Study on the mechanism of Qiju Dihuang Pills in the treatment of diabetes mellitus combined with hypertension based on network pharmacology

Objective To explore the mechanism of Qiju Dihuang Pills in the treatment of diabetes mellitus combined with hypertension by network pharmacology. Methods Through traditional Chinese medicine systems pharmacology database and analysis platform (TCMSP), the main active components of Lycii Fructus, Chrysanthemi Flos, Alismatis Rhizoma, Rehmanniae Radix Praeparata, Dioscoreae Rhizoma, Poria, Corni Fructus, Moutan Cortex were searched, and the target prediction was carried out. The related targets of diabetes mellitus and hypertension were searched by GeneCards, TTD and DisGeNET databases. The targets of traditional Chinese medicine were screened by PubChem and Swiss target prediction database. The common targets were selected by intersection of disease and drug targets. The STRING platform was used to construct a protein-protein interaction network model, and the network was topologically analyzed using Cytoscape software to obtain the core target. GO-BP biofunctional enrichment analysis and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway analysis were performed on the core targets by Metascape software. Results After screening, Qiju Dihuang Pills contained 100 active compounds, 890 targets, 138 targets with diabetes mellitus and hypertension, and 39 core targets. The biological function enrichment function of GO-BP included blood circulation, peptidylserine phosphorylation, transmembrane receptor protein tyrosine kinase signaling pathway, etc. KEGG signaling pathway included VEGF signaling pathway, ErbB signaling pathway, Toll-like receptor cell signaling pathway, and so on. Conclusion This study deeply explores the complex mechanism of Qiju Dihuang Pills in the treatment of diabetes mellitus combined with hypertension with multiple components, multiple targets and multiple pathways, and provides basis and reference for further research on the basic experiments of Qiju Dihuang Pills in the treatment of diabetes mellitus combined with hypertension in the future.

Network pharmacological study on the mechanism of Prunellae Spica in the treatment of Graves’ ophthalmopathy

Objective To explore the mechanism of Prunellae Spica in the treatment of Graves’ ophthalmopathy (GO) on the basis of network pharmacology. Methods The main active components and targets of Prunellae Spica were screened by traditional Chinese medicine systems pharmacology database and analysis platform (TCMSP) database; the related targets of GO were obtained by GeneCards, OMIM and DisGeNET database; the protein-protein interaction (PPI) network was constructed by using STRING database. The KEGG signal pathway and Gene Ontology bioaccumulation were analyzed by Metascape platform, and molecular docking verification was carried out by using AutoDock Vina software. Results The main active components of Prunellae Spica in the treatment of GO including quercetin, luteolin, kaempferol, and so on. The core targets may be interleukin-6 (IL-6), tumor necrosis factor (TNF), vascular endothelial growth factor A (VEGFA), interleukin-1β; and the core pathways including TNF signal pathway, nuclear factor κB signal pathway and phosphatidylinositol 3 kinase-protein kinase B signal pathway. The results of molecular docking showed that the interaction of quercetin with VEGFA and IL-6 may play a major role in the treatment of GO by Prunellae Spica. Conclusion Prunellae Spica has an impact on immunity and inflammation through multi-components, multi-targets and multi-pathways, and plays an important role in the treatment of GO.

Systematic review and meta-analysis of Aβ and tau protein in cerebrospinal fluid in the diagnosis of Alzheimer’s disease

Objective To evaluate the accuracy of β-amyloid protein 42 (Aβ42), total tau (t-tau) and phosphorylation tau (p-tau) proteins in cerebrospinal fluid in the diagnosis of Alzheimer’s disease (AD). Methods CNKI, WanFang, VIP, China Biology Medicine disc, PubMed, Embase, and Cochrane Library were searched for diagnostic tests of cerebrospinal fluid biomarkers in the diagnosis of AD, and the search time was from January 2000 to May 2020. According to the inclusion and exclusion criteria, the literatures were screened, the quality of the included literatures was evaluated, and the data were extracted for meta analysis using STATA15 software. Results A total of ten literatures were included. The results of meta-analysis showed that the sensitivity of cerebrospinal fluid Aβ42 in the diagnosis of AD was 0.85(95%CI [0.67, 0.94]), the specificity was 0.90(95%CI [0.83, 0.94]), the area under the summary receiver operating characteristic (SROC) curve (AUC) was 0.91(95%CI [0.89, 0.94]). The sensitivity of cerebrospinal fluid t-tau in the diagnosis of AD was 0.79(95%CI [ 0.65, 0.89]), the specificity was 0.83(95%CI [0.76, 0.89]), the AUC was 0.88(95%CI [0.85, 0.90]). The sensitivity of cerebrospinal fluid p-tau in the diagnosis of AD was 0.77(95%CI [0.69, 0.84]), the specificity was 0.82(95%CI [0.65, 0.91]), the AUC was 0.84(95%CI [0.80, 0.87]). Conclusion The accuracy of Aβ42, t-tau and p-tau in cerebrospinal fluid in the diagnosis of AD is relatively high. However, due to the limitation of the number of included studies and publication bias, more high-quality studies are needed to verify the results.

Expressions and clinical significance of CXCR2 and GROα in esophageal squamous cell carcinoma and their correlation studies

Objective To investigate the expression, clinical significance and correlation of chemokine receptor 2 (CXCR2) and growth regulatory gene α (GROα) in esophageal squamous cell carcinoma. Methods Seventy cases of esophageal squamous cell carcinoma and paracancerous normal tissues were selected from the thoracic surgery department of the First Affiliated Hospital of Hebei North University from January 2017 to June 2018. The expression of CXCR2 mRNA and GROα mRNA in the two groups were detected by reverse transcription-polymerase chain reaction (qRT-PCR). Immunohistochemistry (IHC) was used to detect the expression of CXCR2 and GROα protein in the two groups. At the same time, the clinical data of the patients were collected to analyze the relationship between the expression of the two and the clinicopathological characteristics, Spearman test was used to analyze the correlation between the two proteins. Results The qRT-PCR results showed that the expression level of CXCR2 mRNA in cancer tissue was significantly higher than that in normal tissue, and the difference was highly statistically significant (P < 0.01). The expression level of GROα mRNA in cancer tissue was significantly higher than that in normal tissue, and the difference was highly statistically significant (P < 0.01). The results of IHC method showed that the positive expression rates of CXCR2 and GROα in the cancer tissue were significantly higher than those in the normal group, and the difference was highly statistically significant (P < 0.01). The expression levels of CXCR2 and GROα in the cancer tissue were correlated with the degree of differentiation, TNM stage, lymph node metastasis and liver metastasis (all P < 0.05). And not related to tumor size (P > 0.05), Spearmen correlation analysis showed that CXCR2 was positively correlated with GROα in cancer tissues (r = 0.476, P < 0.001). Conclusion Abnormal expression of CXCR2 and GROα plays an important role in the occurrence and development of esophageal squamous cell carcinoma. Combined detection of the expression levels of CXCR2 and GROα in esophageal squamous cell carcinoma can be used as a potential molecular biological indicator for early diagnosis and treatment monitoring of esophageal squamous cell carcinoma.

Expression of Hsa-miR-29c-5p in ovarian clear cell carcinoma and its effect of cell proliferation and migration

Objective To explore the expression of Hsa-miR-29c-5p in ovarian clear cell carcinoma and its effect on cell proliferation and migration. Methods The expression of Hsa-miR-29c-5p in human ovarian clear cell carcinoma cell lines (ES-2) and human normal ovarian epithelial cell lines (IOSE80) was detected by qRT-PCR. ES-2 was divided into transfection group and control group. The transfection group was transfected with Hsa-miR-29c-5p mimics by siRNA transfection, and the control group was transfected with miR-NC. Fluorescence microscopy and qRT-PCR were used to detect the expression of Hsa-miR-29c-5p in the two groups, CCK-8 method was used to detect the proliferation of the two groups, and scratch test was used to detect the migration ability of the two groups. Results The expression of Hsa-miR-29c-5p in ES-2 was significantly lower than that in IOSE80, and the difference was statistically significant (P < 0.05). Fluorescence microscopy showed uniform bright red fluorescence in two groups, and qRT-PCR showed that the expression of Hsa-miR-29c-5p in the transfection group was higher than that in the control group, with statistical significance (P < 0.05). There was no significant difference in proliferation ability between the two groups at 24, 48 h after transfection (P > 0.05). At 72, 96 h after transfection, the proliferation ability of transfection group was lower than that of control group at the same time, and the difference was highly statistically significant (P < 0.01). The results of scratch test showed that the lateral migration of ES-2 was highly inhibited after transfection of Hsa -miR-29c-5p for 48 h. The scratch area calculation showed that the scratch area of the transfection group was larger than that of the control group 48 h after transfection, and the difference was highly statistically significant (P < 0.01). Conclusion Comparing with IOSE80, the expression of Hsa-miR-29c-5p in ES-2 is down-regulated. Overexpression of Hsa-miR-29c-5p inhibits ES-2 cell proliferation and migration.

