Discussion on the mechanism of Shema Oral Liquid in the treatment of Coronavirus disease 2019 based on network pharmacology

Objective To investigate the mechanism of Shema Oral Liquid in the treatment of Coronavirus disease 2019 (COVID-19). Methods The active components and targets of Shema Oral Liquid were searched by Chinese medicine pharmacology database and analysis platform (TCMSP). COVID-19 related targets were obtained from GeneCards database. The potential targets of Shema Oral Liquid in the treatment of COVID-19 were obtained by mapping the drug targets and disease targets for subsequent analysis. Protein-protein interaction (PPI) network and drug-component-target-disease network were constructed by STRING database and Cytoscape software, and disease and function analysis were carried out through Ingenuity Pathway Analysis (IPA). The Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment analysis was carried out by using STRING database. Results A total of 106 active components and 53 potential therapeutic targets of COVID-19 from 10 traditional Chinese medicines in Shema Oral Liquid were screened. The main active components included quercetin, luteolin, β-sitosterol, kaempferol, stigmasterol, etc., and the key targets involved MAPK3, TNF, IL-6, MAPK8, CXCL8, etc. The 53 therapeutic targets were mainly involved in cell death and survival, body injury and abnormality, inflammatory response and other biological functions. KEGG pathway enrichment resulted in 169 biological pathways, mainly involving IL-17, Toll-like receptor, HIF-1, TNF signaling pathways, etc. Conclusion Shema Oral Liquid may act on IL-6, TNF, MAPK8, CXCL8 and other targets through various active ingredients such as quercetin, luteolin, β-sitosterol, kaempferol, and then regulate multiple pathways such as IL-17, TNF, Toll-like receptor, and HIF-1 to treat COVID-19.

Effect and mechanism reserch of Gabexate on cerebral ischemia reperfusion injury in rats

Objective To explore the effect and mechanism of Gabexate on cerebral ischemia reperfusion injury in rats. Methods A total of 125 experimental rats were selected to establish focal cerebral ischemia reperfusion model by modified thread occlusion method. All rats were divided into sham operation group, model group and Gabexate high, medium and low dose groups according to random number table method, with 25 rats in each group. Low, medium and high dose Gabexate groups were intervened by intraperitoneal injection of 25, 50 and 75 mg Gabexate, respectively. Sham operation group and model group were given the same volume of normal saline. The rats in each group were scored by blind method. The pathological changes of brain tissue and the apoptosis of nerve cells were observed. The activities of antioxidant enzymes, malondialdehyde (MDA), inflammatory factors, apoptosis related gene expression and related protein expression in brain tissue were determined. Results The neurological deficit score, glutathione peroxidase (GSH-Px), B cell lymphoma factor 2 (bcl-2) mRNA and nuclear factor kappa B (NF-κB) in model group were lower than those in sham operation group, the apoptosis rate of nerve cells, MDA, superoxide dismutase (SOD), catalase (CAT), tumor necrosis factor α (TNF-α), interleukin (IL)-1β, IL-6, water soluble protein (Bax) mRNA, caspase-3 and NF-κB in model group were higher than those in sham operation group, the differences were statistically significant (P < 0.05). The neurological deficit score, MDA, SOD, bcl-2 mRNA and NF-κB in medium and high dose Gabexate groups were higher than those in low dose Gabexate group, the apoptosis rate, GSH-Px, CAT, TNF-α, IL-1β, IL-6, Bax mRNA, caspase-3 and NF-κB of medium and high dose Gabexate groups were lower than those of low dose Gabexate group, the differences were statistically significant (P < 0.05). The scores of neurological deficit, MDA, SOD, bcl-2 mRNA and NF-κB in high dose Gabexate group were higher than those in medium dose Gabexate group, the apoptosis rate, GSH-Px, CAT, TNF-α, IL-1β, IL-6, Bax mRNA, caspase-3 and NF-κB of high dose Gabexate group were lower than those of medium dose Gabexate group, the differences were statistically significant (P < 0.05). The neurological deficit score, MDA, SOD, bcl-2 mRNA and NF-κB in Gabexate low, medium and high dose groups were higher than those in model group, the apoptosis rate of nerve cells, CAT, TNF-α, IL-1β, IL-6, Bax mRNA, caspase-3 and NF-κB in Gabexate low, medium and high dose groups were lower than those in model group, the differences were statistically significant (P < 0.05). The level of GSH-Px in Gabexate low dose group was higher than that in model group, and the levels of GSH-Px in Gabexate middle and high dose groups were lower than those in model group, the differences were statistically significant (P < 0.05). Conclusion Gabexate can effectively reduce the pathological damage caused by cerebral ischemia reperfusion in rats, inhibit lipid oxidation reaction after reperfusion, play a protective role, and reduce the damage caused by inflammatory reaction. The above effects may be caused by inhibiting NF-κB signal pathway and increasing the activity of antioxidant enzymes, so as to reduce the inflammatory injury of brain tissue and the apoptosis of nerve cells.

Regulating effect of Yiqi Yangyin Xiaoying Decoction on B cell activation in autoimmune thyroiditis model rats

Objective To observe the regulating effect of Yiqi Yangyin Xiaoying Decoction on B lymphocyte activation in rats with autoimmune thyroiditis (AIT) model. Methods According to random number table method, 50 female SD rats at four-five weeks were divided into normal control group, model group, low, medium and high dose Yiqi Yangyin Xiaoying Decoction groups, with 10 rats in each group. The AIT model was made by subcutaneous injection of porcine thyroglobulin and feed with high iodine water. The low, medium and high dose Yiqi Yangyin Xiaoying Decoction groups were given 6, 12 and 24 g/kg solution of Yiqi Yangyin Xiaoying Decoction by gavage, respectively. Normal control group and model group were given the same amount of normal saline by gavage, intervention for six weeks. Serum levels of thyroid stimulating hormone (TSH), thyroid globulin antibody (TGAb), thyroid peroxidase antibody (TPOAb) and the concentration of serum B cell activating factor (BAFF) were determined by enzymy-linked immunosorbent assay. The expression level of interleukin-10 (IL-10) in thyroid tissues of rats was detected by immunohistochemistry. Results Compared with the normal control group, serum TSH, TGAb, TPOAb levels and BAFF level in model group were increased, while the expression level of IL-10 in thyroid tissue was decreased, and the differences were highly statistically significant (all P < 0.05). Compared with model group, the concentrations of TSH, TgAb and TPOAb in the medium and high dose Yiqi Yangyin Xiaoying Decoction groups were decreased, and the differences were highly statistically significant (all P < 0.01); the concentration of TGAb and TPOAb in low dose Yiqi Yangyin Xiaoying Decoction group were decreased, and the differences were highly statistically significant (P < 0.01); BAFF content in all dose groups of Yiqi Yangyin Xiaoying Decoction decreased, and the relative expression level of IL-10 in thyroid tissue increased, and the differences were statistically significant (all P < 0.05). TSH concentrations in the medium and high dose Yiqi Yangyin Xiaoying Decoction groups were lower than those in the low dose Yiqi Yangyin Xiaoying Decoction group, and the differences were highly statistically significant (P < 0.05 or P < 0.01). The concentration of TGAb in the high dose Yiqi Yangyin Xiaoying Decoction group was lower than that in the medium and low dose Yiqi Yangyin Xiaoying Decoction groups, and that in the medium dose Yiqi Yangyin Xiaoying Decoction group was lower than that in the low dose Yiqi Yangyin Xiaoying Decoction group, and the differences were highly statistically significant (P < 0.05 or P < 0.01). The content of BAFF in the high dose Yiqi Yangyin Xiaoying Decoction group was lower than that in the low and medium dose Yiqi Yangyin Xiaoying Decoction groups, and the differences were statistically significant (all P < 0.05). The expression level of IL-10 in the medium and high dose Yiqi Yangyin Xiaoying Decoction groups was higher than that in the low dose Yiqi Yangyin Xiaoying Decoction group, and the differences were statistically significant (all P < 0.05). Conclusion Yiqi Yangyin Xiaoying Decoction decreases TSH, TGAb and TPOAb levels in AIT rats, and improves immune imbalance, which may be related to the mechanism of inhibiting B lymphocyte activation and upregulating thyroid IL-10 expression.

