Effect of different penetration enhancers on the transdermal absorption of phloretin

Objective To investigate the effect of azone, menthol, borneol and muscone on the transdermal promotion of phloretin in order to provide basis for the research and development of external preparations of phloretin. Methods The skin of 18 male SD rats with 2-month-old as the permeation barrier, the Franz diffusion cell was used to conduct the in vitro transdermal absorption experiment. The content of phloretin was determined by HPLC, the permeation kinetic parameters were calculated, and the permeation promoting effect of different transdermal enhancers on phloretin was evaluated. HPLC method for the determination of phloretin was as followed, chromatographic column: Inertsil ODS-3 (150 mm×4.6 mm, 5 μm); mobile phase: acetonitrile water (45:55); flow rate: 1.0 mL/min; detection wavelength: 280 nm; injection volume: 20 μL; column temperature: 25℃. Results The regression equation of phloretin in the concentration range of 0.1030-51.48 μg/mL was Y = 86.45C+3.901 (r = 0.9999). RSD of intraday and intraday precision was less than 2.0%. When the concentrations of azone, menthol, borneol and muscone were 4.0%, ER was 8.825, 13.561, 6.540 and 3.466 respectively. Conclusion Azone, menthol, borneol and muscone can enhance the transdermal effect of phloretin in a concentration-dependent manner. 4.0% penetration enhancer has the best promoting effect. The better transdermal enhancer for phloretin is menthol.

Study on the expression of bax and bcl-2 in kidney of rats with contrast-induced nephropathy and the intervention of Calcium Dobesilate

Objective To investigate the expression of bax and bcl-2 in kidney of rats with contrast-induced nephropathy (CIN) and the effects of Calcium Dobesilate. Methods Eighty-four rats were divided into sham operation group (group A), model group (group B) and Calcium Dobesilate intervention group (group C) according to random number table, 28 rats in each group. Group A was given saline, group B was injected with INDO, L-NAME and 760 g/L Diatrizoate, while group C was given Calcium Dobesilate for intragastric administration on the basis of group B. After 12, 24, 48, 72 h model establishment, blood urea nitrogen (BUN) and creatinine (Scr) were measured. Nephropathy was observed by HE staining. Renal cell apoptosis was detected by TUNEL. The expression of bax and bcl-2 were detected by immunohistochemistry and Western blot. Results The levels of BUN and Scr in group B and group C were significantly higher than those in group A at 12, 24, 48, 72 h after the establishment of model, while those in group C were significantly lower than those in group B at 12, 24, 48, 72 h after the establishment of model (P < 0.05). Compared with 12 h after modeling, serum BUN and Scr levels in group B and group C were significantly higher at 24, 48, 72 h after modeling (P < 0.05). Compared with 24 h after model establishment, serum Scr level in group B increased significantly at 48 h after model establishment, serum BUN and Scr levels in group C increased significantly at 48 h after model establishment, and serum Scr level in group C decreased significantly at 72 h after model establishment (P < 0.05). Compared with 48 h after modeling, serum BUN and Scr levels in group B and group C were significantly lower at 72 h after modeling (P < 0.05). In group A, the pathological changes were not significant; in group B, extensive tubular epithelial cell vacuolation, partial tubular necrosis and renal interstitial fibrosis appeared 12 h after modeling; in group C, the renal tubular interstitial lesions were significantly improved. There was no significant apoptosis in group A; the apoptosis index in group B was significantly higher than that in group A; the apoptosis index in group C was significantly lower than that in group B, but higher than that in group A (P < 0.05). The expression level of bax in group B was significantly higher than that in group A, bax in group C was significantly lower than that in group B, but higher than that in group A; the expression level of bcl-2 in group B was significantly lower than that in group A, and the expression level of bcl-2 in group C was significantly higher than that in group B, but lower than that in group A (P < 0.05). The relative expression of bax protein in group B was significantly higher than that in group A, and the relative expression of bax protein in group C was significantly lower than that in group B, but higher than that in group A; the relative expression of bcl-2 protein in group B was significantly lower than that in group A, and the relative expression of bcl-2 protein in group C was significantly higher than that in group B, but lower than that in group A (P < 0.05). Conclusion Apoptosis has close relationship with CIN, and Calcium Dobesilate may play a protective role by reducing the expression of bax and increasing the expression of bcl-2.

Effect of Qiling Yinao Decoction on the expression of nod-like receptor protein 3 in inflammasome of Alzheimer′s disease mice

Objective To investigate the effect of Qiling Yinao Decoction (QYD) on the expressions of nod-like receptor protein 3 (NLRP3) in inflammatory microglia of Alzheimer′s disease (AD) mice. Methods According to the random number table method, 24 male APP/PS1 dual-transgenic mice were divided into the QYD experimental group (18.330 g/kg), Donepezil group (0.001 g/kg) and model group, with 8 mice in each group. And 8 mice of the same litter and the same sex were divided into the control group. Model group and control group were given 2 mL/time of normal saline, and gavage treatment for 30 d. Morris water maze was used to test the learning and memory ability of APP/PS1 dual-transgenic mice. Transmission electron microscope was used to observe the changes of microglia cells in the cerebral cortex of mice. Immunohistochemistry and real-time quantitative polymerase chain reaction (RT-qPCR) were used to detect the expressions of NLRP3 in microglia cells in cerebral cortex and β-amyloid (Aβ) in hippocampus. Results Compared with the model group, the ability of learning and memory in QYD group and Donepezil group was significantly improved (all P < 0.05). Compared with the model group, the structure of microglia in cerebral cortex in QYD group and Donepezil group was significantly improved. Compared with the model group, the expression of NLRP3 in cerebral cortex and Aβ mRNA in hippocampus of QYD group and Donepezil group were significantly decreased (P < 0.05). Conclusion QYD can improve the learning and memory ability of AD mice by reducing the expression of NLRP3 in microglia cells in the cerebral cortex of AD mice.

Investigation on biofilm formation and gene polymorphism of the accessory gene regular (agr) in blood-derived Staphylococcus epidermidis

Objective To investigate the gene polymorphism of the accessory gene regular (agr) in blood-derived Staphylococcus epidermidis from clinical isolates, and to understand the molecular characteristics and biofilm formation ability. Methods From March 2012 to February 2015, strains of clinical isolated blood source from Sichuan Province People′s Hospital were collected. Staphylococcus epidermidis was further identified and confirmed by molecular biology method after being identified by automatic microbial identification system. The semi-quantitative adhesion test was used to detect biofilm formation ability, the gene polymorphism of agr was analyzed by multiplex PCR. Results During the experiment, 81 strains of Staphylococcus epidermidis were collected and all of them were included in the next identification and typing experiment. Among 81 strains of the epidermis Staphylococcus epidermidis in blood-derived, 51 (62.96%) agr strains were type Ⅰ, 6 (7.41%) agr strains were type Ⅱ, 7 (8.64%) agr strains were type Ⅲ, 17 (20.99%) agr strains were not parting. There were 54 biofilm-positive strains, accounting for 66.67%. Conclusion The agr gene of in blood-derived from clinical isolated is significantly polymorphic. Blood-derived Staphylococcus epidermidis has a strong biofilm formation ability, that is, has a strong pathogenic rate. Clinicians should pay attention to this phenomenon.

