7 Clinical effect of Jiawei Xiaoyao Powder in the treatment of depression with liver stagnation and spleen deficiency

Objective To observe the clinical effect of Jiawei Xiaoyao Powder in the treatment of depression with liver stagnation and spleen deficiency. Methods A total of 72 patients with mild to moderate depression of liver stagnation and spleen deficiency type admitted to the Third School of Clinical Medicine of Beijing University of Chinese Medicine from August 2020 to June 2021 were selected, according to the random number table method, they were divided into experimental group and control group, with 36 cases in each group. The experimental group was given internal application of Jiawei Xiaoyao Powder combined with Fluoxetine, and the control group was given internal application of the western medicine Fluoxetine, both of which were treated for four consecutive weeks. The antidepressant treatment effect, Hamilton depression scale (HAMD) score, traditional Chinese medicine syndrome score, and HAMD factor scores were compared between the two groups, and adverse events during the treatment were recorded. Results After treatment, the antidepressant treatment effect of the experimental group was better than that of the control group (P < 0.05). Overall analysis showed that the total score of HAMD and the total score of traditional Chinese medicine syndrome had statistical significance in the comparison of time points and the difference of interaction (P < 0.05). After 2 and 4 weeks of treatment, the total HAMD score and traditional Chinese medicine syndrome score of the two groups were lower than those before treatment (P < 0.05). After treatment, the scores of all factors in the experimental group were better than those before treatment, while the scores of anxiety/somatization, cognitive impairment, day/night variation, delay/block, sleep disorder and hopelessness in the control group were better than those before treatment, and the scores of anxiety/somatization and sleep disorder in the experimental group were better than those in the control group (P < 0.05). There was no obvious discomfort during treatment in the experimental group. One case of constipation and one case of dry mouth occurred in the control group, which were improved after patients were advised to increase dietary fiber intake and daily water intake, without drug withdrawal or special treatment. Conclusion Jiawei Xiaoyao Powder combined with Fluoxetine in the treatment of depression with liver stagnation and spleen deficiency is more effective than Fluoxetine alone, and has certain advantages in improving anxiety and depression, somatization discomfort and sleep quality, has high safety and is worthy of clinical promotion.

11 Regulatory effect and diagnostic value of piR-128 on colorectal cancer through DNA methylase 3B

Objective To detect the regulatory effect and diagnostic value of piR-128 on colorectal cancer through DNA methylase (DNMT) 3B. Methods A total of 54 patients diagnosed with colorectal cancer in the Department of General Surgery, the First Affiliated Hospital of Hebei North University from February 2018 to January 2020 were selected. The expression levels of piR-128 and DNMT1, DNMT3A, and DNMT3B in colorectal cancer tissues and adjacent normal tissues were detected by qRT-PCR. Spearman correlation analysis was used to evaluate the association between piR-128 and DNMT3B, and the relationship between patients with different levels of piR-128 and DNMT3B mRNA expression and their clinical characteristics was further analyzed. Receiver operation characteristic curve was used to evaluate the diagnostic and prognostic value of piR-128 in colorectal cancer. Human colorectal cancer cell line LIM1215 was selected and divided into control group (transfected with blank inhibitor and si-blank), piR-128 inhibitor group (transfected with piR-128 inhibitor), SI-DNMT3B group (transfected with si-DNMT3B) and inhibitor +Vector group (transfected with piR-128 inhibitor and DNMT3B Vector). Western blot and qRT-PCR were used to analyze the mutual regulatory relationship between piR-128 and DNMT3B. Transwell assay, cell scratch assay, and flow cytometry were used to analyze the effects of piR-128 and DNMT3B on the invasion, migration, and apoptosis of colorectal cancer cells. Results The levels of piR-128 and DNMT3B mRNA expression in colorectal cancer tissues were higher than those in adjacent normal tissues, and the differences were statistically significant (P < 0.05). The results of correlation analysis showed that there was a positive correlation between the expression level of piR-128 and DNMT3BmRNA in colorectal cancer tissues (rs > 0, P < 0.05). The levels of piR-128 and DNMT3B mRNA in the piR-128 inhibitor group and inhibitor +Vector group were lower than those in the control group, the levels of DNMT3B mRNA in the si-DNMT3B group were lower than those in the control group, and the levels of DNMT3B mRNA in the inhibitor +Vector group were higher than those in the si-DNMT3B group, the protein levels of DNMT3B in the piR-128 inhibitor group, si-DNMT3B group, and inhibitor +Vector group were lower than those in the control group, and the protein level of DNMT3B in the inhibitor +Vector group were higher than those in the si-DNMT3B group, and the differences were statistically significant (P < 0.05). The absorbance of si-DNMT3B group, piR-128 inhibitor group, and inhibitor +Vector group at 24, 36, 48, and 72 h were lower than those of the control group, and the differences were statistically significant (P < 0.05). The invasion ability and migration rate of cells in the piR-128 inhibitor group, si-DNMT3B group, and inhibitor +Vector group were lower than those in the control group, and the apoptosis rate was higher than that in the control group, the invasion ability and migration rate of cells in the inhibitor +Vector group were higher than those in the si-DNMT3B group and piR-128 inhibitor group, and the apoptosis rate was lower than that in the si-DNMT3B group, the differences were statistically significant (P < 0.05). Conclusion piR-128 participates in the progression of colorectal cancer through DNMT3B.

17 Experimental study of IGF1 gene lentivirus transfected bone marrow mesenchyml stem cells in the treatment of osteoarthritis

Objective To investigate the effect of IGF1 gene lentivirus transfected bone marrow mesenchyml stem cells (BMSCs) in the treatment of osteoarthritis (OA) based on animal experiments. Methods A total of 40 SPF male Wistar rats (10-12 week old, weighing 280-320 g) were divided into BMSCs+IGF1 group, BMSCs group and model group according to the random number table method, with ten rats in each group. The OA model was established by anterior cruciate ligament resection of knee joint, and the other ten rats were used for BMSCs isolation. The 0.1 ml IGF1 gene lentivirus transfected BMSCs (1×107 cells) were injected into knee joint cavity in rats of BMSCs+IGF1group. The 0.1 ml BMSCs (1×107 cells) were injected into knee joint cavity in rats of BMSCs group. The 0.1 ml normal saline were injected into knee joint cavity in rats of model group. The serum levels of C-terminal peptide of type Ⅰ collagen (CTX-Ⅰ), CTX-Ⅱ, interleukin (IL)-1α, and IL-6 in rats were determined by enzyme-linked immunosorbent assay. The mRNA expression levels of osteocalcin (OC), glycogen synthase kinase-3β (GSK-3β), alkaline phosphatase (ALP), and Runt-associated transcription factor 2 (Runx2) in the knee cartilage tissues of rats were detected by real-time quantitative polymerase chain reaction. The protein expression levels of β-catenin,Wnt1, and GSK-3β in the knee cartilage tissues of rats were detected by Western blot. Results BMSCs surface antigens CD29,CD34,CD45, and CD105 were highly expressed, and the transfection efficiency of lentivirus was (90.36±2.18)%. The knee cartilage of rats was intact without obvious degeneration and the articular surface was flat in BMSCs+IGF1 group. The other two groups had varying degrees of cartilage degeneration and uneven articular surface. Compared with the model group, the serum levels of CTX-Ⅰ, CTX-Ⅱ, IL-1α, and IL-6, the mRNA expression levels of OC, GSK-3β, ALP, and Runx2, the protein expression levels of β-catenin, Wnt1, and GSK-3β were decreased in the BMSCs group, the differences were statistically significant (P < 0.05). Compared with the BMSCs group, the above indexes in the BMSCs+IGF1 group were decreased, the differences were statistically significant (P < 0.05). Conclusion IGF1 gene lentivirus transfected BMSCs can effectively improve the pathological injury of OA rats, and significantly promote the repair of knee joint injury.

