4 Role of endoplasmic reticulum stress pathway in palmitic acid-induced apoptosis of human periodontal membrane stem cells#br#

Objective To investigate the mechanism of endoplasmic reticulum stress pathway in the apoptosis of human periodontal ligament stem cells (hPDLSCs) induced by palmitic acid (PA). Methods The cultured hPDLSCs were randomly divided into control group (medium without PA), low, medium, and high concentrations of PA (0.1, 0.2, and 0.4 mmol/L) groups to explore the mechanism of PA inhibiting hPDLSCs proliferation; the cultured hPDLSCs were divided into control group (medium without PA), PA group (0.4 mmol/L) and PA (0.4 mmol/L) +4-PBA group (5 μmol/L) to explore the effect of 4-PBA on the apoptosis of hPDLSCs induced by PA. After 24 h of culture, the proliferation rate of hPDLSCs in each group was detected by MTT assay, the apoptosis of hPDLSCs in each group was observed by Annexin V-FITC/PI probe, and the expression of endoplasmic reticulum stress apoptosic-related proteins was detected by Western blot. Results The proliferation rate of hPDLSCs in low, medium, and high PA groups was significantly lower than that in control group (P < 0.05). The nuclear red fluorescence and cell membrane green fluorescence of PA high concentration group were increased significantly compared with cortrol group. The expressions of IRE1phos, PERKphos, ATF4, ATF6, and CHOP in PA high concentration group were significantly higher than those in control group (P < 0.05). Cleaved caspase-3 expression in PA medium and high concentration groups was significantly higher than that in control group (P < 0.05). Compared with PA low and medium concentrations groups, IRE1phos, PERKphos, ATF6, ATF4, CHOP, and Cleaved caspase-3 protein expressions in PA high concentration group were significantly increased (P < 0.05). Compared with PA group, PERKphos, CHOP, and Cleaved caspase-3 expressions were significantly down-regulated in PA+4-PBA group (P < 0.05). Conclusion PA may promote the apoptosis of hPDLSCs by regulating the protein expression of endoplasmic reticulum stress apoptosis pathway.

9 Effects of Tangluo Ning on apoptosis and MAPKs signaling pathway of rat dorsal root ganglion neurons in high glucose environment#br#

Objective To study the effect of Tangluo Ning on apoptosis and mitogen-activated protein kinases (MAPKs) signaling pathway of rat dorsal root ganglion neurons (DRGn) in high glucose environment. Methods Sixty clean grade male SD rats with body weight of (215±15) g at 6-8 weeks were divided into high, medium, and low concentration glucolonin groups and normal group according to random number table method, with 15 rats in each group. Each group was given 5, 2.5, 1.25 g/(kg·d) crude medicine of Tangluo Ning prescription and equal amount of distilled water intragastric administration to prepare drug-containing serum and normal control serum. DRGn cells were prepared from newborn SD fetal rats and were divided into normal group (10% normal rat serum +Neurobasal medium), high glucose group (based on normal group +50 mmol/L glucose), and Tangluo Ning concentration groups (based on high glucose group +10% low, medium, and high concentration Tangluo Ning drug-containing serum). Cell apoptosis and protein expression of JNK1, p-JNK1, c-Jun, p-c-Jun, Map3k8, and p-Map3k8 were detected after 24 h culture. Results The apoptosis rate, expression of p-JNK1, p-c-Jun, Map3k8, and p-Map3k8 and ratio of p-JNK1/JNK1, p-c-Jun/c-Jun, and p-Map3k8/Map3k8 in high glucose group were higher than those in normal group, the differences were statistically significant (P < 0.05 or P < 0.01). The apoptosis rate, expression of p-JNK1, p-c-Jun, and p-Map3k8 and ratio of p-JNK1/JNK1, p-c-Jun/c-Jun, and p-Map3k8/Map3k8 in Tangluo Ning concentration groups were significantly lower than those in high glucose group, the differences were highly statistically significant (P < 0.01). Apoptosis rate, expression of p-JNK1, p-c-Jun, and p-Map3k8 and ratio of p-JNK1/JNK1, p-c-Jun/c-Jun, and p-Map3k8/Map3k8 in middle concentration Tangluo Ning group were significantly lower than those in high and low concentration Tangluo Ning group, the differences were statistically significant (P < 0.01 or P < 0.05). The expression of c-Jun in the medium and low concentration Tangluo Ning groups was lower than that in the high concentration Tangluo Ning group, and the differences were statistically significant (P < 0.05). There was no significant difference in the expression of JNK1, Map3k8 and c-Jun in the Tangluo Ning concentration groups (P > 0.05). Conclusion Tangluo Ning can reduce apoptosis by inhibiting MAPKs signaling pathway.

14 Evaluation of left heart structure and function in patients with atrial fibrillation by real-time three-dimensional echocardiography combined with two-dimensional speckle tracking echocardiography #br#

Objective To evaluate of left heart structure and function in patients with atrial fibrillation by real-time three-dimensional echocardiography (RT-3DE) and two-dimensional speckle tracking echocardiography(2D-STE). Methods A total of 65 patients with atrial fibrillation hospitalized in Department of Cardiology, Beijing Jishuitan Hospital from January 2018 to December 2020 were selected as observation subjects. According to the type of atrial fibrillation, they were divided into persistent atrial fibrillation (PerAF) group (30 cases) and paroxysmal atrial fibrillation (PAF) group (35 cases). RT-3DE and 2D-STE were used to collect images, and QLAB 13.0 analysis software was used, maximum left atrial volume (LAVmax), left atrial emptying fraction (LAEF), left atrial volume index (LAVI), left ventricular end-diastolic volume (LVEDV), left ventricular end-systolic volume (LVESV), left atrial reservoir strain (LASr), left atrial conduit strain (LAScd), left atrial contraction strain (LASc), and left ventricular global longitudinal strain (GLS) were calculated. The related indicators of two groups were compared, and the correlation between LAVI and left atrium and left ventricle strain were analyzed. Results There were no significant differences in LAVmax between two groups (P > 0.05); LAVmin, LAVI, LVEDV, and LVESV in PerAF group were higher than those in PAF group, while LAEF was lower than that in PAF group, and the differences were statistically significant (P < 0.05). LASr, LAScd, LASc, and GLS in PerAF group were lower than those in PAF group, and the differences were statistically significant (P < 0.05). LAVI was negatively correlated with LASr, LAScd, LASc, and GLS (r =-0.47, -0.49, -0.63, -0.49, P < 0.01). Conclusion The left heart remodeling and functional impairment in patients with PerAF are more serious than those in patients with PAF. RT-3DE combined with 2D-STE can effectively evaluate the left heart structure and function in patients with atrial fibrillation.

18 Effects of Qufeng Tongluo Fomula on the inflammatory responce of human glomerular mesangial cells induced by high glucose

Objective To investigate the effects of Qufeng Tongluo Fomula on the inflammatory responce of human mesangial cells induced by high glucose. Methods In vitro culture of human mesangial cells, they were divided into normal group (low-glucose DMEM culture medium + normal rat serum), model group (high-glucose DMEM culture medium + normal rat serum), inhibitor group (high-glucose DMEM culture medium + normal rat serum +5 μmol/L SB203580 pretreatment), Qufeng Tongluo group (high-glucose DMEM culture medium + serum of rat taking Qufeng Tongluo Fomula). The levels of interleukin (IL)-1β, IL-6, tumor necrosis factor-α (TNF-α), and transforming growth factor-β1 (TGF-β1) were detected by enzyme-linked immunosorbent assay. Western blot was used to detect the expression levels of p38 mitogen activation protein kinase (p38 MAPK), intercelluar adhesion molecule-1 (ICAM-1), and monocyte chemotactic protein-1 (MCP-1). The mRNA expressions of IL-1β, IL-6, TNF-α, TGF-β1, ICAM-1, MCP-1, and p38 MAPK were determined by RT-PCR. Results The levels and mRNA expression of IL-1β, IL-6, TNF-α, TGF-β1, ICAM-1, MCP-1, and p38 MAPK in model group were higher than those in normal group, the differences were statistically significant (P < 0.05). The levels of IL-1β, IL-6, TNF-α, TGF-β1, ICAM-1, MCP-1, and p38 MAPK and their mRNA expression in Qufeng Tongluo group were lower than those in model group, and the differences were statistically significant (P < 0.05). Conclusion Qufeng Tongluo Formula may reduce the expression of inflammatory factors in human glomerular mesangial cells induced by high glucose by inhibiting the p38 MAPK signaling pathway, thus alleviating the inflammatory response.

