DOI: 10.20047/j.issn1673-7210.2023.21.02
Objective To study the functional role of BMH2 gene in Cryptococcus neoformans. Methods Using CRISPR-Cas9 combined electroporation, the original promoter of BMH2 gene was replaced with the inducible promoter CTR4 construction conditional knockout mutant strain. The original promoter of BMH2 gene was supplemented to construct a complement strain. The strains were cultured in basic YPD/copper-rich YPD solid medium to observe the growth of the strains; the growth curve was plotted in copper-rich YPD liquid medium; the cell morphology was observed in basic YPD/copper-rich YPD liquid medium; the hypha growth was observed in basic V8/copper-rich V8 solid medium; Basic YPD/copper-rich YPD solid medium was cultured at 25℃, 30℃, and 37℃, and the growth was observed at different temperatures; the growth was observed in copper-rich YPD and copper-rich YPD+stress solid medium. Results There was no difference in the growth of all strains in basic YPD solid medium; in copper-rich YPD solid medium, the growth of mutant train was weakened, but there was no difference between wild type and complement strain. Compared with the wild type, the survival curve of mutant train decreased, and the trend of complement strain was consistent with that of wild type. There was no difference in cell morphology of all strains in basic YPD liquid medium; in copium-rich YPD liquid medium, there was no difference in cell morphology between wild-type and complement strain, but the cells of mutant strain became larger and irregular. In copper-rich YPD liquid medium, the cell diameter of mutant strain was larger than that of wild type and complement strain (P<0.05), but there was no significant difference in cell diameter between wild type and complement strain (P>0.05). In base V8 solid medium, the mutant strain produced a small amount of hypha compared with the wild type. In copper-rich V8 solid medium, the mutant strains did not produce hypha at all compared with the wild type. In basic YPD solid medium, all strains showed no difference in growth at different temperatures; in copper-rich YPD solid medium, there was no difference in the growth of complement strain and wild type, but the growth of mutant strain showed high temperature sensitivity. Under different stress conditions, there was no difference in the growth of complement strain and the wild type. Compared with wild-type and normal growing strains without stress, mutant strain showed growth inhibition under the stress of 0.005% SDS, 0.250% CR, 1 mol/L KCL, 1 mol/L NaCl, and 1 mol/L Sorbitol; compared with the wild type, the mutant strain showed growth inhibition under the stress of 10, 15 mmol/L, and 20 mmol/L DTT; compared with the normal growing without stress, the growth of the mutant strain under the stress of 10, 15 mmol/L, and 20 mmol/L DTT was enhanced.  Conclusion BMH2 gene plays an important role in normal growth, hyphae growth, high-temperature growth, and stress response of Cryptococcus neoformans.

GENG Qiang1 ZHANG Jingjing2 Yiheshan·Ainiwaer2 ZHANG Bing2
DOI: 10.20047/j.issn1673-7210.2023.21.05
Objective To investigate the role of nuclear factor κB (NF-κB)/ cysteine aspartic acid specific protease-1 (caspase-1) in Dexmedetomidine (Dex) in alleviating myocardial vulnerability after renal ischemia reperfusion in diabetic rats. Methods Thirty SPF male Wistar rats aged 8-12 weeks, weighing 200-300 g, were fed with high fat for four weeks and intraperitoneal injection of 1% Streptozotocin (30 mg/kg) was used to establish a diabetes model. The animals were divided into control group, model group, and Dex group with eight animals in each group. Renal ischemia reperfusion model was established in the model group and the Dex group, while the control group was laparotomy only. Dex was injected into the tail vein at a rate of 2 μg/(kg·h) from 30 min before ischemia to 4 h after reperfusion in the left kidney in the Dex group, and the same amount of normal saline was injected into the model group and the control group simultaneously. At the end of the experiment, the myocardial and serum samples of the three groups were collected, and serum cardiac troponin I (cTnI), interleukin (IL)-1β, IL-18, IL-17a, IL-10, and tumor necrosis factor-α (TNF-α) levels were determined by enzyme- linked immunosorbent assay, and the apoptosis rate of cardiomyocytes was determined by TUNEL staining. The expressions of NF-κB p65, p-NF-κB p65, caspase-1, and GSDMD-N in myocardial tissues of the three groups were determined by Western blot. Results The levels of IL-1β, IL-18, cTnI, TNF-α, IL-17a, p-NF-κB, caspase-1, GSDMD-N, and apoptosis rate of cardiomyocytes in model group were higher than those in control group, and the levels of IL-10 were lower than those in control group (P<0.05). The levels of IL-1β, IL-18, cTnI, TNF-α, IL-17a, p-NF-κB, caspase-1, GSDMD-N, and apoptosis rate of cardiomyocytes in Dex group were lower than those in model group, and the levels of IL-10 were higher than those in model group (P<0.05). Conclusion Dex can reduce myocardial injury after renal ischemia-reperfusion in diabetic rats, improve cardiac function, and reduce the release of inflammatory factors, the mechanism of which may be related to inhibiting the expression of myocardial pyrogenic protein.