Study of multi-center cost estimation of cervical cancer screening in parts of China

Objective To develop cost estimation for providing financial-based quantitative basis for the sustainable development of cervical cancer screening project. Methods Ten medical pilots institutions were selected from 8 provinces/municipality, as Liaoning, Zhejiang, Guangdong, etc. by convenient sampling for taking time-driven activity-based costing to estimate cost difference between cytological examination and human papillomavirus (HPV) testing screening schemes for cervical cancer. Results The cost of cytological examination, HPV testing and cytology examination after HPV testing accounted for the highest proportion of the cost of consumables were 53.8%, 71.8% and 68.5%, respectively. The other expenses such as the organization was the lowest proportion of project. Between the cost components of colposcopy examination and pathology diagnosis, the manpower cost accounted for the highest proportion as 77.1% and 67.0%, respectively, while followed by consumables and equipment costs. Calculated based on capitation, the average cost per patient of cytological examination was 69.29 yuan per time, while the average cost per patient of cytology examination after HPV testing was 93.67 yuan per time. Sensitivity analysis reviews that manpower costs have the greatest impact on screening costs. Conclusion No matter using cytological examination or HPV testing, considering the sustainability of the project, in addition to the necessary equipment and consumables costs, the investment of personnel and counterpart funds should also be considered.

Influential factors and preventive measures of residual low back pain after percutaneous vertebral augmentation

Percutaneous vertebral augmentation, including vertebroplasty and kyphoplasty, is a classic method for the treatment of osteoporotic vertebral compression fractures and is widely used in clinical practice. However, some patients are still accompanied with varying degrees of residual low back pain, resulting in delayed bed rest, increased risk of complications, reduced surgical satisfaction and trust in doctors. However, the exact causes and countermeasures of residual low back pain after percutaneous vertebral augmentation have not been conclusive. Therefore, the review will explore the influencing factors and preventive measures of residual low back pain after percutaneous vertebral augmentation in the aspects of osteoporosis, soft tissue injury, lumbar degeneration, sagittal imbalance and vertebral recurrence fracture. It can contribute to the prevention and treatment of such diseases.

Progress of research on financial toxicity of breast cancer

The term “Financial toxicity” is widely used to describe the distress or hardship suffering from the financial burden of cancer treatment. High medical costs and reduced income after diagnosis of the disease have contributed to financial toxicity. A large proportion of breast cancer patients have experienced financial toxicity in Western counties, however, this has been rarely reported in China. Financial toxicity has many effects on patients and their families, including lifestyle changes, poor treatment compliance and delayed treatment, which in turn affects quality of life and mental health, and leads to poor prognosis. A variety of ways need to be implemented in order to reduce financial toxicity, including government policy support and discussion about cost-effective between doctors and patients. Further research is needed to provide Chinese patients evidence of most appropriate intervention targets, content and timing.

Development and application status of traditional Chinese medicine nursing in community elderly hypertension disease

The treatment time for elderly hypertension is long and the complications are numerous. It has a high disability rate and death rate, which has become a heavy burden for our family and society in our country. Traditional Chinese medicine nursing focuses on the overall concept and dialectical nursing. It can carry out different nursing interventions for elderly hypertensive patients according to individual differences, and can improve the control level of blood pressure. However, some problems have become increasingly obvious, such as the lack of certain talents in traditional Chinese medicine nursing, the small promotion efforts in community traditional Chinese medicine nursing, and the limited application scope. It needs to be continuously improved and solved in the future work in order to improve the quality of life and community service of elderly hypertensive patients and achieve the aim of promoting healthy aging.

Research status of LncRNA expression in non-small cell lung cancer

Lung cancer is currently ranked first in both morbidity and mortality of malignant tumors in the world. Long non-coding RNA (LncRNA), as a kind of non-coding RNA molecule that cannot encode proteins, is widely involved in the differentiation and cycle regulation of cells in organisms. Many long non-coding RNA can also play a role in promoting or inhibiting the occurrence and development of tumors. This article will summarize the regulatory mechanism of LncRNA on cell differentiation and tumor invasion and metastasis of non-small cell lung cancer, as well as its potential value in diagnosis and targeted therapy.

Experimental study and clinical progress of posterior scleral reinforcement

In recent years, clinical research on posterior scleral reinforcement has increased,mainly on the treatment of pathological myopia and its complications. The mechanism of posterior scleral reinforcement is to mechanically reinforce the posterior sclera by using foreign or autologous biomaterials or synthetic materials to prevent scleral deformation. The surgical methods of posterior scleral reinforcement include band, slice and injection. The surgical materials are mainly divided into biological and non-biological. Many new surgical methods and materials are gradually developed. A large number of experimental studies have explained the mechanism of posterior scleral reinforcement,and clinical trials have confirmed its effectiveness from the aspects of posterior scleral staphyloma, macular clefts and macular holes, which lay the foundation for further development.

Progress in the diagnosis and treatment of perianal abscess

Perianal abscess is a common disease in anorectal surgery, with a high incidence rate. It is mainly treated with surgery, but the incidence of postoperative abscess recurrence and anal fistula formation is high, which greatly affects the quality of life of patients. Postoperative routine use of antibacterial drugs to promote rehabilitation and prevent complications, but the efficacy reported in various studies is uneven, and there is no consensus on the use of antibacterial drugs after perianal abscess operative at home and abroad. As anti-infective treatment of antibacterial drugs has gradually failed, research on the application of traditional Chinese medicine after surgical treatment of perianal abscess has emerged in recent years. This article summarizes the progress in the diagnosis and treatment of perianal abscess in the past 5 years.

Establishment of behavior intention model of Chinese medicine health management in patients with non-alcoholic fatty liver disease

Objective To explore the influencing factors of non-alcoholic fatty liver disease (NAFLD) patients′ participation in health management of tradition Chinese medicine, so as to provide evidence for optimizing health management of tradition Chinese medicine strategies and improving patients′ participation and compliance. Methods The theory of planned behavior is used as the theoretical basis for establishing behavioral intention model of health management of tradition Chinese medicine for NAFLD patients. A self-developed questionnaire was used to investigate NAFLD patients in the Medical Examination Center and Liver Disease Outpatient of Hubei Provincial Hospital of Traditional Chinese Medicine from January to June 2018. AMOS 22.0 software was used for model fitting, analysis and evaluation. Results The behavioral intention model of health management of tradition Chinese medicine for NAFLD patients was fitted well with the sample data. The effects of behavioral attitude, subjective norm and perceived behavior control on behavioral intention were 0.218, 0.214 and 0.211, respectively, and the differences were statistically significant (all P < 0.05). Conclusion The theory of planned behavior is applicable to explain NAFLD patients′ intention for health management of tradition Chinese medicine. When promoting the behavioral intention of patients according to the theory of planned behavior, it is necessary to establish a positive behavioral attitude, attach importance to the role of subjective norms and improve perceived behavior control. Jointly enhance the patients′ behavioral intention and promote the development and improvement of NAFLD health management of tradition Chinese medicine.

Research on the acceptance of internet medical services and its influencing factors

Objective To study the acceptance of Internet medical services and its influencing factors, while to put forward countermeasures and suggestions for the development of Internet medical services. Methods From January to June 2018, the outpatients of five tertiary general hospitals in Tianjin were selected. Questionnaires were used to understand and analyze the patients′ needs and willingness to use Internet medical services. Results The factors of gender, age, occupation, educational level and the number of chronic diseases and so on were compared, and the differences were statistically significant (P < 0.05 or P < 0.01). The respondents had the highest expectation of improving the medical environment and providing effective and convenient medical services. Conclusion Improving the Internet medical environment, building the quality of internet medical services, increasing government support, strengthening the standardization of internet medical services, and enhancing publicity and guidance have practical significance for the development of Internet medical services.

