4 Expert consensus of the whole blood viscosity testing by cone and plate method

At present, cone and plate method is the most widely used method to detect whole blood viscosity in clinical practice. Although the quality of cone and plate method to detect whole blood viscosity has been significantly improved in recent years, clinicians still often question the test results in daily work, which involves many factors such as instruments and equipment, calibration substances, and personnel operations. In order to standardize the cone and plate method for detecting whole blood viscosity, this consensus drafting group, through in-depth research on the law and quality management of the other side, and reference to relevant domestic and foreign literatures, put forward standardization suggestions for the pre-test, middle, and post-test stages of cone and plate method for detecting whole blood viscosity, and explain the reasonable interpretation of the test results, to promote the effective application of cone and plate method to detect whole blood viscosity in clinical diagnosis, disease early warning, and chronic disease management.

8 Effect of absence of natural killer cells and CD8+T cells on liver function in mice infected with metacestode of Echinococcus multilocularis

Objective To observe the effect of the absence of natural killer (NK) cells and CD8+T cells on the liver function in mice infected with metacestode of Echinococcus multilocularis. Methods A total of 30 SPF grade female C57BL/6 mice aged six to eight weeks and with a body weight of 18-20 g were selected and divided into control group, metacestode of Echinococcus multilocularis infection group, NK cell clearance group, NK cell clearance control group, CD8+T cell clearance group, and CD8+T cell clearance control group according to random number table method, with five mice in each group. Metacestode of Echinococcus multilocularis infection group, NK cell clearance group, NK cell clearance control group, CD8+T cell clearance group, CD8+T cell clearance control group were all injected into the portal vein with the scolices of Echinococcus multilocularis Leuckart (10 scolices/μl, 200 μl), and the control group was injected into the portal vein with equal amount of normal saline. At the fourth week of the experiment, the NK cell clearance group was intraperitoneally injected with NK cell clearance antibody, 100 μg each time, once a week, while the NK cell clearance control group was intraperitoneally injected with the same amount of IgG2a homologous control antibody; CD8+T cell clearance group was intraperitoneally injected with CD8+T cell clearance antibody 200 μg each time, once a week, while CD8+T cell clearance control group was intraperitoneally injected with the same amount of IgG2b homologous control antibody. At the 12th week of the experiment, the peripheral blood of six groups was collected, the peripheral blood plasma was separated, and the liver and kidney function indexes of total protein (TP), alkaline phosphatase, glutamic-pyruvic transaminase (GPT), glutamic-oxaloacetic transaminase (GOT), albumin (ALB), lactate dehydrogenase, uric acid, urea, and creatinine were detected. Results The levels of GPT and GOT in the peripheral blood of metacestode of Echinococcus multilocularis infection group were higher than those of control group, and the differences were statistically significant (P<0.05 or P<0.01). The levels of GPT and GOT in peripheral blood of NK cell clearance group were higher than those of NK cell clearance control group, and TP and ALB were lower than those of NK cell clearance control group, the differences were statistically significant (P<0.05 or P<0.01). The level of  TP inperipheral blood of CD8+T cell clearance group was lower than that of CD8+T cell clearance control group, and the difference was statistically significant (P<0.05). Conclusion Metacestode of Echinococcus multilocularis infection can cause liver function damage to a certain extent, especially in the state of NK cell depletion.

13 Study on the mechanism of Linggui Zhugan Decoction regulating macrophage polarization to improve non-alcoholic steatohepatitis

Objective To investigate the mechanism of Linggui Zhugan Decoction regulating macrophage polarization to improve non-alcoholic steatohepatitis (NASH). Methods A total of 24 SPF male SD rats, aged 6-8 weeks and weighing 180-200 g, were divided into blank group, model group, Linggui Zhugan Decoction group, and Polyene Phosphatidylcholine Capsules (Yishanfu) group according to random number table method, with six rats in each group. NASH rat model was induced by high fat diet. Linggui Zhugan Decoction group was given 3.465 g/(kg·d) Linggui Zhugan Decoction Formula Granules intragastric administration, Yishanfu group was given 143.640 mg/(kg·d) Yishanfu suspension intragastric administration, blank group and model group were given equal amounts of distilled water intragastric administration, once a day, for a total of four weeks. The changes of body weight, liver wet weight, liver index, liver histopathology, and the levels of M1 and M2 macrophages in the liver of rats were observed after treatment. Results Compared with blank group, the body weight, liver wet weight, and liver index of model group were increased, and the differences were highly statistically significant (P<0.01). Compared with model group, the body weight, liver wet weight, and liver index of Linggui Zhugan Decoction group were decreased, and the differences were highly statistically significant (P<0.01). Compared with blank group, the liver color of model group was whiter, hepatocyte balloon-like degeneration, steatosis, and inflammatory cell infiltration were increased. Compared with model group, the liver color of Linggui Zhugan Decoction group was obviously restored, hepatocyte balloon-like degeneration, steatosis, and inflammatory cell infiltration were decreased. Compared with blank group, M1-type macrophages in liver of model group were increased, the differences were highly statistically significant (P<0.01). Compared with model group, liver M1-type macrophages in Linggui Zhugan Decoction group were decreased, and the differences were highly statistically significant (P<0.01). There were no significant differences in liver M2-type macrophages between model group and blank group (P>0.05). There was no significant difference in liver M2-type macrophages between Linggui Zhugan Decoction group and model group (P>0.05). Conclusion Linggui Zhugan Decoction can significantly relieve the lipid deposition and inflammatory cell infiltration of NASH rat hepatocytes, which may play a role in the treatment of NASH by regulating macrophage polarization.

18 Discuss on the therapeutic effect and mechanism of sinomenine on osteo-arthritis

Objective To study the therapeutic effect and mechanism of sinomenine (SIN) on rabbit osteoarthritis (OA) model. Methods A total of 15 normal healthy adult New Zealand rabbits with body weight of 2.5-3.0 kg were randomly divided into control group, osteoarticular model group (OA group),  and sinomenine treatment group (SIN group), with five rabbits in each group. The control group was injected with normal Saline in joint cavity, the OA group and SIN group were injected with a mixture of 4% Papain injection and 0.03 mol/L L-cysteine (2∶1) at 0.1 ml/kg intraarticular cavity, respectively. Three injections were given on the 1st, 4th and 7th day respectively. After two weeks of modeling, the knee joint of rabbits showed swelling and movement disturbance, and the modeling was successful, after OA modeling was successful, SIN group was given 30 mg/(kg·d) SIN Hydrochloride sustained-release tablets by intragastric administration, the drug was administered continuously for 15 days. No special treatment was given to OA group and control group. Serum levels of interleukin-1β (IL-1β), tumor necrosis factor-α (TNF-α), matrix metalloproteinase-1 (MMP-1), MMP-13, Collagen Ⅱ,  and cartilage oligomer matrix protein (COMP) were detected in rabbit blood samples after four weeks. Joint pathological anatomy was observed. The expression levels of Collagen Ⅱ, polyproteoglycan and aging-related genes p21 and p53 were detected by immunohistochemistry in rabbit cartilage tissues of each group. Results After four weeks, the serum levels of IL-1β, TNF-α, MMP-1, MMP-13, Collagen Ⅱ, and COMP in OA group were higher than those in control group (P<0.05); the levels of IL-1β, TNF-α, MMP-1, MMP-13, Collagen Ⅱ, and COMP in SIN group were lower than those in OA group (P<0.05). Gross specimen observation showed that the joints of the control group were normal, the joint cavity of the OA group had an equal amount of joint effusion, synovial edema, and rough articular cartilage surface, while the joint cavity of the SIN group had less effusion and improved synovial congestion. Compared with the control group, the articular surface of the knee cartilage in OA group was not smooth, the cartilage was obviously thinner, the synovial membrane was infiltrated by a large number of inflammatory cells, and the synovial membrane hyperplasia and edema were obvious, the infiltration of synovial inflammatory cells, edema, and cartilage injury in SIN group were less than those in OA group. Immunohistochemical detection showed that the expression levels of p21 and p53 in OA group were higher than those in control group (P<0.05), and the expressions of p21 and p53 in SIN group were lower than those in OA group (P<0.05). The expression of polyproteoglycan in OA group was lower than that in control group, and the expression in SIN group was higher than that in OA group. There were no significant differences in the expression of type Ⅱ collagen among all groups (P>0.05). Conclusion SIN has a therapeutic effect on OA, which is related to inhibiting the secretion of inflammatory factors and MMP, and inhibiting the senescence of chondrocytes, providing a theoretical basis for the use of SIN in OA treatment.

