Effect of U18666A on APP metabolism and Aβ generation in U373 cells of astroglioma

Objective To investigate the effect of cholesterol transport inhibitor U18666A (UA) on amyloid precursor protein (APP) metabolism and β-amyloid protein (Aβ) generation in U373 cells of astroglioma. Methods U373 cells were treated with UA at the dose of zero, one, three and five μg/mL for 24 h or 5 μg/mL UA for different times (0, 12, 24, 48 h). The expressions of APP and its metabolites α-CTF and β-CTF were detected using Western blot. The cellular levels of Aβ1-40 were measured with enzyme linked immunosorbent assay and the activity of β-secretase was determined by fluorescence assay. Results The expression levels of APP, α-CTF and β-CTF, and Aβ1-40 in 5 μg/mL UA group were higher than those in 0 μg/mL UA group, and the differences were statistically significant (P < 0.05 or P < 0.01). The expression levels of APP, α-CTF and β-CTF, and Aβ1-40 in co-cultured 24 h and 48 h groups were significantly higher than those in co-cultured 0 h group, and the differences were highly statistically significant (all P < 0.01). The activity of β-secretase in 5 μg/mL UA group was significantly higher than that in the control group (P < 0.01). Conclusion UA enhances APP expression and metabolism by increasing β-secretase activity, promotes Aβ1-40 generation, accelerates the pathological process of Alzheimer′s disease.

Protective effect of inhibiting inflammasome activation product caspase-1 on acute liver failure in rats

Objective To investigate the protective effect of inhibiting cysteinyl aspartate-specific proteinase (caspase)-1 on acute liver failure (ALF) in rats. Methods A total of 60 healthy male Wistar rats, weighing about 150-180 g, were divided into the control group, the model group and the intervention group by random number table method, with 20 rats in each group. In the control group, serum and liver tissue were collected. In the model group, rats were intraperitoneally injected with 1200 mg/kg D-galactosamine (D-Galn) and 100 g/kg liponolysaccharide (LPS) to construct the ALF model. Serum and liver tissue were collected 24 h after modeling. In the intervention group, rats were injected with 6 mg/kg of thespecific inhibitor of caspase-1, fluoromethylketone by tail vein in the two hours before modeling. The D-GalN and LPS were given to the same dose as the model group during modeling. Serum and liver tissue were collected in the 24 hours after modeling. The liver tissues were observed by HE staining, and serum levels of alanine aminotransferase (ALT), aspartate aminotransferase (AST), total bilirubin (TBil) and interleukin (IL) -1β were measured. Results HE staining results indicated that the model was successful. ALT, AST, TBil and IL-1β levels in the model group were higher than those in the control group, the differences were all highly statistically significant (all P < 0.01), and ALT, AST, TBil and IL-1β levels in the intervention group were lower than those in the model group, the differences were all highly statistically significant (all P < 0.01). Conclusion The inflammatory response caused by caspase-1 is one of the important causes of ALF injury and is expected to be a predictor of ALF and a future therapeutic target.

Effects of tPA gene methylation / demethylation in Propofol protection of learning and memory function after electroshock therapy in depressive rats

Objective To investigate the protective effect of Propofol on learning and memory function in depressive rats after electroshock therapy (ECT) by regulating tissue plasminogen activator (tPA) gene methylation/demethylation. Methods Seventy-two adult male SD rats were used in this study and eighteen rats were randomly selected as the control group (group C). The remaining fifty-four rats were randomly divided into group D, group E and group F after depressive modeling, with 18 rats in each group. Group C was not given experimental treatment; group D was intraperitoneally injected with normal saline + pseudo-ECT; group E was intraperitoneally injected with normal saline + ECT; group F was intraperitoneally injected with Propofol + ECT. Depressive behavior and learning function of rats were measured by sucrose preference percentage (SPP), escape latency (EL) and space exploration time (SET). The mRNA levels of hippocampus tPA, DNA methyltransferase (DNMT) 1, DNMT3a, DNMT3b and ten-elevan translocation (TET) 1 were detected by reverse transcription PCR. The expressions of tPA and DNMT1 in hippocampus were detected by Western blot. MeDIP-qPCR was used to detect tPA methylation rate. After ECT treatment, compared with group D, SPP increased in Group E and F, EL prolonged and SET shortened in group E, tPA mRNA and protein expression in hippocampal CA1 area decreased, DNMT1 mRNA and protein expression increased, and tPA methylation rate increased, with statistically significant differences (all P < 0.05). Compared with group E, in group F, EL shortened and SET prolonged, tPA mRNA and protein expression in hippocampal CA1 area increased, DNMT1 mRNA and protein expression decreased, and tPA methylation rate decreased (all P < 0.05). There was no significant difference in mRNA expression of DNMT3a, DNMT3b and TET1 in each group (all P > 0.05). Conclusion Propofol can effectively reduce ECT-induced learning and memory impairment in depressive rats. The mechanism may be related to the downregulate the expression of DNMT1 in the hippocampal CA1 region to reduce the methylation level of the tPA gene resulting in increase the expression level of tPA mRNA and protein.

Study on correlation between the quality of life of COPD patients and debilitating state

Objective To study the correlation between the quality of life and the degree of degeneration of patients with chronic obstructive pulmonary disease (COPD), and to analyze the statistical correlation between the two to explore whether degeneration affects the quality of life of COPD patients. Methods The convenience sampling method was used to select 326 inpatients with COPD in the respiratory medicine department of a top three hospital from January to October 2019 as the research object, and the quality of life and the degree of degeneration were evaluated by the clinical COPD assessment test questionnaire (CAT) and the Tilburg frailty indicator scale (TFI). The impact of various indicators on quality of life was compared, while the correlation between the quality of life and the degree of weakness of COPD patients were analyzed. Results The differences in the CAT scores of COPD patients with different ages, financial burden, exercise time, body mass index, COPD grade, and number of complications were statistically significant (P < 0.05). A total of 191 patients were found to be in a debilitating state, and the degeneration detection rate was 58.7%. Pearson correlation analysis showed that CAT scores was positively correlated with TFI total score (r = 0.484, P < 0.01). The CAT score of COPD patients was related to TFI score (β [95%CI]: 1.021 [0.922, 1.120]). Conclusion The degree of degeneration of COPD patients will affect their quality of life, suggesting that improving degeneration in the treatment of COPD may be beneficial to the final recovery effect.

Changes and clinical significance of plasma ischemia-modified albumin in patients with acute pancreatitis

Objective To investigate the changes and clinical significance of plasma ischemia-modified albumin in patients with acute pancreatitis. Methods A total of 300 patients with acute pancreatitis admitted to Panzhihua Central Hospital, Sichuan Province (hereinafter referred to as “our hospital”) from February 2017 to December 2018 were selected as the observation group, and 60 healthy people who underwent physical examination in our hospital during the same period were selected as the control group. According to the different severity of acute pancreatitis, the observation group was divided into the mild group (95 cases), the moderate group (118 cases) and the severe group (87 cases). Plasma ischemia-modified albumin, tumor necrosis factor-α (TNF-α), and C-reactive protein (CRP) levels in each group were analyzed. The diagnostic value of ischemia-modified albumin levels in acute pancreatitis was analyzed by receiver operating characteristic (ROC) curve. Results The levels of plasma ischemia-modified albumin, TNF-α and CRP in the observation group were higher than those in the control group (P < 0.05). The levels of plasma ischemia-modified albumin, TNF-α, CRP and Ranson score in the mild group were lower than those in the moderate and severe groups, and the above indexes in the moderate group were lower than those in the severe group (all P < 0.05). Plasma ischemia-modified albumin was positively correlated with TNF-α, CRP and Ranson score (r = 0.464, 0.565, 0.407, P < 0.05). The AUC of plasma ischaemic-modified albumin in the diagnosis of acute pancreatitis was 0.956 (95%CI: 0.937-0.976), the youden index was 0.55, the optimal cut-off value was 83.61 U/mL, the sensitivity and specificity were 82.91% and 75.21%, respectvely and the accuracy was 80.72%. Conclusion Abnormal increase of plasma ischemia-modified albumin in patients with acute pancreatitis is significantly correlated with TNF-α, CRP and Ranson score. Plasma ischemia-modified albumin has diagnostic value for the severity of acute pancreatitis.

