Study on the effect and mechanism of total saponins of panax notoginseng on late-life anxiety
DU Yao1,2 LI Xin1 ZHANG Guixian1 YUAN Yajin1 ZHOU Ningna1
Objective To explore the effect and mechanism of total saponins of Panax notoginseng leaves (tSPNL) on late-life anxiety. Methods Fifty aging male mice (12 months) were reared in group (n = 15, normal control group) or isolation (n = 60) according to the random number table method. After 4 weeks, the isolated rearing animal were then randomly divided into model group (vehicle), Diazepam (BDZ) group, tSPNL 500 group (500 mg/kg tSPNL) and tSPNL 100 group (100 mg/kg tSPNL). All mice were intragastrically administered desired compounds once a day for consecutive 14 days. The anxiety-like behaviors were evaluated by elevated plus maze (EPM), and levels of GABA and BDNF in hippocampus were determined by enzyme linked immunosorbent assay after the behavior test. In another set, aging male mice (n = 135) were rearing isolated for 4 weeks and were then randomly divided into model (vehicle), BDZ group, fluoxetine group, tSPNL 500 group. After administration of desired compounds intragastrically once a day for 14 days, animals underwent intraventricular injection of either BDNF receptor TrkB antagonist K252a or GABAA receptor antagonist Bicuculline (Bic), and the anxiety-like behaviors were measured by EPM 1 hour later. Results Compared with model group, the percentage of open arm time (OT%) increased significantly in tSPNL 500 group (P < 0.01), and the levels of BDNF and GABA also increased in hippocampus significantly (P < 0.05 or P < 0.01). After intraventricular injection of K252a and Bic, OT% in tSPNL 500 group decreased significantly (P < 0.05). Conclusion tSPNL can alleviate the anxiety-like behaviors in aging mice, and the mechanism is involved in BDNF-GABA signaling pathway, which may restore the GABAergic plasticity by increasing BDNF levels, thereby reducing the anxiety susceptibility of aging mice.

LIU Mengdan JI Linlin YE Yang ZHU Chaoyu XIAO Yuanyuan GAO Qingge WEI Li
Objective To investigate the changes of the expression and function of glucagon receptor (GCGR) in db/db mice liver and hepatocellular carcinoma cell line HepG2 cells treated with high glucose. Methods Intraperitoneal glucose tolerance test (IPGTT) and intraperitoneal insulin tolerance test (IPITT) were performed in both db/db mice and control mice. The blood of mice in the two groups was collected for biochemical test; the expression of GCGR, protein kinase A (PKA) and phosphorylated protein kinase A (p-PKA) in the liver of the two groups were detected by Western blot or immunohistochemistry. The expression of GCGR in HepG2 cells treated with high glucose (30 mmol/L) in different glucose concentrations or different time gradients was detected by Western blot without stimulation of glucagon (GLN). Under GLN stimulation, the expression of GCGR, PKA and p-PKA proteins in HepG2 cells treated with high glucose at different time gradients was also detected by Western blot. Results Compared with control mice, the expressions of GCGR were markedly elevated in liver of db/db mice, but phosphorylated PKA levels were decreased (P < 0.05 or P < 0.01). GCGR protein expressions were increased following the growth of glucose concentrations in HepG2 cells, meanwhile, the expressions of GCGR protein were decreased at first, then increased with time extended (all P < 0.01). Under GLN stimulation, the GCGR protein levels were upregulated by high glucose in a time-dependent manner (P < 0.01). However, p-PKA/PKA levels were decreased compared with control group (0 h) (P < 0.05 or P < 0.01). Conclusion Chronic hyperglycemia can increase the expression of GCGR in the liver and lead to glucagon resistance in the liver.

Reserch progress of glutamate receptor and schizophrenia treatment
WANG Yingyi LYU Qinyu LU Yanhua YI Zhenghui
Schizophrenia is a group of serious mental diseases without specific causes, with a high prevalence rate, high disability rate and a heavy social burden. The global prevalence of schizophrenia is 1%. At present, antipsychotic drugs can well control the positive symptoms, but the negative symptoms and cognitive impairment of patients have not been effectively treated. Currently, it is generally believed that the defect of N-methyl D-aspartate receptor is the main cause of negative symptoms and cognitive impairment in patients with schizophrenia. Therefore, glutamate receptor has gradually become a key target for the development of a new generation of antipsychotic drugs. In this paper, we will summarize the progress of glutamate receptor-related drugs for schizophrenia in recent years in order to provide some suggestions for future clinic and researches in this field.

Correlation between the ratio of mean platelet volume to platelet count and left ventricular hypertrophy in patients with essential hypertension
YIN Jilai GAO Fei LI Hao
Objective To investigate the correlation between mean platelet volume (MPV)/platelet count (PLT) and left ventricular hypertrophy in patients with essential hypertension. Methods A total of 109 patients with essential hypertension admitted to the First Affiliated Hospital of Hainan Medical University from March 2016 to March 2018 were selected as subjects. All patients were examined by echocardiography and were divided into the left ventricular hypertrophy group (51 cases) and the non-left ventricular hypertrophy group (58 cases) according to the level of left ventricular myocardial mass index. The clinical data of gender, age, body mass index (BMI), systolic blood pressure and diastolic blood pressure were compared between the two groups. The levels of fasting blood glucose (FBG), triglyceride (TG), total cholesterol (TC), low density lipoprotein cholesterol (LDL-C), high density lipoprotein cholesterol (HDL-C), hemoglobin (HGB), red blood cell distribution width (RDW) and MPV/PLT were compared. Multivariate Logistic regression analysis was used to analyze the relationship between the above indexes and left ventricular hypertrophy in patients with essential hypertension. Results Systolic blood pressure in left ventricular hypertrophy group was higher than that in non-left ventricular hypertrophy group, the difference was statistically significant (P < 0.05). RDW in left ventricular hypertrophy group was higher than that in non-left ventricular hypertrophy group, while MPV/PLT in left ventricular hypertrophy group was lower than that in non-left ventricular hypertrophy group, the differences were statistically significant (P < 0.05). Multivariate Logistic regression analysis showed that systolic blood pressure and RDW were independent risk factor for left ventricular hypertrophy in patients with essential hypertension, while MPV/PLT was protective factor (P < 0.05). Conclusion MPV/PLT is closely related to left ventricular hypertrophy in patients with essential hypertension. Detection of MPV/PLT in clinical work can help prevent target organ damage.