Relationship between urinary immunoglobulin G and all-cause mortality in patients with acute ST-segment elevation myocardial infarction

Objective To explore the relationship between urine immunoglobulin G (UIgG) and in-hospital death in patients with acute ST elevation myocardial infarction (STEMI). Methods A total of 1854 STEMI patients admitted to Beijing Friendship Hospital from 2013 to 2019 were retrospectively included, UIgG and other baseline data were collected. According to the UIgG tertiles, they were divided into three groups, including 668 cases in the low UIgG group, 570 cases in the medium UIgG group, and 616 cases in the high UIgG group. The primary clinical endpoint was all-cause death in the hospital, and the secondary endpoint was cardiovascular death and adverse cardiovascular events. The differences in the clinical indicators of the three groups were compared, while the related factors of hospital death were analyzed, and the ROC curve was draw. Results The three groups of age, male ratio, history of hypertension, white blood cells, blood creatinine, total bilirubin, high-sensitivity C-reactive protein, and glycosylated hemoglobin were compared, and the differences were statistically significant (P < 0.05). The above indicators in the high UIgG group were higher than those in the low UIgG group, and the difference was statistically significant (P < 0.05). The three groups of hospital death, cardiogenic death, MACE ratio, N-terminal pro-brain natriuretic peptide, creatine kinase isoenzyme-MB, cardiac troponin, and left ventricular ejection fraction were compared, and the differences were statistically significant (P < 0.05). The peaks of NT-proBNP, CK-MB, and cTnI in the high UIgG group were higher than those in the low UIgG group, while LVEF was lower than that in the low UIgG group, and the differences were highly statistically significant (P < 0.01). The incidence of in-hospital death, cardiogenic death and MACE in the high UIgG group were higher than those in the low UIgG group, and the differences were highly statistically significant (P < 0.01). UIgG was an independent risk factor for in-hospital death in STEMI patients (OR = 1.042, 95%CI: 1.002-1.083, P = 0.042). When UIgG was the cut point of 2.09 mg/dL, the AUC was 0.75 (95%CI: 0.67-0.82, P < 0.01), the sensitivity was 67.4%, and the specificity was 76.5%. Conclusion UIgG is an independent risk factor associated with hospital death in STEMI patients.

Changes of levels of serum HMGB1 and lipoprotein a in newborns with hypoxic ischemic encephalopathy and its clinical significances

Objective To investigate of levels changes of serum high mobility group box-1 protein (HMGB1) and lipoprotein a in newborns with hypoxic ischemic encephalopathy and its clinical significances. Methods A total of 105 newborns with hypoxic ischemic encephalopathy which diagnosed and treated in Maternity and Child Care Center of Qinhuangdao, Hebei Province (referred to as “our hospital” for short) from January 2018 to March 2020 were selected as the observation group. According to the grading standard, they were further divided into mild group (57 cases), moderate group (33 cases) and severe group (15 cases). Fifty-three healthy newborns born in the department of obstetrics and gynecology of our hospital during the same period were selected as the control group. Serum levels of HMGB1, lipoprotein a and neonatal behavioral neurological assessment (NBNA) scores were compared in different group. The correlation between serum levels of HMGB1, lipoprotein a and NBNA score was analyzed. Receiver operating characteristic (ROC) curve was used to evaluate the diagnostic value of serum HMGB1 and lipoprotein a levels in newborns with hypoxic ischemic encephalopathy. Results The levels of serum HMGB1 and lipoprotein a in the observation group were significantly higher than those in the control group, and the differences were statistically significant (all P < 0.05). The levels of serum HMGB1 and lipoprotein a in the severe group were higher than those in the moderate and mild groups, while the NBNA score was lower than that in the moderate group and mild group, with statistically significant differences (all P < 0.05). The serum HMGB1 and lipoprotein a levels of the moderate group were higher than those in the mild group, while the NBNA score was lower than that in the mild group, with statistically significant differences (all P < 0.05). Serum levels of HMGB1 (r = -0.9123,P < 0.05) and lipoprotein a (r = -0.9045,P < 0.05) were both negatively correlated with NBNA score. ROC curve analysis results showed that the combined detection of serum HMGB1 and lipoprotein a was more valuable in the diagnosis of newborns with hypoxic ischemic encephalopathy than the individual detection. Conclusion The serum levels of HMGB1 and lipoprotein a of newborns with hypoxic ischemic encephalopathy are increased and correlated with the severity of the disease, which can be used as molecular markers for the diagnosis of hypoxic ischemic encephalopathy.

Prognostic model of hospital acquired pneumonia in the elderly based on geriatric nutritional risk index

Objective To explore the value of the geriatric nutritional risk index (GNRI) in evaluating the prognosis of patients with hospital-acquired pneumonia (HAP) in geriatric patients, and a histogram model was established to predict the prognosis of elderly HAP patients. Methods A total of 411 patients in the Emergency Center of the People’s Hospital of Xinjiang Uygur Autonomous Region from January 2013 to January 2020 were included in this study. All patients were divided into modeling group (308 cases) and validation group (103 cases) in a 3∶1 ratio by simple randomization method. Multivariate logistic regression analysis was used to analyze the independent risk factors for poor prognosis in elderly HAP patients. R software was used to construct a histogram model to predict the risk of poor prognosis in elderly HAP patients, and the model was verified. Results Univariate regression analysis showed that age, gender, systolic blood pressure, diastolic blood pressure, heart rhythm, congestive heart failure, chronic kidney disease, fasting blood glucose, low density lipoprotein cholesterol, GNRI scores were associated with poor prognosis in elderly HAP patients (P < 0.05), multivariate regression analysis showed that age, gender, congestive heart failure, and GNRI score were independent risk factors for poor prognosis in elderly HAP patients (P < 0.05). The model was validated internally and externally, and the AUC value in the modeling group was 0.854 (95%CI: 0.827-0.881), AUC value in the validation group was 0.841 (95%CI: 0.803-0.879), Hosmer-Lemeshow test results indicate that the prediction model has good stability: modeling queue (P = 0.739) and validation queue (P = 0.469). Conclusion The prediction model established in this study can accurately predict the individual risk of poor prognosis in elderly HAP patients, and has a high potential clinical application value.

Research progress on the mechanism of ligustrazine in anti-liver fibrosis from the method of promoting blood circulation to remove meridian obstruction

Liver fibrosis is the only way for the occurrence of liver cirrhosis, which belongs to the early stage and is reversible. Therefore, how to effectively delay the sustained progress or even reverse is a key issue in the treatment of liver fibrosis at present. Traditional Chinese medicine believes that the basic pathogenesis belongs to blood stasis blocking meridians, and promoting blood circulation to remove meridian obstruction is basic treatment method. Comprehensive studies at home and abroad have found that ligustrazine can inhibit hepatic stellate cells (HSCs) activation by regulating a variety of cytokines, induce activated HSCs apoptosis, regulate adipocytokines, liver inflammation, anti-oxidant stress, regulate sinusoidal endothelial cells, autophagy and so on. It has a prospect in the treatment of liver fibrosis in the future.

Research progress of animal models of spinal interbody fusion

Spinal interbody fusion is a routine technique for the treatment of spinal trauma, spinal tumor, degenerative diseases and spinal deformity. Spinal interbody fusion enables solid bony fixation of the fused bone graft segments, and it can also open and maintain the normal height of interbody fusion. However, spinal fusion failure is a problem that spinal surgeons must face in clinical work. A large number of clinical observations have shown that spinal fusion currently has a 10% to 15% non-fusion rate with internal fixation. Many researchers have investigated the effects of new surgical procedures, fusion devices, and bone graft materials on fusion rate in animals. However, there are many kinds of animals with different characteristics. Large animals are better able to simulate the biomechanics of bone graft materials or spinal instruments in the human body, while small animals have more advantages in the study of conceptual strategies. The selection of suitable animal models is the key to the success of experiments, but there is no unified standard for animal selection at present. This paper reviews the characteristics of different animal models of spinal intervertebral vertebrae, in order to provide a theoretical basis for animal research.

Research progress of artificial intelligence technology in clinical diagnosis and treatment of traditional Chinese medicine

With the development and maturity of computer technology, artificial intelligence has ushered in the golden age of development. This article aims to summarize the development and application of intelligent diagnosis and treatment technology in the field of traditional Chinese medicine (TCM), in order to explore the development direction of TCM modernization. The data comes from CNKI and Wanfang Data. Keywords such as “artificial intelligence”, “Chinese medicine diagnosis and treatment”, “intelligent diagnosis and treatment instrument”, “intelligent syndrome differentiation”, and “intelligence of traditional Chinese medicine” are searched for filtering out literatures related to the intelligence of Chinese medicine diagnosis and treatment. The literatures are categorized into four aspects of TCM diagnosis, TCM syndrome differentiation, health management, ophthalmology, etc. Finally, it can be concluded that artificial intelligence technology is continuously developing in TCM clinical diagnosis and treatment. The application of artificial intelligence technology makes TCM diagnosis and treatment technology more accurate and visualized, which can effectively guide clinical practice.

Research progress of autologous menstrual blood derived stem cells in the treatment of intrauterine adhesions

The intrauterine adhesions are a kind of common endometrial injury diseases. In addition, the occurrence of intrauterine adhesions is related to the decrease of endometrial stem cells and functional defects, so stem cell transplantation provides a new possibility for the treatment of intrauterine adhesions. Therefore, how to treat intrauterine adhesions, prevent the recurrence of adhesions, and solve the pregnancy needs of patients is one of the urgent clinical problems. Among them, menstrual blood stem cells have been widely studied for their advantages such as rapid proliferation, multiple sampling, non-trauma, and no ethical issues and so on. This article reviews the prospect of autologous menstrual blood stem cells in the treatment of intrauterine adhesions in recent years, and provides a new idea for the treatment of intrauterine adhesions.

