MA Jinhui1 LIU Jie1 LI Yuanyuan2 SONG Yankun3 ZHENG Lishuang4 TAN Yinghui4 XI Xin4
Objective To explore the effects of insulin on glutathione peroxidase (GSH-Px) expression and its significance. Methods The differentiation of 3T3-L1 preadipocytes were induced by cocktail method and the degree of differentiation was identified by oil red O staining; after 3T3-L1 adipocytes were treated with 500 nmol/L insulin for 1 h, the mRNA expression levels of GSH-PX1, GSH-PX4, and GSH-PX7 were detected by real-time quantitative PCR; 3T3-L1 adipocytes were pretreated with GSH-Px inhibitor Mercaptosuccinate (MS, 10, 20 mmol/L) for 30 min and then stimulated with or without 500 nmol/L insulin for 4 h, the expression level of glucose transporter 4 (GLUT4) protein was detected by Western blot. Results 3T3-L1 preadipocytes were spindle-shaped; and the cells gradually became spheroid and small lipid droplets were observed in the cells on the 5th day after induced differentiation; the cells further became spherical and the number of intracellular lipid droplets increased significantly on the 8th day after induced differentiation. The mRNA relative expression levels of GSH-Px1,GSH-Px4, and GSH-Px7 in insulin group were lower than those in control group, and the differences were statistically significant (P < 0.05); with or without insulin, there was no significant in expression of GLUT4 protein between the 10 mmol/L MS treatment group and the MS untreated group (P > 0.05); the level of GLUT4 protein in 20 mmol/L MS treated group was lower than that in MS untreated group, and the difference was statistically significant (P < 0.05). Conclusion Long-term insulin stimulation inhibited GSH-Px mRNA expression, and MS inhibition of GSH-Px activity negatively regulated GLUT4 protein expression.

GONG Yurong1* LIN Qiaoxuan2* LIU Jing3 GUO Zexing4 LU Liming4 XIU Zhongbiao3
Objective To explore the effect of akupotomye on stiffness and fibrosis of rectus femoris muscle in rabbits with knee osteoarthritis (KOA). Methods Twenty-four healthy male New Zealand rabbits were divided into blank group, model group, and akupotomye group by random number table method, with eight rabbits in each group. The KOA rabbit model was made by plaster fixation of left hind limb in straight position for six weeks by modified Videman method. The model success criteria was evaluated by the Lequesne MG score of knee joint behavior ≥4 points. Akupotomye group received akupotomye release treatment for “top of the cranium”, “upper of the external iliac”, and “lower of the internal iliac”, intervention once every seven days, a total of four interventions. The blank group and model group underwent the same grasping and fixation, but no akupotomye treatment. One week after the intervention, real-time tissue elastography was performed to measure the elastic strain rate ratio to evaluate the changes in tissue hardness of rectus femoris muscle. Masson staining was used to observe the changes of tissue morphology and collagen fiber content of rectus femoris muscle. Results Compared with the blank group, the elastic strain rate ratio and collagen volume fraction of rectus femoris muscle in the model group were all increased (P < 0.05). Compared with the model group, the ratio of elastic strain rate and collagen volume fraction of rectus femoris in the akupotomye group decreased significantly (P < 0.05). Conclusion Akupotomye intervention can improve the hardness of rectus femoris tissue in KOA rabbits and promote the restore of chronic injury fibrosis of rectus femoris. It can be one of the mechanisms of acupotomy in treating KOA.

129 Discussion on the scientificity of traditional Chinese medicine from cognitive mode
YAN Huiqian1 GE Haitao2 SUN Lingzhi3▲

[Abstract] At each stage of science development, there are overlapping of different cognitive methods. The vagueness of terminology leads to the limitation of cognition. The separate research fields have their own emphasis and lead to the tilt of evaluation. Human beings often respond to the scientific problems of traditional Chinese medicine based on the value, but ignored the medical culture. There is a philosophical dialectic between “comparative state” and double-blind testing, neither of which is invulnerable. The defenders themselves are also factors that create prejudice in traditional Chinese medicine. The education system of traditional Chinese medicine that has never kept pace with the times has delayed its scientific modernization. Traditional Chinese medicine was once a science because it conformed to the cognition and interpretation of science at that time, and it stayes in the pre-scientific era because of the contemporary definition of science.

133 Application of “treating upper jiao is like a feather” in the treatment of otorhinolaryngology disease
SUN Hong LIU Jianhua YAN Zhanfeng JIAO Lulu ZHU Yajing LIU Lili HUANG Xiaorui ZHANG Linjing
Wu Jutong, a warm disease epidemiologist, proposes that “treating upper jiao is like a feather”, that is for the warm disease in the disease of upper jiao disease with “light” drugs should be applied. Ears, nose, and throat on the head and neck of the human body, are clear orifices, in the upper jiao, if the medicine is not light, they can not reach the upper jiao. Clinical treatment of the disease of this part often selects the light texture of the product. At the same time, with the flavoring and channel ushering drugs to help medicine to reach the disease site, belongs to the category of “treating upper jiao is like a feather”. This article is based on “treating upper jiao is like a feather” of the common diseases in otorhinolaryngology, in particular, the specific application in the initial treatment of diseases is discussed, in order to guide clinical treatment better.

171 Diagnosis of two cases of critical chlamydia psittaci pneumonia by metagenomic next-generation sequencing
WANG Jiali1 LIU Jian2 CAO Huidan1
Chlamydia psittaci pneumonia is a long-neglected pulmonary infection caused by chlamydia psittaci infection. This paper reports two cases of critical chlamydia psittaci pneumonia diagnosed by metagenomic next-generation sequencing in Department of Respiratory and Critical Care Medicine, the Second Affiliated Hospital of Tianjin University of Traditional Chinese Medicine. The clinical manifestations of this disease are lack of specificity, and the misdiagnosis rate is high, in serious cases, it may cause systemic multiple organ failure and other critical manifestations. Metagenomic next-generation sequencing technology is highly sensitive and can detect pathogens that cannot be detected by other traditional methods, which brings significant changes to the accurate diagnosis and treatment of respiratory tract infection, effectively shortening the course of disease and improving the prognosis of patients to the greatest extent.

CHEN Chongde GAO Xing WANG Yan HU Hongpu▲
Infectious diseases have been one of the most important threats to mankind for a long time, its prevention and control is of great importance. However, the early warning ability of the direct reporting system of infectious diseases in China is not comprehensive at present, and it still needs to be upgraded and improved. In this study, based on the literature review, taking the symptom assignment matrix of infectious diseases established according to the principle of symptom monitoring as the core, a symptom monitoring module model of infectious disease surveillance system is constructed, which is composed of four parts of information input and human-computer interaction, symptom recognition and semantic standardization, disease judgment matrix based on patients’ symptoms, and output and subsequent decision-making and disposal, including the functions of symptom semantic standardization, disease identification according to symptoms, epidemiological data collection, automatic information reporting and comprehensive intelligent judgment. The model can simulate the expert clinical diagnosis and treatment judgment, and perform auxiliary differential diagnosis for similar diseases according to the symptoms of patients, so as to strengthen the probability of infectious disease identification by grass-roots health personnel, and further reduce the layout depth of infectious disease identification to the bottom of the national health coverage network, so as to improve the efficiency of infectious disease monitoring and response system.