22 Analysis of the effect of UBE2C, TTK and CP on prognosis of ovrian cancer based on bioinformatics#br#

Objective To screen out important genes associated with poor prognosis of ovarian cancer based on comprehensive bioinformatics analysis. Methods The expression profiles were obtained from the gene expression compilation database, and the differential expression genes (DEG) with │logFC│> 1.5 and P < 0.05 were identified from the gene expression omnibus database. Functional enrichment, protein-protein interaction (PPI) network construction, functional modules analysis, survival analysis, and correlation analysis were performed. Results Firstly, 135 uniformly expressed genes were identified, and 33 up-regulated DEG genes were mainly concentrated in mitotic spindle assembly checkpoint and chromosome separation regulation; a total of 102 DEG downregulation was mainly concentrated in the regulation of neurotransmitter level and protein serine / threonine kinase activity. Then, a PPI network was constructed, and 20 hub genes were screened for survival analysis and expression correlation analysis. At the same time, suitable modules were screened and pathway enrichment analysis was conducted for genes. Finally, the intersection of UBE2C, TTK and CP were found to be highly expressed in ovarian cancer and lead to poor prognosis. Conclusion Three key candidate genes affecting the prognosis of ovarian cancer patients are screened out through comprehensive bioinformatics analysis based on database which can provide new ideas for treatment.

28 Visualization analysis of self-compassion research literatures based on Web of Science#br#

Objective To visually analyze the research literatures of self-compassion to understand the current research status, hotspots, and development trends of this field. Methods “Self-compassion” and “compassion for oneself” were used as the subject headings, documents related to self-compassion included in the Web of Science core collection database were searched from January 2016 to December 2020. CiteSpace Ⅴ software was used to make statistics on the literature characteristics of institutions, journals, co-citations, and key words. Results The volume of publications in the self-compassion field was increasing year by year; publication agencies and journals were mainly distributed in the United States and Britain; research hotspots were included research on the relationship between self-compassion and multiple variables (burnout, compassion fatigue, body image, etc.), self-compassion-related intervention research, and self-compassion literature processing research; the research front was the study of the correlation between self-compassion, social support, and eating disorders, and exploratory research on the potential mechanism of self-compassion intervention. Conclusion Self-compassion research has gradually attracted the attention of scholars, and research hotspots and frontiers can provide reference and reference for our country to carry out related research.

33 Application value of donor-derived cell-free DNA in renal graft ischemia-reperfusion injury#br#

Objective To explore the application value of donor-derived cell-free DNA (dd-cfDNA) in the treatment of renal graft ischemia-reperfusion injury after renal transplantation. Methods Prospectively enrolled 40 subjects who underwent allograft renal transplantation at Renal Transplantation Center, Renji Hospital, Shanghai Jiao Tong University School of Medicine from March to May 2020. Peripheral blood was collected on the 1st, 3rd, and 7th day after operation, and dd-cfDNA% in peripheral blood of the patients was detected by second-generation sequencing method, and creatinine at the corresponding time point of the patients was detected. Results In this study, renal function of all patients recovered smoothly after renal transplantation, without delayed recovery of graft function or rejection occurred. Overall analysis found that the overall comparison of creatinine measurement and dd-cfDNA% at three time points showed highly statistically significant difference (P < 0.01). Further pairwise comparison showed that there were highly statistically significant differences in creatinine measurements and dd-cfDNA% concentration at any two time points in the group (P < 0.01). Creatinine measurement on 3rd day after operation was 245.65 μmol/L (95% confidence interval:172.70 to 318.60 μmol/L) lower than that on 1st day after operation. Creatinine measurement was further reduced by 195.95 μmol/L (95% confidence interval: 153.02 to 238.88 μmol/L) on 7th day after operation compared with 3rd day after operation. The dd-cfDNA% concentration on 3rd day after operation was 8.97% (95% confidence interval: 7.71% to 10.24%) lower than that on 1st day after operation. The dd-cfDNA% concentration was further reduced by 1.63% (95% confidence interval: 1.32% to 1.94%) on 7th day after operation compared with 3rd day after operation. Conclusion dd-cfDNA is differentially expressed in different periods of renal graft ischemia-reperfusion after renal transplantation, which is of certain value in evaluating the degree of renal graft ischemia-reperfusion injury.

37 Diagnostic value of fluorescence PCR melting curve method for drug resistance of Isoniazid, Ethambutol, and Levofloxacin against tuberculosis#br#

Objective To evaluate the diagnostic value of fluorescence PCR melting curve method for drug resistance of Isoniazid, Ethambutol, and Levofloxacin against tuberculosis. Methods A retrospective analysis was performed on 170 tuberculosis patients admitted to Beijing Chest Hospital, Capital Medical University from January 2018 to December 2019. A comparison was made between phenotypic drug susceptibility testing (DST) and fluorescence PCR melting curve method for anti-tuberculosis drug resistance. The diagnostic value of fluorescence PCR melting curve method for anti-tuberculosis drug resistance was analyzed using phenotypic DST results as the gold standard. The evaluation indexes included sensitivity, specificity, coincidence rate, Kappa value, and area under the curve (AUC) value. The drug resistance of quinolones (Levofloxacin and Moxifloxacin) was further compared. Results The number of cases of DST sensitivity by fluorescence PCR melting curve method and phenotype DST resistance was 5 cases in Isoniazid, 5 cases in Ethambutol, and 3 cases in Leofloxacin. The number of cases with DST resistance by fluorescence PCR melting curve method and phenotype DST sensitive was 9 cases in Isoniazid, 16 cases in Ethambutol, and 24 cases in Leofloxacin. The overall inconsistent rates of Isoniazid, Ethambutol, and Levofloxacin were 8.24%, 12.35%, and 15.88%, respectively. The sensitivity, specificity, coincidence rate, Kappa value, and AUC value of fluorescence PCR melting curve method to detect Isoniazid resistance were 91.53%, 91.89%, 91.76%, 0.821, and 0.917, respectively; the sensitivity, specificity, coincidence rate, Kappa value, and AUC value of fluorescence PCR melting curve method to detect Ethambutol resistance were 77.27%, 89.19%, 87.65%, 0.548, and 0.832, respectively; the sensitivity, specificity, coincidence rate, Kappa value, and AUC value of fluorescence PCR melting curve method to detect Levofloxacin resistance were 89.66%, 82.98%, 84.12%, 0.564, and 0.863, respectively. The drug resistance rate of Moxifloxacin against Mycobacterium tuberculosis (2.94%) was significantly lower than that of Levofloxacin (17.06%) (P < 0.01). Conclusion Fluorescence PCR melting curve method has good auxiliary diagnostic value for drug resistance of Isoniazid, Ethambutol, and Levofloxacin.

The proportion of coronary heart disease patients complicated with anxiety and depression is increasing. Long-term use of anti-anxiety and depression drugs will have many side effects. Non-drug intervention effectively improve the prognosis, has important clinical value. This article will discuss the relationship between coronary heart disease and anxiety and depression, and non-drug therapy of coronary heart disease combined with anxiety and depression. At present, non-drug therapy, such as psychotherapy, nursing intervention, music therapy, exercise therapy, and traditional Chinese medicine characteristic external treatment plays an important role in the treatment of coronary heart disease complicated with anxiety and depression.

45 Research progress on the value of blood routine in the prognostic evaluation of breast cancer#br#

Breast cancer has become the most common malignant tumor in women, which seriously threatens women’s physical and mental health. Recently, more and more studies have confirmed that the indicators such as neutrophils, platelets, lymphocytes, and the combined indicators such as the ratio of neutrophils to lymphocytes, the ratio of platelets to lymphocytes, and the ratio of lymphocytes to monocytes in blood routine are related to the therapeutic effect and prognosis of breast cancer. Blood routine indicators have the advantages of simplicity, economy and non-invasive, but there are also deficiencies such as lack of unified standards, small sample size, and unclear mechanism. This article reviews the research progress of the value of blood routine in the evaluation of prognosis of breast cancer.