Mechanism study of long non-coding RNA ROR1-AS1 promoting osteosarcoma cell invasion

To investigate the effect of lncRNA ROR1-AS1 on the invasion ability of osteosarcoma (OS). Methods RT-PCR detected the expression of lncRNA ROR1-AS1 and ROR1 in osteoblasts and OS cell lines; cell growth was observed after cell transfection, ROR1 was overexpressed, and cell invasion ability was detected by Transwell and scratch test. The expression of epithelial to mesenchymal transformation -related molecular markers (E-caderin, N-caderin, abcam) was detected by Western-blot. Results LncRNA ROR1-AS1 and ROR1 were highly expressed in OS cells compared with osteoblasts (P < 0.01). After lncRNA ROR1-AS1 was inhibited, mRNA expression of ROR1 in OS cells was down-regulated (P < 0.05). After OS cells knocked down lncRNA ROR1-AS1, Transwell and scratch experiments confirmed that the invasion ability of cells was weakened, while the invasion ability was restored after overexpression of ROR1 (P < 0.05). After lncRNA ROR1-AS1 inhibition, the expression of epithelial phenotypic marker E-caderin was decreased, while that of mesenchymal phenotypic marker N-caderin was increased. Overexpression of ROR1 presents the opposite effect. Conclusion LncRNA ROR1-AS1 promotes OS cell invasion dependent on ROR1.

Effect of Radix Aucklandiae on the resistance of Gefitinib in non-small cell lung cancer

Objective To observe the effect of Saussurea costus and Indian costus combined with Gefitinib on the model of C. elegans which was resistant to epidermal growth factor receptor tyrosine kinase inhibitors (EGFR-TKI), and to research the main effective active components. Methods Genotyped anthropogenic (LET-23::hEGFR-TK[T790M- L858R]) C. elegans mutant jgIs25 was used to ensure the effective inhibitory concentration of Saussurea costus and Indian costus. The effective active components were preliminarily determined by HPLC (Hypersil BDS C18 250 mm×4.6 mm, 5 μm, detection wavelength: 220 nm). Results There were significant differences in the vulva expression of C. elegans jgIs25, when the concentration of Indian costus was 100-750 μg/mL and Saussurea costus was 500-750 μg/mL (P < 0.01). It was preliminarily speculated that the active monomers were costunolide and dehydrocostus lactone. Conclusion Radix Aucklandiae combined with Gefitinib can reverse EGFR-TKI resistance in non-small cell lung cancer. It can provide the basis for the follow-up clinical research.

Study on the optimization of the ethanol precipitation process of Penyanling Granules by using Box-Behnken response surface methodology

Objective To optimize the ethanol precipitation process for Penyanling Granules. Methods Based on the single factor test, the relative density of the liquid before ethanol precipitation, the final ethanol concentration and the ethanol precipitation time were used as factors. The transfer rates of chlorogenic acid, paeoniflorin, ferulic acid, salvianolic acid B, norisoboldine, liquiritin, tetrahydropalmatine, astilbin and the composite score of solid removal rate were selected as evaluation indexes. The ethanol precipitation process of Penyanling Granules was optimized by Box-Behnken response surface method. Chromatographic conditions: the column was Agilent TC-C18 (250 mm×4.6 mm, 5 μm), the flow rate was 1.0 mL/min. The mobile phase A was acetonitrile and the mobile phase B was 0.2% phosphoric acid solution with gradient elution (0-10 min, 2% A; 10-40 min, 2%-12% A; 40-80 min, 12%-18% A; 80-110 min, 18%-25% A; 110-120 min, 25%-50% A). The column temperature was 30℃ and the detection wavelength was 200-400 nm. Results The optimal ethanol precipitation process was as followed: the relative density of the extract was 1.13, and ethanol was added to allow the content of ethanol to reach 60%, and then been placed for 21 h. The test results indicated that the relative difference of comprehensive scoring value between predicated value and measured value was 0.7%. Conclusion The optimal ethanol precipitation process is simple, easy to operate and the results are accurate and stable, which can provide a reliable theoretical basis for the preparation production of Penyanling Granules.

Clinical research progress of acupuncture and moxibustion in the treatment of chronic pelvic pain

Chronic pelvic pain is a common and complex disease in clinic, mainly manifested as periodic or aperiodic pain in the pelvis and surrounding tissues, which seriously affects people′s quality of life. Traditional Chinese Medicine holds that the basic pathogenesis of chronic pelvic pain is internal stasis of blood, which is often caused by dampness and heat, qi stagnation, cold coagulation, kidney deficiency, qi deficiency and other factors. Therefore, promoting blood circulation for removing blood stasis, eliminating dampness and heat, warming channel for dispelling cold, and promoting qi circulation to relieve pain are often applied in the treatment. As the Traditional Chinese Medicine treatment, acupuncture and moxibustion has been proved to be effective in treatment of chronic pelvic pain in long-term clinical practice. Through the excitation on the meridian entry points, the needle sesation of “qi reaching affected area” is directly affect to the lower abdomen, perineum and other pain position, which could stimulate the meridians bullishness, dredge the channel, and improve the ciculation, so as to significantly relieve the symptoms of pelvic pain, lumbosacral soreness, abdominal distension, micturition unwell and so on. Acupuncture and moxibustion is an effective treatment method with quick effect, significant effect and little side effect. Therefore, acupuncture and moxibustion therapy will provide new ideas for the clinical treatment of chronic pelvic pain.

Review of risk factors and prevention strategies for anterior cruciate ligament injuries

The anterior cruciate ligament is a collagenous structure that arises from the medial wall of the lateral condyle of the femur. It′s an intra-articular but extracapsular structure with limits healing capacity. It maintains the static stability of the knee primarily by resisting overextension, over translation, and over rotational motion. It resists all varus and valgus movements. Injuries are common orthopedic disorders and affect young people, especially girls, who often participate in jumping, turning and changing directions. Previous studies have identified a variety of internal and external risk factors for anterior cruciate ligament injures, including anatomical variation, neuromuscular defects, biomechanical abnormalities, race environment and hormonal status. Many prevention programs have proven effective in reducing incidence in young people. Treatment options include aerobic training, proprioception and balance training, as well as education and feedback on body mechanics and proper landing patterns.

Research progress on quality evaluation of Lycium barbarum

Lycium barbarum is food and nutritious Chinese medicinal material, and its medicinal use began in Shen Nong′s Herbal Classic. Lycium barbarum contains many components such as Lycium barbarum polysaccharide, Lycium barbarum medlar flavonoids, vitamins, trace elements, amino acids, etc., and has high medicinal value and nutritional value. With the deepening of the quality evaluation research, it was found that different germplasm, producing area, harvesting time and processing method had significant influence on the appearance, morphological characteristics and nutrient content of Lycium barbarum. Fourier transform infrared spectroscopy, high performance liquid chromatography, NOSE, DNA and other technologies are applied to the identification and fingerprinting of Lycium barbarum, which can quickly distinguish its origin and variety. A variety of new technologies and methods have been applied to the quantitative detection of active ingredients such as Lycium barbarum polysaccharides, betaines, flavones, carotene, organic acids, vitamins, and trace elements. The detection of organochlorine and organophosphorus pesticide residues in Lycium barbarum has improved the safety evaluation level. These related research results provide a valuable reference for the improvement of the quality standard of Lycium barbarum and the in-depth development of the industry.

Advances in research on the hope level for breast cancer patients

Female breast cancer has a high incidence and high mortality rate, showing a younger trend in recent years. With the advancement of medical technology, most of the patients′ physical symptoms are controlled, but the treatment period is long, the treatment costs are high, and the patients are under various pressures. With the rise of positive psychology, scholars have begun to pay attention to the mental health of patients. Hope level has received extensive attention as one of the research contents in the field of positive psychology. This article reviews the literature on the concept of the hope level for breast cancer patients, the evaluation tools involved, the current status,influencing factors, and intervention methods, so as to provide reference for psychological intervention for breast cancer patients.

The enlightenment of American higher education methods on medical education in China

Ever since problem-based learning (PBL) has been introduced into medical higher education in China, it has had a significant role in improving the quality of teaching and comprehensive literacy of students. This article reviews current conditions in higher education in the United States, investigates the necessity of and issues with PBL teaching in China, and explores solutions in the implementation process. Through the enlightenment of American higher education methods on medical education in China, in order to solve the problem that the teaching method is not easy to be adapted and accepted by teachers and students in the specific practice process, and provide reference for domestic counterparts to carry out education and teaching. Arguably, the accumulation of experience, as well as the solution of important issues, will eventually improve teachers′ understanding of higher education teaching methods and principles, and support the education of talented, innovative medical students.

