39 Regulation mechanism of adipocyte transdifferentiation on puberby sexual development and bone growth
DUAN Juan1 LIU Lailai2
DOI: 10.20047/j.issn1673-7210.2022.27.08
Puberty sexual development is accompanied by rapid bone formation and fat remodeling, estrogen regulates bone growth at the global level, sexual initiation is controlled by the hypothalamic-pituitary-gonadal axis (HPGA) and is related to body fat. Recent studies have found that leptin can stimulate hypothalamic gonadotropin-releasing hormone (GnRH) to stimulate HPGA, thus promoting the onset of puberty, but leptin is not an essential initiator. Peroxisome proliferator-activated receptor γ (PPARγ) is an important link between energy metabolism and reproductive axis. The activation of PPARγ triggers leptin signal, which can stimulate the hypothalamic GnRH to stimulate HPGA. As an endocrine organ, bone can inhibit the formation of adipocytes, which in turn can inhibit the proliferation and differentiation of osteoblasts and antagonize bone formation. PPARγ is a key factor regulating adipogenic and osteogenic differentiation. Therefore, adipocyte transdifferentiation is an important mechanism regulating puberty sexual development and bone growth. This paper expands the new understanding of adipocyte transdifferentiation on the regulation of children’s sexual development and bone metabolism, and provides a new idea for clinical multi-target and multi-way intervention of children’s precocious puberty and improvement of lifelong high.

XIE Anni YU Yanting WANG Xiaoyan
DOI: 10.20047/j.issn1673-7210.2022.27.10
Acute kidney injury (AKI) is characterized by rapid loss of renal function in a short period of time. Sepsis, ischemia-reperfusion, and nephrotoxic substances are the main causes of AKI. The mechanism of AKI is still unclear and there is a lack of effective diagnosis and treatment. With the novel development of sequencing technology, more and more long non-coding RNAs (lncRNAs) have been identified to abnormally expressed in AKI patients and animal and cell models. Studies have shown that lncRNA may play an important role in the occurrence, development, and prognosis of AKI by targeting inflammation, oxidation, and apoptosis signaling pathways through sponge adsorption of miRNA. This article reviews the mechanism and potential application of lncRNA in AKI.

Methods From January 2016 to December 2021, 196 patients with spiral fractures of the middle and lower 1/3 of tibia admitted to the Department of Orthopedics, the Third Affiliated Hospital of Zunyi Medical University were selected. They were divided into posterior malleolus group and non-posterior malleolus group according to the presence or obsence of posterior malleolus fracture. The general information and imaging data of two groups were recorded and analyzed, logistic regression was used to analyze the influencing factors of spiral fracture of the middle and lower 1/3 of the tibia combined with posterior malleolus fracture. Results The length of the tibia fracture line, the distance from the lowest point of the tibial fracture line to the tibial-talar surface, the distance from the lowest point of the tibial fracture line to the tibial-talar surface accounted for the proportion of the tibial total length (hereinafter referred to as “distance ratio”) in posterior malleolus group were greater than those in non-posterior malleolus group, and the differences were statistically significant (P < 0.05). The length of the tibia fracture line and distance ratio were independent risk factors for the middle and lower 1/3 tibial spiral fracture combined with posterior malleolus fracture (OR > 1, P < 0.05). Conclusion The middle and lower 1/3 tibial spiral fracture complicated posterior malleolus fractures may be affected by various factors. Clinicians can pay close attention to the length of the tibial fracture line and distance ratio. If the above indicators are large, it is necessary to be alert to the occurrence of posterior malleolus fracture to avoid clinical missed diagnosis and bury hidden dangers for later treatment.

