Study of effects of Anmian Babu Plaster in rats with insomnia induced by p-chlorophenylalanine based on 5-hydroxytryptamine and its receptor

Objective To study the effects of Anmian Babu Plaster in rats with insomnia induced by p-chlorophenylalanine based on 5-hydroxytryptamine (5-HT) and its receptor. Methods A total of 48 SPF SD rats with the body weight of (200±10) g were installed with electroencephalography monitoring electrodes and divided into control group, model group, intervention group, and comfort group according to random number table method, with 12 rats in each group. Model group, intervention group, and comfort group were intraperitoneally injected with 300 mg/(kg·d) p-chlorophenylalanine for two days to make insomnia model, and control group was injected with the same amount of normal saline. After successful modeling, the control group and model group were routinely fed, shénquē in the intervention group were pasted with Anmian Babu Plaster, and shénquē in the comfort group were pasted with comfort plaster, the intervention was lasted for seven days. The sleep-wake accumulations of the four groups were observed. The levels of 5-HT and 5-hydroxyindoleacetic acid (5-HIAA) and the expressions of 5-HT1A receptor (5-HT1AR) and 5-HT2AR in the dorsal raphe nucleus, hypothalamus, and hippocampus of the four groups were detected by enzyme-linked immunosorbent assay and immunohistochemical staining. The expression levels of 5-HT1ARmRNA and 5-H2ARmRNA were measured by RT-PCR. Results The 12 h daytime and 24 h all-day awakening volume of model group were higher than those of control group, while the rapid eye movement (REM) and non-rapid eye movement (NREM) sleep volume of model group were lower than those of control group, and the differences were statistically significant (P<0.05). The 12 h daytime and 24 h all-day awakening volume in the intervention group were lower than those in the model group, while the REM and NREM sleep volume were higher than those in the model group, and the differences were statistically significant (P<0.05). The levels of 5-HT, 5-HIAA, and the expression levels of 5-HT1AR mRNA in dorsal raphe nucleus, hypothalamus, and hippocampus in model group were lower than those in control group, while the expression levels of 5-HT2AR mRNA were higher than those in control group, and the differences were statistically significant (P<0.05). The levels of 5-HT, 5-HIAA, and expression levels of 5-HT1AR mRNA in the dorsal raphe nucleus, hypothalamus, and hippocampus in the intervention group were higher than those in the model group, while the expression levels of 5-HT2AR mRNA were lower than those in the model group, and the differences were statistically significant (P<0.05). The cell membrane staining of 5-HT1AR positive cells in the dorsal raphe nucleus, hypothalamus, and hippocampus were more deep in the control group and the intervention group, while the cell membrane staining was shallow and the number of positive cells was less in the model group. The cell membrane staining of 5-HT2AR positive cells in the dorsal raphe nucleus, hypothalamus, and hippocampus of the control group and the intervention group was shallow and the number was small; the cell membrane staining of positive cells in model group was deep and the number was large. There was no significant difference between the comfort group and the model group in each index(P>0.05). Conclusion Anmian Babu Plaster may play a therapeutic role in rats with insomnia induced by p-chlorophenylalanine by regulating the central expression of 5-HT, 5-HT1AR, and 5-HT2AR.

Study on the regulatory mechanism of modified Yinchenhao Decoction on rats with bronchial asthma damp-heat syndrome based on metabolomics

Objective To investigate the regulatory mechanism of modified Yinchenhao Decoction on rats with bronchial asthma (“asthma”for short) damp-heat syndrome based on metabolomics. Methods A total of 65 SPF male SD rats, 4 weeks old, (180±10)g, were divided into blank group (10 rats), model group (10 rats), western medicine group(15 rats), and traditional Chinese medicine group(15 rats). The damp-heat syndrome model was established by intraperitoneal injection of Ovalbumin Hydroxide saline suspension for sensitization, Ovalbumin to stimulate asthma, high-fat feeding, Escherichia coli gavaged, and placing in a damp-heat environment. After successfully modeling, the traditional Chinese medicine group was given modified Yinchenhao Decoction, the western medicine group was given Dexamethasone, the blank group and the model group were given normal saline for four weeks. The general state of rats was observed, different metabolites were screened, and metabolic pathways were analyzed. Results Compared with model group, after medication, the mental, dietary, and bowel status of traditional Chinese medicine group was improved, the response was sensitive, breathing was gradually stable, and oral and nasal secretions were reduced; the levels of 11, 14, 17-eicosanotrienoic acid, docosahexaenoic acid, docosapentaenoic acid, α-linolenic acid, L-linoleic acid and L-tryptophan, and L-lysine were increased in the traditional Chinese medicine group, while the levels of histidine were decreased. In the metabolic pathway, it has the strongest correlation with linoleic acid metabolism, followed by α-linolenic acid metabolism, arachidonic acid metabolism, tryptophan metabolism, histidine metabolism, arginine, and proline metabolism. Conclusion Modified Yinchenhao Decoction has therapeutic effect on damp-heat syndrome, and the mechanism may be related to the regulation of fatty acid and amino acid metabolism.

Visual analysis of research literature on the intervention of traditional Chinese medicine in necrotizing fasciitis based on VOSviewer and CiteSpace

Objective To visual and analysis the research literature on the intervention of traditional Chinese medicine in necrotizing fasciitis (NF) based on VOSviewer and CiteSpace. Methods Literatures related to traditional Chinese medicine intervention in NF were retrieved and screened from China National Knowledge Infrastructure, China Biomedical Literature Database, VIP and Wanfang Database from January 1982 to May 2022. Integrate the literature information of the number of publications, authors, institutions and so on to draw a visual map, and standardize the co-occurrence analysis and breakout analysis of keywords. Results A total of 354 articles were published. There were 96 core authors, and the author with the highest number of articles was Lu Jingen. The First Affiliated Hospital of Tianjin University of Traditional Chinese Medicine was the core institution. High-frequency keywords included NF, integrated traditional Chinese and Western medicine, and perianal necrotizing fasciitis, etc. Seven clusters were formed, and Chinese medicine lavage, Chinese medicine collapse, poison into the ying blood, and wound healing may be future research hotspots. Conclusion The visual analysis shows the research context of the intervention of traditional Chinese medicine in NF in China. The hot areas involved have been expanding and the research heat has continued to rise.

Visualization analysis of trends in synovial cell and cell scorch studies based on CiteSpcae and VOSviewer

Objective To analyze the current situation of synovial tissue and cell scorch with CiteSpace and VOSviewer software, and to provide ideas for research direction. Methods The literatures related to synovial tissue and cellular scorch death included in China National Knowledge Infrastructure (CNKI) and Web of Science database from March 2010 to March 2022 was searched. NoteExpress and Endnote software were used for screening and transcription. Author co-occurrence analysis, clustering analysis and highlighting analysis of keywords were performed. Results A total of 2 066 articles were included in the literature. There were 1 850 authors and 1 179 core authors. The study was centered on “rheumatoid arthritis” and was closely related to fibroblasts, proliferation, arthritis, and apoptosis. Conclusion In the last 10 years, for synovial cells and cell scorching theme tends to rheumatoid arthritis, signaling pathways, cell proliferation and proliferation, tumor necrosis factor is the direction of future research.

Analysis of chemical constituents of Dracocephalum rupestre Hance based on UHPLC-QE-HRMS/MS method

Objective To study the chemical constituents of Dracocephalum rupestre Hance by the ultra-high performance liquid chromatography-quadrupole electric field orbitrap with high-resolution mass spectrometry (UPLC-QE-HRMS/MS). Methods UHPLC was performed on a Hypersil GOLD aQ (2.1 mm×100 mm,1.9 μm) column at 40℃. The mobile phase was 0.1% formic acid water-acetonitrile with gradient elution at a flow rate of 0.4 ml/min. QE-HRMS/MS was scanned simultaneously by electron spray ionization positive and negative ions; the data were processed by the unknown identification and analysis software and compared with the first-order mass spectrometry quasi-molecular ion peak of the compound reference substance in the library, the main fragment ions of the second-order mass spectrometry and the relevant literature. Results A total of 31 chemical constituents were identified, of which 14 compounds were identified for the first time from Dracocephalum rupestre Hance. Conclusion UPLC-QE-HRMS/MS method can quickly and accurately identify a variety of chemical components in Dracocephalum rupestre Hance, and provides reference for quality control and pharmacodynamic material basis study.