24 Effects of dipsacus prescription added with Rhizoma Drynariae on bone tissue and PI3K/Akt signaling pathway in rabbits with steriod-induced

Objective To observe the therapeutic effect and mechanism of dipsacus prescription added with Rhizoma Drynariae (DPRD) on steroid-induced avascular necrosis of the femoral head(SANFH) rabbits. Methods Twenty-four 2.5-3.0 kg normal grade 4-week-old healthy male New Zealand rabbits were randomly number table method divided into blank group, model group, DPRD low-dose group (group L), and DPRD high-dose group (group H), with six rabbits in each group. The model group, group L, and group H were injected with E. coli endotoxin combined with glucocorticoid for modeling, and MRI was performed six weeks later to evaluate the modeling results. After successful modeling, blank group, and model group were administrated with 5 ml/kg normal saline daily, while group L, and group H were administrated with 2.5 ml/kg and 5.0 ml/kg DPRD decoction, respectively. After six weeks of intervention, MRI was performed, then the animals were killed and the femoral head was removed. The expression levels of phosphatidylinositol 3 kinase (Pi3K), seronine/threonine protein kinase (Akt), phosphorylated protein kinase (P-Akt), human forkhead protein O1 (FOXO1), and vascular endothelial growth factor (VEGF) in the femoral head of rabbits in each group were detected by immunohistochemistry and Western blot. Results After six weeks of modeling, MRI showed that the femoral head of the model group, group L, and group H had collapsed and flattened symptoms, and linear or circular high signal shadow appeared in the femoral head. After six weeks of DPRD intervention, MRI showed that DPRD could effectively improve femoral head necrosis in model rabbits. Western blot and immunohistochemical detection results showed that, compared with blank group, the expression levels of PI3K, Akt, and P-Akt were significantly increased, while the expression level of FOXO1 was significantly decreased in model group (P<0.05). Compared with model group, the expression levels of PI3K, Akt, and p-Akt in group L and group H were significantly decreased, while the expression level of FOXO1 was significantly increased (P<0.05). Compared with group L, the expression levels of PI3K, P-Akt, and FOXO1 in group H were increased (P<0.05), but there was no statistical significance in the expression level of Akt between group L and group H (P>0.05). In addition, compared with blank group, the expression levels of Akt and P-Akt were increased, and the expression level of VEGF was decreased in model group (P<0.05). Compared with model group, the expression levels of Akt and P-Akt in group L and group H were decreased, and the expression levels of VEGF were increased (P<0.05). There were no significant differences in Akt, P-Akt, and VEGF between group L and group H (P>0.05). Conclusion By regulating PI3K/Akt pathway, DPRD increases the expression of FOXO1, hormone-induced necrosis of femoral head VEGF and reverses the effect of glucocorticoids.

29 Genotype and homology analysis of carbapenem-resistant Acinetobacter baumannii in intensive care medicine

Objective To understand the genotypes of carbapenem-resistant Acinetobacter baumannii (CRAB) in patients with lower respiratory tract infection in the Department of Critical Care Medicine, Congjiang County People’s Hospital, Guizhou Province, and perform homology analysis on some strains. Methods A retrospective analysis of the proportion of CRAB in patients with lower respiratory tract infection in the Department of Intensive Care Medicine, Congjiang County People’s Hospital, Guizhou Province from January 2018 to December 2021, with 40 isolated CRAB strains as the research objects. VITEK2-compact automatic bacterial identification drug sensitivity system was used for bacterial identification and drug sensitivity. PCR was used to detect seven common carbapenemase genes (SHV, TEM, IMP, VIM, ADC, OXA-23, OXA-58). Multisite sequence typing (MLST) was used to analyze the homology of 40 strains. Results From 2018 to 2021, the detection rate of CRAB in patients with lower respiratory tract infection in the Department of Intensive Care Medicine of the Congjiang County People’s Hospital increased year by year. The 40 CRAB strains were 97.50% sensitive to Colistin and 80.00% sensitive to Amikacin. The detection of drug resistance genes revealed that the carrying rates of TEM, ADC, and OXA-23 genes were 47.5%, 22.5%, and 22.5%, respectively, as well as the presence of  expression of multiple genotypes. The results of MLST typing showed that there were five sequence types, among which ST195 was the main type, accounting for 62.5%, followed by ST208 type, accounting for 20.0%. Conclusion The detection of CRAB in patients with lower respiratory tract infection in the intensive care unit is increasing year by year, and the choice of antibiotics is limited. Carrying TEM, ADC and OXA-23 genes is the main reason for the resistance of Acinetobacter baumannii to carbapenems. MLST results shows that there are clones in the ICU ward.

34 Determination of Meropenem concentration in children’s trace cerebrospinal fluid by high performance liquid chromatography-tandem mass spectrometry

Objective To establish a rapid high performance liquid chromatography-tandem mass spectrometry (HPLC-MS/MS) method for the rapid determination of Meropenem in children’s trace cerebrospinal fluid. Methods Using Ertapenem as the internal standard, 10 μl cerebrospinal fluid samples were precipitated by 0.1% formic acid-acetonitrile. The liquid phase separation was performed on a Hypersil GOLDTM column (50 mm×2.1 mm, 3 μm) with 0.1% formic-water and 0.1% formic-acetonitrile as mobile phases, and gradient elution at the flow rate of 0.5 ml/min, and column temperature was 40℃; the detection was carried out by positive electrospray ionization mass spectrometry in multiple reaction monitoring mode. Samples collected from the Department of Neurology of Hebei Children’s Hospital from February 2020 to March 2020 were analyzed. Results The concentration of Meropenem in cerebrospinal fluid was in the range of 0.01-50.00 μg/ml, and the linear equation was Y=0.880X+0.000 168 (r=0.998 9), the lowest detection limit was 0.01 μg/ml; the extraction recoveries were (93.24±9.48)%-(104.62±4.63)%; Meropenem had no significant matrix effect in cerebrospinal fluid. The concentration of Meropenem in cerebrospinal fluid samples was 0.10-1.25 μg/ml. Conclusion The method is sample-saving, simple, rapid, accurate, and sensitive, and is suitable for the concentration monitoring of Meropenem in children’s trace cerebrospinal fluid.

38 Mechanism of iron accumulation and its research progress in neurodegenerative diseases

The iron-dependent accumulation associated with lipid peroxides in cells, mitochondrial atrophy, and increased mitochondrial membrane density are the main pathological features of ferroptosis. It is different from the cell necrosis, autophagy, and apoptosis. The mechanism of ferroptosis usually involved with the accumulation of lipids and lipid peroxides, the imbalance of amino acid antioxidant system, and the disorder of iron metabolism. Therefore, ferroptosis is considered as a novel programmed cell death process. The signaling of mevalonic acid, transsulfur, and heat shock protein may play important roles in the ferroptosis process. Existing studies have shown that ferroptosis is strongly associated with the development of some neurodegenerative diseases. But the exact mechanism of this form of cell death has not been fully explored. This review summarizes the latest research development of the role of ferroptosis in neurodegenerative system diseases, and further understanding of ferroptosis may inform future clinical treatment research of neurodegenerative diseases.

