Analysis of the medication rules of Professor Yu Rui treated coronary heart disease combined with insomnia by harmonizing liver and heart based on the theory of double heart

Objective To explore the medication rules of Professor Yu Rui treated coronary heart disease combined with insomnia by harmonizing liver and heart based on the theory of double heart. Methods The prescriptions of patients with coronary heart disease and insomnia treated by Professor Yu Rui in the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine and Shenyang Tianren Heyi Intelligent Traditional Chinese Medicine (Internet) Hospital from August 2021 to February 2023 were collected. The traditional Chinese medicine inheritance computing platform V 3.0 was used to perform frequency, four nature, five flavours of drugs, meridian, association rules, and cluster analysis of the traditional Chinese medicine data to excavate the core prescriptions, and the rules of medication were explored. Results A total of 107 effective prescriptions were included, 197 Chinese medicine were used, and 11 Chinese medicine with frequency more than 40 were obtained, mainly for carminative formula, tranquillizing formula, and dissipating phlegm and eliminating blood stasis, etc. The four nature of drugs were mainly warm, cold, and calm drugs. The five flavours of drugs were bitter, sweet, and pungent. The main meridians were liver, spleen, lung, and heart meridians. There were 29 groups of common drug combinations, and 107 prescriptions could be divided into five clusters. Conclusion Professor Yu Rui treated coronary heart disease combined with insomnia with the method of harmonizing liver and heart based on the theory of double heart, and added or subtracted according to clinical syndrome differentiation.

Visual analysis of traditional Chinese medicine in the treatment of pulmonary fibrosis based on CiteSpace

Objective To explore the based on CiteSpace software, this paper discusses the domestic research hotspot and trend of traditional Chinese medicine treatment of pulmonary fibrosis (PF). Methods Literature related to PF treatment in traditional Chinese medicine was retrieved from CNKI, Wanfang Data, VIP, and China Biomedical Literature Database from the establishment of the database to December 2022, and the author, organization and key words were used as nodes for visualization analysis using CiteSpace software. Results A total of 1 408 articles were included, and the number of published papers showed an overall increasing trend; 759 authors were included, forming multiple research teams; a total of 582 key words were included, and the frequency of the top 10 key words were all>30, forming a total of 19 key words clusters. Conclusion The study of traditional Chinese medicine treatment of PF mainly focuses on the pharmacological action of prescription and traditional Chinese medicine monomer and the mechanism of the treatment of PF, it is mainly based on experimental research and clinical observation. At present, medication regularity, lung function and quality of life of patients are the main research hotspots, but there are few studies on pathological mechanisms, and there is less cross-regional and cross-institutional cooperation among domestic authors.

Effect of acupotomy on the cartilage matrix degradation in knee osteoarthristis rabbit model of knee osteoarthristis based on tissue inhibitor of metalloproteinase and matrix metalloproteinases balance

Objective To investigate the effect of acupotomy on cartilage matrix degradation in rabbit model of knee osteoarthristis(KOA) based on the balance between tissue inhibitor of metalloproteinase (TIMP) and matrix metalloproteinases (MMP). Methods Twenty-four healthy male New Zealand rabbits, aged six months and weighing (2.0±0.5) kg were divided into blank group, model group, and acupotomy group by random number table method, with eight rabbits in each group. The KOA rabbit model group was prepared by the modified Videman method, the left hind limb was immobilized with extended plaster for 6 weeks. Acupotomy group was treated four times with acupotomy once a week to loosen tendon nodules. The blank group and model group made the same grab, but does not take any intervention. One week after the intervention, HE staining morphology and saffron solid green staining were used to observe the pathological changes of rabbit cartilage in each group. The expression levels of Collagen Ⅱ, aquaporins(AQP)3, and MMP13 in chondrocytes were detected by RT-PCR, while the expression levels of Collagen Ⅱ, AQP3, TIMP1, and MMP13 were detected by Western blot. Results Compared with blank group, the appearance of cartilage in model group was uneven, the chondrocytes gathered irregularly, some necrotic cells disintegrated, the tide line disorganized, and some broke. The number of chondrocytes decreased significantly, the saffron in cartilage matrix decreased significantly, some cartilage defects, and the tide line was blurred and unclear. Compared with the model group, the appearance of cartilage in acupotomy group was smoother, the structure level was clearer, the cells tended to be neatly arranged, and the tide line was more complete than that in the model group. Not only the amount of cartilage increased, the saffranine coloring in the cartilage matrix was normal, but also the appearance of the cartilage was relatively smooth and smooth, and the tide line was relatively complete. Compared with blank group, the expression of Collagen Ⅱ mRNA in cartilage tissues of model group was down-regulated, while the expressions of AQP3, TIMP1 and MMP13 mRNA were up-regulated (P<0.05). Compared with model group, the expression of Collagen Ⅱ mRNA in cartilage tissues in acupotomy group was up-regulated, while the expressions of AQP3, TIMP1 and MMP13 mRNA were down-regulated (P<0.05). Compared with blank group, the expression of Collagen Ⅱ protein in cartilage tissue of model group was down-regulated, while the expressions of AQP3, TIMP1 and MMP13 protein were up-regulated (P<0.05). Compared with model group, the expressions of Collagen Ⅱprotein in cartilage tissues of acupotomy group were up-regulated, while the expressions of AQP3 and MMP13 protein were down-regulated (P<0.05). There was no significant difference in TIMP1 protein between acupotomy group and model group (P>0.05). Compared with blank group, behavioral scores in model group were significantly increased (P<0.05). Compared with the model group, the behavioral scores of acupotomy group were significantly decreased (P<0.05). Conclusion By regulating the balance of TIMP/MMP, acupuncture treatment can slow down the degradation of cartilage extracellular matrix, thus effectively reducing the level of KOA inflammation.

Exploration of the medication rule for the use of Ding Zhiguo in the treatment of hyperthyroidism based on traditional Chinese medicine inheritance assistance system

Objective To explore the prescription and medication rule of Professor Ding Zhiguo in the treatment of hyperthyroidism with the traditional Chinese medicine inheritance assistance system (V2.5) software. Methods Traditional Chinese medicine prescriptions for the treatment of hyperthyroidism issued by Professor Ding Zhiguo in the thyroidology outpatient Department of Thyropathy Dongzhimen Hospital, Beijing University of Chinese Medicine from July 2019 to November 2020 were collected and a database was established. Frequency statistics, association rules, and so on were used for data mining to analyze and study the core drugs and their nature, flavour, channel tropism, and prescription formation laws. Results A total of 197 prescriptions meeting the criteria were screened out, involving 84 kinds of traditional Chinese medicine, and 22 drugs with frequency ≥110 in the prescriptions. The main channel tropism of drugs were liver and lung channels. The nature and flavour of drugs were bitter and cold respectively. There were 33 couplet medicines with frequency ≥180 and 19 groups of three medicines with frequency ≥185. There were six core prescriptions, six new formula core combinations, and three new prescriptions. The core prescriptions were: Prunellae Spica, Albiziae Flos, Ranunculi Ternati Radix, Bupleuri Radix, Cyperi Rhizoma, Os Draconis, Ostreae Concha, Scutellariae Radix, Platycodonis Radix, Moutan Cortex, Polygoni Multiflori Caulis, Anemarrhenae Rhizoma, Gypsum Fibrosum, Astragali Radix, Paeoniae Radix Rubra, Salviae Miltiorrhizae Radix Et Rhizoma, Taraxaci Herba, Belamcandae Rhizoma, Alismatis Rhizoma, Paeoniae Radix Alba, Margaritifera Concha, Ziziphi Spinosae Semen. Conclusion Professor Ding Zhiguo’s treatment of hyperthyroidism is based on the theory of “Yeben Xiangying”. He is good at using bitter and cold drug to improve the warm-heat disease caused by hyperthyroidism, combined with sweet drug to regulate and tonify qi, blood, yin and yang, sooth liver and strengthen spleen, regulate liver and lung, and treat both symptoms and root causes.