Application of scenario simulation teaching method in the skill operation teaching of fresh graduates of clinical medicine

Objective To explore the effect of scenario simulation teaching method on the skills operation teaching of fresh graduates in clinical medicine. Methods Twenty fresh graduates of clinical medicine who were internships in seven teaching affiliated hospitals of Xinjiang Medical University from July 2018 to July 2019 were selected as the research objects (140 people), and 35 students were randomly selected for scenario simulation teaching. Through questionnaire survey and operation assessment, before and after scenario simulation teaching, the skills operation theory and operation level of fresh graduates of five-year clinical medicine were evaluated, and comparative research were conducted. Results A total of 140 questionnaires were distributed and 137 valid questionnaires were withdrawn, with an effective recovery rate of 97.8%. The overall good rate of seven hospitals was 62.0% (85/137). There were no statistically significant differences in the good rate among the hospitals (P > 0.05). After 35 students conducted the scenario simulation teaching method, a total of 14 students with excellent and good results. Compared with the good rate before and after the training, and the difference was statistically significant (P < 0.05). Conclusion At present, there are problems such as less opportunities for clinical skills operation and less teaching training among undergraduate interns, which seriously affect the quality of clinical teaching. The use of situational simulation teaching method can better improve the clinical skills operation ability of undergraduate interns, and has good use value in formulating scientific internship programs and other aspects.

The importance of inheritance education in the training of Chinese medicine talents

The development and promotion of traditional Chinese medicine are inseparable from inheritance and innovation, and the important way of training traditional Chinese medicine talents is reflected in inheritance education. Traditional Chinese medicine inheritance has great significance to traditional Chinese medicine education and the development of traditional Chinese medicine. The dual attributes of traditional Chinese medicine, the advantages of traditional Chinese medicine treatment, and the need to improve the practical ability of traditional Chinese medicine determine the necessity of traditional Chinese medicine education. Inheritance education can target medical students, doctors at all levels and other multi-level training objects, and carry out work in a variety of modes: “Famous Chinese Physician” “Famous Chinese Medicine Studio” “Talent Research Project”, and “Graduate Education”. Only when the inheritance of traditional Chinese medicine is implemented in the work, relevant norms and standards are formulated and implemented, while the management and evaluation system is rationalized and standardized can the education of traditional Chinese medicine be truly deepened and developed.

Discussion on the construction of traditional Chinese medicine outpatient teaching standard based on the educational pattern of teacher-apprentice education

The academic inheritance of traditional Chinese medicine (TCM) and the cultivation of clinical diagnosis and treatment ability are the key problems to be solved in higher education of TCM. The educational pattern of TCM outpatient of teacher-apprentice education can effectively make up for the malpractice of the disconnection between theoretical learning and clinical practice under the mode of college education, but it also needs new connotation to support the development. In view of some outstanding problems existing in the teaching of TCM outpatient service in affiliated hospitals, this paper expounds the specific objectives and requirements from five aspects of teaching, learning, management, supervision and evaluation. Combining the characteristics of teacher-apprentice educational pattern and linking the theoretical teaching and clinical teaching closely, this paper puts forward the idea of construction TCM outpatient teaching standard under the educational pattern of teacher-apprentice education, which is characterized by “giving knowledge to solve puzzles, continuing learning to create, assisting in management, supervision and evaluation”. In this standard, teachers, students, schools and hospitals are all in parallel, which promotes and deepens the reform of the training mode of TCM talents, so as to improve the teaching quality of TCM outpatient service and provide ideas for improving TCM outpatient teaching standard.

Analysis of recognition of the mixed teaching of basic medicine in general and key medical colleges

Methods From January to June 2019, 250 students were selected of the first and second-year undergraduates of clinical medicine from two general medical schools and two key medical schools, Changsha Medical College (160 students), Xiamen Medical College (130 students), Xiangya Medical College of Central South University (130 students) and Southern Medical University (100 students), they were investigated by questionnaires to find out their attitudes and suggestions on adopting mixed teaching in basic courses. Results A total of 481 valid questionnaires were received (including 273 students from general medical universities and 208 students from key medical universities), and 93.56% of the students thought that mixed teaching had been successful. The number of people who thought it was necessary to carry out mixed teaching in general medical colleges was less than that in key medical colleges. In general medical colleges, the number of people who thought it was need to reform the mixed teaching of basic courses was more than that of key medical colleges. The survey showed that the satisfaction degree of mixed teaching in general medical colleges was lower than that in key medical colleges. The existing problems and improvement suggestions in the mixed teaching proposed by students. In the general medical colleges, the top three items from high to low promotion frequency were: strengthening the assessment of process, more diversified types of mixed teaching, unreasonable arrangement. In the key medical colleges from high to low frequency, the top three items were: the content should be more perfect and standard, the mixed teaching content was not consistent with the professional direction, and the arrangement was unreasonable. Conclusion Harvest feeling are higher to the teaching of basic medical courses of the general medical colleges than key medical medical colleges. Student′s identity in mixed teaching of basic medicine of the general medical colleges far less than the key medical colleges. There are still many aspects to be improved in mixed teaching by way of study groups.

As the key team of medical development in China, medical graduate students need more attention due to the particularity of their occupational requirements. Especially on how to relieve psychological pressure, maintain mental health, which needs urgent attention. Medical graduate student education is an important part of higher education, with the quickly development of modern medicine, higher requirements are put forward for the scientific research ability of medical graduate students, and the scientific research pressure increased. Resilience means psychological recovery, which can decrease the scientific research pressure, the improvement of resilience can have a good effect on relieving the scientific research pressure. Combined with the practical problems of medical graduate students in medical university, this paper aims to improve the cultivation quality of medical graduate students and discuss the methods on how to improve the resilience of the medical graduate students against the present situation of the scientific research pressure from three levels, such as the school′s construction, mentor′s care and the medical graduate student′s subjective dynamics. This paper also discusses the theory and the classes of psychological health in university, aims to give the reference for educating the medical graduate students.

Study on the correlation between the condition of chronic kidney disease patients with stage 3-5 and α-Klotho, FGF23, calcium and phosphorus

Objective To investigate the changes of α-Klotho, fibroblast growth factor 23 (FGF23) and calcium and phosphorus levels in patients with stage 3-5 chronic kidney disease (CKD) and the correlation between CKD and α-Klotho, fibroblast growth factor-23 (FGF23) and calcium and phosphorus metabolism. Methods A total of 20 patients with CKD three to five stage who were admitted to the Department of Nephrology of Hubei Provincial Hospital of Traditional Chinese Medicine from June to December 2018 were selected as the CKD group. Another 22 healthy people were selected as the normal group. The serum levels of α-Klotho, FGF23, parathyroid hormone (PTH), estimation of glomerular filtration rate (eGFR) and calcium and phosphorus were measured in the two groups. Results The serum levels of phosphorus, PTH and FGF23 in the CKD group were higher than those in the normal group, and the levels of calcium, α-Klotho and eGFR were lower than those in the normal group, with statistically significant differences (P < 0.05). Compared with CKD3 stage, serum calcium and α-Klotho levels and eGFR in patients with stage 4 and 5 CKD stage decreased, while phosphorus, FGF23 and PTH levels were increased, with statistically significant differences (P < 0.05). The degree of CKD 3-5 stage disease was positively correlated with phosphorus, PTH, and FGF23 (r = 0.706, 0.963, 0.915, P < 0.05), and negatively correlated with calcium, eGFR, and α-Klotho (r = -0.821, -0.865, -0.718, P < 0.05). Conclusion The bone mineral metabolism disorder in patients with stage CDK 3-5 gradually worsened, and the severity of the disease is closely related to the levels of α-Klotho, FGF23 and calcium and phosphorus.

