XUE Ruijun1 LI Wenping2 HUANG Weizhe3 PAN Shuyi1 GUO Dazhi1
DOI: 10.20047/j.issn1673-7210.2023.17.05
Objective To investigate the action mechanism of hyperbaric oxygen (HBO) on white matter damage in adult rats with hypoxic-ischemic encephalopathy. Methods Forty-eight ten- week-old clean grade male SD rats aged 300-350 g were divided into four groups by completely randomized method: control group (no treatment), sham operation group (the skin was sutured directly after only bilateral carotid arteries were exposed), model group (two weeks after bilateral carotid arteries were ligation, rats were placed in a sealed chamber with 8%O2 and 92%N2 mixture for three hours), HBO group (HBO treatment was immediately given on the basis of model group), 12 rats in each group. The learning and memory function was evaluated by water maze before modeling and in the 4th week respectively. Fast Blue staining was used to observe the white matter myelin sheath damage in rat hippocampus. The expressions of ionized calcium binding adapter molecule 1 (Iba-1) and glial fibrillary acidic protein (GFAP) in the hippocampus were determined by immunofluorescence method. The expressions of myelin basic protein (MBP), interleukin (IL)-1β, IL-6, and tumor necrosis factor-α (TNF-α) in hippocampus were analyzed by Western blot. Results There were no significant differences in the average incubation period and the number of platform crossing between the sham operation group and the control group (P>0.05). The average incubation period of the model group was longer than that of the control group, and the number of platform crossing was lower than that of the control group (P<0.05). The average incubation period of the HBO group was shorter than that of model group, and the number of platform crossing was higher than that of the model group (P<0.05). The sham operation group was compared with the control group, the hippocampal staining degree of rats was basically the same, and there was no significant difference in myelin sheath. Compared with the control group, the hippocampal staining of rats was weakened, the myelin sheath was reduced, and vacuoles were appeared in some areas in the model group. Compared with the model group, hippocampal staining degree of rats was improved, myelin sheath was increased and regional vacuoles were decreased significantly in the HBO group. There was no significant difference in the number of GFAP-positive activated astrocytes and Iba-1 positive activated microglia in the hippocampus between the sham operation group and the control group (P>0.05). The number of GFAP-positive activated astrocytes and Iba-1 positive activated microglia in the hippocampus of the model group was higher than that of the control group (P<0.05). The number of GFAP-positive activated astrocytes and Iba-1 positive activated microglia in hippocampus of the HBO group was lower than that of the model group (P<0.05). There were no significant differences in the expressions of MBP, IL-1β, IL-6, and TNF-α between the sham operation group and the control group (P>0.05). The expression of MBP protein in the brain of the model group was lower than that of the control group, and the expressions of IL-1β, IL-6, and TNF-α were higher than those of the control group (P<0.05). The expression of MBP protein in the HBO group was higher than that in the model group, but the expression levels of IL-1β, IL-6, and TNF-α in the HBO group were lower than that in the model group (P<0.05). Conclusion HBO may reduce the activation of microglias and astrocytes in the hippocampus, inhibit the release of inflammatory factors, relieve myelin sheath damage, and promote myelin sheath regeneration, thereby improving the cognitive impairment of adult rats with hypoxic-ischemic encephalopathy.

DOI: 10.20047/j.issn1673-7210.2023.17.06
Objective To analyze the literature related to hallux valgus in China, the research status and future development direction of hallux valgus are understood to provide scientific basis for the research in this field. Methods From January 1982 to December 2021, “hallux valgus” OR “hallux valgus” OR “hallux deformity” were searched by China National Knowledge Network. Statistical analysis was made on the publication time, publication sources, regions, disciplines involved and fund distribution of relevant literatures; by using bibliometric method and VOSviewer and CiteSpace software, the clustering situation of the current research team of bunion valgus was studied through co-authoricity analysis, and the current relevant research hotspots were studied through co-occurrence analysis. Results A total of 1 170 literatures on hallus valgus were retrieved, among which 857 were valid. The number of related literatures showed a trend of fluctuating year by year with a slow rise. Although the research institutions ware spread across most of the country, there was an imbalance in regional development, and the degree of cooperation between different regions, institutions of higher learning and authors was relatively small. The relevant literatures were published in more than 200 sources, which were relatively scattered. The National Natural Science Foundation of China provided the largest amount of funding for this research, but less than 20% of the literatures were funded by the foundation. The research focused on hallux valgus mainly include etiology, pathogenesis, surgical treatment, surgical efficacy, and complications. Conclusion In this study, it is found that the author team has been initially formed in the related research on hallus valgus, but the overall research level is still in a relatively immature stage, and the cooperation between the institutions and the authors is not strong, and the regional characteristics are prominent, so it is necessary to strengthen the communication and cooperation between the regional institutions. Hallux valgus researchers should improve the quality of scientific research and form more intensive evidence-based treatment.

