Application value of donor-derived cell-free DNA in renal graft ischemia-reperfusion injury#br# |

Abstract Objective To explore the application value of donor-derived cell-free DNA (dd-cfDNA) in the treatment of renal graft ischemia-reperfusion injury after renal transplantation. Methods Prospectively enrolled 40 subjects who underwent allograft renal transplantation at Renal Transplantation Center, Renji Hospital, Shanghai Jiao Tong University School of Medicine from March to May 2020. Peripheral blood was collected on the 1st, 3rd, and 7th day after operation, and dd-cfDNA% in peripheral blood of the patients was detected by second-generation sequencing method, and creatinine at the corresponding time point of the patients was detected. Results In this study, renal function of all patients recovered smoothly after renal transplantation, without delayed recovery of graft function or rejection occurred. Overall analysis found that the overall comparison of creatinine measurement and dd-cfDNA% at three time points showed highly statistically significant difference (P < 0.01). Further pairwise comparison showed that there were highly statistically significant differences in creatinine measurements and dd-cfDNA% concentration at any two time points in the group (P < 0.01). Creatinine measurement on 3rd day after operation was 245.65 μmol/L (95% confidence interval:172.70 to 318.60 μmol/L) lower than that on 1st day after operation. Creatinine measurement was further reduced by 195.95 μmol/L (95% confidence interval: 153.02 to 238.88 μmol/L) on 7th day after operation compared with 3rd day after operation. The dd-cfDNA% concentration on 3rd day after operation was 8.97% (95% confidence interval: 7.71% to 10.24%) lower than that on 1st day after operation. The dd-cfDNA% concentration was further reduced by 1.63% (95% confidence interval: 1.32% to 1.94%) on 7th day after operation compared with 3rd day after operation. Conclusion dd-cfDNA is differentially expressed in different periods of renal graft ischemia-reperfusion after renal transplantation, which is of certain value in evaluating the degree of renal graft ischemia-reperfusion injury.