43 Research progress on the mechanism of geraniin against osteoporosis

The ability of geraniin (Ge) to inhibit the proliferation and differentiation of osteoclasts (OC) and promote the proliferation and differentiation of osteoblasts (OB), promotes bone marrow mesenchymal stem cells towards osteogenic differentiation. This paper reviews the literature and studies on Ge for osteoporosis by searching the databases of Vipshop, CNKI, and PubMed in recent years, and reviews the literature on the relevant cellular and signaling pathways involved in Ge, the mechanism of action of Ge against osteoporosis is found to promote proliferation and differentiation of OB mainly through upregulation of Wnt/β-catenin and expression of osteogenic transcription factor 2; inhibition of RANKL-induced nuclear factor-κB expression to exert inhibitory effects on OC differentiation. This paper further explores the mechanism of action of Ge against osteoporosis through a review, which provides new options and basis for the treatment of osteoporosis and research of new drugs.

47 Research progress on mechanism of chemotherapy induced myelosuppression and its prevention and treatment with traditional Chinese medicine

Myelosuppression is a common complication during chemotherapy. Patients may have immunosuppression, anemia, bleeding, and other clinical manifestations. In severe cases, patients cannot complete sufficient and full chemotherapy. The mechanism of myelosuppression mainly includes two aspects: on the one hand, chemotherapy affects the hematopoietic function of bone marrow by inducing the aging of hematopoietic stem cells and apoptosis of hematopoietic progenitors; on the other hand, chemotherapy can also destroy the hematopoietic ecological niche by inducing abnormal osteogenic-dipogenic differentiation of bone marrow, leading to abnormal signal pathways, disorder of hematopoietic growth factors, and affecting the hematopoietic microenvironment. In the treatment of myelosuppression, traditional medicine has gained clinical attention by virtue of its multiple targets, small side effects, low cost, and other advantages. Relevant clinical research and basic animal research are ongoing and have made some progress. Therefore, this article mainly reviews the mechanism of myelosuppression caused by chemotherapy and related research progress of traditional Chinese medicine prevention and treatment, providing reference for clinical chemotherapy drug selection and prevention and treatment of myelosuppression.

51 Research advances in drugs targeting endoplasmic reticulum stress for treatment of type 1 diabetes mellitus

Type 1 diabetes mellitus (T1DM) is an organ-specific autoimmune disease. Currently, there is a lack of radical treatment for T1DM. Endoplasmic reticulum stress is an important pathological feature of the early onset of T1DM and an important factor promoting its occurrence and development. Pathological endoplasmic reticulum stress is a new target for the treatment of T1DM. Drugs targeting pathological endoplasmic reticulum stress are conducive to restoring the quantity and quality of pancreatic β cells, so as to treat T1DM. Accumulation of misfolded proteins in the endoplasmic reticulum in islet β cells induces endoplasmic reticulum stress, which in turn activates three classical pathways of excessive unfolded protein response: inositol-requiring enzyme 1, proteinkinase R-like endoplasmic reticulum kinase, and activating transcription factor-6 pathways. This article reviews the progress of drug research targeting these three pathways and folded or misfolded proteins for the treatment of T1DM.

56 Research progress of pulmonary hypertension in risk stratification of acute pulmonary embolism

Acute pulmonary embolism (APE) has a high mortality rate in China. Early correct risk stratification of APE patients can improve the prognosis of patients. The traditional risk stratification is based on the patient’s hemodynamics as a reference. Currently, pulmonary arterial pressure in APE patients is a hot topic of clinical attention, and the incidence of clinical adverse events in APE patients with pulmonary arterial hypertension is generally increased. At the same time, some biomarkers, such as brain natriuretic peptide, D-dimer, cardiac troponin I are related to pulmonary artery pressure. These indicators also have reference value for risk stratification and prognosis of APE patients. This article reviews the pathophysiology and pulmonary artery pressure of APE, as well as the combination of the three biomarkers mentioned above, in the risk stratification and prognosis of APE patients, providing new ideas for APE risk stratification.

60 Research progress on external treatment of gout by traditional Chinese medicine

Gout is a common and frequently occurring disease in clinic. Conventional western medicine is expensive, many adverse reactions, the effect is uneven. Traditional Chinese medicine, especially the external treatment of traditional Chinese medicine, has irreplaceable advantages in alleviating the clinical symptoms of acute gout, improving the local inflammatory response and reducing the level of inflammatory factors in the body. This article summarizes the research progress of fumigation, external application, foot bath, enema, acupuncture, electric acupuncture, fire needle, warm acupuncture, blood pricking, moxibustion, and other treatment methods in the prevention and treatment of gout in recent five years, in order to provide more reference for the prevention and treatment of gout by traditional Chinese medicine, especially the external treatment of traditional Chinese medicine.

64 Research progress of local antimicrobial peptides preparation for chronic wound treatment

Antimicrobial peptides (AMPs) have good antibacterial properties and promote wound healing, and are potential substitutes for traditional antibiotics. However, AMPs is sensitive to the external environment and has a short action time, which is limited in local applications. The novel local AMPs preparation formed by carrier delivery or change of dosage form can achieve the purpose of slow release of AMPs, enhance the stability of AMPs and optimize the biological efficiency, and show important advantages in the clinical application of chronic wounds. To date, a variety of AMPs preparation have been developed in the form of nanoparticles, hydrogels, nanoparticle supported hydrogels, and self-assembling peptides, etc. In this paper, the general situation, antibacterial activity, and wound healing promoting properties of AMPs, preparation type of local AMPs preparations currently developed, and efficacy influencing factors are reviewed. The future development prospect of AMPs is discussed.

69 Application of BOPPPS blended in Gamification idea teaching model in teaching of internal medicine for MBBS overseas students

Under the call of “study in China” from the Ministry of Education, MBBS has became an inevitable topic of reform in education. As the vital course of MBBS, internal medicine is particularly urgent to reform teaching methods. In the current teaching model, how to mobilize students’ initiative, pay attention to two-way feedback, and effectively improve teaching efficiency is an important issue that various medical schools need to explore. This article combines the teaching experience of many years in the past and the early teaching reform achievements, combines the idea of Gamification with the teaching model of BOPPPS, and applies it to the teaching of MBBS internal medicine. Taking the chapter on heart failure as an example, this article discusses the impact of this teaching model on the teaching effect. The results show that after adopting the Gamification idea and the BOPPPS teaching model, students’ learning interest and efficiency significantly increase, and teacher-student interaction is enhanced, achieving satisfactory teaching results, and which is worthy of promotion.

Improving the teaching quality of internal medicine in the military medical university and better serving the military medical service are the key content of the teaching reform in the military medical university, while exploring the teaching quality evaluation system that reflects the characteristics of military medicine is the core link to improve the teaching quality. This research attempts to integrate the military medical content with the characteristics of the air force into the teaching while doing a good job in the construction of the basic course of internal medicine, add the corresponding practice link, integrate it into the assessment system, combine the qualitative and quantitative indicators of the course assessment system, improve the design of the assessment content, and reasonably balance the weight of the indicators. The teaching quality evaluation system of internal medicine suitable for the characteristics of military medical university has been established and initially implemented, and certain results have been achieved. Through the exploration of the teaching quality evaluation system of internal medicine with military medical characteristics, it can make a useful attempt to improve the clinical medical teaching quality of military medical universities, enhance the ability of graduates to serve the army better.