49 Research progress on the mechanism of ginsenoside action in the prevention and treatment of myocardial infarction#br#

Myocardial infarction (MI) is one of the major causes of death and disability in the world and involves complex pathological processes. Ginsenosides, the active ingredient of ginseng, have been extensively studied, which can prevent MI through multiple approaches and multiple targets, and have a good protective effect on the cardiovascular system. In this review, the relevant mechanisms of ginsenosides action in the prevention and treatment of MI are elaborated from aspects of myocardial cell apoptosis, inflammatory response, myocardial fibrosis, cardiac angiogenesis, cardiac remodeling, oxygen / glucose supply, etc., in order to provide a reference for the development of new drugs for the prevention and treatment of MI by ginsenosides.

52 Research progress in application of 3D printing patient-specific navigation osteotomy instrument on knee arthroplasty#br#

Due to the good clinical effect of knee arthroplasty, its application in the treatment of knee osteoarthritis is increasing year by year. There are various types of knee arthroplasty and accurate osteotomy is one of the key factors to determine the curative effect of knee arthroplasty. There are many kinds of patient-specific navigation osteotomy instruments made by computer aided design and 3D printing technology, and their functions are different. It can assist the surgeon to find the important anatomical points of knee joint; and can play the role of fixing nails, fixation of osteotomy instrument plate; as the osteotomy instrument to complete part or more osteotomy, it has the advantages of simple operation, small trauma, accurate osteotomy, and can predict the size of knee prosthesis, short learning curve, and realize the personalized and accurate treatment of knee arthroplasty. This article reviews the application of 3D printing patient specific navigation osteotomy instrument in knee arthroplasty.

57 Progress in the clinical application of miniaturized percutaneous nephrolithotomy for different types of kidney stones#br#

In order to reduce the complications associated with standard channel percutaneous nephrolithotomy (PCNL), miniaturized percutaneous nephrolithotomy (MPCNL) has been used for the treatment of kidney stones since 2001. MPCNL not only reduces the channel size, but also comes equipped with kits that allow it to reduce complications while maintaining efficient stone removal rates. As MPCNL technology continues to improve and mature, its indications continue to expand. In this paper, the application of MPCNL in the treatment of different types of kidney stones is briefly reviewed. Compared with other treatment methods for kidney stones, MPCNL has certain advantages in the treatment of 1-2 cm single kidney stone, especially lower calyceal kidney stone, and tends to gradually replace standard PCNL in the treatment of 2-3 cm single kidney stone. In the treatment of complex or special types of kidney stones, single-channel MPCNL often has the limitation of small channel, while multi-channel MPCNL or multi-mirror combination can improve the stone clearing rate under the premise of ensuring safety. Therefore, MPCNL-based treatment is expected to become an important means for the treatment of all types of kidney stones.

62 Application of rain classroom in laboratory animal surgery teaching#br#

Objective To explore the effect of rain classroom in laboratory animal surgery teaching. Methods A total of 90 five-year clinical medicine students (Grade 2016) from Air Force Military Medical University were selected as the research subjects and divided into experimental group (45 students) and control group (45 students) according to random number table method. In the laboratory animal surgery teaching from March to June 2020, rain classroom was used for the experimental group, while traditional mode was used for the control group. After the completion of teaching, the two groups were assessed for their performance and questionnaire survey of teaching satisfaction. Results There was no significant difference between the two groups in theoretical knowledge scores (P > 0.05). The practical operation score and total score of experimental group were higher than those in the control group, the differences were statistically significant (P < 0.05). The recovery rate of teaching satisfaction questionnaire was 100% and the effective rate was 100%. The satisfaction rating scores of “conducive to theoretical teaching”, “conducive to practical teaching”, “conducive to autonomous learning”, “easy to grasp the key and difficult points”, “operation teaching”, and “review test” in the experimental group were higher than those in the control group, the differences were statistically significant (P < 0.05). There was no significant difference in satisfaction score of “class discussion” between the two groups (P > 0.05). Conclusion Rain classroom has shown good classroom effect in the surgery teaching of experimental animals and has good promotion potential.

65 Thoughts on the curriculum setting of Bencao Gangmu research and read from the perspective of inheritance essence#br#

In order to inherit the essence of traditional Chinese medicine (TCM), combined with teaching practices, the article analyses the intension of inheriting the essence of TCM, and clarifies that researching and reading herb ancient books is an important way to inherit TCM essence. Through the investigation of contemporary college students’ reading status of TCM classics, focus on the current situation of reading herb ancient books of materia medica, analyses existing problems, this paper puts forward suggestions to solve the highlighting problems in studying herb ancient books. Combined with curriculum of Bencao Gangmu research and read teaching practice, through designing and improving course content, and inheriting the essence of TCM and the humanistic spirit of TCM, illustrates the importance and necessity of the curriculum setting, can moderately solve the problems of insufficient intensity and depth of herb ancient books, it is helpful to guide students to study and apply herb ancient books creatively, can promote training and execise of professional knowledge skills, serves the clinical practice, and provides the most fundamental guarantee for the culture confidence of TCM.

69 Discussion of traditional Chinese medicine education reform based on theory of “preventive treatment of disease”#br#

With the development of society, the focus of public demand for medical services has gradually shifted from “treatment” to “disease prevention”. However, at present, clinical treatment is still more important than prevention, which has not completely kept up with the new changes in the medical situation. One of the reasons for this phenomenon is the unreasonable teaching concept and curriculum in medical colleges. Therefore, it is urgent to reform the existing teaching mode. As far as traditional Chinese medicine colleges and universities are concerned, the reform should be carried out in the following aspects: establishing the teaching idea of “preventive treatment of disease”, strengthening the infiltration of the thought of “preventive treatment of disease”, and strengthening the inculcation of modern preventive medicine idea. The teaching reform based on the concept of “preventive treatment of disease” will certainly improve students’ ability of “disease prevention”, which is of great significance to realize the medical mode of “health as the center” as soon as possible and promote the construction of healthy China.

72 Analysis on the path of integrating traditional Chinese medicine culture into college traditional physical education curriculum#br#

The integration of traditional Chinese medicine culture into the traditional health and physical education curriculum in colleges and universities is of great significance to promote the inheritance of Chinese excellent culture. This paper mainly starts from the common cultural basic theories such as “unity of knowledge and action”, “harmony of man with nature”, and “yin-yang theory”, and discusses the positive influence of traditional Chinses medicine culture on the country, society, and individuals by integrating traditional health care and physical education curriculum. It also proposes that the overall design of the integration of traditional Chinese medicine culture into the traditional health physical education curriculum should follow the “three-dimensional and six-sided” layout policy, focus on improving the cultural literacy of traditional health physical education teachers with “adept with pen and sword”, and optimize the implementation methods of traditional health physical education to be more comprehensive, to promote the inheritance and promotion of traditional Chinese medicine culture in the traditional health sports curriculum of colleges and universities.

76 Construction of evaluation index system for continuing medical humanities education in six tertiary hospitals#br#

Objective To explore the construction of evaluation index system for continuing medical humanities education in six tertiary hospitals. Methods Thirty experts from Peking Union Medical College Hospital, Huashan Hospital Affiliated to Fudan University, the Third Affiliated Hospital of Sun Yat-sen University, Xiangya Hospital of Central South University, Zhuzhou Central Hospital, Hunan Province, and Xiangtan Central Hospital, Hunan Province were selected for three rounds of consultation. Delphi method was used to study its positive coefficient, authority degree, coordination degree, and constructed the evaluation index system of continuing medical humanities education. Results Four first-level indicators and 20 second-level indicators were determined on the basis of the full absorption of expert opinions from the three rounds. In the three rounds of expert consultation, the positive coefficients of experts were 96.7%, 100.0%, and 100.0% respectively, and the authority coefficients were 0.873, 0.872, and 0.893 respectively. After three rounds of consultation, the expert coordination coefficient of the first-level indicator was 0.783, and that of the second-level indicator was 0.716, showing consistency (P < 0.05). Conclusion Through three rounds of expert consultation, this study constructs a set of evaluation index system of medical humanities continuing education with wide coverage, strong pertinence, and high reliability, which plays a positive role in promoting medical humanities continuing education.