Effects of PLGF gene silencing on proliferation, migration, and invasion of human extravillous cytotrophoblasts and Nrf2/HO-1 signaling pathway in high glucose environment

Objective To investigate the effect of placental growth factor (PLGF) gene silencing on human extravillous cytotrophoblasts (EVCT) in high glucose environment. Methods HTR-8/SVneo cells were cultured in vitro, and the cells were divided into control group (no treatment), empty group (empty plasmid transfection) and PLGF silencing group (PLGF double-incision enzyme plasmid transfection). The cell proliferation, migration, and invasion ability of each group were detected by CCK-8 method, scratch test, and Transwell test; the cell mRNA and protein expressions of PLGF, nuclear factor-erythroid 2-related factor 2 (Nrf2), and heme oxygenase-1 (HO-1) in each group were detected by qRT-PCR and Western blot. Results There were no significant differences in cell proliferation inhibition rate, scratch healing rate, and cell invasion number between control group and empty group (P>0.05); compared with control group, the cell proliferation inhibition rate was increased, the scratch healing rate and the number of invasive cells were decreased in PLGF silencing group (P<0.05). The mRNA and protein expressions of PLGF, Nrf2, and HO-1 were not significantly different between control group and empty group (P>0.05); compared with control group, the mRNA and protein expressions of PLGF, Nrf2, and HO-1 in PLGF silencing group were decreased (P<0.05). Conclusion PLGF gene silencing can inhibit the proliferation, migration, and invasion of EVCT in high glucose environment, and its mechanism may be related to inhibiting the activity of Nrf2/HO-1 signaling pathway.

Visual analysis of acupoint catgut embedding intervention in lipid meta- bolism related diseases based on CiteSpace and VOSviewer

Objective To make a bibliometric analysis of the domestic literatures on the intervention of acupoint catgut embedding in lipid metabolism related diseases in the past 30 years by using knowledge atlas, and to explore the research hotspot and development trend. Methods Literatures on acupuncture point embedding intervention for lipodystrophy were searched from CNKI, Wanfang Data, and VIP databases from January 1992 to September 2022. CiteSpace and VOSviewer software were used to visually analyze the number of publications, authors, institutions, and keywords of relevant literature. Results A total of 821 papers were included, and the number of papers published in this field showed an overall upward trend in the past 30 years. Several stable core research teams represented by Yang Caide’s team have been formed; Nanjing University of Chinese Medicine and Guangzhou University of Chinese Medicine published the most papers, and the research institutions showed regional clustering and weak cooperation. Key words cluster analysis had obtained 11 cluster modules such as obesity, acupoint catgut embedding, postpartum obesity, blood lipid, body weight, etc. The research focus was mainly on the clinical application of acupoint embedding to intervene in lipid metabolism, methodology mining, biological research. Among the 17 emergent keywords, curative effect and abdominal obesity had become the research hotspots in recent years. Conclusion In recent years, the heat of this field has increased, the research scope has gradually deepened, and the future development trend is good. In the later stage, cooperation and communication between teams should be increased to discuss the best treatment plan and materials used, so as to provide references for clinical practice.

Research progress on the mechanism of ferroptosis and its application in myocardial ischemia-reperfusion injury

Ferroptosis is a new type of cell death discovered in recent years, which is usually accompanied by a large amount of iron accumulation and lipid peroxidation during the cell death process. The factors that induce ferroptosis can directly or indirectly affect glutathione peroxidase through different pathways, leading to the decline of cellular antioxidant capacity and the accumulation of lipid reactive oxygen species, and ultimately lead to cellular oxidative death. Studies have shown that ferroptosis is closely related to pathophysiological processes of many diseases, such as tumors and ischemia-reperfusion injury. How to intervene in the occurrence and development of related diseases by regulating cell ferroptosis has become a hotspot and focus of etiological research and treatment, but the functional changes and specific molecular mechanisms of ferroptosis still need to be further explored. This article systematically summarizes the latest progress in the study of ferroptosis, providing reference for further understanding of its pathogenesis, and proposing new targets for treatment of related diseases.

Research progress of nitric oxide in Parkinson disease

The main pathological features of Parkinson disease (PD) are the degeneration and disappearance of dopamine neurons in the substantia nigra pars compacta, reduction of dopamine in the striatum and accumulation of abnormally folded α-synuclein in the cytoplasm of residual neurons in the substantia nigra, forms eosinophilic soma-Lewy bodies. Although the pathogenesis of PD is unknown, the main influencing factors are oxidative stress, inflammatory response, and apoptosis. The action characteristics of nitric oxide (NO) at different levels play different physiological and pathological roles in the influencing factors of PD. During the pathogenesis of PD, high levels of NO have pathogenic effects, while low levels of NO have anti-inflammatory, anti-oxidation, and anti-inflammatory responses, thereby playing a role in repair and protection. Based on the role of NO in PD, this article describes the dual role of NO in PD.

Mechanistic advances in the impairment of synaptic plasticity under learning memory impairment caused by Propofol

Propofol anesthesia can cause learning and memory impairment in rats. The mechanisms involved include synaptic plasticity, oxidative stress, autophagy, and iron death. Damage to synaptic plasticity leads to a reduced ability to learn and an inability to form long-term memories. Restoring synaptic plasticity is an important way to reverse Propofol induced learning and memory impairment. In recent years, epigenetic modifications, expression of synaptic genes and related proteins have played an increasingly important role in promoting the restoration of synaptic plasticity, alleviating neuronal apoptosis, and promoting the formation and consolidation of learning memories. In this paper, we review the above mechanisms to promote the formation and maintenance of synaptic plasticity and alleviate the learning memory impairment caused by Propofol in recent years, in order to provide a theoretical basis for clinical and basic research to alleviate the neurotoxic effects of Propofol.

Research progress on genes related to occurrence and differences in treatment response of chronic obstructive pulmonary disease

Chronic obstructive pulmonary disease (COPD) is a lung disease caused by a combination of genetic and environmental factors, and is characterized by irreversible airflow limitations. In recent years, new discoveries have been made in COPD genetics-related studies, such as the rs8004738 locus of serpin family A member 1, the rs1982073 locus of transforming growth factor β1, protein tyrosine phosphatase non-receptor type 6, telomerase reverse tranasecriptase gene, and matrix metalloproteinases have all been significantly associated with COPD; on this basis, several genes associated with differences in treatment response of COPD were also identified, such as potassium inwardly rectifying channel subfamily J member 2, nuclear receptor subfamily 3 group C member 1, and glucocorticoid inducible transcription factor 1. These newly identified genes and loci provide new insights into the pathogenesis of COPD, provide directions for finding new targets for the diagnosis and treatment of COPD, establish the basis for typing and prediction models for different phenotypes of COPD patients and promote the development of individualized therapy.

