DOI: 10.20047/j.issn1673-7210.2022.30.03
Objective To investigate the effect of Fluoxetine on synaptic plasticity of optic cortical neuron cells in monocular deprived amblyopia rats and its related mechanism. Methods Ten of 60 SPF male SD rats were selected as the control group, and the other 50 rats were used to construct the monocular deprived amblyopia rat model. The 50 rats with successful modeling were divided into positive drug group, model group, low, medium, and high dose groups by random number table method, with 10 rats in each group. The low, medium, and high dose groups were intraperitoneally injected with 5, 10, and 20 mg/(kg·d) Fluoxetine; the positive drug group was intraperitoneally injected with 80 mg/(kg·d) Levodopa; and the model group and the control group were intraperitoneally injected with the same amount of normal saline. Each group was treated for 28 days. P100 latency and P100 amplitude in each group were measured; the ultrastructural changes of synapses of neurons in visual cortex in each group were observed; the ERK1/2, p-ERK1/2, CREB, and p-CREB protein in visual cortex in each group were detected. Results The P100 latency, synaptic gap, and synaptic density thickness in the model group were higher than those in the control group, and the P100 amplitude and p-ERK1/2/ERK1/2, and p-CREB/CREB in visual cortex were lower than those in the control group (P < 0.05). The P100 latency, synaptic gap, and synaptic density thickness in the low, medium, and high dose groups were lower than those in the model group, and the P100 amplitude, p-ERK1/2/ERK1/2, and p-CREB/CREB in the visual cortex were higher than those in the model group; the P100 latency, synaptic space, and synaptic density thickness in the middle and high dose groups were lower than those in the low dose group, and the P100 amplitude, p-ERK1/2/ERK1/2, and p-CREB/CREB in the visual cortex were higher than those in the low dose group; the P100 latency, synaptic gap, and synaptic density thickness in the high-dose group were lower than those in the medium dose group, and the P100 amplitude, p-ERK1/2/ERK1/2, and p-CREB/CREB in the visual cortex were higher than those in the medium dose group (P < 0.05). Compared with the model group, the synaptic structure of the visual cortex neurons in the low, medium, and high dose groups was slightly clear, and the myelin sheath lamellar structure was clear. Conclusion Fluoxetine can remodel the synaptic ultrastructure of visual cortex neurons in monocular deprived amblyopic rats, activate the structural plasticity of visual cortex, and improve the visual function of rats, which may be achieved by activating ERK/CREB pathway.

36 Research progress of electrocardiogram automatic diagnosis based on deep learning in multiple system diseases
ZHANG Xiling WANG Xinkang
DOI: 10.20047/j.issn1673-7210.2022.30.07
Electrocardiogram is a non-invasive method for detection, which can reflect the cardiac electrophysiology. Clinical findings show that many diseases can be complicated with cardiovascular events or change the normal working state of the heart. These changes can be reflected in small changes in electrocardiogram, which are not easy to be recognized by manual or traditional machine algorithms. In recent years, as the core of artificial intelligence development, deep learning can find nonlinear and subtle changes in electrocardiogram, and its application in this field has become an inevitable trend. At present, screening, monitoring, classification, diagnosis, and prediction of a variety of diseases can be realized through deep learning models. This paper reviews the research progress of electrocardiogram automatic diagnosis based on deep learning in multiple system diseases, and analyzes its future development trend.

WEN Yan PENG Hui
DOI: 10.20047/j.issn1673-7210.2022.30.08
With the advent of anti-vascular endothelial growth factor drugs, the treatment of neovascular senile macular degeneration has been greatly developed. However, there are still some patients who have poor effect after long-term treatment with the same drug, which may be related to drug resistance and rapid drug resistance response. At this time, drug replacement is a feasible option. As a representative of fusion protein drugs, Aphercept has the advantages of high affinity binding to vascular endothelial growth factor and multiple targets. This article reviews the current research status of switching to Aphercept in patients with refractory neovascular senile macular degeneration, in order to provide ideas for the treatment of this disease.

CAI Zichun LI Jiming
DOI: 10.20047/j.issn1673-7210.2022.30.10
Arterial stiffness is recognized as an independent factor for assessing cardiovascular events. A growing number of clinical studies have shown that the assessment of arterial stiffness can predict the risk of cardiovascular disease, neurodegenerative disease, and chronic kidney disease. However, changes in arterial stiffness are often mistakenly equated with the development of arteriosclerosis or atherosclerosis, and little is known about its pathogenesis, and its rationale and limitations are unknown when applying assessment methods. Therefore, this article aims to review the concept of increased arterial stiffness, describe the research progress of researchers on the mechanism of arterial stiffness in recent years, and evaluate the principles, limitations, and clinical applications of arterial stiffness methods.