Compilation and reliability and validity test of disease knowledge, attitude and behavior questionnaire for patients with intracranial aneurysm

Objective To compile and test the reliability and validity of a questionnaire for the knowledge, attitude, behavior of patients with intracranial aneurysms in China. Methods Systematic retrieval of domestic and foreign literature through evidence-based methods to summarize the best evidence, semi-structured interviews and expert consultations were combined, and based on the theoretical model of knowledge, attitude and behavior, the first draft of the disease knowledge, attitude and behavior questionnaire for patients with intracranial aneurysm was compiled, while the method of stratified random sampling was adopted among 20 patients in department of neurosurgery for pre-investigated and the questionnaire was revised. The convenience sampling method was used to select 152 patients in the department of neurosurgery of a tertiary a hospital in Xinjiang from July 2019 to May 2020 to investigate the reliability and validity of the questionnaire. Results The intracranial aneurysm knowledge, attitude and behavior questionnaire contains three dimensions of knowledge, attitude and behavior, with a total of 30 items; the total questionnaire Cronbach’s α coefficient was 0.966, while the Cronbach’s α coefficient of each dimension were 0.941, 0.913, 0.936, and its half-reliability was 0.899; KMO value was 0.919, and the cumulative variance contribution rates of the common factors of the three dimensions were 25.501%, 47.463%, and 63.216%. Conclusion Intracranial aneurysm patients’ disease knowledge, attitude and behavior questionnaire has good reliability and validity, and can be used in patients with intracranial aneurysm.

Psychological stress status and personality characteristics of patients with artificial insemination with husband’s semen and its relationship with the success rate of fertilization

Objective To observe the psychological stress and personality characteristics of patients with husband’s artificial insemination, to analyze the relationship between psychological stress and personality characteristics and the success of fertilization. Methods The clinical data of 127 infertility patients who underwent husband’s artificial insemination in Fuyang People’s Hospital from January 2018 to January 2020 were retrospectively collected. According to the success or failure of fertilization, they were divided into successful group (33 cases) and failure group (94 cases). The psychological stress status (assessed by the life event scale [LES]) and personality characteristics (assessed by the Eysenck personality questionnaire [EPQ], including the validity scale [L], the psychopath scale [P], the emotional scale [N] and the internal and external tendency scale [E]) of the two groups were observed, while the relationship between psychological stress status and personality characteristics and the success of artificial insemination patients were analyzed. Results There was no statistically significant difference between the two groups of positive life events on the LES scale (P > 0.05). The scores of negative life events and total life events on the LES scale of the failure group were higher than those of the successful group, and the differences were statistically significant (P < 0.05). The comparison of L score, P score and E score between the two groups of EPQ were not statistically significant (P > 0.05); while the N score of the failure group was higher than that of the successful group, and the differences were statistically significant (P < 0.05). Negative life events, total life events, and N scores were the influencing factors for the fertilization success of artificial insemination patients (OR = 1.297, 1.369, 2.453, P < 0.05). Conclusion Patients with artificial insemination of husband’s sperm generally have different degrees of psychological stress and bad personality characteristics, which may have a certain negative impact on the success rate of fertilization of patients with husband’s artificial insemination. Clinically, the psychological stress status and personality characteristics of patients with artificial insemination of husband’s sperm should be evaluated as soon as possible. Propose reasonable interventions to help patients improve their psychological stress and personality characteristics, which is of great significance for improving the fertilization success rate of artificial insemination patients.

Objective To understand the incidence of postoperative perineal swelling in children with developmental dislocation of the hip, to analyze the causes of the swelling, and to provide a certain clinical basis for active prevention and early treatment. Methods From January 2015 to January 2020, 456 children with developmental dislocation of the hip in the department of pediatric surgery of the First Medical Center of PLA General Hospital were selected for observation, and relevant data were recorded according to different ages and different genders. Three intervention methods of magnesium sulfate wet compress, Iodophor wet compress and ice compress were adopted for children with severe perineal swelling, while a control group was established. The relevant data of the magnesium sulfate wet compress group (13 cases), the iodophor wet compress group (13 cases), the ice compress group (13 cases) and the control group (13 cases) were recorded for analysis and summary. Results The incidence of swelling in children >4-7 years old and >7-14 years old were lower than that in the children 1.5-4 years old, and the differences were statistically significant (P < 0.05). The time of perineal swelling in the magnesium sulfate wet compress group, iodophor wet compress group, and ice compress group was shorter than that of the control group, while the pain score was lower than that of the control group, and the differences were statistically significant (P < 0.05). The prevalence of developmental dislocation of the hip in female children was higher than that in male children, and the difference was statistically significant (P < 0.05). The prevalence of developmental dislocation of the hip in female children was higher than that in male children, and the difference was statistically significant (P < 0.05). Conclusion The incidence of postoperative swelling is different in children with developmental dislocation of the hip with different genders and different ages. All three intervention methods can reduce the degree of perineal swelling in children after surgery, and reduce the pain of children, provide scientific basis for clinical nursing work.

Influence of hypertension on ascending aorta, aortic root and the formation of coronary artery plaques

Objective To investigate the effect of hypertension on ascending aorta, aortic root and the formation of coronary artery plaques. Methods Clinical data of 200 patients who received CTA examination due to suspected coronary heart disease in Nantong Hospital of Traditional Chinese Medicine, Jiangsu Province from May 2014 to May 2019 were retrospectively analyzed. They were divided into hypertensive group (150 cases) and non-hypertensive group (50 cases) according to whether they had hypertension or not. All patients underwent coronary artery CTA examination. The aortic root and internal diameter of ascending aorta, the incidence of coronary plaques, the stenosis degree of the involved segments, the condition and incidence of coronary artery disease were observed and compared in the two groups. Results Internal diameter of ascending aorta in hypertensive group was larger than that in non-hypertensive group (P < 0.05). There were no significant differences in the long diameter, short diameter of middle aortic valve ring, the internal diameter at sinus canal junction and the maximum internal diameter of Warwick sinus between the two groups (P > 0.05). The incidence of coronary plaques in the hypertension group was higher than that in the non-hypertension group, and the difference was statistically significant (P < 0.05). There was no significant difference in the degree of coronary artery stenosis between the two groups (P < 0.05). There was statistical significance in the comparison of the condition and incidence of coronary artery disease between the two groups (all P < 0.05). Conclusion Hypertension can cause the internal diameter of ascending aorta to dilate and promote the formation of coronary plaque.

Effect of recombinant human brain natriuretic peptide combined with Wenxin Granules in the treatment of end-stage heart failure and its effect on hemodynamics and vascular endothelial function

Objective To investigate the effect of recombinant human brain natriuretic peptide combined with Wenxin Granules in the treatment of end-stage heart failure and its effect on hemodynamics and vascular endothelial function. Methods A total of 114 patients with end-stage heart failure admitted to Dalian Municipal Central Hospital Affiliated of Dalian Medical University from January to December 2019 were selected and divided into control group and observation group according to random number table method, with 57 cases in each group. The control group was treated with recombinant human brain natriuretic peptide, and the observation group was treated with recombinant human brain natriuretic peptide combined with Wenxin Granules. Both groups were given continuous treatment for four weeks. Clinical efficacy was evaluated after treatment. Left ventricular end-systolic volume (LVESV), left ventricular end-diastolic volume (LVEDV), left ventricular ejection fraction (LVEF), cardiac output (CO), cardiac index (CI), diastolic function index (O/C), serum B-type natriuretic peptide (BNP) and endothelin-1 (ET-1) levels were compared between the two groups before and after treatment. The incidence of adverse reactions in the treatment process of the two groups was observed. Results The clinical total effective rate of observation group was significantly higher than that of control group (P < 0.05). After treatment, the LVESV, LVEDV, O/C, serum BNP and ET-1 levels in both groups were significantly lower than those before treatment (P < 0.05), and LVEF, CO and CI were significantly higher than those before treatment (P < 0.05). Meanwhile, the LVESV, LVEDV, O/C, serum BNP and ET-1 levels in the observation group were significantly lower than those in the control group (P < 0.05), and LVEF, CO and CI were significantly higher than those in the control group (P < 0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P > 0.05). Conclusion Recombinant human brain natriuretic peptide combined with Wenxin Granules has a significant effect in the treatment of end-stage heart failure, which can significantly improve cardiac function and vascular endothelial function, stabilize hemodynamic status, and has good safety.

