DOI: 10.20047/j.issn1673-7210.2023.02.02
Objective To explore the clinical acupoint characteristics, acupoint selection rule, and prescription characteristics of Professor Wang Leting in treating apoplexy. Methods From November 2018 to February 2020, Professor Wang Leting’s medical record study was carried out. A total of 54 acupuncture and moxibustion prescriptions were collected and recorded in the medical records of Beijing Hospital of Traditional Chinese Medicine from January 1961 to November 1963. The frequency analysis, association rule analysis, and cluster analysis were carried out by the reusable data mining tools developed by our research office. Results Professor Wang Leting used 45 acupoints in the treatment of stroke, with a total frequency of 490 times. The most used acupoints were Hégǔ (48 times), Bǎihuì (44 times), Yánglíngquán (41 times), Qūchí (40 times), and Zúsānlǐ (34 times). The most frequently used meridians are Yangming large Intestine Channel of hand, Shaoyang Gallbladder channel of foot and governor channel. The number of specific acupoints was 32, accounting for 71.1% of the total number of meridian points, and the total frequency of use was 578 times. The number and frequency of acupoints used at the intersection point was the highest (177 times), followed by the five-shu point (152 times), the yuan-primary point (57 times), the eight influential point (52 times), and the Luo -Connecting point (50 times). The results of association rules showed that in the treatment of stroke, the highest degree of acupoint correlation was Sǎnyīnjiǎo→Zúsǎnlǐ, Baǐhuì→Fēngfǔ. The clustering results showed that there were three kinds of acupoint combinations. Conclusion Based on real medical records, this study analyzed the acupoint characteristics and prescription characteristics of Professor Wang Leting in the treatment of stroke through data mining technology, and found that the rule of acupoint selection was in line with the prescription of “twelve needles on hands and feet”, paid attention to the adjustment of the overall function of patients, and at the same time with the “opening the body and awakening the spirit”, to improve the symptoms of dizziness. Professor Wang Leting, as the founding figure of modern acupuncture prescriptions, can express and inherit his academic thoughts through acupuncture prescriptions.

DOI: 10.20047/j.issn1673-7210.2023.02.08
Sparganii Rhizoma-Curcumae Rhizoma is a Chinese medicine pair commonly used in clinical medicine to promote qi and blood circulation and remove blood stasis, which is widely used in tumor treatment. Modern physicians often achieve the therapeutic goal of anti-cancer and tumor suppressive by Chinese medicine pair of Sparganii Rhizoma-Curcumae Rhizoma with softening hardness and dissipating mass, strengthening body resistance, and invigorating spleen. Modern pharmacological studies have proved that Chinese medicine pair of Sparganii Rhizoma-Curcumae Rhizoma can not only inhibit the proliferation and growth of tumor cells, interfere with the cell cycle, but also play an anti-tumor role by regulating the body’s immunity and anti-angiogenesis. However, at present, there are few studies on its combination with immunotherapy and targeted therapy, as well as on the generation of new substances and pharmacological effects in their compatibility. It is expected that the summary of this paper can provide ideas for further research of this Chinese medicine pair.

ZHAO Jinghong1 QIAO Yan2 ZHANG Rongyi1 DENG Jianping1 CHEN Yong1
DOI: 10.20047/j.issn1673-7210.2023.02.12
Objective To investigate the relationship between serum C-X-C motif chemokine ligand 16 (CXCL16) and soluble suppression of tumorigenicity 2 (sST2) levels and the prognosis of patients with severe heart failure (SHF). Methods A total of 168 patients with SHF admitted to Nanchong Central Hospital from January 2020 to February 2021 were selected and they were divided into 41 cases in the death group and 127 cases in the survival group according to their prognosis. General information of patients was collected and serum CXCL16 and sST2 levels were measured by enzyme-linked immunosorbent assay. Multi-factor logistic regression was used to analyze the factors influencing death in SHF patients, and subject work characteristic curves were used to analyze the predictive value of serum CXCL16 and sST2 levels on death in SHF patients. Results The age of death group was longer than that of survival group, and the course of disease was longer than that of survival group. The New York Heart Association grade Ⅳ ratio, blood uric acid, B-type natriuretic peptide, CXCL16, and sST2 levels were higher than those of survival group, and systolic blood pressure and left ventricular ejection fraction were lower than those of survival group, with statistical significances (P<0.05). Multivariate logistic regression analysis showed that disease duration, New York Heart Association grade, B-type natriuretic peptide, CXCL16, and sST2 were independent risk factors for death in patients with SHF, while systolic blood pressure and left ventricular ejection fraction were independent protective factors (P<0.05). The area under the curve of the combined prediction of serum CXCL16 and sST2 was greater than that of serum CXCL16 and sST2 alone (P<0.05). Conclusion Elevated serum CXCL16 and sST2 levels are independent risk factors for death within one year in SHF patients. Combined testing of serum CXCL16 and sST2 levels can enhance the predictive value of death in SHF patients.

