DAI Yi1 ZHANG Zhimin1 YUE Xing1 QING Hengzhen1 YUAN Changshen2 DUAN Kan2 ZHENG Hangbing3
DOI: 10.20047/j.issn1673-7210.2023.25.01
Objective To screen the differentially expressed genes (DEGs) methylated in osteosarcoma (OS) patients related to prognosis based on bioinformatics methods, and to provide a direction for the treatment and research of OS. Methods The OS gene chip (GSE36001) and OS methylation gene chip (GSE36002) were downloaded from the gene expression omnibus database, and the differences were analyzed and pooled to obtain the DEGs. Enrichment analysis of DEGs was performed, and the protein- protein interaction (PPI) network of DEGs was constructed by the STRING database. Cytoscape 3.6 software was used to screen the core genes of PPI network, and survival analysis was performed. Results A total of 532 up- regulated and 281 down-regulated DEGs were identified in differential analysis of GSE36001, and 3 433 hypermethylated DEGs and 2 212 hypomethylated DEGs were identified in differential methylation analysis of GSE36002, after intersection, 128 down-regulated OS hypermethylated DEGs and 44 up-regulated OS hypomethylated DEGs were obtained. OS hypermethylated DEGs were mainly involved in epidermal development and skin cell differentiation, and hypomethylated DEGs were mainly involved in cell differentiation and other biological processes. OS hypermethylated DEGs involved cytokine- cytokine receptor interactions and hypomethylated DEGs involved Ca2+ and other pathways. Eight core genes were identified by PPI analysis. Survival analysis results showed that the survival rate of OS patients with high expression of ALOX5AP, HCK, LAPTM5, RAC2, LCP2, AIF1, TYROBP, and PLEK were higher than those of patients with low expression (P<0.05). Conclusion Eight core methylated DEGs, including ALOX5AP, HCK, and LAPTM5, etc. may affect the prognosis of patients with OS.

ZHOU Dingjie GE Wei CAO Dedong
DOI: 10.20047/j.issn1673-7210.2023.25.02
Objective To construct a senescence-associated secretory phenotype(SASP) related risk model that can independently predict the prognosis of hepatocellular carcinoma (HCC), and to improve the clinical efficacy of HCC. Methods Sequencing and clinical data of HCC were obtained from the cancer genome atlas(TCGA) database, and lncRNA were isolated. SASP related genes with correlation coefficient > seven were searched and screened from the GeneCard website, and genes related to differentially expressed SASP associated lncRNAs were screened using the R language “limma” package. Cox regression analysis and lasso were used to construct the lncRNAs prediction model related to SASP. HCC patients were divided into high-risk group and low-risk group according to the median risk score of the samples. Kaplan-Meier curve was used for survival analysis. Single sample gene set enrichment analysis (ssGSEA) was used to investigate the association between risk scores and immune status in samples. Results A total of 374 tumor samples and 50 normal tissue samples were obtained from the TCGA database. Six lncRNAs associated with SASP were selected form a prognostic model, and the risk scores of each sample were calculated. Survival analysis showed that the prognosis of patients in the low-risk group was better than that in the high-risk group. Area under one-year survival receiver operating characteristic curve of risk model was 0.812. Univariate and multivariate regression analysis showed that patient risk score was an independent risk factor for HCC prognosis (OR > 1, P < 0.05). ssGSEA results showed higher expression levels of immune checkpoint specific targets and associated pathways in the high-risk group. Conclusion In this study, the SASP-related lncRNAs risk model is successfully constructed, and the risk model can independently predict the prognosis of HCC patients.

47 Research progress of LRG1 in glucolipid metabolic diseases
ZHENG Shuyue XIA Yue JIA Jue DENG Xia WANG Dong
DOI: 10.20047/j.issn1673-7210.2023.25.08
Leucine-rich-α-2-glycoprotein 1 (LRG1) is one of the highly conserved member of the leucine-rich repeat protein family. Recent studies have found that LRG1 is a novel adipokine, which is closely related to glycolipid metabolism. In terms of glucose metabolism, LRG1 gene knockout can improve insulin resistance induced by high fat feeding, and LRG1 can inhibit insulin signaling and promote liver gluconeogenesis. In terms of lipid metabolism, LRG1 gene knockout can inhibit lipid synthesis and reduce serum triglyceride and cholesterol levels. In addition, the levels of LRG1 in obese people and obese mice are significantly higher, and the level of LRG1 is significantly correlated with a variety of diabetes complications, suggesting that LRG1 may be a risk factor for obesity and diabetes vascular complications. This article reviews the research progress of LRG1 in glycolipid metabolism and related diseases, in order to provide a new direction for the prediction and treatment of glycolipid metabolism related diseases.

58 Research progress on surgical methods of submandibular gland resection
LI Nian LI Tiancheng
DOI: 10.20047/j.issn1673-7210.2023.25.11
Resection of submandibular gland via neck approach is the preferred treatment for submandibular gland benign tumors, however, there is a risk of injury to the mandibular branch of the facial nerve in this operation, and the neck will leave scars, affecting the appearance of the neck. The increasing aesthetic requirements of patients promote the development of surgical incision of submandibular gland in the direction of beauty and concealment. Endoscopic-assisted surgery can avoid obvious neck scars and help to observe important anatomical structures of the neck. Meanwhile, it is found in surgical practice that the risk of complications is not significant, and the glands and tumors can be completely removed. At present, the application of endoscopic-assisted submandibular gland excision is very limited. This article will review endoscopic submandibular gland surgeries with various approaches, in order to provide reference for clinicians.

