ZHAO Yuling HUANG Yi ZHOU Yanqiong JIANG Feifei CHEN Haiyan
Objective To explore the effects of hot medicinal compress therapy on neuronal ultrastructure and expression of autophagy factors in rats with cervical spondylotic radiculopathy (CSR) based on Akt / mTOR signaling pathway. Methods A total of 18 4-week-old SPF SD rats, weighing 150-200 g, and six of them were randomly selected as a blank group, and the rest were established as CSR models. After the molding was successful, they were divided into model group and hot medicinal compress group according to random number table method, with six rats in each group. Blank group and model group were fed normally without any treatment. In the hot medicinal compress group, the bladder channel, the governor channel, and the gallbladder channel were ironed for seven days. After the experiment, the structure of nerve cells was observed by HE staining. Transmission electron microscopy was used to observe the ultrastructure of nerve cells. The expression levels of p-Akt, p-mTOR, Beclin-1, and LC3-Ⅱ/LC3-Ⅰ proteins were detected by Western blot. Results HE staining showed that the tissue morphology of nerve root in the blank group was basically normal. In the model group, the tissue was severely damaged, the outer membrane of nerve root was incomplete, the fibers were disordered, and the axons of some fibers disappeared. The number of cell bodies decreased obviously, but nucleoli were not obvious. In the hot medicinal compress group, the tissue morphology was partially damaged, the nerve root outer membrane was more complete, the fibers were arranged more neatly, and the interstitium was infiltrated with mild inflammatory cells. Transmission electron microscopy showed that cells in blank group had abundant cytoplasm and normal organelles. In the model group, there were nuclear cavities, mitochondrial membrane was fuzzy and some cristae was broken, many lysosomes and autophagosomes were found in the cytoplasm. In the hot medicinal compress group, cytoplasm was abundant, part of mitochondrial membrane was broken, part of cristae was broken and dissolved, lysosomes and autophagosomes were found in cytoplasm. The expression levels of p-Akt and p-mTOR in the model group were lower than those in the blank group, while LC3-Ⅱ/LC3-Ⅰ and Beclin-1 were higher than those in the blank group, the differences were highly statistically significant (P < 0.01). The expression levels of p-Akt and p-mTOR in hot medicial compress group were higher than those in model group, while LC3-Ⅱ/LC3-Ⅰ and Beclin-1 were lower than those in model group, the differences were highly statistically significant (P < 0.01). Conclusion Hot medicinal compress therapy may inhibit excessive autophagy, reduce apoptosis, and promote recovery of damaged nerve roots by enhancing p-Akt and p-mTOR protein expression, and activating Akt / mTOR pathway.

111 Discussion on the participation of traditional Chinese medicine in the clinical management of pulmonary nodules#br#
LI Yaoyuan HUA Baojin ZHENG Honggang
In recent years, the detection rate of pulmonary nodules has been significantly improved, and its reasonable clinical management has great practical value. Traditional Chinese medicine can cut off and reverse the development trend of isolated and incompletely resected pulmonary nodules, eliminate or control their growth and reduce the incidence of lung cancer; combining disease differentiation, constitution differentiation and imaging characteristics, we can provide characteristic treatment scheme of traditional Chinese medicine for patients with asymptomatic pulmonary nodules. Traditional Chinese medicine can reduce the postoperative clinical symptoms of patients with pulmonary nodules and prevent recurrence and metastasis. Comprehensive therapy of traditional Chinese medicine can reduce the incidence of pulmonary nodules / lung cancer by intervening relevant risk factors in advance. Therefore, traditional Chinese medicine can be applied to the prevention and treatment of pulmonary nodules in all stages of the whole cycle to improve the efficiency of clinical management, which is an effective supplement to the current diagnosis and treatment norms.

115 Professor Liu Ying’s differentiation and treatment experience of Sj?觟gren syndrome based on “theory of latent toxin”#br#
LUO Yawen1 LIU Ying2
Sj?觟gren syndrome is a common clinical rheumatic immune disease, which can lead to multi-system damage in addition to exocrine gland damage, seriously affecting patientsem damacal and mental health and quality of life. Modern medicine shows no specific therapeutic drugs for this disease, while Chinese medicine provides a reliable cure. This disease, which comes under the category of “zaobi” in Chinese medicine, characterized by pathologically insidious and severe onset, and clinically prolonged course, increasingly severer condition, and not responding to treatment, matches “latent toxin” in the pathogenic characteristics. Based on years of clinical experience, Professor Liu Ying concludes that the key pathogenic factor of this disease is “hidden toxicity due to vital qi deficiency”, and proposes the treatment method of “clearing dryness to relieve toxins, resolving stasis to remove toxins, and nourishing yin to benefit qi”. This method focuses on “regulation of body and spirit together” and the combination of regulation and nourishment in the treatment. In the meantime, removing toxins and reinforcing vital qi have their own emphasis in different stages of the disease. All of these combine to reflect the concept of “people-oriented” in Chinese medicine. This paper is intended to elaborate the treatment experience of the “theory of latent toxin” on Sj?觟gren syndrome in order to broaden our mind on the treatment of this disease.

