DOI: 10.20047/j.issn1673-7210.2023.03.01
Objective To explore the medication rules of the prescriptions for the treatment of hypochondriac pain contained in the Chinese Medical Code. Methods Prescriptions for the treatment of hypochondriac pain in the Chinese Medical Code were collected, prescription data and drug information were input by Excel (Microsoft Office 2019), and frequency statistics were made on drug efficacy, four nature of drugs and five flavours, and channel tropism. SPSS Modeler 18.0 and Cytoscape 3.9.1 were used to calculate drug co-occurrence frequency and perform visual network display. Association rule analysis and visualization of the drugs in the screened prescriptions were performed using arules and arulesViz packages in R software, and clustering analysis of high-frequency drugs was performed using SPSS Statistics 19.0. Results A total of 359 effective prescriptions were screened, and 240 flavors of traditional Chinese medicine were used, with a total medication frequency of 3 032 times. The core drugs were Angelicae Sinensis Radix, Glycyrrhizae Radix et Rhizoma, Paeoniae Radix Alba, Bupleuri Radix, Citri Reticulatae Pericarpium Viride, Citri Reticulatae Pericarpium, and so on. Three types of drugs were mainly used to tonify deficiency, regulate qi, and promote blood circulation for removing blood stasis. The four nature of drugs were mainly warm, the five flavours were mainly pungent, bitter, and sweet, and the channel tropism was mainly spleen, liver, lung, stomach, and heart channels. The association rule analysis yielded 15 groups of drug association rules, with the highest confidence level of Pinelliae Rhizoma, Zingiberis Rhizoma Recens and Glycyrrhizae Radix et Rhizoma. The efficacy of the 15 groups of drug association rules was mainly based on tonifying qi, nourishing blood, promoting qi, resolving blood stasis, and removing dampness and heat. High-frequency drugs could be clustered into five categories, mainly soothing liver-qi stagnation, clearing heat and promoting dampness, activating qi and relieving pain, promoting blood circulation and removing blood stasis. Conclusion The prescriptions recorded in Chinese Medical Code for the treatment of hypochondriac pain present a rule of prescriptions based on soothing liver and qi, activating blood circulation and resolving blood stasis, softening liver and relieving pain, resolving phlegm and drying dampness, supporting vital qi and tonifying deficiency, which is consistent with the core pathogenesis of hypochondriac pain and provides a reference for clinical traditional Chinese medicine treatment of hypochondriac pain.

DOI: 10.20047/j.issn1673-7210.2023.03.02
Objective To explore the mechanism of Qixuan Yijianing in the treatment of Graves disease (GD) based on network pharmacology and animal experiments. Methods The active constituents and targets of Qixuan Yijianing were detected by traditional Chinese medicine systems pharmacology database and analysis platform, and chemical database, and GD related targets were detected from Genecards, online human Mendelian genetic database, etc. The common targets of drugs and diseases were introduced into STRING platform to construct protein-protein interaction (PPI) network and screen core targets. Cytoscape 3.8.2 software was used to map the drug-active ingredient-target network and screen core components. The core target was enriched by Metascape platform. Forty 6-week-old female BALB/c mice with SPF grade and body weight of (20±2) g were selected, ten of which were set as normal control group, and the remaining 30 were immunized with recombinant adenovirus Ad-TSHR-289 for three times to construct GD model. The mice that modeled successfully were divided into GD model group, Methimazole group, and Qixuan Yijianing group by random number table method, with eight mice in each group. Qixuan Yijianing group was intragastric administration of Qixuan Yijianing 50 g/kg, Methimazole group was intragastric administration of Methimazole Tablets 3.75 mg/kg, normal control group and GD model group were intragastric administration of drinking water of the same volume, once a day. After four weeks of continuous administration, serum vascular endothelial growth factor (VEGF) content was detected by enzyme-linked immunosorbent assay. The expression of platelet endothelial cell adhesion molecule-1 (PECAM-1) in thyroid tissues of the four groups was detected by immunohistochemistry. Results A total of 63 active ingredients with 281 targets and 1 489 targets of GD were obtained. There were 124 targets of drugs and diseases. PPI obtained 21 core targets of signal transduction and transcription activator 3, serine/threonine protein kinase 1, VEGFA, etc. Eighteen core components including luteolin, kaempferol, quercetin, ursolic acid, and β-sitosterol were obtained by drug-target analysis. 954 items of inflammatory response, angiogenesis, and vascular morphogenesis were obtained GO by analysis, and 92 pathways including MAPK, PI3K/AKT, and VEGF were obtained by KEGG analysis. Animal experiments showed that the expression levels of serum VEGF and thyroid PECAM-1 in GD model group were higher than those in normal control group, and those in Qixuan Yijianing group were lower than those in GD model group, the differences were highly statistically significant (P<0.01). Conclusion Qixuan Yijianing is able to treat GD by regulating MAPK, PI3K/AKT, VEGF, and other signaling pathways to inhibit angiogenesis.

