JI Min1 SU Liping2 LIU Li2 GUAN Yalin1 XIAO Jinling1 PU Hongwei3
DOI: 10.20047/j.issn1673-7210.2023.19.01
Objective To analysis the role of the inhibitor KN-93 to intervene in diacetylmorphine-induced rhythm abnormalities in cardiac muscle cells. Methods Myocardial tissue was extracted from 60 SPF-grade SD rats, regardless of male or female. Primary myocardial cells of SD rats were cultured in vitro and divided into the control group and the experimental group. The control group was treated with 100 μmol/L diacetylmorphine for 24 h, and the experimental group was treated with 100 μmol/L diacetylmorphine +1 μmol/L KN-93 for 24 h. The TMT relative quantitative protein technique was used to screen out the proteins with significant expression difference between the control group and the experimental group. The screening criteria were: up-regulation of expression ploidy > 1.2-fold or down-regulation of expression ploidy < 0.83-fold and P<0.05. The main functions, cellular localization, and biological processes involved in differentially expressed proteins were analyzed by GO functional enrichment. The possible metabolic or signaling pathways involved in differentially expressed proteins were analyzed by KEGG pathway enrichment. Protein interaction network was used to analyze the binding or interaction between proteins. Results Compared with the control group, the phosphorylation level of Ca2+/Calmodulin-dependent protein kinaseⅡ (CaMKⅡ) in the experimental group was decreased, and the difference was statistically significant (P<0.05). A total of 346 differentially expressed proteins were screened by TMT technology, which were mainly involved in the reaction to cytokines and cell response to cytokine stimulation and other processes, and affected molecular functions such as the same protein binding and signal receptor binding, as well as cellular component proteins such as extracellular regions and extracellular regions. It is mainly concentrated in cancer pathway, PI3K-Akt signaling pathway, human papillomavirus infection, Alzheimer disease, and other signaling pathways. The protein interaction network showed that MSH6, NFkB1, and CaMKⅡ might have protein interaction relationship. Compared with the control group, the protein expressions of ADCY7, NFkB1, MSH6, MMP2, MMP9, NQO1, and TXNRD1 in the experimental group were significantly decreased, with statistical significance (P<0.05). Conclusion CaMKⅡ may regulate cardiomyocyte apoptosis through cancer pathway signaling pathway by regulating the changes of NFkB, MSH6 and other proteins, and participate in the abnormal rhythm of cardiac muscle cells induced by diacetylmorphine

49 Advances in the diagnosis and treatment of multiple myeloma after renal transplantation
PENG Yue LI Jinke ZHOU Jiangqiao
DOI: 10.20047/j.issn1673-7210.2023.19.10
Multiple myeloma (MM) is a malignant proliferative disease caused by abnormal hyperplasia of plasma cells in bone marrow. It is characterized by the secretion of monoclonal immunoglobulin and light chain, which can lead to the damage of multiple organs and tissues throughout the body. The kidney is one of the most commonly affected target organs. MM after renal transplantation can cause kidney function impairment in the short term. The mechanism of MM leading to kidney damage is mainly related to the direct toxicity of light chain protein, the deposition of uromodulin, inflammatory reaction fibrosis, and other factors. At present, immunoregulatory drugs and proteasome inhibitors are the main treatment of the disease. Blood purification and autologous hematopoietic stem cell transplantation can also be used. Timely and effective intervention measures are helpful to improve the prognosis. In this paper, the mechanism and diagnosis of kidney injury caused by MM after kidney transplantation is elaborated, the latest clinical strategies and existing problems for the treatment of kidney injury are analyzed, and further research directions are put forward to provide theoretical basis for clinical work.

57 Pathogenesis and treatment of hepatic encephalopathy based on intestinal microbiota
ZHOU Jianbo1 FU Weiqiang1 WU Xiongjian2
DOI: 10.20047/j.issn1673-7210.2023.19.12
Hepatic encephalopathy is a common complication of liver cirrhosis. Cognitive dysfunction and abnormal signs of nervous system are the main clinical manifestations, and the fatality rate is relatively high. Currently, the pathogenesis of hepatic encephalopathy is still unclear and specific treatment is lacking. The intestinal microecosystem of patients with hepatic encephalopathy is dysfunctional, and it may induce hepatic encephalopathy by promoting the production of blood ammonia, releasing inflammatory factors, affecting the production of neurotransmitters, and reducing the synthesis of bile acids and short-chain fatty acids. Therefore, the treatment of hepatic encephalopathy based on intestinal flora has become a new entry point, including the use of probiotics, prebiotics, and antibiotics and fecal microbiota transplantation. This review summarizes the main mechanism of action of intestinal flora in hepatic encephalopathy and related treatments based on intestinal flora, in order to provide a new theoretical basis for clinical treatment of hepatic encephalopathy.