The diagnostic value of fractional exhaled nitric oxide in bronchial asthma and its relationship with disease severity

Objective To investigate the diagnostic value of exhaled fractional exhaled nitric oxide (FeNO) in patients with bronchial asthma and its relationship with disease severity. Methods A total of 140 patients with suspected bronchial asthma treated in the Department of Respiratory of Shijiazhuang First Hospital from January 2015 to June 2019 were selected as observation objects. According to the test results, they were divided into 77 cases in the control group (non-bronchial asthma) and 63 cases in the observation group (bronchial asthma), the observation group was divided into 21 cases of mild group, 25 cases of moderate group, and 17 cases of severe group according to the severity of bronchial asthma. The FeNO, blood oxygen saturation (SaO2) and peak expiratory flow rate (PEF) of each group were detected. Results The FeNO in the observation group was higher than that in the control group, SaO2 and PEF were lower than those in the control group (P < 0.05). FeNO level of the severe group was higher than that of the mild and moderate group, SaO2 and PEF were lower than those of the mild and moderate group (P < 0.05), FeNO level of the moderate group was higher than that of the mild group, and SaO2 and PEF were lower than those of the mild group (P < 0.05). Pearson correlation analysis showed that FeNO was negatively correlated with SaO2 and PEF (r = -0.171, -0.163,P = 0.032, 0.043). The best cutoff value for the diagnosis of bronchial asthma by FeNO was 46.88 μg/L, the sensitivity, specificity, positive predictive value, and negative predictive value of diagnosis were 72.36%, 91.45%, 93.07% and 75.21%, respectively. Conclusion FeNO has certain reference value in diagnosing bronchial asthma and judging the severity of the disease.

Analysis of correlation between serum hs-CRP, Cys C and PCT levels and prognosis of patients with pneumonia and heart failure

Objective To explore the relationship between the levels of serum high-sensitivity C-reactive protein (hs-CRP), cystatin C (Cys C), procalcitonin (PCT) and the prognosis of patients with pneumonia and heart failure. Methods Fifty patients with pneumonia and heart failure who admitted to the Second Affiliated Hospital of Wannan Medical College from March 2018 to June 2019 were included. The patients were followed up for six months and they were divided into good group (38 cases) and bad group (12 cases) according to the prognosis. Serum hs-CRP, Cys C and PCT levels were detected in the two groups, and the subject operating characteristic curve (ROC) was drawn to analyze the predictive value of each indicator for prognosis. The prognostic indicators of the patients were analyzed, including the changes of plasma brain natriuretic peptide (BNP), cardiac troponin I (cTnI) and left ventricular ejection fraction (LVEF). The correlation of hs-CRP, Cys C and PCT with prognostic indicators was analyzed by Pearson correlation. Results The levels of hs-CRP, Cys C and PCT, BNP and cTnI in the good group were lower than those in the bad group, and LVEF was higher than that in the bad group (P < 0.05). The AUC of hs-CRP, Cys C and PCT were 0.784, 0.723 and 0.792, respectively. The AUC of the combination of the three levels was 0.893. Serum hs-CRP, Cys C and PCT were positively correlated with plasma BNP and cTnI content (r = 0.528, 0.607, 0.596; 0.623, 0.604, 0.653, P < 0.05), but negatively correlated with LVEF (r = -0.633, -0.628, -0.639, P < 0.05). Conclusion The levels of hs-CRP, Cys C and PCT in patients with good prognosis of pneumonia and heart failure are significantly lower than those with poor prognosis, and they had certain predictive value for prognosis. The combined predictive effect is the best. The serum hs-CRP, Cys C and PCT levels are positively correlated with plasma BNP and cTnI levels and negatively correlated with LVEF levels.

Comparison of planned offline and empirical offline in ICU patients with cerebral hemorrhage

Objective To compare the effect of planned offline and empirical offline in patients with intensive care unit (ICU) cerebral hemorrhage. Methods A total of 150 patients with cerebral hemorrhage who admitted to the ICU of Bozhou People′s Hospital from January 2018 to December 2019 were selected and they were divided into the experience group and the planning group according to the random equalization method, with 75 cases in teach group. The experience group adopted experiential offline while the planning group adopted planned offline. The success rate, ventilation time and hospitalization time of the two groups were compared, and the incidence of complications was counted. Results In the planning group, the estimated mechanical ventilation time, full-process mechanical ventilation time, ICU stay time, and hospital stay were shorter than those in the experience group. The success rate and tolerance rate of the offline group were higher than those in the experience group. The total incidence of complications rate was lower than the experience group, and the difference was statistically significant (P < 0.05). Conclusion Compared with empirical offline, the complication rate of planned offline is low, and the success rate of offline is high, which is worthy of promotion.

Comparative study of minimally invasive percutaneous nephrolithotomy under local anesthesia and general anesthesia in the treatment of renal calculi in elderly patients

Objective To investigate the efficacy and safety of minimally invasive percutaneous nephrolithotomy (MPCNL) under local anesthesia and general anesthesia in the treatment of renal calculi in elderly patients. Methods From January 2019 to March 2020, 84 cases with unilateral renal calculi who received MPCNL treatment in the First Affiliated Hospital of Gannan Medical University were selected as study objects. They were divided into local anesthesia group (experimental group) and general anesthesia group (control group) according to the different anesthesia methods, with 42 patients in each group. 18 F cutaneous and renal channels were established by ultrasound-guided puncture, and holmium laser lithotripsy was performed in two groups. The operation time, postoperative fever, postoperative visual analogue scale (VAS) score, calculus residual rate, postoperative hospitalization time and hospitalization cost were compared between two groups. Results The operation time and postoperative hospitalization time of experimental group were shorter than those of control group, and VAS score and hospitalization cost were lower than those of control group, with statistically significant differences (all P < 0.05). There were no significant differences in the postoperative fever rate, calculus residual rate between two groups (P > 0.05). Conclusion Compared with MPCNL under general anesthesia, MPCNL under local anesthesia has the same therapeutic effect. MPCNL under local anesthesia has shorter operative time and hospitalization time, no need to resuscitate in the resuscitation room after surgery, less postoperative pain and hospitalization cost, which is worthy of clinical promotion and application.

Effect of different anesthesia inductions on the emergence delirium of preschool children and the anxiety level of their parents during operation

Objective To investigate the effect of different anesthesia inductions on the emergence delirium of preschool children and the anxiety level of their parents during operation. Methods From July 2018 to March 2019, 60 cases of children and their families to be circumcised under general anesthesia in the Second Affiliated Hospital of Guangxi Medical University were selected as study subjects. According to random number table method, they were divided into group K (30 cases) and group S (30 cases). Group K received intramuscular Ketamine for anesthesia induction before entering the operating room. Group S was included in anesthesia induction by inhaling 6% Sevoflurane + oxygen by mask after operating room. The modified Yale preoperative anxiety scale (M-YPAS), induction compliance checklist (ICC) scale, pediatric anesthesia emergence delirium (PAED) scale and modified children′s hospital of Eastern Ontario pain scale (m-CHEOPS) were compared between two groups. Scores of patients′ parents trait anxiety inventory (T-AI) scale and state anxiety inventory (S-AI) scale between two groups were compared. Results There were no significant differences in children′s m-YPAS score, m-CHEOPS score (P > 0.05); ICC score and PAED score in group S were lower than those in group K (all P < 0.05). There was no significant difference in T-AI score of patients′ parents (P > 0.05); and S-AI score of Group S was lower than that of group K (P < 0.05). ICC scores were positively correlated with S-AI scores and PAED scores (r = 0.450, 0.439, P = 0.004, 0.005). PAED scores were positively correlated with S-AI scores (r = 0.518, P = 0.001). Conclusion Compared with intramuscular Ketamine, induction with Sevoflurane is effective in reducing induction period ICC scores and post-operative PAED scores, as well as reduce anxiety levels in the families during the waiting period for surgery.

