LI Kairui1 LIU Jie1 LI Xiaona1 HE Yingchun2,3,4 XU Zhaojun5,6 SONG Lan2,3,4
Objective To investigate the effects of oleanolic acid on proliferation and apoptosis of lung adenocarcinoma A549 cells, and to explore its mechanism. Methods A549 cells were divided into control group (DMSO), different concentrations of oleanolic acid group (0.625, 1.25, 2.5, 5, 10 μmol/L) and positive control group (Cisplatin, 0.004 g/L) according to different culture methods. MTT and real-time cellular analysis technology (RTCA) were used to detect cell proliferation. Hoechst 33342 staining, mitochondrial membrane potential test, Annexin V-FITC/PI double fluorescent staining were used to detect cell apoptosis. Western blot was used to detect protein expression. Results MTT assay showed that the proliferation rate of A549 cells treated with oleanolic acid at different concentrations for 24, 48 and 72 hours was lower than that of the control group (P < 0.01). The results of RTCA showed that oleanolic acid could inhibit the proliferation of A549 cells. The results of Hoechst 33342 staining showed that compared with the control group, the fluorescence intensity of each group increased significantly (P < 0.05 or P < 0.01). The results of JC-1 staining showed that compared with control group, the red fluorescence intensity decreased significantly and the green fluorescence intensity increased significantly (P < 0.05 or P < 0.01). Annexin V-FITC/PI double fluorescent staining results showed that compared with the control group, the apoptosis of the treated groups increased significantly (P < 0.05 or P < 0.01). Oleanolic acid can down regulate the expression of anti apoptotic protein XIAP, survivin, Bcl-2, up regulate the expression of apoptotic protein Bax, cleaved casepase 3, cleaved caspase 8. Compared with control group, the difference was statistically significant (P < 0.05 or P < 0.01). Conclusion Oleanolic acid can inhibit the proliferation and induce apoptosis of lung adenocarcinoma cells, and its induction of apoptosis may be through the death receptor pathway or the mitochondrial pathway.

ZHANG Shuyu1,2 XU Zhe3 ZHOU Shouning2 ZHENG Zhuoling2 TANG Shifan2 ZHANG Xi3 HUANG Min2 LI Jiali2
Objective To investigate the effect of VKORC1 and CYP2C9 gene polymorphism on the lasting dose and dose fluctuation of warfarin at the initial anticoagulation phase (90 d after administration). Methods A total of 201 patients who underwent mechanical heart valve replacement in the First Affiliated Hospital, Sun Yat-sen University from June 2014 to December 2016 were recruited. The genotypes of VKORC1 rs7294 and CYP2C9*3 were detected by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP). The effects of VKORC1 and CYP2C9 genotypes on daily warfarin lasting dose (LD) and daily warfarin dose difference (DD) were  analyzed. Results The mutation frequency of VKORC1 rs7294 C>T and CYP2C9*3 was 12.5% and 2.5%, respectively, meeting the hardy weinberg equilibrium. There was no significant effect of CYP2C9*3 on LD and DD (P > 0.05). LD and DD of VKORC1 rs7294 T mutant carriers were significantly higher than that of CC wild homozygous carriers (P < 0.001,P < 0.01). Conclusion The mutation of VKORC1 rs7294 C>T leads to the increase of warfarin anticoagulant lasting dose required by patients, which is more likely to cause dose fluctuation. Therefore, the monitoring of international standardized ratio should be strengthened.

46 Research progress on the correlation between high mobility group box chromosomal protein l and rheumatoid arthritis
LIU Wei1* HUANG Jian1* CHEN Caixia2 WANG Dan3
Rheumatoid arthritis is a chronic non-specific inflammatory disease of the joint synovium with joint lesions as the main manifestation, and the course of disease is progressive. High mobility group box chromosomal protein 1 is involved in the evolution and progression of various autoimmune diseases as a potential inflammation-inducing factor. And the abnormal expression of high mobility group box chromosomal protein 1 is closely related to the occurrence and development of rheumatoid arthritis. This article further elaborates and demonstrates that the expression of high mobility group box chromosomal protein 1 in serum of patients with rheumatoid arthritis promotes the up-regulation of inflammatory factor-related receptors, but its correlation with clinical indicators is still unclear. The expression of high mobility group box chromosomal protein 1 in synovial membrane in patients with rheumatoid arthritis promotes synovial inflammatory expression. High mobility group box chromosomal protein 1 promotes the synthesis and release of inflammatory cytokines, pathological proliferation, apoptosis, autophagy and irreversible progression of phenotypic transformation in synovial fibroblasts in rheumatoid arthritis. In theory, it provides a new target for the clinical diagnosis and disease intervention of rheumatoid arthritis.

127 Relation between traditional Chinese medicine constitution and prevention and treatment of malignant tumor
ZHAO Hongbo GONG Weijun
Malignant tumor has now become the main disease that threatens human health and causes death. How to prevent and treat cancer disease is a common problem faced by human beings. The constitution is closely related to the occurrence, development, and prognosis of malignant tumor. Therefore, the prevention and treatment of tumor disease also needs the participation of traditional Chinese medicine (TCM) constitutional theory. The identification of TCM constitution has great significance for the prevention, treatment and rehabilitation of malignant tumor. This article focuses on the relationship between tumor disease and lopsided-constitution, and how to apply TCM to regulate and improve TCM constitution, so that it tends to be normal-constitution. Normal-constitution is not only beneficial to the prevention of tumor diseases, but also has a certain effect on the rehabilitation of tumor diseases as well as reduce the side effects of chemotherapy. It is worthy of further clinical research.