Expression of transcription factor SOX2 in non-small cell lung cancer and its significance

Objective To investigate the expression between transcription factor SOX2 in non-small cell lung cancer (NSCLC) and association clinicopathological characteristics and prognosis. Methods From January 2017 to May 2019, 74 NSCLC specimens were collected from the First Affiliated Hospital of Chengdu Medical College and confirmed by the pathology department. Immunohistochemistry was used to detect the expression of SOX2 in NSCLC tissues and paracancerous tissues, and its clinicopathological characteristics and the prognosis of patients were analyzed. Results The positive rate of SOX2 in NSCLC tissues was higher than that in paracarcinoma tissues, and the difference was highly statistically significant (P < 0.01). There were statistically significant differences in SOX2 positive rate among different histological types, lymph node metastasis and clinical stages (P < 0.05 or P < 0.01). This study was followed up from April 2017,and follow-up was conducted up to April 2020. NSCLC patients with SOX2 positive expression lived longer than that with SOX2 negative expression, and the difference was highly statistically significant (P < 0.01). SOX2 positive expression and lymph node metastasis were independent influence factors in prognosis of patients with NSCLC (RR > 1, P < 0.05). Conclusion SOX2 expression in NSCLC tissues is related to histological type and clinical stage, which can be used as a potential biomarker for diagnosis and prognosis of NSCLC.

Research progress on the correlation between nocturnal pruritus and clockwork allergy mechanism of CLOCK/BMAL1 in atopic dermatitis

Pruritus is the most prominent clinical symptom of atopic dermatitis (AD), presenting a significant circadian rhythm, with the aggravation of nocturnal pruritus. The treatment of nocturnal pruritus is an important basis for the control of AD. Biological clock is a biological rhythm with a cycle of about 24 h in physiology or behavior. At present, many studies have shown that the clock is closely related to the aggravation of nocturnal pruritus in AD. These studies suggest that clock gene can induce interleukin-33 to activate mast cells by regulating the expression of ST2 receptor, which plays an important role in regulating skin barrier function, hormone secretion level, mast cell activation and inflammatory mediators release, rather than a simple concept of time. This review aims to understand the biology of allergy related to time, and provide new ideas for the treatment of clinically refractory AD at nocturnal pruritus.

The occurrence of hyperemesis gravidarum is related to genetic,physiological,psychological factors,and Helicobacter pylori is one of its independent risk factors. Western medicines Metoclopramide,Promethazine and Ondansetron can treat vomiting,but only vitamin B6 combined with Doxeramine are recommended as a first-line drug. Traditional Chinese medicine believes that pulsed chongqi up and reverse,stomach loss and fall are the main pathogenesis of hyperemesis gravidarum,which is often caused by deficiency of stomach,liver heat,and phlegm stagnation. Traditional Chinese medicine is mainly used to regulate qi, reduce adverse and stop vomiting. In severe cases, it is advisable to nourish qi and nourish yin. Acupoint stimulation can relieve some symptoms,and acupuncture treatment provides a new green direction that requires high-grade evidence-based medicine.

Research progress in treating IgA nephropathy from blood stasis

IgA nephropathy is the most common clinical primary glomerular disease. Modern physicians usually treat this disease from the aspects of damp-heat, deficiency, and blood stasis. Among them, blood stasis occupies a pivotal position in IgA nephropathy and is one of the important excessive pathogens. It runs through the beginning and the end of the disease and is an important factor affecting the development and outcome of this disease. In this paper, the literatures related to etiology and pathogenesis of IgA nephropathy, pathogenesis of Western medicine, and the treatments of traditional Chinese medicine in four perspectives of qi and yin deficiency with blood stasis, spleen and kidney deficiency with blood stasis, intermingled of blood stasis and toxin, and kidney deficiency and blood stasis, are collected, summarized, and summarized, to analyze the application value of the treatment of blood stasis in the field of IgA nephropathy.

Effect of serum potassium level on hypertension combined with overweight/obesity in Kazakh herdsmen

Objective To study the effect of serum K+ on hypertension combined with overweight/obesity in Kazak herdsmen. Methods Based on the physical examination conducted by the Health Hospital of Urumqi County in 2018, 5327 herdsmen aged≥18 years in Nanshan pastoral area were selected by stratified random cluster sampling method for questionnaire survey and blood specimen detection. According to the presence or absence of hypertension, the influencing factors of hypertension and overweight/obesity were analyzed. Results The prevalence rate of hypertension combined with overweight/obesity in the pastoral area was 26.43%. There were significant differences in body mass index, height, weight, waist circumference, waist-to-hip ratio, systolic blood pressure, diastolic blood pressure, pulse pressure difference, serum K+, total cholesterol, triacylglycerol, high-density lipoprotein cholesterol, low-density lipoprotein cholesterol between the non-hypertension combined with overweight/obesity group and the hypertension combined with overweight and obesity group (P < 0.05). Serum K+ 4.57-5.50 mmol/L was taken as reference, the OR values of hypertension and overweight / obesity in the level of 4.11-<4.57, 3.90-<4.11 mmol/L and 3.50-<3.90 mmol/L were 1.369 (95%CI: 1.106, 1.695), 1.489 (95%CI: 1.206, 1.838) and 1.594 (95%CI: 1.301, 1.953), respectively. Conclusion Low serum K+ level of Kazak herdsmen is a risk factor for hypertension complicated with overweight/obesity.

Influencing factors analysis of the curative effect of transverse tibial bone transport on diabetic foot

Objective To analyze the influencing factors of the curative effect of transverse tibial bone transport on diabetic foot. Methods From January 2017 to December 2019, a total of 80 patients with diabetic foot who underwent tibial lateral bone transport surgery at Xintang Hospital of Zengcheng, Guangzhou City were taken as the research objects, and their clinical data were collected. According to the treatment effect, the patients were divided into 63 cases in the healing group and 17 cases in the unhealed group. The clinical data of the two groups were compared, and multivariate logistic regression was used to analyze the influencing factors of the curative effect of the treatment of diabetic foot. Results In this study, 80 patients with diabetic foot completed the tibial lateral bone transport surgery. The foot skin temperature and ankle-brachial blood pressure index were higher than those of the preoperative one week after the operation, and the visual analogue score was lower than that of the preoperative (P < 0.05). Follow-up showed that the number of ulcers healed was 63 cases. The proportions of under 60 years old, the course of the disease less than 10 years, diabetic nephropathy, the nature of the ulcer was neurogenic, the area of the ulcer less than 5 cm2, single infection, normal dorsal artery pulsation, albumin (Alb)≥35 g/L and hemoglobin (Hb)≥90 g/L of the healing group were greater than those of the unhealed group, and the differences were statistically significant (P < 0.05). The osteotomy area of 10 cm×2 cm, the moving speed of 1.0 mm/d, while the moving time of the 4th day after surgery of healing group were all smaller than those of the unhealed group, and the differences were statistically significant (P < 0.05). Multivariate logistic regression analysis showed that diabetic nephropathy (OR = 2.815, 95%CI: 1.161-6.827), nature of ulcers (OR = 2.694, 95%CI: 1.204-6.029), dorsal artery pulsation (OR = 2.754, 95%CI: 1.202-6.310), Alb (OR = 0.359, 95%CI: 0.161-0.799), Hb (OR = 0.373, 95%CI: 0.172-0.808), osteotomy area (OR = 2.762, 95%CI: 1.196-6.378), moving speed (OR = 2.651, 95%CI: 1.178-5.968) and moving time (OR = 2.740, 95%CI: 1.168-6.428) were the influencing factors of the curative effect of tibia lateral bone transport surgery in the treatment of diabetic foot (P < 0.05). Conclusion Tibia lateral bone transport surgery has good application value in the treatment of diabetic foot, but the curative effect will be affected by diabetic nephropathy, ulcer nature, dorsal artery pulsation, Alb, Hb, osteotomy area, moving speed and moving time.

Effect of Atorvastatin on myocardial fibrosis, bone mass and muscle strength in elderly patients with heart failure complicated with low bone mass

Objective To investigate the effect of Atorvastatin on myocardial fibrosis, bone mass and muscle strength in elderly patients with heart failure complicated with low bone mass. Methods A total of 100 elderly patients with heart failure complicated with low bone mass who were admitted to the Third People’s Hospital of Bengbu Affiliated to Bengbu Medical College from September 2019 to June 2020 were selected as the research subjects, and they were divided into group A and group B according to random number table method, with 50 patients in each group. Group A was treated with conventional treatment, and group B was treated with Atorvastatin on the basis of group A. Both groups were treated for three months. The changes of myocardial fibrosis, bone mass and muscle strength before and after treatment were compared between the two groups, and the incidence of adverse reactions between the two groups was counted. Results There were no significant differences in the levels of type Ⅰ procollagen (PC Ⅰ), type Ⅲ procollagen (PC Ⅲ), laminin (LN) and hyaluronic acid (HA) between the two groups before treatment (P > 0.05). After treatment, the levels of PC Ⅰ, PC Ⅲ, LN and HA in both groups were lower than those before treatment, and group B was lower than group A, the differences were statistically significant (all P < 0.05). The changes of bone mineral density of lumbar anterior position L1-4, femoral neck and hip in group B were higher than those in group A, and the differences were statistically significant (all P < 0.05). There were no significant differences in the change of left and right hand grip strength and the total incidence of adverse reactions between the two groups (P > 0.05). Conclusion Atorvastatin treatment for elderly patients with heart failure complicated with low bone mass can effectively inhibit myocardial fibrosis and promote bone formation, but it has no obvious effect on muscle strength and has good safety.