Research progress on exosome targeted delivery of nucleic acid molecules
JIA Yue1 LI Yan2 WANG Yingji1
DOI: 10.20047/j.issn1673-7210.2023.03.07
Exosomes, as small nanoscale extracellular vesicles are an important mode of cellular information delivery in living organisms. Exosomes are characterized by high delivery efficiency, low immunogenicity, the ability to cross multiple biological barriers, and targeting properties. This has made exosomes an important member of drug delivery carriers to overcome the disadvantages of poor stability, insufficient bioavailability, low solubility, and high toxicity of some drugs. This article reviews the use of exosomes in vesicle form as novel drug delivery molecules, including microRNA, small interfering RNA and deoxyribo nucleic acid, which have received much attention in recent years. This article aims to provide a theoretical basis and design ideas for the development of efficient and safe delivery of exosome targeted nucleic acid like molecules and to facilitate their clinical translation.

Research advances in brain metastasis of non-small cell lung cancer
MA Li ZHU Ya WANG Zhaoxia
DOI: 10.20047/j.issn1673-7210.2023.03.08
Lung cancer is a common malignant tumor with high morbidity and mortality. Many patients die of distant metastases, especially to the brain. Due to the existence of blood-brain barrier and blood-tumor barrier, drug therapy is not effective. Many studies have shown that imbalanced expression of molecules plays an important role in the progression of malignant tumors. Epidermal growth factor receptor, non-coding RNA, cell adhesion molecules and others are reported to participate in regulating the process of brain metastases from non-small cell lung cancer, they also have high clinical translational value in diagnosis, treatment and evaluation of patient prognosis. This paper summarizes the recent progress of brain metastasis in non-small cell lung cancer, hoping to provide ideas and clues for future research.

Research progress of Chinese and western medicine intervention on platelet transfusion refractoriness in patients with liver cancer
NONG Yaobin1 HUANG Hongna2 HUANG Jingjing3 DONG Zhixin4 SONG Wenxuan1 XIAO Xiyu1 DU Yuanqin1
DOI: 10.20047/j.issn1673-7210.2023.03.09
The occurrence of liver cancer is often evolved from the progression of viral hepatitis, cirrhosis, aflatoxin contamination, and so on. Most patients with liver cancer are complicated with hyperplenism, which leads to the reduction of their platelets in peripheral blood to varying degrees. At the same time, targeted drug therapy can produce strong inhibition on megakaryocytes and bone marrow function, and can also lead to a decrease in the number of platelets. In the treatment of platelet transfusion in patients with liver cancer, there may be several times of infusion but the ideal effect can not be achieved, the bleeding status is not alleviated, and even platelet transfusion refractoriness (PTR) occurs. Abnormal bleeding in patients with liver cancer is closely related to platelets level. Therefore, it is very important to explore the regulation of platelets by Chinese and western medicine in the treatment of PTR in patients with liver cancer. In this paper, platelet generation, PTR mechanism in patients with liver cancer, regulation and intervention measures of Chinese and western medicine are briefly reviewed, so as to provide reference for the clinical treatment of PTR in patients with liver cancer and broaden treatment ideas.