23 Experimental study on silymarin regulating proliferation and apoptosis of non-small cell lung cancer cells through Slit2

Objective To study the effect and mechanism of silymarin (SM) on proliferation and apoptosis of non-small cell lung cancer (NSCLC) cells through Slit2. Methods NSCLC cell line A549 was cultured and divided into control group and SM groups with different doses (10, 20 and 40 mg/L), si-negative control (NC) group transfected with NC siRNA, si-NC+40 mg/L SM group transfected with NC siRNA and intervened with 40 mg/L SM, si-Slit2+40 mg/L SM group transfected with Slit2 siRNA. After 48 hours of intervention, cell proliferation A490 level, apoptosis rate, and the expression levels of Slit2, ROBO1, and C-X-C motif chemokine receptor 4 (CXCR4) were detected. Results The A490 level and apoptosis rate of A549 cells in 10, 20 and 40 mg/L SM groups were lower than those in control group, and the differences were statistically significant (P < 0.05). The A490 level of A549 cells in 20 mg/L SM group was lower than that in 10 mg/L SM group, and the apoptosis rate was higher than that in 10 mg/L SM group, the differences were statistically significant (P < 0.05). The A490 level of A549 cells in 40 mg/L SM group was lower than that in 10 and 20 mg/L SM groups, and the apoptosis rate was higher than that in 10 and 20 mg/L SM groups, and the differences were statistically significant (P < 0.05). Compared with the control group, the expression levels of Slit2 and ROBO1 in A549 cells of 10, 20 and 40 mg/L SM groups were increased, and the expression level of CXCR4 was decreased, and the differences were statistically significant (P < 0.05). Compared with 10 mg/L SM group, the expression level of CXCR4 in A549 cells in 20 mg/L SM group was decreased, and the difference was statistically significant (P < 0.05). The expression levels of Slit2 and ROBO1 in A549 cells in 40 mg/L SM group were increased, and the expression level of CXCR4 was decreased, the differences were statistically significant (P < 0.05). Compared with 20 mg/L SM group, the expression levels of Slit2 and ROBO1 in A549 cells in 40 mg/L SM group were increased, and the expression level of CXCR4 was decreased, and the differences were statistically significant (P < 0.05). Compared with SI-NC group, the expression level of Slit2 and apoptosis rate of A549 cells in SI-NC +40 mg/L SM group were increased, and the level of A490 was decreased, and the differences were statistically significant (P < 0.05). Compared with SI-NC +40 mg/L SM group, the expression level of Slit2 and apoptosis rate of A549 cells in SI-SLIT2 +40 mg/L SM group were decreased, and the level of A490 was increased, and the differences were statistically significant (P < 0.05). Compared with si-NC group, the expression level of ROBO1 in A549 cells of si-NC+40 mg/L SM group was increased, and the expression level of CXCR4 was decreased, and the differences were statistically significant (P < 0.05). Compared with si-NC+40 mg/L SM group, the expression level of ROBO1 in A549 cells in si-slit2 +40 mg/L SM group was decreased, and the expression level of CXCR4 was increased, and the differences were statistically significant (P < 0.05). Conclusion SM inhibits the proliferation and CXCR4 expression, promotes cell apoptosis in NSCLC cell by activating Slit2 / ROBO1 pathway.

28 Relationship between perioperative period blood transfusion and postoperative survival and tumor-free survival after cholecystectomy

Objective To explore the relationship between perioperative period blood transfusion and postoperative survival and tumor-free survival after cholecystectomy. Methods The clinical data of 72 patients who underwent cholecystectomy in Jinhua People’s Hospital and Gansu Provincial People’s Hospital from June 2015 to June 2019 were retrospectively analyzed. The two groups of basic data were matched by propensity score matching. The general data, two-year overall survival rate, and tumor-free survival rate were compared between the two groups. Cox risk regression model was used to analyze the risk factors of postoperative survival and tumor-free survival. Results After propensity score matching, 60 cases of the 72 patients were successfully matched, 30 cases in the transfusion group and 30 cases in the non-transfusion group. There was no significant difference in general data between the two groups (P > 0.05). The follow-up ended in June 2021, and the median survival time of the patients in the blood transfusion group and the non-transfusion group were 12 and 24 months, respectively. There was a statistically significant difference in the overall survival time between the two groups (P < 0.05); there was a statistically significant difference in the tumor-free survival time between the two groups (P < 0.05). Cox risk regression model analysis showed that after adjusting for TNM stage and liver invasion factors, perioperative blood transfusion increased the risk of death by 2.021 times and increased the risk of tumor recurrence by 1.762 times. Conclusion Perioperative blood transfusion reduces two-year overall survival and tumor-free survival in patients undergoing cholecystectomy.

33 Study on the disgnostic efficacy of contrast-enhanced ultrasound and ultrasound elastosonography in benign and malignant breast masses

Objective To compare the clinical value of contrast-enhanced ultrasound and ultrasound elastosonography in the differential diagnosis of benign and malignant breast masses. Methods Ninety patients with breast masses admitted to the Department of Breast Surgery, the First People’s Hospital of Lianyungang of Jiangsu Province from January to December 2020 were selected. Before operation, all patients were examined by contrast-enhanced ultrasound and ultrasound elastosonography, the imaging features, accuracy, specificity, and sensitivity of differential diagnosis of benign and malignant masses were compared. Results A total of 90 cases of breast mass were proved to be benign in 52 cases and malignant in 38 cases. The sensitivity and specificity of contrast-enhanced ultrasound in the diagnosis of benign and malignant breast masses were 86.84% and 84.62%, respectively; Kappa value was 0.66, with medium consistency. The sensitivity and specificity of ultrasound elastography in diagnosing benign and malignant breast masses were 92.11% and 90.38%, respectively; Kappa value was 0.82, which was in good agreement. Conclusion Contrast-enhanced ultrasound and ultrasound elastography have high diagnostic efficiency in the differential diagnosis of benign and malignant breast masses.

37 Research progress on rehabilitation mechanism of acupuncture-rehabilitation therapy in the treatment of stroke

In recent years, the incidence rate of stroke has been increasing. After treatment, patients often have various sequelae, which has brought heavy burden to families and society. As an effective strategy for rehabilitation after stroke, acupuncture and massage is widely used in clinical practice, it refers to the organic combination of the advantages of acupoint cluster acupuncture in the treatment of brain-derived diseases and modern rehabilitation methods. This paper introduces the definition, guiding ideology, and clinical application of acupuncture-rehabilitation therapy, and summarizes its rehabilitation mechanism in the treatment of stroke, mainly including promoting vascular regeneration, inhibiting cell apoptosis, controlling inflammatory response, remodeling neurovascular unit, regulating intestinal flora and other mechanisms, so as to provide theoretical basis and ideas for clinical rehabilitation after stroke.