Effect of Yiqi Huoxue Shengji Prescription on refractory wound healing and vessel regeneration

Objective To investigate the effect of Yiqi Huoxue Shengji Prescription on ulcer wound healing and vessel regeneration in diabetic mellitus rats. Methods Sixty male SD rats with SPF grade 281-320 g and age of eight-ten weeks were divided into drug administration group, refractory wound group, normal wound group, and Sijunzi Decoction group according to random number table method, with 15 rats in each group. Among them, the drug administration group, refractory wound group, and Sijunzi Decoction group were prepared into diabetes mellitus model by streptozutocin method, fasting blood glucose >16.7 mmol/L after 48 h, with the occurrence of polyuria and polydipsia, indicating successful modeling. Skin defect wounds were made on the back of all rats. The drug administration group and Sijunzi Decoction group were given 3 ml Yiqi Huoxue Shengji Prescription and Sijunzi Decoction by gavage, respectively, and the other groups were given 3 ml double steaming water by gavage, once in the morning and once in the evening. On the 5th, 10th, and 20th day after surgery, five rats in each group were selected to measure the wound size and calculate the wound healing rate. Hematoxylin-eosin staining was used to observe and calculate the number of capillaries on the ulcer wound. The content of vascular endothelial growth factor (VEGF) in the blood of rats was detected by enzyme-linked immunoadsordent assay. The content of VEGF, transforming growth factor-β1 (TGF-β1), and Smad3 in the local tissue of the wound was detected by Western blot. Results Wound healing rate, serum VEGF expression level, and VEGF protein expression level in local tissue of wound in each group on the 20th day after surgery were higher than those on the 5th and 10th day after surgery; wound healing rate, serum VEGF expression level, and VEGF protein expression level in local tissue of wound in each group on the 10th day after surgery were higher than those on the 5th day after surgery (P<0.05). The protein expression levels of TGF-β1 and Smad3 in drug administration group and normal wound group on the 20th day after surgery were lower than those in this group on the 5th and 10th day after surgery, and the protein expression levels of TGF-β1 and Smad3 in drug administration group and normal wound group on the 10th day after surgery were lower than those in this group on the 5th day after surgery (P<0.05). The protein expression levels of TGF-β1 in refractory wound group on the 20th day after surgery were lower than those on the 5th and 10th day after surgery (P<0.05). The expression level of Smad3 protein in refractory wound group on the 10th and 20th day after surgery were higher than those on the 5th day after surgery (P<0.05). On the 5th day after surgery, the wound healing rate of refractory wound group was lower than that of normal wound group (P<0.05). On the 10th and 20th day after surgery, the wound healing rate in the refractory wound group was lower than that in the normal wound group, and the wound healing rate in the drug administration group was higher than that in the refractory wound group (P<0.05). The serum VEGF expression level and VEGF protein expression level in local wound tissue of refractory wound group were lower than those of normal wound group at all time points after surgery, and the serum VEGF expression level and VEGF protein expression level in local wound tissue of drug administration group were higher than those of refractory wound group (P<0.05). On the 5th and 10th day after surgery, the protein expression levels of TGF-β1 and Smad3 in refractory wound group were lower than those in normal wound group, and the protein expression levels of TGF-β1 and Smad3 in drug administration group were higher than those in refractory wound group (P<0.05). On the 20th day after surgery, the protein expression levels of TGF-β1 and Smad3 in refractory wound group were higher than those in normal wound group, while the protein expression levels of TGF-β1 and Smad3 in drug administration group were lower than those in refractory wound group (P<0.05). Pathological observation showed that the inflammatory cell infiltration and tissue necrosis in the drug administration group were lighter than those in the refractory wound group, and the formation of capillaries was obvious. Conclusion Yiqi Huoxue Shengji Prescription can promote the secretion of VEGF in diabetic rats, regulate the expression of TGF-β1 and Smad3 proteins, promote neovascularization, and then promote the healing of refractory wounds.

Research progress of serum vascular endothelial growth factor in the diagnosis and treatment of colorectal cancer

As the third largest cancer in the world, colorectal cancer has seriously threatened the life and health of Chinese residents. The vascular endothelial growth factor (VEGF) family plays an important role in the progression of colorectal cancer. In the special hypoxic microenvironment of tumors, multiple subtypes of VEGF bind to receptors respectively to affect the generation of tumor blood vessels and lymphatic vessels, thus regulating tumor proliferation and metastasis. Compared with VEGF in tissues, serum VEGF is easier to monitor and also plays a role in the diagnosis and treatment of colorectal cancer. It is not only related to precancerous lesions, diagnosis and stage of colorectal cancer, but also related to patients’ recurrence, survival time, treatment effect and other prognostic conditions, which is expected to become a new important indicator in the diagnosis and treatment of colorectal cancer. This article discusses the research progress of serum VEGF in the diagnosis and treatment of colorectal cancer, and specifically lists its role in the diagnosis and prognosis of colorectal cancer, so as to analyze its further clinical application.

Research progress of monocyte distribution width in early diagnosis and prognostic assessment of sepsis

Sepsis is one of the diseases with high mortality in emergency department and intensive care department, early identification, diagnosis, and intervention can effectively reduce incidence rate and mortality. At present, biomarkers used for sepsis have certain limitations. Monocyte distribution width is a new cell measurement parameter. Studies have found that when monocytes are activated, their morphological heterogeneity can be used for early diagnosis and prognosis evaluation of sepsis. This article will briefly review the morphological changes, detection methods, basic research, and reference range of monocyte distribution width in infection in order to provide clinical reference for the early diagnosis and prognosis evaluation of sepsis.

Clinical and mechanistic advances of chinese medicine intervention in membranous nephropathy

Traditional Chinese medicine can regulate immunity, weaken adverse reactions, and improve renal function from various aspects, and to some certain extent, it can be used as the choice before the clinical application of hormones and immunosuppressants in membranous nephropathy. This article reviews the clinical and experimental research of famous doctors and the intervention of traditional Chinese medicine in membranous nephropathy, aiming to discuss the treatment concepts, methods and intervention mechanisms of traditional Chinese medicine for membranous nephropathy, promote clinical and experimental research, and better guide clinical practice. In recent years, the clinical and experimental research progress of traditional Chinese medicine in the treatment of membranous nephropathy is summarized. Multi-channel intervention of traditional Chinese medicine delays renal fibrosis, anti-inflammatory and oxidative stress, and repairs foot cells, thereby improving the pathological damage of membranous nephropathy. At present, the efficacy of traditional Chinese medicine in the treatment of membranous nephropathy is very good, but it still needs to be supported by large samples and long-term research data, and the drug and formulation mechanism need to be further studied.

Progress in clinical application of intra-operative cell salvage in patients with malignant tumors

With the aggravation of the aging population in China, the incidence rate of malignant tumors remains high. More and more patients with malignant tumors choose surgical treatment, which makes the clinical blood demand increase day by day. In order to relieve the pressure of clinical blood use, various blood protection methods emerge in endlessly. Intra- operative cell salvage (IOCS) is one of the most widely used and cost-effective methods in recent years. It has been listed as a routine measure in major operations such as heart and trauma. Malignant tumors usually have extensive blood supply, large surgical trauma and a large amount of bleeding. Allogeneic blood transfusion is often needed to ensure the safety of patients, but allogeneic blood transfusion is not conducive to the prognosis of patients. Therefore, in recent years, the application of IOCS technology in patients with malignant tumors has been reported, especially the application of combined leukocyte-depleted filter is increasing. This paper reviews the current situation of IOCS in patients with malignant tumors through literature review.

