| Theoretical basis and innovation path of traditional Chinese medicine integrated community health management under active health |

Abstract In view of the fact that the existing “passive health” medical service model in China is far from meeting the increasing demand for health, in order to further solve the problems of population aging, increasing chronic diseases and younger patients at the same time, this paper proposes the “active health” service model. The idea of “preventive treatment of disease” should be integrated into modern health management, and the appropriate techniques of traditional Chinese medicine should be fully tapped. The collaborative governance path should be realized with the participation of the government, universities, medical institutions, communities, and families, and the advantages of traditional Chinese medicine health intervention should be integrated. A “active health” model of health maintenance and health promotion based on the “nationwide system” has been formed, and traditional Chinese medicine has been promoted in communities to provide convenient access to the health of the whole people.