Changes and clinical significance of serum EMPs and hs-CRP in patients with acute myocardial infarction
YUAN Xiaohong1 LI Peng2 LIU Junhui2
Objective To investigate the changes of serum endothelial-derived microparticles (EMPs) and hypersensitive C-reactive protein (hs-CRP) levels in patients with acute myocardial infarction (AMI) and its clinical significance. Methods A total of 146 patients with AMI hospitalized in the Second Affiliated Hospital of Xi′an Jiaotong University (“our hospital” for short) from January 2016 to January 2018 were selected as the observation group. Another 50 non-AMI patients with ventricular premature contractions who were treated in our hospital in the same period were selected as the control group. The levels of serum EMPs and hs-CRP were detected and compared between the two groups. Gensini score was used to evaluate the degree of vascular stenosis in the observation group, and coronary angiography was used to detect the number of vascular lesions in the observation group. The serum levels of EMPs and hs-CRP in patients with different Gensini score and vessel lesion branches were compared. The correlation between serum level of EMPs and hs-CRP and Gensini score and coronary artery lesion branches were analyzed. Results The levels of serum EMPs and hs-CRP in the observation group were higher than those in the control group, the differences were statistically significant (P < 0.05). The levels of serum EMPs and hs-CRP in the observation group increased gradually with the increase of Gensini score, the differences were statistically significant (P < 0.05). The levels of serum EMPs and hs-CRP in the observation group increased gradually with the increase of the number of vascular lesion branches, the differences were statistically significant (P < 0.05). The levels of serum EMPs and hs-CRP were positively correlated with Gensini score and the number of coronary artery lesions (EMPs: r = 0.583, 0.527; P < 0.05, hs-CRP: r = 0.634, 0.582, P < 0.05). Conclusion The levels of serum EMPs and hs-CRP in patients with AMI are abnormally elevated, and their level are closely related to the severity and extent of coronary artery involvement. EMPs, hs-CRP may be used as markers for judging the severity of AMI.

Analysis on the fairness of the allocation of TCM hospitals in 31 provinces of China in 2016
WANG Xi GUO Yiwei BAI Qian HUANG Lieyu LI Shuo YANG Yong SHI Xuefeng▲
Objective To analyze the fairness of the financial input and resource allocation of Chinese medicine hospitals in 2016, and provide suggestions for the improvement of fairness. Methods Descriptive statistical analysis, Gini Coefficient and Lorenz curve were used to analyze the relevant indicators in the Chinese Statistical Yearbook of Chinese Medicine 2016 and the Chinese Statistical Yearbook 2017 and the fairness of financial investment and resource allocation in the national TCM hospitals in 2016 were studied. Results Between the provinces, the proportion of investment in Chinese medicine hospitals and medical and health institutions varied greatly, with the highest ratio being 2.75 times that of the lowest ratio. The number of TCM hospitals per million people was the highest in Qinghai Province, which was 6.46/1 million. The number of beds per 10 000 population was the highest in Beijing, and the number of beds was 10.48 per 10 000 people. The population of 10 000 people had the largest number of practitioners, at 7.83/10, 000. Conclusion There is still a lack of emphasis on the development of traditional Chinese medicine in various provinces in China. It is imperative to implement the Party′s policy of "Be Equal to Chinese and Western Medicine". Increasing investment in TCM undertakings and taking into account efficiency in the context of ensuring fairness are a good way for the development of TCM practitioners.

Objective To investigate the bacterial biological characteristics of catheter-related bloodstream infection (CRBSI) in patients with central venous catheter (CVC) catheterization. Methods The bacterial biological characteristics of CRBSI in 16 841 patients with CVC catheterization in the Second Hospital of Lanzhou University from January 2012 to December 2015 were retrospectively analyzed. Hospital information system, laboratory information system and SPSS software were used to analyze the pathogenic microorganisms of CRBSI. Results There were 16 507 cases of single pathogen infection, 331 cases of double infection and 3 cases of triple infection. In total, 16 905 strains of pathogenic bacteria, 8709 strains of Gram-positive bacteria (51.4%), 7875 strains of Gram-negative bacteria (46.7%) and 321 strains of fungi (1.9%) were isolated. MRSA and CNS both showed strong resistance to Penicillin and Oxazacillin. Acinetobacter baumannii had a high resistance to antibiotics, Klebsiella pneumoniae had the highest resistance rate to Cefazolin (82.8%). The drug resistance rate of Pseudomonas aeruginosa to antibiotics was > 50%. Escherichia coli was highly resistant to Ampicillin and Cefazolin (> 70%). Conclusion Patients with CVC catheterization are prone to complicated CRBSI due to pathogenic bacteria infection or multiple complications, and may further develop sepsis or septic shock. The pathogens causing CRBSI are mainly Gram-positive bacteria, Gram-negative bacteria and fungi.