114 Analysis of Professor Lü Wenliang’s thinking of diagnosis and treatment of jaundice
XU Lei1 QIANG Rui2 ZHANG Lili2 YAO Ziang1 LYU Wenliang2
DOI: 10.20047/j.issn1673-7210.2022.27.26
Jaundice is a common clinical disease of liver diseases in traditional Chinese medicine, with yellow eyes, yellow body and yellow urine as the typical manifestations. Professor Lü Wenliang, based on Zhong Jing’s theory that “Huang family, get it from dampness”, combined with the comments of medical experts and clinical experience in the past dynasties, believes that jaundice, exogenous six climatic exopathogens, and internal injuries and miscellaneous diseases can be the cause of jaundice, but the evil of “dampness” is the most important. Although there is a change from cold to heat, most of them are heat. And dampness evil has been living for a long time, accumulating stagnation in the liver and spleen. The liver regulates qi and stores blood, and the spleen regulates dampness and regulates blood. If it is qi stagnation and blood injury and heat trapping blood tier, it can cause blood stasis, heat and yellowing. Therefore, in the treatment of clinical syndromes, it is often to strengthen vital qi to eliminate pathogenic factor as the key link, mainly to clear the dampness-heat, cool the blood and remove blood stasis, supplemented by nourishing the liver and spleen, regulating qi and blood. And combined with the patient’s actual situation, attention and syndrome change, and to consider the profit and loss.

119 Relationship between traditional knowledge protection of traditional Chinese medicine and intellectual property system
LI Yu1 LIU Qian1 GUO Hua2 AN Hong1 ZHAO Zhupei1 SHI Xueqin1 ZHANG Fengxia1 LIU Jianfeng1
DOI: 10.20047/j.issn1673-7210.2022.27.27
The traditional knowledge protection of traditional Chinese medicine and the intellectual property system are relatively independent and cross each other. The existing intellectual property system in China protects traditional knowledge of traditional Chinese medicine in four aspects: patent, trademark right, copyright and trade secret. However, due to the particularity of differences in protection purpose, right object and protection mode, relying only on the intellectual property system cannot improve and effectively protect traditional knowledge of traditional Chinese medicine. The intellectual property system and traditional knowledge protection will complement and integrate each other, and a special protection mode of traditional Chinese medicine knowledge will be set up on the basis of intellectual property protection, so as to build a comprehensive protection system of traditional Chinese medicine knowledge in line with the characteristics of traditional Chinese medicine.

DOI: 10.20047/j.issn1673-7210.2022.27.44
Objective To evaluate the value of double-balloon enteroscopy (DBE) in the diagnosis and treatment of obscure lower gastrointestinal bleeding (OGIB) in children. Methods Thirty-one children with OGIB in Anhui Provincial Children’s Hospital from June 2014 to September 2021 were selected. After admission, symptomatic treatment such as hemostasis, blood transfusion, acid inhibition, and fluid replenishment were given as appropriate. The children with negative colonoscopy and imaging examination were selected for DBE examination. The causes and distribution of bleeding in children with OGIB were observed, DBE examination time, laparoscopic operation time, and hospital stay were counted. All children were revisited regularly to observe the treatment effect. Results There were 31 children, including 21 males and 10 females; the average age was (9.13±3.11) years; the average hemoglobin at admission was (86.90±19.31) g/L. The DBE of 26 children was positive, among which 25 patients were confirmed to have Mecker diverticulum (MD) by laparoscopic exploration, and 1 patient was found to have flake mucosal erosion in the intestinal lumen by DBE examination but was negative by laparoscopic exploration; DBE examination was negative in 5 children, and 1 patient with negative DBE examination was confirmed as MD after laparoscopic exploration. The average examination time of DBE was (148.65±44.47) min; the average operation time was (176.58±34.62) min; the average hospital stay was (5.90±1.51) days. No postoperative complications occurred. Postoperative pathology revealed ectopic gastric mucosa in 2 cases. There was no recurrence of lower gastrointestinal bleeding in 26 cases diagnosed with MD and 1 case with negative laparoscopic exploration but positive for DBE. However, in the 4 cases of negative DBE exploration without laparoscopic exploration, one of them showed recurrent hematochezia at 7 months, and the result was still negative after laparoscopic exploration, no gastrointestinal bleeding symptom has occurred again. Conclusion DBE examination is safe, intuitive, and effective. It is suitable for the clinical diagnosis and treatment of MD children with unexplained lower gastrointestinal bleeding as the main manifestation. Combined with laparoscopic exploration, it can effectively complete the minimally invasive treatment of MD.