Role of long non-coding RNA-NEF in the genesis and development of hepatocellular carcinoma, gastric carcinoma, and esophageal carcinoma

Long non-coding RNA (lncRNA) are a class of RNAs>200 nucleotides in length that do not encode proteins. They regulate gene expression by interacting with RNA and DNA, and their functions are closely related to subcellular localization. In the nucleus, lncRNA regulate gene expression at the epigenetic and transcriptional levels, and in the cytoplasm, it regulate gene expression at the post-transcriptional and translational levels. As a kind of exosome, the abnormal expression of lncRNA has been confirmed to have tumor inhibition or carcinogenic effect, and play an important role in the biological processes of tumor proliferation, apoptosis, immune escape, and so on. LncRNA-NEF is a newly discovered lncRNA in liver cancer, which can inhibit epithelial mesenchymal transformation and metastasis of liver cancer. Follow-up studies have found that lncRNA-NEF plays a role in a variety of tumors, mainly manifested as a cancer suppressor. The incidence of malignant tumor of digestive system is high, the prognosis is poor and the survival time is short. In this paper, the main characteristics of lncRNA-NEF and the progress of its role in liver cancer, gastric cancer, and esophageal cancer were reviewed, in order to provide a basis for the discovery of new tumor diagnostic markers and prognostic indicators, and provide new ideas for its treatment and explore new targets for tumor treatment.

Research advances in the study of serological markers of cholestasis

The diagnosis of cholestasis is currently facing a major challenge, and the development of new markers has become a hot topic of cholestasis-related research in recent years. Newly discovered serological markers in recent years, including transferrin, high mobility group protein B1, myeloid cell trigger receptors, chitin Plasma-3-like protein-1, cytokeratin, microRNA, long non-coding RNA, circular RNA, lamin B receptor antibody, and anti-Kelch-like 12 protein antibody, IgM, etc. in the diagnosis and prognosis of cholestasis have been summarized in the present manuscript, that are classified into inflammation-related markers, non-coding RNAs, and immunological indicators. The results and value of the research were analyzed and summarized, in order to provide reference for follow-up research.

Epidemiological survey and analysis of related influencing factors of children with perforated appendicitis in Taizhou area

Objective To investigate the epidemiological survey and related high-risk factors of perforated appendicitis in children in Taizhou area, and to provide a basis for effective clinical treatment of perforated appendicitis in children. Methods The clinical data of 529 children with appendicitis hospitalized in pediatric surgery from January 2017 to December 2021 in Taizhou Hospital of Zhejiang Province, Enze Medical Hospital, and Taizhou Central Hospital, Zhejiang Province were selected and divided into 123 cases in the perforated group and 406 cases in the non-perforated group according to whether they were perforated, and the clinical indicators of both groups were collected for independent risk factor analysis of the occurrence of perforated appendicitis. Results There was no statistically significant difference between the two groups when comparing fever, chills, anorexia, abdominal pain, diarrhea, metastatic right lower abdominal pain, abdominal pressure and muscle tension, slow movement, and body anterior drive (P<0.05). The time from onset to surgery, first ultrasound examination, rural proportion of household registration, CT report, and intraoperative finding of appendicular fecal stone, and body mass index were higher in the perforated group than those in the non-perforated appendicitis group, and the differences were statistically significant (P<0.05). Multifactorial logistic results showed that time from onset to surgery (OR[95%CI]: 2.259[1.449-3.519]), first ultrasound examination (OR[95%CI]: 2.347[1.402-3.930]), CT report and intraoperative finding of appendiceal fecal stone (OR[95%CI]: 2.076[1.337-3.225]), and body mass index (OR[95%CI]: 1.943[1.188-3.178]) were risk factors for the development of perforated appendicitis in children (P<0.05). Conclusion Perforated appendicitis in children has several high-risk factors, and health care providers should pay attention to the above-mentioned signs and ancillary tests during the treatment process to avoid progression to critical illness.

Study on the relationship between medical staff’s intrinsic driving and satisfaction with democratic management

Objective To study the relationship between medical staff intrinsic driving and satisfaction with democratic management. Methods Using a whole-group sampling method to survey employee representatives of 22 municipal hospitals in Beijing from October to November 2020, the relationship between internal motivation and satisfaction with democratic management was analyzed using one-way and multi-way logistic regression. Results A total of 2 975 questionnaires were collected, with 2 293 valid questionnaires and an effective rate of 77.1%. The satisfaction scores of the study participants for the openness of the hospital, the labor union, and the staff council were (3.98±0.87), (4.30±0.79), and (4.12±0.79) points, respectively. Diligence, level of gratitude, sense of organizational support, and age (>40 to 50 years old) were factors influencing satisfaction with hospital affairs (OR>1, P<0.05); achievement motivation, due diligence, level of gratitude, sense of organizational support, and political appearance (CCP member), years of work (5-<10 years), and job title (mid-level, deputy high-level) were factors influencing union satisfaction (P<0.05); achievement motivation, due diligence, sense of organizational support and age (>40 to 50 years old), and education level (master’s degree) were factors influencing satisfaction with the union (OR>1, P<0.05). Conclusion The internal drive has a positive impact on the satisfaction of hospital affairs, labor union, and staff council work respectively. We should focus on stimulating the internal drive of medical staff and drive them to actively participate in democratic management.

Prevalence of atopic dermatitis and its influencing factors among preschool children in Chaoshan region in 2020—2021

Objective To study the prevalence of atopic dermatitis (AD) and its influencing factors in preschool children in Chaoshan region from 2020 to 2021. Methods A whole-group sampling method was used to survey 240 preschool children in Chaoshan region from January 2020 to December 2021 to count the prevalence of AD, and a questionnaire was used to investigate and analyze basic information, living habits, skin health, and emollient use, and their influencing factors were analyzed. Results Fifty-seven of the 240 preschoolers had AD and 183 did not, for a prevalence of 23.75%. The AD group had shorter daily outdoor exercise time than the non-AD group, used emollients in a lower proportion than the non-AD group, and had a higher proportion of exposure to stuffed toys, passive smoking, pet ownership, allergic plants near housing, and history of dry skin than the non-AD group, and differences were statistically significant (P<0.05). After logistic regression analysis, exposure to stuffed toys (OR[95%CI] = 3.924[1.707-9.017]), passive smoking (OR[95%CI] = 4.116 [1.791-9.451]), pet ownership (OR[95%CI] = 3.991[1.736-9.172]), allergic plants near housing (OR[95%CI] = 4.272 [1.858-9.817]), and history of dry skin (OR[95%CI] = 5.068[2.205-11.648]) were risk factors for AD (P<0.05); time spent outdoors per day (OR[95%CI] = 0.426[0.185-0.978]), use of emollients (OR[95%CI] = 0.378[0.165-0.869]) were protective factors for AD (P<0.05). Conclusion The prevalence of AD in preschool children in Chaoshan region is high and the influencing factors are complex and diverse, and measures should be taken accordingly to reduce the occurrence of AD.

Survey of health psychological control sources and health promotion behaviors in patients with ulcerative colitis

Objective To analyze the source of health psychological control and health promotion behaviors in ulcerative colitis, and to provide a reference basis for psychological interventions for patients. Methods A questionnaire was applied to 150 patients with ulcerative colitis admitted to the Gastroenterology Ward of the First Affiliated Hospital of Air Force Medical University from January 2021 to March 2022, and the applied questionnaire included the general information questionnaire, the multidimensional health locus of control (MHLC), and the health promotion lifestyle profile Ⅱ (HPLP-Ⅱ), and the scores were tallied after the questionnaires were collected. The findings were also compared with the normative model to analyze the correlation between the source of health psychological control and health promoting behaviors. Results The ulcerative colitis group scored lower than the normative model group for the role of intrinsicity and opportunity, and higher than the normative model group for the influence of potent others, and differences were statistically significant (P<0.05). The scores of self-actualization, health responsibility, exercise and sports, nutrition, interpersonal relationship, and stress management in the ulcerative colitis group were lower than those in the normative group, and the differences were statistically significant (P<0.05). The results of the correlation analysis showed that intrinsicity was positively correlated with self-actualization, exercise, nutrition, and interpersonal relationships (P<0.05); the influence of powerful others was positively correlated with self-actualization, health responsibility, nutrition, and interpersonal relationships (P<0.05); and the role of opportunity was negatively correlated with health responsibility, nutrition, and interpersonal relationships (P<0.05), and positively correlated with stress management (P<0.05). Conclusion In patients with ulcerative colitis, health psychological control sources and health promotion behaviors differ from normal groups, and health psychological control is significantly correlated with health promotion behaviors, so nursing staff need to implement effective interventions to improve health according to the mental health status of patients.

