Abstract Objective To study the relationship between serum hepatitis B virus (HBV) load and inflammatory factors, peripheral blood T lymphocyte subsets, and liver enzymes in chronic hepatitis B patients. Methods A total of 85 patients with chronic hepatitis B admitted to Putuo Hospital, Shanghai University of Traditional Chinese Medicine from March 2019 to August 2022 were collected and divided into HBV DNA negative group (29 cases), HBV DNA low load group (28 cases), and HBV DNA high load group (28 cases) according to their HBV DNA expression load. The differences of inflammatory factors, peripheral blood T lymphocyte subsets, and liver enzyme levels among the three groups were compared, and the correlation was further analyzed. Results Interleukin (IL-6), IL-8, IL-10, tumor necrosis factor (TNF), percentage of CD8+, absolute count of CD3+, absolute count of CD8+, glutamic-pyruvic transaminase (GPT), and glutamic-oxaloacetic transaminase (GOT) in HBV DNA low and high load groups were higher than those in HBV DNA negative group, percentage of CD4+, absolute count of CD4+, CD4+/CD8+ were lower than those in HBV DNA negative group (P<0.05). The levels of IL-6, IL-8, IL-10, TNF, GPT, GOT, and percentage of CD8+ in HBV DNA high load group were higher than those in HBV DNA low load group, and the percentage of CD4+, absolute count of CD3+, absolute count of CD4+, absolute count of CD8+, CD4+/CD8+ were lower than those in HBV DNA low load group (P<0.05). Correlation analysis showed that the levels of IL-6, IL-10, and TNF were positively correlated with GPT and GOT in patients with chronic hepatitis B (r>0, P<0.05). Conclusion HBV infection causes changes in the levels of inflammatory factors, T lymphocyte subsets and liver enzymes in patients with chronic hepatitis B. Some inflammatory factors are correlated with liver enzymes, which may promote the immune cascade in the body.