Effect of different CO2 pneumoperitoneum pressure on cognitive function of elderly patients after general anesthesia laparoscopic surgery

Objective To explore the influence of different CO2 pneumoperitoneum pressure on cognitive function of elderly patients after general anesthesia laparoscopic surgery. Methods From December 2017 to January 2019, 60 cases with elderly patients who undergoing general anesthesia laparoscopic surgery were selected in the Second People′s Hospital of Hefei. According to the pneumoperitoneum pressure, they were divided into low pressure group (6-8 mmHg), medium pressure group (9-11 mmHg), high pressure group (12-14 mmHg), with 20 cases in each group. The heart rate (HR), mean arterial pressure (MAP), arterial blood CO2 partial pressure (PaCO2) of three groups at before pneumoperitoneum, half an hour and one hour after pneumoperitoneum, half an hour after the end of pneumoperitoneum were compared; neuron specific enolase (NSE) and S100 β at ten minutes before anesthesia, end of operation, 6 h and 24 h after operation were compared; the mini mental state scale (MMSE) of three groups were evaluated at one day before surgery, 6 and 24 h after surgery. Results HR levels at half an hour and one hour after pneumoperitoneum in high pressure group were higher than those in low pressure group, with statistically significant differences (all P < 0.05). MAP levels at half an hour and one hour after pneumonperitoneum in middle and high pressure groups were higher than those in low pressure group, with statistically significant differences (all P < 0.05). The PaCO2 levels at half an hour and one hour after pneumoperitoneum in middle and high pressure groups were higher than those in low pressure group, and the PaCO2 levels at half an hour end of the pneumoperitoneum in high pressure group were higher than those in low pressure group, with statistically significant differences (all P < 0.05). The NSE protein levels at the end of operation and six hours after operation of high pressure groups were higher than those of low and middle pressure group, and middle pressure group was higher than low pressure group, with statistically significant differences (all P < 0.05). The S100β protein levels at the end of operation in middle and high pressure groups were higher than those in low pressure group and high pressure group was higher than middle pressure group, and the differences were statistically significant (all P < 0.05). MMSE scores of three groups were compared at different time points, and the differences were not statistically significant (all P > 0.05). Conclusion Different CO2 pneumoperitoneum pressure has no significant effect on the cognitive function of elderly patients after general anesthesia laparoscopic surgery, and hypobaric pressure may reduce the risk of postoperative cognitive dysfunction.

Systematic evaluation and meta-analysis of the efficacy and safety of Shuxuetong Injection in the treatment of vascular dementia

Objective To systematically evaluate the efficacy and safety of Shuxuetong Injection in the treatment of vascular dementia(VD). Methods Randomized controlled trial (RCT) were retrieved from CNKI, SinoMed, WanFang, VIP, PubMed, EMbase, Cochrane Library, Web of Science. RCT of Shuxuetong Injection in the treatment of VD was selected. The retrieval time was from the establishment of the database to May 2019. The data was searched and extracted by two researchers independently, and the quality of the included literatures were evaluated. Meta analysis was performed using RevMan5.3 software. Results A total of 662 patients were included in eight RCTs. Meta analysis showed that compared with control group, Shuxuetong could improve mini-mental state examination (MMSE) score [MD = 1.81, 95%CI (1.35, 2.27), P < 0.000 01], improve activity of daily living scale (ADL) score [MD = 3.23, 95%CI (2.03, 4.42), P < 0.000 01], shorten P300 latency [MD = 23.51, 95%CI (16.99, 30.03), P < 0.000 01] and increase amplitude [MD = 1.40, 95%CI (0.71, 2.08), P < 0.0001]. No serious adverse reactions occurred in all studies. Conclusion Shuxuetong adjuvant therapy for VD can better improve the cognitive function and the ability of daily living, with no serious adverse reactions. Due to such factors as short follow-up time, low quality, and small sample size, more high-quality, large-sample, multicenter and longer follow-up time RCT should be carried out.

Meta-analysis of the incidence of reproductive adverse events in the treatment of rheumatoid arthritis by Tripterygium wilfordii

Objective To systematic evaluate reproductive toxicity adverse of Tripterygium wilfordii in the treatment of patients with rheumatoid arthritis. Methods PubMed, EMbase, Cochrane Library, Web of Science, CNKI, CBM, Wanfang, VIP and other databases were searched. From the establishment of the database to January 2020, all randomized controlled trials of Tripterygium wilfordii in the treatment of rheumatoid arthritis were searched for the key words of “Tripterygium wilfordii multiglucoside”, “Tripterygium wilfordii polyglycoside”, “Tripterygium wilfordii”, “rheumatoid arthritis”, “rheumatoid arthritis”, “arthralgia syndrome” and “arthralgia”. According to the literature of reproductive toxicity, Jadad scale was used to score and RevMan5.3 software was used to analyze the incidence of reproductive toxicity. Results A total of 1454 articles were searched and analyzed. Twenty-two of them met the inclusion criteria. Total sample size was 2085 cases, including 924 cases in experimental group (Tripterygium wilfordii) and 1161 cases in control group (non Tripterygium wilfordii). Compared with control group, the incidence of menstrual disorder, amenorrhea and sperm motility decreased in experimental group increased (RR = 4.58, 95%CI: 2.65-7.92, P < 0.000 01; RR = 4.97, 95%CI: 1.81-13.63, P = 0.002; RR = 11.02, 95%CI: 2.73-44.44, P = 0.0007). Conclusion The side effects of reproductive system toxicity will appear in patients using Tripterygium wilfordii to a certain extent. However, due to the limited literature available, the above results need to be further verified by higher quality randomized controlled trials.

Effect of Xixian Tongshuan Pills combined with Edaravone in the treatment of ischemic stroke

Objective To explore clinical effect of Xixian Tongshuan Pills combined with Edaravone in the treatment of ischemic stroke. Methods Ninety-eight patients with ischemic stroke admitted to Beijing Hepingli Hospital from March 2017 to December 2018 were selected as research objects, they were divided into control group (49 cases) and study group (49 cases) according to the random number table. Control group was treated with Edaravone, and study group was treated with Xixian Tongshuan Pills and Edaravone. Clinical efficacy, inflammatory factor indexes [interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α), high sensitivity C-reactive protein (hs-CRP)], scale scores [improved Barthel index (BI), National Institutes of Health stroke scale (NIHSS)] and cerebral blood flow indexes (plasma viscosity, hematocrit, fibrinogen) were compared between two groups. The adverse reactions of two groups during the treatment were recorded. Results Total clinical effective rate of study group was higher than that of control group (P < 0.05). The levels of IL-6, TNF-α and hs-CRP in two groups after treatment were lower than those before treatment, and those in study group were lower than those in control group (P < 0.05). The plasma viscosity, hematocrit and fibrinogen of two groups after treatment were lower than those before treatment, and those in study group were lower than those in control group (P < 0.05). BI score of two groups after treatment was higher than that before treatment, and NIHSS score of two groups was lower than that before treatment (P < 0.05), and BI score of study group was higher than that of control group, and NIHSS score of study group was lower than that of control group (P < 0.05). There was no difference in the incidence of adverse reactions between two groups (P > 0.05). Conclusion The effect of Xixian Tongshuan Pills combined with Edaravone in the treatment of ischemic stroke is definite. It can effectively improve inflammatory factors and cerebral blood flow indexes, and has good safety, which has certain clinical application value.

Research and analysis of clinical application of acupoint massage

Objective To explore clinical application status of acupoint massage, and to provide reference and basis for the treatment of diseases with Chinese medicine technology. Methods CNKI, Wanfang med online, VIP and China biology medicine disc were searched from January 2010 to December 2018. Six kinds of nursing journal acupoint massage literature were collected. Information was searched, screened and extracted by four reviewers independently, the quality of literature was evaluated and cross checked. Database was established by Excel 2016 to conduct quantitative analysis on the data of acupoint massage treatment of diseases, acupoints and methods. Results A total of 195 literatures were included, including 18 specialties. The first was Obstetrics and Gynecology Department, followed by Encephalopathy Department, Cardiology Department and Orthopedics Department. The top six diseases and syndromes of hot research were pain, constipation, insomnia, anxiety and depression, oligogalatic/lactation, abdominal distension/nausea and vomiting. The acupoints with frequency≥15 were zús?觀nlǐ, nèigu?觀n and hégǔ, and other ten acupoints. Comprehensive therapy was a common method. Conclusion It is necessary to unify and standardize the treatment plan of acupoint massage and the treatment plan combined with other traditional Chinese medicine techniques. The common problems in other traditional Chinese medicine technical literatures are reflected from the research on acupoint massage operation.