The concept of “curriculum ideological and political” with characteristics in traditional Chinese medicine colleges is not only based on the overall consideration of ideological and political education in traditional Chinese medicine colleges, but also an important consensus formed during its long-term exploration and construction. Under the collaborative education mechanism, the construction of the characteristic “curriculum ideological and political” and the formation of the collaborative education of traditional Chinese medicine colleges and universities are the important carriers to improve the ideological and political education of traditional Chinese medicine colleges and universities. This paper starts with the main problems existing in the construction of characteristic “curriculum ideological and political” in traditional Chinese medicine colleges and universities, and analyzes how to construct the collaborative pattern of “curriculum ideological and political” in traditional Chinese medicine colleges and universities based on the actual reform and development of education in traditional Chinese medicine colleges and universities. It is proposed that the key points of the construction of the characteristic “curriculum ideological and political” in traditional Chinese medicine colleges should be centered on teaching resources, curriculum methods and teachers’ leading positions. Only in this way the construction of “curriculum ideological and political” in traditional Chinese medicine colleges and universities can be batter carried out, and achieve the goal of cultivating talents with traditional Chinese medicine characteristics.

The objective structured clinical assessment (OSCE) plays an important role in the teaching of anesthesia practice. However, the anesthesiology specialty has a short response period when encountering a disease in the clinic, a high incidence of various crisis events, and a greater impact on survival and prognosis. It is necessary to train students who can deal with problems with high quality and efficiency in the internship stage of anesthesia, so that they can quickly respond to high-intensity and high-risk clinical anesthesia work after graduation, serve the public and improve anesthesia quality and safety of anesthesia, and the OSCE concept can closely meet the requirements and cultivate high-quality students in all aspects. Therefore, the OSCE assessment method is effectively integrated into the whole process of internship. It is an assessment for interns of anesthesiology major in acquiring professional medical knowledge and skills, establishing good doctor-patient relationship and communication skills, training the ability to care for patients, improving the medical practice ability and other aspects.

The standardized training of resident residents is a necessary way for every medical student to become a qualified clinician. It is of great practical significance to implement the dual tutor training system in the the standardized training of ultrasound residents. As the dual tutor system is still in the primary stage in the regulation and training of ultrasound residents, there are still many problems exposed in the implementation process to be solved urgently. This paper analyzes the important role and long-term significance of implementing the dual tutor system in the course of the regulation and training of ultrasound medicine, and puts forward some reasonable suggestions for improving the process.

91 Analysis of independent risk factors for non-solid pulmonary nodules and establishment of a benign and malignant predictive model#br#

Objective To screen independent risk factors for non-solid pulmonary nodules and establish a predictive model for benign and malignant pulmonary nodules. Methods The clinical data of 309 patients with non-solid pulmonary nodules diagnosed in the First People’s Hospital of Zhenjiang, Jiangsu Province from 2015 to 2020 were retrospectively analyzed, and they were divided into observation group (malignant, 61 cases) and control group (benign, 248 cases) according to benign and malignant of the nodules. Single factor and binary logistic regression were used to analyze the risk factors in malignant nodules, and a prediction model was established. The diagnostic value of the model was analyzed by the receiver operation characteristic curve. Results Age, carcinoembryonic antigen (CEA) level, nodule diameter, solid part length diameter, mean CT (m-CT) value, and the proportions of calcification, vascular sign, vacuole sign, lobulation sign, and spicule sign in the observation group were higher than those in the control group, and the differences were statistically significant (P < 0.05). Multivariate analysis showed that high CEA level, long solid part length diameter, high m-CT value, and vascular sign, vacuolar sign, lobulation sign, and spicule sign were independent risk factors for malignant non-solid pulmonary nodules (OR > 1, P < 0.05). The prediction model was P = ex/(1 + ex), x=-6.078 + (2.002 × spicule sign) + (3.438 × vacuole sign) + (3.877 × vascular sign) + (0.622 × CEA) + (0.149 × solid part length diameter) +(0.003 × |m-CT|) + (5.494 × lobulation sign). The area under the curve of the model was 0.882 (P < 0.05), and the cut-off value was 0.842. Conclusion The benign and malignant prediction model of nonsolid pulmonary nodules is established in this study, which has certain diagnostic value and great clinical application prospect for benign and malignant diagnosis.

95 Influencing factors of the prognosis of COPD complicated with type Ⅱ respiratory failure and the value of serum UA, cTnT, and NT-proBNP levels in predicting the prognosis#br#

Objective To explore the influencing factors of the prognosis of chronic obstructive pulmonary disease (COPD) complicated with type Ⅱ respiratory failure and the value of serum uric acid (UA), cardiac troponin T (cTnT), and N-terminal pro-brain natriuretic peptide (NT-proBNP) levels in prediciting the prognosis. Methods The clinical data of 100 patients with COPD and type Ⅱ respiratory failure admitted to Zhangjiagang Hospital Affiliated to Soochow University from February 2018 to June 2020 were retrospectively analyzed. Patients were divided into good prognosis group (76 cases) and poor prognosis group (24 cases) according to the prognosis. The prognostic factors of COPD patients with type Ⅱ respiratory failure were analyzed, and the predictive value of serum UA, cTnT, and NT-probNP levels for the prognosis of patients were analyzed. Results In the poor prognosis group, 15 patients developed myocardial infarction, three patients developed heart failure, two patients developed multiple organ dysfunction, two patients developed angina pectoris, and two patients died. The acute physiology and chronic health evaluation Ⅱ (APACHE Ⅱ) score, UA, cTnT, NT-probNP, D-dimer, and C-reactive protein levels in the poor prognosis group were higher than those in the good prognosis group, and the differences were statistically significant (P < 0.05). Multivariate analysis showed that high APACHE Ⅱ score, and high serum UA, cTnT, and NT-probNP levels were independent risk factors for poor prognosis in COPD patients with type Ⅱ respiratory failure (OR > 1, P < 0.05). The receiver operating characteristic curve of subjects showed that serum UA, cTnT, NT-probNP, and their combination had certain diagnostic value for the prognosis of COPD patients with type Ⅱ respiratory failure (area under the curve>0.5, P < 0.05). Conclusion Serum UA, cTnT, and NT-probNP in COPD patients with type Ⅱ respiratory failure are correlated with prognosis, and the three have certain diagnostic value of prognosis alone or in combination.

99 Application value of fiberoptic ductoscopy in the treatment of pathologic nipple discharge#br#

Objective To explore the application value of fiberoptic ductoscopy in the treatment of pathological nipple discharge. Methods The clinical data of 431 patients with pathological nipple discharge who underwent fiberoptic ductoscopy in Tongzhou Maternal and Child Health Hospital of Beijing from January 2019 to December 2020 were analyzed retrospectively, patients with mammary duct inflammation and ductal ectasia were treated with breast duct lavage therapy, the curative effect was observed by telephone follow-up; the differences in color and character of discharge, single / porous nipple discharge, discharge volume, side of discharge (unilateral / bilateral) were observed in benign lesions and breast cancer, space occupying lesions and non-space-occupying lesions, benign space occupying lesions (intraductal papilloma, precancerous lesions, other benign diseases, mammary duct inflammation and ductal ectasia). Results In 257 patients with mammary duct inflammation and ductal ectasia, 200 patients had significant reduction or disappearance of discharge volume after breast duct lavage therapy, and the total effective rate was 77.8%, the color and character between benign lesions and breast cancer, space occupying lesions and non-space-occupying lesions were compared, the differences were statistically significant (P < 0.05), the benign lesions and breast cancer in single / porous discharge were compared, the differences were statistically significant (P < 0.05), and the the discharge volume compared, the difference was not statistically significant (P > 0.05). In benign space occupying lesions, in single / porous discharge were compared, the difference was not statistically significant (P > 0.05), the discharge volume compared, the difference was statistically significant (P < 0.05), the space occupying lesions and non-space-occupying lesions in single / porous discharge and discharge volume were compared, the differences were statistically significant (P < 0.05). Conclusion Fiberoptic ductoscopy can diagnose the etiology of pathological nipple discharge and achieve good efficacy in the treatment of inflammatory lesions in the milk ducts by irrigation. Fiberoptic ductoscopy can accurately locate the space occupying lesions in the milk ducts, accurately remove the lesions and avoid the blindness of surgery.