Clinical effect of PEEP mechanical ventilation in the treatment of acute left heart failure combined with cardiogenic shock and its effect on hemodynamics and blood gas status

Objective To investigate the clinical effect of positive end-expiratory pressure (PEEP) mechanical ventilation in the treatment of acute left heart failure combined with cardiogenic shock and its effect on hemodynamics and blood gas status. Methods A total of 80 patients with acute left heart failure combined with cardiogenic shock admitted to the Anhui Wanbei Coal and Power Group General Hospital from May 2018 to August 2020 were selected and divided into control group and observation group according to random number table method, with 40 cases in each group. The control group was given conventional basic treatment, and the observation group was given PEEP mechanical ventilation treatment. After one week of treatment, the clinical efficacy, hemodynamics (heart rate [HR], systolic blood pressure [SBP], dopamine) and blood gas status (artenal partial pressure of carbon dioxide [PaCO2], partial pressure of arterial oxygen (PaO2), PaO2/fraction of inspiration O2[FiO2] and pH value) before and after treatment were compared and analyzed between the two groups after one week of treatment. Results The total effective rate of the observation group was higher than that of the control group, and the difference was statistically significant (P < 0.05). After treatment, the levels of HR and SBP in the observation group were higher than those in the control group, while the level of dopamine in the observation group was lower than that in the control group, and the differences were statistically significant (all P < 0.05). After treatment, the level of PaCO2 in the observation group was lower than that in the control group, and the levels of PaO2, PaO2/FiO2 and pH value in the observation group were higher than those in the control group, and the differences were statistically significant (all P < 0.05). Conclusion PEEP mechanical ventilation in the treatment of acute left heart failure combined with cardiogenic shock has a significant clinical effect, can promote the improvement of hemodynamics and blood gas status of patients, relieve clinical symptoms, and is worthy of clinical application.

Study on risk factors of cognitive disorder in patients with mild acute ischemic stroke

Objective To explore risk factors of cognitive disorder in patients with mild acute ischemic stroke. Methods A total of 84 patients with mild acute ischemic stroke admitted to the Department of Encephalopathy, Dongfang Hospital, Beijing University of Chinese Medicine from January 2015 to September 2018 were selected as subjects. Their general clinical data and biochemical indicators were collected. On the 10th to 14th day after stroke, they were evaluated by the Montreal cognitive assessment (MOCA) scale. According to the evaluation results, the patients were divided into the non-cognitive disorder group (30 cases) (MOCA scores≥26 points) and the cognitive disorder group (54 cases) (MOCA scores<26 points). The two groups were statistically analyzed and discussed. Results The age and serum homocysteine (Hcy) level of the cognitive disorder group were higher than those of the non-cognitive disorder group, and the standard of culture was lower than that of the non-cognitive disorder group, the differences were statistically significant (all P < 0.05). There were no significant differences between the two groups in gender, previous history, smoking, alcohol consumption, white matter demyelination, infarct location and number, total cholesterol, low density lipoprotein, high density lipoprotein, triglyceride, fasting blood glucose, etc. (P > 0.05). Multivariate logistic regression analysis showed that increased age (OR = 1.071, 95%CI [1.010-1.137], P = 0.022) and serum Hcy level (OR = 1.102, 95%CI [1.002-1.213], P = 0.046) were the independent risk factors for cognitive disorder in patients with mild acute ischemic stroke (P < 0.05). While the highe standard of culture (OR = 0.487, 95%CI [0.268-0.884], P = 0.018) was its independent protective factor (P < 0.05). Conclusion Advanced age, low standavd of culture level and elevated serum Hcy level are closely related to cognitive disorder in patients with mild acute ischemic stroke. As a modifiable physical and chemical factor, monitoring and controlling serum Hcy level in the early acute phase may be helpful to prevent the occurrence of cognitive disorder after stroke.

Effect of mechanical thrombus removal combined with rt-PA intravenous thrombolysis on nerve function and the levels of hs-CRP and Lp-PLA2 in massive cerebral infarction

Objective To explore the effect of mechanical thrombus removal combined with recombinant tissue-type plasminogen activator (rt-PA) intravenous thrombolysis on nerve function and the levels of high-sensitivity C-reactive protein (hs-CRP) and lipoprotein-related phospholipase A2 (Lp-PLA2) in massive cerebral infarction. Methods The clinical data of 80 patients with massive cerebral infarction admitted to Lu’an People’s Hospital, Anhui Province from January 2018 to July 2020 were retrospectively analyzed, they were divided into observation group and control group according to different treatment methods, with 40 cases in each group. The control group was given rt-PA intravenous thrombolytic therapy, and the observation group was combined with mechanical thrombectomy on the basis of the control group. After three months of treatment, the clinical efficacy, the scores of National Institutes of Health stroke scale (NIHSS), modified Rankin scale (mRS), activity of daily living (ADL), levels of hs-CRP and Lp-PLA2 in the two groups were compared. And the adverse reactions of the two groups were compared. Results After treatment, the total effective rate of the observation group was higher than that of the control group, and the difference was statistically significant (P < 0.05). After treatment, the NIHSS and mRS scores, the levels of hs-CRP and Lp-PLA2 of the two groups were lower than those before the treatment, and the ADL score was higher than that before the treatment, and the differences were statistically significant (all P < 0.05). NIHSS and mRS scores, the levels of hs-CRP and Lp-PLA2 of the observation group were lower than those in the control group, and the ADL score was higher than that in the control group, the differences were statistically significant (P < 0.05). The incidence of adverse reactions in observation group was lower than that in control group, and the difference was statistically significant (P < 0.05). Conclusion The mechanical thrombectomy combined with rt-PA intravenous thrombolysis has a significant clinical effect in the treatment of massive cerebral infarction.

Expression levels of ubiquitin carboxy-terminal hydrolase-L1 and neuron-specific enolase in peripheral blood of children with febrile convulsion and their clinical significance

Objective To investigate the expression levels of ubiquitin carboxy-terminal hydrolase-L1 (UCH-L1) and neuron-specific enolase (NSE) in peripheral blood of children with febrile convulsion. Methods Retrospective analysis was performed on 97 children with febrile convulsion admitted to the Second People’s Hospital of Wuhu, Anhui Province (“our hospital” for short) from January 2017 to June 2019 as case group. They were divided into simple febrile convulsion group (simple group, 55 cases) and complex febrile convulsion group (complex group, 42 cases) according to disease type; according to the prognosis, the patients were divided into good prognosis group (76 cases) and poor prognosis group (21 cases). A total of 60 children with high fever diagnosed as acute upper respiratory tract infection were selected as control group. Clinical data of each group was compared, the correlation between serum UCH-L1 and NSE levels in children with febrile convulsion and clinical data was analyzed, and the diagnostic value of serum UCH-L1, NSE and adverse prognosis in febrile convulsion were evaluated. Results The levels of blood glucose, lactic acid, UCH-L1 and NSE in case group were higher than those in control group, and the level of hemoglobin in case group was lower than that in control group (all P < 0.01). The levels of blood glucose, lactic acid, UCH-L1 and NSE in complex group were higher than those in simple group, and the level of hemoglobin in complex group was lower than that in simple group (all P < 0.01). The levels of blood glucose, lactic acid, UCH-L1 and NSE in poor prognosis group were higher than those in good prognosis group, and the level of hemoglobin was lower than that in the good prognosis group (all P < 0.01). The levels of UCH-L1 and NSE in children with febrile convulsion were positively correlated with frequency of seizures, duration of seizures, blood glucose and lactic acid (r > 0, P < 0.05), and negatively correlated with hemoglobin (r < 0, P < 0.05). The cut-off value of UCH-L1 in the diagnosis of febrile convulsion was 2.23 ng/mL, area under the curve (AUC) was 0.878 (95%CI: 0.839-0.917), the sensitivity and specificity were 0.83 and 0.87, respectively. The cut-off value of NSE in the diagnosis of febrile convulsion was 9.85 ng/mL, AUC was 0.838 (95%CI: 0.792-0.885), the sensitivity and specificity were 0.77 and 0.83, respectively. The AUC of UCH-L1 combined with NSE in the diagnosis of febrile convection was 0.915 (95%CI: 0.886-0.944), and the sensitivity and specificity were 0.86 and 0.92, respectively. The cut-off value of UCH-L1 for predicting adverse prognosis in children with febrile convulsion was 3.65 ng/mL, AUC was 0.818 (95%CI: 0.768-0.853), the sensitivity and specificity were 0.77 and 0.86, respectively. The cut-off value of NSE for predicting adverse prognosis in children with febrile convulsion was 18.79 ng/mL, AUC was 0.798 (95%CI: 0.760-0.834), the sensitivity and specificity were 0.75 and 0.82, respectively. The AUC of UCH-L1 combined with NSE for predicting poor prognosis in children with febrile convulsion was 0.931 (95%CI: 0.889-0.964), and the sensitivity and specificity were 0.83 and 0.89, respectively. Conclusion The up-regulated expression of UCH-L1 and NSE in serum is involved in the pathogenesis of children with febrile convulsion, and is closely related to the degree of brain injury. Early combined detection can be used as an important indicator for diagnosis of febrile convulsion and prediction of prognosis.

Efficacy and prognosis of HEOS system in the treatment of endometrial polyps

Objective To investigate the clinical efficacy and prognosis of double hysteroscopic surgery system (HEOS system) in the treatment of endometrial polyps. Methods A total of 35 patients with endometrial polyps treated by HEOS system in the First Affiliated Hospital of University of Science and Technology of China (“our hospital” for short) from January 2018 to June 2020 were selected as the observation group, and 35 patients with endometrial polyps who received traditional hysteroscopic electrosurgical system in our hospital during the same period were selected as the control group. The operative time, biochemical indexes before surgery and one day after surgery (after surgery), endometrial recovery and complications on two months after surgery were compared between the two groups. Results The operation time of the observation group was shorter than that of the control group (P < 0.01). There were no statistically significant differences in biochemical indexes between the two groups before and after surgery (P > 0.05). There were no significant differences in menstrual recovery time, vaginal fluid or bleeding time between the two groups (P > 0.05); after 2 months of follow-up, the endometrial thickness of the observation group was greater than that of the control group (P < 0.05). After 2 months of follow-up, no complications such as cervical adhesion occurred in all the patients. Conclusion The clinical effect of HEOS system in the treatment of endometrial polyps is significant, which can reduce the operation time, high safety, and can protect the endometrium of women of childbearing age, which is worthy of clinical application.