Investigation and training strategies on medical professionalism of medical students in higher vocational colleges

Objective To further study the present cultivation status on professionalism of medical students in higher vocational colleges, while explore measures to improve professionalism of medical students in higher vocational colleges, provide reference for the training and promotion the professionalism of medical students, and supply empirical data for the construction and management of medical related majors. Methods This study investigated 1300 students′ professional recognition, job-hunting and employment, and school professionalism training in 4 medical higher vocational colleges in Hu′nan Province. While in-depth interviews were conducted to investigate the situation of four majors in different majors and grades of Changsha Health Vocational College. Results The professional recognition and willingness of medical students in higher vocational colleges were generally low, while the schools′ overall satisfaction with the cultivation of medical students′ professionalism was low. Conclusion It is suggested to strengthen the training and learning of professionalism in higher vocational colleges. According to the results of investigation and in-depth interviews, after theoretical exploration and expert consultation and discussion, the training strategies of medical students′ professionalism in higher vocational colleges should be put forward from the aspects of system mechanism, curriculum design and teaching reform.

Analysis of dietary patterns and influencing factors of urban and rural residents in the regions of Liaoning Province

Objective To analyze the dietary patterns and influencing factors of urban and rural residents in Liaoning Province. Methods Dietary survey data of 2049 residents in Liaoning Province in the “nutrition and health monitoring of Chinese residents” project from 2010 to 2012 were used to obtain dietary patterns by factor analysis. The influence of different social demographic characteristics on dietary patterns were analyzed by multivariate Logistic regression analysis. Results Three dietary patterns were obtained by factor analysis including flour pattern, rice pattern and nut pattern. The flour pattern was positively correlated with male and urban residents (P < 0.01), which was negatively correlated with the education level of primary school or below crowd(P < 0.01). The rice pattern was positively correlated with male, 18-<45 years old and low income level crowds (P < 0.05), which was negatively correlated with urban residents (P < 0.01). Nut pattern was positively correlated with primary school education and below crowd (P < 0.05), which was negatively correlated with male, 45-<60 years old and urban residents (P < 0.05). Conclusion There are three dietary patterns for urban and rural residents in Liaoning Province. Gender, age, region, culture degree and income level are the influencing factors of dietary patterns. Health education should be strengthened and dietary balance should be advocated for the key population according to the influencing factors of different dietary patterns.

The relationship between the levels of 25 hydroxy vitamin D and endothelin and the degree of coronary artery disease in patients with coronary heart disease

Objective To study the relationship between the level of 25 hydroxy vitamin D [25(OH)D] and endothelin (ET) and the severity of coronary artery disease in patients with coronary heart disease (CHD). Methods From September 2017 to August 2018, 200 patients with CHD admitted to the Second People′s Hospital of Guangdong Province (hereinafter referred to as "our hospital") were selected as CHD group, which was divided into 89 patients of mild lesion group, 66 patients of moderate lesion group and 45 patients of severe lesion group according to Gensini score. Another 60 normal persons who received physical examination in our hospital in the same period were taken as normal control group. General data between CHD group and normal control group was compared. The levels of serum 25(OH)D and plasma ET in four groups were measured, and the correlation and multivariate Logistic regression analysis were carried out. Results The levels of total cholesterol, triglyceride and low-density lipoprotein cholesterol in CHD group were higher than those in normal control group, the difference was statistically significant (P < 0.05). Serum levels of 25(OH)D in the mild, moderate and severe lesions groups were lower than those in normal control group, the serum levels of 25(OH)D in moderate and severe lesions groups were lower than those in mild lesion group, and the serum level of 25(OH)D in severe lesion group was lower than that in moderate lesion group (P < 0.05). Serum levels of ET in mild, moderate and severe lesions groups were higher than those in normal control group, serum levels of ET in moderate and severe lesions groups were higher than those in mild lesion group, and serum level of ET in severe lesion group was higher than that in moderate lesion group (P < 0.05). Pearson correlation analysis showed that serum 25(OH)D level was negatively correlated with Gensini score (r = -0.597, P < 0.05), while ET level was positively correlated with Gensini score (r = 0.644, P < 0.05). Multivariate Logistic regression analysis showed that 25(OH)D was the protective factor of CHD (OR = 0.735, 95%CI: 0.608-0.889, P < 0.05), while ET was the independent risk factor of CHD (OR = 2.498, 95%CI: 1.338-4.663, P < 0.05). Conclusion The decrease of serum 25(OH)D level and the increase of ET level in patients with CHD are closely related to the severity of coronary artery disease. They are risk factors of CHD and are expected to become new clinical diagnostic indicators.

The influence of double heart medical model on patients with chronic heart failure

Objective To explore the influence of double heart medical model on patients with chronic heart failure. Methods 120 patients with chronic heart failure treated in Department of ICU of the Second Affiliated Hospital of Harbin Medical University from February 2017 to February 2019 were selected as study objects. According to the random paper method, they were divided into two groups: 56 patients in control group (conventional medical mode) and 64 patients in observation group (double heart medical mode). The score of self-rating anxiety scale (SAS), score of self-rating depression scale (SDS), standard diviation average of NN intervals (SDANN), standard diviation of NN intervals (SDNN), the difference between root mean square of succe successive differences (rMSSD), left ventricular ejection fraction, cardiac output and stroke output were observed before and after the intervention in two groups. Results Before intervention, there were no significant differences in SAS score, SDS score, SDANN, SDNN, rMSSD, left ventricular ejection fraction, cardiac output and stroke output between two groups (P > 0.05). After intervention, the scores of SAS and SDS in two groups were significantly lower than those before intervention, SDANN, SDNN and rMSSD in two groups were significantly faster than those before intervention, left ventricular ejection fraction in two groups was significantly higher than that before intervention, cardiac output and stroke output in two groups were significantly greater than those before intervention; the scores of SAS and SDS in observation group were lower than those in control group, SDANN, SDNN and rMSSD in observation group were faster than those in control group, left ventricular ejection fraction in observation group was higher than that in control group, cardiac output and stroke output in observation group were higher than those in control group, the differences were statistically significant (P < 0.05). Conclusion The application of double heart medical model in patients with chronic heart failure can improve bad mood, reduce heart rate variability and improve heart function, which is worthy of clinical application.

Effect of Modified Huanglian Wendan Decoction on post-stroke depression and its influence on the expression level of neurometabolites in brain

Objective To explore the clinical efficacy of Modified Huanglian Wendan Decoction in treating post-stroke depression (PSD) and its effect on the changes of brain neurometabolites in patients. Methods A total of 96 patients with PSD admitted to Tangshan Hospital of Traditional Chinese Medicine from June 2017 to December 2018 were selected and randomly divided into the observation group and the control group according to random number table method, 48 cases in each group. The control group was treated with Sertraline Hydrochloride Tablets orally, and the observation group was added with Modified Huanglian Wendan Decoction on the basis of the control group. All patients were treated for 8 weeks. The clinical efficacy of the two groups was compared. The depression symptoms [17 Hamilton depression scale (HAMD), Zung self-rating depression scale (SDS)], cognitive function [minimum mental state examination (MMSE)], the quality of sleep [Pittsburgh sleep quality index (PSQI)], daily living ability[improved Barthel index (MBI)] in the two groups were evaluated once before and after treatment. At the same time, the levels of brain neurometabolism indexs [norepinephrine (NE), dopamine (DA), the ratio of 5-hydroxytryptophan (5-HT) to 5-hydroxyindoleacetic acid (5-HIAA) (5-HT/5-HIAA ratio), and the ratio of 3,4-dihydroxyphenylacetic acid (DOPAC) to high vanillic acid (HVA) (DOPAC/HVA ratio)] were measured. Results The total effective rate of the observation group was significantly higher than that of the control group (P < 0.05). Compared with before treatment, the scores of HAMD, SDS and PSQI were significantly lower, while the scores of MMSE and MBI were significantly increased (all P < 0.05). Compared with the control group, the improvement was more obvious in the observation group (P < 0.05). The concentrations of serum NE, DA and the ratios of 5-HT/5-HIAA and DOPAC/HVA were significantly higher in the two groups after treatment (all P < 0.05). Compared with the control group, the improvement was more obvious in the observation group ( P < 0.05). Conclusion The overall efficacy of Modified Huanglian Wendan Decoction in treating PSD is effective, which is helpful to relieve depressive symptoms, improve cognitive function and sleep quality of patients, which may be related to the significant positive regulation of the expression level of relevant cerebral nerve metabolites by this clinical experience prescription.