103 Infected situation of Helicobacter pylori in maintenance hemodialysis patients and its effect on microinflammation and nutritional status#br#

Objective To investigate Helicobacter Pylori (Hp) infection in maintenance hemodialysis (MHD) patients, and to analyze the relationship between Hp infection and microinflammation and nutritional status. Methods A total of 150 MHD patients treated in the First People’s Hospital of Fuyang District, Hangzhou, Zhejiang Province from January 2019 to December 2020 were selected as the study subjects. The Hp infection was investigated by 14C-Urpa breath test 14C-UBT. Hp positive patients were included in Hp positive group and Hp negative patients were included in Hp negative group. The clinical data and serum hypersensitive C-reactive protein (hs-CRP), albumin (ALB), and prealbumin (PAB) levels were compared between the two groups. Results In this study, 79 of the 150 MHD patients had 14C-UBT≥100 dpm/mol, and the infection rate of Hp was 52.67%, the remaining 71 patients had 14C-UBT < 100 dpm/mol. The dialysis age in Hp positive group was significantly shorter than that in Hp negative group, and the difference was statistically significant (P < 0.05); Hp infection rate in MHD patients with dialysis age ≤1 year, > 1 - 3 years, > 3 - 5 years and > 5 years was 72.22%, 64.70%, 50.00%, and 39.13%, respectively, and the differences were statistically significant (P < 0.05); the levels of ALB and PAB in Hp positive group were significantly lower than those in Hp negative group, and the hs-CRP level was significantly higher than that in Hp negative group, and the differences were statistically significant (P < 0.05). Conclusion Patients with MHD have higher Hp infection rate, while patients with shorter dialysis age have higher Hp infection rate, and MHD patients with Hp infection have more severe microinflammation and malnutrition.

107 Effect of Dexmedetomidine on myocardial injury after lower extremity ischemia reperfusion in elderly hypertensive patients#br#

Objective To investigate the effect of Dexmedetomidine on myocardial injury after lower extremity ischemia reperfusion in elderly hypertension patients. Methods A total of 60 elderly hypertension patients who underwent knee replacement in the First Hospital of Hebei Medical University from January 2019 to December 2020 were selected, they were divided into Dexmedetomidine group (group D, 30 cases) and control group (group C, 30 cases) by random number table method. Group D was given Dexmedetomidine 1 μg/kg after anesthesia induction, and maintained at 0.4 μg/(kg·h) until 30 min before the end of surgery; group C was given normal saline in the same way. Venous blood was collected at before tourniquet (T0), 20 min (T1), 2 h (T2), and 12 h (T3) after tourniquet loosening in two groups. The levels of serum inflammatory factors (tumor necrosis factor-α [TNF-α], interleukin-6 [IL-6]), oxidative stress index (malondialdehyde [MDA], superoxide dismutase [SOD]), and myocardial injury markers (creatine kinase isoenzyme [CK-MB], troponin I [cTnI]) were determined. Results The levels of TNF-α, IL-6, and MDA in two groups at T1-T3 were higher than those at T0, and the levels of TNF-α, IL-6, and MDA in group D at T1-T3 were lower than those in group C, and the differences were statistically significant (P < 0.05). The levels of SOD at T1-T3 in two groups were lower than those at T0, and the levels of SOD at T1-T3 in group D were higher than those in group C, and the differences were statistically significant (P < 0.05). The levels of CK-MB and cTnI at T2, T3 in two groups were higher than those at T0, and the levels of CK-MB and cTnI in group D at T2, T3 were lower than those in group C, and the differences were statistically significant (P < 0.05). Conclusion Dexmedetomidine can significantly reduce tourniquet related inflammation and oxidative stress in elderly hypertension patients and reduce myocardial injury after lower extremity ischemia reperfusion.

111 Comparison of analgesic effect of ultrasound-guided adductor canal block and traditional intravenous administration on fracture of tibia after open reduction and internal fixation#br#

Objective To investigate the curative effect of ultrasound-guided adductor canal block and traditional intravenous administration for fracture of tibia. Methods A total of 80 patients with fracture of tibial admitted to Wuxi Ninth People’s Hospital, Jiangsu Province from December 2018 to December 2019 were selected, they were divided into two groups by random number table method, with 40 patients in each group. The control group was given traditional intravenous administration analgesia, and the observation group was given ultrasound-guided adductor canal block analgesia. Visual analogue scale (VAS) was used to evaluate the pain degree at postoperative knee bend and rest, and the sedation state score (Ramsay score) was used to evaluate the postoperative rest state of the two groups, the treatment satisfaction of two groups were compared, the amount of Fentanyl in the postoperative analgesic pump, the effective pressing times of the analgesic pump and the amount of Flurbiprofen Axetil were monitored, and the length of hospital stay and the occurrence of adverse reactions were recorded. Results VAS scores at knee bend and rest of two groups were pairwise compared at different time points, and the differences were statistically significant (P < 0.05); VAS score at knee bend and rest of observation group were lower than those of control group at 8, 24 h, and 48 h postoperatively, and the differences were statistically significant (P < 0.05). At 24 h postoperatively, Ramsay scores of both groups were lower than those at 4 h postoperatively, and the differences were statistically significant (P < 0.05); at 4 h and 24 h postoperatively, the Ramsay scores of the observation group were lower than those of the control group, and the differences were statistically significant (P < 0.05). The amount of Fentanyl, the effective pressing times of the analgesic pump and the total dosage and usage rate of Flurbiprofen Axetil of observation group were lower than those of control group, and the length of hospital stay were shorter than that of control group, and patient satisfaction was higher than that of control group, and the differences were statistically significant (P < 0.05). The total incidence of adverse reactions in observation group was lower than that in control group, and the difference was statistically significant (P < 0.05). Conclusion Ultrasound-guided adductor canal block has better efficacy and safety than traditional intravenous administration.

115 Precise adaptation of taijiquan aiming at improving cardiopulmonary function#br#

With the encounter of aging society and environmental pollution, the incidence of cardiovascular and respiratory system increases year by year. Traditional Chinese mediciine holds that “the heart controls blood circulation”, “the lung governs qi and performs respiration”. Heart and lung are not only the driving force for the movement of qi and blood, but also the internal focus and basis for the rehabilitation of various chronic diseases. Especially in the post-COVID-19 era, it is crucial to improve the cardiopulmonary function of Chinese residents. Taijiquan is an excellent traditional skill in China. However, the perform routines of 24-type simplified taijiquan have less specific stimulation on cardiopulmonary function rehabilitation, and the breathing training, especially the synergistic interaction between breathing and limbs, is neglected. First of all, we extract the commencing form, wave hands like clouds, repulse monkey, part the wild horse’s mane on both side, hold the lute, closing form from the 24-type simplified taijiquan, then simplify the six-style lower limb gait route, focus on stimulating upper limb muscle groups, such as biceps brachii and triceps brachii which covered by taiyin lung meridian of hand and shaoyin heart meridian of hand. Finally, as a breathing training, the abdominal breathing are integrated into the movement routines of six forms. The adaptation is named as “six forms of taijiquan”. It has the effect of dredging qi and blood and promoting heart and lung function.

119 Exploration on the medication rules of traditional Chinese medicine in the treatment of high risk human papilloma virus infection based on data mining#br#

Objective To explore the medication rules of traditional Chinese medicine in the treatment of high risk human papilloma virus (HR-HPV) infection. Methods Effective prescriptions for HR-HPV infection were collected from CNKI, Wanfang and VIP databases, and the retrieval time was from January 2000 to February 2021. The frequency and nature of traditional Chinese medicine (four qi, five flavors, meridian entry) were counted by using cloud platform of ancient and modern medical records; SPSS Modeler 18.0 was used for cluster analysis and association rule analysis, and a complex network of traditional Chinese medicine was build to screen the core combination of traditional Chinese medicine. Results A total of 95 prescriptions were included, involving 152 kinds of traditional Chinese medicine, of which tonifying and replenishing medicine, heat-clearing medicine and dampness-draining medicine were the most common. The main meridian entries were liver and spleen, the four qi was mainly cold and flat, the five flavors were mainly sweet and bitter. High-frequency traditional Chinese medicine were divided into seven groups by cluster analysis, and 20 pairs of drug combinations were screened out by association analysis. Eight drugs which were Astragali Radix, Atractylodis Macrocephalae Rhizoma, Phellodendri Chinensis Cortex, Coicis Semen, Hedyotis, Codonopsis Radix, Poria and Glycyrrhizae Radix et Rhizoma found by complex network were the core prescriptions for the treatment of HR-HPV infection. Conclusion The basic principles of traditional Chinese medicine in the treatment of HR-HPV infection are “invigorating spleen for eliminating dampness, clearing heat and removing toxicity”, and nourishing and promoting blood circulation, and the main drugs are tonifying and replenishing medicine, heat-clearing medicine and dampness-draining medicine.