Research progress on application and action mechanism of Chaihu Jia Longgu Muli Decoction in neuropsychiatric diseases

Chaihu Jia Longgu Muli Decoction is a classical prescription of traditional Chinese medicine, which has the effects of reconciling Shaoyang, promoting yang circulation and relieving heat, and calming nerves. Modern studies have found that Chaihu Jia Longgu Muli Decoction plays a role in the treatment of neuropsychiatric diseases mainly through the mechanisms of anti-oxidative stress, anti-infection, regulation of neuroplasticity, regulation of neurotransmitters, regulation of intestinal flora, and other mechanisms. Clinically, the prescription has been widely used in the treatment of epilepsy, insomnia, Alzheimer’s disease, depression, Parkinson disease, and other neuropsychiatric diseases with remarkable curative effect. This article reviews the mechanism of action, clinical and experimental studies of Chaihu Jia Longgu Muli Decoction in regulating neuropsychiatric diseases, providing theoretical basis for the in-depth study of Chaihu Jia Longgu Muli Decoction, and clarifying the research direction in future.

Research progress of miRNA in autoimmune hepatitis#br#

MicroRNA (miRNA) can potentially regulate various aspects of cellular function by regulating the expression of multiple genes, which in turn is involved in immune and inflammatory responses in living organisms. Autoimmune hepatitis (AIH) is an immune-mediated chronic inflammatory liver disease. In recent years, miRNA has played an important role in the immune response and disease progression of AIH by regulating multiple inflammatory signaling pathways and influencing the expression of inflammatory factors. Some miRNAs are expected to become a novel non-invasive biomarker for the diagnosis and treatment of AIH. This article will review the research progress of miRNA in AIH, in order to provide new ideas for clinical diagnosis and treatment of AIH.

Research progress on the expression and prognostic value of microRNA-122 in perioperative period of primary liver cancer

Primary liver cancer is one of the most common malignant tumors in clinic. It has the characteristics of poor prognosis and high mortality. With the deepening of research, many new techniques in clinic have been developed in the diagnosis and treatment of liver diseases. Among them, microRNA (miRNA) have become a new target for the treatment of liver diseases due to their high conservation, timing, stability, and tissue specificity. MiRNA-122 is a specific miRNA in liver tissue, which can inhibit the progression of primary liver cancer by regulating the expression of corresponding target molecules in liver cells. In recent years, studies on the value of miRNA-122 in primary liver cancer surgery have been carried out one after another, and they have shown certain advantages. Monitoring miRNA-122 during perioperative period of liver surgery is of great value for disease assessment and prognosis. This article reviews the expression and prognostic value of miRNA-122 in perioperative primary liver cancer.

Satisfaction and its influencing factors of clinical epidemiology teaching effect of medical graduate students in military school based on the training goal of “Four Hui”

Objectives To explore the satisfaction degree and its influencing factors of teaching effect of clinical epidemiology course for medical graduate students in military school based on the training goal of “Four Hui”. Methods A total of 178 medical postgraduates who participated in the clinical epidemiology course based on the training goal of “Four Hui” in the autumn semester of the Graduate School of the Chinese People’s Liberation Army General Hospital in December 2021 were selected as research objects. Online questionnaires were distributed to them about the importance of the course, the reasons affecting the teaching effect, the teaching satisfaction, and the factors influencing the teaching satisfaction, and the teaching effect was analyzed. Further, binary logistic regression model was used to analyze the influencing factors of clinical epidemiology teaching effect based on “Four Hui” training goal. Results A total of 178 questionnaires were sent out in this study, and 160 valid questionnaires were recovered, with an effective recovery rate of 90%. There were statistically significant differences between doctoral students and master students in course interest, course teaching satisfaction, course learning difficulty, number of articles published since admission, and number of articles published in the past one year (P<0.05). The proportion of doctoral students who considered “1 history of epidemiology”, and “7 epidemiology of infectious diseases” important were higher than those of master students, and the differences were statistically significant (P<0.05). The proportion of doctoral students who thought that “R1 too much information in each class” was the reason affecting the teaching effect was lower than that of master students, and the proportion of doctoral students who thought that “R3 clinical epidemiology foundation is too poor” and “R4 is not reviewed in time” were the reason affecting the teaching effect were higher than those of master students, the differences were statistically significant (P<0.05). Age, whether to apply for national entrance examination, “S1 degree of interest in the course”, “S2 cognition of the importance of the course”, “S5 role in clinical research or academic study”, “S6 degree of satisfaction with the course”, “S11 interest in the overall teaching content”, and “S12 wants to increase or reduce the number of hours” were all influencing factors of graduate students’ satisfaction with the teaching of clinical epidemiology courses (P<0.05). The results of multi-factor analysis showed that age, whether to apply for national entrance examination, “S2 cognition of the importance of the course”, and “S6 degree of satisfaction with the course” were independent influencing factors of graduate students’ satisfaction with the teaching of clinical epidemiology courses (P<0.05). Conclusion The teaching of clinical epidemiology should be further improved in the medical education of military schools, and targeted teaching should be carried out according to the age of the students, whether to apply for national entrance examination, the degree of emphasis on the course, and the learning ability.

Microscopical exploration of practical teaching of “medical rescue of chemical agents poisoning”

With the development of medical and health undertakings, the traditional medical education and teaching model has been unable to meet the needs of medical personnel training in the new era. Due to its clear training objectives, prominent focus, timely feedback, and accurate evaluation, microteaching has been applied and promoted in more and more professional practical teaching. By analyzing the present situation and existing problems in the practical teaching of “medical rescue of chemical poison poisoning”, this paper explores the application of microteaching mode is explored. Objective To explore the role of real-time distance teaching system based on 5G intelligent color Doppler ultrasound in interventional ultrasound teaching. Methods A total of 68 students who received standardized resident training in the Department of Ultrasound Medicine, Shuguang Hospital Affiliated to Shanghai University of Traditional Chinese Medicine from May to December 2022 were selected and numbered, with the single number as the study group and the even number as the control group, with 34 students in each group. The study group adopted 5G intelligent color Doppler ultrasound real-time thoracic puncture distance teaching, while control group adopted pre-recorded thoracic puncture operation video explanation. After the completion of the four training sessions, students of two groups were respectively assessed on theoretical knowledge, operational skills, and teaching satisfaction. Results The scores of theoretical knowledge and operational skills of study group were higher than those of control group, and the differences were statistically significant (P<0.05). The levels of theoretical understanding, clinical thinking ability, practical operation ability, teamwork ability, doctor-patient communication ability, and teaching satisfaction of study group were higher than those of control group, and the differences were statistically significant (P<0.05). Conclusion The 5G intelligent color Doppler ultrasound real-time distance teaching system can improve students’ understanding and interest in interventional ultrasound, significantly improve their theoretical level and operational ability, and has good teaching application value.

Focusing on national strategic needs and major scientific issues of traditional Chinese medicine, it is extremely necessary to establish a multidisciplinary integrated research platform for traditional Chinese medicine artificial intelligence. How to integrate artificial intelligence into traditional Chinese medicine discipline has become a key issue that needs to be urgently solved in Chinese medicine colleges and universities. Chinese medicine colleges and universities need to strengthen the construction of intelligent disciplines, explore a new model of “artificial intelligence+” interdisciplinary group construction, cultivate new disciplinary growth points, and promote the benign development of interdisciplinary research. The article proposes a cross integration model for the construction of “artificial intelligence+” disciplines: on the one hand, relying on biomedical engineering discipline, build the direction of basic disciplines of artificial intelligence, promote the connotation construction of artificial intelligence, and consolidate the foundation of artificial intelligence discipline; on the other hand, in combination with the significant demand for the transformation of new and old kinetic energy in Shandong Province and the development layout of key fields of traditional Chinese medicine in the Shandong University of Traditional Chinese Medicine, relying on advantageous disciplines, a new direction of intelligent disciplines in the field of traditional Chinese medicine will be build, the intersection and integration of artificial intelligence discipline construction and traditional Chinese medicine discipline construction will be achieved, and a new cross discipline of traditional Chinese medicine artificial intelligence will be build. In the discipline construction of our university, this model has played a significant role in the discipline construction and provided a useful reference for the construction of “artificial intelligence+” interdisciplinary in traditional Chinese medicine colleges and universities.