Comparison of short-term efficacy between Albumin-Bound Paclitaxel and Docetaxel combined with Cisplatin in the treatment of locally middle and advanced nasopharyngeal carcinoma

Objective To compare the short-term efficacy of Albumin-Bound Paclitaxel combined with Cisplatin (AC) and Docetaxel combined with Cisplatin (DC) in the treatment of locally middle and advanced nasopharyngeal carcinoma. Methods From January 2016 to December 2019, the clinical data of stage Ⅲ-Ⅳa nasopharyngeal carcinoma patients who received AC or DC as induction chemotherapy from Sun Yat-sen University Cancer Center were collected retrospectively. After propensity score matching, there were 54 cases in each group. The short-term efficacy after induction chemotherapy and 3 months after completion of radio-chemotherapy were compared between the two groups, the disease progression and the incidence of adverse reaction in the two groups was observed. Results After induction chemotherapy and 3 months after completion of radio-chemotherapy, there were no significant differences in the total effective rate between two groups (P > 0.05). There was no significant difference in disease progression rate between the two groups (P > 0.05). There was no significant differences in the toxicity and side effects of leukopenia, neutropenia, anemia and thrombocytopenia between the two groups (P > 0.05). Conclusion AC is well tolerated and effective in middle and advanced nasopharyngeal carcinoma, and the comparison of efficacy with DC still needs longer follow-up to be further confirmed.

Expression and clinical significance of miR-155 and HER-3 in breast cancer

Objective To investigate the expression of microRNA-155 (miR-155) and human epidermal growth factor receptor-3 (HER-3) in breast cancer tissues and to analyze their relationship with clinical data. Methods A total of 87 cases of breast cancer who treated in Benxi Central Hospital of Liaoning Province from March 2016 to August 2019 were selected as study objects. Quantitative real-time PCR was used to detect the expression of miR-155 in breast cancer tissues and adjacent tissues. Immunohistochemistry was used to detect the expression of HER-3. The expression of miR-155 and HER-3 and their relationship with clinical data were analyzed, and the correlation between miR-155 and HER-3 was analyzed. Results The relative expression levels of miR-155 and HER-3 in breast cancer tissues were higher than those in adjacent tissues, and the difference were highly statistically significant (all P < 0.01). The expression of miR-155 and HER-3 in breast cancer tissues with different degree of tumor cell differentiation, TNM stage and lymph node metastasis had statistical significance (P < 0.05). There was a positive correlation between miR-155 and HER-3 expression in breast cancer tissues (r = 0.576, P < 0.05). Conclusion The expression levels of miR-155 and HER-3 in breast cancer tissues are elevated, which are related to tumor TNM stage, differentiation degree and lymphatic metastasis, and are expected to become a new biomarker to guide the diagnosis and treatment of breast cancer.

Effect of PD-1 inhibitor combined with FOLFOX4 chemotherapy regimens in the treatment of advanced gastric cancer

Objective To investigate the efficacy of PD-1 inhibitor combined with Oxaliplatin and 5-fluorouracil (FOLFOX4) chemotherapy regimens in the treatment of advanced gastric cancer. Methods A total of 80 patients with advanced gastric cancer who admitted to the 960th Hospital of the Joint Logistic Support Force of the Chinese People’s Liberation Army from February 2016 to May 2019 were selected as study objects. They were divided into control group and study group, with 40 cases in each group. Control group was treated with FOLFOX4 chemotherapy regimens, and study group was treated with PD-1 inhibitor on the basis of control group. The curative effect, immune function, serum oncology indexes and adverse reactions were compared between two groups. Results The total effective rate of study group was higher than that of control group (P < 0.05). After treatment, carcinoembryonic antigen, carbohydrate antigen (CA) 125, and CA199 in two groups were lower than those before treatment, and study group was lower than control group (P < 0.05). After treatment, the levels of CD3+, CD4+, and CD4+/CD8+ in both groups were lower than before treatment, and CD8+ was higher than before treatment. The levels of CD3+, CD4+, and CD4+/CD8+ in study group were higher than those in control group, and CD8+ was lower than those in control group (P < 0.05). There was no significant difference in the incidence of adverse reaction between two groups (P > 0.05). Conclusion The combination of PD-1 inhibitor and FOLFOX4 chemotherapy regimens in the treatment of advanced gastric cancer have a significant effect, which can prevent tumor proliferation, reduce immunosuppression, and are safe and effective.

Diagnostic value of serum miR-127-3p expression level in primary liver cancer

Objective To study the expression level of serum microRNA-127-3P (miR-127-3p) in patients with primary liver cancer and its diagnostic value in patients with primary liver cancer. Methods A total of 93 patients with primary liver cancer who treated in Bazhong Central Hospital of Sichuan Province (“our hospital” for short) from June 2018 to May 2019 as primary liver cancer group. In addition, 60 healthy volunteers who underwent physical examination in our hospital during the same period were selected as control group. Real-time quantitative PCR was used to detect the relative expression level of serum miR-127-3p, and its value in the diagnosis of primary liver cancer was analyzed. Results The relative expression level of serum miR-127-3p in primary liver cancer group was lower than that of control group, and the difference was highly statistically significant (P < 0.01). The relative expression levels of serum miR-127-3p in patients with different cell differentiation, TNM stage and lymph node metastasis showed high statistical significance (all P < 0.01). The cut-off value of serum miR-127-3p relative expression in the diagnosis of primary liver cancer was 0.420, AUC was 0.793 (95%CI: 0.758-0.816, P < 0.01), the sensitivity and specificity were 0.860 and 0.850, respectively. Conclusion The expression of serum miR-127-3p in patients with primary liver cancer is low. There are differences in TNM stage, cell differentiation degree and lymph node metastasis, which can be used as a molecular marker for the diagnosis of primary liver cancer.

Analysis of fear of cancer recurrence and social status support in patients with non-small cell lung cancer

Objective To observe fear of cancer recurrence (FCR) and social support status in patients with non-small cell lung cancer (NSCLC), and analyze their relationship. Methods A prospective study of 81 NSCLC patients admitted to Qingdao Municipal Hospital of Shandong Province (hereinafter referred to as “our hospital”) from January 2017 to June 2019 was conducted. According to the fear of cancer recurence inventory (FCRI), the patients’ FCR status on admission was assessed and grouped. Patients with FCRI score ≤84 points were enrolled as the mild group (46 cases), and patients with FCRI score >84 points were enrolled as the moderate to severe group (35 cases). The social support status of patients was assessed according to the social support rating scale (SSRS). The baseline data of patients were recorded by self-made questionnaire of our hospital, and the social support situation was compared with emphasis, and the relationship between social support and FCR in NSCLC patients was analyzed. Results The proportion of clinical stage Ⅲ-Ⅳ and poor mental status of patients in the moderate to severe group was higher than that of the mild group, and the SSRS score was lower than that of the mild group (P < 0.05). The results of multivariate regression model showed that poor mental state (OR = 3.130 [95%CI: 1.229-7.972], P = 0.017) and low SSRS score (OR = 0.331 [95%CI: 0.150-0.731], P = 0.006) were the influencing factors for aggravation of FCR degree in NSCLC patients. Correlation test results showed that FCR score of NSCLC patients was negatively correlated with social support (r = -0.804, P < 0.05). Conclusion NSCLC patients generally have severe FCR, which may be affected by the level of social support of patients. Therefore, clinical attention should be paid to the social support intervention of NSCLC patients, which may be of positive significance to reduce the degree of FCR in patients.

Comparison of the application of FOCUS ultrasonic knife and straight bipolar electrocoagulation forceps in thyroid tumor whole process fine anatomy

Objective To compare the application effect of FOCUS ultrasonic knife and straight bipolar electrocoagulation forceps in thyroid tumor whole process fine anatomy. Methods Retrospective analysis was performed on the diagnosis and treatment of 100 patients who received thyroid surgery in Bozhou People’s Hospital, Anhui Province from June 2016 to January 2020. All patients were treated with the method of whole process fine anatomy and they were divided into ultrasonic knife group and bipolar electrocoagulation group according to the different use of intraoperative instruments, with 50 cases in each group. Perioperative conditions, changes in serum parathyroid hormone (PTH), Ca2+ levels and incidence of postoperative complications were compared between the two groups. Results The incision length, intraoperative blood loss and postoperative drainage volume of the operative area in the bipolar electrocoagulation group were lower than those in the ultrasonic knife group, and the duration of operation and hospital stay were shorter than those in the ultrasonic knife group, and the differences were statistically significant (all P < 0.05). Overall analysis showed that there were statistically significant differences in serum PTH and Ca2+ levels between groups, time point comparison and interaction (all P < 0.05). Intra-group comparison: 1, 3 and 7 days after surgery, serum PTH and Ca2+ levels were lower than those before surgery in both groups, and the differences were statistically significant (all P < 0.05).Comparison between groups: at 1, 3 and 7 days after surgery, serum PTH and Ca2+ levels in bipolar electrocoagulation group were higher than those in ultrasonic knife group, and the differences were statistically significant (all P < 0.05). The total incidence of postoperative complications in the bipolar electrocoagulation group was lower than that in the ultrasonic knife group, and the difference was statistically significant (P < 0.05). Conclusion Straight bipolar electrocoagulation forceps is more effective in thyroid tumor whole process fine anatomy, which is worthy of application and promotion.