Effect of Buyang Huanwu Decoction combined with enhanced lipid-lowering anticoagulant in the treatment of ischemic stroke with syndrome of qi deficiency and blood stasis

Objective To explore the effect of Buyang Huanwu Decoction combined with enhanced lipid-lowering and anticoagulation in the treatment of ischemic stroke with syndrome of qi deficiency and blood stasis. Methods Two hundred patients of ischemic stroke with syndrome of qi deficiency and blood stasis admitted to Anhui Huaibei People′s Hospital from December 2015 to December 2019 were selected as research objects. Simple random number table method was used to divide the patients into two groups, with 100 cases in each group. Two groups were given antihypertensive, hypoglycemic, antiplatelet aggregation and other conventional treatment. Control group was treated with intensive lipid-lowering and anticoagulant therapy, and observation group was treated with Buyang Huanwu Decoction for two weeks. After treatment, the effect of two groups was compared. National Institute of Health stroke scale (NIHSS) score, traditional Chinese medicine syndrome score, serum nerve growth factor (NGF), brain-derived neurotrophic factor (BDNF) level and quality of life score of two groups were recorded before and after treatment. Results Total effective rate of observation group was significantly higher than that of control group, the difference was statistically significant (P < 0.05). NIHSS score and traditional Chinese medicine syndrome score of two groups after treatment were lower than those before treatment, and observation group was lower than control group, the differences were statistically significant (P < 0.05). The levels of NGF and BDNF in two groups after treatment were higher than those before treatment, and observation group was higher than control group, the differences were statistically significant (P < 0.05). The scores of health status, physiological function, emotional function and social function of two groups after treatment were higher than those before treatment, and observation group was higher than control group, the differences were statistically significant (P < 0.05). Conclusion Buyang Huanwu Decoction combined with enhanced lipid-lowering and anticoagulation can effectively alleviate clinical symptoms of patients, inhibit the expression levels of serum NGF and BDNF, and improve the quality of life, which can be widely used in clinic.

Effects of early, middle and late three stages of Chinese medicine decoction gauze sequential dressing change on wound healing after low simple anal fistula operation

Objective To investigate the effect of early, middle and late three stages of Chinese medicine decoction gauze sequential dressing change on wound healing after low simple anal fistula operation. Methods One hundred and eighteen patients with low simple anal fistula from January 2018 to January 2020 in Lu′an Hospital of Traditional Chinese Medicine of Anhui Province were selected by using stratified sampling method. They were divided into two groups by using random number table method, with 59 cases in each group. Control group was treated with Vaseline Oil, and study group was treated with gauze of traditional Chinese medicine decoction at early, middle and late three stages. Visual analogue scale (VAS), wound bleeding score, wound exudate score and wound area were compared between two groups before and after treatment, and the wound healing rate, wound healing time and postoperative wound related complications were counted. Results The overall analysis showed that there were significant differences in VAS, wound bleeding and exudate score between groups, time point comparison and interaction (P < 0.05). It was suggested that the postoperative wound bleeding and exudation of two groups were significantly improved, and the pain became lighter and lighter. Further pairwise comparison, intra group comparison: VAS score of the patients in two groups decreased gradually compared with that before treatment in the first week, the second week and the third week after operation (P < 0.05). Comparison between groups: VAS of study group at the first week, the second week and the third week after operation was significantly lower than that of control group (P < 0.05). Intra group comparison: at the first week, the second week and the third week after operation, the wound bleeding, exudate integral and wound area of two groups were gradually reduced compared with those before treatment (P < 0.05). Comparison between groups: the wound bleeding, exudation integral and wound area of study group at the first week, the second week and the third week after operation were lower or smaller than those of control group (P < 0.05). The wound healing rate of study group was higher than that of control group at the third week after operation, and the wound healing time was shorter than that of control group (P < 0.05). The incidence of wound related complications in study group was lower than that in control group (P < 0.05). Conclusion The sequential dressing change of traditional Chinese medicine decoction gauze in the early, middle and late three stages after the operation of low simple anal fistula can reduce the degree of pain, reduce the blood and liquid oozing from the wound, improve the wound healing effect and reduce the risk of wound related complications.

Application of CT-guided coaxial needle biopsy in early qualitative diagnosis of pulmonary nodules

Objective To explore the application value and safety of CT-guided coaxial needle biopsy in the early qualitative diagnosis of pulmonary nodules. Methods A total of 122 patients with pulmonary nodules (≤3 cm) admitted to Fujian Provincial Cancer Hospital from January to December 2017 who underwent percutaneous biopsy with CT-guided coaxial needle biopsy were retrospectively analyzed. The efficacy and safety of this technique were evaluated through the analysis of puncture success rate, incidence of complications and related influencing factors. Results The puncture biopsy was successfully performed in 121 cases (99.18%), among them, 92 cases (75.4%) were malignant tumors, and the puncture was terminated in one case due to pneumothorax compression more than 20%. Among 122 cases, pneumothorax was found in 11 cases (9.02%). Three patients with lung compression more than 20% (2.5%) were improved by self-absorption after observation (without drainage). There were 14 cases of concurrent hemorrhage (11.5%), including one case (0.8%) of hemoptysis, which was improved after hemostasis treatment. There was no gas embolism and no needle implantation occurred in the follow-up period of two years. Pneumothorax was related to emphysema and obstructive pneumonia after pulmonary nodule puncture of patients (all P < 0.05). Hemorrhage after puncture was related to the size of emphysema and pulmonary nodules (all P < 0.05). Conclusion CT-guided coaxial needle biopsy has higher application value and safety in the early qualitative diagnosis of pulmonary nodules.

Diagnostic value of contrast-enhanced ultrasound combined with ultrasound tissue palpation for benign and malignant thyroid nodules of TI-RADS type 4

Objective To investigate the diagnostic value of contrast-enhanced ultrasound (CEUS) score combined with ultrasound tissue palpation in thyroid imaging report and data system (TI-RADS) type 4 thyroid nodule. Methods A total of 62 patients with thyroid nodules of TI-RADS type 4 in Qinghai Provincial People′s Hospital were selected as the research objects. CEUS, virtual touch tissue imaging (VTI) and virtual touch tissue imaging quantification (VTIQ) were performed preoperatively and shear wave velocity (SWV) was obtained. The receiver operating characteristic curve was drawn with pathology results as the gold standard, and the area under the curve (AUC) was compared to evaluate the diagnostic value of single application and combined application of the three methods for TI-RADS type 4 nodules. Results There were 66 nodules, 45 malignant nodules and 21 benign nodules. The cutoff value of CEUS score was four scores, and the sensitivity, specificity and accuracy were 86.7%, 81.0% and 84.8%, respectively. Ⅳ level for VTI cutoff value, sensitivity, specificity and accuracy were 84.4%, 81.0% and 83.3%, respectively. The average value of SWV in VTIQ technology was 2.70 m/s as the cutoff value and the sensitivity, specificity and accuracy were 80.0%, 85.7% and 81.8%, respectively. The sensitivity, specificity and accuracy of the combined application of the three methods were 93.3%, 85.7% and 90.9%, respectively. The AUC of the combination method, CEUS score, VTIQ technique and VTI were 0.952, 0.874, 0.847 and 0.800, respectively. The AUC of the combined method was the largest, and the sensitivity and accuracy of the combined method were higher than those of the single method, the differences were all statistically significant (all P < 0.05). Conclusion CEUS score combined with VTI and VTIQ can significantly improve the determination of the thyroid nodule nature of TI-RADS 4, providing an imaging reference for the detection of malignant nodules.

Feasibility of gastric contrast-enhanced ultrasound for high-level spinal cord injury and the evaluation of gastric emptying function

Objective To investigate the feasibility of contrast-enhanced ultrasound in patients with high-level spinal cord injury (SCI) and to evaluate gastric emptying function. Methods From March to December 2019, 33 patients (SCI group) and 30 healthy volunteers (control group) with high-level SCI and mechanical ventilation in the Department of Critical Care Medicine, Beijing Bo′ai Hospital, China Rehabilitation Research Center were selected for gastric contrast-enhanced ultrasound examination. The gastric wall structure of gastric fundus, gastric body, gastric antrum and gastric antrum peristalsis were observed. The peristalsis frequency and gastric emptying time were recorded. The results of gastric antrum peristalsis frequency and gastric emptying time were statistically analyzed. Results In the SCI group, 31 cases successfully completed contrast-enhanced ultrasound, and obtained complete images of gastric fundus, gastric body and gastric antrum. The peristalsis frequency of gastric antrum in SCI group was lower than that in control group and the gastric emptying time of SCI group was longer than that of control group, with all statistically significant differences (all P < 0.05). Conclusion Contrast-enhanced ultrasound can perform gastric examination and evaluate gastric emptying function in patients with high-level SCI and mechanical ventilation. Gastric antrum peristalsis is reduced and gastric emptying time is prolonged in patients with high-level SCI.

