WU Rong1 ZHU Wei1 WANG Wenjuan1 CHI Li1 JIA Chunrong2 YUE Zhujun3
Objective To observe the effect of Bushen Shengxue Decoction on bone marrow cells in the early stage of chemotherapy. Methods Sixty-five male SPF class BALB/c mice aged six to eight weeks with body weight of (20±2) g were selected and divided into blank group, model group, positive control group, high dose Bushen Shengxue Decoction group, and low dose Bushen Shengxue Decoction group according to random number table method, with 13 mice in each group. For five days of preventive administration, the high (47.0 g/kg) and low (23.5 g/kg) dose Bushen Shengxue Decoction groups were given intragastric administration, while the other groups were given intragastric administration with equal amount of distilled water. After preventive administration, Cyclophosphamide (100 mg/kg) was intraperitoneally injected into model group, positive control group, high and low dose Bushen Shengxue Decoction groups, and the same amount of normal saline was intraperitoneally injected into blank group for consecutive three days. At the same time, the positive control group was subcutaneously injected with recombinant human granulocyte colony stimulating factor (30 μg/kg), and the other groups were given the same prophylactic administration for consecutive five days. After the intervention, bone marrow cells of each group were collected, and ultrastructure of the cells was observed. The contents of tumor necrosis factor -α (TNF-α), transforming growth factor -β (TGF-β), and interferon-γ (IFN-γ) in bone marrow were detected. The mRNA expression of JAK kinase 2 (JAK2), signal transduction and transcription activator 5 (STAT5), and the protein expression of JAK2, STAT5, and phosphorylated STAT5 (P-STAT5) in bone marrow cells were detected. Results In blank group, the distribution of nucleated cells was dense, the nuclear membrane and cell membrane were intact, and the organelle structure was clear. There were few nuclear cells in the model group and a large number of cell fragments. The number of nuclear cells in the other groups increased to varying degrees, while the cell body decreased and the organelles were good. The contents of TNF-α, TGF-β, and IFN-γ in the model group were higher than those in the blank group, the contents of TNF-α, TGF-β, and IFN-γ in the high and low dose Bushen Shengxue Decoction groups were lower than those in the model group, and the contents of TGF-β and IFN-γ in the high dose Bushen Shengxue Decoction group were lower than those in the low dose Bushen Shengxue Decoction group, the differences were all statistically significant (P < 0.05 or P < 0.01). Compared with blank group, the mRNA expression of JAK2 and STAT5 in model group was down-regulated. Compared with model group, STAT5 mRNA expression in the high and low dose Bushen Shengxue Decoction groups was up-regulated. Compared with the low dose Bushen Shengxue Decoction group, the expression of JAK2 mRNA in the high dose Bushen Shengxue Decoction group was down-regulated, the differences were all statistically significant (P < 0.05 or P < 0.01). The expression levels of JAK2, P-STAT5, and STAT5 protein in the model group were lower than those in the blank group, the expression levels of JAK2 and P-STAT5 protein in the high and low dose Bushen Shengxue Decoction groups were higher than those in the model group, the expression level of P-STAT5 protein in the high dose Bushen Shengxue Decoction group was lower than that in the low dose Bushen Shengxue Decoction group, the differences were statistically significant (P < 0.05 or P < 0.01). Conclusion Bushen Shengxue Decoction can alleviate bone marrow cell damage in the early stage of chemotherapy, and the mechanism is related to the reduction of hematopoietic negative regulatory factor content and positive regulation of JAK2-STAT5 pathway.

51 Research progress on acquired immune deficiency syndrome patients with depression#br#
HUANG Mingyan1 DING Xue2 MA Xiuxia2 MENG Pengfei2 GUO Jiale1 ZHONG Pengzhan1 CAI Xiaoyang1 XU Liran2
In the course of long-term highly active anti-retroviral therapy (HAART), acquired immune deficiency syndrome (AIDS) patients can easily lead to depression due to physiological and psychological reasons. Depression can also reduce the compliance of HAART, affect the HAART effect, and the quality of life. The occurrence of depression in AIDS patients is related to general factors such as gender, economic level, and education level, psychological factors, social support, and AIDS related factors. The treatment of AIDS patients with depression must be carried out simultaneously with HAART. For mild depression, comprehensive therapy such as psychotherapy and traditional Chinese medicine can be used. Moderate and above depression should be treated with antidepressants. This paper reviews the related research progress of AIDS complicated with depression, in order to provide reference for the clinical diagnosis and treatment of AIDS complicated with depression.