Effect of Compound Chaijin Jieyu Tablets in the treatment of depression and its influence on serum inflammatory factors and neurotransmitters

Objective To explore the effect of Compound Chaijin Jieyu Tablets in treating depression and its influence on serum inflammatory factors and monoamine neurotransmitters. Methods A total of 80 patients with depression admitted to the Department of Neurology, the First Affiliated Hospital of Hu’nan University of Chinese Medicine from May 2018 to December 2019 were selected. All observed cases were divided into observation group and control group according to the random number table method, with 40 cases in each group. The course of treatment was eight weeks. The observation group was treated with self-prepared Compound Chaijin Jieyu Tablets, and the control group was treated with Western medicine Hydrochloride Sertraline Tablets. The clinical efficacy and traditional Chinese medicine (TCM) syndromes efficacy were compared between the two groups after four and eight weeks of treatment. The levels of serum monoamine neurotransmitters (5-hydroxytryptamine, norepinephrine, dopamine) and inflammatory factors (tumor necrosis factor-α, interleukin-6, interleukin-2) were compared between the two groups after eight weeks of treatment. The safety of the two groups was observed. Results During the treatment, two patients in the control group and three patients in the observation group fell off. After four weeks of treatment, there was no significant difference in clinical efficacy and TCM syndrome efficacy between the two groups (P > 0.05). After eight weeks of treatment, there was no statistically significant difference in clinical efficacy between the two groups (P > 0.05), but the total effective rate of TCM syndromes in the observation group was higher than that in the control group, with statistically significant difference (P < 0.05). After eight weeks of treatment, the levels of monoamine neurotransmitters were higher than those before the treatment, and the levels of inflammatory factors were lower than those before the treatment, with statistical significant differences (all P < 0.05). But there was no statistical significance in serum levels of inflammatory factors and neurotransmitters between groups (P > 0.05). The patients’ liver and kidney function, myocardial enzymes, three routine tests, and electrocardiogram examination before and after treatment were not found any obvious abnormal changes. Conclusion The Compound Chaijin Jieyu Tablet has a positive effect on the treatment of depression with liver depression and spleen deficiency, and has high safety, which is suitable for clinical application.

Analysis on risk factors associated with death in patients with pulmonary carbapenem-resistant Klebsiella pneumoniae infection / colonization

Objective To analyze the risk factors associated with death in patients with pulmonary infection / colonization of carbapenem-resistant Klebsiella pneumoniae (CRKP). Methods A total of 190 inpatients with CRKP cultured from respiratory tract specimens in Inner Mongolia People’s Hospital from January 2017 to June 2019 were collected for general data analysis and single (multiple) logistic regression analysis of mortality. Results The results of single logistic regression analysis showed that 15 other factors were correlated with death of patients with pulmonary CRKP infection /colonization, except for complications of hypertension, organ injury ≥ 3 sites on admission and the use of aminoglycoside antibiotics, with statistical significance (all P < 0.05). Multivariate logistic regression analysis showed that age ≥65 years (OR = 3.578, 95%CI [1.028-12.451], P = 0.034), male (OR = 7.030, 95%CI [1.164-42.463], P = 0.045), with diabetes (OR = 12.888, 95%CI [1.967-84.421], P = 0.008), use of mechanical ventilation (OR = 14.967, 95%CI [2.708-82.724], P = 0.002), history of surgical (OR = 28.026, 95%CI[4.964-158.237], P < 0.001) and use of penicillin (OR = 80.626, 95%CI [8.263-786.737], P < 0.001), cephalosporins (OR = 25.757, 95%CI [1.149-577.407], P = 0.041) were independent risk factors for death in patients with pulmonary infection/colonization of CRKP. Conclusion According to the independent risk factor analysis of CRKP infection, some invasive procedures should be avoided as far as possible in clinical practice, antimicrobial agents should be used rationally, management of high-risk population should be strengthened, and the occurrence of CRKP infection should be reduced and prevented.

Characteristics and risk factors of recurrence of lower extremity deep venous thrombosis in patients with fracture after discharge

Objective To investigate characteristics and risk factors of recurrence of lower extremity deep venous thrombosis (DVT) in patients with fracture after discharge. Methods Clinical data of 90 patients with primary lower extremity DVT admitted to Department of Orthopaedic Trauma in Honghui Hospital Affiliated to Xi’an Jiaotong University from April 2014 to August 2019 were retrospectively analyzed. According to the results of venous ultrasound, they were divided into recurrent thrombosis group (44 cases) and non recurrent thrombosis group (46 cases). Univariate analysis was used to screen the risk factors of recurrent DVT, and multivariate logistic regression analysis was used to determine the independent risk factors. Results Forty-four cases of DVT recurred, the incidence was 48.89%. There were significant differences in age, gender, primary thrombus side, check hemoglobin and check D-dimer levels between recurrent thrombosis group and non recurrent thrombosis group (P < 0.05). Multivariate logistic regression analysis showed that age (OR = 1.082, 95%CI: 1.035-1.131), primary bilateral thrombosis (OR = 5.946, 95%CI: 1.034-34.181) and high check D-dimer level (OR = 2.059, 95%CI: 1.209-3.508) were independent risk factors for recurrent DVT (P < 0.05). Conclusion The recurrence rate of DVT with fracture patients after discharge is higher, but the distal thrombus is predominant. Elderly, primary bilateral thrombosis and high check D-dimer should be alert of recurrence of DVT after discharge.

Clinical effect of transanal contour-curved stapler in the treatment of rectocele

Objective To study clinical effect of partial resection of rectum anterior wall of transanal contour-curved stapler (CCS) in the treatment of rectocele. Methods From January 2018 to January 2020, 40 adult females with rectocele in Shenyang Anorectal Hospital received partial resection of rectum anterior wall of transanal CCS were selected as research objects. The status of postoperative outlet obstruction and the effect of operation were evaluated through obstruction defecation syndrome (ODS) score, efficacy evaluation, imaging improvement and satisfaction of patient survey. Results Forty cases were operated successfully, the operation time was 25 to 40 minutes, the average operation time was (32±4) minutes. The blood loss was 10 to 30 milliliters, and the average blood loss was (20±5) milliliters. The signs of rectocele decreased significantly after operation. There were no complications such as massive hemorrhage, rectovaginal fistula, rectal stricture, anal stricture, perianal abscess and perioperative death. The postoperative ODS score was significantly lower than that before operation (P < 0.05). The depth of rectocele after operation was significantly shorter than that before operation (P < 0.05). Curative effect evaluation: five cases were cured, 15 cases were markedly effective, 20 cases were effective, no one was invalid, the total effective rate was 100%. Postoperative patient satisfaction survey: 24 cases were satisfied, 16 cases were basically satisfied, and no one was dissatisfied. Conclusion Partial resection of rectum anterior wall of transanal CCS is an effective method for the treatment of ODS with rectocele, which is characterized as safe, simple, short operation time and less postoperative complications etc.

Study on pulmonary infection factors in patients with severe closed craniocerebral injury after craniotomy

Objective To investigate the influence factors of pulmonary infection in patients with severe closed craniocerebral injury after craniotomy. Methods Data of 160 patients with severe closed craniocerebral injury after craniotomy in the 904th Hospital of Joint Service Support Force of Chinese People’s Liberation Army from January 2017 to December 2018 were retrospectively studied. Multivariate logistic analysis was used to analyze the basic data of patients with pulmonary infection. Results There were 60 cases of pulmonary infection in 160 patients, and the incidence rate of pulmonary infection was 37.5%. There were significant differences In incidence rate of pulmonary infection in different age, smoking history, course of diabetes, GCS score before operation, blood transfusion during operation and secondary operation, tracheotomy or not (P < 0.05). Multivariate logistic regression analysis showed that age (OR = 9.646, 95%CI: 2.381-39.076), preoperative hyperglycemia (OR = 7.435, 95%CI: 1.834-30.134), intraoperative blood transfusion (OR = 3.052, 95%CI: 1.057-95.622), secondary operation (OR = 6.435, 95%CI: 1.414-29.287), tracheotomy (OR = 11.769, 95%CI: 3.642-43.005), preoperative GCS score (OR = 4.199, 95%CI: 2.322-99.613), smoking (OR = 8.511, 95%CI: 2.012-35.787) were the influencing factors of pulmonary infection in patients with severe closed craniocerebral injury after craniotomy (P < 0.05). Conclusion There are many factors of pulmonary infection in patients with severe closed craniocerebral injury after craniotomy. Comprehensive prevention and control measures should be taken to prevent infection.