76 Clinical application of synchronous clinical practice teaching mode of traditional Chinese medicine pediatrics under information platform

Objective To explore the clinical application of synchronous clinical practice teaching mode of traditional Chinese medicine pediatrics under the information platform. Methods Sixty interns of traditional Chinese medicine pediatrics in grade 2020 from the Affiliated Hospital of Changchun University of Chinese Medicine were selected and divided into experimental group and control group by random number table method, with 30 cases in each group. The control group adopted the traditional teaching mode, and the experimental group adopted the synchronous clinical practice teaching of traditional Chinese medicine pediatrics under the information platform. After a year of practice, the results of theoretical knowledge and practical ability assessment, students’ and teachers’ teaching satisfaction were compared between the two groups. Results After intervention, the test scores of theoretical knowledge and practical ability of the two groups were higher than those before intervention, while those of the test group were higher than those of the control group, and the differences were statistically significant (P<0.05). The teaching form, teaching content, and teaching evaluation satisfaction of experimental group were higher than those of control group, and the differences were statistically significant (P<0.05). There were no significant difference in the satisfaction of troubleshooting between the two groups (P>0.05). The teaching satisfaction of the experimental group was better than that of the control group, and the difference was statistically significant (P<0.05). Conclusion The establishment of clinical practice teaching mode of traditional Chinese medicine pediatrics under the information platform is more conducive to improving the overall teaching quality than the traditional teaching mode.

80 Application of Miller Pyramid teaching method and CBL teaching method in the teaching of nursing students in the operating room

Objective To explore the application effect of Miller Pyramid teaching method combined with case-based learning (CBL) teaching method in the teaching of nursing students in the operating room, in order to provide new ideas for the teaching methods of nursing students in the follow-up practice. Methods This study adopted a prospective randomized controlled trial method, and included 146 nursing students who practiced in the Operating Room of Hefei Second People’s Hospital from August 2019 to August 2020 as research objects. They were divided into control group (73 students) and observation group (73 students) according to random number table method. The control group adopted CBL teaching method, and the observation group adopted Miller Pyramid teaching method combined with CBL teaching method. At the end of the teaching, the two groups were compared in terms of assessment (theoretical knowledge and operational skills), post competence of nurses, emergency response ability, and satisfaction. Results The test scores of the observation group were higher than those of the control group, and the differences were statistically significant (P<0.05). The nursing practice dimension and interpersonal communication ability scores of the observation group were higher than those of the control group, and the differences were statistically significant (P<0.05). Observation and judgment ability, emergency handling ability, and emergency cooperation ability scores of the observation groups were higher than those of the control group, and the differences were statistically significant (P<0.05). The teaching effect satisfaction of the observation group was better than that of the control group, and the difference was statistically significant (P<0.05). Conclusion The combination of Miller Pyramid teaching method and CBL teaching method can improve the teaching effect more effectively, further improve the professional theoretical knowledge and operational skills of nursing students in operating room practice, and effectively strengthen nursing students’ nursing practice, interpersonal communication and emergency response ability.

85 Construction of Nomogram prediction model for risk of pulmonary thromboembolism in elderly patients with acute exacerbation of chronic obstructive pulmonary disease

Objective To analyze the risk factors of pulmonary thromboembolism (PTE) in elderly patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD), and construct a Nomogram risk warning model. Methods A retrospective analysis of 265 elderly patients with AECOPD admitted to Xinjiang Uygur Autonomous Region People’s Hospital from January 2019 to June 2021 were conducted, they were divided into PTE group and non-PTE group according to whether PTE occurred. General data, laboratory indicators, and underlying diseases of two groups were collected, and the risk factors of PTE in elderly AECOPD patients were analyzed; the Nomogram risk model to predict the risk of PTE in elderly patients with AECOPD based on the risk factors and its predictive efficacy was verified. Results Among 265 elderly patients with AECOPD, the incidence of PTE was 20.38%. There were no significant differences in gender, body mass index, drinking history, smoking history, hypertension, diabetes mellitus, cerebrovascular disease, coronary heart disease, atrial fibrillation, hemoglobin, and white blood cell count between two groups (P>0.05); there were statistically significant differences in age, history of venous thrombosis, lower limb swelling, lower limb pain, fever time, bed time, D-dimer (D-D), arterial partial pressure of carbon dioxide (PaCO2), and arterial partial pressure of oxygen (PaO2) between two groups (P<0.05). Age>69 years old (OR=3.205), history of venous thrombosis (OR=4.035), lower limb swelling (OR=3.149), lower limb pain (OR=8.193), fever time≥3 d (OR=2.323), bed time ≥7 d (OR=3.451), D-D≥200 μg/L (OR=2.426), PaCO2<60 mmHg (OR=3.786), and PaO2<39 mmHg (OR=2.860) were independent risk factors for PTE in elderly patients with AECOPD (P<0.05). The equation was Y=-4.780+1.165×age+1.395×history of venous thrombosis +1.147×lower limb swelling+2.103×lower limb pain+0.843×fever time +1.239×bed time + 0.886×D-D+1.331×PaCO2+1.051×PaO2. The Nomogram prediction model to predict the risk of PTE in elderly patients with AECOPD showed that the model consistency index was 0.862; the area under the curve of receiver operating characteristic curve of the model was 0.851. Conclusion Age, history of venous thrombosis, lower extremity swelling, lower extremity pain, fever time, bed time, D-D, PaCO2, and PaO2 are closely related to the occurrence of PTE in elderly AECOPD patients. The Nomogram prediction model based on these risk factors can effectively predict the risk of PTE in elderly patients with AECOPD.

92 Expression and clinical significance of serum SDC-1 and CitH3 levels in elderly patients with sepsis

Objective To investigate the expression and clinical significance of serum syndecan-1 (SDC-1) and citrullinated histone H3 (CitH3) in elderly patients with sepsis. Methods A total of 125 elderly patients with sepsis admitted to Fukuang General Hospital of Liaoning Health Industry Group (“our hospital” for short) from August 2017 to September 2022 were selected as the sepsis group. According to the severity of the condition, they were divided into general sepsis group (51 cases) and septic shock group (74 cases). According to the 28 day prognosis, they were divided into death group and survival group. In addition, 50 healthy individuals who underwent physical examination in our hospital during the same period were selected as the control group. The serum levels of SDC-1 and CitH3 were measured; multivariate logistic regression was used to analyze the influencing factors of poor prognosis in elderly patients with sepsis, and receiver operating characteristic curve (ROC) curve analysis was used to analyze the predictive value of serum SDC-1 and CitH3 for poor prognosis in elderly patients with sepsis. Results The levels of serum SDC-1 and CitH3 in the sepsis group were higher than those in the control group (P<0.05). The levels of serum SDC-1 and CitH3 in septic shock group were higher than those in general sepsis group (P<0.05). The levels of serum SDC-1 and CitH3 in the death group were higher than those in the survival group (P<0.05). Increasing age, septic shock, organ injuries≥3 concurrent, prolonged anti infection initiation time, increased sepsis-related organ failure assessment scores, elevated SDC-1, and elevated CitH3 were independent risk factors for poor prognosis in elderly patients with sepsis (P<0.05). The ROC curve analysis showed that the area under the curve of SDC-1 combined with CitH3 in predicting poor prognosis in elderly sepsis patients was larger than that of SDC-1 and CitH3 alone (P<0.05). There was no difference between the SOFA score and the area under the curve of SDC-1 combined with CitH3 in predicting poor prognosis in elderly sepsis patients (P>0.05). Conclusion The levels of serum SDC-1 and CitH3 in elderly patients with sepsis are elevated, and the combined detection of the two can be used as predictor of poor prognosis in elderly patients with sepsis.

