DOI: 10.20047/j.issn1673-7210.2022.26.02
Objective To investigate the mechanism of ferroptosis in hepatocellular carcinoma and the prognostic value of long non-coding RNA (lncRNA). Methods Hepatocellular carcinoma transcriptome data were downloaded from TCGA database, ferroptosis differential genes were screened and enriched analyzed; lncRNA associated with ferroptosis was obtained by correlation test; Cox regression analysis was used to construct a prognostic model. According to the risk scoring formula, the risk scores were calculated, and the patients with liver cancer were divided into high, low risk groups, and survival analysis and immune differential analysis were performed. Results Eighty-four ferroptosis differential genes were selected. Enrichment analysis showed correlation with mTOR metabolism, glycolysis, and γ interferon respond, and 17 ferroptosis lncRNAs were included to construct a prognostic model. The prognosis of low-risk group was better than that of high-risk group (P < 0.01); receiver operating characteristic curve showed that the model had good predictive ability. The analysis of immune microenvironment revealed that a variety of immune cells had different infiltration, and 17 immune checkpoints with different expression were identified. Conclusion Both ferroptosis genes and related lncRNAs play important regulatory roles in liver cancer, and the changes of immune microenvironment are also potential targets for liver cancer treatment.

DOI: 10.20047/j.issn1673-7210.2022.26.03
Objective To construct a nomogram prediction model for the risk of constipation in patients with coronary heart disease after percutaneous coronary intervention, and to conduct internal verification. Methods The clinical data of 398 patients with coronary heart disease who underwent percutaneous coronary intervention in the Department of Cardiology, the Second Affiliated Hospital of Air Force Military Medical University from January 2019 to December 2020 were collected retrospectively. The patients were divided into normal defecation group (262 cases) and constipation group (136 cases) according to whether constipation occurred. The differences of clinical data between the two groups were compared, and the influencing factors of constipation were screened by logistic regression model. R 3.4.3 software was used to construct a nomogram prediction model for the risk of constipation in patients with coronary heart disease after percutaneous coronary intervention, and the Bootstrap method and Hosmer-Lemeshow goodness-of-fit test were used to verify the model internally. Receiver operating characteristic (ROC) curve was used to analyze the diagnostic value of the model. Results In the constipation group, the proportions of age > 60 years, Killip grade Ⅲ-Ⅳ, opioid use after surgery, number of stents implanted > 3, anxiety/depression, diabetes mellitus, and smoking history were higher than those in the normal defecation group, and the proportion of regular exercise was lower than that in the normal defecation group, and the differences were statistically significant (P < 0.05). Logistic regression analysis showed that age > 60 years old, Killip grade Ⅲ-Ⅳ, opioid use after surgery, anxiety/depression, and diabetes mellitus were independent risk factors for constipation after percutaneous coronary intervention (OR = 2.945, 4.859, 3.624, 3.082, 2.770, P < 0.05), regular exercise was a protective factor (OR = 0.447, P < 0.05). The internal validation results of the nomogram prediction model showed that the observed curve was basically consistent with the acceptable curve and the ideal curve (P > 0.05). The area under ROC curve of the nomogram prediction model was 0.788 (95%CI: 0.742-0.833, P < 0.05). Conclusion The nomogram prediction model for the risk of constipation in patients with coronary heart disease after percutaneous coronary intervention has a good degree of calibration and discrimination, which is helpful for medical staff to individualize the risk of constipation in patients.

DOI: 10.20047/j.issn1673-7210.2022.26.07
Infant body composition is an important index to evaluate infants’ nutritional status. By infering nutritional status through body composition, nutritional problems can be found in time and nutritional interventions can be formulated. However, the unique physiology and behavior of infants bring some challenges to the assessment of body composition, and the range of methods to measure infant body composition is relatively narrow. At present, a series of studies have been carried out on the assessment of infant body composition, and some progress has been made. This article reviews the research and application progress of commonly used infant body composition assessment methods. These methods mainly include anthropometry, air displacement plethysmography, dual-energy X-ray absorption, bioelectrical resistance, isotope dilution, and magnetic resonance imaging.