Effect of coronary microcirculation resistance on left ventricular remodeling after STEMI intervention

Objective To investigate the effect of coronary index of microcirculatory resistance (IMR) on left ventricular remodeling after ST-segment elevation myocardial infarction (STEMI) intervention. Methods A total of 83 STEMI patients admitted to Suzhou Jiulong Hospital, Shanghai Jiao Tong University School of Medicine from July 2018 to December 2019 were enrolled and all underwent direct percutaneous coronary stent implantation (PCI) after admission. IMR after PCI treatment was measured intraoperatively, and the patients were divided into high IMR group (38 cases, IMR≥25 U) and normal IMR group (45 cases, IMR<25 U). All patients were routinely followed up with color ultrasound six months after surgery, and the changes of heart function and the condition of left ventricular remodeling were compared between the two groups, and the relationship between IMR and left ventricular remodeling and its predictive value were evaluated. Results Compared with the hospital, six months after surgery, LVEF was decreased and LVEDV was increased in the high IMR group, and the differences were statistically significant (all P < 0.05). Six months after surgery, the left ventricular ejection fraction (LVEF) of the high IMR group was lower than that of the normal IMR group, and the left ventricular end-diastolic volume (LVEDV) and the incidence of left ventricular remodeling were higher than that of the normal IMR group, and the differences were statistically significant (all P < 0.05). Pearson correlation analysis showed that IMR was negatively correlated with LVEF (r = -0.357) and positively correlated with LVEDV (r = 0.608) (all P < 0.05). Multiple logistic regression analysis showed that IMR was an risk factor for left ventricular remodeling after intervention in patients with STEMI (OR = 1.87, 95%CI: 1.07-3.25, P < 0.05). Receiver operating characteristic curve indicated that the area under curve of IMR predicting left ventricular remodeling six months after intervention in STEMI patients was 0.84 (95%CI: 0.76-0.93), the best diagnostic threshold was 32.64 U, and the sensitivity and specificity were 69% and 78%, respectively. Conclusion IMR affects cardiac function and left ventricular volume after STEMI intervention, and is an risk factor for left ventricular remodeling in STEMI.

Effect of Xintongtai Granules in treatment of stable angina pectoris with syndrome of qi stagnation and blood stasis and influence on myocardial energy metabolism

Objective To investigate the clinical effect of Xingtongtai Granules in the treatment of stable angina pectoris with syndrome of qi stagnation and blood stasis and influence on myocardial energy metabolism of patients. Methods A total of 60 patients with stable angina pectoris with syndrome of qi stagnation and blood stasis treated in the outpatient department and inpatient department of the First Hospital Hu’nan University of Chinese Medicine and the Second Affiliated Hospital of Hu’nan University of Chinese Medicine from June 2018 to June 2020 were selected and they were divided into observation group and control group according to random number table method, with 30 patients in each group. The observation group was treated with Xintongtai Granules combined with Western medicine standard treatment, while the control group was treated with Trimetazidine Hydrochloride combined with Western medicine standard treatment. After four weeks of treatment, the clinical efficacy of the two groups was evaluated, and myocardial energy expenditure level and serum free fatty acids content were detected before and after treatment. Results The total effective rate of angina pectoris and electrocardiogram in the observation group was higher than that in the control group, and the differences were statistically significant (all P < 0.05). The stopping rate of observation group was higher than control group, and the difference was statistically significant (P < 0.05). After treatment, myocardial energy expenditure level and serum free fatty acids content in both groups were lower than those before treatment, and observation group was lower than control group, the differences were statistically significant (all P < 0.05). Conclusion Xintongtai Granules combined with Western medicine standard treatment has a significant clinical effect in the treatment of stable angina pectoris with syndrome of qi stagnation and blood stasis, the mechanism of which may be related to the decrease of myocardial energy expenditure level and serum free fatty acids content.

Relationship between TG / HDL-C ratio and the risk of coronary heart disease and its diagnostic value

Objective To investigate the relationship between triglyceride / high density lipoprotein cholesterol (TG / HDL-C) ratio and the risk of coronary heart disease (CHD) and its diagnostic value. Methods One hundred patients with CHD (CHD group) and 100 patients without CHD (non-CHD group) admitted to the People’s Hospital of Liaoning Province from June 2017 to June 2019 were selected as the research objects. The differences of TG / HDL-C ratio and general data between the two groups were compared. The risk of CHD induced by TG / HDL-C ratio was analyzed by multivariate logistic regression. Receiver operating characteristic (ROC) curve was used to evaluate its diagnostic value. Results The levels of TG / HDL-C ratio and TG and the percentages of history of hypertension and family history of CHD in CHD group were significantly higher than those in non-CHD group, while the level of HDL-C was significantly lower than that in non-CHD group, the differences were statistically significant (all P < 0.05). Multivariate logistic regression analysis showed that TG (OR = 1.021, P < 0.001), TG/HDL-C ratio (OR = 4.231, P = 0.006), history of hypertension (OR = 5.724, P < 0.001) and family history of CHD (OR = 3.614, P < 0.001) were all independent risk factors for CHD, while HDL-C (OR = 0.196, P = 0.010) were protective factors. ROC curve analysis showed that among TG, HDL-C and TG/HDL-C ratios, the area under curve (AUC) of TG/HDL-C ratio was the largest (AUC=0.859, P < 0.05). Conclusion The TG / HDL-C ratio of CHD patients is significantly higher than that of non-CHD patients, which has higher diagnostic value for CHD and is an independent risk factor for the occurrence of CHD.

Study on correlation of serum trimethylamine oxide and coronary heart disease

Objective To study the correl ation of serum trimethylamine oxide (TMAO) and the occurrence of coronary heart disease (CHD). Methods A total of 98 patients with CHD hospitalized in the Department of Cardiovascular Medicine, the Second People’s Hospital of Lianyungang Affiliated to Bengbu Medical College (hereinafter referred to “our hospital”) from July 2019 to June 2020 were selected as the CHD group and 104 healthy volunteers at the same period in our hospital were selected as the control group. Serum TMAO in the two groups were detected by enzymatic linked reaction adsorption assay. The general data and serum TMAO level of the two groups were compared. Multivariate logistic regression was used to analyze the risk factors of CHD, and Spearman correlation coefficient was used to analyze the correlation between serum TMAO level and the severity of CHD. Results The number of male, body mass index (BMI), smoking proportion, serum TMAO, blood glucose, hyperlipidemia and hypertension proportion in the CHD group were significantly higher than those in the control group, with statistical significance (all P < 0.05). Multivariate logistic regression analysis showed that high TMAO level, high BMI, high blood glucose level, complicated with hyperlipidemia, smoking history, complicated with hypertension and high uric acid level were all risk factors for CHD (OR > 1, P < 0.05). Spearman correlation coefficient showed that TMAO was positively correlated with CHD severity (rs = 0.443, P < 0.05). Conclusion The serum TMAO level is higher in patients with CHD, which has a certain correlation with the occurrence of CHD.

Application of multidisciplinary collaborative follow-up model in patients with atrial fibrillation in the blank period after radiofrequency ablation

Objective To explore the application of multidisciplinary collaborative follow-up model in patients with atrial fibrillation in the blank period after radiofrequency ablation. Methods A total of 120 patients with radiofrequency ablation of atrial fibrillation for the first time admitted to the Cardiovascular Center of Beijing Tongren Hospital, Capital Medical University from January 2018 to May 2020 were enrolled. According to the random number table method, they were divided into two groups, the control group (60 cases) received routine follow-up model, and the experimental group (60 cases) received multidisciplinary collaborative follow-up model. Both groups were followed up at 2 weeks, 1 month, 2 months and 3 months after discharge. The differences in all dimensions scores of atrial fibrillation effect on quality-of-life (AFEQT) scale between the two groups on the day of discharge and three months after follow-up and other outcome indicators (smoking cessation rate, alcohol abstinence rate, drug omission rate, atrial fibrillation recurrence rate, body mass index difference of overweight patients, readmission rate, bleeding event rate, self-evaluation of health) were compared. Results After three months of follow-up, the differences in all dimensions scores of AFEQT scale, abstinence rate and body mass index difference of overweight patients in experimental group were higher than those in control group, drug omission rate was lower than that in control group, and the self-evaluation of health was better than that in control group, with statistical significance (all P < 0.05). There were no significant differences in smoking cessation rate, atrial fibrillation recurrence rate, readmission rate and bleeding event rate between the two groups (P > 0.05). Conclusion Multidisciplinary collaborative follow-up model is effective in alcohol consumption, weight control, medication management and self-evaluation of health, which can improve the quality of life of patients with atrial fibrillation in the postoperative blank period, and is worthy of promotion and application in clinical nursing work.