NIU Shaoqian WANG Rui YANG Weidong FENG Ling▲
DOI: 10.20047/j.issn1673-7210.2023.25.14
Limb dysfunction is a complication of stroke, which can affect the quality of life. The early post-hospital rehabilitation nursing is an important part of the treatment of stroke complicated with limb dysfunction. However, rehabilitation is a long-term process, and most patients who survive the acute phase will leave sequelae, and the incidence of limb dysfunction is very high, reducing self-care ability and quality of life of patients. At present, our country has not realized the multi-linkage of hospitals, families, and patients, and the community service capacity is relatively insufficient, and the management of chronic diseases is limited. In recent years, transitional nursing model has been proposed, which is a nursing model adopted to ensure the coordination and continuity of nursing work during the transition period of patients at all levels of medical and health care institutions, and is widely used in continuous nursing from hospital to community. This paper reviews the application of this model in limb dysfunction of stroke patients, hoping to provide reference for rehabilitation nursing of patients with this disease.

DOI: 10.20047/j.issn1673-72102023.25.26
This article mainly introduces Professor Zhang Peitong’s experience in the differentiation and treatment of pancreatic cancer from disease-syndrome-symptom mode. Professor Zhang believes that the pathogenesis of pancreatic cancer is based on root deficiency and branch excess. The deficiency should be distinguished the difference between qi, blood, yin, and yang, and the excess should be discriminated between primary and secondary among phlegm, blood stasis, qi stagnation, heat, and toxicity. The treatment focus should be adjusted according to specific situation. The disease location is mainly in spleen, which is closely related to liver. In the disease-syndrome-symptom mode, using self-prescripted Liujun Ermu Decoction can play the functions of strengthening the spleen and removing phlegm, clearing heat and detoxify, promoting blood and qi, which is corresponding to the pathogenesis of pancreatic cancer to achieve early curettage of disease progression. According to the rules of disease progression, the common three stages and six syndromes and corresponding prescriptions are summarized. Accurate differentiation of syndroms is the prerequisite and key to obtain curative effect. For the symptoms and signs that seriously affect the quality of life of patients, timely intervention should be conducted and treatment according to the symptoms can create conditions for subsequent treatment. In clinical practice, the disease-syndrome-symptom mode should be flexibly applied in the whole process of treatment in order to improve the clinical effect of pancreatic cancer.

DOI: 10.20047/j.issn1673-72102023.25.27
This article mainly summarizes the experience of researcher Jiao Yongzheng in the treatment of erectile dysfunction combined with insomnia. Erectile dysfunction and insomnia are common clinical diseases, and they are closely related and often affect each other, which seriously endangers men’s health. Researcher Jiao Yongzheng based on the theory of yin and yang of traditional Chinese medicine, combined with many years of clinical experience, believes that the disharmony between yin and yang is the core pathogenesis of this disease, secondary pathological transformation such as phlegm turbidness, blood stasis, collateral damage, and intermingled deficiency and excess. The four therapeutic principles of clinical diagnosis were put forward, the first is to tonify kidney and calm the mind, harmonize yin and yang; the seconnd  is to invigorate spleen and eliminate phlegm, activate blood and clear collaterals; the third is to fill the essence and nourish collaterals, and moisten convergent tendon; the fourth is to move in order to start the static, and to lead the medicine to the penile. Keep the pathogenesis, play a harmonizing yin and yang, invigorate impotence, and attached a representative test case, in order to provide clinical ideas for reference.

129 Professor Zhao Haibin’s experience in treating chronic heart failure combined with depression based on “yang qi, essence nourishes the spirit, and gentleness nourishes the tendons” theory
LI Dongfang1 WANG Chao1 ZHAO Haibin1 HOU Jiqiu1 DING Wanli2 SHI Jinyu2 GAO Yaru1
DOI: 10.20047/j.issn1673-72102023.25.28
Chronic heart failure combined with depression is a common dual heart disease in clinical practice, and the treatment effect of traditional Chinese medicine is significant. “Yang qi, essence nourishes the spirit, and gentleness nourishes the tendons” is from Suwen·Shengqi Tongtian Lun. Based on the theory, Professor Zhao Haibin thinks that the core pathogenesis of chronic heart failure combined with depression is root deficiency empty and manifestation excess. “Insufficient heart yang” is the root and initial factor of the disease. “Blood stasis”, “retained fluid”, and “depression” are the symptoms and key pathological factors. In terms of treatment, Professor Zhao Haibin advocates “warming yang and benefiting the heart” to treat its root cause, “removing blood stasis and promoting water circulation, nourishing the heart and calming the mind” to heal its manifestation, and the clinical application of Linggui Zhugan Shuangxin Fang has a good therapeutic effect.