124 Clinical effect of Chailing Decoction combined with traditional swallowing and neuromuscular electrical stimulation in the treatment of dysphagia after ischemic stroke#br#

Objective To observe the clinical effect of Chailing Decoction combined with traditional swallowing and neuromuscular electrical stimulation (NMES) in the treatment of dysphagia after ischemic stroke. Methods One hundred patients with dysphagia after ischemic stroke treated in Huzhou First People’s Hospital from July 2019 to June 2020 were selected and divided into control group and treatment group according to random number table method, with 50 cases in each group. Control group was given routine drugs for stroke, and the swallowing function was repaired by traditional swallowing therapy and NMES. In addition to the above treatment, treatment group was treated with Chailing Decoction orally or nasal feeding. Both groups were treated for four weeks. Before and after treatment, the clinical effects were compared by water swallow test, National Institutes of Health stroke scale (NIHSS) and videofluoroscopic swallowing study (VFSS). Results The total clinical effective rate in treatment group was higher than that in control group (P < 0.05). After treatment, the scores of water swallow test and NIHSS in two groups were lower than those before treatment, and VFSS scores in two groups were higher than those before treatment, the scores of water swallow test and NIHSS in treatment group were lower than those in control group, and the score of VFSS in treatment group was higher than that in control group, the differences were statistically significant (P < 0.05). Conclusion Chailing Decoction combined with traditional swallowing and neuromuscular electrical stimulation has good effect in relieving dysphagia after ischemic stroke and it is worthy of clinical promotion.

Objective To explore the application effect of traditional Chinese medicine hot compress combined with acupoint massage in the treatment of sequelae of pelvic inflammatory disease (PID) with syndrome of qi stagnation and blood stasis. Methods A total of 80 PID sequelae patients with syndrome of qi stagnation and blood stasis admitted to First People’s Hospital of Nantong City from May 2018 to October 2020 were selected, and they were divided into the observation group (40 cases) and the control group (40 cases) by random number table method. The control group was given Western medicine combined with acupoint massage therapy, and the observation group was given traditional Chinese medicine hot compress on the basis of the control group. After three courses of treatment, clinical efficacy, the changes of traditional Chinese medicine syndromes scores, signs integral, total integral, serum inflammatory factors, and hemorheology were compared between the two groups. Results During the follow-up, two patients in the observation group and fours patients in the control group were lost to follow-up. A total of 74 patients were included, 38 in the observation group and 36 in the control group. The effective rate of the observation group was better than that of the control group, the difference was statistically significant (P < 0.05). The traditional Chinese medicine syndromes scores, signs integral, and total integral of the two groups after treatment were lower than those before treatment (P < 0.05), and those of the observation group were lower than the control group (P < 0.05). The serum interleukin-8 (IL-8) and tumor necrosis factor-α of the two groups after treatment were lower than those before treatment, and the observation group were lower than the control group (P < 0.05); the IL-10 levels of the two groups after treatment were higher than those before treatment, and those of the observation group were higher than the control group (P < 0.05). The high-cut whole blood viscosity, low-cut whole blood viscosity, plasma viscosity, and hematocrit of the two groups after treatment were lower than those before treatment, and those of the observation group were lower than control group (P < 0.05). Conclusion The combination of traditional Chinese medicine hot compress and acupoint massage can improve the clinical symptoms of PID sequelae patients, improve treatment effect, which may be related to inhibit inflammation and regulate blood rheology.

Objective To explore the clinical effect of the acupoints combination of kidney-benefiting and marrow-filling of electroacupuncture in the treatment of Alzheimer’s disease with syndrome of marrow sea insufficiency. Methods A total of 60 patients recruited from the outpatient and inpatient department of the First Affiliated Hospital of Heilongjiang University of Chinese medicine from September 2019 to November 2020 were selected, according to random number table method, they were divided into acupuncture group (30 cases) and electroacupuncture point group (30 cases). The acupuncture group was treated with traditional acupuncture and moxibustion, and the electroacupuncture point group was treated with acupoints combination of kidney-benefiting and marrow-filling. Both groups were treated for four weeks. The mini mental state examination (MMSE), classification of dementia syndrome in traditional Chinese medicine (SDSD), and Montreal cognitive assessment (MoCA) was compared between the two groups before and after treatment, and the efficacy index was calculated as the efficacy evaluation. Results After treatment, the MMSE and MOCA scores of the two groups were higher than those before treatment (P < 0.05), and the electroacupuncture point group was higher than that of the acupuncture group (P < 0.05); the SDSD scores of the two groups were lower than those before treatment (P < 0.05), and the electroacupuncture point group was lower than that of the acupuncture group (P < 0.05). The therapeutic indexes of MMSE, SDSD and MOCA in electroacupuncture point group were higher than those of the acupuncture group (P < 0.05). Conclusion Both traditional acupuncture and acupoints combination of kidney-benefiting and marrow-filling have definite curative effects on Alzheimer’s disease with syndrome of marrow sea insufficiency, and acupoints combination of kidney-benefiting and marrow-filling is better than traditional acupuncture in the treatment of Alzheimer’s disease with syndrome of marrow sea insufficiency.

Objective To explore the effect of acupuncture in the treatment of patients with post-stroke upper limb hemiplegia and its influence on limb motor function. Methods A total of 120 patients with post-stroke upper limb hemiplegia admitted to Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from May 2018 to May 2020 were selected as the research subjects. They were divided into control group and observation group by random number table method, with 60 cases in each group. The control group was given conventional intervention, the observation group was treated with acupuncture on the basis of the control group. The results of electromyography measurement, upper limb Brunnstrom grading, modified Ashworth grading, Fugl-Meyer assessment (FMA) scale score, activities of daily living scale (ADL) score, and the satisfaction of patients were compared between the two groups before and after intervention. Results The Hmax, Mmax, and H/Max values of the two groups after intervention were lower than those before intervention, and the observation group were lower than the control group (P < 0.05). Upper limb Brunnstrom grading, modified Ashworth grading of the two groups after intervention were greater than those before intervention, and the observation group was greater than the control group (P < 0.05). The FMA scale scores and ADL scale scores of the two groups after intervention were higher than those before intervention, and the observation group were higher than the control group (P < 0.05). The satisfaction of the observation group was higher than that of the control group (P < 0.05). Conclusion Acupuncture in the treatment of patients with post-stroke upper extremity hemiplegia can effectively improve limb function, regulate muscle tension, relieve spasm state, effectively improve limb function, and patients have high satisfaction.