Meta-analysis of sperm count and morphological differences between overweight or obesity men and normal men

Objective To evaluate the sperm count and morphological differences between overweight or obesity men and normal men by meta-analysis. Methods The combined search was conducted with keywords such as overweight, obesity, sperm quantity, volume, and morphology, and the search time was set from January 2013 to December 2022 to collect studies on the influence of overweight or obesity on male sperm quantity and morphology by searching Wanfang Data, CNKI, and VIP. A meta-analysis of key data was performed by R 4.2.2 software. Results In this study, 12 literatures with a sample size of 4 299 were included. The results of meta-analysis showed that semen volume (MD=-0.25, 95%CI: -0.44 to -0.06, P=0.010), total sperm count (MD=-21.85, 95%CI: -39.44 to -4.26, P=0.015), normal sperm morphology rate (MD=-1.89, 95%CI: -3.48 to -0.30, P=0.020) of overweight group were lower than those of normal group. The semen volume (MD=-0.31, 95%CI: -0.55 to -0.07, P=0.010), total sperm count(MD=-37.82, 95%CI: -45.80 to -29.84, P< 0.001), sperm concentration (MD=-9.56, 95%CI: -16.03 to -3.10, P=0.004), and normal sperm morphology rate (MD=-2.10, 95%CI: -3.71 to -0.05, P=0.010) of obesity group were lower than those of normal group. The meta-analysis results were robust after the conversion of the effect model, and the Egger test results showed that there was no publication bias in the combined effect size (P>0.05). Conclusion Overweight or obesity men have abnormal sperm count and morphology compared to normal men.

Objective To explore the effect of Zhibai Dihuang Pills combined with Oberning Gel in treating refractory recurrent aphthous ulcer with syndrome of hyperactivity of fire due to yin deficiency in middle-and-old age patients. Methods A total of 47 middle-and-old age patients with refractory recurrent aphthous ulcer with syndrome of hyperactivity of fire due to yin deficiency treated in the Affiliated Hospital of Chengde Medical College from January 2019 to June 2020 were selected. They were divided into control group (Thalidomide + Oberning Gel) in 23 cases and Chinese medicine group (Zhibai Dihuang Pills + Oberning Gel) in 24 cases by random number table method. The treatment course of both groups was four weeks, and the follow-up was once every four weeks after the treatment, for a total of 24 weeks. The local symptom improvement before treatment and after one week of treatment were compared between the two groups, and the therapeutic effect, serum tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6) levels before treatment and after four weeks of treatment were compared between the two groups. The total time without ulcers and the total number of ulcers recurrences were compared between the two groups during the follow-up period, and the occurrence of adverse reactions was recorded between the two groups. Results After one week of treatment, the number of ulcers, the size of ulcers, and the scores of visual analogue scale in two groups were lower than those before treatment, and the differences were statistically significant (P<0.05). After one week of treatment, there were no significant differences in the number of ulcers, size of ulcers, and scores of visual analogue scale between the two groups (P>0.05). The healing time of ulcer in medicine group was longer than that in Chinese control group, and the difference was statistically significant (P<0.05). After four weeks of treatment, the levels of TNF-α and IL-6 were lower than those before treatment, and those of Chinese medicine group were lower than those of control group, the differences were statistically significant (P<0.05). After 24 weeks of follow-up, there was no significant difference in total time without ulcers and the total number of ulcers recurrences between the two groups (P>0.05). There was no significant difference in efficacy between the two groups (P>0.05). The total incidence of adverse reactions in Chinese medicine group was lower than that in control group, and the difference was statistically significant (P<0.05). Conclusion Zhibai Dihuang Pills combined with Oberning Gel in the treatment of refractory recurrent aphthous ulcer with syndrome of hyperactivity of fire due to yin deficiency in middle-and-old age patients has a significant effect on the overall recurrence, can reduce the occurrence of ulcers, prolong the interval time, help to reduce the recurrence rate of ulcers, the effect is similar to Thalidomide, and has fewer adverse reactions, worthy of clinical promotion.

Analysis of influencing factors of non-suicidal self-injury behavior in adolescents with depressive disorder

Objective To explore the influencing factors of non-suicidal self-injury(NSSI) behavior in adolescents with depressive disorder. Methods A total of 314 adolescent patients with depressive disorder were retrospectively selected from the First Hospital of Hebei Medical University from January to October 2022. There were 126 adolescent fathers and 188 adolescent mothers. After evaluation, they were divided into NSSI group and control group. Adolescents self-injury questionnaire, self-rating anxiety scale self-rating depression scale, and family assessment device were used to investigate and compare the differences among groups. Logistic regression was used to analyze the influencing factors of NSSI behavior in depressed adolescents. Results Among 314 adolescents with depressive disorder, a total of 150 had NSSI behavior (47.78%). The scores of self-rating anxiety scale and self-rating depression scale of parents in NSSI group were higher than those in control group, and the differences were statistically significant (P<0.05). The scores of affective communication, positive communication, egoism and family rules and total scores in NSSI group were lower than those in control group, and the differences were statistically significant (P<0.05). Anxiety and depression of parents were the risk factors for NSSI behavior(OR>1,P<0.05), while family function was the protective factor (OR<1,P<0.05). Conclusion The incidence of NSSI in adolescents with depressive disorder is higher, and their NSSI behavior is affected by parental depression, parental anxiety, and family function.

Clinical study of high-frequency repetitive transcranial magnetic stimulation combined with suspension exercise training in treatment of incomplete spinal cord injury

Objective To analyze the efficacy of high-frequency repetitive transcranial magnetic stimulation (HF rTMS) combined with suspension exercise training (SET) in the treatment of incomplete spinal cord injury (ISCI). Methods According to the random number table method, a total of 90 patients with ISCI admitted to Traditional Chinese Medicine Hospital of Lu’an from December 2021 to October 2022 were divided into a combination group (45 cases) and control group (45 cases). SET intervention was given to the control group, and HF rTMS intervention was added to the combined group on the basis of the control group. Both groups were treated for four weeks. The clinical effect was evaluated, and the changes of the score of function independent measure (FIM), simplified McGill pain questionnaire (SF-MPQ), Barthel index (BI), Berg balance scale (BBS), ASIA-lower extremity muscle function score (ASIA-LEMS), and gait parameters were compared before and after the intervention. Results The clinical effect of the combined group was better than that of the control group, and the difference was statistically significant (P<0.05). After intervention, FIM and BI scores of the two groups were higher than those before intervention, while those of the combined groups were higher than those of the control group, and the differences were statistically significant, and the differences were statistically significant (P<0.05). After intervention, the SF-MPQ scores of the two groups were lower than those before intervention, while those of the combined group were lower than those of the control group, and the differences were statistically significant (P<0.05). After intervention, the BBS and ASIA-LEMS scores of the two groups were higher than those before treatment, while those of the combined groups were higher than those of the control group, and the differences were statistically significant (P<0.05). After intervention, the step length, step speed and step frequency of the two groups were higher than those before intervention, while those of the combination group were higher than those of the control group, and the differences were statistically significant (P<0.05). Conclusion HF rTMS combined with SET is effective in the treatment of ISCI patients, which can improve the functional independence of spinal cord, balance ability and independent living ability, and reduce the degree of neuropathic pain.