97 Effect of early rehabilitation on neurodevelopment and swallowing function in children with severe craniocerebral trauma and dysphagia

Objective To investigate the effect of early rehabilitation on neurodevelopment and swallowing function in children with severe craniocerebral trauma and dysphagia. Methods A total of 84 children with severe craniocerebral trauma and dysphagia admitted to Hunan Provincial Children’s Hospital from January to October 2022 were selected and divided into control group (conventional treatment) and observation group (conventional treatment combined with early rehabilitation treatment) by random number table method, 42 cases in each group. Both groups were treated for four weeks. The dysphagia degree and nutritional status of the two groups before and after treatment were assessed by the Kubota water swallowing test, dysphagia disorders survey scale (DDS), weight for height Z score (WHZ), and the neurodevelopment of the two groups was assessed by gross motor function measure-88 (GMFM-88) and developmental quotient (DQ) of Gesell developmental diagnostic scale. Results After treatment, the scores of Kubota water swallowing test and part two of DDS in two groups were lower than those before treatment, and those of observation group were lower than those of control group (P<0.05). After treatment, WHZ, GMFM-D part score, GMFM-E part score, gross motor DQ, and fine motor DQ in two groups were higher than those before treatment, and those of observation group were higher than those of control group (P<0.05). Conclusion Early rehabilitation therapy has significant effect on children with severe craniocerebral trauma and dysphagia, which can effectively improve nutritional status and promote the recovery of neurodevelopmental swallowing function.

101 Predictive value of fibrin degradation product and D-dimer in short-term prognosis of septic shock patients

Objective To investigate the predictive value of fibrin degradation product (FDP) and D-dimer on the short-term prognosis of septic shock patients. Methods A total of 86 patients with septic shock admitted to Anqing Municipal Hospital in Anhui Province from January 2019 to July 2022 were selected as the study subjects, and they were divided into survival group (65 cases) and death group (21 cases) according to their survival conditions 28 days after admission. The clinical data and the differences of serum FDP and D-dimer within 48 h after admission were compared between the two groups, and the influencing factors of prognosis in patients with septic shock were analyzed, and their predictive value for short-term prognosis in patients with septic shock was further analyzed. Results Acute physiology and chronic health evaluationⅡ (APACHEⅡ) score, sepsis-related organ failure assessment (SOFA) score, and levels of FDP, D-dimer in death group were higher than those in survival group, and platelet count was lower than that in survival group (P<0.05). Logistic multivariate analysis results showed that APACHEⅡ score (OR=4.116, 95%CI: 1.393-12.163), SOFA score (OR=4.003, 95%CI: 1.355-11.827), and levels of FDP (OR=4.051, 95%CI: 1.371-11.970) and D-dimer (OR=3.991, 95%CI: 1.351-11.792) were the influential factors for death within 28 days of admission in septic shock patients (P<0.05). The results of receiver operation characteristic curve analysis showed that the area under the curve of FDP and D-dimer combined predicted the short-term prognosis of septic shock patients was higher than that of FDP and D-dimer alone (P<0.05). Conclusion Levels of FDP and D-dimer are associated with the short-term prognosis of patients with septic shock, and could be used to predict the risk of death within 28 days of admission.

Objective To systematically evaluate the efficacy and safety of massage and massage combined with other traditional Chinese medicine external treatment methods in treating scapulohumeral periarthritis (SP) in adhesive period. Methods Randomized controlled trial (RCT) of massage related therapy in the treatment of SP in adherent period were retrieved from CNKI, Wanfang Data, China Biomedical Literature System, VIP, PubMed, Web of Science, and Cochrane Library from inception to September 2022, quality evaluation and data extraction were carried out for the selected literatures. Clinical effective rate, visual analogue scale (VAS) score, and adverse reactions were used as outcome indexes, and Stata 15.0 was used for data analysis. Results A total of 18 RCTs were included, involving 1 378 patients, including seven interventions: conventional western medicine treatment, simple massage, massage + acupuncture, massage + akupotomye, massage + warming needle moxibustion, massage + external use of traditional Chinese medicine, massage + floating acupuncture. The top three interventions in the surface under the cumulative ranking curve (SUCRA) of clinical effective rate were massage + akupotomye (87.4%), massage + floating acupuncture (70.5%), massage + warming needle moxibustion (65.9%). In terms of reducing VAS score, the top three SUCRA interventions were massage + floating acupuncture (83.3%), massage + akupotomye (69.7%), massage + external use of traditional Chinese medicine (66.9%). Two studies included in RCTs reported adverse reactions, which disappeared after symptomatic treatment. Conclusion Massage + akupotomye and massage + floating acupuncture has the better effect on the clinical effective and pain improvement in the treatment of SP in adhesion period, but the safety of each therapy needs to be further compared.

Impotence caused by alcohol is a common type of impotence, which has its own unique disease regularity. Jiao Yongzheng researcher from the long-term clinical experience summed up that the alcohol of pungent, dryness, and heat nature, damage to the qi, blood, and essence, and then lead to impotence. Its disease position gradually deepens, from the meridian into the collaterals to the viscera, and gradually occurs the pathogenesis evolution of “liver and spleen deficiency stagnation in the qifen”, “penile meridian blockage in the xuefen”, and “kidney essence deficiency loss in the jingfen”. In terms of treatment, Jiao Yongzheng researcher put forwards four treatment principles and methods, the first is to “dissipate the toxins of alcohol” as the core; the second is to transport spleen and disperse stagnated liver qi, tonify the qi of the middle-jiao and promote diuresis; the third is to cool blood for removing blood stasis, sweeten and moisten the penile meridian; the fourth is to fill the essence and draw the medicine to the penile meridian. Objective To visually analyze the research on Buzhong Yiqi Decoction based on CiteSpace software. Methods The literatures on Buzhong Yiqi Decoction in the field of traditional Chinese medicine collected by CNKI from January 2011 to May 2021 were searched as the objects, and the literature metrology analysis was carried out using CiteSpace 6.1.R2 software. The literature trends, subject distribution, author cooperation situation, keyword co-occurrence, clustering, and emergence situation of the included literatures were analyzed respectively, and the corresponding knowledge graph was drawn, and the visual analysis results were discussed in depth. Results A total of 2 313 literatures were included, and the publication volume showed a steady increase on the whole, among which the publication volume of Chinese medicine, Chinese pharmacy, and obstetrics and gynecology was higher. There were a total of 23 core authors in this field, and a research team of a certain scale had been formed, but there was a lack of communication between the teams. The keywords with high frequency included clinical effect, Yiqi Decoction, clinical observation, constipation, acupuncture and moxibustion, forming 12 clusters. “Acupuncture”, “adverse reactions”, and “cancer-related fatigue” were the key words that had emerged so far. Conclusion Buzhong Yiqi Decoction is a classic prescription used in traditional Chinese medicine to treat a variety of diseases, which has attracted extensive attention from scholars at home and abroad in recent years, and meta-analysis, acupuncture, moxibustion, and other research methods have become research hotspots, but there are still many scientific problems to be solved, which are of great significance for its clinical application. The theory of “Chuan-she” in Chinese medicine originates from the Huangdi Neijing and is an important theory in modern Chinese medicine to understand the recurrence and metastasis of tumors. Professor Wu Xianwen understands breast cancer metastasis from the theory of “Chuan-she”, and believes that breast cancer metastasis is rooted in vital qi deficiency and cancer toxicity, and it is combined with other evils such as qi stagnation,phlegm coagulation, and blood stasis to metastasize through the “channels” system such as meridians. 123 Clinical effect of Zhiqiao Chenpi Decoction on functional dyspepsia with liver-stomach disharmony

