Clinical research progress of heart failure with mid-range ejection fraction#br#
SAI Yaoqing LU Shengyu CAO Bangming
Heart failure (HF) is the terminal stage of heart disease, with high incidence and poor prognosis. In 2016, European Society of Cardiology defined HF with the left ventricular ejection fraction (LVEF) of 40%-49% as the heart failure with mid-range ejection fraction (HFmrEF). This article summarizes the clinical research on HFmrEF finding that ischemic heart disease is one of the most important factors of HFmrEF, combines with atrial fibrillation and hypertension and other diseases, it emphasizes that personalized treatment strategies based on etiology and complications could bring more benefits to patients suffering from HFmrEF. In terms of treatment, traditional drugs, including renin-angiotensin-aldosterone system antagonists, β-receptor blockers, and aldosterone receptor antagonists still have positive effects on HFmrEF; the emerging drug angiotensinreceptor neprilysin enzyme inhibitor has become a recommendation treatment for HFmrEF; some patients with HFmrEF can benefit from Vericiguat and ferric carboxymaltose; sodium-glucose cotransporter 2 inhibitors, and Omecamtiv mecarbil have potential for the treatment of HFmrEF, and they can be used as important research directions in the future. In addition, the changes of LVEF have suggestive effects on the prognosis of HFmrEF.

Objective To investigate the changes and significance of five items of coagulation in third trimester of pregnancy and puerperium under different delivery modes in order to the prevention and treatment of obstetric venous thromboembolism. Methods The clinical data of 1216 healthy single parturient women who delivered at term in Yangzhou Maternal and Child Health Hospital from January to May 2019 were retrospectively analyzed. According to the delivery mode, they were divided into delivery group (661 cases), cesarean section group (515 cases) and forceps group (40 cases). Five items of coagulation levels were compared during third trimester of pregnancy (within seven days before delivery) and puerperium (48 h after delivery). Results In the third trimester of pregnancy, there were no significant differences in five items of coagulation among three groups (P > 0.05). During puerperium, thrombin time (TT) in delivery group was shorter than that in the third trimester of pregnancy, fibrinogen (FIB) level was higher than that in the third trimester of pregnancy, D-dimer (DDI) level was lower than that in the third trimester of pregnancy, and the differences were statistically significant (P < 0.05); prothrombin time (PT) and activated partial prothrombin time (APTT) in cesarean section group were longer than those in the third trimester of pregnancy, TT was shorter than that in the third trimester of pregnancy, FIB level was higher than that in the third trimester of pregnancy, DDI level was lower than that in the third trimester of pregnancy, and the differences were statistically significant (P < 0.05); PT in the forceps group was longer than that in the third trimester of pregnancy, and FIB level was higher than that in the third trimester of pregnancy, and the differences were statistically significant (P < 0.05). PT and APTT in cesarean section group were longer than those in delivery group, FIB level was higher than that in delivery group, TT was shorter than that in delivery group; PT and APTT in forceps group were shorter than those in cesarean section group, while FIB level was lower than that in cesarean section group, and the differences were statistically significant (P < 0.05). Conclusion According to the five standards of coagulation for normal people, PT, APTT, and TT of the five items of coagulation in third trimester of pregnancy and puerperium pregnant women are all within the normal range, but FIB and DDI level are higher than the normal range, which is in line with the conclusion of blood hypercoagulability. Routine postpartum screening of DDI levels is not recommended as a basis for predicting venous thromboembolism disease in the puerperium. PT and APTT in puerperium of cesarean delivery pregnancy are longer than those in the third trimester of pregnancy, and the phenomenon of hypocoagulability appears.

Discussion on traditional Chinese medicine treatment of malignant ascites from the theory of “once the pectoral qi runs, the water qi is dispersed”
SUO Feiya1 CHEN Chen1 YANG Zhenhuan1 ZHU Xiaoran1 YAO Shukun2
“Once the pectoral qi runs, the water qi is dispersed” is the classical theory of treatment of water pathogen disease in Jingui Yaolue. The pectoral qi governs qi and blood in the body, the physical characteristics of malignant tumour patients, such as deficiency in root and excess in superficiality, qi stagnation and phlegm stasis, and internal binding of toxin factor, lead to the abnormal operation of the pectoral qi in the body, which affects the functions of internal organs and meridians, leading to the production of malignant ascites. Under the guidance of the theory of “Once the pectoral qi runs, the water qi is dispersed”, the malignant ascites was classified by traditional Chinese medicine syndrome differentiation, including the types of qi deficiency and water stagnation, qi stagnation and water stagnation, water-heat interconnection, yang deficiency and water stagnation, and stasis and water interconnection, and discusses the traditional Chinese medicine treatment of malignant ascites, including invigorating qi and disinhibiting water, activating qi and disinhibiting water, clearing heat and and disinhibiting water, warming yang and disinhibiting water, and activating blood and disinhibiting water, pectoral qi can function normally, so that malignant ascites could be dispersed.

Zhang Limei’s clinical experience in treating palpitation with traditional Chinese medicine#br#
LIU Meihan LI Xiaoning WANG Yifan ZHANG Limei
Arrhythmia is a common clinical cardiovascular disease or complication. It belongs to the category of “palpitations” and “stunned” in traditional China medicine. Clinical treatment is often very tricky, and traditional Chinese medicine is effective in treating palpitations. Director Zhang Limei is a well-known cardiovascular expert in Guang’anmen Hospital, China Academy of Chinese Medical Sciences, and has unique clinical experience in the treatment of arrhythmia. This article summarizes Director Zhang Limei’s experience in the treatment based on syndrome differentiation. Professor Zhang Limei believes that the pathogenesis of palpitations is deficient root and excessive superficial, and the fundamental pathogenesis is deficiency of qi and yin. Among them, the deficient root is deficiency of qi, blood, yin, and yang, and the excessive superficial is excess of phlegm dampness, phlegm stasis, and phlegm fire. Traditional Chinese medicine is based on the principles of replenishing qi and nourishing yin, and warming blood vessels. Clinical emphasis on holism, as well as the use of the methods of tranquilizing spirit to calm mind, soothing liver and regulating qi, invigorating spleen and resolving phlegm, promoting qi and activating blood, and extinguishing wind and restore pulsation, have significant effects, and worthy of promotion.

Discussion on the management system of traditional Indian medicine#br#
LI Fuwu CUI Yu WU Feng
The management system of traditional India medicine adopts a parallel model system, which is similar to the dual medical system in South Korea, but very different from the management system of traditional Chinese medicine in China. The development experience of traditional Chinese medicine proves that the parallel management mode still has its merits today. This paper summarizes the development status of traditional India medicine from the aspects of medical infrastructure, medical practitioners, education and medical services. This study found that the system protected the independence and diversity of traditional Indian medicine. To a large extent, it promoted the development of traditional Indian medicine. Although traditional Indian medicine is currently facing problems of low education quality, low quality of practitioners and low utilization rate, there are still experiences and lessons worth learning in the management of traditional Chinese medicine.