119 Thoughts on differentiation and treatment of three yin-meridian in fundus macular edema#br#
SONG Haijiao1 JIN Ming2
Fundus macular edema can be caused by various eye diseases, it could result in central vision loss and visual distortion. At present, Western medicine treatment is not satisfactory, however Chinese medicine shows unique advantages. This paper discusses the pathogenesis and treatment of fundus macular edema by Chinese medicine from the viewpoint of “three yin-meridian obstruction can lead to water disease” in Huangdi Neijing. The auther thinks that chill obstruction in hand taiyin lung, foot taiyin spleen or foot shaoyin kidney meridian could lead to fundus macular edema, so the treatment is first to recognize the organ which is lack of vital energy, then to tonify them, and at the same time, to eliminate damp by freeing lung-qi, activating spleen, promoting diuresis and activating blood circulation. To cure both the symptoms and root causes is effective.

174 A case of gastric adenocarcinoma of the fundic gland type and literature review#br#
KOU Mengjia1 GONG Yucheng1 WANG Zhibin2
Gastric adenocarcinoma of the fundic gland type (GA-FG) is a rare tumor that belongs to the new gastric cancer subtype. However, its pathogenesis is not clear, the submucosal infiltration rate is high, but the degree of malignancy is low, lymphatic and venous metastases are less common, and the prognosis is good. In previous reports, GA-FG was mostly submucosal bulge, and the tumor surface was whitish or faded. One case of GA-FG patient reported in this article was admitted to the Department of Spleen, Stomach and Hepatobiliary, Dongfang Hospital, Beijing University of Chinese Medicine on July 30, 2020. The shape of the lesion is a rare flat type, the surface of the tumor is reddish, and there are fundus gland polyps around the lesion. The clinical manifestations were acid reflux heartburn, history of proton pump inhibitor drugs, and endoscopic submucosal dissection treatment. There was no obvious abnormality after the operation. This article reports on the diagnosis and treatment process of patients, reviews and reviews relevant literature, and concludes that the endoscopic diagnosis of GA-FG has not yet formed a consensus and has limitations, and it is very easy to misdiagnose and miss diagnosis. However, it has unique clinicopathological characteristics, combined with morphology and immunophenotype, it can be diagnosed clearly. In terms of etiology, whether proton pump inhibitor is related to the occurrence of GA-FG requires more reports for research and demonstration.

LI Yuan1 JIANG Yan1 DONG Shi2 XU Xunhang1 AN Xuzhao1 LOU Cuidi1 CHENG Wei1
Objective To analyze the input and output of traditional Chinese medicine hospitals in 680 poverty-stricken counties in China in 2018, and to analyze the operating efficiency of hospitals in each province, and to propose efficiency improvement measures. Methods This study was carried out in December 2019. Data envelopment analysis was conducted to calculate the efficiency of 680 traditional Chinese medicine hospitals in poverty-stricken counties in China (from 22 provinces) in 2018 by collecting the number of health technicians, beds available, revenue from financial subsidies, total expenditure, total number of patients, and the number of discharged patients. Results Traditional Chinese medicine hospitals in poverty-stricken counties were mainly secondary hospitals. There were great differences in hospital quantity, input-output between eastern, central and western regions. According to data envelopment analysis, 8 of the 22 provinces had comprehensive technical efficiency of 1, and 10 provinces had pure technical efficiency of 1. Among 14 provinces with scale efficiency < 1, 9 provinces showed decreasing returns to scale, while the other 5 provinces showed increasing returns to scale. Conclusion The overall operating efficiency of provincial hospitals is not high, and the technical efficiency and scale efficiency need to be further improved. The state should strengthen technical training for grass-roots health personnel, adopt differentiated strategies for targeted poverty alleviation, increase investment in hospitals in areas with increasing returns to scale, and pay attention to improving the technical management level of hospitals in areas with decreasing returns to scale.