Effect of Jinshuibao Capsules combined with Levothyroxine Tablets on hashimoto thyroiditis and its effect on peroxidase antibody and inflammatory factors

Objective To investigate the efficacy of Jinshuibao Capsules combined with Levothyroxine Tablets in the treatment of hashimoto thyroiditis (HT) and its effect on peroxidase antibody and inflammatory factors. Methods From February 2014 to February 2019, 80 cases with HT patients who admitted to Ningxia Armed Police Corps Hospital were selected as study objects. According to the random number table method, they were divided into the study group (40 cases) and the control group (40 cases). The control group was treated with Levothyroxine Tablets, and the study group was treated with Jinshuibao Capsules on the basis of the control group, both groups were treated for two courses. The clinical efficacy, peroxidase antibody, thyroid function indexes and inflammatory factors were recorded, and the adverse reactions during the treatment were observed. Results The total effective rate of the study group was higher than that of the control group (P < 0.05). There were no statistically significant differences in the levels of thyroid peroxidase antibodies (TPOAb), thyroglobulin antibody (TGAb), free iodine thyroid glycine (FT3) and free thyroxine (FT4), thyroid stimulating hormone (TSH), interleukin 6 (IL-6), tumor necrosis factor-α (TNF-α), γ interferon (IFN -γ) before treatment (P > 0.05). After two courses of treatment, the levels of TPOAb, TGAb, FT3, TSH, IL-6, TNF-α and IFN-γ in the two groups were lower than those before treatment, and the study group was lower than the control group (all P < 0.05); FT4 level in the two groups was higher than that before treatment, and the study group was higher than the control group (all P < 0.05). There were no significant differences in the incidence of adverse reactions between the two groups (P > 0.05). Conclusion Jinshuibao Capsules combined with Levothyroxine Tablets can effectively improve the levels of peroxidase antibody and inflammatory factors, improve the thyroid function. At the same time, it has good safety and high clinical application value.

The efficacy and safety of Nitzumab combined with concurrent radiotherapy and chemotherapy in the treatment of locally advanced unresectable esophageal cancer

Objective To explore the efficacy and safety of Nitzumab combined with concurrent radiotherapy and chemotherapy in the treatment of locally advanced unresectable esophageal cancer, and to provide a reference for clinical treatment. Methods From January to December 2017, 94 cases with locally advanced unresectable esophageal cancer in the 902th Hospital of Chinese′s People′s Liberation Army were selected as the study subjects. They were divided into the observation group and the control group according to the random number table method, with 47 cases in each group. The control group was treated with concurrent radiotherapy and chemotherapy, and the observation group was treated with Nitzumab on the basis of the control group. The short-term efficacy, one-year and two-year survival rates, severity of toxic and side effects were compared between the two groups. Results The effective rate and control rate of the observation group were higher than those of the control group, and the differences were all statistically significant (all P < 0.05). The one-year and two-year survival rates of the observation group were higher than those of the control group, and the differences were statistically significant (P < 0.05). There were no significant differences in the severity of nausea and vomiting, rash, radiation pneumonia, radiation esophagitis and myelosuppression between the two groups(P > 0.05). Conclusion Nitzumab combined with concurrent radiotherapy and chemotherapy can improve the short-term efficacy and prolong the survival time of patients, and has the advantages of good tolerance and high safety, which has a application value.

Effect of Robocare′s full intervention mode in robotic assisted radical prostatectomy

Objective To investigate the effect of Robcare′s full intervention mode in robotic assisted radical prostatectomy. Methods A total of 102 patients with radical prostatectomy underwent robotic surgery from March 2017 to March 2019 were enrolled in Drum Tower Hospital Affiliated to School of Medicine. They were divided into control group and observation group according to the random number table, 51 cases in each group. The control group underwent routine intervention, and the observation group underwent the full range of Robcare intervention. The postoperative situation, negative emotion before and after intervention, and postoperative complication rate were compared between the two groups. Results The time of getting out of bed, the time of first anal ventilation, the time of defecation, the time of drainage, and the length of postoperative hospital stay were shorter in the observation group than in the control group, the differences were statistically significant (P < 0.05). The scores of self rating anxiety scale (SAS) and self rating depression scale (SDS) in the two groups on the admission were lower than on adnission those three months after discharge; the scores of SAS and SDS in the observation group were lower than those in the control group three months after discharge, the difference was statistically significant (P < 0.05). The incidence of total complication was lower in the observation group than in the control group, the difference was statistically significant (P < 0.05). Conclusion Robcare′ full-intervention model can be used in robotic-assisted radical prostatectomy to promote postoperative recovery, relieve negative emotions and reduce the incidence of complications. It is worthy of promotion.

Study on the preventive effect of Guanyuan acupoint massage combined with traditional Chinese medicine package therapy on the recovery of bladder micturition function and the prevention of urinary retention in parturients with analgesia delivery

Objective To explore the preventive effect of Guanyuan acupoint massage combined with traditional Chinese medicine package therapy on the recovery of bladder micturition function and the prevention of urinary retention in parturients with analgesia delivery. Methods From September 2017 to September 2019, 100 parturients with analgesia delivery in Zhejiang Hospital were selected and divided into study group and control group, 50 cases in each group. Routine treatment was carried out postpartum, such as postpartum health education, dietary care and urination guidance. The study group was treated with Guanyuan acupoint massage and traditional Chinese medicine package on the routine treatment basis. The spontaneous micturition, induced micturition and urinary retention of the two groups were observed, and the first postpartum micturition interval, micturition volume and bladder residual urine volume were recorded among autonomous urinator. Results The difference of urination mode between the two groups was statistically significant (P < 0.05). The incidence of postpartum urinary retention in the study group was significantly lower than that in the control group (P < 0.05). The interval of first urination was lower in the study group than that in the control group, the residual urine volume in the bladder was lower than that in the control group, the average urine flow rate was higher than that in the control group, the micturition score index was lower than that in the control group, and the micturition duration was shorter than that in the control group, all the differences were statistically significant (all P < 0.05). There was no significant difference in micturition volume between the two groups(P > 0.05). Conclusion Guanyuan acupoint massage combined with traditional Chinese medicine package can effectively promote the recovery of postpartum bladder micturition function and prevent the occurrence of urinary retention in parturients with analgesia delivery.

Effect of early multidisciplinary team in the prevention and treatment of acquired weakness in ICU

Objective To explore the effect of early multidisciplinary team in the prevention and treatment of acquired weakness in ICU. Methods From June 2018 to June 2019, 112 patients with mechanical ventilation in the ICU of Department of Intensive Care Unit, Shiyan Taihe Hospital (Affiliated Hospital of Hubei University of Medicine) were selected as subjects. A total of 57 patients from June to December 2018 were in the control group, and 55 patients from January to June 2019 were in the observation group. The control group was given routine nursing, and the observation group used early multidisciplinary collaboration for intervention. The incidence of intensive care unit acquired weakness, the score of Barthel index, the time of getting out of bed, the mechanical ventilation time and ICU staying time were compared between the two groups. Results In the observation group, the incidence of intensive care unit acquired weakness was significantly lower than that in the control group (P < 0.01), the score of Barthel index was significantly higher than that in the control group (P < 0.01), and the time of getting out of bed, the mechanical ventilation time and ICU staying time were significantly shorter than those in the control group (P < 0.01). Conclusion Early multidisciplinary team model can effectively reduce the incidence of acquired weakness in ICU, it is worthy of clinical application to improve patients′ self-care ability and shorten the initial time of getting out of bed, mechanical ventilation time and ICU staying time.

Effect of feedback health education combined with dietary intervention on patients with peptic ulcer

Objective To investigate the effect of feedback health education combined with dietary intervention on patients with peptic ulcer (PU). Methods From April 2017 to January 2019, 148 PU patients in the Third People′s Hospital of Hainan Province were selected as the study subjects, and the patients were divided into control group (74 cases) and observation group (74 cases) according to the random number table method. Both groups were given routine nursing, and the observation group was given feedback health education combined with diet intervention on the basis of the routine nursing, the PU health knowledge, helicobacter pylori positive rate, hospital stay, recurrence rate, treatment compliance and efficacy of two groups were compared. Results The scores of disease cause, clinical manifestation, diet and nutrition, rest and exercise, and prevention knowledge in the observation group were higher than those in the control group (all P < 0.05). Comparison of the positive rate of helicobacter pylori between the two groups showed no statistically significant difference (P > 0.05). The length of hospital stay and recurrence rate in the observation group were significantly lower than those in the control group (P < 0.05). The treatment compliance of the observation group on time medication, reasonable diet, regular sleep and rest, moderate exercise, smoking cessation and alcohol treatment compliance were significantly higher than that of the control group (P < 0.05). The effective rate of treatment in the observation group was significantly higher than that in the control group (P < 0.05). Conclusion Feedback health education combined with diet intervention can significantly improve PU patients′ understanding of PU related health knowledge, reduce the length of hospital stay and recurrence rate, and improve patients′ treatment compliance and efficiency, which is worthy of clinical promotion and application.