CHEN Kun WANG Zonghua LUO Yu
DOI: 10.20047/j.issn1673-7210.2023.03.10
Post-stroke depression is one of the most common neurological and mental symptoms after stroke, the unsafe and inaccurate efficacy of drug treatment in the past, often results in patients with prolonged disease, which seriously affects the quality of life of patients. The discovery of the unique regulation of gut microbiota on the gut-brain axis provides a breakthrough for the prevention and treatment of post-stroke depression. The gut microbiota can regulate the gut-brain axis through neuroendocrine, immune and metabolic system pathways to form positive and negative feedback, allowing patients to obtain neuro- and gut-protective benefits, allowing them to play a precise regulatory role in the prevention and treatment of post-stroke depression. This paper attempts to summarize the interaction mechanism between gut microbiota and the gut-brain axis, the correlation between intestinal microbiota and post-stroke depression, new methods and interventions in the treatment, and their mechanism. To provide new ideas for the prevention, and treatment of post-stroke depression.

Research progress of non-drug intervention for poststroke cognitive impairment
XING Weizu MAI Yongmeng SU Qingjie▲
DOI: 10.20047/j.issn1673-7210.2023.03.11
Poststroke cognitive impairment (PSCI) refers to any type of cognitive deterioration that occurs after stroke, they range from mild impairment to Alzheimer. Stroke survivors have a significantly increased risk of cognitive impairment, this can affect independence of daily activities, social participation and quality of life. Due to the limited effectiveness of medications for patients with PSCI, therefore, many scholars attach great importance to the exploration of non-drug treatment methods. There are many kinds of non-drug treatment methods, which are safe, economical, effective, convenient and practical. At present, many studies have confirmed that non-drug methods have certain effects in improving motor and cognitive functions of PSCI patients, and different non-drug interventions have different mechanisms of action. In this paper, non-drug methods for the treatment of PSCI patients including exercise therapy, acupuncture therapy, physical therapy, and computer aided cognitive training were reviewed, to provide reference for selecting non-drug intervention methods for PSCI patients.

Research progress on the effect of iliac fascia block on early analgesia and rapid recovery after total hip replacement in the elderly
YANG Honghong1 LIU Rongrong1 LU Hongxiu2
DOI: 10.20047/j.issn1673-7210.2023.03.12
Total hip arthroplasty is a successful procedure for people with hip fractures to reduce chronic pain and enhance joint function. However, how to effectively relieve postoperative pain in elderly patients with poor cardiopulmonary function and multiple complications, and accelerate postoperative rehabilitation has become an important problem that anesthesiologists pay attention to. This paper reviews the studies on iliac fascia block in PubMed, CNKI, and Wanfang Data. It is found that ultrasound-guided iliac fascia space block based on the concept of accelerated rehabilitation surgery, especially the superior inguinal ligament approach, has significant postoperative analgesic effect and little impact on joint motor function. It has obvious advantages in early analgesia and rapid recovery after total hip replacement in the elderly.

OUYANG Xianjin KANG Xu GUO Tingting LIU Yueping CHANG Mingming
DOI: 10.20047/j.issn1673-7210.2023.03.13
Mesoporous silica nanoparticles with ordered mesoporous structure, large specific surface area, easy surface modification, and high colloidal stability have attracted more and more attention in the field of drug targeted therapy and controlled-release. As a drug carrier, unmodified mesoporous silica has a relatively obvious “sudden release” effect, by modifying small organic molecules, inorganic nanoparticles, and ring molecules on its surface, the drug release efficiency can be effectively improved, which is beneficial to improve the stability and biocompatibility of the system, and promote the further application of mesoporous silica nanoparticles in the field of drug delivery.