Study on factors affecting postoperative bed rest compliance of patients with knee fractures and nursing countermeasures

Objective To investigate the factors affecting postoperative bed rest compliance in patients with knee fractures and to propose corresponding nursing countermeasures. Methods The clinical data of patients with knee fractures admitted to Cangzhou Hospital of Integrative Traditional Chinese and Western Medicine in Hebei Province from June 2020 to June 2022 who were treated with parallel surgery were retrospectively analyzed, and 200 cases were selected for analysis using convenience sampling method. A self-administered postoperative bedside compliance questionnaire was used to evaluate patients’ postoperative bedside compliance, and compliance scores ≥30 points were included in the compliance group (112 cases) and <30 points in the non-compliance group (88 cases). General patient data were collected, and the factors influencing postoperative bed rest compliance in patients with knee fractures were analyzed using univariate and multifactorial logistic regression, and nursing countermeasures were formulated accordingly. Results The differences were statistically significant of bed rest time, education level, pain level, adaptation level, own awareness, and family support between the two groups (P<0.05). The logistic regression results showed that the duration of bed rest, pain level, education level, adaptation level, own awareness, and family support were the influential factors affecting postoperative bed rest compliance in patients with knee fractures (OR>1, P<0.05). Conclusion Postoperative bedside compliance of patients with knee fractures is influenced by the duration of bed rest, pain level, education level, adaptation level, own awareness, and family support, etc. Health care providers should develop appropriate nursing countermeasures according to risk factors to improve patient compliance and promote postoperative recovery.

Serum 25(OH)D and β2 microglobulin levels in elderly patients with diabetes mellitus complicated with cognitive dysfunction and their clinical significance

Objective To investigate the serum levels of 25- hydroxyvitamin D (25 [OH] D ) and β2 microglobulin (β2-MG) in elderly patients with diabetes mellitus complicated with cognitive dysfunction and their clinical significance. Methods The clinical data of 80 elderly patients with diabetes treated in Yancheng First Hospital Affiliated to Nanjing University School of Medicine from January 2019 to January 2022 were retrospectively analyzed. According to whether the patients had cognitive impairment, they were divided into cognitive impairment group (38 cases) and normal cognitive function group (42 cases). The general data of the two groups were compared, and the levels of blood lipids, blood sugar, 25(OH)D, and β2-MG were detected. Multivariate logistic regression analysis was used to determine the risk factors of cognitive dysfunction in elderly diabetic patients. Receiver operator characteristic curve (ROC) was used to analyze the predictive value of 25(OH)D and β2-MG in elderly diabetic patients with cognitive dysfunction. Results The proportion of patients with hypertension, microangiopathy, leukoencephalopathy, glycated hemoglobin(HbA1c), insulin resistance index(HOMA-IR) and β2-MG in cognitive impairment group were higher than those in normal cognitive function group, and 25(OH)D was lower than that in normal cognitive function group (P<0.05). Logistic regression analysis showed that hypertension (OR=2.714, 95%CI:1.258-5.855), microangiopathy (OR=2.542, 95%CI:1.176-5.495), white matter lesions (OR=2.669, 95%CI:1.176-5.495), HbA1c (OR=3.648, 95%CI:1.483-8.974), HOMA-IR (OR=3.841, 95%CI:1.520-9.706), 25(OH)D (OR=3.144, 95%CI:1.316-7.511) and β2-MG (OR=3.356, 95%CI:1.417-7.948) were influencing factors for cognitive dysfunction in elderly patients with diabetes mellitus (P<0.05). ROC analysis showed that the areas under the curve of 25(OH)D and β2-MG in predicting cognitive dysfunction in elderly diabetic patients were 0.753 and 0.744, respectively (P<0.05). Conclusion Monitoring the level of 25(OH)D and β2-MG have important clinical significance in early recognition of cognitive impairment in elderly diabetes mellitus.

Clinical effect of Somalutide on patients with type 2 diabetes mellitus

Objective To explore the clinical effect of Somalutide on patients with type 2 diabetes mellitus (T2DM). Methods A total of 160 patients with T2DM admitted to Suzhou Hospital Affiliated to Anhui Medical University from January 2019 to December 2021 were selected, and they were divided into observation group and control group by random number table method, with 80 cases in each group. The control group was treated with Metformin orally, the observation group was treated with Somarlutide based on the control group. The treatment cycle of the two groups was three months. Body mass index (BMI), blood glucose indexes (fasting blood glucose [FBG], 2-hour postprandial blood glucose [2hPBG], glycosylated hemoglobin [HbAlc]), blood lipid indexes (total cholesterol [TC], triglyceride [TG], low-density lipoprotein cholesterol [LDL-C], high-density lipoprotein cholesterol [HDL-C]), and serum inflammatory index (the levels of hypersensitive C-reactive protein [hs-CRP], tumor necrosis factor-α [TNF-α], interleukin-6 [IL-6] ) were compared between the two groups before and after treatment, and the incidence of adverse reactions were compared between the two groups. The incidence of adverse reactions was compared between the two groups. Results After treatment, BMI and blood glucose levels in the two groups were lower than before treatment, and the observation group was lower than the control group, the differences were statistically significant (P<0.05). After treatment, TG, TC and LDL-C in two groups were lower than before treatment, and HDL-C was higher than before treatment. TG, TC and LDL-C in observation group were lower than control group, and HDL-C was higher than control group, the differences were statistically significant (P<0.05). After treatment, the levels of inflammatory factors in the two groups were lower than before treatment and the observation group was lower than the control group, the differences were statistically significant (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). Conclusion Somaluptide is more effective in the treatment of T2DM. It can effectively improve blood glucose, blood lipid and inflammatory indicators, reduce the weight of patients without obvious adverse reactions. It is recommended for clinical application.

Effect of adjuvant treatment of Huangkui Capsules on urinary albumin excretion rate and oxidative stress in patients with early diabetic nephropathy

Objective To investigate the effect of Huangkui Capsules on urinary albumin excretion rate and oxidative stress in patients with early diabetic kidney disease. Methods A total of 60 patients with stage Ⅳ diabetic nephropathy admitted and diagnosed in Tangshan People’s Hospital of Hebei Province from December 2019 to December 2021 were selected as the study objects. They were divided into control group and observation group by random number table method, with 30 cases in each group. The control group was treated with Valsartan 80 mg once a day, and the observation group was treated with Huangkui Capsules three times a day on the basis of the control group, and observed for sie mouths weeks. The clinical efficacy, urinary protein excretion rate (UARE), blood creatinine (SCr), blood β2-microglobulin (β2-MG), blood superoxide dismutase (SOD), blood malondialdehyde (MDA) and blood glutathione peroxidase (GSH-Px) before and after treatment were compared between the two groups. Results The clinical efficacy of the observation group was better than that of the control group, and the difference was statistically significant (P<0.05). After treatment, UAER, blood β2-MG, Scr and MDA in two groups were lower than those before treatment, GSH-PX and SOD were higher than before treatment, and UAER, blood β2-MG, Scr and MDA in observation group were lower than control group, while GSH-PX and SOD in observation group were higher than control group, the differences were statistically significant (P<0.05). Conclusion Huangkui Capsules adjuvant treatment can effectively reduce the levels of UARE, β2-MG and Scr in patients with diabetic kidney disease. The mechanism may be that Huangkui Capsules can inhibit oxidative stress by reducing serum MDA, and increasing SOD and GSH-PX, so as to exert renal protective effect.