Meta-analysis of dose-response relationship of noise exposure and risk of hypertension

Objective To investigate the dose-response relationship between noise exposure and the risk of hypertension. Methods Literatures on the dose-response relationship between noise exposure and hypertension published in CNKI, Wanfang Data, VIP, Chinese Biomedical Literature Service System, PubMed, EMBase, Web of Science, Cochrane Library were searched by computer. The search period was from inception to August 2022. Two researchers independently screened literatures, extracted data, and evaluated literatures quality. Stata 16.0 software was used for meta-analysis. Results A total of ten literatures were included, including 214 775 subjects. Meta-analysis showed that noise exposure increased the risk of hypertension (RR=1.31, 95%CI: 1.19-1.45, P<0.001). The results of dose-response meta-analysis showed that there was a nonlinear relationship between noise exposure and hypertension (P<0.05). Compared with 50 dB(A) noise exposure, 40 dB(A) noise exposure reduced the risk of hypertension by 3% (RR=0.97, 95%CI: 0.96-0.98, P<0.001). 45 dB (A) noise exposure reduced the risk of hypertension by 1% (RR=0.99, 95%CI: 0.99-0.99, P<0.001); 55 dB (A) noise exposure increased the risk of hypertension by 3% (RR=1.03, 95%CI: 1.03-1.04, P<0.001); 60 dB (A) noise exposure increased the risk of hypertension by 4% (RR=1.04, 95%CI: 1.03-1.05, P<0.001); 65 dB (A) noise exposure increased the risk of hypertension by 5% (RR=1.05, 95%CI: 1.03-1.06, P<0.001); 70 dB (A) noise exposure increased the risk of hypertension by 5% (RR=1.05, 95%CI: 1.02-1.08, P<0.001); 75 dB (A) noise exposure increased the risk of hypertension by 5% (RR=1.05, 95%CI: 0.99-1.11, P<0.001); 80 dB (A) noise exposure increased the risk of hypertension by 6% (RR=1.06, 95%CI: 0.98-1.15, P<0.001); 85 dB (A) noise exposure increased the risk of hypertension by 9% (RR=1.09, 95%CI: 1.00-1.18, P<0.001); 90 dB (A) noise exposure increased the risk of hypertension by 17% (RR= 1.17, 95%CI: 1.09-1.26, P<0.001); 95 dB (A) noise exposure increased the risk of hypertension by 31% (RR=1.31, 95%CI: 1.20-1.43, P<0.001). The results of funnel plot showed that this study was less affected by publication bias and the results of meta-analysis were more reliable. Conclusion Noise exposure is associated with the risk of hypertension, and a large sample of high-quality literature needs to be included in future studies in order to provide a reliable reference for the prevention of hypertension.

Association of cognitive function with immune biomarkers and comprehensive inflammatory scores in adolescents with unipolar and bipolar depressive disorder

Objective To explore the correlation between cognitive function, immune biomarkers and inflammation score in adolescents with unipolar and bipolar depressive disorder. Methods A total of 58 adolescent patients with unipolar depressive disorder (unipolar group) and 41 patients with bipolar depressive disorder (bipolar group) admitted to Guangdong Provincial People’s Hospital and Zhongshan Third People’s Hospital of Guangdong Province from December 2020 to December 2021 were included. Clinical data of two groups were collected, and the cognitive function was assessed by Stroop color-word test, Wisconsin card sorting test (WCST), and digital span test (DST). Fasting venous blood of the two groups was collected, C-reactive protein levels, platelet, white blood cell, granulocyte, and lymphocyte counts were detected, and immune biomarkers and comprehensive inflammatory scores were calculated. Clinical data, cognitive function, and immune biomarkers and comprehensive inflammatory score were compared between the two groups to analyze the association between cognitive function and immune biomarkers and comprehensive inflammatory score in adolescents with unipolar and bipolar depression. Results The incidence age, frequency of attacks, and positive family history in bipolar group were higher than those in unipolar group, and the course of disease was longer than that in unipolar group, the differences were statistically significant (P<0.05). The correct number of Stroop color-word test, the percentage of correct WCST response, and the DST score in the bipolar group were lower than those in the unipolar group, while the number of Stroop color-word test errors, the number of responses required for WCST to complete the first classification, and the immune biomarkers and comprehensive inflammatory score were higher than those in the unipolar group, the differences were statistically significant (P<0.05). Correlation analysis showed that the number of Stroop color-word test errors was positively correlated with immune biomarkers and comprehensive inflammatory scores (rs>0, P<0.05), and the percentage of correct WCST response in the bipolar group were negatively correlated with immune biomarkers and comprehensive inflammatory scores (r<0, P<0.05). Conclusion Cognitive impairment and inflammatory changes in adolescents with bipolar depression were more severe than those with unipolar depression, and cognitive function was correlated with immune biomarkers and comprehensive inflammatory scores in bipolar depression.

Effect of prone position ventilation in children with PICU ventilator-associated pneumonia#br#

Objective To analyze the effect of prone position ventilation in children with ventilator-associated pneumonia in pediatric intensive care unit (PICU). Methods A total of 120 children with PICU admitted to the Second Affiliated Hospital of Wenzhou Medical University from December 2020 to July 2022 were selected. They were divided into study group (60 cases) and experimental group (60 cases) in alternating order of using mechanical ventilation. The body position was changed every two to three hours, the control group was replaced by supine position and side position, while the experimental group was replaced by supine position, side position and prone position. The incidence of complications, lung function, levels of inflammatory factors, and clinical efficacy of two groups were detected and compared. Results After treatment, the maximum volume of air, forced expiratory volume in one second, forced vital capacity, and maximum expiratory flow in experimental group were higher than those in control group, and the differences were statistically significant (P<0.05). The levels of transforming growth factor-β1, interleukin-6, high mobility group protein box1, and tumor necrosis factor-α in experimental group were lower than those in control group, and the differences were statistically significant (P<0.05). The total complication rate in experimental group was lower than that in control group, and the difference was statistically significant (P<0.05). The total effective rate of treatment in experimental group was higher than that in control group, and the difference was statistically significant (P<0.05). Conclusion After prone position ventilation in PICU, the incidence of ventilator-associated pneumonia is reduced, lung function is improved, inflammation is alleviated, and symptoms are relieved.

Objective To investigate the application effect of Esketamine on neuroendoscopic resection of pituitary tumor by one-and-a-half nostril-sphenoid sinus approach. Methods A total of 98 patients who underwent neuroendoscopic resection of pituitary tumor by one-and-a-half nostril-sphenoid sinus approach from 904th Hospital of Joint Logistic Support Force of Chinese People’s Liberation Army from March 2020 to January 2022 were selected, and they were divided into study group (49 cases) and control group (49 cases) by random number table method. In study group, Esketamine 0.5 mg/kg was injected intravenously before anesthesia induction, and 0.1 mg/(kg·h) Esketamine was pumped continuously during the operation until the end of the operation; and control group was given intravenous saline in the same way. The heart rate (HR) levels of two groups were compared before anesthesia induction (T0), immediately after tracheal intubation (T1), at the end of surgery (T2), 1 h after surgery (T3) and 3 h after surgery (T4); visual analog scale (VAS) scores of T3, T4, and 6 h after surgery (T5) were compared between two groups; the dosage of Remifentanil and Propofol were compared between the two groups; the coughing degree of two groups was compared; and the occurrence of complications during recovery period was observed in the two groups. Results There was significant difference in HR between two groups at different time points within the group (P<0.05); at T1-T4, there were statistical significances in HR between two groups (P<0.05). The VAS scores of two groups at different time points were compared, and the difference was statistically significant (P<0.05); at T3-T5, the VAS scores of study group were lower than those of control group, and the differences were statistically significant (P<0.05). The dosage of Remifentanil and Propofol in study group were lower than those in control group, and the differences were statistically significant (P<0.05). The coughing degree of study group was lower than that of control group, and the difference was statistically significant (P<0.05). There was no significant difference in the total incidence of complications during recovery between two groups (P>0.05). Conclusion Esketamine is effective on neuroendoscopic resection of pituitary tumor by one-and-a-half nostril-sphenoid sinus approach, maintaining hemodynamic stability, safety and reliability.