41 Research progress on the mechanism of action of gas signal molecules on idiopathic pulmonary fibrosis

The gaseous signaling molecules nitric oxide, carbon monoxide, and hydrogen sulfide have anti-inflammatory, antioxidant, and anti-apoptotic properties. In addition, by regulating TGF-β/Smads, PI3K/AKT, Nrf2/HO-1, and nuclear factor-κB signaling pathways, it can inhibit the proliferation and activation of fibroblasts, reduce collagen deposition and epithelial-mesenchymal transition, thereby reducing the proliferation and activation of fibroblasts, and improving idiopathic pulmonary fibrosis. This article reviews the anti-pulmonary fibrosis effects of three gas signal molecules and their molecular mechanisms, and opens up a new therapeutic approach for the use of gas signal molecules and their donors in the treatment of pulmonary fibrosis in the future.

46 Research advances in non-surgery treatment of tooth enamel opaque spots

Tooth enamel opaque spots is a chalky spot or spot formed by enamel demineralization, if not controlled in time, it will continue to develop and form cavities. Dental caries in children has the characteristics of high incidence and early onset, among which tooth enamel opaque spots is a common feature. With the increase of early treatment and orthodontic treatment in children, enamel demineralization, and tooth enamel opaque spots are more likely to occur. At present, non-surgical treatment is generally adopted in clinical practice to terminate or eliminate the lesion. Fluoride is a classic remineralization therapy. Amorphous calcium phosphate and its derivatives can mimic the physiological enamel mineralization process. In addition, bioactive glass and self-assembling peptide P11-4 can also promote the remineralization of demineralized enamel. Resin penetration and micro-grinding can obtain timely and beautiful treatment effect of tooth enamel opaque spots. This article reviews the research progress of enamel remineralization system and non-surgical treatment, so as to provide reference for the control of caries and the treatment of tooth enamel opaque spots.

50 Research progress of ferroptosis function in the tumor-infiltrating lymphocyte

Ferroptosis is a novel kind of cell death which is induced by the accumulation of iron-dependent lipid peroxidation. It has been proved ferroptosis modulates the tumorigenesis and tumor progression. Tumor infiltrating immune cells are critical components of tumor microenvironment. By modulating immune response, tumor infiltrating immune cells regulate tumor development and affect immunotherapy response. Previous studies indicate ferroptosis affects tumor infiltrating immune cell function to modulate tumor progression. This review introduces the molecular mechanism of ferroptosis and summarizes the regulatory role of ferroptosis in tumor infiltrating immune cells, which is expected to provide new strategy to improve immunotherapy efficacy.

54 Research progress on biological characteristics of interleukin-38 and its role in autoimmune diseases

Interleukin (IL)-38 is a newly discovered anti-inflammatory cytokine and a member of the IL-1 family, which can be widely expressed in a variety of human cells and tissues. Its gene sequence is highly homologous to the genes encoding IL-1 receptor antagonist (IL-1Ra) and IL-36 receptor antagonist (IL-36Ra). Thus, the antagonism of IL-1Ra and IL-36Ra was similar. IL-38 can act on a variety of immune cells, affect the secretion of cytokines, and play an important role in the regulation of immune response. Studies have shown that IL-38 is closely related to the occurrence and development of a variety of autoimmune diseases, which is expected to become a potential target in the future clinical treatment of these diseases. The mechanism of IL-38 is complex, involving the recruitment of specific receptors and the regulation of T helper cell 17 differentiation. A deeper understanding of the role of IL-38 in autoimmune diseases will help further explore its application value in the diagnosis and treatment of diseases. In this review, the biological characteristics of IL-38 and its role in autoimmune diseases are reviewed.

59 Research advance of long non-coding RNA and chronic pain

Chronic pain is one of the diseases that have the greatest negative impact on people’s quality of life in the world, and brings huge economic burden to society. The drug methods for treating chronic pain are limited and ineffective. Long non-coding RNA (lncRNA) is a class of non-coding RNA with sequence length more than 200 nucleotides and lack of protein-coding potential, but it plays an extensive and important biological regulatory role in vivo. Recent studies have found that lncRNA expression is dysregulated in a variety of chronic pain models, and it plays an important role in the development of chronic pain by affecting the function of various micrornas, inflammatory cytokines and ion channel activation. In this paper, lncRNA related studies involved in neuropathic pain, chronic inflammatory pain and bone cancer pain in recent years are analyzed and summarized, so as to discover valuable therapeutic targets and lay a theoretical foundation for the development of more effective analgesics.

63 Research progress on cerebellar regulation of swallowing nerve mechanism

Swallowing is an activity controlled by a complex nervous system, involving the brain stem, cerebral cortex, and cerebellum. The cerebellum, a key part of the brain responsible for the regulation of movement, is involved in the neurophysiological control of swallowing, and cerebellar activity is enhanced during swallowing. In addition, there is a pathological connection between cerebellum and swallowing. Stimulation of cerebellum can improve the control of swallowing movement by the nervous system and trigger the pharyngeal motor response. This review focuses on the role of the cerebellum in swallowing control, explores new findings from nerve stimulation and imaging studies, better understands the connection between the cerebellum and the swallowing motor area of the brain in healthy conditions, and outlines the clinical application of cerebellum stimulation in the treatment of dysphagia, in order to provide a potential treatment for neurogenic dysphagia.

67 Clinical research progress of polysaccharide iron complex in the treatment of iron deficiency anemia in pregnancy and its effect on neonatal outcome

Iron deficiency and iron deficiency anemia have serious short-term and long-term effects on pregnant women and their fetuses. The consensus of clinical guidelines in many countries suggests iron supplementation for iron deficiency and iron deficiency anemia in pregnancy. Clinical studies have found that polysaccharide iron complex is a kind of clinical oral organic iron agent with high iron content, high bioavailability, and little gastrointestinal irritation. Polysaccharide iron complex in the treatment of iron deficiency and iron deficiency anemia in pregnancy effect significantly, can significantly improve maternal natural births, reduce premature occurrence, increase the weight of newborn babies, reduce the incidence of fetal distress, and neonatal asphyxia pregnancy outcomes, and with high safety in clinical use at the same time, only a tiny nausea, constipation, and other adverse reactions occur. This paper reviews this issue, which aims to provide a new drug choice for the clinical treatment of iron deficiency and iron deficiency anemia in pregnant women and improve maternal and infant outcomes.

The medical science has both natural and humanistic attributes. In the new era, the concept of “to focus on health promotion” for medicine development, fully reflects the “people-oriented” idea. Enriching the humanistic connotations and improving the humanistic quality are requirements to advance the professionalism of stomatology students and to train the dentists of a new era, as well as the need to build a harmonious doctor-patient relationship and promote the development of medicine. However, at present there are still some problems such as insufficient attention to the humanistic quality education of stomatology students, and the related curriculum, education methods, and evaluation systems still need to be improved. Combining teaching practice and based on the professional characteristics of stomatology, this paper discusses the reform strategies of stomatology education led by medical humanities from the aspects of education concept, teacher team construction, curriculum setting, teaching mode, training plan, teaching evaluation and educational environment, in order to strengthen the humanistic quality education and comprehensive ability training of stomatology students, and lay the foundation for cultivating stomatology talents who meet the needs of the new era.