Contrastive interpretation of diagnosis and treatment norms of pelvic inflammatory diseases in China and the United States
LI Ting LIU Zhaohui
DOI: 10.20047/j.issn1673-7210.2023.03.20
Pelvic inflammatory disease (PID) is one of the common diseases of women of childbearing age, which can lead to sequelae such as tubal infertility, ectopic pregnancy, and chronic pelvic pain, and cause great harm to female reproductive health. In 2021, the Centers for Disease Control and Prevention (CDC) issued Sexually Transmitted Infections Treatment Guidelines, 2021 (“2021 CDC guidelines” for short), completing a significant update to Sexually Transmitted Diseases Treatment Guidelines, 2015 (“2015 CDC guidelines” for short), most of the symptoms and signs of PID are mild or latent, so the diagnosis still relies on the minimum clinical standards. The treatment is mainly based on the use of broad spectrum, empirical antibiotics covering all possible pathogens, emphasizing the addition of metronidazole to cover anaerobic bacteria. Different intravenous and non-intravenous drug regimens have shown good efficacy. This paper compares the 2021 CDC guidelines, 2015 CDC guidelines, and Chinese Guidelines for Diagnosis and Treatment of Pelvic Inflammatory Diseases (2019 Revision), in order to provide reference and help for the current clinical practice of PID in China.

Beiji Qianjin Yao Fang on dysentery with cold syndrome causes and pathogenesis, treatment principles, and prescription drugs are detailed. This paper summarizes the characteristics of syndrome and treatment of Sun Simiao’s treatment of dysentery with cold syndrome by analyzing the medication rules of dysentery with cold syndrome articles. Sun Simiao believes that the disease is “more cold” and “more deficiency”. In clinical treatment, Aconiti Lateralis Radix Praeparata, Zingiberis Rhizoma, Cinnamomi Cortex, and other warm and hot drugs are used to “treating cold syndrome with heat methods”. and Ginseng Radix Et Rhizoma, Glycyrrhizae Radix Et Rhizoma, Angelicae Sinensis Radix, Asini Corii Colla, and so on are used to “treating deficiency syndrome with tonifying methods”. At the same time, dysentery is often stagnant. A small amount of Rhei Radix Et Rhizoma and Coptidis Rhizoma should be increased, that are also used to clear away accumulated stagnation, such as “treating retardation syndrome with dredging methods”. Chronic diarrhea separation, should add Mume Fructus, Halloysitum Rubrum that are used to relieve diarrhea with astringents. In addition, the dysentery with cold syndrome more virtual labor, when necessary should be added with nourishing drugs. Through systematic analysis of dysentery with cold syndrome in Beiji Qianjin Yao Fang, this paper summarizes the medication rules of Sun Simiao’s treatment of dysentery with cold syndrome and discusses its syndrome and treatment characteristics, in order to provide new ideas for clinical treatment of diarrhea.

Experience of traditional Chinese medicine master Professor Wang Lie in treating recurrent respiratory tract infections in children
ZHU Jinpu1 WANG Lie2 SUN Liping2
DOI: 10.20047/j.issn1673-7210.2023.03.27
This paper summarizes the clinical experience of Professor Wang Lie, a traditional Chinese medicine master, in treating recurrent respiratory tract infections in children. According to the pathogenesis and morbidity characteristics, Professor Wang divides the disease into three phases: the acute infection phase, the non-acute infection phase and the consolidation treatment phase. Professor Wang believes that during the acute infection phase, the intertwining of external evils and internal toxins is the main pathogenesis that leads to persistent fever. The treatment should clearly distinguish between the excess and the deficiency. The patient who is suffering from excess should be given the modified Qinggan Decoction which could clear sense and toxic materials. For the patient who is suffering from deficiency, medicines that clear deficiency heat and nourish yin should be used. During the non-acute infection phase, the lung, spleen and kidney are mainly to be regulated, and the treatment relies on the syndrome differentiation and the identification of deficiencies and excesses. For the patient with food accumulation and lung heat, Professor Wang matches Scutellariae Radix and other herbs to transport the spleen. In this way, the lung heat could be cleared and accumulation and stagnation could be eliminated. If there is deficiency in the lung, spleen, and kidney, Professor Wang uses modified Yiqi Qiangshen Decoction to benefit qi and strengthen the vital qi. In the case of lung yin in deficiency syndrome, Professor Wang uses modified Bufei Decoction to moisten the lung and nourish yin. During the consolidation treatment phase, great attention should be paid to the prevent recurrence of disease after treatment, and therefore Professor Wang adopts combined internal external therapy. In this situation, the Pediatric Longmu Capsules and Yiqi Guben Capsules are commonly used in combination with the Fangfu Sachet. Scientific maintenance methods and a balanced diet are advocated as well. The clinical results are significant, and one medical record is attached to prove our view.

