MA Wenjun1 SHI Tingting2 BAI Minghui1 CHEN Xiaowei1 YANG Xingxin3▲
Objective To investigate the effect of Rabdosia Rubescens on the expression of lipid metabolism-associated proteins in rats with metabolism-associated fatty liver disease (MAFLD). Methods The rats were fed with high-fat diet to replicate the metabolic-related fatty liver disease model. A total of 36 SD male rats were adaptively fed for one day, and they were divided into normal group, model group, and Rabdosia Rubescens low, medium, and high-dose groups by random number table method, each with six animals. The normal group was fed with ordinary feed, and the other groups were fed with high-fat feed. The low, medium, and high-doses of Rabdosia Rubescens were given 50, 100 mg/(kg·d) and 150 mg/(kg·d) per day, respectively; the normal group and the model group were given a gavage with the same volume of normal saline as the other experimental groups every day. HE staining was used to observe the degree of fatty degeneration of rat MAFLD liver tissue; automatic biochemical analyzer was used to detect triacylglycerol (TG), total cholesterol (TC), high-density lipoprotein cholesterol (HDL-C), and low-density lipoprotein cholesterol (LDL- C), fasting blood glucose (FBG), fasting insulin (FINS); Western blot method was used to detect the expressions of peroxisome proliferator-activated receptor-α (PPAR-α), sterol regulatory element binding protein-1c (SREBP-1c), fatty acid synthase (FAS), acetyl-CoA carboxylase (ACC) protein. Results The liver tissue of rats in the normal group showed clear and complete structure under microscope, the structure of liver lobules was normal, and the hepatocyte cords were neatly arranged. Hepatocyte damage in the model group showed diffuse hepatocyte steatosis with balloon-like hepatocytes, inflammatory cell infiltration in the portal area, and partial punctate hepatocyte necrosis and detrital necrosis. The degree of liver damage in the Rabdosia Rubescens high-dose group was significantly less than that of the model group. The fatty degeneration of hepatocytes was reduced. There was only a few punctate necrosis, a few fat droplets, and no obvious inflammatory cell infiltration in the portal area. The levels of TC, TG, HDL-C, LDL-C, FBG, and FINS of rats in each group were compared, and the differences were statistically significant (P < 0.05). The levels of serum TC, TG, LDL-C, FBG, and FINS in the model group were higher than those in the normal group, while the level of HDL-C was lower than that in the normal group, and the differences were statistically significant (P < 0.05). The content of TG and FINS in the low-dose Rabdosia Rubescens group were lower than those of the model group, and the differences were statistically significant (P < 0.05); the content of TC, TG, LDL-C, FBG, and FINS in the medium-dose Rabdosia Rubescens group were lower than those of the model group, while TG and FBG were lower than those of the low-dose Rabdosia Rubescens group, and the differences were statistically significant (P < 0.05). The content of TC, TG, LDL-C, FBG, and FINS in the high-dose Rabdosia Rubescens group were lower than those of the model group, while the level of HDL-C was higher than that of the model group, and TC, TG, LDL-C, FBG were lower than those of the low-dose Rabdosia Rubescens group, and the differences were statistically significant (P < 0.05). The expressions of PPAR-α and ACC protein in the model group were lower than those of the normal group, while the expressions of SREBP-1c and FAS protein were higher than those of the normal group, and the differences were statistically significant (P < 0.05). The expressions of PPAR-α and ACC protein in the low dose of Rabdosia Rubescens group were higher than those of the model group, while the expression of FAS protein was lower than that of the model group, and the differences were statistically significant (P < 0.05). The expressions of PPAR-α and ACC protein in the medium and high-doses Rabdosia Rubescens group were higher than those of the model group and the low-dose Rabdosia Rubescens group, while the expressions of SREBP-1c and FAS protein were lower than those of the model group and the low-dose Rabdosia Rubescens group, and the differences were statistically significant (P < 0.05). Conclusion Rabdosia Rubescens can regulate lipid metabolism and associated protein expression in rats with MAFLD, which lays a foundation for further study on the mechanism of the occurrence and development of MAFLD.