Anti adhesion effects of absorbable medical membrane in stage two cholecystectomy

Objective To observe the efficacy of Dikang absorbable medical membrane in preventing adhesion during stage two cholecystectomy. Methods One hundred and twenty patients with cholecystolithiasis and cholecystitis admitted to People’s Hospital of China Medical University from January 2017 to January 2019 were selected and divided into control group and observation group according to random number table method, 60 cases in each group. Control group was treated with laparoscopic cholecystolithotomy and cholecystostomy; on the basis of control group, observation group covered the gallbladder surface and gallbladder triangle with Dikang absorbable medical membrane. The second stage operation situation, the second stage operation adhesion, the first stage postoperative follow-up one, two, three months operation area pain of two groups were compared. Results The operation time of observation group was shorter than that of control group, the intraoperative blood loss of observation group was less than that of control group, and the conversion rate of observation group was lower than that of control group, the differences were statistically significant (P < 0.05). The degree of adhesion in observation group was significantly lighter than that in control group, the difference was statistically significant (P < 0.05). The incidence rate of postoperative pain and visual simulation score in observation group were lower than those in control group at one, two, three months after operation, and the differences were statistically significant (P < 0.05). Conclusion Dikang absorbable medical membrane can effectively prevent local tissue adhesion after surgery.

Gallbladder in traditional Chinese medicine has the function of storing and dispersing gallbladder essence as well as gallbladder qi. When gallbladder loses the function of dominating decision, different emotional manifestations and even diseases will be shown, such as depression, anxiety, and so on. Professor Jia Liqun thinks that occurrence of malignant tumors is closely related to qi stagnation caused by disfunction of gallbladder. Patients usually have clinical manifestation of emotional disorder, bitterness in the mouth and distending rib pain. According to different types and period of tumor, Professor Jia Liqun thinks that treatment should be established in the view of the “gallbladder dominating decision” theory and syndrome differentiation, such as harmonizing shaoyang to treat thyroid disease, thyroid cancer and breast cancer, soothing liver-gallbladder to treat gallbladder cancer and pancreatic cancer, nourishing spleen and gallbladder to treat advanced stage caner including esophageal cancer and colorectal cancer. Aromatic drugs are the general name of traditional Chinese medicine with aromatic smell, which is closely related to spleen and stomach diseases. This paper expounds the historical origin, characteristics and clinical application of aromatic drugs, the reasons for the extensive use of aromatic drugs in spleen and stomach diseases and the general situation of their application in spleen and stomach diseases, in order to promote the development, research and application of aromatic drugs. Aromatic drugs have the characteristics of dispersing and resolving exterior, eliminating dampness, promoting qi to relieve depression, warming meridian to relieve pain, etc. The main functions of aromatic drugs in the treatment of spleen and stomach diseases are to remove dampness and regulate qi. Examples are given of the commonly used aromatic drug combinations, such as Agastache rugosa, Eupatorium fortunei, Amomum villosum for resolving dampness with aromatics, Bupleurum chinense, Citrus medica, Rosa rugosa for soothing liver and spleen, Amomum villosum, Alpinia katsumadai Hayata, Syzygiumaromaticum for promoting qi and stopping vomiting, Cinnamomum cassia Presl, Alpinia officinarum Hance, Lindera aggregata for warming spleen and yang, etc. This paper summarizes the modern pharmacological mechanism of aromatic drugs.

Effect of Chongcao Bushen Capsules in the treatment of diabetic nephropathy patients with deficiency of both qi and yin type

Objective To observe the effect of Chongcao Bushen Capsules on serum levels of transforming growth factor-β1 (TGF-β1), connective tissue growth factor (CTGF), monocyte chemoattractant protein-1 (MCP-1), tumor necrosis factor-α (TNF-α), reactive oxygen species (ROS) and superoxide dismutase (SOD) in patients with diabetic nephropathy of qi and yin deficiency type. Methods Sixty patients with diabetic nephropathy in Department of Endocrinology and Nephrology of Shijiazhuang Hospital of Traditional Chinese Medicine from March 2018 to June 2019 were selected and divided into treatment group and control group according to random number table method, with 30 cases in each group. Treatment group was given Chongcao Bushen Capsules and conventional treatment, and control group was given Chongcao Bushen Capsules Simulant and conventional treatment. The course of treatment was 6 weeks. The serum levels of TGF-β1, CTGF, MCP-1, TNF-α, ROS and SOD were compared between two groups. Results After treatment, the levels of TGF-β1, CTGF, MCP-1 and TNF-α in two groups were lower than those before treatment, and those in treatment group were lower than those in control group, the differences were statistically significant (P < 0.05). After treatment, the level of ROS in two groups was lower than that before treatment, and the level of SOD was higher than that before treatment, and the level of ROS in treatment group was lower than that in control group, and the level of SOD was higher than that in control group, the differences were statistically significant (P < 0.05). Conclusion Chongcao Bushen Capsules can slow down the process of renal fibrosis in diabetic nephropathy patients, which may be related to regulating cytokines, inhibiting inflammatory response and oxidative stress. Chongcao Bushen Capsules is an effective drug for the treatment of diabetic nephropathy and the control of its progress.

Effects of the subject and intervention mode of the main body and its integration, emotional support and quality of life in patients with colorectal cancer after surgery

Objective To explore the application of the subject and intervention mode of the main body and its integration in patients with colorectal cancer after surgery, and to observe its effects on disease cognition, emotional support and quality of life. Methods Eighty cases of colorectal cancer admitted to Beijing Shijitan Hospital, Capital Medical University from March 2018 to March 2020 were selected as the research subjects, they were divided into control group and observation group by random number table method, with 40 cases in each group. The control group was given routine postoperative intervention, and the observation group was given the subject and intervention mode of the main body and its integration on the basis of the control group, the cognition of disease, emotional sustenance and quality of life were compared between the two groups. Results The qualification rate of disease cognition of each item in the observation group was higher than that in the control group, with statistical significance (P < 0.05). The proportion of patients with hope after intervention was higher in the two groups than before intervention, the proportion of patients with personal belief in the observation group was higher than before intervention, and the proportion of patients with hope, personal belief in the observation group was higher than that in the control group after intervention, the proportion of patients with no emotional suppont was lower than that in the control group, with statistical significances (P < 0.05). After intervention, the physical function, role function, emotional function, cognitive function, social function and fatigue scores in the two groups were higher than those before intervention, and all scores in the observation group were higher than those in the control group, and the differences were statistically significant (P < 0.05). Conclusion The subject and intervention mode of the main body and its integration in patients with colorectal cancer can significantly improve their correct attitude towards the disease, and set up a higher emotional support, so as to obtain a better quality of life.

Application effect of precision nursing based on evidence-based concept in senile sarcopenia patients

Objective To explore the application effect of precision nursing based on evidence-based concept in elderly patients with sarcopenia. Methods From October 2018 to June 2020, a total of 177 elderly patients with sarcopenia who were admitted to the Affiliated Hospital of Nanjing University Medicine School were selected as the research subjects. According to random number table method, they were divided into control group (89 cases) and observation group (88 cases). The control group was given routine sarcopenia nursing intervention, and the observation group was given precision nursing intervention based on evidence-based concept on the basis of the control group, and the intervention time was 12 weeks. The skeletal muscle index (SMI), muscle function grade and short physical performance battery (SPPB), self-rating anxiety scale (SAS), self-rating depression scale (SDS) and intervention satisfaction rate of the two groups were compared before and after intervention. Results After intervention, there was no statistical significance in SMI, muscle function grade and SPPB score of control group compared with before intervention (P > 0.05). SMI, muscle function grade and SPPB score of the observation group were higher than before intervention, and the observation group was higher than the control group, the differences were statistically significant (P < 0.05). SAS and SDS scores in both groups were lower than those before intervention, and observation group were lower than those of control group, the differences were statistically significant (P < 0.05). The total satisfaction rate of intervention in the observation group was higher than that in the control group, with statistical significance (P < 0.05). Conclusion Precision nursing based on the evidence-based concept can significantly improve the psychological status and muscle function attenuation of elderly patients with sarcopenia, and improve the satisfaction rate of intervention, which is worthy of promotion and application.

Effect of Orem self-care model on Self-care ability and quality of life in patients with chronic gastric ulcer

Objective To explore the effect of Orem self-care model on self-care ability and quality of life improvement in patients with chronic gastric ulcer. Methods A total of 174 patients with chronic gastric ulcer admitted to Yangjiang People’s Hospital of Guangdong Province from February 2018 to December 2019 were selected as the research subjects. The patients were divided into control group and observation group by convenience sampling method, with 87 cases in each group. The control group was given traditional conventional nursing guidance, and the observation group was given Orem self-care mode. The self-care ability score and quality of life evaluation were compared between the two groups at admission and discharge. Results The scores of all dimensions of the self-care ability scale at discharge were higher in the two groups than at admission, with highly statistical significance (P < 0.01), the scores of each dimension of the self-care ability scale in the observation group were significantly higher than those in the control group, and the differences were highly statistically significant (P < 0.01). The scores of all dimensions of the quality of life scale in the two groups were higher at discharge than at admission, with statistical significance (P < 0.05 or P < 0.01), and the scores of each dimension of the quality of life scale in the observation group were significantly higher than those in the control group, and the differences were highly statistically significant (P < 0.01). Conclusion Based on the Orem self-care model, not only effectively enhance the self-care ability of patients with gastric ulcer, but also greatly improve their quality of life.