Serum 25(OH) D3 and IgE levels in the two groups were detected by Enzyme-linked Immuno Sorbent Assay. In addition, according to the grading scheme, the children in the lesion group were divided into 25 cases in the mild lesion group, 48 cases in the moderate lesion group and 27 cases in the severe lesion group. The serum 25(OH)D3 and IgE levels of the three groups were compared and the correlation was analyzed. Results Serum 25(OH)D3 level in the lesion group was lower than that in the control group, while serum IgE level was higher than that in the control group (all P < 0.05). Serum 25(OH)D3 level of RSV bronchitis children in mild, moderate and severe lesion groups decreased gradually, while serum IgE level increased gradually. The statistically significant differences among the groups (all P < 0.05). The severity of RSV bronchitis in children was negatively correlated with serum 25(OH)D3 level (r = -0.629, P < 0.05). It was positively correlated with serum IgE level (r = 0.604, P < 0.05). Conclusion The serum level of 25(OH)D3 in children with RSV bronchiolitis is significantly low expression, while the serum level of IgE is significantly high expression. The above two indicators may be jointly detected in clinical work to help determine the severity of the disease in children with RSV bronchiolitis.

160 Practice and consideration of hospital propaganda performance management
WANG Lei DU Mengkai ZHANG Guojun ZHANG Wei
Taking Xuanwu Hospital of Capital Medical University as an example, the concepts and methods of performance management are applied in hospital propaganda management. The significance, objectives and principles of hospital publicity performance management are clarified from four dimensions including publicity management, publicity workload, we-media construction and public opinion management. A hospital publicity performance evaluation index system covering 20 indicators at 2 levels was constructed. It provides a powerful starting point for the fine management of hospital publicity, forms a large pattern of hospital publicity from horizontal to bottom and vertical to the edge, and forms a closed loop system of continuous optimization of hospital publicity. This paper puts forward the ways and methods to improve the efficiency, quality and effect of hospital propaganda in the new period.

169 Clinical effect of calcium alginate wound dressing on the prevention of complications at the PICC puncture site in patients with lung cancer
WAN Guangming1 WANG Yi1 PAN Luyi1 CHEN Pei2 ZHONG Hua1
Objective To investigate the clinical effect of calcium alginate wound dressing on preventing complications of puncture site of peripherally inserted central catheter (PICC) in patients with lung cancer. Methods A total of 557 patients with lung cancer who received PICC in the Chest Hospital Affiliated to Shanghai Jiao Tong University from January to December 2018 were selected as the study subjects. According to random number table method, the patients were divided into the observation group (n = 278) and the control group (n = 279). The observation group was treated with calcium alginate wound dressing nursing method, while the control group was treated with traditional nursing method. The difference of incidence and degree of complications occurred at the early stage (d≤7) and the number of cases and degree of complications after catheterization, PICC indwelling time, average daily cost, etc were analyzed between the two groups. Results The number of cases of early complications in the observation group was significantly lower than that in the control group, and the difference was statistically significant (P < 0.05). The number of cases of skin injury related to puncture site in the observation group were less than those in the control group, and the difference was statistically significant (P < 0.05). In addition, the observation group was superior to the control group in terms of postoperative pain, adhesion, healing time and scar size (P < 0.05). Compared with the control group, the PICC retention time in the observation group was longer, the average daily cost was less, and the nursing satisfaction was higher (P < 0.05). Conclusion The application of calcium alginate wound dressing after PICC catheterization can significantly reduce the incidence of early and late complications at the puncture site of lung cancer patients, while prolong the duration of PICC indwoundage, and improve postoperative recovery, which is worthy of clinical promotion.

MI Jiaxi LI Ping▲ ZHUANG Junlong
Objective To investigate the preventive effect of standard operating procedure of gradient elastic stockings on deep venous thrombosis of lower extremity after transurethral resection of prostate. Methods A total of 168 patients with transurethral resection of prostate from January 2016 to December 2018 were selected from Affiliated Drum Tower Hospital of Medicine School of Nanjing University, and randomly divided into observation group and control group with 84 patients in each group. Gradient elastic stockings were given to the control group, and the observation group was given the standard operating procedure of the gradient elastic stockings. The incidence of deep venous thrombosis and gastrocnemius pain, D-dimer level, lower limb blood flow velocity and lower limb skin temperature were compared between the two groups. Results The incidence of lower limb deep venous thrombosis and gastrocnemius pain in the observation group were significantly lower than those in the control group, with statistically significant differences (P < 0.05). The postoperative D-dimer level in the observation group was lower than that in the control group, with statistically significant difference (P < 0.05). The flow velocity of superficial femoral vein and popliteal vein of both lower extremities in the observation group were higher than those in the control group, and the skin temperature of both lower extremities in the observation group was higher than that in the control group, with statistically significant differences (P < 0.05). Conclusion The standard operating procedure of gradient elastic socks can effectively prevent the occurrence of lower extremity deep vein thrombosis in patients undergoing transureal prostatectomy, reduce the incidence of gastrocrocker pain and D-dimer level, and improve the venous blood flow rate of lower extremity and skin temperature of lower extremity, which is worthy of promotion.