35 Reasearch progress of signaling pathways and related therapies in neurogenesis after ischemic stroke
CHEN Yanxi1 XU Zhidong2 SUN Fangling1 WANG Wen1
DOI: 10.20047/j.issn1673-7210.2023.21.07
Ischemic stroke is a cerebrovascular disease that seriously reduces people’s quality of life. The effective therapeutic drug of this disease, Alteplase, has a narrow thrombolytic time window, which limits the application of patients. Wnt/β-catenin, Notch, PI3K/Akt, MAPK, and Eph-ephrin pathways play important roles in the development of central nervous system and are involved in cell growth, migration, and adhesion. Since adult nerve cells growth have been stopped, reactivation of these pathways may bring hope for endogenous regeneration and repair of brain injury. Finding mechanisms and drugs targeting these pathways involved in neurogenesis will be beneficial to stroke rehabilitation. This article reviews the key processes and drug development of these pathways involved in neurogenesis.

40 Research progress on atherosclerosis induced by clonal hematopoiesis of indeterminate potential and the therapeutic effect of NLRP3 inflammasome
CHEN Yafang1 LI Si1 SHI Yang2 WEI Liping3
DOI: 10.20047/j.issn1673-7210.2023.21.08
In recent years, some studies have shown that the increase of inflammatory markers, which results from the clonal hematopoiesis of indeterminate potential (CHIP) caused by somatic cell driven gene mutation, is one of the factors for the occurrence and development of atherosclerotic cardiovascular disease. The NLRP3 inflammasome in the Pyrin domain of the nucleotide-binding oligomeric domain-like receptor family, is an important regulator of pyroptosis and plays a key role in the inflammatory process. As a specific small-molecule inhibitor, MCC950 has a selective inhibitory effect on NLRP3 inflammasome mediated pyroptosis. Therefore, MCC950 is expected to become an effective drug for the clinical treatment of atherosclerotic cardiovascular diseases caused by inflammation mediated by CHIP. Starting from the related mutation of CHIP induced gene, this paper summarizes the pathogenesis of atherosclerosis mediated by the regulation of pyroptosis by NLRP3 inflammasome, and the role and mechanism of selective inhibitor MCC950 of NLRP3 inflammasome on the induction of atherosclerosis by CHIP, so as to provide a basis for clinical treatment of atherosclerosis.

58 Research progress on the application of novel oral anticoagulants after percutaneous left atrial appendage closure
WANG Wei WANG Yong
DOI: 10.20047/j.issn1673-7210.2023.21.12
Atrial fibrillation includes valvular atrial fibrillation and non-valvular atrial fibrillation. It has the characteristics of high morbidity and poor prognosis, which seriously threatens human life and health. Percutaneous left atrial appendage closure (PLAAC) has been widely used in the treatment of patients with non-valvular atrial fibrillation who have a high risk of bleeding or contraindications to oral anticoagulants. Although PLAAC has some risks for patients who have difficulty tolerating the risks of anticoagulant therapy, it remains an important step in the treatment of patients with atrial fibrillation. Device-related thrombus and ischemic stroke are common complications after PLAAC. Anticoagulant drugs such as Warfarin and vitamin K antagonists are often used in clinical treatment, but the actual effect of these drugs is often sub-optimal. At present, various guidelines recommend the regimentation of anticoagulation therapy after PLAAC surgery, the safety and effectiveness of anticoagulation therapy, especially the novel oral anticoagulant (NOACs) after PLAAC surgery still needs further research. This article summarizes and discusses the application research of NOACs after PLAAC surgery.