AN Jingwen1 CHANG Yi1 LIU Jing1 XU Yuting1 ZHANG Xiangan2
DOI: 10.20047/j.issn1673-7210.2023.19.13
The pathogenesis of chronic constipation (CC) is still not yet clearly established, and with the deepening of intestinal microecology research, the imbalance of intestinal flora and its metabolites is considered to be the potential predisposing factor of CC. Exercise as one of the basic measures of CC treatment, it can participate in metabolism, anti-inflammatory, oxidative stress, and other processes by regulating the composition of intestinal microorganisms, it is suggested that there is some internal relationship between intestinal flora, exercise, and CC. This paper summarizes and reviews the potential mechanism of exercise to improve CC based on intestinal flora, in order to provide new ideas for further exploration of exercise, prevention and treatment in CC.

65 Effects of catch-up growth of very preterm and extremely preterm infants on physical and neuropsychological behavior development
ZHANG Di LI Lihua▲
DOI: 10.20047/j.issn1673-7210.2023.19.14
With the opening up and implementation of the comprehensive “two child”“three child” policy, China’s birth population will continue to increase, and the birth rate and survival rate of extremely premature and ultra premature infants will also increase year by year. Due to their small age, low birth weight, disability and high mortality, the physical, neuropsychological, and behavioral development of premature infants at small gestational age has become the focus of public health undertakings. Postnatal physical catch-up growth is not parallel to neuropsychological development. Good physical growth does not mean premature infants are ahead in adaptability, movement, language, cognition, and social interaction. This article mainly summarizes the definition and mechanism of postnatal catch-up growth of premature infants, and summarizes the impact of postnatal catch-up growth of ultra early/extremely premature infants on later neuropsychological and behavioral development.

Tic disorder is a common neuropsychiatric disease in clinical practice. The incidence of tic disorder has increased year by year in recent years. Professor Lu Fang, a traditional Chinese medicine master, has achieved good results in the diagnosis and treatment of tic disorder by using traditional Chinese medicine theory. Professor Lu Fang believes that the core disease of this disease is in the liver, which can extend to the heart, lung, and spleen. The core pathogenesis is the internal stirring of liver wind, wind-phlegm invading upward, and the disorder of form and spirit. The main treatment is to suppress hyperactive liver for calming endogenous wind, dissipate phlegm and relieve spasm, and regulate both form and spirit. Professor Lu Fang uses the self-designed Xifeng Zhidong Prescription as the basic prescription of the disease, and according to the characteristics of the disease and syndrome, treatment based on classification differentiation, and finally according to the specific disease and specific prescription, add or change, at the same time emphasizing the importance of life protection in the treatment of disease. This paper summarizes the clinical experience of Professor Lu Fang, a traditional Chinese medicine master, in the treatment of tic disorders with self-designed Xifeng Zhidong Prescription. Case one is attached for proof.

Assisted reproductive technology is one of the modern mainstream diagnosis and treatment methods for the infertility. However, its success rate is low and there are also many complications in the process of follicle extraction and transplantation. Multidisciplinary integration is the trend of current medical development. In this paper, starting from the perspective of acupuncture and moxibustion assisted treatment, according to the reproductive physiological characteristics of in vitro fertilization-embryo transfer at different time nodes, it is divided into six stages: pre-in vitro fertilization conditioning period, pituitary down-regulation period, controlled superovulation period, ovulation period, pre-embryo transfer period, and post-transplantation period. In this paper, staging and syndrome differentiation treatment of traditional Chinese and western medicine is adopted, and a unique set of acupuncture and moxibustion auxiliary treatment program of “nourishing kidney and blood” is constructed from the perspective of acupuncture and moxibustion auxiliary treatment program, in order to provide a relatively safe and effective treatment method to improve the rate of fertilization, embryo implantation rate and pregnancy rate of clinical in vitro fertilization-embryo transfer patients, while reducing the amount of drugs and toxic side effects.