Objective To observe the effect of magnetic resonance electromagnetic therapy on chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) with damp-heat and blood-stasis and its effect on monocyte chemotactic protein-1 (MCP-1) and vascular cell adhesion molecule (VCAM-1). Methods A total of 110 CP/CPPS patients with damp-heat and blood-stasis treated in Guangdong Provincial Hospital of Chinese Medicine Zhuhai Hospital from January 2021 to January 2022 were selected. They were divided into experimental group and control group by random number table method, with 55 cases in each group. The control group was treated with Qianliebeixi Capsules and the experimental group was treated with magnetic resonance electromagnetic therapy, and the for two courses. National Institutes of Health chronic prostatic inflammation (NIH-CPSI), clinical efficacy, traditional Chinese medicine syndrome score, traditional Chinese medicine syndrome efficacy, prostatic fluid level of VCAM-1, MCP-1, and incidence of adverse reactions were compared between two groups. Results After treatment, the scores of NIH-CPSI and traditional Chinese medicine syndrome of two groups were lower than those before treatment, and experimental group was lower than control group (P<0.05). The clinical efficacy of experimental group was better than that of control group (P<0.05). The traditional Chinese medicine syndrome efficacy of experimental group was better than that of control group (P<0.05). After treatment, the levels of MCP-1 and VCAM-1 in two groups were lower than those before treatment, and experimental group was lower than control group (P<0.05). No adverse reactions such as local pain, rubefaction, blister, and general discomfort were observed in both groups. Conclusion Magnetic resonance electromagnetic therapy has a good effect in treating CP/CPPS with damp-heat and blood-stasis, and its mechanism may be related to regulating MCP-1 and VCAM-1 levels, stimulating immune function and inhibiting inflammation.

Clinical effect of Bushen Jianpi Zhuanggu Decoction on spinal compression fracture

Objective To investigate the effect of Bushen Jianpi Zhuanggu Decoction on spinal compression fracture. Methods Sixty patients with single vertebral compression fracture admitted to Department of Orthopedics, Jiading District Hospital of Traditional Chinese Medicine, Shanghai from March 2019 to May 2021 were selected and divided into treatment group (percutaneous vertebral cement injection combined with oral administration of Bushen Jianpi Zhuanggu Decoction, oral administration of traditional Chinese medicine for four weeks) and control group (percutaneous vertebral cement injection only), with 30 cases in each group. Visual analog scale (VAS) score, Japanese Orthopaedic Association (JOA) low back pain score, and Cobb angle of the two groups were observed before surgery and one week, three months after surgery, and the effects of the two groups and the changes of bone metabolism indexes before treatment and three months after surgery were compared. Adverse reactions during treatment were recorded in both groups. Results VAS score and Cobb angle at one week and three months after surgery were lower than those before surgery, JOA low back pain score was higher than that before surgery, VAS score and Cobb angle at three months after surgery were lower than those at one week after surgery, JOA low back pain score was higher than that at one week after surgery, the differences were statistically significant (P<0.05). The VAS score and Cobb angle of the treatment group at one week and three months after surgery were lower than those of the control group at the same points, and the JOA low back pain score was higher than that of the control group at the same time points, the differences were statistically significant (P<0.05). Three months after surgery, the levels of osteocalcin (OCN) and bone alkaline phosphatase (BALP) were higher than those before surgery, and the level of human cross-linked carboxy-terminal peptide (CTX) was lower than that before surgery, the OCN and BALP of treatment group were higher than those of control group, and CTX was lower than that of control group, the differences were statistically significant (P<0.05). The therapeutic effect of the treatment group was better than that of the control group, and the difference was statistically significant (P<0.05). There was no significant difference in the total incidence of adverse reactions between the two groups (P>0.05). Conclusion Bushen Jianpi Zhuanggu Decoction combined with vertebral bone cement injection in the treatment of spinal compression fracture has an ideal effect on the improvement of lumbago and function, and the improvement of bone metabolism is more obvious.

Hematologic evaluation and short-term clinical effect of reverse needle fixation in high-risk elderly patients with proximal humeral fractures

Objective To investigate the hematologic evaluation and short-term clinical effect of reverse needle fixation in high-risk elderly patients with proximal humeral fractures. Methods The retrospective analysis was performed on 28 high-risk elderly patients with proximal humerus fracture treated by reverse needle puncture external fixation in the First Department of Orthopedics, Hebei Hospital of Traditional Chinese Medicine from January 2018 to January 2022. Data on operative time, intraoperative blood loss, length of stay, removal time of external fixator, and complications were collected. The levels of red blood cell count, hemoglobin, and hematocrit were compared before surgery and seven days, three months, six months after surgery. Constant-Murley score and shoulder surgery system score were used to evaluate the clinical efficacy. Shoulder joint X-ray image was used to observe the degree of fracture healing. Results The mean operation time of the 28 patients was (66.57±18.09) min, the mean intraoperative blood loss was (12.02±2.52) ml, the mean hospital stay was (14.39±2.86) d, and the mean removal time of external fixation was (6.75±1.70) months. No obvious complications occurred. There was no significant difference in the red blood cell count, hemoglobin, and hematocrit before surgery and at seven days, three months, six months after surgery (P>0.05). The total Constant-Murley score and scores of pain, daily activities, range of motion of shoulder joint, and muscle strength were increased gradually before surgery and at seven days, three months, six months after surgery, the differences were statistically significant (P<0.05). The scores of the shoulder surgery scoring system of patients before surgery and at seven days, three months, six months after surgery were gradually decreased, and the differences were statistically significant (P<0.05). X-ray images of the shoulder joint showed good healing. Conclusion The hematologic evaluation of the treatment of high-risk elderly patients with proximal humerus fracture by reverse needle fixation is stable, and the short-term clinical effect after surgery is satisfactory, which is suitable for high-risk elderly patients.

Objective To investigate the therapeutic effect of bone transport and Masquelet induced membrane technique in the treatment of long bone infection combined with large segmental bone defect. Methods Forty patients with long bone infection combined with large segment bone defect admitted to the Second Hospital of Zhangjiakou, Hebei Province from January 2017 to January 2020 were selected as the research objects, and they were divided into observation group (Masquelet induced membrane technique, 20 cases) and control group (bone transport, 20 cases) according to random number table method. The therapeutic effect, number of operations, clinical healing time, full weight-bearing time of lower limbs, treatment cost, and postoperative visual analog scale (VAS) score, Fereadez-Esteve calluses score, Iowa score were compared between the two groups. Complications were recorded in the two groups. Results There was no significant difference in treatment effect between the two groups (P>0.05). The number of operations, and treatment cost in the observation group were lower than those in the control group, clinical healing time, full weight-bearing time of lower limbs in the observation were shorter than those in the control group, the differences were statistically significant (P<0.05). The VAS score of the observation group was lower than that of the control group, and the of Fereadez-Esteve calluses score, and knee, ankle joints scores of Iowa score were higher than those of the control group, the differences were statistically significant (P<0.05). The incidence of complications in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05). Conclusion Masquelet induced membrane technique can effectively accelerate the postoperative recovery of patients with long bone infection combined with large segment bone defect.

Correlation between the level of thrombomodulin during pregnancy and postpartum vascular endothelial function in patients with preeclampsia

Objective To study the correlation between the level of thrombomodulin (TM) during pregnancy and postpartum vascular endothelial function in patients with preeclampsia (PE). Methods A total of 108 PE patients in Huai’an First People’s Hospital, Jiangsu Province from February 2018 to February 2021 were selected. All patients were tested for TM levels during pregnancy, according to endothelial dependent flow mediated dilation value at six weeks postpartum, they were divided into abnormal vascular endothelial function group and normal vascular endothelial function group. The influencing factors of postpartum vascular endothelial dysfunction were analyzed, and the predictive value of TM on vascular endothelial function was evaluated by receiver operating characteristic curve. Results Systolic blood pressure (SBP), diastolic blood pressure (DBP), homocysteine (Hcy), hypersensitive C-reactive protein (hs-CRP), and TM in abnormal vascular endothelial function group were higher than those in normal vascular endothelial function group (P<0.05). TM level during pregnancy was negatively correlated with vascular endothelial function in PE patients (r=-0.416, P<0.05). SBP, DBP, hs-CRP, and TM were the influencing factors of vascular endothelial function (OR=3.592, 2.641, 4.675, 3.872,P<0.05). The cut-off value of TM during pregnancy for predicting postpartum vascular endothelial dysfunction was 44.87 μg/L, area under the curve was 0.865, sensitivity was 85.11%, and specificity was 83.61%. Conclusion The TM level of PE patients during pregnancy is closely related to postpartum vascular endothelial function, which can provide reasonable basis for clinical diagnosis and treatment.