Experience summary of Professor Wang Junge in the differentiation and treatment of allergic rhinitis with classical prescriptions
DONG Hanyi1 WANG Junge2
DOI: 10.20047/j.issn1673-7210.2023.03.28
This article introduces Professor Wang Junge’s experience in treating allergic rhinitis with classical prescriptions. Professor Wang Junge has inherited Zhongjing’s academic system, and has been engaged in clinical medicine and clinical research for decades. He is good at using classical prescriptions in the differentiation and treatment of allergic rhinitis otolaryngology diseases. Professor Wang Junge uses “three views of the classical prescriptions” to interpret the pathogenesis of allergic rhinitis as “deficient vital qi and cold-dampness encumbering the exterior”, dual disease of the exterior and interior, and take the exterior as the anxious and suffering. The treatment should be based on the method of “warm yang and secure the exterior, dissipate cold and eliminate dampness”. According to the clinical performance, allergic rhinitis is divided into wind-cold and rheum-inverse syndrome, yang-deficiency and rheum-inverse syndrome, cold rheum transform into heat syndrome, exterior vacuity without securing and containing syndrome. Classification of pathogenesis based on six meridians syndrome differentiation, identifies the state of body fluid distribution clutch. And applying classical prescriptions based on the principle of yin and yang. To take the prescription with the principle and method, and the clinical effect is good.

Professor Lin Guohua’s subtlety of experience in the treatment of pruritic skin diseases with cotton moxibustion
XIAO Qing’e1 LIN Shiyu2 WEI Yongzheng3 ZHAN Sha4 CHEN Siyu5 JIAO Junyue1 LIN Guohua2
DOI: 10.20047/j.issn1673-7210.2023.03.29
Professor Lin Guohua’s clinical experience in applying cotton moxibustion in the treatment of pruritic skin diseases is summarized and typical cases are introduced in this paper. Professor Lin often uses cotton moxibustion to treat skin diseases. He believes that the four main contradictions of “wind pathogen”, “fire stagnation”, “dampness and turbidity” and “blood stasis” run through the whole process of pruritus. The treatment is based on the principles of “internal treatment for internal and external treatment for external” in the Sincerely True Theories of Suwen, with the theory of “searching wind and dredging collaterals”, “fire stagnation requiring dissipation” and “removing blood stasis and generating new”. Therefore, Professor Lin skillfully applies cotton moxibustion therapy, combines with pricking-cupping, fire needle therapy, cotton moxibustion and other external therapy with traditional Chinese medicine characteristics, so as to further achieve the function of promoting yang and eliminating dampness, wind expelling, fire dispersing, dampness clearing, stasis removing, and itching stopping.

Breast cancer is currently the highest incidence of malignant tumor. Chemotherapy is one of the commonly used treatments and many drugs can cause dry eyes, which seriously affects the quality of life and treatment compliance of patients. At present, there is no recommendation in the guidelines and most of the methods such as suspending chemotherapy and symptomatic treatment have great limitations. While traditional Chinese medicine has significant advantages in this aspect. Through many years of clinical observation and practice, Professor Li Peiwen has accumulated rich experience in the treatment of dry eyes related to chemotherapy for breast cancer, and believes that its basic pathogenesis is xuanfu occlusion. In terms of treatment, Professor Li Peiwen takes opening xuanfu as the method and puts forward the basic treatment of “soothing liver and regulating qi, opening xuanfu; nourishing liver and kidney, nourishing xuanfu; wind medicine rising and dispersing, clearing xuanfu”, throughout the treatment. According to different pathological factors, it is supplemented by removing dampness and heat, resolving phlegm and coagulation, removing blood stasis treatments, adding or decreasing with the syndrome. Professor Li Peiwen persists in the combination of disease differentiation and syndrome differentiation, and has achieved satisfactory clinical results.