Effect of low-dose Aspirin combined with Tiaojing Cuyun Pills on follicular blood flow and pregnancy outcome of patients with polycystic ovarian syndrome

Objective To study the clinical effect of low dose Aspirin combined with Tiaojing Cuyun Pills on the follicular blood flow and pregnancy outcome of patients with polycystic ovary syndrome (PCOS) and its clinical application value. Methods From September to December 2018, 78 infertility women (107 cycles) with PCOS were selected and they were randomly grouped according to the odd-even of admission number: 38 cases of experimental group were treated with Letrozole (LTZ), Aspirin and Tiaojing Cuyun Pills, while 40 cases of control group were treated with LTZ only. The endometrial thickness, follicles maturation, values of follicle peripheral artery [pulsatility index (PI), resistance index (RI), peak systolic velocity (PSV), end diastolic velocity (EDV)], periodic ovulation rate and periodic pregnancy rate on the day of human chorionic gonadotropin (HCG) administration were measured by B ultrasound, and the clinical intervention value was observed. The patients who failed pregnancy in the control group were reintegrated into the experiment group in the next cycle (n = 29), and the ovulation treatment effect was observed. Results The rate of pregnancy cycle in the experiment group was higher than that in the control group (P < 0.05). Patients who failed pregnancy were given the same intervention as experimental group in the next cycle, compared with before changing medication regimen, PI and RI were significantly lowered, while EDV was significantly increased (P < 0.05), the rate of pregnancy cycle was significantly higher of the same patients. Conclusion Compared with simple Western medicine treatment, low-dose Aspirin combined with Tiaojing Cuyun Pills can effectively improve follicle perifollicular artery blood flow, enhance the oocyte development and clinical pregnancy rate. It is an effective way to help pregnancy.

Effect of abdominal aorta balloon block technique on renal function, hemostasis and pregnancy outcome in patients with dangerous placenta previa undergoing cesarean section

Objective To investigate the effect of abdominal aorta balloon block technique on renal function, hemostasis and pregnancy outcome in patients with dangerous placenta previa undergoing cesarean section. Methods Clinical data of 80 patients with dangerous placenta previa underwent cesarean section from January 2016 to January 2019 in Baise People′s Hospital of Guangxi Zhuang Autonomous Region were analyzed retrospectively. According to whether or not performed abdominal aorta balloon block technique before caesarean section, the patients were divided into control group (42 cases, underwent caesarean section) and observation group (38 cases, underwent caesarean section after abdominal aorta balloon block technique). The clinical index, pregnant outcome and postoperative complications of the two groups were compared. The renal function indexes before operation, 24, 48 and 72 h after operation were compared between the two groups. Results The amount of bleeding and blood transfusion during operation in the observation group were less than those in the control group (P < 0.01), and the operation time and hospitalization time were shorter than those in the control group (P < 0.01). The creatinine level in the control group 48 h after operation and in the observation group 24 h and 48 h after operation were higher than those before treatment (P < 0.05), while the uric acid in the observation group was lower than that in the control group 48 h after operation (P < 0.05). The postpartum hemorrhage rate and infection rate in the observation group were lower than those in the control group, but the Apgar score of newborn was higher than that in the control group (P < 0.05). There were no significant differences in the rate of hysterectomy, neonatal malformation and postoperative complications between the two groups (P > 0.05). Conclusion Abdominal aorta balloon block technique in caesarean section patients with dangerous placenta previa can effectively improve perioperative indicators and pregnancy outcomes, and will not have any great impact on renal function of patients. It has a high clinical value.

The role of breast milk combined with breast milk fortifier in improving the growth and development of very low birth weight premature infants

Objective To explore the role of breast milk combined with breast milk fortifier in improving the growth and development of premature infants with very low body weight (VLBW). Methods From January 2016 to December 2017, 200 cases of VLBW premature infants born and completed in the Department of Obstetrics and Gynecology, Guiyang City Maternal and Child Health Hospital of Guizhou Province were selected as subjects, and they were divided into breast milk group and breast milk enhancer group according to feeding methods, 100 cases in each group. Growth data and growth deviation of preterm infants between the two groups at 1, 2 months and 3 months were compared. The incidence of feeding intolerance, complications, and infant neurological international battery (INFANIB) scores were compared between the two groups. Results Compared with the premature infants in the breast milk group, the age-specific body weight Z score, age-head circumference Z-score, age-specific body length Z score, body length and body weight Z score, body mass index Z score of breast milk enhancer group showed a significant increase in corrected age at 1, 2 months and 3 months (P < 0.05), growth deviation rate was significantly lower (P < 0.05), incidence of infection within 3 months corrected age decreased significantly (P < 0.05). However, there were no statistically significant differences in the incidence of feeding intolerance, retinopathy of prematurity, necrotizing enterocolitis and bronchopulmonary dysplasia, and INFANIB scores between the two groups (P > 0.05). Conclusion Breast milk combined with breast milk fortifier can significantly promote the growth and development of VLBW premature infants, reduce the deviation of growth and development, and reduce the incidence of infection, which is worthy of clinical promotion.

Application value of tandem mass spectrometry in screening high-risk infants with inherited metabolic diseases in Kashi Area

Objective To explore the application value of tandem mass spectrometry in screening high-risk infants with inherited metabolic disorder (IMD) in Kashi Area of Xinjiang. Methods The blood samples of 321 suspected IMD high risk infants in Kashi Area from March 2017 to December 2018 were collected by dry blood paper method. API3000 tandem mass spectrometer was used for detection, the concentrations of amino acids and acyl carnitine in blood samples were calculated, and the IMD types distribution, age distribution, and clinical manifestations of IMD positive children were analyzed. Results Among 321 suspected IMD high risk infants, 17 were positive, the total positive rate of screening was 5.30% (17/321). There were 6 cases occurred in 24 hours old to 1 month old, 8 cases in 1 month old to 1 year old, 2 cases in 1 year old to 4 years old, and 1 cases in 4 years old to 12 years old; the main clinical manifestations of children with onset at 24 hours old to 1 month old were intermittent convulsions, faint from fear, or psychobehavioral reactions, and so on; the main manifestations of children with onset at 1 month old to 4 years old were growth retardation and feeding difficulties; the main manifestations of children with onset at 4 to 12 years old were anemia of unexplained anemia and enlargement of liver and spleen. Conclusion The incidence of IMD in Kashi Area is complex, involving all age groups, and the application of tandem mass spectrometry is conducive to the early screening and diagnosis of IMD.