Based on the numerous occurrences, complex meaning, and vague concept of “six climatic factors” in Huangdi Neijing, this paper analyzes the content of “six climatic factors” in Huangdi Neijing, and refers to the commentaries of later medical scholars, and concludes that the six climatic factors can be divided into two major parts, namely, the six climatic factors of nature and the six climatic factors of human body. Among them, the six climatic factors of nature divides the meaning of six climatic factors into two parts, one is wind, cold, heat, humidity, dryness and fire, which are the six climatic characteristics of nature, another is in the doctrine of five evolutive phases and six climatic factors of Huangdi Neijing, plusing three yin and three yang on the top of wind, cold, heat, humidity, dryness and fire, together called six climatic factors. It has certain time rules and corresponding climatic characteristics. In the six climatic factors of the human body, the meaning of six climatic factors covers two kinds, one is essence, qi, clear fluid, turbid fluid, blood and veins, the basic living substance of human body, another is the concept of six climatic factors of internal generation which derived by later medical practitioners based on the theory of six-excess external contraction of Huangdi Neijing. Objective To investigate the imaging characteristics of 3.0T magnetic resonance imaging (MRI), diffusion weighted imaging (DWI), and dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) in the clinical diagnosis of patients with liver cancer. Methods A total of 94 patients with liver cancer diagnosed pathologically in Huaibei People’s Hospital of Anhui Province from February 2018 to October 2021 were analyzed, the MRI, DWI, and DCE examination data of patients at the time of admission were collected for analysis, and T1WI and T2WI signals, DWI signals and DCE images of diseased liver area and normal liver area, the apparent diffusion coefficient (ADC) of patient’s diseased liver area and normal liver area were calculated and compared. Results T1WI and T2WI signals of patient’s normal liver tissue showed equal signal levels, while DWI signal showed a lower level; T1WI signal of diseased liver area showed low signal level, and T2WI signal of diseased liver area showed equal signal or a little high signal level, and DWI signal presents a significantly high signal level. The average ADC value in diseased liver area was lower than that in normal liver area, and the difference was statistically significant (P < 0.05). Conclusion MRI, DWI, and DCE examinations of patients with liver cancer all show independent imaging characteristics, and the combined application in clinical practice can effectively improve the accuracy of pre-imaging diagnosis, and the calculation of ADC value at the same time has a certain value for judging the biological characteristics of the lesion area of patients with liver cancer.

153 Manifestations and diagnostic value of MRCP in patients with cholecystolithiasis complicated with cholecystitis#br#

Objective To explore manifestations and diagnostic value of magnetic resonance cholangiopancreatography (MRCP) in patients with cholecystolithiasis complicated with cholecystitis. Methods The clinical data of 80 patients who underwent laparoscopic cholecystectomy in Anqing First People’s Hospital of Anhui Province from July 2018 to April 2021 were retrospectively analyzed. All subjects underwent upper abdominal MRCP examination and color ultrasound examination before surgery. MRCP and color ultrasound manifestations of patients with cholecystolithiasis complicated with cholecystitis were observed, and the diagnostic efficacy of MRCP and color ultrasound in cholecystolithiasis complicated with cholecystitis was compared. The receiver operator characteristic curve was drawn and the area under the curves was calculated. Results A total of 37 cases were diagnosed as cholecystolithiasis complicated with cholecystitis, and the remaining 43 cases were chronic cholecystitis. Among MRCP examination results, 33 patients with cholecystolithiasis complicated with cholecystitis were clearly diagnosed in 37 patients, the gallbladder was obviously dilated, and the common bile duct, left and right bile ducts, and pancreatic duct were clearly displayed, different numbers of stones were seen in the duct, with round, oval, strip, and irregular shapes. A total of 24 cases were diagnosed clearly by color ultrasound, the gallbladder was obviously dilated and the ducts and stones were clearly displayed. The sensitivity, specificity, and accuracy of MRCP in the diagnosis of cholecystolithiasis complicated with cholecystitis were 89.19%, 97.67%, and 93.75%, respectively. The area under curve of MRCP was significantly higher than that of color ultrasound, and the difference was highly statistically significant (P < 0.01). Conclusion The accuracy of MRCP in the diagnosis of cholecystolithiasis complicated with cholecystitis is higher than that of color ultrasound. Preoperative MRCP examination is of great significance for patients with cholecystitis suspected of having gallstones, which is worthy of active promotion in clinical practice.

157 Analysis of epidemiological characteristics and complete blood count results of 16 cases of imported malaria#br#

Objective To analyze the epidemiological characteristics of imported malaria and laboratory test results. Methods The clinical data of 16 imported malaria patients who were treated in Tongling People’s Hospital from January 2017 to August 2020 were retrospectively collected, set as the malaria group, and 30 healthy persons were selected as the healthy control group. The complete blood count and white blood cell differential count between the two groups were compared. Results In 16 patients, the proportion of abnormal white blood cell count was 25.0%; the proportion of people with reduced red blood cell count was 31.3%; the proportion of people with reduced hemoglobin was 37.5%; the proportion of patients with reduced hematocrit was 43.8%; the proportion of abnormal platelet counts was 93.8%. Comparison of the percentage and absolute value of lymphocytes, the percentage and absolute value of monocytes, and the absolute value of eosinophils between the two groups, the differences were statistically significant (P < 0.05 or P < 0.01). Comparison of three red blood cell parameters and platelet count between the two groups, the differences were highly statistically significant (P < 0.01). Conclusion Malaria infection often causes a significant decrease in red blood cell related parameters and a significant decrease in platelet count, which requires special attention.

Objective To explore the application value of PDCA introducing cycle in the whole process fine management of medical equipment, in order to promote the fine management level of the whole life cycle of hospital medical equipment. Methods PDCA cycle management mode was introduced to manage all medical devices from June to December in 2018, which compared with the June to December in 2017 when PDCA cycle management was not introduced. The series data was collected through the hospital information system. The acceptance rate of medical equipment, equipment use rate, equipment scrap rate, equipment file integrity rate and other indicators was compared before and after the introduction of PDCA cycle. Results After PDCA cycle management, the acceptance rate of medical equipment, the completion rate of medical equipment and the abandonment rate of medical equipment were higher than those before PDCA cycle management, with statistical significances (P < 0.05). Conclusion The PDCA cycle in the fine and full process management of medical equipment can not only ensure the clinical application and improving the utilization rate of equipment, but also make the equipment management more standardized and systematic with saving the purchase and maintenance cost of hospital medical equipment while.

Objective To explore the impact of Internet interactive health education on disease cognition and treatment compliance of young and middle-aged hepatitis B patients. Methods A total of 150 hepatitis B patients admitted to the Huai’an Fourth People’s Hospital from January to December 2019 were selected and divided into observation group and control group by random number table method, with 75 cases in each group. The control group was given regular health education, while the observation group was jointly applied with the Internet interactive health education intervention. Follow-up for six months, the disease awareness and treatment compliance of two groups were compared. Results Follow-up for six months, the total scores of onset causes, exercise rest, medication management, self-protection, diet management, and disease awareness of the two groups were higher than those of the same group before education, while the observation group was higher than the control group, and the differences were statistically significant (P < 0.05). Follow-up for six months, the treatment compliance scores of the two groups were higher than those of the same group before education, and the differences were statistically significant (P < 0.05). The total scores of medication compliance, self-monitoring, healthy lifestyle, and treatment compliance of observation group were higher than those of the control group, and the differences were statistically significant (P < 0.05). The nursing satisfaction of the observation group was higher than that of the control group, and the difference was statistically significant (P < 0.05). Conclusion Internet interactive health education can help improve the awareness of young and middle-aged hepatitis B patients, promote the development of treatment compliance behavior, and improve patients’ care satisfaction.

170 Impact of continued care based on the cloud follow-up platform on the health behavior and quality of life of patients with chronic obstructive pulmonary disease#br#

Objective To explore the impact of continued care based on the cloud follow-up platform on the health behavior and quality of life of patients with chronic obstructive pulmonary disease (COPD). Methods A total of 78 COPD patients who were discharged after treatment in Jiangsu Province Hospital from January 2020 to June 2020 were selected. According to the random number table method, they were randomly divided into control group and observation group, with 39 cases in each group. The control group received routine telephone follow-up, and the observation group performed continuous care based on the cloud follow-up platform. After five months follow-up, the health promotion lifestyle profile (HPL) was used to evaluate the health behaviors of the two groups, and the St. George’s respiratory questionnaire (SGRQ) was used to evaluate the quality of life of the two groups. Results After five months of follow-up, the HPL score of the treatment group was significantly higher than that before the follow-up (P < 0.05). The physical activity, nutrition, and mental health scores of the control group were significantly higher than those before the follow-up (P < 0.05). The scores of health responsibility, physical activity, interpersonal relationship, stress management, and mental health of the treatment group were significantly higher than those of the control group (P < 0.05). The SGRQ scores of the two groups were significantly lower than before the follow-up (P < 0.05), and the scores of symptoms, activity capacity and the impact of disease on daily life in the treatment group were significantly lower than those of the control group (P < 0.05). Conclusion Continuing care based on the cloud follow-up platform can promote COPD patients to develop good health behaviors and improve the quality of life.