76 Exploration of teaching practice of patch-clamp technique in medical undergraduate experimental course

Patch-clamp recording technology is the core technology in neuroelectrophysiological research, and it is an important technical means to detect the firing activity of excitable cells (neurons and cardiomyocytes, etc.) at the single cell level in the field of basic scientific research. In addition, due to the authority and accuracy of patch-clamp technology in the study of physiological functions at the cell level, the study and mastery of this technology for medical undergraduates will greatly promote the understanding and application of the chapter “basic functions of cells”, and lay a good foundation for the subsequent theoretical study. Currently, due to the high hardware requirement and complex operation procedures, it is still very difficult to apply this technique in the experimental course of medical undergraduate students. The department of physiology in the Army Medical University (hereinafter referred to as “our school”) has applied patch-clamp recording in scientific research for a long time. On this basis, the department of physiology of our school has made efforts to apply this important technique in the experimental course of undergraduate students who major in biotechnology first, and then apply it in the physiological innovation experimental class of medical undergraduate students in recent years. In this process, the department of physiology of our school aim at arousing the students’ interest for basic research and cultivating their innovative practice ability. Here, the present papers have summarized the contents, methods, as well as experiences during this teaching exploration, in order to further improve the introduction of patch-clamp recording technique into the experimental course of undergraduate students.

Biopharmaceuticals is the required course for pharmacy undergraduates, and plays an important role in the whole teaching process of pharmacy and biotechnology. Curriculum ideological and political education in colleges and universities refers to that dip the ideological and political education resources of each course, achieving ideological, and political education function in each course with all staff, whole process and all-round participation. Curriculum ideological and political education is not a course, but a kind of educational idea and educational mode. According to the characteristics of biopharmaceutics and educational concept, this paper explores how to integrate the ideological and political element into the biopharmaceutical curriculum. By improving the means and methods, improving the teaching supervision system, and the construction of teaching team, the quality of ideological and political education has been improved, the enthusiasm and initiative of students have been greatly  aroused, the creativity of students has been stimulated, and good teaching effect has been achieved.

In this paper, the coping strategies for the common problems in the course of antibiotic teaching are analyzed, in order to provide teaching ideas for the further promotion of clinical pathophysiology and therapy teaching model in clinical teaching.

88 Influence of integrated clinical teaching platform on the post competency of intern medical students

Objective To explore the influence of integrated clinical teaching platform on the post competency of medical students. Methods The implementation of integrated clinical teaching platform construction in Department of Breast Surgery and Department of Cardiology of the Affiliated Tumor Hospital of Harbin Medical University in July 2020 was taken as the dividing line, 50 intern medical students who received conventional teaching method from July 2019 to June 2020 were enrolled as the control group, and another 50 intern medical students trained by integrated clinical teaching method between July 2020 and June 2021 were included in the observation group. The examination results, clinical competency, critical thinking ability (critical thinking disposition inventory-Chinese version [CTDI-CV]), and teaching evaluation of the two groups were compared before and after the reform of teaching platform. Results The examination results of interns in the observation group were higher than those in the control group (P < 0.05). After the internship, the scores of each part of job competency and the CTDI-CV score of the two groups were increased than before internship, and the observation group was higher than the control group (P < 0.05). The scores of parts of teaching evaluation of interns in the observation group were significantly higher compared with those in the control group (P < 0.05). Conclusion Integrated clinical teaching platform can significantly improve the theoretical knowledge and practical skills of medical interns, and enhance the post competency and critical thinking ability. In addition, it has good clinical teaching evaluation, thus it can be used for clinical teaching.

96 Meta-analysis of the clinical effect of different sputum suction methods on mechanically ventilated patients

Objective To evaluate the clinical effect of different sputum suction methods on the prevention of ventilator-associated pneumonia (VAP). Methods The Cochrane Library, Wanfang Data, PubMed, VIP, and Chinese journal full-text databases were searched to collect randomized controlled trials (RCTs) comparisons of the effects of open sputum suction (OTSS) and closed sputum suction (CTSS) on VAP in mechanically ventilated patients trial, the search period was from the establishment of the database to May 2021. Two researchers conducted quality evaluation and data extraction of the literature, and used RevMan 5.3 statistical processing to conduct a meta-analysis of the included RCTs. Results A total of 15 RCTs with a total of 3 029 patients were included. Meta-analysis results showed that CTSS could reduce the incidence of VAP (OR = 0.74, 95%CI[0.59 to 0.93], P = 0.01) and the incidence of arrhythmia (OR = 0.23, 95%CI [0.07 to 0.74], P = 0.01), there was no statistically significant difference between the two in terms of shortened ICU stay (MD = -0.13, 95%CI[-2.23 to 1.96], P = 0.9), decreased respiratory tract microbial colonization rate (OR = 1.14, 95%CI[0.84 to 1.23], P = 0.89). Conclusion CTSS has advantages over OTSS in preventing the occurrence of VAP and reducing arrhythmia in mechanically ventilated patients, but not in shortening ICU hospital stay, reducing mortality, shortening mechanical ventilation time and reducing respiratory tract microbial colonization rate.

102 Clinical characteristics and influencing factor of prognosis of human immuno-deficiency virus combined with disseminated Mycobacterium avium complex patients in parts of northern China

Objective To investigate the clinical characteristics and influencing factor of prognosis factors of human immunodeficiency virus (HIV) with disseminated Mycobacterium avium complex (DMAC) patients in some areas of northern China. Methods The clinical data of HIV with DMAC patients who were hospitalized in Beijing Youan Hospital, Capital Medical University, Tianjin Second People’s Hospital, and Shijiazhuang Fifth Hospital from January 2014 to January 2020 were collected, and the information of HIV with disseminated Mycobacterium tuberculosis (DMTB) patients who were hospitalized in these three hospitals at the same time were also collected. The clinical characteristics of HIV with DMAC patients were compared, and further follow-up was conducted to analyze the influencing factor of prognosis. Results The levels of alkaline phosphatase and γ-glutamyl transpeptidase, the incidence of hepatomegaly, the incidence of splenomegaly, and the initiation rate of anti-retroviral therapy (ART) in HIV with DMAC patients were higher than those in HIV with DMTB patients, the incidence of pleural effusion was lower than that of HIV combined with DMTB infection, and the differences were statistically significant (P < 0.05). The proportion of non-initiated ART, the level of CRP, and HIV viral load in patients who died after HIV with DMAC were higher than those in survival patients, and the level of HGB was lower than that in survival patients, and the differences were statistically significant (P < 0.05). ART initiation, hemoglobin, and C-reactive protein were prognostic factors of HIV with DMAC patients (P < 0.05). Conclusion The clinical and imaging features are helpful for the preliminary differentiation between HIV with DMAC patients and HIV with DMTB patients. ART is of great significance for the prognosis of HIV with DMAC patients.

106 Application effect of Braun anastomosis in pancreaticoduodenectomy

Objective To investigate the application effect of Braun anastomosis in pancreaticoduodenectomy. Methods A prospective study was conducted on 101 patients who underwent pancreaticoduodenectomy in the Department of General Surgery, Yancheng NO.1 People’s Hospital of Jiangsu Province from February 2017 to February 2020. They were grouped according to their wishes. In the traditional group, 49 cases were treated with standard pancreaticoduodenectomy. In the modified group, 52 cases received lateral small bowel Braun anastomosis on the basis of standard pancreaticoduodenectomy. The relevant laboratory indicators, perioperative indicators, and postoperative complications were compared between the two groups. Results The levels of albumin, total cholesterol, and total bilirubin in the two groups after operation were lower than those before operation, and the differences were statistically significant (P < 0.05). The postoperative hospital stay, intestinal recovery time, and drainage tube indwelling time in the modified group were shorter than those in the traditional group, and the differences were statistically significant (P < 0.05). The total incidence of postoperative complications and the incidence of afferent loop obstruction in the modified group were lower than those in the traditional group, and the differences were statistically significant (P < 0.05). Conclusion The application of Braun anastomosis in pancreaticoduodenectomy can reduce the incidence of postoperative complications, accelerate the recovery of gastrointestinal function and shorten the length of hospital stay.