Clinical efficacy of Anlotinib in the treatment of advanced non-small cell lung cancer

Objective To observe the efficacy and adverse effect of Anlotinib in the treatment of advanced non-small cell lung cancer (NSCLC). Methods A total of 40 patients with advanced NSCLC (after second-line or further treatment) admitted to the Department of Oncology and Respiration of Xuzhou Central Hospital from May to October 2018 were admitted. They were randomly divided into observation group and control group according to the random number table method, with 20 cases in each group. The observation group was treated with Anlotinib (12 mg/d, 2 weeks on-treatment followed by 1 week off-treatment in a 21-day cycle), while the control group was treated with best supportive care. Objective response rate, disease control rate, progression free survival, overall survival and adverse effect were observed. Results There was no statistically significant difference in objective response rate between two groups (P > 0.05). The disease control rate in the observation group was significantly higher than that in the control group (P < 0.05). The progression free survival and overall survival in the observation group were significantly higher than those in the control group (P < 0.05), grade 1-2 adverse effects were common in the observation group, grade 3-4 adverse effects were mainly associated with hand-foot syndrome, oral mucositis, poor appetite and fatigue. Conclusion Anlotinib is effective in the treatment of advanced NSCLC, and the adverse effects were tolerable.

Expression and significance of CXCR1 and IL-17 in patients with inflammatory bowel disease

Objective To study the expression and significance of CXC chemokine receptor 1 (CXCR1) and interleukin-17 (IL-17) in patients with inflammatory bowel disease (IBD). Methods The clinical data of 80 patients with IBD admitted to Huizhou Third People′s Hospital from January 2016 to December 2017 was analyzed retrospectively, 70 patients with ulcerative colitis (UC group), 10 patients with Crohn′s disease (CD group), and 50 healthy volunteers who underwent colonoscopy at the same time were selected as control group. The expression of IL-17 and CXCR1 mRNA in serum and intestinal mucosa of each group was compared, and the protein expression of CXCR1 in intestinal mucosa of each group was analyzed. The correlation between the expression of IL-17 and CXCR1 mRNA in serum and in intestinal mucosa of patients was analyzed. Results Compared with the control group, the expression of IL-17mRNA in serum of UC group and CD group were higher (all P < 0.05), but there was no significant difference between UC group and CD group (P > 0.05). There was no significant difference in serum CXCR1 mRNA expression between the two groups (P > 0.05). Compared with the control group, the expression of IL-17 and CXCR1 mRNA in intestinal mucosa of UC group and CD group was higher (P < 0.05). The expression of CXCR1 mRNA in intestinal mucosa of UC group was higher than that of CD group (P < 0.05), however, there was no significant difference in IL-17 mRNA expression between the two groups (P > 0.05). Compared with the control group, the protein expression of CXCR1 in intestinal mucosa of UC group and CD group was higher, and the protein expression of CXCR1 in UC group was higher than that in CD group (P < 0.05). Spearman correlation analysis showed that there was a weak correlation between the expression of IL-17 and CXCR1 in serum and intestinal mucosa (P < 0.05). Conclusion The expression levels of CXCR1 mRNA and IL-17 mRNA in serum and intestinal in IBD patients are higher, and there is a positively correlation between the expression of CXCR1 mRNA and IL-17 mRNA in serum and in intestinal mucosa. It is clinically possible to monitor the levels of CXCR1 and IL-17 to better assist in the treatment of IBD.

Analysis of susceptible factors of oral infection in ICU patients with coma and intervention measures

Objective To explore the susceptible factors and intervention measures of oral infection in coma patients in intensive care unit (ICU). Methods From January 2017 to January 2019, 117 ICU coma patients admitted to Beijing Tiantan Hospital, Capital Medical University were selected to observe their oral infection. Data were collected including gender, age, education, body mass index, oral health status, oral mucosal injury, antibiotic use, and systemic diseases including hypertension and diabetes. Chi-square test or Fisher′s exact probability method were used to analyze the factors related to oral infection in ICU patients with coma, and multivariate Logistic regression was used to analyze the risk factors of oral infection. Results Among the 117 patients, 24 had oral infection, with an infection rate of 20.51%. No oral infection occurred in 93 cases, accounting for 79.49%. There were statistically significant differences between the two groups in complicated with diabetes mellitus or not, oral health status, oral mucosal injury, and antibiotics use or not (all P < 0.05). Multivariate Logistic regression analysis showed that patients complicated with diabetes mellitus (OR = 1.648, 95%CI: 1.271-2.381, P < 0.05), poor oral hygiene (OR = 2.367, 95%CI: 1.628-3.319, P < 0.05), accompanied by oral mucosal injury (OR = 2.981, 95%CI: 1.892-3.946, P < 0.05), and absence of antibiotics (OR = 1.983, 95%CI: 1.462-2.763, P < 0.05) were risk factors affecting oral infection in ICU patients with coma. Conclusion ICU patients with coma have a high rate of oral infection, which is affected by many factors. Complications of diabetes, poor oral hygiene, accompanied by oral mucosa injury, and absence of antibiotics are the susceptible factors for oral infection. In order to reduce oral infection, relevant oral care evaluation and intervention should be taken according to the susceptible factors.

Clinical effect of Adefovir Dipivoxil combined with Rougan Huaxian Granules on chronic hepatitis B cirrhosis

Objective To observe the clinical efficacy of Adefovir Dipivoxil combined with Rougan Huaxian Granules in the treatment of patients with chronic hepatitis B virus (HBV) cirrhosis. Methods From March 2017 to March 2019, 100 patients with chronic hepatitis B cirrhosis diagnosed by The First Affiliated Hospital of Guangxi University of Chinese Medicine were selected. According to the random number table method, they were divided into the control group and the treatment group, with 50 patients in each group. The control group was treated with Adefovir Dipivoxil alone, and the treatment group was treated with Rougan Huaxian Granules on the basis of the control group for a total of 6 months. The clinical symptoms, liver function, liver fibrosis 4 items, liver B-mode ultrasound, the content of HBV-DNA, hepatitis B e antigen (HBeAg) and hepatitis B e antibody (HBeAb) in the two groups were observed before and after treatment. Results The traditional Chinese medicine syndrome scores of patients in both groups after treatment were better than those before treatment, and the treatment group was better than the control group at the same period (P < 0.05). In the two groups after treatment liver function index [aspartic transaminase (AST), alanine transaminase (ALT), total bilirubin (HBIL)], liver fibrosis index [serum hyaluronic acid (HA), laminin (LN), type Ⅲ procollagen (PCⅢ), type Ⅳ collagen (Ⅳ-C)], portal vein diameter, the thickness of the spleen decreased significantly before the treatment (all P < 0.05), and compared with the control group, the degree of the treatment group decreased more significantly (P < 0.05). Effect of Hydrocortisone combined with Ulinastatin in treatment of septic shock

Objective To explore clinical efficacy of Hydrocortisone combined with Ulinastatin in treatment of septic shock. Methods From September 2015 to September 2018, 80 cases with septic shock admitted to Guangxi Zhuang Autonomous Region Liuzhou People′s Hospital were selected. According to the random number table method, they were divided into the control group (40 cases) and the observasion group (40 cases). Both groups were given conventional treatment, the control group was given Hydrocortisone on the basis of conventional treatment, and the observasion group was treated with Hydrocortisone combined with Ulinastatin. The two groups were continuously treated for 2 weeks. Clinical efficacy was evaluated after treatment. The mechanical ventilation time, hospitalization time were compared between the two groups. The acute physiology and chronic health evaluation Ⅱ (APACHEⅡ) score, systolic blood pressure (SBP), white blood cell count (WBC), levels of serum C-reactive protein (CRP), tumor necrosis factor-α (TNF-α) and procalcitonin (PCT) of the two groups before and after treatment were compared. The occurrence of adverse reactions and 28-day mortality during the treatment of the two groups was recorded. Results The total clinical effective rate of the observasion group was significantly higher than that of the control group, the difference was statistically significant (P < 0.05). The mechanical ventilation time and hospitalization time of the observasion group were significantly shorter than those of the control group, the differences were statistically significant (all P < 0.05). The APACHEⅡ score, WBC of the observasion group were significantly shorter than those of the control group, SBP of the observasion group was significantly higher than that of the control group, the differences were statistically significant (all P < 0.05). The level of serum CRP, TNF-α and PCT of the observasion group were significantly lower than those of the control group, the differences were statistically significant (all P < 0.05). The 28-day mortality of the observasion group was lower than that of the control group, the difference was statistically significant (P < 0.05). There was no obvious adverse reactions in the course of treatment in both groups. Conclusion Hydrocortisone combined with Ulinastatin is significantly effective in treatment of septic shock, which can significantly alleviate the illness, improve the body′s level of inflammation and reduce mortality.