174 Application effect of health education in perioperative nursing of patients with chronic sinusitis#br#

Objective To explore the application of health education in perioperative nursing of patients with chronic sinusitis operation and its influence on patients’ quality of life (QOL) scores. Methods A total of 158 patients with chronic sinusitis that had functional nasal endoscopic surgery treatment treated in Department of Otolaryngology and Head and Neck Surgery, Changzhi People’s Hospital from November 2019 to October 2020, they were divided into control group and observation group by random number table method, with 79 cases in each group. The control group used conventional nursing methods, observation group on the basis of the control group used health education. The health knowledge mastery, QOL score, implementation of health behaviors and nursing satisfaction were compared between the two groups after intervention. Results After the intervention, the postoperative precautions, nose blowing and diet of the control group were better than those before the intervention, and the difference were statistically significant (P < 0.05). The observation group had better knowledge of sinusitis after intervention than before the imtervention and higher than the control group, and the difference was highly statistically significant (P < 0.01); after the intervention, QOL score of observation group was higher than control group, the difference was highly statistically significant (P < 0.01); the compliance rate of sinusitis related behaviors and nursing satisfaction of the observation group were higher than those of the control group, and the differences were highly statistically significant (P < 0.01). Conclusion In perioperative nursing of patients with chronic sinusitis, health education can improve patients’ health knowledge and compliance, improve patients’ quality of life, and improve patients’ satisfaction, which is worthy of clinical promotion.

Objective To investigate the effect of bundle management based on key point control theory on psychological states, self-care ability, quality of life and the incidence of complications in patients with aplastic anemia(AA). Methods A total of 186 AA patients treated in the Department of Hematology, West China Hospital, Sichuan University from February 2018 to February 2021 were selected as the subjects. They were divided into control group (n = 93) and observation group (n = 93) according to the sirgle and double order. The control group was given routine nursing intervention, while the observation group was given bundle management intervention based on key point control theory on the basis of routine nursing intervention. Hamilton anxiety scale (HAMA), Hamilton depression scale (HAMD), self-care ability (ESCA) scores, short form health survey questionnaire-36 (SF-36) scores, the incidence of complications and patient satisfaction were compared between the two groups. Results HAMA and HAMD scores of the two groups after intervention were lower than before intervention, ESCA and SF-36 scores were higher than before intervention. HAMA and HAMD scores in observation group were lower than those in control group, ESCA and SF-36 scores were higher than those in control group, the differences were statistically significant (P < 0.05). The incidence of complications in the observation group was lower than that in the control group, the difference was statistically significant (P < 0.05). Conclusion Bundle management based on key point control theory can relieve unhealthy emotions, improve self-care ability and quality of life, and reduce the incidence of complications in patients with AA.

According to the odd and even number method, they were divided into the control group and the observation group, with 45 cases in each group. The control group received routine health education, the observation group used knowledge, belief, practice, and health education, the cognitive level, emotional change, self-efficacy and coping style of the two groups were compared before and after nursing. Results After nursing, the scores of the observation group’s cognitive level were higher than those of the control group, and the differences were statistically significant (P < 0.05); after nursing, anxiety score, depression score, negative coping scores of two groups were lower than before nursing, self-efficacy scores and positive coping scores were higher than before nursing, and anxiety and depression score, negative coping scores of the observation group after nursing were lower than the control group, self-efficacy scores and positive coping scores were higher than those of control group, the differences were statistically significant (P < 0.05). Objective To explore and analyze the impact of multidisciplinary collaborative nursing care on the quality of life and mental state of patients undergoing coiled intracranial aneurysm (CIA) surgery. Methods A total of 100 patients with intracranial aneurysm who received CIA treatment in Nanjing Hospital of Nanjing Medical University from May 2019 to May 2021 were randomly divided into control group and observation group, with 50 cases in each group. The control group received routine nursing care during the perioperative period, while the observation group used multi disciplinary collaborative nursing care, both of which were continuously nursing for a week. The wound healing time, hospitalization time, and other indicators of the two groups were recorded; before nursing and a week after nursing, the generic quality of life inventory-74 (GQOLI-74) was used to assess the quality of life of the two groups; the self-rating anxiety scale (SAS) and the self-rating depression scale (SDS) were used to assess the mental state of patients; the complications of the two groups were recorded. Results The wound healing time and hospital stay in the observation group were shorter than those in the control group, and the differences were statistically significant (P < 0.05). At a week of nursing, the GQOLI-74 scores and total scores of the two groups were higher than those before nursing, while the observation group was higher than the control group, and the differences were statistically significant (P < 0.05). At a week of nursing, the scores of SAS and SDS in the two groups were lower than those before nursing, while the observation group was lower than the control group, and the differences were statistically significant (P < 0.05). The incidence of complications such as local hematoma at the puncture point in the observation group was lower than that in the control group, and the difference was statistically significant (P < 0.05). Conclusion Multidisciplinary collaborative care is more conducive to improving the quality of life and mental state of patients with intracranial aneurysm CIA, and is conducive to reducing the incidence of complications.

190 Application of nursing model combined with sensorimotor training based on stress system theory to patients with cerebral infarction in recovery period#br#

Objective To analyze the application effect of nursing model combined with sensorimotor training (SMT) based on stress system theory in patients with cerebral infarction (CI) during recovery period. Methods A total of 80 patients with CI recovery period accepted by Qingdao Municipal Hospital from January 2019 to January 2020 were prospectively selected as the research objects, and they were grouped according to the odd and even numbers of the medical records. Single-number patients were admitted as the control group (40 cases), and routine care combined with SMT; double-number patients were admitted as the observation group (40 cases), and routine care + nursing combined SMT based on stress system theory were implemented, intervention to the end of discharge (hospitalization time was 21-28 days). Before the intervention and one day before discharge, the motor function of lower limbs (assessed by the simplified Fugl-Meyer assessmont [FMA] scale), balance ability (assessed by the Berg balance scale [BBS]), post traumatic growth (PTG) (evaluated by using the PTG inventory [PTGI]) were compared between two groups. Results One day before discharge, the lower limb FMA scores, BBS scores, and PTGI scores of the two groups were higher than those before the intervention, while the scores of the observation group were higher than those of the control group, and the differences were statistically significant (P < 0.05). Conclusion The nursing model based on the theory of stress system combined with SMT is applied to patients in the recovery period of CI, which is beneficial to improve the patients’ lower limb motor function and balance ability, and has positive significance for improving the patients’ PTG.

194 Mediating effect of cognitive function on anxiety and activities of daily living in patients after acute carbon monoxide poisoning#br#

Objective To observe the mediating effect of cognitive function between anxiety and daily living ability in patients with acute carbon monoxide (CO) poisoning. Methods The convenience sampling method was used to select 100 patients with CO poisoning admitted to NO. 2 People’s Hospital of Fuyang City from January 2018 to January 2021 as the research objects. After the patient was out of life-threatening, the Montreal cognitive assessment scale (MoCA) was used to assess the patient’s cognitive function, the self-rating anxiety scale (SAS) was used to assess the patient’s anxiety, and the daily life ability scale (ADL) was used to assess the daily life ability of the patient. After preliminary Pearson correlation analysis was used to test the relationship between cognitive function, anxiety, and daily life ability, the mediation effect test procedure was used to test the mediating effect of cognitive function between anxiety and daily life ability. Results The SAS score of patients with CO poisoning was (64.75±5.61) points, the ADL score was (36.38±5.19) points, and the MoCA score was (24.57±3.32) points. Pearson linear correlation analysis showed that SAS scores of patients with CO poisoning were negatively correlated with ADL scores and MoCA scores (r = -0.321, -0.440, P < 0.001); ADL scores were positively correlated with MoCA scores (r = 0.459, P < 0.001). Using the equation Y=cX+e1 to investigate the influence of anxiety on the ability of daily living, it was concluded that the regression effect was significant; using the equation M=AX+e2 to investigate the impact of anxiety on cognitive function, it was concluded that the regression effect was significant; the equation Y=c’X+bM+e3 was used to investigate the mediating effect of cognitive function between anxiety and daily living ability, and it was concluded that the regression effect was significant, which was a complete mediating effect model. Conclusion Cognitive function plays a completely mediating effect between anxiety and daily living ability in patients with CO poisoning.
