ZHANG Weiwei1 CAI Mingxing1 MO Qian1 YANG Dan2
Objective To investigate the acupuncture point specificity of electroacupuncture at the cìliáo point in rats with stress incontinence. Methods Fifty 8-week-old female SD rats, weighing 180 - 220 g, were divided into normal group, model group, cìliáo point group, sānyīnjiāo group, and non-channel point group according to random number table method, with ten rats in each group. The normal group was only tested, and the other four groups were treated with bilateral ovariectomy first, followed by vaginal dilation to establish the model of stress incontinence. After modeling, urine flow dynamics was detected before and after electroacupuncture intervention, and the changes of abdominal leak point pressure (ALPP) and maximal bladder capacity (MBC) were observed. Immunohistochemistry was used to detect the expression levels of type Ⅰ collagen (collagen Ⅰ) and connective tissue growth factor (CTGF) in the tissues of caudal gluteus muscle and bladder cervical urethral sphincter. Results The results of urine flow dynamics showed that MBC and ALPP values of model group were significantly decreased after intervention compared with normal group (P < 0.05). Compared with model group, MBC and ALPP values of cìliáo point group, sānyīnjiāo group, and non-channel point group were increased after intervention (P < 0.05). Compared with cìliáo point group, MBC and ALPP values of sānyīnjiāo group and non-channel point group were decreased (P < 0.05). Immunohistochemical results showed that compared with normal group, the expression levels of collagen Ⅰ and CTGF in model group were significantly decreased (P < 0.01). Compared with model group, the expression levels of collagen Ⅰ and CTGF in cìliáo point group, sānyīnjiāo group, and non-channel point group were significantly increased (P < 0.05). Compared with the cìliáo point group, the expression levels of collagen Ⅰ and CTGF in sānyīnjiāo group and non-channel point group were decreased (P < 0.05). Conclusion Electroacupuncture on cìliáo point can effectively improve the symptoms of stress urinary incontinence and repair the pelvic floor tissue damage, and the bladder regulation effect has acupoint specificity.

WU Xiaoxi WANG Tao DU Shengnan JIANG Yuanye▲ CAO Qin▲
Objective To explore the diagnostic value of liver fibrosis and cirrhosis in patients with chronic hepatitis B (CHB) among the aspartate aminotransferase / platelet ratio (APRI), cirrhosis discriminant score (CDS), and fibrosis index based on the four factors (FIB-4), Forns index, glutamyltransferase / platelet ratio (GPR), King’s score, S index model for chronic. Methods From January 2019 to December 2020, a total of 166 patients with CHB who were admitted to Putuo Hospital Affiliated to Shanghai University of Traditional Chinese Medicine and underwent liver biopsy and blood biochemical coagulation tests were selected. According to the liver fibrosis stage (S) of the liver biopsy results, there were 55 cases in the non-obvious liver fibrosis group (S0-1), 97 cases in the significant liver fibrosis group (S2-3), and 14 cases in the early-stage cirrhosis group (S4). The APRI, CDS, FIB-4, Forns index, GPR, King’s score, and S index according to the formula were calculate. At the same time, the receiver operating characteristic (ROC) curves of the above seven models were drawn, and the area under the ROC curve (AUC), sensitivity, and specificity were calculated to evaluate the value of each model in the diagnosis of liver fibrosis. Results Compared with the three groups of model scores, and the difference was highly statistically significant (P < 0.01). Except for the CDS model, each model index was positively correlated with the inflammation grade (r > 0, P < 0.05); the scores of each model were positively correlated with liver fibrosis stage (r > 0, P < 0.05). The AUC of S index, APRI, GPR, and King’s score for the diagnosis of significant liver fibrosis>0.7. The AUC for the diagnosis of early liver cirrhosis from high to low as GPR, S index, APRI, Forns index, King’s score, CDS, and FIB-4. The AUC analysis of the seven models showed that there was a statistically significant difference between APRI and King’s score in the significant liver fibrosis group (Z = 2.197, P < 0.05); there was no significant difference between the other models (P > 0.05). Conclusion APRI, GPR, King’s score, and S index can better diagnose significant liver fibrosis and early cirrhosis in CHB patients. CDS, FIB-4, Forns index have certain diagnostic value for early liver cirrhosis, but the diagnostic accuracy for significant liver fibrosis is low. APRI and GPR models are simple to calculate, serological indicators are readily available, and the cost is low. It can be recommended as a detection method for liver fibrosis in patients with CHB in the grassroots and areas with limited medical resources, so that some patients can avoid invasive liver biopsy and reduce medical costs.

38 Research progress on the interaction between sepsis and intestinal microbe#br#
BAI Yuanyuan DONG Beibei YU Yonghao▲
The gut microbiome is involved in the regulation of a variety of homeostatic mechanisms in the healthy host including immune function and gut barrier, etc. Recent studies have confirmed the important linkages between the gut microbiome and sepsis. This paper focuses on the potential mechanisms of intestinal microbes involved in the occurrence and development of sepsis from the following three aspects: Firstly, the activation of pathogenic intestinal flora, activation of the immune system and inflammatory response, as well as the reduction of beneficial microbial products, and other pathological changes affect the susceptibility of the host to sepsis; the second, sepsis leads to the destruction of intestinal microbiota and induces and aggravates terminal organ dysfunction; the third, microbial-based treatment strategies, such as selective intestinal decontamination and fecal microbiological transplantation, may reduce the incidence of sepsis and improve the prognosis of patients with sepsis. Intervention in the pathophysiological process of sepsis from the perspective of intestinal microorganisms may be a new target for the prevention or treatment of sepsis.

