12 The role of Hepatic insulin resistance in the pathogenesis of spontaneously diabetic OLETF rats
LI Jia1 XUE Yaoming2 PAN Yonghua2 ZHU Bo2 ZHANG Yan2
Objective To study the effects of hepatic insulin resistance, including the lipid deposition in livers, on preventing diabetes in Otsuka Long-Evans Tokushima Fatty (OLETF) rats, and explore the probability of new mechanism in preventing diabetes in laboratory animals. Methods Twenty male spontaneous diabetic OLETF rats were treated as OLETF (group OLETF). Ten male Long-Evans Tokushima Otsuka (LETO) rats were used as normal control group(group LETO). Blood glucose is determined by oral glucose tolerance test. Rats were sacrificed at 8, 32 and 40 weeks and liver tissue isolated. Serum triglyceride, serum cholesterol, and serum free fatty acid (FFA) were determined. The morphological changes of livers were measured by HE stain. Based on HE results, frozen liver sections of OLETF rats of 32-week-old and 40-week-old respectively were stained with fatty Sudan Ⅲ as well as PAS before and after amylase digestion, the purpose of which is to further clarify the nature of the lesions. Results OLETF rats developed diabetes since 23 weeks of age and the incidence rate of diabetes became higher with the time goes by until got to 92.9% at 40 weeks of age when the research came to the end. No rats have developed diabetes or IGT in group LETO. The serum triglyceride, serum cholesterol, serum FFA was significantly lower in group LETO than that of group OLETF since 32 weeks (P < 0.05). With the week age increasing, liver vacuolation in group OLETF became serious and it was suggested that there appeared a gradual increase on liver fat infiltration and glycogen accumulation with a further improvement on the specific staining of fat and glycogen; while there was no significant damage in the liver structure of group LETO. Conclusion Hepatic insulin resistance may play an important part in the onset of T2DM, and the improvement of hepatic insulin resistance may work as a probable mechanism of preventing diabetes.

17 Effects of high glucose state on oxidative stress in prostate cancer cells
ZHANG Zhitao1 ZHANG Hongyan2 NIU Fangfang1
Objective To explore the effects of high glucose state on oxidative stress in prostate cancer cells. Methods Human prostate cancer cell line LNCap was treated with different glucose concentrations, and the level of LNCap cells reactive oxygen species (ROS) was detected by DCFH-DA fluorescent probe. The hydrogen peroxide (H2O2), total superoxide dismutase (T-SOD), catalase (CAT) and glutathione peroxidase (GPX) kits were used to detect H2O2 level and antioxidant enzymes activities in LNCap cells. Western blot was used to detect the expression levels of SOD1 and SOD2 protein in LNCap cells. The level of H2O2 was detected after LNCap cells transfected with si-SOD2. Results The levels of ROS and H2O2 in LNCap cells were increased with the increase of glucose concentration, and were concentration-dependent (P < 0.05). The activities of T-SOD and CAT in LNCap cells  were increased significantly in the high glucose environment (P < 0.05), but there was no significant change in the activity of GPX (P > 0.05). In the high glucose environment, the expression level of SOD2 in LNCap cells was increased significantly (P < 0.05), but the expression level of SOD1 was not significantly changed (P > 0.05). After the SOD2 gene was silenced, the level of H2O2 in the LNCap cells was decreased significantly in the high glucose environment (P < 0.05). Conclusion High glucose state can promote the oxidative stress level of prostate cancer cells, and the increase of H2O2 level may be regulated by the expression of SOD2.

21 Research progress and clinical application prospect in 3D-sphere culture of mesenchymal stem cell with chitosan membrane
LI Huajie XU Xiongfeng QIU Bo
Mesenchymal stem cells (MSCs) with a wide range of sources and multipotential differentiation are thought as a new hope in the field of tissue repair and reconstruction. However, recent research shows that traditional adherent culture changes the initial growing environment of MSCs, leading to self-differentiation, stemness recession and other problems, which brings a negative effect to the treatment after in vivo transplantation. And the evidence suggests that, compared with the traditional adherent culture, the 3D-sphere culture plays a significant role in promoting the biological activity of MSCs. Chitosan membrane (CM) culture is a new type of biomaterial substrate-mediated multicellular spheroid cultivation for MSCs. This review discusses the CM culture of MSCs mainly from the change of morphology, biological functions and potential mechanisms, as well as its potential in clinical application.

83 Clinical diagnosis and genetic mutation analysis of hereditary medullary thyroid carcinoma with CEA levels abnormal elevation as the initial clinical manifestation
ZHANG Gang CHEN Yi XU Yan
In clinical work, patients with abnormal serum CEA (carcino-embryonic antigen) levels increased at first visit, in the conventional examination showed no signs of tumor, easily missed diagnosis of medullary thyroid carcinoma. PET/CT (positron-emission tomography/computer tomography) can provide clues for early localization of lesions. A case of abnormal increase of serum CEA level for the first time treated in Daping Hospital, the Army Military University, first of gastrointestinal tumors for examination had no system signs of tumor, further PET/CT examination revealed thyroid lesions. The postoperative pathology proved to be a thyroid medullary carcinoma. Further RET proto oncogene detection suggested that p.Cys634Tyr heterozygote mutation, two members of the family carrying the gene mutation. Both parathyroid hyperplasia and adrenal hyperplasia were found at the same time. The clinical manifestations of family patients may be in line with familial hereditary medullary thyroid carcinoma or multiple endocrine neoplasm type 2A (Family medullary thyroid carcinom/Multiple endocrine neoplasia type 2A; FMTC/MEN2A).

CHEN Xu ZHOU Wen WANG Lin LI Xiangchen CHEN Li
Objective To investigate the clinical isolates and their changes in antimicrobial resistance in the First Hospital of Liangshan Yi Autonomous Prefecture of Sicuan Province ("our hospital" for short) from 2015 to 2017. Methods Automated bacterial identification and drug sensitivity system was used to explore the clinical specimens of microbial pathogens in our hospital from January 2015 to December 2017, and the drug susceptibility to conventional antibiotics was determined. The data was analyzed using WHONET 5.6 software. Results A total of 6529 non-duplicated clinical isolates was collected. Of the isolated germs, Gram-positive and Gram-negative bacteria accounted for 31.75% and 68.25%, respectively. The Gram-negative bacteria were mainly composed of Escherichia coli [1399 (21.43%)], Acinetobacter baumannii haemolyticus [1017 (15.58%)], Klebsiella pneumonia [518 (7.93%)] and Pseudomonas aeruginosa [324 (4.96%)]. The most common gram-positive bacteria were Staphylococcus aureus [782 (11.98%)], Streptococcus pneumoniae [261 (4.00%)], Staphylococcus epidermidis [199 (3.05%)] and Enterococcus faecium [196 (3.00%)]. The positive rate of extended-spectrum β-lactamase (ESBL) producing Escherichia coli was lower than before, but ESBL producing Klebsiella pneumoniae increased slightly. In addition, resistant multiple antibiotics of Acinetobacter Baumannii haemolyticus was severely. There were no methicillin resistant staphylococcus aureus, but 20 strains (14.49%) methicillin resistant coagulase negative staphylococci were isolated in 2017. Conclusion The problem of antimicrobial resistance is still serious. Measures, such as rational use of antibiotics, monitoring of drug resistance and medical asepsis, should be further strengthened.