DOI: 10.20047/j.issn1673-7210.2022.30.24
Objective To establish a method for determination of Iron Polysaccharide Complex Capsules and to evaluate the practical application of the method. Methods Under heating conditions, the Iron Polysaccharide Complex solution was digested by hydrochloric acid. The free Fe3+ in the solution was reduced to Fe2+ by hydroxylamine hydrochloride, and Fe2+ reacts with 1,10-phenanthroline to form a chemically stable red complex. Based on this principle, the iron content was indirectly determined by UV-Vis spectrophotometry, and the dissolution of Iron Polysaccharide Complex Capsules was evaluated. Results The specificity, reproducibility, and stability of the method in this research were good. At the same time, the Fe2+ concentration has a good linear relationship in the range of 0.1-2.0 μg/ml (r2 = 0.999 9), and the average recovery rate was 99.3% (RSD = 1.03%). The results of actual sample measurement showed that the results of this study are basically consistent with those of the methods in the literature. Conclusion The method is simple, convenient, and suitable for determining the dissolution of Iron Polysaccharide Complex Capsules.

DOI: 10.20047/j.issn1673-7210.2022.30.25
Professor Wu Rongzu is a descendant of Yunnan School of the Fire God. Based on the in-depth study of Zheng Qinan’s fuyang thought and Peng Ziyi’s circular movement theory, Professor Wu Rongzu believes that the circular movement of human qi movement takes middle-earth as the axis, and the four images of water, fire, metal, and wood as the wheel, which together constitute the circular movement of the wheel axis. The three yin(Shaoyin, Taiyin, and Jueyin)yang qi deficiency and failure is the key pathogenesis of dysfunction of circular movement in transportation. Liver cancer is the result of the three yin yang qi unable to rise, abnormal circular movement of human qi movement, immobility of cold and dampness, phlegm coagulated by fluid, disharmony of qi and blood, blood stasis, phlegm and stasis coagulating with lump forming. In the treatment, Wuyu Sini Decoction and Linggui Zhugan Decoction are used to warm the cold of kidney water, dry the wet of spleen soil, warm the depression of liver wood. Kidney yang of Shaoyin is warmly lifted to warm the spleen yang, the axis works to change the phlegm and dampness of liver cancer; liver yang of Jueyin is warmly raised to run the outer wheel to regulate qi and blood, except the blood stasis caused by liver cancer.

116 Clinical application of  classical prescriptions in the treatment of heart failure
GUO Yuna1 CHEN Liming2 LIU Kun1
DOI: 10.20047/j.issn1673-7210.2022.30.26
Heart failure is the terminal stage of the progression of various heart diseases, and its pathological mechanisms mainly include oxidative stress, inflammatory response, mitochondrial dysfunction, and abnormal calcium cycle. At present, western medicines for the treatment of heart failure mainly include angiotensin receptor enkephalinase inhibitor, angiotensin-converting enzyme inhibitor, angiotensin Ⅱ receptor blocker, β-receptor blocker blockers, aldosterone receptor antagonists, and sodium-glucose cotransporter 2 inhibitors. However, long-term use can lead to adverse reactions such as hypotension, electrolyte imbalance, and even increase the readmission rate. Reducing the fatality rate and readmission rate of heart failure and improving the quality of life of patients are clinical difficulties to be solved urgently. Traditional Chinese medicine has a long history of treating heart failure. Through long-term clinical observation, it has been found that traditional Chinese medicine has obvious advantages in improving the clinical symptoms of heart failure patients and reducing the recurrence rate. Based on the analysis of prescriptions related to heart failure in treatise on cold pathogenic diseases and synopsis of the golden chamber, this paper expounds the effects of Lingguizhugan Decoction, Zhenwu Decoction, Fuling Gancao Decoction, Xiaoqinglong Decoction, Guizhi Qushaoyao Decoction and Shenqi Pills in the development of heart failure. Different stages of application, explore the application of traditional Chinese medicine in the treatment of heart failure, in order to better guide the clinical.

DOI: 10.20047/j.issn1673-7210.2022.30.28
Acquired immunodeficiency syndrome has always been one of the major infectious diseases threatening the safety of human life, and its prevention and treatment is the key and difficult problem to be solved clinically. Through many years of clinical practice, Professor Li Fazhi believes that the disease manifestation of acquired immunodeficiency syndrome is similar to the “consumption disease” of traditional Chinese medicine. Professor Li Fazhi pays attention to the theoretical idea that the spleen and stomach is the foundation of the five viscera, and emphasizes the nursing effect of the spleen and stomach on the five viscera. He points out that acquired immunodeficiency syndrome attacks people. First of all, acquired immunodeficiency syndrome damages the spleen and stomach. Gradually, acquired immunodeficiency syndrome leads to the loss of the heart, lung, kidney, and liver. Finally, acquired immunodeficiency syndrome causes the weakness of qi and blood, yin and yang of the five viscera. And it also causes the accumulation of pathological products such as blood stasis and phlegm, thus leading to a variety of concomitant diseases. The key to the pathogenesis is that the human immunodeficiency virus damages the primordial qi, spleen and stomach deficiency accompanied by internal dampness, loss of nourishment of the five viscera, weakness of qi and blood, and yin and yang loss. The change of the pathogenesis throughout the whole process of acquired immunodeficiency syndrome. Professor Li Fazhi emphasizes the importance of strengthening the spleen and stomach and caring for the middle soil in the treatment to correct the deficiency of the five viscera and various complications caused by human immunodeficiency virus. At the same time, he attaches great importance to distinguishing between deficiency and excess to achieve the effect of dispelling the evils of acquired immunodeficiency syndrome and nourishing primordial qi. The case is attached here to support the theory.