The role and clinical significance of peripheral blood T lymphocyte subsets in patients with aplastic anemia

Objective To explore the level of T lymphocyte subsets in peripheral blood of patients with aplastic anemia (AA) and its clinical significance. Methods A total of 70 AA patients treated at Huaibei People’s Hospital from January 2017 to April 2020 were selected as the observation group, of which 30 patients were severe AA (severe AA group) and 40 patients were non-severe AA (non-severe AA group), and 40 healthy people were selected as the control group. After receiving treatment in the observation group, 45 cases were relieved (remission group), (including significant improvement, remission and basic cure), and 25 cases were not remission (non-remission group). The levels of natural killer (NK) cells, regulatory T (Treg) cells, CD4+, CD3+, CD8+, CD4+/CD8+, and Treg+/CD4+ were detected by flow cytometry. Results In the observation group, the celluar levels of NK, Treg, CD4+, CD4+/CD8+, and Treg+/CD4+ were all lower than those in the control group, and CD3+ and CD8+ were higher than those in the control group (P < 0.01). The celluar level of Treg, CD4+, CD4+/CD8+, and Treg+/CD4+ in the severe AA group were lower than those in the non-severe AA group (P < 0.05). The Treg cells and CD4+/CD8+ in the non-remission group were lower than those in the remission group (P < 0.05). In patients with AA, the areas under the curve of Treg cells and CD4+/CD8+ predicting AA disease remission were 0.71 and 0.75, the area under the curve for the combination of the two was 0.82, the specificity was 67.5%, and the sensitivity was 85.1%. Conclusion Patients with AA have abnormal distribution of T cell subsets. Treg combined with CD4+/CD8+ have certain value in assessing the condition and treatment effect.

Relationship between PON1 Q192R gene polymorphism and recurrence of cerebral infarction patients treated with Clopidogrel

Objective To study the relationship between PON1 gene polymorphism and recurrence of cerebral infarction patients treated with Clopidogrel. Methods A total of 205 patients with cerebral infarction who received Clopidogrel treatment in Hebei Petrochina Central Hospital from October 2012 to March 2019 were selected, according to the inclusion and exclusion criteria, 169 patients were finally included as the study subjects. Situ hybridization was used to detect PON1 Q192R and CYP2C19 genes. Patients without gene mutation were included in QQ group (61 cases), and patients with gene mutation were included in QR+RR group (108 cases). The relative risk was calculated and the correlation between PON1 Q192R gene and recurrence of cerebral infarction patients was analyzed. Results There were no significant differences in the recurrence rate of cerebral infarction at 90 d and 1 year between two groups (P > 0.05). Under the same CYP2C19 genotype background, there were no significant differences in the recurrence rate of cerebral infarction in 90 d and 1 year between two groups (P > 0.05). The relative risk of recurrence of cerebral infarction in QR+RR group was 1.2, 1.3 at 90 d and 1 year, respectively. PON1 Q192R gene was weakly correlated with the reate of recurrence of cerebral infarction. Conclusion PON1 Q192R gene polymorphism may be a genetic susceptibility factor for recurrence of cerebral infarction patients at 90 d and 1 year.

Relationship between serum lipoprotein-related phospholipase A2 and angiotensin 1-7 levels and vascular dementia in elderly patients with acute cerebral infarction

Objective To investigate the relationship between serum lipoprotein-related phospholipase A2 (Lp-PLA2) and angiotensin 1-7 (Ang1-7) levels and vascular dementia (VaD) in elderly patients with acute cerebral infarction (ACI). Methods A total of 122 ACI patients admitted to Chongqing University Central Hospital from May 2018 to May 2020 were selected as study objects. They were divided into ACI-VaD group (54 cases) and ACI-NVaD group (68 cases) according to whether VaD was combined; according to clinical dementia rating scale, ACI-VaD group was divided into mild dementia group (13 cases), moderate dementia group (16 cases), and severe dementia group (25 cases). The serum Lp-PLA2 and Ang1-7 levels in each group were compared, the multivariate logistic regression analysis was used to analyze the influencing factors of VaD in elderly patients with ACI, and the ROC curve was used to analyze the predictive value of serum Lp-PLA2 and Ang1-7 levels in ACI complicated with VaD. Results There were statistically significant differences in serum total cholesterol, high density lipoprotein cholesterol, high-sensitivity C-reactive protein (hs-CRP), Lp-PLA2 and Ang1-7 between two groups (P < 0.05). The level of serum Lp-PLA2 in severe dementia group was higher than that in moderate dementia group and mild dementia group, and the level of serum Ang1-7 was lower than that in moderate dementia group and mild dementia group; the level of serum Lp-PLA2 in moderate dementia group was higher than that in mild dementia group, and the level of serum Ang1-7 in moderate dementia group was lower than that in mild dementia group, with statistical significance (all P < 0.05). Hs-CRP and Lp-PLA2 were independent risk factors for VaD in elderly patients with ACI (OR > 1, P < 0.05), Ang1-7 was an independent protective factor (OR < 1,P < 0.05). The area under the curve, sensitivity, specificity and accuracy of Lp-PLA2+Ang1-7 predicting elderly patients with ACI complicated with VaD were greater than those of Lp-PLA2 and Ang1-7 alone. Conclusion Serum Lp-PLA2 level is significantly increased and Ang1-7 level is significantly decreased in elderly ACI patients complicated with VAD, which are independent influencing factors for VAD, and combined detection could enhance the predictive value of VAD.

Clinical comparative analysis of patients with early onset severe preeclampsia with or without chronic hypertension expectant treatment

Objective To analyze the differences of expected treatment time, influencing factors and maternal and infant outcomes between patients with chronic hypertension complicated with early onset severe preeclampsia (CHEOSP) and patients with early onset severe preeclampsia (EOSP). Methods Four hundred and ninety-three cases of singleton EOSP patients admitted to Maternal and Child Health Hospital of Anhui Medical University from January 2018 to January 2020 were selected as research objects. They were divided into CHEOSP group (group A) with 98 cases and EOSP group (group B) with 395 cases according to whether complicated with chronic hypertension. The general situation, expected treatment time, influencing factors and maternal and infant outcomes of two groups were compared. Results The proportion of multipara, systolic blood pressure and diastolic blood pressure in group A were higher than those in group B, and serum uric acid in group A was lower than that in group B, the differences were statistically significant (P < 0.05). Gestational age less than 28 weeks, the gestational age and delivery gestational age of group A were shorter than those of group B, and expected treatment time of group A was longer than that of group B, the differences were statistically significant (P < 0.05). Gestational age from 28 to 32 weeks, expected treatment time and delivery gestational age of group A were longer than those of group B, the differences were statistically significant (P < 0.05). Gestational age from 32 to 33+6 weeks, the gestational age of group A was shorter than that of group B, and expected treatment time and delivery gestational age of group A were longer than those of group B (P < 0.05). The incidence rate of hypoproteinemia, hydrothorax and ascites in group A were lower than those in group B, and the incidence rate of fetal growth restriction and fetal distress in group A were higher than those in group B (P < 0.05). Pearson product moment correlation showed that expected treatment time of group A and B was positively correlated with plasma albumin level (r = 0.800, 0.780, P < 0.01) and negatively correlated with 24 hours urinary protein level (r = -0.398, -0.433, P < 0.01). Conclusion Patients with CHEOSP have longer expected treatment time than those with EOSP alone, there is no serious complication on the basis of the disease itself. However, in the course of expected treatment, we should pay attention to the level of plasma albumin and the occurrence of complications such as pleural effusion and ascites. It may cause the termination of pregnancy.

Effect of Simo Decoction combined with blue light irradiation in the treatment of neonatal jaundice and its influence on immune function

Objective To explore the effect of Simo Decoction combined with blue light irradiation in the treatment of neonatal jaundice and its influence on immune function. Methods From January 2017 to January 2020, 100 newborns with pathological jaundice in Taihe Hospital of Traditional Chinese Medicine of Anhui Province were divided into control group of 50 cases and study group of 50 cases according to random number table method. Control group was treated with blue light irradiation, and study group was treated with Simo Decoction on the basis of control group. Both groups were treated for five days. Clinical efficacy, jaundice index level, jaundice regression time, serum transferrin (TRF), C-reactive protein (CRP) and peripheral blood CD4+, CD8+, CD4+/CD8+ levels were compared between two groups. The incidence of adverse reactions was observed in two groups. Results Total effective rate of study group was higher than that of control group, the difference was statistically significant (P < 0.05). After treatment, the jaundice index levels of two groups were lower than those before treatment, and study group was lower than that control group, the differences were statistically significant (P < 0.05). The jaundice regression time of study group was shorter than that of control group, and the difference was statistically significant (P < 0.05). After treatment, serum TRF level of two groups was higher than that before treatment, CRP level was lower than that before treatment, and serum TRF level of study group was higher than that of control group, CRP level of study group was lower than that of control group, the differences were statistically significant (P < 0.05). After treatment, CD4+, CD4+/CD8+ in two groups were higher than those before treatment, and study group was higher than that control group, the differences were statistically significant (P < 0.05). During the treatment, no serious adverse reactions occurred in two groups. Conclusion Clinical effect of Simo Decoction combined with blue light irradiation in the treatment of neonatal jaundice is significant, which can significantly promote jaundice regression, improve the immune function of the body, and has good safety.

Polymorphism of TYMS gene of patients with colorectal cancer associated with Lynch syndrome

Objective To study the polymorphism of thymidylate synthase (TYMS) gene in patients with colorectal cancer associated with Lynch syndrome in Beijing area, and to analyze the relationship between TYMS gene polymorphism and the effect of 5-Fluorouracil chemotherapy by literature. Methods A total of 102 cases of colorectal cancer tissues were collected from patients who underwent surgery in four hospitals in Beijing from April 2015 to December 2016, including 61 males and 41 females. Immunohistochemistry and gene sequence detection methods were used to screen out Lynch syndrome patients, and then the polymorphisms of TYMS genes in these patients were identified by gene sequencing. Results Among 102 patients with colorectal cancer, there were 10 patients with Lynch syndrome. Among them, TYMS gene polymorphisms of 2R/2R and 3RC/3RC were four patients, while 2R/3RG, 3RC/3RG and 3RG/3RG were six patients. Among the high-expressing group, there were four males and two females; among the low-expressing group, there were one male and three females. Conclusion Most of the TYMS gene polymorphisms in patients with Lynch syndrome and colorectal cancer in Beijing area are 2R/3RG, 3RC/3RG or 3RG/3RG, which are related to the high expression of this gene, and the efficacy of Fluorouracil drugs is not good. The TYMS gene polymorphisms in patients with Lynch syndrome and colorectal cancer in Beijing area are 2R/3RG or 3RG/3RG, which are related to the high expression of this gene, and the efficacy of Fluorouracil drugs is not good.

