycemia may also cause neonatal brain damage. Persistent or recurrent neonatal hypoglycemia is more likely to lead to severe systemic reactions. Hypoglycemia can affect long-term neurodevelopment when brain damage is serious. But at the same time, there are still some problems that need to be solved in the use of the current diagnosis and treatment cards in public hospitals. This paper expounds a series of problems existing in the management practice of diagnosis and treatment card one by one, and puts forward feasible optimization suggestions, so as to offer suggestions for the in-depth implementation of the diagnosis and treatment card in public hospitals.

188 Study on the new media visual communication for traditional Chinese medicine from the cross-cultural perspective

Objective To explore the general situation of the cross-cultural communication of traditional Chinese medicine with the new media visual means, and put forward the targeted improvement strategies and to promote the cross-cultural communication of traditional Chinese medicine. Methods Content analysis was used to conduct stratified sampling and quantitative analysis of Twitter with traditional Chinese medicine as the theme and pictures and videos published on foreign social media Twitter from April to June 2019. Results From the perspective of quantity, pictures were the most widely used means of visual communication, while the application of video was relatively limited. The popularity of videos was higher than that of pictures, and the popularity of topics such as medicinal herbs, news reports, knowledge popularization and traditional Chinese medicine treatment was significantly higher than the sample mean. On the whole, the audiences of heterogeneous cultures had a positive attitude towards the recognition and acceptance of traditional Chinese medicine, while the negative attitude was mainly under the two themes of scientific nature and environmental protection. Conclusion The overall effect of traditional Chinese medicine new media visual communication is relatively ideal, which improves the international image of traditional Chinese medicine. It is suggested to cultivate practitioners′ new media literacy and cross-cultural awareness, enrich communication means, innovate communication content, regulate network public opinion, and actively publicize reports.

192 Cultivation of the connotation of nurses′ clinical thinking mode

This paper expoundeed the important significance of nurses′ clinical thinking mode, and analyzed the connotation elements of clinical thinking mode, Which included people-oriented nursing, effective communication, risk consciousness, emergency medical awareness, specialized thinking, innovation mind and so on. Moreover, this paper finds that critical thinking, systematic thinking, reverse thinking, logical thinking and evidence-based thinking are throughout the whole clinical thinking mode of nurses. This paper preliminarily constructs the overall framework of the model and analyzes how to cultivate it. The purpose of this paper is to provide theoretical support for nursing managers on clinical nurses training.