Effect of Angelica Antipruritic Mixture in the treatment of blood deficiency and wind dryness type of senile pruritus

Objective To evaluate the clinical effect of Angelica Antipruritic Mixture in the treatment of blood deficiency and wind dryness type of senile skin pruritus. Methods From November 2017 to March 2018, 78 cases with senile pruritus in Shuguang Hospital Affiliated to Shanghai University of Traditional Chinese Medicine were divided into control group and treatment group according to the random number table method, with 39 cases in each group. Control group was treated with Cetirizine Hydrochloride and Urea Cream, and treatment group was treated with Angelica Antipruritic Mixture and Urea Cream. The treatment duration of both groups were 4 weeks. Visual analogue score (VAS), scratch marks (SM), percentage of nighttime itch time (PIT), skin hydration (SH), lipid content (LC) and sleep, constipation, dry mouth were compared. Resluts VAS, SM, PIT, SH, LC, sleep, constipation and dry mouth scores were compared between two groups before treatment, the differences were no statistically significant (all P > 0.05). After treatment, SM, PIT, SH, LC, constipation and dry mouth scores of treatment group were lower than those of control group, VAS score of treatment group was higher than that of control group, the differences were statistically significant (all P < 0.05). Sleep score of treatment group was higher than that of control group, the difference was statistically significant (P < 0.05). Conclusion Angelica Antipruritic Mixture has a good clinical effect in the treatment of blood deficiency and wind dryness type of senile skin pruritus. It can effectively reduce the symptoms of itching, enhance SH and LC, sleep and life condition, with no obvious adverse reactions.

Effect of Bifidobacterium Triple Viable Bacteria combined with XELOX chemotherapy on intestinal flora and immune function after operation for colorectal cancer

Objective To investigate the effect of Bifidobacterium Triple Viable Bacteria combined with Oxaliplatin and Capecitabine (XELOX) chemotherapy on intestinal flora and immune function after operation for colorectal cancer. Methods From February 2016 to February 2019, 93 cases with colorectal cancer after operation admitted to the Second Affiliated Hospital of Suzhou University were selected. According to the random number table method, they were divided into study group (47 cases) and control group (46 cases). Surgical treatment were received in both groups, and control group was given XELOX chemotherapy after surgery, study group was given Bifidobacterium Triple Viable Bacteria on the basis of control group. The clinical effect was evaluated after treatment. The immune function, intestinal flora and the improvement of serum tumor markers were compared in two groups. The occurrence of adverse reactions during treatment in two groups were recorded. Results After two cycles of treatment, the total effective rate of study group was higher than that of control group, the difference was statistically significant (P < 0.05). The lactobacillus, bifidobacterium of study group were higher than those of control group, the escherichia coli and enterococcus faecalis of study group were lower than those of control group, the differences were statistically significant (all P < 0.05). CD4+ and CD4+/CD8+ of study group were lower than those of control group, CD8+ was higher than that of control group, the differences were statistically significant (all P < 0.05). Carbohydrate antigen 199 and carcino-embryonic antigen of study group were lower than those of control group, the differences were statistically significant (all P < 0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P > 0.05). Conclusion Bifidobacterium Triple Viable Bacteria combined with XELOX chemotherapy is effective in the treatment of colorectal cancer. It can effectively improve the intestinal flora, immune function and reduce serum tumor markers, and it is safe and effective.

Methods From January 2017 to April 2019 , 80 cases with elderly AECOPD patients complicated with respiration failure in Department of Critical Care Medicine of the First Affiliated Hospital of Xinjiang Medical University were selected as observational objects. According to the random number table method, they were divided into the control group and the observation group, with 40 cases in each group. The control group was intervened with conventional mode of early rehabilitation, while the observation group was intervened with early rehabilitation based on multidisciplinary cooperation. Muscular strength was assessed by medical research council (MRC) score. Days on ventilator, ICU treatment, the hospitalization, incidences of ICU-AW, MRC score, activity of daily living (ADL) scores and occurrence of complications were compared in two groups. Results Days on ventilator, ICU treatment and the hospitalization of the observation group were lower than those of the control group, incidences of ICU-AW were lower than those of the control group, MRC score and ADL score after intervention of the observation group were higher than those of the control group and the incidence of complications were reduced, the differences were statistically significant (all P < 0.05). Conclusion Early rehabilitation based on multidisciplinary cooperation can effectively prevent incidence of ICU-AW in elderly AECOPD patients complicated with respiration failure. It can shorten time of mechanical ventilation, ICU treatment, improve activity of daily living and reduce incidence of complications, which is worthy of clinical application.

Clinical effect of Lipoic Acid combined with non-invasive ventilator in treatment of obstructive sleep apnea hypopnea syndrome

Objective To explore the clinical efficacy and safty of Lipoic Acid combined with non-invasive ventilator in treatment of obstructive sleep apnea hypopnea syndrome (OSAHS). Methods From January 2016 to June 2018, 80 cases with OSAHS admitted to Shijiazhuang First Hospital were selected. According to the random number table method, they were divided into the control group (40 cases) and the observasion group (40 cases). The control group was treated with non-invasive ventilator, and the observasion group was treated with Lipoic Acid combined with non-invasive ventilator. Both groups were continuously treated for 12 weeks. Clinical efficacy was evaluated after treatment. The Epworth somnolence scale (ESS) scores, athens insomnia scale scores, apnea hypopnea index (AHI), blood oxygen saturation (SpO2), medial blood oxygen saturation (MSpO2), and levels of serum malondialdehyde (MDA), superoxide dismutase (SOD) and hypoxia inducible factor-1α (HIF-1α) were compared before and after treatment. The occurrence of adverse reactions were recorded. Results The total clinical effective rate of the observasion group was significantly higher than that of the control group, the difference was statistically significant (P < 0.05). The ESS scores, athens insomnia scale scores of the observasion group after treatment were significantly lower than those of the control group, the differences were statistically significant (all P < 0.05). AHI of the observasion group after treatment was significantly lower than that of the control group, SpO2 and MSpO2 of the observasion group after treatment were significantly higher than those of the control group, the differences were statistically significant (all P < 0.05). The levels of MDA and HIF-1α of the observasion group after treatment were significantly lower than those of the control group, the level of SOD of the observasion group after treatment were significantly higher than that of the control group, the differences were statistically significant (all P < 0.05). During treatment, there was no serious adverse reaction in both two groups. Conclusion Lipoic Acid combined with non-invasive ventilator has significant clinical efficacy in treatment of OSAHS. It can improve the sleep breathing status and blood oxygen saturation, regulate the indicators of oxidative stress response, and has good safety.