DOI: 10.20047/j.issn1673-7210.2023.02.20
Objective To explore the diagnostic value of grip strength in nutritional risk assessment in patients with nasopharyngeal carcinoma undergoing concurrent radiotherapy and chemoradiotherapy. Method A total of 158 patients with nasopharyngeal carcinoma who were hospitalized in the Department of Oncology, the First Affiliated Hospital of Guangxi Medical University from September 2019 to December 2020 were selected. General data, disease-related data, blood indicators, grip strength, and nutritional risk scores were collected. Patients were grouped by nutritional risk screening 2022 (NRS2002), patients with NRS2002 scores ≥3 points were classified into the nutritional risk group (65 cases), and those with NRS2002 < 3 points were classified into the non-nutritional risk group (93 cases). Nutritional risk factors in patients with simultaneous chemoradiotherapy for nasopharyngeal carcinoma were analyzed by logistic regression. The grip strength of patients with nasopharyngeal carcinoma was divided into five groups. From small to large, they were divided into the lowest level group 1 (≤22.60 kg), the low level group 2 (>22.60 to 28.70 kg), the medium level group 3 (>28.70 to 33.70 kg), the high level group 4 (>33.70 to 38.90 kg) and the highest level group 5 (>38.90 kg). Trend test was used to analyze the relationship between grip strength and nutritional risk, and receiver operating characteristic (ROC) curve was used to determine the critical value of nutritional risk in patients with simultaneous chemoradiotherapy of nasopharyngeal carcinoma. Results There were statistically significant differences in gender, arm circumference, calf circumference, height, body weight, body mass index, hemoglobin, albumin, prealbumin, creatinine, and grip strength between the two groups (P<0.05). The results of multivariate analysis showed that creatinine and grip strength were the factors affecting nutritional risk in nasopharyngeal carcinoma patients (P<0.05). From the high level of grip strength to the low level of grip strength group, the nutritional risk of patients showed an upward trend with the decrease of grip strength. The area under ROC curve of grip strength predicting nutritional risk in patients with concurrent radiotherapy and chemotherapy of nasopharyngeal cancer was >0.5. The presence of nutritional risk could be judged when the grip strength of male was <32.0 kg and that of female was <20.2 kg. Conclusion Grip strength can be used as a screening index for nutritional risk in patients with nasopharyngeal carcinoma undergoing concurrent chemoradiotherapy.