YIN Zhucheng LIANG Xinjun
DOI: 10.20047/j.issn1673-7210.2022.26.08
Colorectal cancer is one of the most common malignant tumors in the world. Most patients have metastases at diagnosis, the prognosis is poor. In recent years, immune checkpoint inhibitors have made a breakthrough progress in the treatment of a variety of malignant tumors, which is one of the current research hotspots. Monotherapy or combination therapy with programmed death-1 (PD-1)/programmed cell death ligand-1 (PD-L1) inhibitors may provide clinical benefit for patients with DNA mismatch repair defects/high microsatellite instability metastatic colorectal cancer (mCRC). The efficacy of monotherapy in patients with mismatch repair proficient/microsatellite stable mCRC is poor, and the feasibility of combination therapy with PD-1/PD-L1 inhibitors is being explored. This article reviews the application of PD-1/PD-L1 inhibitors in first-line, second-line, and posterior line treatment of mCRC patients, as well as the clinical research status and progress of PD-1/PD-L1 inhibitors monotherapy and combination therapy in different types of mCRC patients, so as to provide evidence for clinical application.

DOI: 10.20047/j.issn1673-7210.2022.26.26
Objective To investigate the prognostic value of peripheral blood absolute lymphocyte count (ALC)/absolute monocyte count (AMC) combined with serum albumin globulin ratio (AGR) in patients undergoing radical gastrectomy for gastric cancer. Methods A total of 96 patients who planned to undergo radical gastrectomy in Rugao Hospital Affiliated to Nantong University from January 2018 to January 2020 were selected. The patients who underwent radical gastrectomy were divided into recurrence group and non-recurrence group according to their prognosis. The clinicopathological features including TNM stage, differentiation degree, and lymph node metastasis were compared between the two groups. Logistic regression analysis was used to explore the influencing factors of postoperative recurrence, and receiver operator characteristic curve was used to evaluate the predictive value of peripheral blood ALC/AMC and AGR levels on postoperative recurrence. Results The proportion of undifferentiated/poorly differentiated, TNM stage Ⅲ, lymph node metastasis, peripheral blood ALC/AMC, and AGR levels in the recurrence group were higher than those in the non-recurrence group (P < 0.05). Stage Ⅲ, undifferentiated/poorly differentiated, lymph node metastasis, ALC/AMC, and AGR levels were risk factors for recurrence after radical gastrectomy (OR = 2.768, 3.040, 3.083, 4.976, 4.235, P < 0.05). The area under the curve of ALC/AMC and AGR combined in predicting postoperative recurrence was higher than that of ALC/AMC and AGR alone (P < 0.05). Conclusion The stage, undifferentiated/poorly differentiated, having lymph node metastasis, ALC/AMC, and AGR levels were all risk factors affecting the relapse group. ALC/AMC and AGR can affect the prognosis of patients, and the combination of the two can be used as a reference index to predict the recurrence after radical gastrectomy.

125 Pathogenesis of active rheumatoid arthritis induced by “overexertion leading to qi consumption”
JIANG Wen JIANG Quan XU Haodong CHENG Zengyu TANG Xiaopo
DOI: 10.20047/j.issn1673-7210.2022.26.28
Rheumatoid arthritis (RA) is a crippling autoimmune disease with a high prevalence and disability rate, which seriously threatens the quality of life of patients. The clinical manifestation is that active stage and remission stage appear alternately. Repeated activity accelerates the process of bone destruction. How to stabilize the condition, avoid RA activity, and achieve continuous remission is the focus and difficulty of clinical treatment. That “overexertion leading to qi consumption” is an important cause of RA activity. Prevention of insufficient consumption of qi should be carried out throughout the treatment of RA. By explaining the connotation and extension of this classical theory of traditional Chinese medicine, this paper analyzes the pathogenesis characteristics of “overexertion leading to qi consumption” in the process of inducing RA activity from the point of view of five-zang fatigue and five-body arthralgia, expounds its close relationship with the turning point event of RA activity, and links it with the pathogenesis pathology of immune imbalance and inflammatory storm, so as to provide a theoretical basis for explaining the scientific connotation of RA activity caused by “overexertion leading to qi consumption”. In order to provide reference for RA activities of comprehensive prevention and preventive treatment in traditional Chinese medicine.

129 Wei Ni’s experience abstract in treating gouty arthritis in young people
CHEN Xia WEI Ni
DOI: 10.20047/j.issn1673-7210.2022.26.29
Gout is a heterogeneous group of diseases caused by purine metabolism disorder and/or uric acid excretion disorder. Gouty arthritis is the main clinical manifestation. The prevalence of gouty arthritis is increasing year by year, showing a trend of younger onset. Associate chief physician Wei Ni thinks that compared with elderly patients, young patients with gouty arthritis have more congenital kidney qi deficiency and more frequent drinking and high-purine diets. In addition, bad emotions cause stagnation of liver qi, which affects the heart and lungs over time, and aggravates phlegm and dampness, blood stasis. These factors can cause frequent attacks of gouty arthritis, high blood uric acid levels, and a higher chance of tophi. Associate chief physician Wei Ni emphasizes that the five zang viscera should be coordinated to eliminate the source of phlegm, dampness, and blood stasis in the treatment of young patients with gouty arthritis. At the same time, the drug of dispelling stasis and removing obstruction in collaterals should be used as early as possible to prevent the formation of tophi.