Western medicine is effective in the prevention and treatment of coronary heart disease, and the methods of drugs, intervention, and surgery are increasingly perfect, but problems such as recurrent angina pectoris under the treatment of standard western medicine and stent thrombosis restenosis still occur, which seriously affect the prognosis and quality of life of patients. Traditional Chinese medicine has played an important role in the prevention and treatment of coronary heart disease. Based on many years of clinical experience, Professor Zhang Limei advocates the use of traditional Chinese and western medicine to prevent and treat coronary heart disease, which has unique characteristics with differential treatment of traditional Chinese medicine. Professor Zhang Limei believes that coronary heart disease is mainly in the heart, which is closely related to the four zang viscera of the lung, spleen, liver, and kidney. Grasp the main pathogenesis of heart and pulse obstruction, distinguish the primary and secondary, treat both manifestation and root cause of disease, and conduct syndrome differentiation treatment with dredging channel blockade to unclog pulse, treat five zang viscera together as the treatment method, and intervene in each stage of coronary heart disease. It embodies the holistic view of traditional Chinese medicine, the characteristics of syndrome differentiation and the preventive treatment of disease idea of “prevention before disease onset, prevent progression after disease onset, prevent recurrence of disease”.

This paper introduces Professor Yao Chunhai’s clinical experience in treating acne of polycystic ovary syndrome (PCOS) with Modified Xiaoyao Powder combined with Guizhi Fuling Pills. The main clinical manifestations of PCOS are oligo-ovulation or anovulation, acne, hirsutism. Acne is one of the most concerned and distressed problems of PCOS patients. Professor Yao Chunhai believes that the main etiology and pathogenesis of acne in women with PCOS is liver depression and qi stagnation. Liver depression and spleen deficiency are the root causes, qi stagnation, blood stasis, and phlegm coagulation are the manifestations. Therefore, the treatment principles established of soothing liver and invigorating spleen, dispelling blood stasis, and removing phlegm, with prescription of Modified Xiaoyao Powder combined with Guizhi Fuling Pills. Meanwhile, paying attention to the changes of yin and yang during patients’ menstrual cycle, and chooseing proper herbs following the growth and decline of yin and yang, the effect is remarkable.

Obesity is a chronic metabolic disease manifested by excess accumulation of fat. Acupuncture has certain advantages in treating obesity. Professor Li Bin inherited famous traditional Chinese medicine experience, combined with long-term clinical practice, summed up the pathological characteristics of obesity with “ess qi, astringent qi, and more turbidity”, expounded specific manifestations, and further puts forward the acupuncture treatment of “regulating qi”, “clearing turbidity”, and “dredging the yangming”. Professor Li Bin believes that the treatment of obesity relies on the combined application of the three. Regulating qi includes supplementing the qi and straightening out the qi of sanjiao. Clearing turbidity includes expelling invisible stagnation heat, removing phlegm and dampness blood stasis. Dredging the Yangming aims to restore the balance of ingestion and expulsion. Professor Li Bin proposed specific acupuncture and moxibustion methods, emphasizing specific acupoint selection according to sanjiao and flexible use of operation with different reinforcing and reducing characteristics.

Clinical effect of umbilical acupuncture assisted treatment in moderate to severe hyperplasia of mammary gland with syndrome of liver depression and qi stagnation

Objective To investigate the clinical effect of umbilical acupuncture assisted treatment in moderate to severe hyperplasia of mammary gland with syndrome of liver depression and qi stagnation. Methods Eighty patients with moderate to severe hyperplasia of mammary gland with syndrome of liver depression and qi stagnation amitted to Nanchang Hongdu Traditional Chinese Medicine Hospital from October 2020 to October 2022 were selected as the study objects, and they were divided into control group and observation group by random drawing method, with 40 cases in each group. Control group was treated with Chaihu Shugan Decoction, observation group was treated with Chaihu Shugan Decoction combined with umbilical acupuncture, both groups were treated for three months. The clinical efficacy and treatment safety of the two groups were evaluated, and the main traditional Chinese medicine syndrome scores, pain degree scores, sex hormone levels, and inflammatory factors levels were compared between the two groups before and after treatment. Results The clinical effect of the observation group was better than that of the control group (P<0.05). After treatment, the scores of distending pain of breasts, hypochondriac distension, dysphoria, and irritability, red tongue and white fur, and visual analogue scale in both groups were lower than those before treatment, and those of observation group were lower than those of control group (P<0.05). After treatment, the serum levels of estradiol (E2), prolactin (PRL), prostaglandin E2 (PGE2), bradykinin (BK), and basic fibroblast growth factor (bFGF) in the two groups were lower than those before treatment, and the levels of luteinizing hormone (LH) were higher than those before treatment, and the serum levels of E2, PRL, PGE2, BK, and bFGF of the observation group were lower than those of the control group, and the serum levels of LH were higher than those of the control group (P<0.05). There was no significant difference in the total incidence of adverse reactions between the two groups (P>0.05). Conclusion Umbilical acupuncture assisted treatment in moderate to severe hyperplasia of mammary gland with syndrome of liver depression and qi stagnation has good effect, can improve the main traditional Chinese medicine syndromes of patients, adjust the level of sex hormones in the body, reduce the level of inflammatory factors, and has good safety.

Clinical effects of Huangqi Jianzhong Decoction combined with warming meridian and ginger therapy in the treatment of gastralgia with deficient cold of spleen and stomach

Objective To analyze the clinical effects of Huangqi Jianzhong Decoction combined with warming meridian and ginger therapy in the treatment of gastralgia with deficient cold of spleen and stomach. Methods A total of 90 patients with gastralgia admitted to the Minzu Hospital Affiliated to Guangxi Medical University from July 2021 to October 2022 were selected and they were divided into three groups by random number table method, with 30 cases in each group. Huangqi Jianzhong Decoction group were treated with Huangqi Jianzhong Decoction, warming meridian and ginger therapy group were treated with warming meridian and ginger therapy, combined group were treated with Huangqi Jianzhong Decoction combined with warming meridian and ginger therapy, and the three groups were treated continuously for 14 days. The clinical efficacy of the three groups was compared, and the traditional Chinese medicine syndrome score, visual analog scale (VAS) score, Chinese version of 36-item short form health survey (SF-36), and safety were compared before and after treatment. Results The clinical efficacy of the combined group was better than that of the Huangqi Jianzhong Decoction group and the warming meridian and ginger therapy group (P<0.05). After treatment, the traditional Chinese medicine syndrome score and VAS score decreased and SF-36 score increased in the three groups compared with those before treatment, and the traditional Chinese medicine syndrome score, VAS score in the combined group were lower than those in the Huangqi Jianzhong Decoction group and warming meridian and ginger therapy group, and the SF-36 scores were higher than those in the Huangqi Jianzhong Decoction group and warming meridian and ginger therapy group (P<0.05). No significant adverse effects were observed in the three groups. Conclusion The clinical efficacy of Huangqi Jianzhong Decoction combined with warming meridian and ginger therapy in the treatment of gastralgia with deficiency cold in the spleen and stomach is definite and can significantly improve the symptoms, pain level, and enhance life treatment of gastroparesis with high safety.