Review and analysis on the prescription of Chinese herbal pieces in outpatient department of our hospital in 2018

Objective To comprehensively analyze the prescription of Chinese herbal pieces in outpatient department, standardize the prescription of Chinese herbal medicine, and ensure the safety and effectiveness of clinical application of Chinese herbal medicine. Methods 1200 pieces of prescription of Chinese herbal medicine in outpatient department of Department of Pharmacy of the Second Affiliated Hospital of Xinjiang Medical University (hereinafter referred to as “our hospital”) from January to December 2018 for comment were randomly selected, and the medication situation and unreasonable prescription types were analyzed. Results Among the 1200 prescriptions, 227 were unreasonable, accounting for 18.92%. The unreasonable phenomena were as followed: the clinical diagnosis was not standardized, the writing was not standardized, the use method was wrong, the number of taste was too much, the dosage was too large, and the use of toxic traditional Chinese medicine was unreasonable. Conclusion Unreasonable medicine use in prescription of outpatient exists. It is necessary to strengthen management and improve the prescription standardization to ensure reasonable and safe medication for patients.

Clinical study of Tongxinluo Capsules combined with Amlodipine Besylate and Atorvastatin Calcium Tablets in the treatment of angina pectoris of coronary heart disease

Objective To explore the clinical efficacy of Tongxinluo Capsules combined with Amlodipine Besylate and Atorvastatin Calcium Tablets in the treatment of angina pectoris of coronary heart disease. Methods A total of 157 patients with angina pectoris of coronary heart disease were admitted to Third People′s Hospital of Wuxi from February 2017 to March 2019 were selected, they were randomly divided into control group (78 cases) and study group (79 cases) according to the random number table method. The control group was treated with Amlodipine Besylate and Atorvastatin Calcium Tablets, and the study group was treated with Tongxinluo Capsules on the basis of the control group. The clinical efficacy, blood lipid, cardiac function index and hemorheology of the patients in the two groups were compared, and the occurrence of adverse reactions during the treatment was observed. Results After 3 months of treatment, the total effective rate of the study group was significantly higher than that of control group (P < 0.05). Compared with before treatment, total cholesterol, triacylglycerol and low-density lipoprotein in the two groups were decreased after 3 months of treatment (P < 0.05), and the study group was lower than the control group (P < 0.05), while the high-density lipoprotein was increased (P < 0.05), and the study group was higher than the control group (P < 0.05). Pathway analysis of the effects of self-efficacy, social support and mental resilience on benefit discovery in breast cancer patients undergoing chemotherapy

Objective To analyze the status quo of benefit finding in chemotherapy period breast cancer patients and explore the influence of self-efficacy, surgery social support and resilience on the level of benefit finding. Methods From October 2018 to April 2019, 153 breast surgery patients in the Affiliated Hospital of Qingdao University and Weifang Medical University were selected. General data questionnaire, benefit discovery scale, general self-efficacy scale, social support rating scale and psychological resilience scale were used to investigate, Pearson correlation analysis, multiple stepwise regression analysis and path analysis were used for analysis. Results The benefit score of breast cancer patients was (55.35±12.26). The level of benefit discovery was positively correlated with self-efficacy, social support, mental resilience and other dimensions (P < 0.01). Self-efficacy, subjective support and mental resilience were the influencing factors of benefit discovery. Self-efficacy has direct and positive effects on the level of benefit discovery, while social support and mental resilience have direct and indirect positive effects on the level of benefit discovery of breast cancer patients. Conclusion The level of benefit discovery in breast cancer patients during chemotherapy should be improved. Self-efficacy, subjective support and mental resilience can positively predict the level of benefit discovery. Medical staff should seek effective support for patients, take measures to improve patients′ self-efficacy and elasticity level, and improve the level of benefit discovery of breast cancer patients during chemotherapy.

Effect of combined use of dressing in nursing care of patients with ICU incontinence associated dermatitis

Objective To explore the effect of dressing combination in ICU patients with incontinence associated dermatitis (IAD). Methods From October 2016 to November 2018, 74 patients with IAD were selected from ICU in Xuanwu Hospital, Capital Medical University, and they were divided into two groups by random number table methed, each group included 37 cases. On the basis of the consistency of skin care, the control group used skin care powder joint skin protective film, the observation group was protected by a combination of hydrocolloid dressing and transparent lamination, foam dressing and transparent lamination, and continued intervention for 7 d. The healing rate, healing time, dressing replacement times and nursing time of IAD in the two groups were compared. Results The healing rate of IAD in the observation group was higher than that in the control group, with statistically significant difference (P < 0.05). And The healing rate of grade 1 IAD in the observation group was higher than that in the control group, but the difference was not statistically significant (P > 0.05). The healing rate of grade 2 IAD in the observation group was higher than that in the control group, with statistically significant difference (P < 0.05). The healing time and healing time of grade 1 and grade 2 IAD in the observation group were shorter than those in the control group, the number of dressing changes and the time spent on care were less than those in the control group, and the differences were statistically significant (all P < 0.05). Conclusion Treating IAD in ICU incontinence associated dermatitis with dressing combination method can improve the healing rate, shorten the healing time, reduce the number of dressing changes and reduce the care workload.

Application of “standard procedure of analgesic care” in postoperative care of disabled elderly patients after hip replacement under multidisciplinary joint model

Objective To explore the application effect of “standard procedure of analgesic care” in the nursing of disabled elderly patients after hip replacement under the model of multidisciplinary association (MDT). Methods From January to December 2018, 40 disabled elderly patients who planned to undergo artificial hip replacement were selected from the Orthopedic Surgery Department of the Chinese Medicine Gezhouba Central Hospital·the Third Clinical Medical College of Three Gorges University (hereinafter referred to as “our hospital”). Random number table method was used to divide the patients into observation group and control group. 19 cases in control group were given postoperative analgesia routine nursing; 21 patients in the observation group adopted MDT model “standard procedure of analgesic care”. Short-term postoperative analgesia efficacy (pain intensity at 12 h, sleep duration at 72 h, time of first getting out of bed, and length of hospital stay), Harris hip score, Barthel index score, SF-36 quality of life score, postoperative complications and other indicators were compared between the two groups. Results There were significant differences in 12 h postoperative pain intensity, 72 h sleep time, first time out of bed, length of stay, Harris hip score, Barthel index score and SF-36 quality of life score at 1 and 3 months after discharge between the two groups (all P < 0.05). There was no significant difference in the incidence of pulmonary infection, constipation, pressure sore, deep vein thrombosis and dislocation of prosthesis between the two groups (P > 0.05). Conclusion MDT “analgesic nursing standard procedure” has definite analgesic effect on incapacitive elderly patients after hip replacement, and has the advantages of safety, reliability and feasibility.

Application effect of predictive nursing guided by risk early warning system in patients with acute respiratory failure in ICU

Objective To explore the effect of predictive nursing guided by risk warning system on the occurrence of ventilator associated pneumonia (VAP) and mechanical ventilation time in patients with acute respiratory failure in ICU. Methods Ninety-eight patients with acute respiratory failure in ICU received in the 4th People′s Hospital of Qinghai Province from January 2017 to January 2019 were divided into control group (49 cases) and study group (49 cases) according to the different nursing intervention schemes. The control group was given routine nursing intervention, while the study group was given predictive nursing under the guidance of risk early warning system on the basis of the control group. The occurrence of VAP and ventilator weaning were observed, the duration of mechanical ventilation, ICU stay and hospital stay were recorded, the blood lactate level, acute physiology and chronic health score (APACHE Ⅱ) were measured. Results The incidence of VAP, ventilator dependence rate and re-intubation rate in the study group were significantly lower than those in the control group (P < 0.05), and the success rate of one-time withdrawal success rate was significantly higher than that in the control group (P < 0.05). The duration of mechanical ventilation, ICU stay and hospital stay in the study group were significantly lower than those in the control group (P < 0.05). After intervention, the blood lactate level and APACHE Ⅱ score in the two groups were lower than those before intervention (P < 0.05), and the blood lactate level and APACHE Ⅱ score in the study group were lower than those in the control group (P < 0.05). Conclusion Predictive nursing guided by risk early warning system can reduce the incidence of VAP in patients with acute respiratory failure in ICU, reduce ventilator dependence, improve prognosis, and have reliable results.