DOI: 10.20047/j.issn1673-7210.2023.03.39
With the rapid development of rehabilitation medicine in recent years, this paper analyzes the current situation of rehabilitation quality control and refined management in rehabilitation institutions in China from many aspects, include the international situation of rehabilitation quality control, construction of quality control system in domestic rehabilitation institutions, management of key links of rehabilitation quality control etc. It is believed that the quality control system construction of rehabilitation institutions in China is generally perfect. However, the rehabilitation quality management system is still lacking, there is lack of unified standards as a whole, and it is not refined and informative enough. Even in some work, it is contrary to national policies. It also requires administrative departments at all levels, rehabilitation academic groups, and management organizations of rehabilitation institutions to actively carry out research and practical exploration around the fine management of quality control in rehabilitation institutions. Then we can form a modern rehabilitation quality control system suitable for the current situation and future development of rehabilitation services in China, and help the comprehensive and high quality development of rehabilitation.

DOI: 10.20047/j.issn1673-7210.2023.03.42
Objective To summarize the practice and effect of pharmaceutical administration in Beijing Friendship Hospital, Capital Medical University (“our hospital” for short), and to provide reference for drug management in public hospitals under the new situation. Methods The main measures of drug cost management in our hospital were introduced, and the total amount of outpatient drugs, inpatient drugs, adjuvant drugs, total number of outpatients and discharged patients were analyzed statistically from 2016 to 2019. The proportion of average outpatient drug cost, average inpatient drug cost and adjuvant drugs cost in the four years was compared, and the management effectiveness was comprehensively evaluated. Results From 2016 to 2019, the average drug cost per outpatient in our hospital gradually decreased, and decreased by 4.86%, 1.67% and 9.20% in 2017, 2018 and 2019 compared with the previous year (P<0.05). The average drug cost per hospitalization decreased year by year, and decreased by 5.31%, 0.05% and 1.65% in 2017, 2018 and 2019, respectively, compared with the previous year (P<0.05). The expense proportion of auxiliary drugs was decreased year by year, and decreased by 18.45%, 24.82% and 5.83% in 2017, 2018 and 2019 compared with the previous year. Conclusion Through multi-department co-management in our hospital, a new drug expense management mode is formed, which can be summed up as “budget leading, performance ending, to departments, overall management”. The management effectively reduces the average drug expenses per outpatient and inpatient as well as the expense proportion of auxiliary drugs, and improves the level of rational drug use in the hospital.

Analysis of problems and suggestions for optimization in the training of Western medicine learning from traditional Chinese medicine based on in-depth interviews
CAO Yu1 LIU Gang2 ZHAO Danyang1 PANG Bo3 LI Ming2 LI Fusheng2 ZHANG Chuanlong3▲
DOI: 10.20047/j.issn1673-7210.2023.03.43
Objective To discuss the problems in the training and management of “Western medicine learning from traditional Chinese medicine” (hereinafter referred to as “Western-learns-Chinese”), and to provide reference for optimizing the practice of “Western-learns-Chinese” in the future. Methods A total of 12 experts in the field of training and management of “Western-learns-Chinese” in Beijing were selected for the study, and personal one-on-one in-depth interviews were conducted with experts from the management of the institute, clinicians and university teachers. NVivo 12 Plus was used to conduct a qualitative study of the original interview data, using a three-level coding approach, and the interviews were conducted at three levels: trainee development, trainee management, and trainee certification of the “Western-learns- Chinese” training. Results Three themes at the trainee management level: the lack of perfect management standardization, the importance of establishing a sound assessment and evaluation system, and the fact that personnel selection and threshold setting should be both standardized and flexible. Four themes at the trainee training level: the training process of trainees with or without teachers needs to be further discussed, the practicality of the curriculum needs to be improved, different levels of training should be set for different trainees, and the teaching model still lacks innovation. Three themes at the trainee certification level: the lack of policy-level certification and recognition after the training of “Western-learns- Chinese” trainees relies on the introduction of relevant policies by government management departments and the strengthening of communication and collaboration between “Western-learns-Chinese” training and management- related departments. Conclusion The study proposes suggestions for improvement of the problems in the cultivation of “Western-learn-Chinese” talents. Only through the standardization, practicalization, and specialization of “Western-learns- Chinese” education can we train more medical talents with the highest level of sophistication and sincerity for the country.