Objective To observe application effect of EEG combined with EMG biofeedback training combined with resistance training in the treatment of spastic cerebral palsy (SCP). Methods A prospective randomized controlled trial was conducted in this study, 107 children with SCP who were admitted to Aviation General Hospital of China Medical University from July 2015 to July 2021 were selected. They were divided into observation group (54 cases) and control group (53 cases) by random number table method. Control group received routine rehabilitation training+EEG combined with EMG biofeedback training, while observation group combined with resistance training on the basis of control group, the intervention lasted for three months. The gross motor function measure (GMFM) score, gastrocnemius and soleus modified ashworth scale (MAS) scores, 6, 10 min walking test, and physiological consumption index (PCI) were compared between two groups before intervention and three months after intervention. Results Three months after intervention, GMFM scores were higher in both groups than those before intervention, and observation group was higher than control group, with astatistical significant differences (P<0.05); MAS scores of gastrocnemius and soleus were lower than those before intervention, and observation group was lower than control group, with statistical significant differences (P<0.05); the 6, 10 min walking tests in both groups were higher than those before intervention, and PCI was lower than that before intervention, and the 6,10 min walking tests in observation group were higher than those in control group, PCI was lower than that in control group, with statistical significant differences (P<0.05). Conclusion EEG combined with EMG biofeedback training combined with resistance training can more effectively improve children’s SCP motor function and muscle tension, significantly improve their walking endurance, and reduce energy consumption during exercise.

Analysis of high risk factors of postoperative postpartum hemorrhage after cesarean section

Objective To analyze the high risk factors of postoperative postpartum hemorrhage after cesarean section. Methods A total of 142 patients who underwent cesarean section in Department of Obstetrics, Qingdao Municipal Hospital from October 2019 to April 2020 were selected. According to whether postoperative postpartum hemorrhage occurred after cesarean section,the patients were divided into occurrence group (72 cases) and non occurrence group (70 cases), and the high risk factors of postoperative postpartum hemorrhage after cesarean section were analyzed. Results There were significant differences in the timing of cesarean section, uterine fatigue, placenta previa, and giant fetus between two groups (P<0.05). Timing of cesarean section, uterine fatigue, placenta previa, and giant fetus were risk factors for postpartum hemorrhage after cesarean section (OR=12.716, 20.259, 5.381, 3.737, P<0.05). Conclusion The occurrence of postoperative postpartum hemorrhage after caesarean section is affected by many factors, so clinical intervention should be carried out according to the influencing factors.

Objective To rapidly identify bile acid component in sheep bile by UPLC-MS/MS. Methods Thermo HYPERSIL GOLD column (2.1 mm×100 mm, 1.9 μm) was used, gradient elution carried out with 0.1% formic acid-methanol as mobile phase, the flow rate was 0.2 ml/min, the column temperature was 40℃; full scanning and secondary mass spectrometry scanning of samples were performed by electrospray ion source in negative ion mode. Results A total of 15 bile acid components were identified, all the four free bile acids were detected in sheep bile for the first time. Conclusion The method developed in this study has the characteristics of high efficiency and sensitivity, which can quickly realize the identification of bile acid component in sheep bile, and provide theoretical basis for further elucidation of its pharmacodynamic substances.

Analysis on the rules of post-infectious cough prescriptions based on Traditional Chinese Medicine Inheritance Computing Platform

Objective To explore the compatibility rules and formulation ideas of traditional Chinese medicine for post- infectious cough and to guide the clinical medication effectively. Methods Literature on the treatment of post-infectious cough by traditional Chinese medicine was retrieved from CNKI, VIP, and Wanfang Data from January 1997 to September 2021, and the database was established. The Traditional Chinese Medicine Inheritance Computing Platform V3.0 was used to carry out statistics of drug frequency, efficacy, nature and flavour, channel tropism and correlation analysis among drugs, and the core drug combination was obtained by k-means clustering analysis. Results A total of 188 prescriptions involving 162 kinds of traditional Chinese medicine were included, and 30 kinds of high-frequency drugs were screened out. The efficacy of 162 traditional Chinese medicine mainly includes dissipating phlegm, relieving cough, relieving dyspnea, relieving superficies, and tonifying deficiency classes. The medicinal natures were mainly warm, cold, and plain, and the medicinal flavours were mainly bitter, pungent, and sweet, and mainly attributed to the lung, spleen, stomach, heart, and liver channels. Through association rule analysis, 24 commonly used drug combinations and 28 groups of associated drugs were obtained. As a whole, the drug combination with “Glycyrrhizae Radix Et Rhizoma-Platycodonis Radix-Asteris Radix Et Rhizoma-Armeniacae Semen Amarum-Schizonepetae Herba-Stemonae Radix-Cynanchi Stauntonii Rhizoma Et Radix- Ephedrae Herba-Citri Reticulatae Pericarpium” as the core. Five core drug groups were obtained based on cluster analysis. Conclusion The traditional Chinese medicine prescriptions for post-infectious cough is characterized by dissipating phlegm, relieving cough, relieving dyspnea, relieving superficies, and tonifying deficiency, combining warm and cold, taking warm medicine as the key, and focusing on the core treatment features of lung qi, which provides theoretical basis for further exploring the clinical medication law of post-infectious cough and modern pharmacological research.

Visual analysis of research literature on Guide to Clinical Practice with Medical Records based on CiteSpace

Objective To explore the current situation, hot spots, and development trend of the literatures research on Guide to Clinical Practice with Medical Records. Methods The research literatures of Guide to Clinical Practice with Medical Records collected by China National Knowledge Infrastructure, Wanfang Date, VIP, and China Biomedical Literature Database from inception to May 2022 were searched by computer. The year of distribution, authors, institutions, and keywords were analyzed visually by CiteSpace software. Results A total of 721 literatures had been included, and the number of published papers had roughly increased exponentially, forming a group cooperation model of “one core and many associates” with Beijing University of Chinese Medicine as the core. However, no core authors in this field had emerged yet. A total of ten high-frequency keywords such as Ye Tianshi, data mining, and medication rule were obtained, ten keyword clusters were obtained, and 19 emergent words were generated. Data mining, medication rule, and differentiation and treatment ranked the first three. Conclusion In the past 60 years, the Guide to Clinical Practice with Medical Records are rich in literature. The current hot spots and trends in the data mining of disease pulse syndrome treatment and medication rules, the future needs to use artificial intelligence and other information technology to conduct knowledge reasoning and discovery research on medical records.

Exploration of the characteristics of treating morbid vaginal discharge in Mizhen Jiyin by Hunan Province’s doctor Zhou Yiguan

Mizhen Jiyin is a monograph on gynecology written by Zhou Yiguan, a physician in the Qing Dynasty, who studied medicine from Zhou Yulun and Chen Shuhui. He specializes in gynecology and is a famous doctor of obstetrics and gynecology in Hunan. The part of the book on the treatment of morbid vaginal discharge has distinctive features. Unlike most physicians in the Ming and Qing Dynasties, Zhou Yiguan had a different understanding of the pathogenesis of morbid vaginal discharge, Zhou Yiguan did not treat morbid vaginal discharge from “dampness”. Instead, he held the view that morbid vaginal discharge was mainly “caused by deficiency”. He pointed out that the location of the disease was in Xiayuan, and the treatment was based on the principle of nourishing Xiayuan, and he created Yizhi Tiandong Siwu Decoction with the main functions of coordinating chong and conception vessels and invigorating kidney and astringent essence. In addition, the book also discusses in detail the identification of morbid vaginal discharge from white turbidity and Baiyin, and the four kinds of critical symptoms of collapsing leucorrhea. Part of the accompanying medicinal song for morbid vaginal discharge adopts the poetic style, and the medicines are simplified the complex, which is easy for beginners to memorize and also reflects the academic characteristics of the author himself who abandons the complicated and reserved the essential. This article discusses the treatment thought, syndrome differentiation, and medication characteristics of Zhou Yiguan’s treatment of morbid vaginal discharge by studying the contents of the book about morbid vaginal discharge, in order to provide new ideas for clinical treatment of morbid vaginal discharge.