DOI: 10.20047/j.issn1673-7210.2022.26.41
Objective To analyze the current situation of scientific and technological achievements of the tertiary medical institutions in Nanjing from 2010 to 2020, and to explore deficiencies and propose feasible countermeasures. Methods According to the names of 35 tertiary medical institutions published by Nanjing Municipal Health Commission, descriptive analysis of the outcomes was carried out based on the retrieval of China Hospital Knowledge Warehouse database. Results A total of 23 843 scientific and technological achievements, of which 84.7% were journal articles; there were five medical institutions with more than 1 000 achievements. Among the top 12 medical institutions, nine were located in Qinhuai District and Gulou District, all of which were tertiary medical institutions. The average output of general hospitals, traditional Chinese medicine (general) hospitals and integrated traditional Chinese and western medicine hospitals was 2.3 times that of specialized hospitals, 5.5 times that of tertiary medical institutions, and 12.8 times that of public medical institutions. Scientific and technological achievements were on the rise. Conclusion Achievements are mainly journal papers, with few patent. Medical and health institutions ignore the transformation of achievements. The uneven development of the institutions caused the gap in achievements. Measures including creating good institutional environment, building service platform, emphasizing the compound talents, and advocating collaboration and sharing can effectively improve the level of scientific and technological achievements.

DOI: 10.20047/j.issn1673-7210.2022.26.42
Objective To research the resources and services of primary medical institutions in China, and to compare and analyze the overall situation of hospitals and medical service systems, so as to provide references for optimizing the allocation of medical resources and promoting the formation of a reasonable medical treatment pattern. Methods A descriptive analysis was made of the changes in resources and services of primary medical institutions from January 2010 to December 2018, and the proportion of resources and services of primary medical institutions and hospitals in the medical service system were compared and analyzed. The grey system model GM(1,1) was used to predict the number of consultations and general practitioners in primary medical institutions from 2019 to 2021. Results The resources and services of primary medical institutions were on the increase, but the development of hospitals was faster than that of primary medical institutions. The construction of general medicine talent team had achieved initial results, especially the proportion of general practitioners in township health centers had increased rapidly. Conclusion It is necessary to further regulate the total amount and scale of resources of primary medical institutions, strengthen the training and use of talents in primary medical institutions, strengthen the policy guidance and supporting measures for the implementation of the hierarchical diagnosis and treatment system, guide residents to realize the first diagnosis at the primary level, and gradually realize the optimization of the allocation of primary medical resources and the improvement of medical treatment. The order is more reasonable and orderly.

189 Theoretical basis and innovation path of traditional Chinese medicine integrated community health management under active health
LUO Guihua REN Jiarui OUYANG Jing JIA Lili LI Xiuqin ZHANG Yuan
DOI: 10.20047/j.issn1673-7210.2022.26.43
In view of the fact that the existing “passive health” medical service model in China is far from meeting the increasing demand for health, in order to further solve the problems of population aging, increasing chronic diseases and younger patients at the same time, this paper proposes the “active health” service model. The idea of “preventive treatment of disease” should be integrated into modern health management, and the appropriate techniques of traditional Chinese medicine should be fully tapped. The collaborative governance path should be realized with the participation of the government, universities, medical institutions, communities, and families, and the advantages of traditional Chinese medicine health intervention should be integrated. A “active health” model of health maintenance and health promotion based on the “nationwide system” has been formed, and traditional Chinese medicine has been promoted in communities to provide convenient access to the health of the whole people.

DOI: 10.20047/j.issn1673-7210.2022.26.44
This paper sorts out the problems and research limitations of online recruitment interviews with the method of literature review. Combined with the practical process of conducting online recruitment interviews in Beijing Tongren Hospital, Capital Medical University in 2021, the advantages and disadvantages of online interviews are summarized through interviews with experts and interviewers. Online recruitment effectively avoids aggregation and provides a new solution for talent recruitment evaluation in public hospitals, expands the scope of candidates, and ensures the number of hospital talent selection; the employment of graduates has been supported and the recruitment management has been promoted. This article suggests that the recruitment work can be further improved by improving the recruitment materials, improving the recruitment process, and confirming the identity of the candidates for online recruitment. Finally, the application of online recruitment under the multi-district management mode of the hospital was discussed.