Expression and clinical significance of miR-410 and CCNB1 in ovarian cancer tissues

Objective To investigate the expression levels of microRNA-410 (miR-410) and cyclin B1 (CCNB1) in ovarian cancer, and to analyze their relationship with clinical pathological data. Methods Quantitative real-time PCR (qRT-PCR) was used to detect the relative expression of miR-410 and CCNB1 mRNA in 83 cases of ovarian cancer and adjacent tissues in General Hospital of the Northern Theater of the Chinese People’s Liberation Army (from January 2017 to February 2020), the relationship between the two indicators and clinical pathological data was analyzed. Results The relative expression of miR-410 in ovarian cancer was lower than in adjacent tissues, and the relative expression of CCNB1 mRNA was higher than in adjacent tissues (P < 0.05). The relative expression of miR-410 in ovarian cancer tissues in FIGO stage Ⅲ-Ⅳ, tumor invasion depth T3+T4, low-medium cell differentiation, and lymph node metastasis was lower than those of tumor FIGO stage Ⅰ-Ⅱ, tumor invasion depth T1+T2, patients with high cell differentiation and had no lymph node metastasis, and the relative expression of CCNB1 in the tumor FIGO stage Ⅲ-Ⅳ, the tumor infiltration depth was T3+T4, the cell differentiation was low to medium, and the lymph node metastasis was higher than those of the tumor FIGO stage Ⅰ-Ⅱ, the depth of tumor invasion was T1+ T2, and the cells were highly differentiated and had no lymph node metastasis (P < 0.05). Correlation analysis showed that there was a negative correlation between miR-410 and CCNB1 expression in ovarian cancer (r = -0.605, P < 0.05). Conclusion The expression level of miR-410 in ovarian cancer is lower than that in adjacent tissues, and the expression level of CCNB1 mRNA is higher than in adjacent tissues, which is closely related to the tumor FIGO stage, tumor invasion depth, cell differentiation degree and lymphatic metastasis. It is expected to become an biomarkers for ovarian cancer diagnosis, treatment and prognosis.

Expression of CTC, HER2 and PI3KCA in metastatic breast cancer

Objective To explore the expression of circulating tumor cell (CTC), human epidermal growth factor receptor 2 (HER2), phosphatidylinositol-3-kinase catalytic subunit α (PI3KCA) in metastatic breast cancer. Methods A total of 50 cases of metastatic breast cancer patients in Nantong Hospital of Traditional Chinese Medicine (“our hospital” for short) from April 2018 to June 2019 were selected as the case group, and 50 cases of healthy women in our hospital during the same period were selected as the normal group. The levels of CTC, HER2, PI3KCA in CF-DNA and serum levels of carcinoembryonic antigen (CEA) and carbohydrate antigen 15-3 (CA15-3) were measured and compared between the two groups. Kappa test was used to analyze the consistency of CTC, HER2 and PI3KCA content in CF-DNA in diagnosis of metastatic breast cancer and pathological diagnosis. Pearson linear model was used to analyze the correlation between CTC, HER2, PI3KCA content in CF-DNA and serum CEA, CA15-3 levels. Results The levels of CTC, HER2 and PI3KCA content in CF-DNA in the case group were higher than those in the normal group (P < 0.05). The sensitivity of the three combined diagnosis of metastatic breast cancer was 86.00%, the specificity was 90.00%, the accuracy rate was 88.00%, the positive predictive value was 89.58%, the negative predictive value was 86.54%, and the consistency with the pathological diagnosis was 0.760 (highly consistent). The serum CEA and CA15-3 levels in the case group were significantly higher than those in the normal group (P < 0.05). Pearson analysis showed that the number of CTC, the amount of HER2 protein and the content of PI3KCA short fragment in CF-DNA were positively correlated with CEA and CA15-3 in serum(r = 0.752, 0.801; 0.691, 0.702; 0.628, 0.611, all P < 0.05). Conclusion The contents of CTC, HER2 and PI3KCA in CF-DNA in patients with metastatic breast cancer are correlated with serum CEA and CA15-3 levels, and have high diagnostic value for metastatic breast cancer, and have good consistency with pathological diagnosis.

Correlation between self-perceived burden and coping style in patients with gastric cancer after endoscopic mucosal resection

Objective To observe the self-perceived burden of patients with gastric cancer after endoscopic mucosal resection, to analyze the correlation between self-perceived burden and coping style. Methods From June 2014 to June 2017, 100 patients with gastric cancer who underwent endoscopic mucosal resection electively for treatment in Hu’nan Brain Hospital of Hu’nan Province were selected. Before operation, three months, six months, one year and three years after operation, the self-perceived burden scale (SPBS) was used to evaluate the self-perceived burden of patients with gastric cancer, the coping style of patients was evaluated by the cancer coping modes questionnaire (CCMQ), the correlation between self-perceived burden and coping style in patients with gastric cancer was analyzed. Results The scores and total scores of all dimensions of SPBS in gastric cancer patients three and six months after operation compared with those before operation, the differences were not statistically significant (P > 0.05). The scores and total scores of all dimensions of SPBS one and three years after operation were decreased, and the scores and total scores of all dimensions of SPBS 3 years after operation were lower than those one year after surgery (P < 0.05). The CCMQ face scores of gastric cancer patients three and six months after operation were lower than that before operation (P < 0.05), and the vent score was lower than that before operation (P > 0.05). The scores of face and vent one and three years after operation were higher than those before and three, Six months after operation (P < 0.05). The scores of avoidance, stress, fantasy and yield in gastric cancer patients three and six months after operation were higher than those before surgery, and one and three years after operation were lower than those before and three, six months after operation(P < 0.05). Using bivariate straight and Pearson correlation analysis, found that SPBS score of each dimension score (economic burden, emotional burden, body) and total score were negatively correlated with CCMQ scale in the face, vent score of patients with gastric cancer(r < 0, P < 0.05), positively correlated with and CCMQ scale avoidance and yield stress, fantasy, and yield scores (r > 0, P < 0.05). Conclusion The self-perceived burden of patients with gastric cancer after endoscopic mucosal resection is closely related to coping style, and there is a negative correlation between them, clinical intervention can be carried out according to this, in order to improve the patient’s coping style or self-perceived burden.

Application of multidisciplinary collaborative intervention based on mobile APP platform in patients after modified radical mastectomy

Objective To explore the application effect of multidisciplinary collaborative intervention based on mobile APP platform in patients after modified radical mastectomy, to observe the postoperative recovery of patients, to guide the formulation of effective nursing program after modified radical mastectomy in the future. Methods From May 2018 to June 2020, 240 patients with breast cancer who received modified radical mastectomy in the Renmin Hospital of Wuhan University were selected, according to the random number table method, they were divided into control group and observation group, with 120 cases in each group. The control group was given conventional multidisciplinary collaborative intervention, and the observation group was given multidisciplinary collaborative intervention based on mobile APP platform, both were intervened for three months. Before intervention and three months after intervention, self-rating anxiety scale (SAS) and self-rating depression scale (SDS) were used to evaluate the psychological state of the two groups, and exercise self-care agency (ESCA) was used to evaluate the self-care ability. The incidence of complications was recorded. Results Before intervention, there were no significant differences in SAS and SDS scores between the two groups (P > 0.05). After three months of intervention, SAS and SDS scores in both groups were lower than before treatment, and observation group was lower than the control group, the differences were statistically significant (P < 0.05). Before intervention, there was no significant difference between the two groups in the scores and total scores of ESCA scale (P > 0.05). After three months of intervention, the each dimensions scores and total scores of ESCA scale in the two groups were higher than before treatment, and those in the observation group were higher than the control group, with statistical significance (P < 0.05). The incidence of complications in the observation group was lower than that in the control group, with statistical significance (P < 0.05). Conclusion The multidisciplinary collaborative intervention based on mobile APP platform is more conducive to improve the mental state of patients with breast cancer after modified radical mastectomy, improve their self-care ability, and can effectively reduce the incidence of complications.

Clinical application value of interpersonal and social rhythm therapy and cognitive behavior therapy in patients with depressive disorder

Objective To study the clinical efficacy of interpersonal and social rhythm therapy (IPSRT) and cognitive behavior therapy (CBT) in patients with depressive disorder. Methods From March 2016 to March 2019, 100 patients with depressive disorder in the Third People’s Hospital of Fuyang, Anhui Province were included as research objects. They were divided into observation group (50 cases) and control group (50 cases) according to the random number table method. Two groups were received drug treatment, and observation group received IPSRT and CBT intervention on the basis of drug treatment. The Hamilton depression scale 17 (HAMD17), Hamilton anxiety scale (HAMA) and simplified coping style questionnaire (SCSQ) scores before and after treatment between two groups were compared. Results After treatment, HAMD17 and HAMA scores in two groups were lower than those before treatment, and observation group was lower than control group, and the differences were statistically significant (P < 0.05). After treatment, in the SCSQ score, the positive coping score was higher than that before treatment, while the negative coping score was lower than that before treatment; and the positive coping score of observation group was higher than that of control group, while the negative coping score was lower than that of control group, and the differences were statistically significant (P < 0.05). Conclusion IPSRT and CBT can improve the negative emotion of patients with depression.