Bibliometric analysis of traditional Chinese medicine in the treatment of endometrial cancer based on CiteSpace

Objective To analysis the research status and development trend of traditional Chinese medicine in the treatment of endometrial cancer through bibliometrics. Methods The relevant literatures on the treatment of endometrial cancer by traditional Chinese medicine collected from CNKI, Wanfang Data, and VIP from inception to December 2021 were searched by computer. CiteSpace 5.8 software was used to analyze the number of articles published, the cooperation between authors and institutions, the co-occurrence of keywords, clustering and emergence, and corresponding maps were drawn to explore the research hotspots and trends in this field. Results A total of 162 literatures were included, and the number of publications showed an increasing trend. A total of 303 authors were included in the collaborative network atlas, and the largest team was a multi-person team represented by Xue Xiaoou. A total of 156 institutions were included in the atlas of institutional cooperation network, and Dongzhimen Hospital, Beijing University of Chinese Medicine published the most papers. Key words cluster graph showed that the hot spots of traditional Chinese medicine in the treatment of endometrial cancer mainly focus on related disease research, traditional Chinese medicine pathogenesis and syndrome type, traditional Chinese medicine clinical experience, curative effect observation, and traditional Chinese medicine mechanism research. Conclusion Research on the treatment of endometrial cancer by traditional Chinese medicine has been on the rise, and the research hotspot tends to be traditional Chinese medicine, prescription and medicine, clinical observation, and mechanism research. It is suggested to strengthen institutional cooperation in the future, carry out high-quality clinical research, and mechanism research, and further explore the clinical application value of traditional Chinese medicine in the treatment of endometrial cancer.

Effects of Esomeprazole combined with Shugan Hewei Anshen Prescription on sleep quality and gastrointestinal hormones in patients with refractory gastroesophageal reflux disease with syndrome of heat stagnation in liver and stomach

Objective To investigate the effects of Esomeprazole combined with Shugan Hewei Anshen Prescription on sleep quality and gastrointestinal hormone in patients with refractory gastroesophageal reflux disease with syndrome of heat stagnation in liver and stomach. Methods A total of 120 patients receiving treatment for refractory gastroesophageal reflux disease in Hangzhou Traditional Chinese Medicine Hospital Affiliated to the Zhejiang Chinese Medical University from June 2021 to June 2022 were selected and divided into traditional Chinese medicine group, western medicine group, and combined group according to random number table method, with 40 cases in each group. The traditional Chinese medicine group was given Shugan Hewei Anshen Prescription; the western group was given Esomeprazole; the combination group was given Esomeprazole combined with Shugan Hewei Anshen Prescription, and the three groups were treated for eight weeks. The traditional Chinese medicine syndrome score level was evaluated, the scavenging time, acid reflux times, post-reflux swallow-induced peristaltic wave (PSPW) index, and the levels of motilin (MTL), gastrin (GAS), and vasoactive intestinal peptide (VIP) were detected. The Pitts-burgh sleep quality index (PSQI) was used to assess sleep quality. Results After treatment, the acid reflux score, the retrosternal burning pain score, the irritability score, the two-side swelling score, the scavenging time of food mass, the acid reflux times, the PSPW index level, the VIP level, and the PSQI score of the three groups were lower than those before treatment, and the combined group was lower than the traditional Chinese medicine group and the western medicine group, and the differences were statistically significant (P<0.05). After treatment, the MTL and GAS levels in the three groups were higher than those before treatment, and the combined group was higher than the traditional Chinese medicine group and western medicine group, and the differences were statistically significant (P<0.05). Conclusion After combined treatment with Espromepazole, Shugan Hewei Anshen Prescription, traditional Chinese medicine syndrome integral level of patients with refractory gastroesophageal reflux disease with syndrome of heat stagnation in liver and stomach can be reduced, esophageal dynamic function can be restored, gastrointestinal hormone levels can be improved and sleep quality can be improved.

Clinical effect observation of Cangfu Daotan Decoction on the treatment of simple obesity with dyslipidemia of phlegm dampness type complicated with dyslipidemia

Objective To survey the clinical effect of Cangfu Daotan Decoction on the treatment of simple obesity with dyslipidemia of phlegm dampness type complicated with dyslipidemia. Methods A total of 72 cases of simple obesity with dyslipidemia of phlegm dampness type were selected from the outpatients or inpatients of Department of Endocrinology of Longhua Hospital Affiliated to Shanghai University of Traditional Chinese Medicine from September 2018 to January 2020. They were divided into control group and treatment group according to random number table method, with 36 cases in each group. The control group were treated with basic treatment and Metformin, the treatment group were treated with Cangfu Daotan Decoction based on the control group. Both groups were treated for 12 weeks. Body mass index (BMI), abdominal circumference, insulin resistance index (HOMA-IR), blood lipid, traditional Chinese medicine syndrome score and clinical efficacy of the two groups before and after treatment were observed. After treatment, BMI, abdominal circumference, HOMA-IR, blood lipid and traditional Chinese medicine syndrome score in both groups were lower than those before treatment, and the treatment group was lower than the control group (P<0.05). After treatment, total cholesterol, triglycerides, and low density lipoprotein cholesterol in both groups were decreased than those before treatment (P<0.05), but there was no significant difference between the two groups (P>0.05). The clinical effect of the treatment group was better than that of the control group (P<0.05). Conclusion Cangfu Daotan Decoction can reduce the abdominal and waist circumference of simple obesity with dyslipidemia of phlegm dampness type, effectively improve the lipid metabolism and insulin resistance, and improve the clinical symptoms of the patients.

Objective To explore visual analysis of research hotspots and trends of traditional Chinese medicine blood turbidity theory based on CiteSpace knowledge map. Methods From inception to July 2022, the published literatures by China National Knowledge Infrastructure on the theory of blood turbidity in traditional Chinese medicine were retrieved. CiteSpace drawed a map of relevant knowledge map through the use of metrological tools. Document keywords, authors, and publishing institutions were visually analyzed. Results A total of 158 relevant literatures had been included, and the annual volume of literatures was on the rise as a whole. The largest number of literatures were issued in 2015 and 2021, with 18 literatures each. Among them, Wang Xinlu and Wang Dongxian had the largest number of individual literatures, and form a high-yield academic team centered on Professor Wang Xinlu. Shandong University of Traditional Chinese Medicine was the largest number of publications in the field of blood turbidity. Hot keywords formed 11 clusters. Conclusion The visual atlas shows the history and development of blood turbidity theory in traditional Chinese medicine, which is in its steady development stage. The main research hotspots and trends in recent years are theoretical discussion, traditional Chinese medicine treatment methods and modern clinical research.

Application effect of Cox health behavior interaction combined with emotion theory in secondary prevention of congenital heart disease after interventional surgery

Objective To explore the application effect of Cox health behavior interaction combined with emotion theory in secondary prevention of congenital heart disease after interventional surgery. Methods A total of 102 children with congenital heart disease admitted to the First Hospital of Hebei Medical University from November 2018 to November 2021 were selected as the research objects. The patients were divided into reference group and study group, with 51 cases in each group. The reference group was given affective theory intervention, and the study group was given Cox health behavior interaction intervention. The effects of mental health scale (MHS-CA), general amount scale of family caregiver task inventory (FCTI), and cardiac rehabilitation were compared between the two groups. Results After intervention, the scores of MHS-CA scale in both groups were lower than those before intervention, and the study group was lower than the reference group, and the differences were statistically significant (P<0.05); the FCTI scale of both groups was higher than those before intervention, and the research group was higher than the reference group, and the differences were statistically significant (P<0.05). The left ventricular ejection fractions of the study group was higher than that of the reference group, while the left ventricular end diastolic diameter of the study group was lower than that of the reference group, and the differences were statistically significant (P<0.05). Conclusion Cox health behavior interaction combined with emotion theory can improve the negative emotions and cardiac rehabilitation effect of children with congenital heart disease after interventional surgery, improve the ability of family members to self-care.