Effect of evidence-based nursing on early drinking water in children after general anesthesia

Objective To explore the effect of evidence-based nursing in early clinical nursing of small amount of water after general anesthesia in children. Methods From July 2017 to January 2018, 167 cases of children undergoing general anesthesia were admitted to the Pediatric Surgery Department of the First Medical Center of the PLA General Hospital. The patients were divided into two groups according to the postoperative drinking water nursing method, in control group, 83 cases were given routine clinical nursing after general anesthesia for children (drinking water 6 h after surgery), in the experimental group, 84 cases were treated with evidence-based nursing (drinking water after awakening under general anesthesia). The effect and safety of the two groups were compared and analyzed. Results The crying rate of children in the experimental group after drinking water was lower than that of the control group, and the difference was statistically significant (P < 0.05). There was no statistically significant difference in vomiting and fever rates between the experimental group and the control group within 6 h after surgery (P > 0.05). Conclusion Evidence-based nursing applied in the early clinical nursing of small amount of water after general anesthesia in children has a definite effect, which can effectively reduce the postoperative crying and other discomfort of children, further reduce the occurrence of postoperative adverse reactions, and is worthy of application and promotion in the postoperative nursing of children.

Innovation of postgraduate training mechanisim of professional degree of Chinese materia medica in military medical university

It is an urgent need for the construction of military medical support force and a new development trend of postgraduate education, as well as a realistic requirement for the new situation of military talent structure adjustment for military medical university to innovate postgraduate training mode of professional degree of Chinese materia medica. However, there existe several problems such as the deviation of ideological understanding, the characteristic insufficience of professional degrees, the military characteristics insufficience of the curriculum system, weak professional practice, and imcomprehensive assessment system in the current postgraduate education of Chinese materia medica. To deal with the problems, we propose the countermeasures such as combining top-level design, innovative training mode, enriching teaching resources, innovating tutor system, and improving the assessment system. It is of great significance to promote postgraduate training of military medical university and build the talent foundation for the cause of Chinese medicine.

Textual research on scrotal diseases recorded in excavated bamboo slips and silk medical books in the Han Dynasty

There are a large number of medical materials recorded in the excavated bamboo slips and silk medical books in the Han Dynasty, among which there are some scrotal disease vocabulary. The methods of textual research are useful to more clearly analyze the principles of Chinese medicine treatment of scrotal diseases. It is found that the use of exegesis examination on scrotal disease helps to clarify the relationship between the scrotum and testicular vocabulary, and reveals the pathogenesis and treatment principles of scrotal disease. At the same time, it is helpful to understand the evolution process and meaning of the words “nang” and “luan”, and to explore the clinical treatment methods of testicular hernia, which is of great benefit to understand scrotal diseases correctly and comprehensively.

Analysis of discharge medication guidances and gender differences for patients with severe mental disorders

Objective To explore the clinical characteristics and gender differences of discharge medication guidance in discharge information sheet of patients with severe mental disorders. Methods The medication guidance on the discharge information sheet of patients with severe mental disorders filled out by psychiatric practitioners in Wuhan Excellent Hospital (“our hospital” for short) in 2018 was taken as the research object. There were a total of 399 discharge information sheets, including 257 male patient discharge information sheets and 142 female patient discharge information sheets. According to the discharge medication guidance filled in by psychiatrists, we conducted a clinical study analysis including gender differences with the antipsychotic drugs used as the main line in this research. Results In 2018, risperidone was the antipsychotic drug with the highest usage rate among patients with severe mental disorders in our hospital, followed by Olanzapine, Clozapine, Aripiprazole, Quetiapine and Sulfamethylate successively. The usage rate of typical antipsychotic drugs was relatively low (accounting for 4.91% of the total number). The proportion of male patients with severe mental disorders receiving antipsychotic treatments with Risperidone and Clozapine was significantly higher than that of female patients (P < 0.01 or P < 0.05). Among the patients receiving oral Risperidone for antipsychotic treatment, there was no statistically significant difference in the dose between male and female patients (0.05 < P < 0.10). The proportion of female patients taking Olanzapine, Aripiprazole, Amisulpiride and Ziprasidone was higher than that of male patients, the differences were statistically significant (P < 0.01 or P < 0.05). The proportion of female patients with severe mental disorders using Aripiprazole for antipsychotic treatment and the mean dose at treatment were higher than those of male patients (all P < 0.05). The proportion of female patients who received Amsulpiride as a major antipsychotic drug was higher than that of male patients (P < 0.01), and the average dose of amsulpiride for female patients in the study was also higher than that for male patients (P < 0.05). There were no statistically significant differences between male and female patients in the use of quetiapine tablets, paliprazone, typical antipsychotics, as well as mood stabilizers, sedative hypnotics, and antidepressants (P > 0.05). Conclusion Atypical antipsychotics are currently the first-line treatment for patients with severe mental disorders. There is a gender difference in the choice of new antipsychotics for patients with severe mental disorders. There is gender difference in the average dose of some antipsychotic. But there is no gender difference in the treatment of emotional stabilizers, sedative hypnotics, and antidepressants.

Exploration and development of the mechanism of standardized training of Chinese medicine resident doctors for the training of clinical master of Chinese medicine

Standardized training of resident doctors has become the leading training mode of professional medical postgraduate in China. The First Affiliated Hospital of Heilongjiang University of Traditional Chinese Medicine (hereinafter referred to as "our hospital") has formulated the corresponding management system and training mode in order to cooperate with the policies and relevant systems established by the state for this new mode, which not only meets the needs of the state, but also highlights the training characteristics of professional postgraduate students in our hospital. This paper mainly discusses the characteristics of standardized training of resident doctors for professional master′s degree students in our hospital from the aspects of system construction, training methods and clinical teaching. Based on the standardized training of resident doctors, the author thinks about the training of professional master′s degree and provides new ideas for the training of professional master′s degree students.

Analysis of overdose toxic Chinese herbal pieces from 1175 prescriptions

Objective To analyze the overdose use of poisonous Chinese herbal pieces in 1175 prescriptions of Pharmacy Department of Beijing Fengtai District Maternity & Baby Care Hospital (hereinafter referred to as “our hospital”), and to standardize the rational use of poisonous Chinese herbal pieces. Methods The 4200 prescriptions from January to December in 2018 in our hospital were counted and analyzed randomly based on the dose regulation by the Pharmacopoeia of the People′s Republic of China (2015 Edition) and Beijing Municipal Regulation Dispensing of Herbal Tablets (2011 Edition). Results The results showed that there were 1175 prescriptions (including oral and external use) containing toxic and mild toxic herbs covering 27.98% of the total prescriptions of which 21 kinds of toxic herbs were overdosed, and among the top frequency herbs were Rhizoma Pinelliae Preparata covering 17.19% of all the herbs. It covers 48.51% of the toxic herbs in the prescriptions containing toxic herbs, and Bitter Apricot Kernel, Radix Aconiti Preparata, Raw Mugwort, Fructus Cnidii were over 40% of the total prescriptions. The prescriptions of toxic herbs were mainly prescribed by Maternity Department, covering 39.71%. Conclusion It is very common that the prescription of overdose toxic Chinese herbal pieces are prescribed in clinic. It is concluded that proper and scientific application of toxic herbs should be practiced and toxic herbs should be strictly controlled to avoid adverse event.