Correlation between cognition of safety culture on patients and career growth of nurse

Objective To investigate the status of awareness of safety culture on patients and career growth of nurse, and to analyze the correlation between them. Methods From May to June 2019, a survey of 500 nurses in three third-class A-level hospitals in Tianjin was conducted using the general data questionnaire, the hospital patient safety culture assessment scale (HSOPSC) and the career growth scale (CGS). In this study, a total of 500 questionnaires were distributed, 465 valid questionnaires, and the effective recovery rate was 93%. Results Compared the total scores of safety culture awareness and career growth of nurses with different ages, educational backgrounds, marital status, titles, positions, seniority and personnel relations, and the differences were statistically significant (P < 0.05). The total score of nurses for the safety culture on patient was (162.74±11.20) points, and the professional growth score was (54.25±6.18) points, while the total score of safety culture on patients was positively correlated with the total score of career growth (r = 0.570, P < 0.01). Except for the overall feelings of patient safety and scores of career goal progress scores, promotion spead, while staffing and career growth, all other dimensions were positively correlated (r > 0, P < 0.01). Multiple regression analysis showed that nurses′ academic qualifications, titles, and positions were the main factors affecting nurses′ career growth (P < 0.05); after controlling general data, inter-department collaboration, organizational learning and continuous improvement, openness of communication, intra-department cooperation, and errors, the non-punitive response and the management support for patient safety were the factors that affected the career growth of nurses (P < 0.05), which can explain 39.3% of the variation in nurse career growth. Conclusion The awareness of safety culture on patients of nurses has a positive effect on their professional growth. The manager should strengthen the safety culture construction and take effective measures to improve the awareness of safety culture on patients of nurses to promote their professional growth.

Influencing factors and correlation analysis of intensive care unit internship experience on professional identity of nursing students

Objective To explore the status of professional identity of nursing students before and after internship in the intensive care unit (ICU), to analyze the influencing factors of professional identity of nursing students before and after ICU internship, and to provide a basis for improving professional identity of nursing students. Methods Using cluster sampling method, a total of 136 nursing students who were internship in Nanjing First Hospital in 2018 to 2019 school year were selected. Before and after the ICU internship, the professional identity scales were used to conduct a questionnaire survey on nursing students, and the linear regression analysis was used to analyze the influencing factors of the change in the professional identity scale score. Results The difference of different ages and the main reason for choosing nursing majors in scores of professional identity change among nursing students were statistically significant (P < 0.05). Whether the internship plan for nursing students is reasonable, different internship stages, whether they have experienced patient rescue, the satisfaction with ICU internship and their own level improvement factors in scores of professional identity change among nursing students were statistically significant (P < 0.05). Nursing students before and after the ICU internship had statistically significant differences in scores of the self-confidence of the will to contribute to the society, the choice of career and growth, and the sense of pride required by the society and the total score of professional identity (P < 0.05). The main reasons for choosing a nursing major, different internship stages, whether the internship plan is reasonable, whether to experience patient rescue, satisfaction with ICU internship, and self-improvement level were the main factors in scores of professional identity change among nursing students before and after ICU internship (P < 0.05). Conclusion After ICU internship, the occupational recognition of nursing students is lower than that before internship. Nursing educators and managers should give measures to improve the occupational recognition of nursing students according to relevant factors.

Effects of evidence-based nursing intervention combined with personalized health education on stress psychology and self-management ability of patients with acute pancreatitis

Objective To explore the effect of evidence-based nursing intervention combined with personalized health education on stress psychology and self-management ability of patients with acute pancreatitis. Methods A total of 90 patients with acute pancreatitis admitted from January 2018 to December 2019 in the Affiliated 3201 Hospital of Medical College of Xi′an Jiaotong University, were selected as the research objects. They were divided into control group and observation group by a digital random table method, with 45 cases in each group. The control group adopted routine nursing intervention, while the observation group implemented evidence-based nursing intervention combined with personalized health education on the basis of the control group. The two groups compared the self-rating anxiety scale (SAS), self-rating depression scale (SDS) and self-management ability score scale score before and after 4 weeks of intervention, patient health knowledge mastery, compliance, and nursing intervention satisfaction were analyzed. Results The SAS and SDS scores of the two groups after intervention were lower than those before the intervention, and the observation group were lower than those of the control group, and the differences were statistically significant (P < 0.05). After intervention, the scores of self-management capacity index such as restrictive behavior, management behavior, psychological activity and state management were higher than before intervention, and the observation group were higher than those of the control group, and the differences were statistically significant (P < 0.05). The health knowledge mastery, compliance and nursing intervention satisfaction of the observation group were higher than those of the control group, and the differences were statistically significant (P < 0.05). Conclusion Evidence-based nursing intervention combined with personalized health education can effectively improve the anxiety and depression of patients with acute pancreatitis and enhance their self-management ability.

Effect of psychological elasticity support combined with cognitive nursing intervention on maintenance hemodialysis patients

Objective To study on the effect of psychological elasticity support combined with cognitive nursing intervention on maintenance hemodialysis patients and provide guidance for clinical treatment. Methods From February 2019 to February 2020, 75 patients with maintenance hemodialysis admitted to Anhui Wanbei Coal Power Group General Hospital were selected and divided into observation group 44 cases and control group 31 cases according to the random number table method. The control group was given routine nursing intervention, and the observation group was given psychological flexibility support and cognitive nursing intervention based on the control group nursing. After 4 weeks of nursing intervention, the self-rating depression scale (SDS), self-rating anxiety scale (SAS) scores, quality of life scores, and mental resilience scores were compared between the two groups. Results After nursing, the SAS and SDS scores of the two groups were significantly lower than before nursing, and those of the observation group were significantly lower than the control group (P < 0.05). The scores of optimism, self-improvement, tenacity, mental elasticity scale, vitality, physical function, mental health, emotional function, social function, and overall health of the two groups were significantly increased compared with those before nursing, and the observation group was significantly higher than the control group, the differences were statictionally significant (all P < 0.05). Conclusion Psychoelastic support combined with cognitive nursing intervention for maintenance hemodialysis patients can significantly improve patients′ bad mood and quality of life, which is worthy of clinical application.

Fusion of narrative medicine and general practice from the perspective of healthy China

General practice aims at promoting the overall health of residents. The development of healthy China can′t be separated from the development of general practice. General practitioners are the main practitioners of general practice and the main force of the healthy China strategy. Only general practitioners with high professional quality and high humanistic quality can shoulder this important task and promote the construction of the healthy China. Starting from the connotation of healthy China, this paper makes a comprehensive analysis of the background and related concepts of narrative medicine. It further points out that the fusion of narrative medicine into general practice can help to implement the “people-centered” concept in general medicine, help general practitioners and their service object to establish a long-term and stable doctor-patient relationship, and can ease the professional burnout feeling of general practitioners. In the end, it puts forward the way of cultivating medical narrative competence in general practice education from the perspective of healthy China, aiming at establishing a harmonious doctor-patient relationship in the long run and accelerating the pace of healthy China strategy.

Research on the integration strategy of medical service system under the background of urban public hospital reform

Objective To construct medical service integration strategy group under the background of urban public hospital reform. Methods According to the framework of dual-core theory, the preliminary scheme of integration strategy was determined by literature analysis. From September to December 2017, the strategy was screened and revised through two rounds of Delphi expert consultation. Results Twenty-seven experts from research institutions, hospitals and governments participated in two rounds of consultations based on the target sampling method. The expert authority coefficient was 0.87, the positive coefficient was 84% and 93%, the average importance of integration strategy was 3.96-4.67 and 4.04-4.88, and the coordination coefficient was 0.07 and 0.10 respectively (P < 0.05). After two rounds of expert consultation and research group discussion, 34 integration strategies were finally constructed in six categories, including system, organization, specialty, specification, function and clinical integration. Conclusion The strategy group of medical service system integration under the background of urban public hospital reform is systematic and scientific. To provide a more systematic strategic guideline for the construction and reform of medical alliance for urban public hospital managers and other stakeholders.