The occurrence of “phlegm” is related to congenital endowments and acquired multiple factors, as well as the resulting substances such as essence, blood, and body fluid, and the retained fluid caused by accumulation of body fluid. The process of phlegm formation affects the movement and excretion of body fluid due to the obstruction of the qi transformation function of the zang-fu viscera. Liver cancer is closely related to phlegm production. Professor Wu Yu believes that “phlegm” is an important pathogenic factor of liver cancer in Chinese medicine. “phlegm” is a necessary factor in the deve- lopment and metastasis of liver cancer and a participant from beginning to end. Eliminating phlegm method is a consistent principle in the treatment of liver cancer in traditional Chinese medicine. Pneumatosis cystoides intestinalis are gas cysts occurring in the submucosa or subserous membrane of the digestive tract, with low incidence and easy misdiagnosis. This paper describes a case of pneumatosis cystoides intestinalis with persistent abdominal distension as the main symptom. After admission, colon endoscopy, endoscopic ultrasound, pathology, and CT examination were completed to confirm the diagnosis of pneumatosis cystoides intestinalis. After the treatment of integrated Chinese and Western medicine during hospitalization and oral treatment of traditional Chinese medicine after discharge, the clinical symptoms disappeared and there was no abnormality in colonoscopy review, and the overall effect was good. Pneumatosis cystoides intestinalis is a rare disease, easily confused with intestinal tuberculosis, intestinal polyps, inflammatory enteritis, intestinal tumors, and other diseases, improper treatment can lead to pneumoperitoneum, intestinal obstruction, intestinal ischemia, and other serious complications. The diagnosis and treatment of this case is detailed, the pathological results are clear, and the treatment effect of integrated Chinese and western medicine is very good, which can provide clinical reference.

influenza-associated acute necrotizing encephalopathy (IANE) is one of the most critical subtypes of an acute encephalopathy, progressing rapidly, with fever, convulsions, and rapid onset of consciousness the most common, and with a poor prognosis. In order to improve clinicians’ ability to recognize and diagnose IANE at an early stage, ensure timely treatment of patients and improve their prognosis. In this paper, analyze and discuss a patient with IANE who was seen in the Children’s Hospital of Soochow University. The child started with a high fever and showed symptoms of neurological involvement within two days. The brain MRI showed bilateral symmetrical brain damage, and the diagnosis of IANE was considered in combination with relevant laboratory tests. The child was discharged after aggressive treatment but was left with mild cognitive impairment. Through a retrospective analysis of the national and international literature, it is mainly believed that early use of intravenous glucocorticoid shock combined with Oseltamivir antiviral, as well as the use of appropriate antibiotics against infection, is the best treatment regimen for IANE.

DOI: 10.20047/j.issn1673-7210.2023.21.43
Pulmonary artery intimal sarcoma (PAIS) is a rare disease with a high misdiagnosis rate. Because the onset is easy to be confused with other cardiovascular diseases, once misdiagnosed or missed diagnosis, resulting in further development of the disease, miss the best treatment opportunity. In order to improve the level of clinical diagnosis and treatment of this disease and make correct diagnosis and treatment plan, a case of PAIS was reported and analyzed in Guangdong Provincial Hospital of Chinese Medicine. The first symptoms of the patient were progressive chest tightness and asthma. The patient was considered to be PAIS through preoperative physical examination, imaging examination, and combined with previous medical history. The pulmonary artery tumor was excised under cardiopulmonary bypass, during which gray and white masses were found to invade the whole layer of blood vessels along the main pulmonary artery. The tumor was completely resected, and PAIS was confirmed by postoperative pathological examination. The patient died of multiple organ failure one month after surgery. Through retrospective analysis of domestic and foreign literature, most of them believe that once PAIS is diagnosed, surgery and concurrent chemorotherapy should be actively treated; at the same time, literature review was carried out on the symptoms, clinical manifestations, imaging features of the disease to improve the understanding of the disease, so as to reduce misdiagnosis or missed diagnosis.