Objective To evaluate the clinical effect of Zhiqiao Chenpi Decoction on functional dyspepsia (FD) with liver-stomach disharmony. Methods Ninety-four FD patients with liver-stomach disharmony who attended the Shanghai Baoshan Hospital of Integrated Traditional Chinese and Western Medicine from July 2020 to March 2022 were selected as study subjects, and they were divided into experimental group and control group by random number table method, with 47 cases in each group. The experimental group was given Zhiqiao Chenpi Decoction Herbal Granules, and the control group was given Zhiqiao Chenpi simulated granules. The course of medication for each group was four weeks. The clinical effect, FD subtype score and total symptom score, traditional Chinese and medicine syndrome effect, quality of life score, and psychopsychological score were compared between the two groups. Results After the trial, six cases fell off in the experimental group and 41 cases remained, five cases fell off in the control group and 42 cases remained. The clinical effect of experimental group was better than that of control group, the difference was statistically significant (P<0.05). After treatment, the total scores of postprandial distress syndrome, epigastric pain syndrome and FD symptoms in two groups were lower than before treatment, and the experimental group was lower than the control group, the differences were statistically significant (P<0.05). After treatment, the score of postprandial fullness of the two groups was lower than that before treatment, and the experimental group was lower than the control group(P<0.05), and the score of early fullness of the experimental group was lower than that before treatment (P<0.05), there was no statistical significance in the scores of upper abdominal pain and upper abdominal burning between the two groups compared with that before treatment and between the two groups (P>0.05). The curative effect of traditional Chinese and medicine syndrome in experimental group was better than that in control group, the difference was statistically significant (P<0.05). After treatment, there was no significant difference in physical health score, mental component score, short from 36 scores, self-rating depression scale and self-rating anxiety scale scores between the two groups compared with those before treatment and between the two groups (P>0.05). No clinically significant abnormalities of safety indicators or serious adverse events were observed in all patients. Conclusion Findings in our study suggest that Zhiqiao Chenpi Decoction has good curative effect and high safety on liver-stomach disharmony syndrome in patients with FD.

128 Effect of Fuzheng Jiedu Decoction and dumai moxibustion in the treatment of non small cell lung cancer patients in the late stage of lung and spleen deficiency syndrome and the level of immune inflammation

Objective To explore the effect of Fuzheng Jiedu Decoction combined with dumai moxibustion in the treatment of non-small cell lung cancer patients in the stage of lung and spleen deficiency syndrome. Methods From January 2020 to January 2022, 82 patients with advanced non-small cell lung cancer admitted to were divided into two groups by random number table method, with 41 cases in each group. The control group was treated with Paclitaxel+Carboplatin, and the study group was treated with dumai moxibustion and Fuzheng Jiedu Decoction on the basis of the control group. After two months of treatment, the scores of traditional Chinese medicine syndromes, levels of immune inflammatory factors (interleukin-6 [IL-6], immunoglobulin A [IgA]) and adverse reactions of the two groups were compared. Results Before treatment, there were no significant differences between the two groups in main syndrome score, secondary syndrome score, IL-6, and IgA (P>0.05). After treatment, the main syndrome score, secondary syndrome score and IL-6 of the two groups decreased, and the study group was lower than the control group, with a statistically significant difference (P<0.05). The levels of IgA in the two groups were higher compared with before treatment, and the study group was higher than the control group (P<0.05). The clinical effect in the study group was better than that in the control group, with a statistically significant difference (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). Conclusion Fuzheng Jiedu Decoction combined with dumai moxibustion in the treatment of advanced non-small cell lung cancer is more helpful for the patients to regulate the expression of immune inflammatory factors, and the clinical effect is superior.

Objective To observe the clinical effect of thumb-tack needle and ear point pressure bean assisted Modified Guipi Decoction in the treatment of fatigue subhealth state of male with deficiency of both heart and spleen. Methods A total of 90 patients with fatigue subhealth state with deficiency of both heart and spleen treated in Department of Andrology, Beijing Hospital of Traditional Chinese Medicine, Capital Medical University from January to December 2022 were selected and divided into observation group and control group by random number table method, with 45 cases in each group. The control group was treated with oral Modified Guipi Decoction, and the observation group was combined with thumb-tack needle and ear point pressure bean treatment on this basis. The thumb-tack needle points were selected as bilateral shénmén and shǒusānlǐ, and ear point pressure bean points were selected as heart and spleen. The treatment course of both groups was four weeks. The scores of traditional Chinese medicine symptoms, and scores of fatigue scale, quality of life scale, sexual confidence scale, self-rating anxiety scale (SAS), and self-rating depression scale (SDS) and sexual satisfaction were compared between the two groups before and after treatment. Results After treatment, traditional Chinese medicine symptoms score, fatigue scale score, SAS score, and SDS score in two groups were lower than those before treatment, and those of observation group were lower than those of control group, the differences were statistically significant (P<0.05). After treatment, the World Health Organization quality of life assessment-bref score, sexual satisfaction, and sexual confidence scale score in both groups were higher than those before treatment, and those of observation group were higher than those of control group, the differences were statistically significant (P<0.05). Conclusion The clinical effect of thumb-tack needle and ear point pressure bean on Modified Guipi Decoction in the treatment of fatigue subhealth state of male with deficiency of both heart and spleen is good, which is worth popularizing.

137 Clinical effect of cervical three needles warming needle moxibustion on cervical vertigo of qi and blood deficiency syndrome

Objective To explore the clinical effect of cervical three needles warming needle moxibustion on cervical vertigo of qi and blood deficiency syndrome. Methods A total of 78 patients with cervical vertigo who were admitted to the Yixing Hospital of Traditional Chinese Medicine from June 2018 to March 2021 were selected and they were divided into treatment group and control group according to the random number table method, with 39 cases in each group. The control group was treated with manual massage, and the treatment group was additionally treated with cervical three needles warming needle moxibustion on the basis of the control group, three times a week, seven times a course, two courses of treatment. The traditional Chinese medicine symptom scores, cervical vertigo symptom, and function assessment scale scores, clinical efficacy, mean vertebral-basilar blood flow velocity, and adverse reactions were compared in both groups. Results After treatment, the scores of traditional Chinese medicine symptom in both groups were lower than those before treatment, and the treatment group was lower than the control group, the differences were statistically significant (P<0.05). The scores of cervical vertigo symptom and function evaluation scale in the two groups were higher than those before treatment, and the treatment group were higher than the control group, the differences were statistically significant (P<0.05). The clinical effect of the treatment group was better than that of the control group (P<0.05). After treatment, the average blood flow velocity of vertebral basilar artery in the two groups was higher than that before treatment, and the treatment group was higher than the control group, the differences were statistically significant (P<0.05). During the treatment period, there were no obvious adverse reactions in the two groups. Conclusion The cervical three needles warming moxibustion combined with manual massage in the treatment of cervical vertigo can improve the clinical symptoms, and the curative effect is exact. At the same time, it can improve the blood flow velocity of the vertebral-basilar artery, which is safe and reliable.

141 Diagnostic value of shear wave elastography technique in the severity of patients with chronic obstructive pulmonary disease

Objective To investigate the diagnostic value of shear wave elastography (SWE) technique in the severity of patients with chronic obstructive pulmonary disease (COPD). Methods A total of 73 patients with stable COPD admitted to Department of Respiratory, Affiliated Hospital of Yunnan University from March to October 2022 were selected as the study objects. According to the results of lung function examination, they were divided into mild-moderate group (44 cases) and severe group (29 cases). Intercostal muscle thickness at end inspiration (ICMTei), diaphragmatic thickness at end inspiration (DTei), shear wave elastography of intercostal muscle (ICMswe), and shear wave elastography of diaphragm (Dswe) were compared between two groups; the influencing factors of disease severity in COPD patients were analyzed by logistic regression; the diagnostic efficacy of ultrasonic parameters on COPD disease severity was analyzed by receiver operating characteristic curve; and Pearson correlation coefficient was used to analyze the correlation between ultrasonic parameters and clinical characteristics of COPD patients. Results  The six-minute walk test (6MWT) in severe group was shorter than that in mild-moderate group, and the difference was statistically significant (P<0.05). ICMswe and Dswe in severe group were higher than those in mild-moderate group, and the difference was statistically significant (P<0.05); there were no significant differences in ICMTei and DTei between two groups (P>0.05). Dswe was a risk factor for severe COPD (OR=1.309, P<0.05). The cut-off value of Dswe in the diagnosis of severe COPD was 18.75 kPa, the area under the curve was 0.859 (95%CI: 0.772-0.945), the sensitivity was 89.7%, the specificity was 75.0%. Dswe was negatively correlated with 6MWT (r=-0.458, P<0.01). Conclusion  SWE technique can quantitatively assess respiratory muscle hardness and has diagnostic value in the severity of COPD patients.