Influence of diversified non-pharmacological nursing intervention on mental state and quality of life of patients with mild to moderate pain tumor

Objective To explore the effect of diversified non-drug nursing intervention on the psychological state and quality of life of patients with mild and moderate pain tumor. Methods A total of 115 patients with mild to moderate pain tumor were selected from Jiangsu Xuzhou Benevolence Hospital and Xuzhou Central Hospital from January 2018 to December 2019. According to the length of hospital stay, 59 patients were divided into the observation group (January to December 2019) and 56 patients in the control group (January to December 2018). The two groups were given anti-tumor drug therapy and routine nursing intervention, and the observation group was combined with multiple non-drug nursing intervention. After two weeks of intervention, the analgesic effect, psychological state, quality of life and satisfaction were compared between the two groups. Results After two weeks of intervention, the numeric rating scales pain (NRS) scale scores in both groups were significantly lower than those before intervention, and the mild pain rates were significantly higher than those before intervention (P < 0.01), the NRS score of the observation group was significantly lower than that of the control group, and the mild pain rate was significantly higher than that of the control group (P < 0.01), SAS and SDS scores were significantly lower than those before intervention (P < 0.01). SAS and SDS scores in the observation group were significantly lower than those in the control group (P < 0.01). The quality of life scores of the two groups were significantly higher than before intervention (P < 0.05 or P < 0.01), the scores of physical function, emotional function, social function and cognitive function of the observation group were significantly higher than those of the control group (P < 0.01). Conclusion Diversified non-drug nursing intervention is helpful to relieve pain, anxiety and depression and improve the quality of life of cancer patients.

Application effect of situational health education in gastrointestinal function exercise for patients undergoing gastrointestinal surgery

Objective To explore the application effect of situational health education in gastrointestinal function exercise for patients undergoing gastrointestinal surgery. Methods A total of 82 patients who planned to undergo elective gastrointestinal surgery in the Affiliated Huai’an No.1 People’s Hospital of Nanjing Medical University from January 2018 to June 2019 were selected as the subjects. By random number table method, 44 cases were divided into experimental group and 38 cases were control group. Both groups were given preoperative and postoperative gastrointestinal function exercise. The control group was treated with routine gastrointestinal function exercise and health education methods such as centralized or individual instruction, and the experimental group adopted the health education method of scenario simulation. The recovery time of gastrointestinal function, length of hospital stay, gastrointestinal complications and patient satisfaction were compared between the two groups. Results The first anal exhaust time, bowel sound recovery time, first defecation time and hospital stay in the experimental group were shorter than those in the control group, and the differences were statistically significant (P < 0.05); the incidence of gastrointestinal complications was lower than that in the control group, and the difference was statistically significant (P < 0.05); the satisfaction of experimental group was higher than that of control group, and the difference was statistically significant (P < 0.05). Conclusion Gastrointestinal function exercise scenario simulation health education is helpful to promote the recovery of gastrointestinal function in patients with gastrointestinal surgery, reduce the incidence of gastrointestinal complications, and improve patient satisfaction.

Evaluation of the effect of multimedia health education in patients undergoing percutaneous lumbar discectomy

Objective To explore the effect of multimedia health education on the postoperative rehabilitation cognition level and early rehabilitation training compliance of patients with percutaneous lumbar discectomy (PLD). Methods A total of 106 PLD patients admitted to Nantong Hospital of Traditional Chinese Medicine of Jiangsu Province from July 2017 to June 2018 were selected and divided into observation group and control group by random number table method, with 53 cases in each group. The control group was given regular health education, while the observation group was given multimedia health education. Follow-up for six months, the postoperative rehabilitation cognition level, early rehabilitation training compliance and lumbar function were compared between the two groups. Results After six months of follow-up, the awareness rate of postoperative rehabilitation knowledge of the two groups were significantly higher than those before education in the same group, while the awareness rate of rehabilitation knowledge of the observation group was significantly higher than that of the control group, and the differences were statistically significant (P < 0.05). The early rehabilitation training compliance rate of the observation group was significantly higher than that of the control group (P < 0.05). After six months of follow-up, the visual analogue scale (VAS) and Oswestry disability index (ODI) of the two groups were significantly lower than those before education in the same group, Japanese orthopaedic association (JOA) score was significantly higher than that before education in the same group, while the VAS and ODI of the observation group were significantly lower than those of the control group, the JOA score was significantly higher than that of the control group (P < 0.05). Conclusion Multimedia health education helps to improve the cognitive level of early rehabilitation training for PLD patients, promotes the development of compliance behaviors for early rehabilitation training postoperative, and improve the patient’s lumbar function.

Effect of traditional Chinese medicine five tone therapy combined with acupoint massage on mood state and sleep quality of patients after radical gastric cancer surgery

Objective To explore the effect of traditional Chinese medicine five tone therapy combined with acupoint massage on mood state and sleep quality of patients after radical gastric cancer surgery. Methods From January to December 2018, a total of 72 patients undergoing chemotherapy after gastric cancer surgery who were admitted to Nanjing of Integrated Traditional Chinese and Western Medicine Hospital Affiliated to Nanjing University of Traditional Chinese Medicine were selected and divided into experimental group and control group by random number table method, with 36 cases in each group. The control group took conventional nursing care, and the experimental group implemented traditional Chinese medicine five tone therapy combined with acupoint massage on this basis. After four weeks of intervention, the self-rating anxiety scale (SAS) and the self-rating depression scale (SDS) were used to evaluate the mood state of the two groups of patients, the Pittsburgh sleep quality index was used to evaluate the sleep quality of the two groups, and the comprehensive quality of life assessment was used to evaluated the quality of life of the two groups. Results After four weeks of intervention, the SAS and SDS scores of the two groups were lower than before the intervention, while the SAS and SDS scores of the experimental group were significantly lower than those of the control group, and the differences were highly statistically significant (P < 0.01). After four weeks of intervention, the scores of all dimensions of sleep quality in the two groups were significantly lower than those before the intervention, and the differences were statistically significant (P < 0.05 or P < 0.01). The experimental group’s sleep delay, night sleep interruption, total sleep time, sleep quality, and daytime impact scores were significantly lower than those of the control group, and the differences were highly statistically significant (P < 0.01). After four weeks of intervention, the scores of all dimensions of the quality of life and the total scores of the two groups were significantly higher than those before the intervention, while the scores of physical function, mental function, material life, and quality of life of the experimental group were significantly higher than those of the control group, and the differences were highly statistically significant (P < 0.01). Conclusion Five tone therapy combined with acupoint massage can help relieve the negative emotions of chemotherapy patients after radical gastric cancer surgery, and improve the quality of sleep and quality of life.

Study on the application value of warning safety signs to patients in department of emergency

Objective To explore the application value of warning safety signs to patients in department of emergency. Methods A total of 194 department of emergency patients in the emergency room of Hai’an People’s Hospital in Jiangsu Province were selected from December 2016 to July 2017, and 101 patients admitted from December 2016 to March 2017 were randomly selected as the control group, another 93 patients admitted from April to July 2017 were randomly selected as the observation group. The patients in the control group were given regular aspiration prevention publicity and education, while the patients in the observation group implemented the aspiration care plan with warning safety signs on the basis of the control group. The incidence of aspiration, the incidence of adverse reactions and the satisfaction of patients and their families were compared between the two groups. Results The incidence of aspiration in the observation group was lower than that in the control group, and the difference was statistically significant (P < 0.05). The incidence of adverse reactions in the observation group was significantly lower than that in the control group, and the difference was statistically significant (P < 0.05). The nursing satisfaction of patients in the observation group was significantly higher than that in the control group, and the difference was statistically significant (P < 0.05). Conclusion Warning safety nursing intervention can effectively reduce the occurrence of aspiration in emergency observation patients, reduce the incidence of adverse reactions, and improve nursing satisfaction.

Bibliometrics analysis on stigma in patients with depression based on Web of Science database

Objective To analyze the research status of relevant literatures on stigma in patients with depression, and to provide references for further research in this field. Methods Based on the Web of Science database, HistCite file management software was used to statistically analyze the literatures of stigma in patients with depression from the inception to November 2018. Results A total of 442 literatures on stigma in patients with depression were retrieved, among which China ranked the fifth in the world in terms of the number of articles published. “stigma” and “depression” were the keywords that appeared most frequently in the literatures. The prolific authors were Prasko and Grambal etc. The main publishing journal was “JOURNAL OF AFFECTIVE DISORDERS”. The main publishing institutions were universities. Conclusion The research on stigma in patients with depression has gradually become a new academic hotspot, and the development of this field in China needs to be further explored.