XU Lingyun1 CHEN Siwei1 LIU Huanyu2 RAN Yan1 CHEN Ningning1 SUN Lin1 CHEN Yu1 LI Yingchao1
The Patients with inflammatory bowel disease (IBD) have significantly higher rates of psychological issues than the general population. Anxiety, depression and chronic abdominal pain are the most notable. The disorder of brain-gut axis is the important mechanism in bi-directional interactions between IBD disease activity and psychological issues. More and more attention has been paid to the diagnostic approaches of psychological conditions and standardized psychological treatments in IBD. Psychotherapy, an important supplement to the traditional treatment of IBD, can not only reduce the symptoms of psychological issues in IBD patients, but also reduce the activity of IBD disease. Moreover it can prevent unnecessary treatment escalation and improve the quality of life of patients. It can also change the disease course. A new “bio-psycho-social” medical model of IBD is emerging. This article reviews the common psychological issues in IBD patients, the interactions between psychological issues and IBD disease activity, and the progress of diagnosis and treatment.

80 Application of 3D model based on CT/MRI in experimental teaching of regional anatomy
YU Penghui LI Qiao JU Xiaojun LIU Yu CAI Shichang
Traditional local anatomy experiment teaching mostly uses cadavers as the main teaching resources, and there are problems such as fewer sources of cadaver specimens, easy damage, and poisonous gas that can damage the health of teachers and students. The application of virtual three-dimensional model in the local anatomy experiment teaching can reduce the resource consumption of cadaver specimens caused by anatomy, shorten the time in the toxic gas environment, and have the advantages of independent design, repeated use, and multi-directional observation, while it is not limited by using time and space. At the same time, it also has certain limitations. Combining the traditional autopsy teaching mode with the virtual three-dimensional model, and learning from each other, can effectively improve the quality of regional anatomy experimental teaching and enhance the level of anatomy.

WANG Zhe1 LYU Jieping2
Objective To analyze the influence of obesity on intra-abdominal pressure (IAP) of patients undergoing retroperitoneal laparoscopic surgery. Methods From October 2018 to July 2019, patients who underwent retroperitoneal partial nephrectomy at the First Hospital of Shanxi Medical University were divided into obese group and normal group according to their body mass index (BMI) level, with 30 cases in each group. The patients in both groups were maintained by retroperitoneal approach and total intravenous anesthesia. The IAP levels of supine position after anesthesia (T), the anterior and lateral position of the pneumoperitoneum (T0), 30 min after the pneumoperitoneum (T1), 60 min after the pneumoperitoneum (T2), 90 min after the pneumoperitoneum (T3), and the end of the pneumoperitoneum after 10 min (T4) were measured. The mean arterial pressure (MAP), airway pressure (Peak), oxygenation index (PaO2/FiO2), radial artery blood pH value and radial artery blood lactic acid value (Lac) at T, T0, T1, T2, T3, and T4 were recorded respectively. Results There were statistically significant differences of IAP levels in time, between groups and the interaction between the two groups (P < 0.05). Among them, T0 time point was higher than T time point, T1-T3 time points were higher than T0 time point, T4 time point was lower than T1-T3 time points; IAP levels of obesity group was higher than that of normal group at each time point, and the differences were statistically significant (P < 0.05). The difference between the two groups of MAP levels were statistically significant (P < 0.05). The differences in time, inter-group and interaction of Peak, PaO2/FiO2, blood pH, blood Lac between the two groups were statistically significant (P < 0.05). Among them, the Peak levels of the obesity group was higher than that of the normal group at each time point, while the T1-T3 time points of the two groups were higher than the T0 and T4 time points, and the differences were statistically significant (P < 0.05); PaO2/FiO2 of obesity group at the T3 and T4 time points were lower than that of normal group, while T1-T4 time points  were lower than T0, T3 and T4 time points were lower than T1 time point, T4 time point was lower than T2 time point, while T2-T4 time points of normal group were lower than T0 and T3 time points, T4 time point was lower than T1 time point, while T4 time point was lower than T2 time point, and the differences were statistically significant (P < 0.05); the blood pH at T3 and T4 points in the obese group were lower than the normal group, while T2-T4 time points were lower than T0 time points, T3 and T4 time points were lower than T1 and T2 time points, while T2 and T3 time points of normal group were lower than T0 time point, T3 time point was lower than T1 time point, and the differences were statistically significant (P < 0.05); the levels of blood Lac at T3 and T4 time points of obesity group were higher than those of control group, while the two groups at T2-T4 time points were higher than T0 time point, T3 and T4 time points were higher than T1 time point, while T4 time point was higher than T2 time point, and the differences were statistically significant (P < 0.05). Conclusion Obese patients will further increase the IAP during retroperitoneal laparoscopic surgery, even exceeding the set pressure of the pneumoperitoneum, and are more likely to have increased airway pressure, decreased oxygenation index, and acid-base imbalance. For obese patients, IAP can not be simply equated with pneumoperitoneal pressure. Intraoperative monitoring should be strengthened, and IAP can be monitored if necessary.