Wendan Decoction is a famous classic prescription, which has always been valued by doctors. This paper combs the drug composition, dosage form, formula meaning, and medical records of the prescription in the historical documents, and discusses the evolution law of its main diseases and syndromes. The results show that Wendan Decoction originated in the Northern and Southern Dynasties. At the beginning, it was used to treat insomnia caused by gallbladder coldness and phlegm-fluid retention. In the Song Dynasty, Chen Wuze added Poria and Jujubae Fructus in the original prescription, and proposed that its chief indications was “phlegm-qi stagnation and binding” and “heart-gallbladder qi deficiency”. Based on this, doctors in Jin Yuan verified this idea with practice. In the Ming and Qing Dynasties, it was considered that Wendan Decoction was composed of Erchen Decoction plus Aurantii Fructus Immaturus and Bambusae Caulis In Taenias, which could also be used to treat “phlegm-heat”. with the increase and decrease of specific decocting pieces, it was applied to mental diseases and digestive diseases. Experts on warm disease put forward the idea of “elimination of pathogens through purgation and diuresis”, which is widely used in damp-heat syndrome. In the process of the evolution of Wendan Decoction, the change of its drug composition, increase, decrease are the key to the change of its main diseases and syndromes.

Professor Deng Gaopi finds that the most common syndrome type of gynecological pain syndrome is blood stasis syndrome. Among the pathogenic factors of the disease, blood stasis is particularly important, or combined withdamp, heat and deficiency. Due to the influence of local region and climate, people in Lingnan area are prone to feel the evil of damp and heat. Therefore, blood stasis syndrome combined with damp and heat is more common in gynecological pain syndrome in Lingnan area, and the symptoms of abdominal pain during menstruation and leukorrhea heavy are common. The couplet medicines of Leonuri Fructus and Herba Artemisiae Lactiflorae are commonly used for etiology and syndrome differentiation. The dosage is 10 - 15 g. Among them, Leonuri Fructus can promote blood circulation for regulating menstruation, activate qi flowing for dissipating blood stasis. Herba Artemisiae Lactiflorae can promote blood circulation for promoting menstruation, regulating qi-flowing for eliminating damp. The combination of the two can make the dampness and the blood stasis dissipate, the evil goes away and the evil remains, and play the functions of activating qi flowing, promoting diuresis, promoting blood circulation for removing blood stasis and relieving pain. The use of the couplet medicines is clinically effective in the treatment of gynecological pain syndrome, which is consistent with the etiology and pathogenesis characteristics of gynecological pain syndrome in Lingnan. It is worthy of reference and promotion.

The theory of healthy qi and pathogenic factors shares similar understanding about the physiological and pathological function of immune system with modern medicine. The “healthy qi” is able to resist external pathogens and maintain normal functional activities, which can be reflected in the defense and monitoring function of the immune system. The “pathogenic factors” can be reflected in pathogen and immune imbalance. Each “pathogenic factor” causes different pathogenesis, which can be reflected in the change of different biomarkers. Dampness, wind, phlegm and blood stasis are the key to the pathogenesis of rheumatic immune diseases. Dampness evil is related to the increasing degree of inflammation, while wind pathogen has a close relationship with higher tumor necrosis factor-α, interleukin-6, interleukin-1β level. Phlegm and blood stasis suggest longer course and higher activity of disease. To deeply explore the relationship between healthy qi and pathogenic factors theory in traditional Chinese medicine and immune homeostasis can not only explain the connotation and extension of the core theory of traditional Chinese medicine in scientific language, but also enlighten the scientific research and clinical treatment of immune related diseases.

124 Discussion on  the biological connotation of “earth stagnation and wood constraint” in metabolic associated fatty liver disease based on “intestinal microbiota-bile acid interaction”

Traditional Chinese medicine believes that “earth stagnation and wood constraint” is the main pathogenesis of metabolic associated fatty liver disease. Combined with the pathogenesis theory of traditional Chinese medicine and modern medical research, it is found that the dysregulation of intestinal microbiota is very likely to be an important biological basis of “earth stagnation” (dysfunction of spleen in transportation), and the imbalance of bile acid metabolism may be the microscopic manifestation of “wood constraint” (dysfunction of liver in conveyance and dispersion). The occurrence and development of metabolic associated fatty liver disease caused by the imbalance of “intestinal microbiota-bile acid interaction” from the perspective of western medicine is consistent with the main pathogenesis of “earth stagnation and wood constraint” from the perspective of  traditional Chinese medicine. Based on the interaction between intestinal microbiota and bile acids, this paper preliminarily explores the biological connotation of the pathogenesis of “earth stagnation and wood constraint” in metabolic associated fatty liver disease, to provide new ideas for studying the pathogenesis of “earth stagnation and wood constraint” and promoting the prevention and treatment of metabolic associated fatty liver disease with traditional Chinese medicine。

This study summarizes the experience and characteristics of diagnosis and treatment of tendon injuries by famous orthopedic expert Liu Shoushan. Liu Shoushan points out that tendon injuries have the characteristic of difficult treatment. Clinically they should be diagnosed early, treated correctly and emphasized the importance of recovery. He establishes four principles which are massage manipulation, the composition of Chinese medicine prescriptions, fixation and functional exercises for the treatment of tendon injuries by focusing on the physiological characteristics of the tendon of traditional Chinese medicine. His six core academic thoughts: accurate, soft and slow are better than blind and vigorous on massage manipulations; correcting the wrong anatomical position of tendon is beneficial to restore its physiological function; pay attention to the whole treatment, use the composition of Chinese medicine prescriptions correctly during different stages to adjust the balance of qi and blood; treatments of tendon injuries should be warm instead of cold; new serious tendon injuries should be fixed with paper binding; pay attention to the exercise on the recovery of tendon injuries. Through combing and refining the above-mentioned academic thoughts, this article hopes to provide some reference for the diagnosis and treatment of tendon injuries in Chinese medicine.

Migraine is a common clinical neurological disease, which is easy to occur repeatedly and has a serious impact on the life of patients. Professor Wang Songling, a famous traditional Chinese medicine doctor in the country, has rich clinical experience and believes that the pathogenesis of the disease is liver depression and blood stasis, which is located in the liver, spleen and kidney. In syndrome differentiation, it is divided into five pattern of syndrome according to the patient’s physique and climatic factors in five seasons. In treatment, it is advocated to take soothing the liver and removing blood stasis as the principle, Toufeng No. 1 Decoction as the main prescription to be added or subtracted with the disease, and Tongqiao Zhitong Powder is used to relieve pain during the acute attack. In the chronic remission period, pills are formulated to consolidate the curative effect and prevent recurrence, with equal emphasis on prevention and treatment, clear clinical thinking and remarkable curative effect. At the end of the article, Professor Wang’s experience of treatment is shared with others in order to provide ideas for the treatment of migraine with traditional Chinese medicine.