55 Progress of the application of therapeutic drug monitoring of carbapenem in patients with critical illness#br#
BAI Jing LIU Jingfeng DUAN Meili
Therapeutic drug monitoring (TDM) studies the process of drugs in vivo based on pharmacokinetics (PK). It is an important means of clinical individualized drug use to achieve optimal pharmacodynamics (PD). Due to the special pathophysiological state of patients with critical illness, there are obvious differences in drug concentration between themselves and individuals during treatment, which increases the risk of insufficient treatment dose or drug toxicity. Carbapenem antimicrobial agents are widely used in anti infection treatment of patients with critical illness. Combined with PK / PD parameters, it is necessary to use TDM to guide clinical treatment, but the clinical implementation is still not standardized at present, and the effectiveness is controversial. This paper reviews the application progress of carbapenem antimicrobial agents TDM in patients with critical illness.

59 Research progress on the relationship between C-reactive protein gene polymorphism and tumorigenesis and development#br#
REN Shan1* REN Minwei2* DAI Wenbin3
The baseline level of human C-reactive protein (CRP) is most significant influenced by single nucleotide polymorphism in CRP gene and has individual differences. In recent years, epidemiology and research have found that the increased CRP level caused by genetic polymorphism in CRP gene is closely related to the increased risk of cancer and the proliferation and metastasis of tumors and poor prognosis. In addition, a number of genome-wide single nucleotide polymorphisms that are involved in the regulation of CRP levels are being identified and have similar functions. But some studies have found no causal relationship between genetically elevated CRP levels and cancer. It is believed that these differences may be related to different races, regions, sample sizes, environmental factors and genetic factors. This article reviews the research progress of CRP gene polymorphism and tumorigenesis and development, in order to provide reference for the study of the pathogenesis of CRP gene polymorphism and tumor immune-related etiology pathway.

123 Experience analysis of Professor Feng Ling in treating hyperlipidemia from “turbidity caused by stagnation”#br#
HOU Chengzhi CHEN Xingjuan HU Mu XU Yun WAN Jiajie FENG Ling
Hyperlipidemia is a common metabolic disease. Professor Feng Ling believes that the disease is mostly caused by “stagnation”. “Stagnation” of this disease is caused by the incoordination of the viscera, qi and blood. The result is that the nutrients are not smooth, zhizhuo is stagnant in the veins, and the fat is turbid. The pathological factors include qi stagnation, phlegm obstruction, and blood stasis. Turbidity caused by qi stagnation is the core pathogenesis of this disease. The main disease is the spleen, which is closely related to the liver and lung. Therefore, Professor Feng Ling proposes to treat the disease based on the basic treatment of “unblocking qi movement, dissipating stagnation and turbidity”. On the one hand, by regulating the spleen and liver, it can unblock qi movement and relieve stagnation and turbidity. On the other hand, by using the method of steam therapy and regulating lipid, it can open the sweat holes to unblock qi movement and dissipate stagnation and turbidity. Internal and external coherence, dispersing depression and stagnation, relieving turbidity and reducing lipid, the effect of clinical treatment for hyperlipidemia is good. A case study is attached at the end of the article. It is expected to provide new thoughts and methods for clinical treatment of the disease.

140 Professor Zhang Zhenyu’s experience in the treatment of knee osteoarthritis by four in one, combining the local, and multi-factor treatment#br#
SU Hong1 ZHANG Zhenyu1 FAN Su1 WAN Youhong2 LI Meijiao2 ZHANG Xi1 HU Qian1
Knee osteoarthritis is more common in clinic, the main symptoms are knee joint pain and even deformity, and movement in all directions can be restricted. Professor Zhang Zhenyu believes that the local pain and other symptoms of knee osteoarthritis patients can be relieved by manipulation. The main goal of treatment is to eliminate knee joint pain, improve walking and squatting functions, and enhance the life quality. The crouch of knee joint movement needs the parts such as knee, hip, spine, and abdomen to participate in to complete jointly. On the contrary, knee, hip, spine, abdomen, and other prats factors can cause knee dysfunction. Therefore, in the treatment of knee osteoarthritis, it is necessary to combine the local and the whole. The four in one, multi-factor treatment, local knee adjustment, overall hip adjustment, spine adjustment, abdomen adjustment, and so on, have a good effect on knee osteoarthritis.

Results ADC, TTP, and SImax of grade Ⅱ and Ⅲ groups were lower than those of grade Ⅰ group, and EER was higher than that of grade Ⅰ group; ADC, TTP, and SImax of grade Ⅲ group were lower than those of grade Ⅱ group, and EER was higher than that of grade Ⅱ group, the differences were statistically significant (P < 0.05). ADC, TTP, and SImax in medium and high expression groups were lower than those in low expression group, and EER was higher than that in low expression group; ADC, TTP, and SImax in high expression group were lower than those in medium expression group, and EER was higher than that in medium expression group, the differences were statistically significant (P < 0.05). Correlation analysis showed that ADC, TTP, and SImax were negatively correlated with Ki67 expression (r < 0, P < 0.05), and EER was positively correlated with Ki67 expression (r > 0, P < 0.05). Conclusion DWI and DCE-MRI parameters are correlated with Ki67 expression in breast cancer, which provided certain reference for selecting reasonable treatment plan and prognosis in the future.