Discussion on Professor Huang Bin’s medication law in treating hypertension based on data mining

Objective To explore the medication rule of Professor Huang Bin in the treatment of hypertension based on traditional Chinese medicine inheritance computing platform (V3.0). Methods The medical records of hypertensive patients treated by Professor Huang Bin in the Department of Outpatient of Henan Hospital of Traditional Chinese Medicine (the Second Affiliated Hospital of Henan University of Chinese Medicine) from June 2019 to December 2022 were collected. The collected medical records were statistically analyzed with the traditional Chinese medicine inheritance computing platform (V3.0), and the prescription characteristics and medication rules of Professor Huang Bin in the treatment of hypertension were explored. Results A total of 340 prescriptions were involved, 196 kinds of traditional Chinese medicine, and the total frequency of use was 4 995 times, and 14 kinds of drugs were used more than 100 times. The four nature of drugs were mainly cold, plain, and warm, and the five flavours were mainly sweet, bitter, and pungent, which mainly return to the liver and spleen channels. Association analysis showed that there were 14 groups of drug core combinations and 13 pairs of high association core couplet medicines. Four core formulas were obtained by cluster analysis. Conclusion Professor Huang’s treatment of hypertension is mainly based on excess syndrome treatment, mostly from phlegm-dampness treatment, taking into account the regulation of emotions.

Analysis of medication rule of Professor Li Zhong in the treatment of colorectal cancer based on data mining

Objective To analyze the medication rule of Professor Li Zhong in the treatment of colorectal cancer. Methods A total of 70 prescriptions of colorectal cancer patients treated by Professor Li Zhong in the Dongzhimen Hospital, Beijing University of Chinese Medicine from December 2018 to December 2021 were collected. The included prescriptions were analyzed by Excel 2010, SPSS Modeler 18.0, and SPSS Statistics 25.0 software. Drug frequency statistics, analysis of four qi properties, five flavors, and meridian tropism association rule analysis, and system cluster analysis were carried out. The core prescriptions, medication rules, and compatibility characteristics of Professor Li Zhong’s prescription for colorectal cancer were systematically summarized, and new prescriptions were explored. Results Totally 70 valid prescriptions were included, involving 161 traditional Chinese medicines. The total frequency of drug use was 2 711 times, and 34 kinds of drugs were used more than 30 times. The top six prescriptions in frequency analysis were milkvetch root, honey-processed milkvetch root, tangshen, smoked plum, liquorice root, and turtle carapace. The drug properties were usually warm, cold, and flat; the five flavors were mainly sweet, followed by bitter and pungent; liver channel, the main channels were lung channel and spleen channel; channel tropism were dominated by liver meridian, lung meridian, and spleen meridian. Association rule analysis and cluster analysis respectively formed 11 groups of strong chain drug combinations and ten categories. Conclusion In the treatment of colorectal cancer, Professor Li Zhong mainly focuses on invigorating the spleen and warming the kidney, promoting blood circulation and dispelling phlegm, softening and dispersing knots, unobstructing qi and clearing cancer toxin. The use of medicine strives to calm and detoxify evil, slowly eliminate accumulation, in order to be flat for a period of time.

Effect of structured report on the diagnosis of Crohn’s disease by CT enterography

Objective To explore the effect of structured report (SR) on the diagnosis of Crohn’s disease (CD) by CT enterography (CTE). Methods A retrospective study was conducted on 223 suspected or confirmed CD patients who underwent both CTE and endoscopic examinations at the Second Affiliated Hospital of Jiaxing University, Zhejiang Province from March 2020 to October 2021. According to the CTE report format, they were divided into SR group (103 cases) and narrative report (NR) group (120 cases). The sensitivity, specificity, negative predictive value, and positive predictive value of CTE reports were compared between two groups using colonoscopy or capsule endoscopy and pathological results as the gold standard; the CTE comprehensive score and Crohn’s disease activity index (CDAI) score reported were compared between two groups, and the correlation between CTE score and CDAI score was analyzed; in the two groups, 25 reports were selected by drawing lots, and the frequency of keyword occurrence was counted; and a questionnaire survey was conducted among seven clinicians to compare their satisfaction with two reporting modes. Results The sensitivity, specificity, positive predictive value, and negative predictive value of CD diagnosis in SR group were higher than those in NR group, and the differences were statistically significant (P<0.05). There was no significant difference in CDAI score between two groups (P>0.05); the CTE score in SR group was higher than that in NR group, and the difference was statistically significant (P<0.05). CTE scores in SR and NR were positively correlated with CDAI scores (r=0.884, 0.651, P<0.05). The most frequent occurrence of SR was the location of disease, the pattern of enhancement, the density of plain scan, the degree of enhancement after enhancement (arterial stage), and the degree of enhancement after enhancement (venous stage), and followed by the thickening, and thickening degree of intestinal disease; the highest frequency of NR occurred in the site of disease, followed by the enhancement mode. The total satisfaction score of SR was higher than NR, and the difference was statistically significant (P<0.05). Conclusion SR can effectively improve the diagnostic efficiency of CTE for CD, reduce the risk of missed diagnosis and misdiagnosis, and provide accurate and efficient evaluation for clinic.

Value of color Doppler ultrasound features in differential diagnosis of br-east asymptomatic inflammatory changes and invasive ductal carcinoma

Objective To analyze the value of color Doppler ultrasound features in differential diagnosis of breast asymptomatic inflammatory changes and invasive ductal carcinoma. Methods Patients with asymptomatic inflammatory changes diagnosed and confirmed by pathology who admitted to Wuxi Maternal and Child Health Hospital Affiliated to Nanjing Medical University from January 2019 to December 2021 were selected as inflammation group (41 cases), and invasive ductal carcinoma patients as tumor group (91 cases). The size, shape, growth orientation, echo, and calcification of the lesions were investigated by color Doppler ultrasonography; Adler semi-quantitative method was used to evaluate the blood flow signal, and the blood flow resistance index (RI) and peak systolic velocity (PSV) were measured; the model equation of color Doppler ultrasonography in diagnosis of invasive ductal carcinoma was analyzed by logistic regression; the receiver operation characteristic curve was drew, the diagnostic value of color Doppler ultrasonography in invasive ductal carcinoma was evaluated. Results There were no significant differences in lesion shape, edge state, and internal echo between two groups (P>0.05); there were significant differences in growth orientation, posterior echo, calcification, Adler grade, RI, and PSV between two groups (P<0.05). The equation for diagnosing invasive ductal carcinoma with color Doppler ultrasound features was Log (P) = 0.654× growth orientation +0.619× posterior echo +0.702× calcification +0.718×Adler grade +0.707×RI+0.668×PSV. The area under the curve of the color Doppler ultrasound characteristic model for diagnosing invasive ductal carcinoma were 0.894 (95%CI: 0.848-0.941), sensitivity was 89.10%, and specificity was 76.80%. Conclusion The logistic regression model based on color Doppler ultrasound features has a high value in the differential diagnosis of breast asymptomatic inflammatory changes and invasive ductal carcinoma.