136 Effect of Yiqi Huoluo Shengji Decoction on diabetic foot patients with qi deficiency and blood stasis type

Objective To observe the effect of Yiqi Huoluo Shengji Decoction on diabetic foot patients with qi deficiency and blood stasis type. Methods A total of 124 diabetic foot patients admitted to the First People’s Hospital of Lianyungang of Jiangsu Province from February 2020 to August 2021 were selected as the research subjects. They were randomly divided into control group and study group according to the random number table method, with 62 cases in each group. The control group was given vacuum sealing drainage treatment on the basis of conventional treatment, and study group was given Yiqi Huoluo Shengji Decoction on the basis of control group. Both groups were treated for eight weeks. The traditional Chinese medicine symptom scores and clinical efficacy of two groups were compared, the changes of wound healing, peripheral nerve sensory threshold, and dorsum of foot blood flow between two groups were recorded; the occurrence of adverse events during treatment in two groups was recorded. Results After treatment, the traditional Chinese medicine syndrome scores of the main symptom and secondary symptom in both groups were lower than those before treatment, and those in study group was lower than those in control group, and the differences were statistically significant, (P < 0.05). The curative effect of study group was better than that of control group, and the difference was statistically significant (P < 0.05). The appearance time of foot ulcer granulation tissue and wound healing time in study group were shorter than those in control group (P < 0.05). After treatment, the peripheral nerve sensory threshold in two groups was lower than that before treatment, that of study group was lower than that of control group, and the differences were statistically significant (P < 0.05). After treatment, inner diameter of blood vessels, blood flow velocity, and pulsatility index in both groups were higher than those before treatment, and those in study group were higher than those in control group, and the differences were statistically significant (P < 0.05). There was no significant difference in the total incidence of adverse events between two groups (P > 0.05). Conclusion Yiqi Huoluo Shengji Decoction can enhance the therapeutic effect, accelerate wound healing, and improve peripheral nerve sensory function and blood circulation in patients with diabetic foot due to qi deficiency and blood stasis, with good safety.

Objective To observe the effect of Xiegan Liangxue Jiedu Decoction in the treatment of psoriasis vulgaris with blood heat syndrome. Methods A total of 80 patients with psoriasis vulgaris treated in Longhua Hospital Affiliated to Shanghai University of Traditional Chinese Medicine from January 2020 to March 2021 were selected and divided into study group (40 cases, Xiegan Liangxue Jiedu Decoction combined with Caspotriol Ointment) and control group (40 cases, Compound Qingdai Capsules combined with Caspotriol Ointment) according to the random number table method. The treatment period of both groups was eight weeks. The efficacy of the two groups was compared. The psoriasis area and severity index (PASI), traditional Chinese medicine syndrome score, and serum interleukin (IL) -17, IL-22, and IL-23 were compared between the two groups before and after treatment. The incidence of adverse reactions during treatment was recorded in the two groups. Results The efficacy of the study group was better than that of the control group, and the difference was statistically significant (P < 0.05). After treatment, PASI, traditional Chinese medicine syndrome score, and serum levels of IL-17, IL-22, and IL-23 in the two groups were lower than those before treatment, and the study group was lower than the control group, and the differences were statistically significant (P < 0.05). There were no obvious adverse reactions in the two groups during treatment. Conclusion Xiegan Liangxue Jiedu Decoction has significant effect in the treatment of psoriasis vulgaris with blood heat syndrome, which can effectively improve the symptoms and reduce the levels of IL-17, IL-22, and IL-23 in patients.

144 Early screening value of optical coherence tomography angiography for retinal microcirculation disorders in patients with diabetic retinopathy

Objective To explore the early screening value of optical coherence tomography angiography(OCTA) for retinal microcirculation disorders in patients with diabetic retinopathy (DR). Methods A total of 125 DR patients admitted to Wuxi No.5 People’s Hospital from May 2018 to March 2021 were selected as the research objects. According to whether retinal microcirculation disorder occurred, they were divided into occurrence group (67 cases) and non-occurrence group (58 cases). All patients received fluorescein fundus angiography (FFA) and OCTA screening within three days of admission. The deep retinal macular vascular density (MVD), chorioretinal MVD, superficial retinal MVD, and the area of the foveal avascular zone (FAZ) were compared between two groups; the results of FFA screening were used as the gold standard to analyze the screening value of OCTA for retinal microcirculation disorders in DR patients. Results The deep retinal MVD, superficial retinal MVD, and chorioretinal MVD in occurrence group were lower than those in non-occurrence group, and the area of FAZ was higher than that in non-occurrence group (P < 0.05). The sensitivity and specificity of OCTA for screening retinal microcirculation disorders in DR patients were 95.52% and 91.38%, Kappa=0.873, indicating good consistency. Conclusion OCTA has a good value in early screening of retinal microcirculation disorders in DR patients and can be further promoted.

Objective To research the the value of high frequency ultrasound, real-time shear wave elastography (SWE), and color Doppler flow imaging (CDFI) in the diagnosis of benign and malignant BI-RADS 4 breast masses. Methods A total of 80 patients with breast masses treated in Anqing First People’s Hospital of Anhui Province from September 2020 to September 2021 were selected as the research objects. They were divided into benign group (53 cases) and malignant group (27 cases) according to surgical pathology result or biopsy result. All patients completed high-frequency ultrasound, SWE, and CDFI before operation or one week before pathological biopsy. SWE parameters and CDFI blood flow grade of different breast masses were compared. Receiver operating characteristic curve was drawn to analyze the diagnostic value of high-frequency ultrasound, SWE, and CDFI alone and in combination in the diagnosis of malignant breast masses. Results The maximum value of elastic modulus, the mean value of elastic modulus, the ratio of breast mass to the mean value of low fat elasticity in the same layer, and the variance value of elastic modulus in malignant group were higher than those in benign group (P < 0.05). The difference of CDFI blood flow grade between two groups was statistically significant (P < 0.05). The area under the curve, sensitivity, and specificity of combined examination were higher than those of individual tests. Conclusion High frequency ultrasound, SWE, and CDFI can provide imaging data for the clinical diagnosis of breast masses from multiple angles, and the combination examination can accurately identify the pathological nature of BI-RADS 4 breast masses, which is worthy of active promotion in clinic.

151 Clinical efficacy and safety of Tofacitinib Tablets combined with Methotrexate Tablets in the treatment of psoriatic arthritis

Objective To investigate the clinical efficacy and safety of Tofacitinib Tablets combined with Methotrexate Tablets in the treatment of psoriatic arthritis (PsA). Methods A total of 62 patients with PsA admitted to the Department of Rheumatology or Ward, Yuyao People’s Hospital of Zhejiang Province from January 2018 to June 2019 were selected, they were divided into observation group (31 cases) and control group (31 cases) according to the random number table method. The observation group was treated with Tofacitinib Tablets combined with Methotrexate Tablets, the control group was treated with Methotrexate Tablets combined with Leflunomide Tablets, the total course of treatment was 24 weeks. The clinical effect and safety of two groups were observed at different time points. Results After 4 and 12 weeks of treatment, the observation group reached the American College of Rheumatology standard 20% improvement rate was higher than that of the control group (P < 0.05). After 8 weeks of treatment, the visual analogue scale (VAS) score of observation group was lower than that of control group (P < 0.05); after 12 weeks of treatment, tender joint count (TJC), swollen joints count, and VAS score in observation group were lower than those in control group (P < 0.05); after 16 weeks of treatment, TJC in observation group was lower than that of control group; after 16 and 24 weeks of treatment, the VAS score and health assessment questionnaire score of observation group were lower than those of control group (P < 0.05). The total incidence of adverse reactions in observation group was lower than that in the control group (P < 0.05). Conclusion Tofacitib Tablets combined with Methotrexate Tablets and Flunomide Tablets combined with Methotrexate Tablets have the same effect in the treatment of PsA, which can reduce adverse reactions and improve the quality of life of patients.