ZHANG Mengke CAO Jingyan ZHOU Shuhan SUN Qingyu
DOI: 10.20047/j.issn1673-7210.2023.17.07
Human epidermal growth factor receptor 2 is one of the driver genes for non-small cell lung cancer and is associated with poor prognosis. Among tyrosine kinase inhibitors, Poziotinib and Pyrotinib have good efficacy; Mobotinib has a high affinity for exon 20 insertion mutations. Treatment with Trastuzumab can be used in patients with first-line progression. The efficacy of antibody drug conjugates is the most encouraging, and Trastuzumab-Deruxtecan has obtained unprecedented efficacy data. Although immune checkpoint inhibitors are not recommended, combination immunotherapy may bring new life to patients.

DOI: 10.20047/j.issn1673-7210.2023.17.08
Heart failure is the terminal stage of heart disease caused by various causes, and abnormal β adrenergic receptor signaling pathway is one of the important pathophysiological mechanisms for the occurrence and development of heart failure. Three kinds of β adrenergic receptor mediated signaling pathways and abnormal number of receptors play different pathophysiological roles in the occurrence and development of heart failure, such as β1 adrenergic receptor mediated Ca2+/calmodulin dependent protein kinase Ⅱ signaling, β2 and β3 adrenergic receptor mediated Gi protein signaling. This article mainly reviews the different pathological significance of the three β adrenergic receptor signaling pathways in heart failure, and the research progress of traditional Chinese and western medicine in the prevention and treatment of heart failure by improving β adrenergic receptor signaling.

WU Yumin1 YAN Wenjun2 QU Song1 ZHANG Wenjie1
DOI: 10.20047/j.issn1673-7210.2023.17.09
Lidocaine, an amide based local anesthetic, has been widely recognized for its clinical effects during the perioperative period, whether used as a regional anesthetic or as an auxiliary drug for general anesthesia. Lidocaine intravenous injection has the effects of analgesia, sedation, anti-infection, organ protection, promoting gastrointestinal function recovery and inhibiting tumor cell recurrence and metastasis. Colorectal cancer patients are more common in middle-aged and elderly people, and due to factors such as decreased appetite and tumor bleeding in the early stages, patients may experience weight loss, anemia, and poor tolerance to surgery and anesthesia. This review elucidates the advantages of intravenous infusion of lidocaine in general anesthesia for colorectal cancer patients, aiming to optimize perioperative management and improve prognosis.

49 Research progress of surgical traumatic stress and proinflammatory cytokines
Subinuer Kurexi1 WANG Ke2 CHEN Tongyu1
DOI: 10.20047/j.issn1673-7210.2023.17.10
Surgical traumatic stress can initiate the immune inflammatory response and promote the release of a large number of proinflammatory cytokines. Excess production of proinflammatory cytokines can cause further damage to tissues and organs, affecting the prognosis and outcome of surgical patients. In the acute stage of surgical trauma, the inhibition of excessive proinflammatory cytokines is the key to reduce postoperative complications and promote the recovery of patients. Among them, proinflammatory cytokines interleukin-6, tumor necrosis factor-α, high mobility group box 1, and C-reactive protein have important effects on surgical traumatic stress. This paper will focus on the above four typical proinflammatory cytokines and explore the potential clinical application value of corresponding cytokine antagonists.

57 Research progress on anti-tuberculosis effect of ion channel blockers
LUO Shuiqing1 ZENG Yali2
DOI: 10.20047/j.issn1673-7210.2023.17.12
Tuberculosis is a global public health disease caused by Mycobacterium tuberculosis (MTB) infection. The treatment of tuberculosis requires long-term use of multiple drugs, and becomes complicated and lengthy due to compliance and drug resistance. The sensitivity of MTB to drugs is to some extent determined by the interaction between bacteria and host macrophages. MTB mainly exists in macrophages after infecting the human body, and by inhibiting the physiological activity of macrophages, the body is in an infected state. Ion channel blockers regulate the concentration of signaling ions in macrophages by affecting the activity of ion channels, enhance the killing effect of host macrophages on MTB, and inhibit the activity of MTB efflux pumps, and improve the sensitivity of MTB to drugs. This article reviews some effective ion channel blockers for MTB and preliminarily explores their anti-tuberculosis mechanisms, hoping to provide new ideas for the development of new anti-tuberculosis drugs.