Application of blood GM test combined with bronchoscope alveolar lavage fluid in the diagnosis of pulmonary aspergillosis

Objective To investigate the value of serum and alveolar lavage fluid galactomannan (GM) test in the diagnosis of pulmonary aspergillosis. Methods From June 2015 to June 2019, 90 patients with suspected pulmonary aspergillosis treated in Jianhu Hospital Affiliated to Nantong University were selected as research subjects. Patients received a combination of serum and alveolar lavage fluid GM therapy. GM values of serum and alveolar lavage fluid of different patients were compared according to clinical diagnosis, and the ROC curve was used to analyze the diagnostic value of GM values of serum and alveolar lavage fluid in diagnosing pulmonary aspergillosis. Results Among 90 patients suspected of pulmonary aspergillosis, pulmonary aspergillosis was confirmed in 39 cases, pulmonary aspergillosis was suspected in 21 cases and non-pulmonary aspergillosis in 30 cases. The GM values of serum and alveolar lavage fluid in confirmed and suspected pulmonary aspergillus patients were higher than those in non-aspergillus patients (P < 0.05). The area under curve of ROC of GM value of serum and alveolar lavage fluid in diagnosing pulmonary aspergillosis were 0.794 and 0.870 respectively, the cut-off values were 0.43 and 0.88 respectively, the sensitivity were 74.00% and 82.00% respectively, the specificity were 75.00% and 100.00% respectively. By parallel test, the sensitivity and specificity of serum combined with alveolar lavage fluid GM value for diagnosis of aspergillus disease were 88.00% and 85.00%; by tandem test, the sensitivity and specificity of serum combined with alveolar lavage fluid GM for diagnosis of aspergillosis were 70.00% and 100.00%. Conclusion The GM test of serum and alveolar lavage fluid has a good application value in the diagnosis of pulmonary aspergillosis and is worth clinical use.

Application of iohexol and barium sulfate as contrast agents in swallowing angiography of patients with dysphagia after stroke

Objective To investigate the application effect of iohexol and barium sulfate as contrast agents in video fluoroscopic swallowing study (VFSS) of patients with dysphagia after stroke. Methods From September 2018 to June 2019, 71 patients with dysphagia after stroke who underwent VFSS in Department of Rehabilitation, Affiliated Hospital of Xuzhou Medical University were selected as study subjects. They were divided into barium sulfate group (36 cases) and iohexol group (35 cases) according to the random number table method. Barium sulfate group used barium sulfate as contrast agent, iohexol group used iohexol as contrast agent. The degree of leakage-aspiration during angiography and the incidence of aspiration pneumonia in two groups were compared, and the effects of two contrast agents on the treatment of swallowing dysfunction were analyzed. Results The penetration-aspiration scale (PAS) score after the second angiography in two groups was significantly lower than that after the first angiography, and the difference was statistically significant (P < 0.05); but there was no significant difference in PAS score between two groups (P > 0.05). The incidence of aspiration pneumonia in barium sulfate group was higher than that in iohexol group, and the difference was statistically significant (P < 0.05). There was no significant difference in the treatment effect of swallowing ability between two groups (P > 0.05). Conclusion The use of iohexol as contrast agent in VFSS does not reduce the sensitivity of swallowing angiography, and the incidence of aspiration pneumonia is low and does not affect clinical treatment.

Clinical effect of open loop procedure for prolapse and hemorrhoids in the treatment of circular hemorrhoids patients

Objective To study the curative effect of open loop procedure for prolapse and hemorrhoids (PPH) in the treatment of circular hemorrhoids. Methods The diagnosis and treatment of 100 patients with circular hemorrhoids admitted to Anhui Wanbei Coal-electricity Group General Hospital from January 2018 to January 2020 were retrospectively analyzed, and they were divided into observation group (64 cases) and control group (36 cases) according to different surgical methods. The control group was treated with conventional PPH, and the observation group was treated with open loop PPH. The clinical efficacy at postoperative one month and perioperative conditions were compared between the two groups, and the changes of the anal canal resting pressure, rectal resting pressure, anal canal maximum systolic pressure, and length of anal canal high pressure band at preoperative and postoperative three months were compared, and the incidence of postoperative complications was compared. Results There was no significant difference in the total effective rate of clinical efficacy between the two groups (P > 0.05). There were no significant differences in operation time, hospitalization time, wound healing time and resumption of work time between the two groups (P > 0.05). The anal canal resting pressure, rectal resting pressure, anal canal maximum systolic pressure and length of anal canal high pressure band at postoperative three months in both groups were higher than those at preoperative, and the observation group was higher than the control group, the differences were statistically significant (all P < 0.05). The total incidence of postoperative complications in the observation group was lower than that in the control group, and the difference was statistically significant (P < 0.05). Conclusion Open loop PPH has obvious effect in the treatment of patients with circular hemorrhoids, which can effectively promote the recovery of anal function and reduce the incidence of complications, and is worthy of application and promotion.

Effect of Roxathat and Recombinant Human Erythropoietin in the treatment of renal anemia in maintenance hemodialysis patients

Objective To investigate the effect of Roxathat and Recombinant Human Erythropoietin (hereinafter referred to as “erythropoietin”) in the treatment of renal anemia in maintenance hemodialysis patients. Methods A total of 60 patients with renal anemia admitted to the Central Hospital of Ma’anshan, Anhui Province from December 2019 to September 2020 were selected and they were divided into observation group and control group according to random number table method, with 30 patients in each group. The control group was treated with Recombinant Human Erythropoietin combined with Ferrous Succinate Sustained-release Tablets, and the observation group was treated with Roxathat combined with Ferrous Succinate Sustained-release Tablets. Both groups were treated for three months continuously. The changes of hemoglobin (Hb), red blood cell count (RBC), hematocrit (Hct), serum iron (SI), unsaturated iron-binding capacity (UIBC), total iron-binding capacity (TIBC), serum iron saturation (ISAT) before and after treatment and the incidence of adverse reactions were compared between two groups. Results After treatment, the levels of Hb, RBC, Hct, SI, TIBC and ISAT in both groups were higher than before treatment, and the observation group was higher than the control group, the differences were statistically significant (all P < 0.05). After treatment, the level of UIBC in the two groups was lower than that before treatment, and the level of UIBC in the observation group was lower than that in the control group, the differences were statistically significant (all P < 0.05). There was no significant difference in the total incidence of adverse reactions between the two groups (P > 0.05). Conclusion Roxastat can more effectively improve the levels of anemia indexes and iron metabolism indexes of renal anemia in maintenance hemodialysis patients, which is safe and reliable and worthy of clinical promotion.

Relationship between breast hyperplasia and breast cancer from the ancient literatures of Ming and Qing Dynasties

Traditional Chinese medicine surgery has developed and matured in Ming and Qing Dynasties, the understanding of breast disease reaches a new height. It is found that there is a correlation between breast hyperplasia and breast cancer, but modern medicine also believes that the occurrence of breast cancer is related to breast hyperplasia. Based on the analysis of famous doctors and their works in Ming and Qing Dynasties, this paper discusses the relationship between breast hyperplasia and disease name, etiology and pathogenesis, clinical manifestations, treatment principles and methods of breast cancer. The nomenclature of disease is closely related to symptoms and signs, and emotional factors are the main causes of both diseases. Both liver and spleen injury and thoroughfare-controlling vessels disharmony are the common pathogenesis, clinical manifestation of breast tuberculosis is a common symptom, with the further progress of the disease, the difference gradually show up. The key to treat the mastophilia and lactolith is to dredge liver and regulate qi, replenish qi and blood, and pay attention to emotional adjustment.

Analysis on syndrome differentiation and treatment of palmoplantar pustulosis based on the theory of “damp heat producing insects”

Palmoplantar pustulosis is a common surgical disease of traditional Chinese medicine, common itching, exudation and desquamation and other clinical manifestations. The disease is extremely persistent and protracted, which can have a great impact on the daily life of patients. In ancient China, the earliest records of palmoplantar pustulosis can be traced back to Oracle Bone Inscription Dynasty. The ancient doctors discuss the clinical manifestations and treatment methods of palmoplantar pustulosis in detail, but there are few records on its syndrome differentiation and treatment. “Damp heat producing insects” is one of the characteristic theories of traditional Chinese medicine, which is a high generalization of etiology and pathogenesis. The author thinks that the main responsibility of the disease is “damp heat producing insects” based on the research of traditional Chinese medicine literature. On the basis of the theory of “damp heat producing insects” in traditional Chinese medicine, the disease can be divided into four types: insect pathogen in the surface, insect in the muscle, insect in the blood and insect toxin for a long time. And the author summarizes clinical manifestations and therapeutic prescriptions of each type respectively, in order to provide new ideas and methods for the clinical diagnosis and treatment of palmoplantar pustulosis.