135 Professor Li Shuanglei’s experience on diabetic osteoporosis from kidney deficiency and blood stasis in collaterals
DONG Liping1 TANG Aihua2 LI Shuanglei1 ZHOU Yexin1 LAI Chuchu1 ZHANG Xiaoting1
DOI: 10.20047/j.issn1673-7210.2023.02.31
As a systemic bone metabolic disease, diabetic osteoporosis is an important complication of diabetes, and has always been a difficult clinical problem. Professor Li Shuanglei is a famous traditional Chinese medicine doctor in Guangxi Zhuang Autonomous Region. Through many years of clinical practice and observation, Professor Li Shuanglei has rich experience in the prevention and treatment of diabetic osteoporosis. When consumptive thirst is dissipated for a long time, deficiency and excess are mixed, the kidney is deficient and the bone collateral is lost of nutrition, and the kidney dominating bone marrow function is reduced, and bone is withered and marrow is reduced, and then reduced to become “bone flaccid”; when consumptive thirst is dissipated for a long time, long disease into the collaterals, blood stasis blocks the kidney collaterals, blood stasis damages bone, which can further accelerate the bone damage, the formation of a vicious cycle. Professor Li Shuanglei first proposed that the core pathogenesis of diabetic osteoporosis is kidney deficiency and blood stasis in collaterals, and bone collateral damage, and the treatment should take tonifying kidney and removing blood stasis as the main principle. Professor Li Shuanglei makes addition and subtraction application according to Zhuanggu Formula, and achieves good practical results in the clinical diagnosis and treatment of diabetic osteoporosis. Based on the pathogenic characteristics of kidney deficiency and blood stasis in collaterals, Professor Li Shuanglei analyzes the relationship between kidney deficiency and blood stasis in collaterals and the incidence of diabetic osteoporosis, hoping to provide theoretical basis for clinical treatment of diabetic osteoporosis from different perspectives.

DOI: 10.20047/j.issn1673-7210.2023.02.32
Objective To analyze medication rules of traditional Chinese medicine fumigation in the treatment of dry eye based on data mining. Methods A combination of computer and manual search was used to screen the literature on traditional Chinese medicine fumigation in the treatment of dry eyes from January 2004 to January 2022 in CNKI, VIP and Wanfang databases, and the prescriptions were entered into Excel tables and then imported into the Traditional Chinese Medicine Inheritance Support System (V2.5). Association rules, hierarchical clustering and other algorithms were adopted to conduct data mining from multiple dimensions such as drug frequency, four qi, five flavors of traditional Chinese medicine, meridian tropism, core drug combination and potential new formula combination. Results One hundred and twelve prescriptions contained 98 traditional Chinese medicines, most of which were cold, warm, and cold. Sweet, bitter, and pungent drugs were commonly used, and the channel tropism was mainly liver, lung, heart, stomach, and kidney channels. The top three Chinese medicines used were Chrysanthemi Flos, Menthae Haplocalycis Herba, Ophiopogonis Radix. Nineteen drug combinations were obtained. Phellodendri Chinensis Cortex and Menthae Haplocalycis Herba had the highest correlation. Nineteen groups of potential couplet medicines and six groups of core drug combinations were excavated, and three new drug combinations were obtained. Conclusion Traditional Chinese medicine fumigation in the treatment of dry eyes places equal emphasis on dispelling evil and supporting righteousness, mainly from the two perspectives of external wind heat and internal liver blood injury, on the basis of dispersing wind heat and nourishing liver yin, as well as promoting blood circulation, resolving stasis and drying dampness.

WANG Haiqiang1 XIONG Li2 MA Chenxi2 ZHANG Meng2 ZHANG Yang1 ZHENG Lihong3
DOI: 10.20047/j.issn1673-7210.2023.02.33
Gastric precancerous lesion is common digestive system diseases. Professor Xie Jingri believes that gastric precancerous lesion is located in the stomach, closely related to spleen, liver, and lung. Qi stagnation and imbalance of ascending and descending are the basis of the disease. The zang-fu viscera and qi movement are abnormal. The main inducements are deficiency, dampness, phlegm, blood stasis, toxin pathogenic factors, qi and blood loss, and imbalance of cold and heat. Therefore, Professor Xie proposes three principles of treatment of gastric precancerous lesion: first, regulate qi and blood to pass up and down; second, balance cold and heat to reconcile yin and yang; third, balance moisture and dryness to restore the rise and fall, according to the quantity of qi and blood, cold and heat to adjust usage and dosage of prescription, it can effectively relieve the clinical symptoms, delay or reverse the pathological changes of gastric atrophy, intestinal metaplasia and dysplasia.