145 Value of real-time shear wave elastography parameters combined with thyroid imaging reporting and data system in the identification of thyroid nodule properties

Objective To analyze the diagnostic value of real-time shear wave elastography (SWE) parameters combined with thyroid imaging reporting and data system (TI-RADS) in thyroid nodule properties. Methods The clinical data of 78 patients (80 nodules) with thyroid nodules detected by ultrasonography and treated by operation in Anqing First People’s Hospital, Anhui Province from January 2020 to June 2022 were retrospectively analyzed. The values of elastic modulus of thyroid nodules were detected by SWE, and the nodules were graded by TI-RADS. With postoperative pathological results as the gold standard, the diagnostic value of two kinds of examinations for benign and malignant thyroid nodules were compared. Results Postoperative pathological results showed that among 80 nodules, 55 benign nodules; 25 malignant nodules. There was moderate consistency between TI-RADS grading, SWE and pathological results (Kappa=0.554, 0.432, P<0.05), and high consistency between SWE combined with TI-RADS grading and pathological results (Kappa=0.683, P<0.05). The sensitivity, specificity, and accuracy of SWE combined with TI-RADS grading in the diagnosis of thyroid nodules were higher than that of single detection. Conclusion SWE combined with TI-RADS has high diagnostic value for thyroid nodules properties.

150 Analysis of antibiotic resistance and prognostic factors of Pseudomonas aeruginosa in patients with chronic obstructive pulmonary disease complicated with ventilator-associated pneumonia

Objective To analyze the antibiotic resistance and prognostic factors of Pseudomonas aeruginosa (PA) in patients with chronic obstructive pulmonary disease(COPD) complicated with ventilator-associated pneumonia (VAP). Methods The clinical data of 62 patients with COPD complicated with VAP in the Huaian First People’s Hospital Branch Hospital from June 2019 to June 2022 were retrospectively collected, and the drug resistance of PA to antibiotics was analyzed. According to the clinical outcome within 28 days after admission, the patients were divided into death group (18 cases) and survival group (44 cases). The general data of the two groups were compared, and the influencing factors of prognosis were analyzed by logistic multiple regression. Results A total of 17 strains of PA were cultured from 62 patients with COPD complicated with VAP, the total infection rate was 27.42%, and the drug resistance rate to Ampicillin, Cotrimoxazole, Amtreonam and Cefazolin was more than 90%. The clinical pulmonary infection score (CPIS), neutrophil to lymphocyte ratio.(NLR) and procalcitonin(PCT) levels in the death group were higher than those in the survival group, and the proportion of enteral nutrition support and acid inhibitor use was lower than that in the survival group (P<0.05). Logistic regression analysis showed that CPIS score, NLR and PCT were risk factors for the prognosis of patients with COPD complicated with VAP, and nutritional support pathway were protective factors (P<0.05). Conclusion The drug resistance rate of PA in patients with chronic obstructive pulmonary disease complicated with VAP to Ampicillin, Compound Sulfamethoxazole, Amtreonam, and other antibiotics is high, and CPIS score, NLR, PCT, and enteral nutrition support are all factors affecting the prognosis of patients with COPD complicated with VAP.

Based on the application and approval of three main types of General Program of National Natural Science Foundation of China, Key Program and Young Scientists Fund during the “13th Five-Year Plan” period, this paper analyzes the change of basic research competitiveness of Beijing Neurosurgical Institute. The results show that the basic research competitiveness of Beijing Neurosurgical Institute showed a steady growth trend. Through the analysis of a series of policy adjustments and research development orientation of National Natural Science Foundation of China during the “14th Five-Year Plan” period, the scientific model of sustainable development of basic research of neurosurgery center under the new situation was explored. This paper proposes that under the new situation, neurosurgery centers should pay attention to promoting the transformation of basic research into clinical application, promoting interdisciplinary integration, eliminating the obstacles of basic research and clinical transformation, improving the quantity and project quality of National Natural Science Foundation of China, and obtaining long-term and sustainable benign growth of basic research competitiveness.

Objective To explore the innovative ideas of intelligent outpatient service system in public hospitals. Methods Taking the practice of the First Affiliated Hospital of Nanjing Medical University as an example, by comparing the relevant data from July to December 2021 (before optimization) and January to June 2022 (after optimization), the operation effect of the integrated intelligent service platform covering pre-consultation, guidance during diagnosis, post-diagnosis follow-up, patient management, and other aspects was evaluated. Results After the optimization, the number of users of each functional module and the satisfaction rate of patients to each process were higher than before the optimization, and the waiting time of patients after optimization were shorter than those before optimization, with statistical significances (P<0.05). Conclusion The integrated service platform of intelligent outpatient service can play a certain role in meeting the medical needs of patients in public hospitals and improving the matching degree of medical service supply and demand.

167 Effect of ear-point pressing beans combined with humanistic care intervention on curative effect and psychological state of patients with lower extremity arteriosclerosis obliterans

Objective To explore the effect of ear-point pressing beans combined with humanistic care intervention on the efficacy and psychological status of patients with lower extremity arteriosclerosis obliterans (LEASO). Methods A total of 110 LEASO patients diagnosed and treated in the vascular department of Shijiazhuang Hospital of Traditional Chinese Medicine from February 2021 to February 2022 were selected and they were divided into study group and control group according to random number table method, with 55 patients in each group. The control group was treated with routine therapy, while the study group was treated with ear-point pressing beans combined with humanistic care intervention for two weeks. The clinical efficiency, complication rate, visual analogue scale (VAS), psychological status, quality of life, treatment compliance and satisfaction were compared between the two groups. Results Finally, 51 cases in the study group and 47 cases in the control group completed this study. The clinical efficacy of the study group was better than that of the control group, the incidence of complications in the study group was lower than that in the control group (P<0.05). Before treatment, there were no significant differences in VAS, self-rating anxiety scale (SAS), self-rating depression scale (SDS), pain-free maximum walking distance (PFWD), short form 36 (SF-36), and physical and mental health scores between the two groups(P>0.05). After treatment, VAS, SAS and SDS scores of the two groups were lower than before treatment, while PFWD, physical and mental health scores were higher than before treatment. VAS, SAS and SDS scores of the study group were lower than those of the control group, and PFWD, physical and mental health scores were higher than those of the control group (P<0.05). Conclusion Ear-point pressing beans combined with humanistic care intervention can effectively alleviate the clinical symptoms of LEASO patients, reduce the risk of complications, amend negative emotions and improve the quality of life, and the patient compliance and satisfaction is high, worthy of promotion and application.

171 Effect of the self-determination theory model in elderly patients with chronic heart failure

Objective To explore the effect of the self-determination theory model in elderly patients with chronic heart failure. Methods A total of 82 elderly patients with chronic heart failure treated in Fuyang People’s Hospital of Anhui Province from May 2019 to December 2020 were selected as the study objects, they were divided into control group and observation group according to random number table method, with 41 cases in each group. The control group implemented routine nursing intervention, and the observation group implemented self-determination theory model intervention based on the control group. Decision conflict, health behavior ability, and quality of life improvement were compared between the two groups before intervention and two weeks after intervention. Results After two weeks of intervention, the scores of uncertainty, information informing, clear values and external support in decision conflict in both groups were lower than those before intervention, and the observation group was lower than the control group (P<0.05). The scores of exercise, health responsibility, psychological well-being, and nutrition in health behavior ability were higher than those before intervention, and those in the observation group were higher than those in the control group (P<0.05). The scores in the physical, emotional, and other areas of quality of life were lower than those before intervention, and those of the observation group were lower than those of the control group (P<0.05). Conclusion The self-decision theory can reduce the decision conflict and improve the health behavior and quality of life.