Study on English translation of Shanghan Lun from the perspective of empirical functional equivalence

Previous studies on English translation of Shanghan Lun mostly focus on the lexical level, such as medical terms and cultural core words. However, translation is a process of organizing and generating the target text. Therefore, it is necessary to carry out further research on the basis of sentence level. From the perspective of empirical functional equivalence, this paper makes a comparative analysis of the original text and the translation of Shanghan Lun. Combined with examples, it focuses on how to achieve equivalence between the original text and the translation of three types of sentences in the information text: material process, psychological process and relational process, and how to more effectively convey the theme of the original text. Generally speaking, translation equivalence is necessary when translating medical texts, such as Shanghan Lun, which helps to ensure that the translated text is faithful to the original text. However, equivalence is not the final criterion for judging the quality of translation. Translation should not be limited to the formal equivalence between the target text and the original text, but should focus on the meaning, take the effective transmission of the medical gist of the original text as the basic principle, and take the acceptance of the target readers as the ultimate goal.

Appreciation and analysis on the proven cases of master of Chinese medicine and Professor Duan Fujin in the treatment of infantile chronic malnutrition based on syndrome differentiation

Professor Duan Fujin (1930—2019), a master of Chinese medicine, had a significant effect in treating infantile chronic malnutrition. He made good use of classical prescriptions Liujunzi Decoction and Lizhong Decoction for modified treatment, and developed a remarkable curative effect of Xiao’er Wenpi Xiaoji Powder. Professor Duan Fujin pointed out that the etiology of contemporary infantile chronic malnutrition is mostly because of “deficiency resulted from excess”. For this, attention should be paid to “dredging and elimination in combination”. At the same time, children should be required to avoid food accumulation and slowly recover spleen qi. He paid attention to invigorating spleen and stomach when taking medicine, and made good use of medicine that can clear the organs without hurting qi, and warm the spleen without helping fire. His ideas and experience are worthy of promotion and practice. This article analyzes 3 proven cases to show Professor Duan Fujin’s clinical thoughts and differentiation methods, in order to provide references for the clinical treatment of infantile chronic malnutrition based on syndrome differentiation.

Analysis on the medication rules of replenishing qi and nourishing yin method in treating heart palpitation and chest bi by Professor Mao Dexi

Objective To explore the medication rules of replenishing qi and nourishing yin method in treating heart palpitation and chest bi by Professor Mao Dexi. Methods The outpatient medical records of replenishing qi and nourishing yin method in treating heart palpitation and chest bi by Professor Mao Dexi in Hall of Famous Doctor of He’nan Provincial Hospital of Traditional Chinese Medicine from February 2018 to January 2020 were collected, which were input into traditional Chinese medicine inheritance assist platform (V2.5). The frequency analysis, association rules and complex entropy cluster analysis were used to analyze the medication rules of Professor Mao Dexi in the treatment of heart palpitation and chest bi. Results A total of 67 prescriptions were enrolled, including 28 heart palpitation prescriptions and 39 chest bi prescriptions. The commonly used drugs for heart palpitation were Radix Ophiopogonis, Radix Pseudostellariae, Fructus Schisandrae Chinensis, Radix Glycyrrhizae and Radix Salviae Miltiorrhizae, and 13 commonly used couplet medicines and 4 new prescriptions were excavated. The commonly used drugs for chest bi were Radix Ophiopogonis, Fructus Schisandrae Chinensis, Radix Glycyrrhizae, Radix Astragali seu Hedysari, Radix Pseudostellariae, Radix Paeoniae Rubra, and 14 commonly used couplet medicines and 4 new prescriptions were excavated. Conclusion Through data mining technology, it is found that Professor Mao Dexi often uses modified Shengmai Powder to treat heart palpitation and chest bi with syndrome of deficiency of both qi and yin. For patients with heart palpitation due to malnutrition of heart blood, the drugs of invigorating spleen and calming heart, nourishing blood for tranquillization, such as Semen Ziziphi Spinosae, Poria, Radix Angelicae Sinensis, and Radix Rehmanniae Recens, and others were used. For patients with excess syndrome, the drugs of promoting blood circulation for removing blood stasis, clearing heat and promoting diuresis, such as Radix Salviae Miltiorrhizae, Radix Sophorae Flavescentis, and others were added. For patients with chest bi, Ramulus Cinnamomi and Bulbus Allii Macrostemonis were used for warming heart yang, Fructus Trichosanthis and Fructus Aurantii Immaturus were used for relieving chest stiffness and resolving masses, Rhizoma Ligustici Chuanxiong and Radix Salviae Miltiorrhizae were used for promoting blood circulation for removing blood stasis.

Effect of Chinese medicine bamboo cupping therapy on sleep quality and neurological impairment after apoplexy

Objective To observe the effect of Chinese medicine bamboo cupping therapy on sleep quality and neurological impairment after apoplexy. Methods Total 100 cases of patients with sleep disorder after apoplexy admitted to Department of Encephalopathy, Haikou Hospital of Traditional Chinese Medicine from August 2019 to July 2020 were selected and randomly divided into intervention group and control group by random number table method, with 50 cases in each group. The intervention group was given Chinese medicine bamboo cupping therapy, every other day, while the control group was given Dexzopiclone Tablets orally, once a day. Ten days was as a course of treatment. The clinical effect, sleep quality, improvement of neurological impairment and the occurrence of adverse reactions were observed. Results After treatment, the total effective rate of the intervention group was significantly higher than that of the control group, the difference was statistically significant (P < 0.05). The scores of Athens insomnia scale and National Institutes of Health stroke scale of the two groups were lower than those before treatment, and the scores of the intervention group were lower than those of the control group, the differences were statistically significant (P < 0.05). No obvious adverse reactions were observed in intervention group, while the incidence of adverse reactions in the control group was 10%. Conclusion Chinese medicine bamboo cupping therapy can effectively improve the sleep quality and neurological impairment symptoms of patients with insomnia after apoplexy, and reduce the incidence of adverse reactions, which is helpful to promote the rehabilitation process, and it is safe and worthy of promotion.

Effect of Cinacalcet in patients with secondary hyperparathyroidism of chronic kidney disease maintenance dialysis

Objective To explore and analyze the clinical effect of Cinacalcet in patients with secondary hyperparathyroidism (SHPT) of chronic kidney disease maintenance dialysis. Methods A total of 120 SHPT patients with chronic kidney disease maintenance dialysis admitted to Fuyang Second People’s Hospital, Anhui Province from January 2019 to April 2020 were selected as study subjects. According to odd number and even number method, they were divided into control group and experimental group, with 60 patients in each group. The control group was treated with conventional drugs such as vitamin D analogues and phosphorus binders, the experimental group was treated with Cinacalcet on the basis of the control group, and both groups were treated for three months. The laboratory test results (levels of serum calcium [Ca], serum phosphorus [P], calcium-phosphorus product [Ca×P], serum whole parathyroid hormone [iPTH]) were compared between the two groups before and after treatment. The incidence of adverse reactions and clinical efficacy were compared between the two groups. Results The levels of Ca, P, Ca×P and iPTH in the two groups were lower than those before treatment, and the levels of Ca, P, Ca×P and iPTH in the experimental group were lower than those in the control group (all P < 0.05). There was no significant difference in the total incidence of adverse reactions between the two groups (P > 0.05). The clinical total effective rate of the experimental group was higher than that of the control group, the difference was statistically significant (P < 0.05). Conclusion Cinacalcet can effectively reduce the levels of Ca, P, Ca×P and iPTH in SHPT patients with chronic kidney disease and maintenance dialysis, and improve the clinical efficacy without increasing the adverse drug reactions.

Analysis of the effect of personalized nursing based on Omaha system on the degree of cancer pain in patients with vertebral metastases treated with radiofrequency

Objective To investigate the effect of personalized nursing based on Omaha system on the degree of cancer pain in patients with vertebral metastasis treated by radiofrequency therapy. Methods From January 2018 to January 2019, 70 patients with vertebral metastases in Nanjing Drum Tower Hospital, the Affiliated Hospital of Nanjing University Medical School treated by radiofrequency treatment were selected as research object, according to random number table method, they were divided into control group and experimental group, with 35 cases in each group, the control group received routine nursing care, the experimental group received personalized care based on the Omaha system, nursing effect and cancer pain were compared between the two groups. Results After nursing, the general self-efficacyscale (GSES) score of the two groups was higher than that before nursing, and the compliance behavior score was lower than that before nursing, the difference was statistically significant (P < 0.05). After nursing, the GSES score of the experimental group was higher than that of the control group, and the compliance behavior score of the experimental group was lower than that of the control group, with statistically significance (P < 0.05). The total score of brief pain inventory (BPI) after nursing was lower than that before, and the BPI score of experimental group was lower than that of control group, the difference was highly statistically significant (P < 0.01). The MOS item short from health survey (SF-36) score after nursing was higher in the two groups than before, and SF-36 score in the test group was higher than that in the control group, the differences were highly statistically significant (P < 0.01). Conclusion Individualized care based on Omaha system can effectively improve patients’ self-efficacy and compliance behavior, reduce cancer pain, and improve patients’ quality of life in radiofrequency treatment of vertebral metastasis.