55 Research progress on the relationship between selenium and Hashimoto thyroiditis#br#
GENG Wenqian QI Shuo CHEN Xiaoheng LI Zhe LI Huilong LIU Meijun GE Yaxue DING Zhiguo
In recent years, the incidence of Hashimoto thyroiditis (HT) has been increasing year by year. Selenium, as an essential trace element, is related to the pathogenesis of HT. Most studies attribute the immunomodulatory effects of selenium to its antioxidant properties. Glutathione peroxidase system, thioredoxin reductase system and iodothyronine deiodase system together constitute the selenoprotein family, which not only directly participate in the synthesis of thyroid hormones, but also regulate the internal REDOX state of the thyroid gland and protect the thyroid gland from oxidative stress damage. At present, many studies have supported the efficacy of selenium in reducing thyroid auto-antibody titers in HT patients, but there is still a lack of sufficient evidence to confirm the clear efficacy of selenium in the treating of HT. Whether selenium supplementation can become the standard treatment for HT remains to be discussed.

Results The measured values of left ventricular end-diastolic diameter, left atrium diamete, left ventricular volumes index in hemodialysis and hemofiltration after dialysis were significantly lower than those before dialysis, and the differences were statistically significant (P < 0.05); however, there was no significant difference in Sm between the two methods before and after dialysis (P > 0.05). After a single dialysis, the measured values of LVGPSL, LVGLS, LVGCS, LVGAS, LVGRS, LVEDV, and LVESV in the hemodialysis methods were lower than those before dialysis, and the differences were highly statistically significant (P < 0.01); there was no significant difference in LVEF before and after dialysis (P > 0.05). After a single dialysis, the LVESV in the hemofiltration methods was significantly lower than that before dialysis, and the difference was statistically significant (P < 0.05). The serum urea and serum creatinine after a single dialysis in the two methods were lower than those before dialysis, and the differences were statistically significant (P < 0.01). In the hemodialysis methods, the serum β2-MG after dialysis was higher than that before dialysis, and the difference was highly statistically significant (P < 0.01). The serum β2-MG in the hemofiltration methods after dialysis was lower than that before dialysis, and the difference was highly statistically significant (P < 0.01). The changes of serum β2-MG concentration before and after dialysis in the hemofiltration methods were negatively correlated with LVGLS (r = -0.45, P < 0.05). Conclusion Hemodialysis has a certain damage to the overall systolic function of the left ventricle, while hemofiltration has a protective effect on the overall systolic function of the left ventricle. The possible mechanism is through filtering out macromolecular substances.

YU Song1 HONG Jing1 FENG Qiuting1 HU Jiajia1 ZHAI Xiaofeng2
Traditional Chinese medicine has unique advantages in treating liver cancer. Professor Zhai Xiaofeng has rich knowledge and experience, and has unique views on the pathogenesis and clinical treatment of liver cancer. Professor Zhai believes that it is necessary to understand the pathogenesis of liver cancer with an overall concept, pay attention to the influence of hepatitis B and cirrhosis on the characteristics of liver cancer. It is suggested that “liver depression and toxin accumulation” should be the core pathogenesis of hepatitis B related liver cancer. Liver cancer established, the course of disease often presents two main characteristic manifestations of “both ends of dampness and blood stasis”. The core of syndrome differentiation and treatment should be the liver and spleen, give consideration to “dampness”, “blood stasis”, “accumulation”, and “poison”. The basic principle is “soothing liver, strengthening spleen and nourishing health, removing toxin, eliminating accumulation and eliminating evil”. The main treatment methods are soothing liver and activating spleen, eliminating accumulation and removing toxin.

LIU Jiajia SUN Jingna YAN Haiyang HAN Zhengang CHEN Jing ZHAO Shuai▲
Objective To explore the application of loop-mediated isothermal amplification in the detection of lower respiratory tract pathogens in patients with mechanical ventilation. Methods From January to June 2021, a total of 90 patients with lower respiratory tract infection who were newly transferred to the Department of Respiratory Medicine and the Intensive Care Unit of the First Hospital of Hebei Medical University and underwent mechanical ventilation combined with ventilation through tracheal intubation were collected for bronchoalveolar lavage fluid samples for bacterial culture and drug sensitivity identification. At the same time, the loop-mediated isothermal amplification was used to detect the nucleic acid of pathogenic bacteria, and the samples of pathogenic bacteria were detected by drug resistance gene chip to analyze the detection of pathogens and drug resistance genes in patients. Results A total of 90 specimens were tested, among which 69 specimens were positive by the loop-mediated isothermal amplification, with a positive rate of 76.67%, and 31 were positive by bacterial culture, with a positive rate of 34.44%. The positive detection rate of the loop-mediated isothermal amplification was significantly higher than that of the bacterial culture method. The drug resistance test was carried out on the positive strains. Taking each sample as a separate whole, the loop-mediated isothermal amplification had a high detection rate in the detection of mecA, OXA-48, OXA-23, and other drug resistance genes, and was consistent with the detection rate of drug resistance genes. The compliance rate of the “gold standard” for drug resistance detection was high. Conclusion The detection rate of pathogens and drug resistance genes by the loop-mediated isothermal amplification is higher than that of traditional bacterial culture and drug susceptibility tests, and the detection rate of drug resistance genes is high, which has a good application prospect for guiding clinical medication.