156 Professor Li Peiwen’s experience in the treatment of chemotherapy induced oral mucositis of breast cancer based on “igniting fire and restoring yuan”
NAN Yunxin1 ZHANG Lei1 CHEN Hongyang1 WAN Donggui2
DOI: 10.20047/j.issn1673-7210.2023.02.36
Commonly used chemotherapeutic drugs for breast cancer can often lead to oral mucositis, which seriously affects the quality of life of patients, such as paclitaxel, cyclophosphamide and doxorubicin liposomes and so on. Professor Li Peiwen believes that the fundamental pathogenesis of chemotherapy induced oral mucositis in breast cancer lies in deficiency of kidney yang and yang deficiency leads to water cooling. At the same time, the fire doesn’t warm the soil and the drugs attack, leading to deficiency of spleen yang. Therefore, the treatment should be based on igniting fire and restoring yuan and it is also important to replenish the soil. Qianyang Fengsui Dan or Jingui Shenqi Pills are commonly used to add or subtract. At the same time, Professor Li makes good use of wind medicine to disperse it and cooperates with the special medicine of collecting sores and promoting muscle. The clinical effect is satisfactory.

DOI: 10.20047/j.issn1673-7210.2023.02.43
For patients with acute cerebral infarction with easily compressible arterial puncture points, how to prevent complications of puncture point bleeding before intravenous thrombolysis is not elaborated in the current guidelines, and relevant reports are rare. This article reported the two patients with acute cerebral infarction with existing puncture sites were treated pass with intravenous thrombolysis. In case one, right radial artery puncture was performed one week before intravenous thrombolysis, and left radial artery puncture was performed two hours before thrombolysis, after intravenous thrombolysis, subcutaneous hematoma was found at both radial artery puncture sites, and false aneurysm was complicated at the right radial artery puncture site. After comprehensive treatment, such as fresh frozen plasma infusion, cryoprecipitate, elastic bandage, and compression of the radial artery, bleeding complications at the artery puncture site were controlled. In case two, right radial artery puncture was performed 26 hours before intravenous thrombolysis, and the radial artery puncture site was compressed by a compressor before intravenous thrombolysis, and no bleeding complications occurred at the puncture site. Although it is not contraindicated to perform intravenous thrombolysis within the last one week, patients should be assessed for the risk of complications from bleeding at the puncture site after receiving intravenous thrombolysis. For patients at higher risk, compression of the puncture site before thrombolysis may be considered to prevent complications of puncture site bleeding.

189 Application of flexible budget method in budget preparation of public hospitals
WANG Qi MA Shiyuan MA Liya
DOI: 10.20047/j.issn1673-7210.2023.02.44
Effective budget preparation is a key factor in rational allocation of hospital resources. At present, there are problems such as insufficient coverage and insufficient matching with actual medical business in the budget preparation of public hospitals. This paper analyzes the applicability of the flexible budget method. Based on the concept of “business-finance integration”, the hospital budget goals are determined through overall planning, fixed and variable costs are divided, flexible quota standards are formulated and regularly updated, and paths such as information-based data centers are established to integrate flexible budgets. The budget law is reasonably applied to hospital budgeting. The study found that the scientific flexible budgeting method can intuitively reflect the supply and demand of various resources in hospital operation and the efficiency of use, especially the advantages of quantifying and reflecting the hospital budget, which can facilitate the hospital to use a more optimized plan to allocate resources, and ultimately achieve efficient hospitalization. Overall strategic plan.

JIANG Shuai
DOI: 10.20047/j.issn1673-7210.2023.02.45
Resilience governance is an important part of the emergency management of modern hospitals responding to sudden disasters. Combined with the resilience response time and functional recovery of hospitals, the key problems faced by hospitals under the impact of sudden disasters were analyzed. Those problems were the imperfect hospital management system and emergency plan, the insufficient hospital emergency team and funds and materials, and the unreasonable layout of hospital building facilities and equipment. Furthermore, five dimensions of modern hospital resilience governance framework are proposed, which are the unified leadership and coordination management mechanism, building mechanism for disaster emergency response team, material reserve and capital investment mechanism, emergency facilities and equipment guarantee mechanism, intelligent information monitoring, and early warning mechanism. Under the role of resilience governance, hospitals are encouraged to quickly recover and maintain their functions, and ensure the normal development of medical services.