DOI: 10.20047/j.issn1673-7210.2023.17.14
Objective To investigate the relationship between serum soluble intercellular adhesion molecule-1 (sICAM-1), soluble CD163 (sCD163) levels and in-stent restenosis (ISR) after percutaneous coronary intervention (PCI) in patients with unstable angina pectoris(UAP). Methods A total of 182 UAP patients who underwent PCI in Taizhou Hospital of Traditional Chinese Medicine , Jiangsu Province from January 2019 to February 2021 were selected and divided into ISR group (41 cases) and non-ISR group (141 cases) according to whether ISR occurred. The clinical data of the two groups were collected, and the serum levels of sICAM-1 and sCD163 were detected by enzyme-linked immunosorbent assay. Binary logistic regression was used to analyze the influencing factors of ISR in UAP patients after PCI. The receiver operating characteristic (ROC) curve was used to analyze the predictive value of serum sICAM-1 and sCD163 levels for ISR in UAP patients after PCI. Results The levels of serum sICAM-1, sCD163, and low-density lipoprotein cholesterol (LDL-C) in the ISR group were higher than those in the non-ISR group, the stent length was longer than that in the non-ISR group, and the stent diameter was shorter than that in the non-ISR group (P<0.05). Multivariate analysis showed that stent diameter (OR=0.877, 95%CI: 0.797-0.965), LDL-C (OR=1.006, 95%CI: 1.003- 1.009), sICAM-1 (OR=1.014, 95%CI: 1.006-1.023), and sCD163  (OR=1.121, 95%CI: 1.021-1.232) were independent influencing factors for ISR in UAP patients after PCI (P<0.05). The results of ROC curve analysis showed that the area under the curve of serum sICAM-1 and sCD163 combined to predict ISR in UAP patients after PCI was higher than that of each index alone (P<0.05). Conclusion Serum sICAM-1 and sCD163 levels are independent influencing factors of ISR in UAP patients after PCI, which can be used as predictors of ISR.

Hepc of RA-ACD and RA groups were lower than that of control group, and RA-ACD group was lower than that of RA group; those of s-HJV in RA-ACD and RA groups were higher than that in healthy control group, and RA group was higher than that in healthy control group; Hb in RA-ACD group was lower than that in RA group and healthy control group, and the differences were statistically significant (P<0.05). There were statistically significant differences in SI, Tf, and SF levels among the three groups (P<0.05). The SI of RA-ACD group and RA group was lower than that of healthy control group, and RA-ACd group was higher than that of RA group; Tf in RA-ACD group and RA group was lower than that in healthy control group; The SF level of RA-ACD group was lower than that of RA group and healthy control group, and the differences were statistically significant (P<0.05). The levels of IL-6, ESR, and CRP among the three groups were statistically significant (P<0.05). IL-6, ESR, and CRP in RA-ACD and RA groups were higher than those in healthy control group, ESR, CRP, and DAS28 in RA-ACD group were higher than those in RA group, and the differences were statistically significant (P<0.05). Correlation analysis showed that Hepc was positively correlated with IL-6 (r>0, P<0.05), and negatively correlated with DAS28 (r<0, P<0.05). s-HJV was positively correlated with SI, Tf, IL-6, and ESR (r>0, P<0.05), and negatively correlated with Hb (r<0, P<0.05). DAS28 was positively correlated with SI, ESR, and CRP (r>0, P<0.05), and negatively correlated with Hb (r<0, P<0.05). Hb was negatively correlated with SI, SF, ESR, CRP, and s-HJV (r<0, P<0.05). Conclusion Hepc and s-HJV are involved in the progression of RA-ACD disease, and s-HJV may reverse the promoting effect of inflammatory factors on Hepc expression in a compensatory way. The disease activity of RA is closely related to inflammation and anemia. The higher the level of inflammation, the more serious the disease will be, and anemia will aggravate the disease. Serum Hepc level decreased with the increase of DAS28 level, suggesting a certain correlation between Hepc and disease activity in RA patients.

149 Li Dongyuan’s experience in treating perimenopausal syndrome based on Yin fire theory
KONG Lingyuan HAN Xinyuan WU Yue SONG Xiaobin
DOI: 10.20047/j.issn1673-7210.2023.17.34
The Yin fire theory put forward by Li Dongyuan, a famous physician in the Jin Dynasty
, takes “damage to the qi of the spleen and stomach, malfunction of the rising of qi; deficiency of primordial qi, and internal flourishing of yin fire” as the fundamental pathogenesis, which can explain the pathogenesis of perimenopausal syndrome more completely. Based on the analysis of yin fire theory, the basic pathogenesis of perimenopausal syndrome includes “deficiency of primordial qi, yin fire flaming upward”, “malfunction of the rising of qi, stagnation of fire and internal knot”, and “internal flourishing of yin fire, depletion of kidney water”. Li Dongyuan creates the removing heat with sweet-warm natured medicine method for treating yin and fire syndrome, which uses pungent-warm natured medicine for dispersing and penetrating, sweet-warm and sweet-calm natured medicine to benefit qi for tranquillization, and sweet-cold natured medicine to nourish yin for clearing fire, which is important for the treatment of perimenopausal syndrome, and can be used flexibly in the clinical treatment of perimenopausal syndrome according to this method.