Application effect of enhanced recovery after surgery nursing model after laparoscopic surgery for gallstones with cholecystitis

Objective To explore the application effect of enhanced recovery after surgery (ERAS) nursing model after laparoscopic surgery for gallstones with cholecystitis. Methods A total of 80 patients with gallstones with cholecystitis who underwent laparoscopic treatment in Anqing First People’s Hospital of Anhui Province from January 2021 to February 2022 were selected, according to random number table method, the patients were divided into control group and experimental group, with 40 cases in each group. The control group was given conventional nursing mode, and the experimental group was given ERAS nursing mode. Postoperative recovery, numeric rating scales (NRS), Wong-Baker facial pain score, short form-36 of health survey (SF-36) score, complications and satisfaction with nursing were compared between the two groups. Results The feeding, first exhaust, first defecation, getting out of bed, and discharge time of experimental group were shorter than those of control group, and the differences were statistically significant (P<0.05). The overall analysis showed that there were statistically significant differences in pain scores of NRS and Wong-Baker facial makeup among groups, time points and interaction (P<0.05). Further pairwise comparison and intra-group comparison: NRS and Wong-Baker facial pain scores at time points in the two groups showed statistically significant differences (P<0.05); comparison between groups: NRS and Wong-Baker facial pain scores of experimental group were lower than those of control group at 4, 12 h and 24 h after surgery, and the differences were statistically significant (P<0.05); the SF-36 score of experimental group was higher than that of control group, and the difference was statistically significant (P<0.05). The total complication rate of experimental group was lower than that of control group, and the difference was statistically significant (P<0.05). Nursing satisfaction of experimental group was better than that of control group, and the difference was statistically significant (P<0.05). Conclusion The application of ERAS nursing model after laparoscopic surgery has significant effects on patients with gallstones and cholecystitis, promoting the rapid postoperative recovery of patients, accelerating the process of rehabilitation, effectively alleviating postoperative pain and reducing the occurrence of complications, and significantly improving the postoperative quality of life and satisfaction of patients.

Application of core stability training combined with pain nursing in patients after hip arthroplasty

Objective To explore application of core stability training combined and pain nursing in patients after hip arthroplasty (HA). Methods A total of 100 HA postoperative patients in the Department of Orthopedics, Nanjing Drum Tower Hospital from October 2020 to October 2021 were selected and they were divided into control group and observation group by drawing lots, with 50 cases in each group. The control group was given routine rehabilitation nursing and pain nursing, and the observation group was given core stability training on the basis of the control group. The intervention lasted for 12 weeks. Pain status (visual analog scale [VAS] assessment), ability to perform activities of daily living (modified Barthel index [MBI] assessment) and complications rate during intervention were compared between the two groups. Results Based on the analysis of generalized estimation equation, the results showed that the VAS scores of the two groups at three days, one week and two weeks of intervention were lower than those before intervention, and the observation group was lower than the control group (P<0.05); VAS scores at 1, 2 weeks of intervention were lower than those at three days (P<0.05); VAS scores of two groups at two weeks of intervention were lower than those at one week of intervention (P<0.05); MBI scores in both groups were higher than before intervention at 12 weeks, and the observation group was higher than the control group (P<0.05); during the intervention period, there was no significant difference in the incidence of complications between the two groups (P>0.05). Conclusion Core stability training combined with pain care can reduce the degree of pain and improve the ability of daily living after HA.

Construction and application value of evidence-based nursing scheme in the perioperative period of minimally-invasive percutaneous nephrolithotomy with holmium laser

Objective To explore the construction and application value of evidence-based nursing scheme in the perioperative period of minimally-invasive percutaneous nephrolithotomy with holmium laser (MPCNL). Methods A total of 90 patients with kidney stones who received MPCNL treatment at Bozhou People’s Hospital in Anhui Province from January 2019 to December 2021 were selected as the research objects. According to random number table method, the patients were divided into routine group and intervention group, with 45 cases in each group. The routine group received routine perioperative nursing, and the intervention group received evidence-based nursing under perioperative nursing. The degree of postoperative pain the speed of recovery were compared between the two groups. Results After intervention, pain grading index, visual analog score, current pain intensity score, and total score of simplified McGill pain questionnaire in both groups were lower than those before intervention, and the intervention group was lower than the conventional group, and the differences were statistically significant (P<0.05); the time of anal exhaust, getting out of bed, catheter and nephrostomy extraction, and the length of hospital stay in the intervention group were shorter than those in the routine group, and the differences were statistically significant (P<0.05). Conclusion Evidence based nursing and perioperative nursing for patients with renal calculi during MPCNL, can effectively help patients reduce postoperative pain and accelerate their postoperative rehabilitation.

A case report of progressive myoclonus-ataxia syndrome caused by ATL1 gene mutation and literature review

This paper retrospectively analyzes the clinical data of a patient with progressive myoclonus-ataxia admitted to the First Department of Neurology, Dongzhimen Hospital, Beijing University of Chinese Medicine, and discusses its clinical characteristics, diagnosis, and treatment. The patient started with myoclonus, and gradually developed symptoms of ataxia. Molecular genetic testing revealed a heterozygous mutation of the ATL1 gene. After oral administration of Clonazepam, the symptoms of myoclonus were relieved. This paper analyzes the diagnosis and treatment process and clinical data of the patient and reviews the literature reports related to the molecular genetics of the disease, providing experience for the clinical diagnosis of this disease for the reference of clinicians.

In the context of “the Belt and Road Initiative”, acupuncture, moxibustion, and massage have reached a climax of international development and is highly recognized worldwide. Based on the experience of acupuncture, moxibustion, and massage in Switzerland and the understanding of local development, the author believes that comprehensive reform should be carried out from three aspects: colleges, teachers, and students by analyzing the employment and education status of acupuncture, moxibustion, and massage talents at home and abroad. The author also proposes that the training of English ability of acupuncture, moxibustion, and massage professionals should strengthen college cognition, attach importance to the training of bilingual teachers, and broaden learning channels, and other targeted program, with a view to providing some reference for the formulation of the international training program for acupuncture, moxibustion, and massage professionals, and jointly promoting the international development of acupuncture, moxibustion, and massage.

This paper sorts out and analyzes the relevant policies of medical insurance payment in the field of traditional Chinese medicine issued by the national and regional health administrative departments, as well as the typical practices in various regions reported in newspapers, so as to provide a reference for deepening the reform of medical insurance payment methods in the field of traditional Chinese medicine. Retrieve documents on “reform of traditional Chinese medicine medical insurance payment” issued by various health administrative departments and news on “medical insurance payment of traditional Chinese medicine” and “payment of traditional Chinese medicine by disease type” published on the government website through the government website before January 15, 2022, and extract the key information from it. A total of 30 provinces and cities have collected policy documents on traditional Chinese medicine medical insurance payment reform and typical practices in some regions. At present, the payment methods for traditional Chinese medicine diseases mainly include payment by disease, payment by diagnosis related groups, payment by diagnosis-intervention packet, and payment by curative effect value. There are differences in payment methods for different types of traditional Chinese medicine diseases. There are also some problems with different payment methods. It is recommended to strengthen research on price adjustment of traditional Chinese medicine, improve the disease database and surgical operation coding system with traditional Chinese medicine characteristics, and improve the positive incentive mechanism for medical institutions to provide traditional Chinese medicine services and medical insurance payments.

Application and exploration of the monthly report in the management of industrial style in multiple districts of Beijing Jishui Tan Hospital

In order to meet the requirements of homogenized management of industrial style under the new form of multiple districts of a hospital, and present the objective situation of the implementation of management of industrial style, the industrial style office of Beijing Jishui Tan Hospital began to formulate a monthly report in the management of industrial style from September 2021, and released it to all departments in each hospital district. In accordance with the SMART principle, the report has seven modules, including “monthly education and warning on the practice of ethics”, “reminders and requirements on the practice of ethics”, “notification of cases”, “special treatment”, “use of consumables”, “drug comments”, and “supervision and inspection”, which are mainly presented in the form of text, data, and charts. The report has been released for 12 periods, and the industrial style has been significantly improved in the hospital, the awareness of the staff in the hospital has been significantly improved, and the cooperation and communication between various departments have been strengthened, which has improved the homogenized management level of industrial style in multiple districts of the hospital.

Budget performance management is an important strategic tool to improve the effectiveness of using research funds, and scientific and effective research budget performance management will help stimulate the innovation energy of researchers, help managers make correct decisions, and promote the high-quality development of science and technology. By analyzing the problems of scientific research budget performance management in China, the author find that insufficient recognition of its importance, unreasonable management system, and lack of application of evaluation results are the key factors restricting scientific research budget performance management, and then propose countermeasures in terms of improving the recognition of the importance of budget performance, classifying and refining budget performance management, double evaluation of budget performance and project management, and establishing incentive and restraint mechanisms, so as to effectively promote the establishment of standardized and efficient budget performance management system.