Objective To explore the application of hypervariable octameric oligonucleotide finger-prints (HOOF-Prints) genotyping on molecular epidemiologic study of Brucellosis in Shijiazhuang. Methods A total of 145 patients with confirmed Brucellosis admitted to the Fifth Hospital of Shijiazhuang from January 2016 to December 2018 were selected, Brucella strains were isolated and cultured. HOOF-Prints genotyping was used for molecular epidemiologic study. Results The most prevalent species of Brucella in Shijiazhuang were B.melitensis and B.abortus. Among them, B.melitensis biover 3 and B.melitensis biover 2 were the main epidemic strains. Conclusion The main epidemic strains of Brucella in Shijiazhuang are B.melitensis biover 3 and B.melitensis biover 2. HOOF-Prints genotyping is an effective method for the molecular epidemiologic study on Brucella.

Objective To explore the application of disease diagnosis-related groups (DRGs) in the performance evaluation of medical services in tertiary general hospitals, and to provide evidence-based evidence for hospital management decision-making. Methods Relying on the inpatient medical service evaluation system of Qinghai Provincial People’s Hospital, the total weight, case mix index (CMI), efficiency score, and other indicators of the hospital’s inpatients from January 2019 to December 2020 were collected. From the three dimensions of service capability, service efficiency, and service quality, medical service performance evaluation and comparative analysis were carried out. Results From 2019 to 2020, the total weight of the hospital decreased by 5 421.00, and the CMI value increased by 0.06; the time consumption index increased by 0.01, the cost consumption index decreased by 0.04, but the efficiency score increased by 0.02; the low-risk mortality rate decreased by 0.02. The total weight of most departments was decreased, the CMI value was increased, the service efficiency increased, and the low-risk mortality rate was decreased. Compared with the average level, the neurosurgery and surgical oncology departments had insufficient management capacity. Conclusion DRGs performance evaluation indicators are reasonable, fair, and objectively comparable, which can reflect the actual situation of hospitals and departments, and provide data support for the realization of refined hospital management and optimization of resource allocation.

Objective To explore the mediating role of self perceived burden between disease perception and postoperative phobia in patients undergoing cardiac intervention. Methods Convenience sampling was used to study 200 patients undergoing cardiac interventional surgery in the First Affiliated Hospital of Air Force Military Medical University from August 2020 to August 2021. Self-compiled general demographic survey questionnaire, simple disease perception questionnaire, self-perceived burden scale and tampa scale for kinesiophapia were selected to conduct a questionnaire survey. Results A total of 184 valid questionnaires were collected, and the effective recovery rate was 92.0%. In this study, patients undergoing cardiac interventional surgery had fear of movement disorder score of (38.34±5.79) points, a disease perception score of (37.14±13.15), and a self-perceived burden score of (30.29±7.06)points. Disease perception was positively correlated with self-perceived burden and fear of movement disorder (r = 0.842, P < 0.01), and self-perceived burden was significantly positively correlated with fear of movement disorder(r = 0.703, P < 0.01). Self-perceived burden was part of the mediating effect between disease perception and fear of movement disorder, and the mediating effect accounts for 28.57%. Conclusion Self-perceived burden is an intermediary variable between disease perception and fear of movement disorder. In hospital care and family care, it can establish positive awareness of the disease for patients, and promptly and correctly guide negative emotions such as guilt and fear, thereby reducing fear of exercise, and improving the quality of life of patients.

Objective To explore the effect of motivation interview combined with mental resilience intervention on self-perceived burden in patients with pulmonary tuberculosis. Methods A total of 84 pulmonary tuberculosis patients treated in the Fifth People’s Hospital of Shaanxi Province from December 2020 to December 2021 were selected as the study subjects, and they were divided into control group and observation group by computer stratified sampling method, with 42 cases in each group. The control group implemented the routine intervention, and observation group conducted the motivation interview combined with mental resilience intervention based on the control group. Self-perceived burden scale and self-management behavior scale, and Newcastle care satisfaction scale were used to evaluate two groups. Results After eight weeks of intervention, the scores of physical burden and economic burden in control group were lower than those before intervention, and the scores of self-perceived burden in observation group were lower than those before intervention; and the self-perceived burden scores of observation group were lower than that of control group, and the differences were statistically significant (P < 0.05). After eight weeks of intervention, the treatment score of control group was higher than that before intervention, and the self-management ability scores of observation group were higher than those before intervention; and the self-management ability score of observation group were higher than that of control group, and the differences were statistically significant (P < 0.05). After eight weeks of intervention, Newcastle satisfaction with nursing scale scores of two groups were higher than those before intervention, and observation group was higher than control group, and the differences were statistically significant (P < 0.05). Conclusion The motivation interview combined with mental resilience intervention can improve self-management ability and nursing satisfaction of patients with pulmonary tuberculosis, and reduce patients’ self-burden.

Objective To explore the Effect of empowerment psychological guidance combined with nutritional intervention on rehabilitation of patients with primary liver cancer after transcatheter arterial chemoembolization(TACE). Methods A total of 168 patients with primary liver cancer who underwent TACE in the Department of Pain Liver Ward Fifth of Jiangsu Provincial People’s Hospital from March 2019 to March 2020 were selected. They were divided into observation group and control group by random number table method, with 84 cases in each group. The control group was received routine nursing, while the research group was received empowering psychological guidance combined with nutritional intervention nursing based on the control group. The coping ability, negative emotion, and nutritional status of the two groups were compared before and after nursing. Results After nursing, the coping score of the medical coping questionnaire in the two groups increased, and the observation group was higher than the control group, the yield and avoidance were decreased, and the observation group was lower than the control group, and the differences were statistically significant (P < 0.05). The scores of Hamilton depression scale and Hamilton anxiety scale in the two groups were decreased, and the observation group was lower than the control group, and the differences were statistically significant (P < 0.05). The hemoglobin, upper arm circumference and triceps skin fold thickness of the two groups were increased, and the observation group was higher than the control group, and the differences were statistically significant (P < 0.05). Conclusion The empowering psychological guidance combined with nutritional intervention nursing was applied to patients with liver cancer after TACE to improve their coping ability, reduce negative emotions and improve the level of nutritional indicators.

Objective To explore the value of tolerance assessment to guide nutritional management in patients with rectostomy. Methods A random sampling method was used to select 200 patients with rectostomy in the First Affiliated Hospital of Xinjiang Medical University from December 2019 to December 2020. They were divided into experimental group (100 cases) and control group (100 cases) by random number table method. The experimental group was given tolerance assessment to guide nutritional management, while the control group was given routine nutritional care intervention. Before the intervention and after six weeks of intervention, the improvement of nutritional status, the discoloration, erosion, tissue overgronth (DET) score around the stoma and the gastrointestinal function status were compared between the two groups. Results After six weeks of intervention, the body weight, body mass index, body fat percentage, body fat, and grip strength in the two groups were higher than those before the intervention, while those of the observation group were higher than those of the control group, and the differences were statistically significant (P < 0.05). The improvement of the skin around the stoma in the experimental group was better than that in the control group, and the difference was statistically significant (P < 0.05). The time of the first stoma exhaust, the first stoma defecation time, the recovery time of bowel sounds, and the time of the first eating in the experimental group were shorter than those in the control group, and the differences were statistically significant (P < 0.05). Conclusion Tolerance assessment is used to guide nutritional management in patients with rectostomy, improve the nutritional status of the body, maintain the integrity of the skin around the stoma, and promote the recovery of gastrointestinal function.