132 Treatment of chronic heart failure by Professor Zhou Yabin on the “Four-Dimensional theory”
SUN Jing1 CAI Rui2 MAO Qichao2
DOI: 10.20047/j.issn1673-7210.2022.30.30
With the gradual aging of the population, the incidence rate of chronic heart failure is increasing, and the mortality of heart failure is also on the rise. Chronic heart failure is a common clinical syndrome in cardiology, which is mainly caused by insufficient cardiac output due to ventricular filling and / or impaired ejection function. This article summarizes Professor Zhou Yabin’s clinical experience in the treatment of chronic heart failure, and believes that the pathogenesis of this disease is complex and the treatment methods are changeable. Paying attention to the changing characteristics of a single disease pathogen, it is also necessary to grasp the overall disease situation, and integrate and understand it according to the overall syndrome differentiation concept of traditional Chinese medicine, so as to control the changes of the disease more profoundly.

137 Professor Wan Donggui’s experience in treatment of breast cancer related lymph-edema from the theory of “Quyu Chencuo ”
ZHANG Chenyang1 CHEN Hongyang1 CHEN Dongmei2 SU Fei2 WAN Donggui2
DOI: 10.20047/j.issn1673-7210.2022.30.31
Professor Wan Donggui analyzes breast cancer related lymph-edema and proposes that the disease is located in the collaterals, which is the disease of deficiency in origin and excess in superficiality, which is caused by original deficiency. The key pathological factors of breast cancer related lymph-edema are fluid retention and blood stasis. The key pathological basis of breast cancer related lymph-edema is the deficiency of viscera. The key pathogenesis of breast cancer related lymph-edema is the accumulation of fluid retention and blood stasis and the obstruction of collaterals. In prodromal phase of the disease, the key pathogenesis is the deficiency of qi and blood and the newly produced fluid retention and blood stasis. In the symptomatic period, the key pathogenesis is the weakness of viscera and the gradual filling of fluid retention and blood stasis, and at the vesting period of the disease, the key pathogenesis is the adhesion of fluid retention and blood stasis. The principle of treatment should be “Quyu Chencuo”. The method is to remove fluid retention and blood stasis while filling the deficiency of viscera to eliminate the cause of blood stasis and water stasis. In the prodromal phase, invigorating the spleen and kidney, nourishing qi and blood is critical. In the symptomatic period, it is quite important to combine dredging method with tonifying method and to resolve fluid retention and invigorate the circulation of blood. In the vesting period, the key treatment are dissolving blood stasis, and expelling excessive fluid. This paper is intended to elaborate the academic thought and treatment experience of the “theory of Quyu Chencuo” on breast cancer related lymph-edema in order to broaden our mind on the treatment of this disease.

145 Treatment of tourette syndrome based on Lu Zhizheng’s “hold the center and move around” method
WANG Bo1 ZHANG Huadong2
DOI: 10.20047/j.issn1673-7210.2022.30.33
The etiology of tourette syndrome is complex and the pathogenesis is diverse, although the main symptoms can be quickly alleviated by treating only one viscera, it is often easy to relapse and the curative effect is poor. Based on the spleen and stomach, it can take into account the five zang viscera, six fu viscera and all limbs, which is more in line with the etiology and pathogenesis of tourette syndrome, meanwhile, based on the characteristics of the spleen and stomach, the medicine is light and does not hurt the viscera. It is also more in line with the characteristics of children’s long-term physical function weakening and children’s tender yin and yang elements. It can achieve better curative effect in the clinical treatment of tourette syndrome. This method is not only suitable for the etiology and pathogenesis of tic, but also suitable for the physical characteristics of children.

DOI: 10.20047/j.issn1673-7210.2022.30.43
Psittacosis is an infectious disease caused by chlamydia psittaci infection. Due to the nonspecificity of its clinical symptoms and the limitations of detection methods, it is easy to miss diagnosis and misdiagnosis in clinical work. This paper reports a case, male, 67 years old, who was admitted to the first hospital of Lanzhou University on June 25, 2021. The main reason was “chest tightness and shortness of breath with fever for five days”. The empirical anti-infection effect was not good and worsened progressively, chlamydia psittaci infection was confirmed by macrogene next-generation sequencing, and antibiotics were adjusted to give effective anti-infection treatment, the patient’s condition improved and was discharged. This paper sortes out the medical records and analyzes the related literature, the clinical manifestations, diagnosis and treatment of psittacosis infection are reviewed, to improve clinicians’ awareness of the disease.