Methods Retrospective analysis was conducted on 80 patients with schizophrenia in the military who were admitted to a hospital from June 2015 to June 2018, according to the nursing methods of patients, they were divided into observation group and control group. The control group received routine rehabilitation mode, and the observation group received multi-dimensional rehabilitation mode. Social deficiency screening scale (SDSS) scores, family burdening scale (FIS) scores, nerve growth factor (NGF), brain-derived neurotrophic factor (BDNF) and insulin-like growth factor 1 (IGF-1) scores were compared between the two groups before and 1, 2 years after rehabilitation. Results Overall analysis showed that there were statistically significant differences in SDSS and FIS scores, time point comparison and interaction (P < 0.05). Further pair comparison, intra-group comparison: SDSS and FIS scores 1, 2 years after rehabilitation in both groups were lower than before, and SDSS and FIS scores two years after rehabilitation in each group were lower than one year after rehabilitation (P < 0.05). Comparison between groups: SDSS and FIS scores of observation group were lower than those of control group 1, 2 years after recovery (P < 0.05). Overall analysis showed that there were statistically significant differences in IGF-1, BDNF and NGF levels between groups, time point comparison and interaction (P < 0.05). Further pair-to-group comparison: IGF-1, BDNF and NGF levels 1, 2 years after rehabilitation in two groups were higher than before, and IGF-1, BDNF and NGF levels two years after rehabilitation were higher than one year after rehabilitation (P < 0.05). Comparison between groups: The levels of IGF-1, BDNF and NGF in observation group were higher than those in control group 1, 2 years after recovery (P < 0.05). Conclusion The effect of multi-dimensional rehabilitation model on military schizophrenic patients is remarkable, which can improve the current brain nutrition index.

Application of health education based on Skinner’s reinforcement theory in respiratory function training of patients after radical resection of gastric cancer

Objective To explore the effect of health education based on Skinner’s reinforcement theory on rehabilitation self-efficacy and compliance of respiratory function training in patients after radical resection of gastric cancer. Methods A total of 140 cases of elective radical gastric cancer patients who were admitted to the Affiliated Huai’an No.1 People’s Hospital of Nanjing Medical University from July 2017 to June 2018 were selected. According to the random number table method, they were randomly divided into control group and treatment group, with 70 cases in each group. The control group was given regular health education for respiratory function training, and the treatment group was given health education based on Skinner’s reinforcement theory. After six months of follow-up, the two groups’ rehabilitation self-efficacy, respiratory function training compliance, lung function were compared. Results After six months of follow-up, 64 cases in the treatment group and 61 cases in the control group were completed. The rehabilitation training self-efficacy scores of the two groups were higher than those before the same group (P < 0.01). The total scores of rehabilitation training and skill training, symptom self-management, coping with treatment side effects, daily life management, rehabilitation information acquisition and application, and self-efficacy of the treatment group were higher than those of the control group (P < 0.01); the respiratory function training compliance scores of the two groups were higher than those before education (P < 0.01), the total scores of respiratory function exercise compliance, respiratory function exercise precautions compliance, active seeking advice, and respiratory function exercise compliance in the treatment group were higher than those in the control group (P < 0.01); the lung function indexes of the two groups were lower than those before education (P < 0.01), vital capacity, forced expiratory volume in the 1 second (FEV1), forced vital capacity (FVC), and FEV1/FVC in the treatment group were higher than those in the control group (P < 0.01). Conclusion Health education based on Skinner’s reinforcement theory is helpful to promote the development of self-efficacy in patients after radical resection of gastric cancer, improve respiratory function training compliance behavior, and improve patients’ lung function.

Preventive effect of foam dressings in different periods on pressure ulcers associated with medical devices in ICU patients with non-invasive mechanical ventilation

Objective To investigate the preventive effect of foam dressings in different periods on pressure ulcers associated with medical devices in ICU patients with non-invasive mechanical ventilation, to provide guidance for the prevention of pressure ulcers. Methods Sixty-two patients with non-invasive mechanical ventilation in ICU of Xingtai People’s Hospital, Hebei Province from January 2017 to May 2019 were selected, according to random number table method, they were divided into group A and group B, with 31 cases in both groups. In group A, foam dressing was used to prevent pressure ulcers while non-invasive mechanical ventilation was performed, group B received conventional intervention after non-invasive mechanical ventilation. After one day of treatment, foam dressing was used to prevent pressure ulcers. After the treatment, the incidence and severity of pressure ulcers, the comfort level and compliance of the two groups were statistically compared. Results There was no significant difference in the incidence of pressure sores between the two groups (P > 0.05), there was no statistical significance in the severity and comfort of pressure ulcers among patients with pressure ulcers (P > 0.05). The compliance of group B was higher than that of group A, and the difference was statistically significant (P < 0.05). Conclusion Compared with the patients who used foam dressings to prevent pressure ulcers during non-invasive mechanical ventilation, the incidence of pressure ulcers did not increase and the severity of pressure ulcers did not increase foam dressing after one day of non-invasive mechanical ventilation treatment, and the patients had good comfort and higher compliance with the use of foam dressings.

Application of transcranial direct current stimulation combined with family rehabilitation guidance in children with retardation

Objective To explore the application of transcranial direct current stimulation combined with family rehabilitation guidance in children with retardation, and to analyze its effect. Methods From March 2019 to March 2020, a total of 200 children with retardation were admitted to Hu’nan Children’s Hospital. They all received transcranial direct current stimulation and were divided into control group and observation group by random number table method, with 100 cases in each group. The control group was combined with routine rehabilitation guidance, and the observation group was combined with family rehabilitation guidance, and the results were evaluated at three months of nursing. Before nursing and at three months of nursing, the Chinese Webster’s children’s intelligence scale, Gesell developmental diagnostic scale, and Peabody fine motor developmental assessment scale were used to evaluate the two groups of verbal intelligence quotient (VIQ), performance intelligence quotient (PIQ) and full intelligence quotient (FIQ), language and adaptive development quotient, and fine motor function. Results At three months of nursing, the VIQ, PIQ and FIQ of the two groups were higher than before nursing, and those of the observation group were higher than those of the control group, and the differences were statistically significant (P < 0.05). The language development quotient and adaptive development quotient of the two groups were higher than those before nursing, and those of the observation group were higher than the control group, and the differences were statistically significant (P < 0.05). The grasping ability and visual-motor comprehensive ability of the two groups were improved compared with that before nursing, and those of the observation group were higher than the control group (P < 0.05). Conclusion The intervention of transcranial direct current stimulation combined with family rehabilitation guidance to guide children with retardation is more conducive to promoting the multifunctional development of children with retardation, and is more beneficial for the development of the children’s intelligence quotient, and has significant effect.

Impact of individualized health education on children with nephrotic syndrome and their families

Objective To explore the impact of individualized health education on disease cognition, complications and dietary regularity of children with nephrotic syndrome (NS) and their parents. Methods A total of 118 children diagnosed with NS from January 2017 to March 2020 were included and divided into experimental group (60 cases) and control group (58 cases) according to the random number table method. Children in both groups were treated with low-molecular-weight heparin combined with Prednisone. The control group was given regular care, and the experimental group was given individualized health education on the basis of regular care. The intervention effects of the two groups were compared. Results The mastery rate of the experimental group was significantly higher than that of the control group, and the difference was statistically significant (P < 0.05). The incidence of complications in the experimental group was lower than that in the control group, and the difference was statistically significant (P < 0.05). There were statistially significant difference in the proportion of low-salt diet, control of protein intake, low-fat diet, eating fruits and vegetables, and avoiding stimulating food between two groups (P < 0.05). After three months of intervention, the endothelin and soluble interleukin-2 receptor of the two groups were lower than before the intervention, and the experimental group were lower than those of the control group, and the differences were statistically significant (P < 0.05). Conclusion For children with NS, individualized health education during symptomatic treatment can achieve satisfactory results, effectively improve the disease awareness of children and their families, reduce the incidence of complications, and improve the regularity of children’s diet.

Influencing factors of self-care ability in elderly patients with osteoarthritis after total knee arthroplasty and correlation analysis with hope level

Objective To observe the self-care ability of elderly patients with osteoarthritis (OA) after total knee arthroplasty (TKA), and to analyze the relationship between the influencing factors of self-care ability and hope level of patients. Methods A total of 92 elderly patients with OA who received TKA treatment in Department of Orthopedics, West China Hospital of Sichuan University from March 2015 to July 2019 were selected as the research subjects. The exercise of self-care ability scale (ESCA) was used to assess the self-care ability of patients and divided them into a low self-care ability group (34 cases) and a normal self-care ability group (58 case) at three months after operation; baseline data survey questionnaires were designed, baseline data of patients were collected and compared, the hope levels [herth hope index (HHI) for evaluation] of patients were compared, while the relationship between the influencing factors of self-care ability and hope level of patients were analyzed. Results Among 92 elderly patients with OA, 34 cases had low self-care ability after TKA, accounting for 36.96%. The economic status, social support score, hope level score of the low self-care ability group were compared with the normal self-care ability group, and the differences were statistically significant (P < 0.05). Logistic regression analysis showed that poor economic status, low social support, and low level of hope were the influencing factors for the low self-care ability of elderly patients with OA after TKA (OR = 1.947, 1.866, 2.199, P < 0.05). The self-care ability of elderly patients with OA after TKA was positively correlated with the level of hope (r = 0.562, P < 0.01). Conclusion The self-care ability of elderly patients with OA is generally low after TKA, which may be related to the factors such as poor economic status, low social support, and low level of hope, and their level of hope is positively correlated with self-care ability. The clinic should pay attention to the hope level of patients, and early targeted interventions should be given to improve the self-care ability of patients.