Objective To explore the effect of evidence-based nursing strategy on postoperative pain and quality of life in patients with cholecystolithiasis undergoing laparoscopic treatment. Methods A total of 86 patients with gallbladder stones who underwent laparoscopic treatment in Nanjing First Hospital from June 2020 to June 2021 were selected, and divided into the conventional group and the observation group according to the random number table method. A total of 43 patients in the conventional group received traditional nursing, while 43 patients in the observation group received evidence-based nursing strategy on the basis of the conventional group, all patients were nursed for three months. The visual analogue scale (VAS) score, pain duration, hospital for special surgery knee joint WHO quality of life score (WHOQOL-100), self-rating anxiety scale (SAS) score, self-rating depression scale (SDS) score, and satisfaction were compared between the two groups. Results After nursing, the VAS score of the observation group was lower than that before nursing, and the observation group was lower than that of the conventional group (P < 0.05). The duration of pain in the observation group was lower than that in the conventional group (P < 0.05). After nursing, the WHOQOL-100 score of the two groups was higher than that before nursing, and the observation group was higher than that of the conventional group (P < 0.05). After nursing, the SAS and SDS scores of the two groups were lower than those before nursing, and the observation group was lower than that of the conventional group (P < 0.05). The total satisfaction of nursing in the observation group was higher than that in the conventional group (P < 0.05). Conclusion Evidence-based nursing strategy applied in patients with gallstones undergoing laparoscopic treatment is conducive to relieve pain, anxiety and depression, shorten the duration of pain, and improve the quality of life nursing satisfaction.

182 Effects of information-knowledge-attitude-practice health education model on skin itching and quality of life in patients with blood purification

Objective To explore the influence of information-knowledge-attitude-practice (IKAP) health education model on the degree of skin itching and quality of life in patients with blood purification. Methods A total of 88 maintenance hemodialysis patients admitted to the Blood Purification Center of Fuyang Second People’s Hospital from February 2019 to March 2021 were divided into two groups by random number table method. The routine group (44 cases) were given routine health education, while the intervention group was given IKAP-based health education. After eight weeks of intervention, the patients’ self-care ability, skin itching degree, and quality of life were observed. Results After the intervention, the scores of exercise of self-care agency scale in the two groups were higher than those before the intervention, and those in the intervention group were higher than those in the routine group, and the differences were statistically significant (P < 0.05). After the intervention, the pruritus degree scores in both groups were lower than those before the intervention, while those in the intervention group were lower than those in the routine group, and the differences were statistically significant (P < 0.05). After the intervention, the scores of generic quality lifeInventory-74 scale in the two groups were higher than those before the intervention, while those in the intervention group were higher than those in the routine group, and the differences were statistically significant (P < 0.05). Conclusion Health education based on IKAP in the nursing of blood purification patients, can effectively improve the self-care ability, alleviate the degree of skin pruritus, and help to improve the quality of life of patients.

186 Application of problem-oriented health education and nursing based on the theory of health ecology in elderly patients with stable coronary heart disease

Objective To observe and study the intervention and application effect of problem-oriented health education and nursing based on the theory of health ecology in elderly patients with stable coronary heart disease. Methods A prospective study was conducted on 80 elderly patients with stable coronary heart disease who were admitted to the Department of Cardiovascular Medicine, Fuyang Women’s and Children’s Hospital, Anhui Province from July 2019 to August 2021. The patients were divided into the observation group and the control group by random number table method, with 40 cases in each group. The control group was received routine health education and nursing, while the observation group was received problem-oriented health education and nursing intervention based on the theory of health ecology. The scores of the quality of life assessment scale (SF-36) before and after the intervention were compared, and the incidence of adverse cardiovascular events during the treatment period and the mastery of the health knowledge of coronary heart disease before and after the intervention were investigated in the two groups. Results After the intervention, the physiological function, physiological function, social function, emotional function, physical pain, mental status, energy, and overall health scores of the two groups were higher than those before the intervention, while those of the observation group were higher than those of the control group, and the differences were statistically significant (P < 0.05). After the intervention, the scores of coronary heart disease health knowledge in the two groups were higher than those before the intervention, and the observation group was higher than the control group, and the differences were statistically significant (P < 0.05). The incidence of cardiovascular adverse events in the observation group was lower than that in the control group, and the difference was statistically significant (P < 0.05). Conclusion Problem-oriented health education and nursing based on the theory of health ecology has a positive effect on the rehabilitation of elderly patients with stable coronary heart disease. It can significantly improve the quality of life of patients and reduce the occurrence of related cardiovascular adverse events, and has good application value.

190 Effect of predictive nursing  based on Omaha system on nutritional status and complication control in patients with severe craniocerebral injury by nasal feeding

Objective To investigate the effect of the predictive nursing model based on the Omaha system on nutritional status and complication control in patients with severe hcad injury (SHI) by nasal feeding. Methods A total of 172 patients with SHI who were diagnosed and treated in Huzhou First People’s Hospital City, Zhejiang Province from March 2019 to March 2021 were selected and grouped by random number table method. The control group (86 cases) received routine nursing, while the observation group (86 cases) received predictive nursing under the Omaha system. After the intervention, the nutritional status of the two groups improved, the complications of nasal feeding and the prognosis were observed. Results After the intervention, the levels of total protein, prealbumin, and mini-nutritional assessment in the two groups were higher than those before the intervention, while those of the observation group were higher than those of the control group, with statistical significance (P < 0.05). The incidence of vomiting, diarrhea, aspiration pneumonia, tube blockage, gastric retention, respiratory tract infection, and peptic ulcer in the observation group were lower than those in the control group, and the differences were statistically significant (P < 0.05). One month after the intervention, the the Barthel index and Fug-Moyer scores of the two groups were lower than those before the intervention, while those of the observation group were lower than those of the control group, and the differences were statistically significant (P < 0.05). Conclusion Predictive nursing under the Omaha system is used for SHI patients with nasal feeding, which can significantly improve the effect of nutritional support, reduce complications of nasal feeding, and help improve the prognosis of patients.

194 Clinical observation of outcome-oriented intensive nursing intervention in patients with lumbar spinal stenosis

Objective To explore the clinical effect of outcome-oriented intensive nursing intervention in patients with lumbar spinal stenosis, and to provide theoretical reference for clinical practice. Methods A total of 108 patients with lumbar spinal stenosis who were admitted to the Second Affiliated Hospital of Air Force Military Medical University from January 2020 to June 2021 were selected and divided into control group and observation group, with 54 cases in each group. The control group was received routine nursing, and the observation group was received outcome-oriented intensive nursing intervention on this basis. The Japanese Orthopaedic Association (JOA) score, visual analog scale (VAS) score, quality of life comprehensive assessment questionnaire (GQOL-74) score, and satisfaction were compared between the two groups. Results After the intervention, the JOA scores of the two groups were higher than those before the intervention, while those of the observation group were higher than those of the control group; the VAS score was lower than that before the intervention, while the observation group was lower than the control group, and the differences were statistically significant(P < 0.05). The scores of material life, physical function, psychological function, and social function in the two groups were higher than those before the intervention, while those of the observation group were higher than those of the control group, and the differences were statistically significant (P < 0.05). The nursing satisfaction of the observation group was better than that of the control group, and the difference was statistically significant (P < 0.05). Conclusion The application of outcome-oriented intensive care for lumbar spinal stenosis has a positive effect on the improvement of lumbar spine function, pain and quality of life.