175 Application of continuous nursing model based on action research in patients with coronary heart disease

Objective To explore the application effect of continuous nursing model based on action research in patients with coronary heart disease. Methods A total of 125 patients with coronary heart disease treated in First People’s Hospital of Huzhou, Zhejiang Province from March 2021 to January 2022 were selected as the study objects. According to random number table method, they were divided into control group (61 cases) and observation group (64 cases). The control group was given routine nursing, and the observation group was given continuous nursing model based on action research. Cardiac function, self-care ability, treatment compliance, health quotient index, and quality of life were compared between the two groups before and after intervention. Results After intervention, left ventricular ejection fraction (LVEF), left ventricular end-diastolic diameter (LEVDD), and left ventricular end-systolic diameter (LVESD) in control group were all lower than those before intervention, LEVDD and LVESD in observation group were lower than those before intervention, LVEF in observation group was higher than that before intervention, and LVEF in observation group was higher than that in control group (P<0.05). The scores of self-care ability, treatment compliance, therapeutic health quotient index, and quality of life assessment questionnaire of Chinese cardiovascular patients in both groups were higher than those of before intervention, and those of the observation group were higher than those of the control group (P<0.05). Conclusion The continuous nursing mode based on action research can be used for coronary heart disease patients, which might improve patients’ cardiac function, self-care ability, treatment compliance, health quotient index and quality of life.

180 Influence of one core multi-team nursing model on hospice care for patients with advanced tumors

Objective To explore the influence of one core multi-team nursing model on hospice care for patients with advanced tumors. Methods A total of 218 patients with advanced tumors admitted to Nanfang Hospital of Southern Medical University from September 2021 to June 2022 were selected as the study objects, the random number generator was used for random numbering. The odd number was included in the control group and the even number was included in the observation group, with 109 cases in each group. The control group was given routine nursing, the observation group was given one core multi-team nursing mode, and the intervention lasted for three months. Physiological symptoms, negative emotions, and quality of life were compared between the two groups before and after intervention. Results After intervention, the symptom scores of fatigue, pain, and appetite in both groups were lower than those before intervention, and those of the observation group were lower than those of the control group, the differences were statistically significant (P<0.05). After intervention, the scores of self-rating anxiety scale and self-rating depression scale in both groups were lower than those before intervention, and those of the observation group were lower than those of the control group, the differences were statistically significant (P<0.05). After intervention, the quality of life score of the two groups were higher than those before intervention, and those of the observation group were higher than those of the control group, the differences were statistically significant (P<0.05). Conclusion One core multi-team nursing can improve the fatigue, pain, appetite, and other physiological symptoms of advanced cancer patients, depression and anxiety, improve the quality of life.

Objective To explore the preventive effect of predictive nursing intervention on fall and fall out of bed in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD). Methods A total of 80 patients with AECOPD treated in the Emergency Department of Bozhou People’s Hospital in Anhui Province from February 2020 to May 2022 were selected, they were divided into the control group (40 cases, Morse scale risk assessment followed by routine nursing measures) and the observation group (40 cases, Morse scale risk assessment followed by predictive nursing intervention) according to the random odd and even number method. The incidence of fall and fall out of bed and their satisfaction with nursing were compared between the two groups, the activities of daily living (ADL) and fall and fall out of bed prevention questionnaire scores of the two groups were analyzed before nursing and at discharge. Results The incidence of fall and fall out of bed in the observation group were lower than the control group (P<0.05); the satisfaction of patients in the observation group was better than the control group (P<0.05); at discharge, the ADL scores of the two groups were higher than those before nursing, and those of the observation group were higher than those of the control group (P<0.05); at discharge, the knowledge, attitude and behavior scores of the two groups were higher than those before nursing, and those of the observation group were higher than the control group (P<0.05). Conclusion Predictive nursing can effectively reduce or even avoid the occurrence of fall and fall out of bed, and promote the significant improvement of patients’ activities of daily living, and further improve patients’ satisfaction with nursing.

188 Effects of diversified health education on psychological state, rehabilitation quality, and nursing satisfaction of patients with thoracolumbar fracture internal fixation

Objective To explore the effects of diversified health education on psychological state, rehabilitation quality, and nursing satisfaction of patients with thoracolumbar fracture internal fixation. Methods A total of 88 patients who underwent internal fixation of thoracolumbar fracture in Shijingshan District Beijing Chao-yang Hospital, Capital Medical University from August 2021 to January 2022 were selected as the study objects, and divided into the study group and the control group by random number method, with 44 cases in both groups. The control group received routine nursing after surgery, and the study group received diversified health education on the basis of routine nursing. Compliance with rehabilitation training and lumbar function (Oswestry disability index [ODI], Japanese Orthopaedic Association scores [JOA]), visual analogue scale (VAS), lumbar joint motion, activity of daily living (ADL) score, mental state, and nursing satisfaction before and after intervention were compared between the two groups. Results The compliance of rehabilitation training in the study group was better than that in the control group (P<0.05). After intervention, ODI and VAS scores of the two groups were lower than those before intervention, JOA scores were higher than those before intervention, and ODI and VAS scores of the study group were lower than those of the control group, JOA scores were higher than those of the control group (P<0.05). The lumbar joint motion of the two groups was greater than those before intervention, and the ADL score was higher than those before intervention, and the lumbar joint motion of the study group was greater than that of the control group, and the ADL score was higher than that of the control group (P<0.05). After intervention, the scores of Hamilton anxiety scale (HAMA) and Hamilton depression scale (HAMD) in both groups were lower than before intervention, and the Newcastle nursing satisfaction scale (NSNS) was higher than before intervention, and the HAMA and HAMD scores of the study group were lower than those of the control group, and the NSNS scores were higher than those of the control group. Conclusion Diversified health education can effectively relieve bad mood, improve compliance with rehabilitation exercise, and thus improve rehabilitation effect and nursing satisfaction.

193 Effect of protection motivation theory intervention mode on postoperative recovery, self-efficacy, and quality of life of patients with spinal fractures

Objective To explore the effect of protection motivation theory (PMT) intervention mode on postoperative recovery, self-efficacy, and quality of life in patients with spinal fractures. Methods A total of 106 patients with spinal fractures admitted People’s Hospital of Ganzi Tibetan Autonomous Prefecture from April 2020 to April 2022 were selected. They were divided into conventional group and PMT group by random number table method, with 53 cases in each group. The conventional group received routine intervention, PMT group received PMT intervention mode on the basis of routine intervention for two months. The postoperative recovery and self-efficacy, quality of life before and after intervention were compared between two groups. Results The detumescence time, hospitalization time, and fracture healing time in PMT group were shorter than those in conventional group, and the differences were statistically significant (P<0.05). After intervention, the scores of exercise efficacy, calcium intake efficacy, and osteoporosis self-efficacy scale total score in two groups were increased than those before intervention, and PMT group was higher than control group, and the differences were statistically significant (P<0.05). After intervention, the scores of energy, physical activity, social life, sleep emotion and Nottingham health profile total score in two groups were higher than those before intervention, and PMT group was higher than control group, and the differences were statistically significant (P<0.05). Conclusion The PMT intervention mode can effectively enhance the self-efficacy, promote healthy behaviors, then improve the quality of life and promote postoperative recovery of patients with spinal fractures.