Objective To analyze the effect of cognitive behavioral group psychological nursing combined with family members’ participation in nursing to improve the psychological state of infertility patients treated by assisted reproductive technology. Methods A prospective study was conducted on 100 patients with infertility who were treated with assisted reproductive technology in Reproductive Medicine Center, Maternity Hospital Affiliated to Nanjing Medical University from December 2021 to June 2022. They were divided into control group and observation group according to random number table method, with 50 cases in each group. Family members participated in nursing in control group. Observation group was combined with cognitive behavioral group psychological nursing on the basis of control group, family function, social supports, and psychological state were compared between the two groups. Results After nursing, the scores of family function and social support in two groups were higher than those before nursing, and the observation group was higher than that of the control group, the differences were statistically significant (P<0.05); the scores of anxiety, depression, and stress in the two groups were lower than those before nursing, and those of the observation group were lower than those of the control group, the differences were statistically significant (P<0.05). Conclusion Cognitive behavioral group psychological nursing combined with family participation in nursing for assisted reproductive technology treatment of infertility patients can improve family function, relieve negative emotions, and improve social support.

Objective To analyze the application effect of “Internet +” health management nursing intervention in patients with atherosclerosis (AS). Methods A total of 100 AS patients who were detected in the Cadre Medical Examination Center, Jiangsu Provincial People’s Hospital from January 2020 to January 2022 were selected, and they were divided into control group and observation group by random number table method, with 50 cases in each group. The control group was given routine nursing intervention, and the observation group was given “Internet +” health management nursing intervention on the basis of the control group. The exercise of self-care agency scale (ESCA) score, self-report psychosocial adjustment to illness scale (PAIS-SR) score and medication compliance were compared between the two groups. Results After nursing, the ESCA scores of the two groups were higher than those before nursing, and that of the observation group was higher than that of the control group (P<0.05). After nursing, the scores of PAIS-SR in the two groups were lower than those before nursing, and that of the observation group was higher than that of the control group (P<0.05). The medication compliance of the observation group was better than that of the control group (P<0.05). Conclusion “Internet +” health management nursing intervention can effectively improve the social adaptation ability, self-care ability and medication compliance of AS patients in health examination centers, and win the recognition and approval of patients.

Application effect of standard cluster nursing on prevention and control of catheter related blood flow infection in critically ill patients under evidence-based concept

Objective To observe the effect of standard cluster nursing on prevention and control of catheter related blood stream infection (CRBSI) in critically ill patients under evidence-based concept. Methods From February 2016 to March 2022, 88 patients with central venous catheterization in the the Second People’s Hospital of Lianyungang were randomly divided into control group and experimental group, with 44 cases in each group. The control group received routine care, while the experimental group received standard cluster care based on evidence-based concept. The incidence of catheter blood flow infection, hospitalization time, incidence of catheter adverse events, and infection- related indicators were compared between the two groups. Results The incidence of CRBSI and hospitalization time in the experimental group were lower than those in the control group, and the differences were statistically significant (P<0.05). The total incidence of adverse events in experimental group was lower than that in control group, and the difference was statistically significant (P<0.05). The body temperature, white blood cell count, and C-reactive protein in the experimental group were lower than those in the control group, and the differences were statistically significant (P<0.05). Conclusion Under the evidence-based concept, standard cluster nursing can significantly reduce the incidence of CRBSI in critically ill patients, reduce the occurrence of adverse events and maintain the stability of infection indicators.

Establishment and application of standardized procedure of bedside power cycling in patients with mechanical ventilation in intensive care unit

Objective To explore the establishment and application of standardized procedure of bedside power cycling in patients with mechanical ventilation in intensive care unit (ICU). Methods A total of 122 patients admitted to the Department of ICU of the Sixth People’s Hospital Affiliated to Shanghai Jiao Tong University were selected as research objects from September 2020 to May 2021, and they were divided into control group and experimental group by random number table method, with 61 cases in each group. The control group received ICU routine treatment, and patients in the experimental group were subjected to bedside power cycling exercise with standardized procedures. After treatment, Medical Research Council (MRC) muscle strength score, activities of daily living Barthel index (BI) mechanical ventilation duration, and satisfaction were compared between the two groups. Results Upon Leaving the ICU, MRC muscle strength score and BI index of two groups were higher than those of before treatment, and those of the experimental group were higher than those of the control group (P<0.05). The length of mechanical ventilation in both groups were longer than those of before treatment, the experimental group was shorter than that of the control group (P<0.05). The satisfaction of the two groups was better than that before treatment, and that of the experimental group was better than that of the control group (P<0.05). Conclusion The establishment of the standardized process of exercise of bedside power bicycle can significantly improve the relevant evaluation indicators of patients, promote the rehabilitation of patients and improve the satisfaction of patients, which is worthy of clinical application.

Effect of comfort care on pain perception and health behavior ability after laparoscopic cholecystectomy

Objective To observe the effect of comfort care on pain perception and health behavior ability after laparoscopic cholecystectomy (LC). Methods A total of 100 middle-aged and elderly patients with laparoscopic LC treated in Lishui District People’s Hospital of Nanjing (hereinafter referred to as “our hospital”) from February 2020 to June 2022 were selected by convenient sampling method as the study objects, comfort nursing service was launched in our hospital in April 2021, and the patients were grouped according to the order of admission, from February 2020 to Aprol 2021, conventional perioperative nursing mode was used as the control group, and from May 2021 to June 2022, comfort nursing was used as the experimental group, with 50 cases in each group. The changes of postoperative pain degree and healthy behavior ability were compared between the two groups. Results The overall analysis showed that there were statistically significant differences in VAS scores between groups, time point comparison and interaction (P<0.05). Further pin-wise comparison, intra-group comparison: the VAS scores of the experimental group were lower at discharge and seven days after discharge than before nursing care, and the VAS scores at seven days after discharge were lower than those at discharge, the differences were statistically significant (P<0.05); comparison between groups: VAS scores of test group at discharge and seven days after discharge were lower than those of control group, the differences were statistically significant (P<0.05). At seven days after discharge, the nutrition, health responsibility and total score of the control group were higher than those before nursing, the nutrition, exercise, health responsible and total score of the experimental group were higher than those before nursing, and the nutrition, psychological well-being, exercise, and total score of the experimental group were higher than those of the control group, the differences were statistically significant (P<0.05). Forced expiratory volume in first second and forced vital capacity in test group were higher than those in control group seven days after discharge, and the differences were statistically significant (P<0.05). Conclusion Comfort nursing for middle-aged and elderly LC can reduce respiratory insufficiency, improve the degree of body pain, and improve the ability of self-health behavior.

Effect of different tube feeding methods on enteral nutrition support intervention in ICU patients with ventilator-assisted ventilation

Objective To investigate the effect of different tube feeding methods on enteral nutrition support intervention in ICU patients with ventilator-assisted ventilation. Methods Ninety-eight patients with ICU ventilator-assisted ventilation treated in Shijingshan Hospital, Beijing Chaoyang Hospital Affiliated to the Capital Medical University from January 2020 to December 2022 were selected and divided into group A (49 cases) and group B (49 cases) according to random number table method. Group A received traditional enteral nutrition support intervention through nasogastric tube, while group B received enteral nutrition support intervention through nasogastric tube. The nutritional status, respiratory function, and complications of the two groups were observed. Results After intervention, haemoglobin and albumin levels in two groups were higher than those before intervention, while those of group B were higher than those of group A, and the differences were statistically significant (P<0.05). After intervention, there was no significant difference in respiration rate between the two groups (P>0.05). The level of tidal volume in two groups was higher than before intervention, and the level in group B was higher than that in group A, and the difference was statistically significant (P<0.05). There was no significant difference in the incidence of diarrhea and pulmonary infection between the two groups (P>0.05). The incidence of food reflux and gastric retention in group B was lower than that in group A, and the difference was statistically significant (P<0.05). Conclusion Compared with the traditional nasogastric tube, nasojejunal nutrition intervention has more advantages and safety in improving the nutritional status and respiratory function of patients treated with ventilator-assisted ventilation in ICU.
