4 Effects of reinforcing-reducing method by twirling of acupuncture on contents of malondialdehyde and glutathione peroxidase in hippocampus of spontaneously hypertensive rats
XUE Yanjun WU Jiaojuan ZHANG Xudong YU Pengcheng WU Xiaoli LIU Qingguo
Objective To explore the effects of reinforcing-reducing method by twirling of acupuncture on contents of malondialdehyde (MDA) and glutathione peroxidase (GSH-Px) in hippocampus of spontaneously hypertensive rats. Methods A total of 18 spontaneously hypertensive rats were divided into three groups by random number table method: reducing method group, reinforcing method group, and model group, with six rats in each group. Six WKY rats were served as blank group. All rats were SPF grade, nine-week-old male rats. After one week of acclimatization, reducing method group and reinforcing method group were intervened with corresponding manipulation of reducing and reinforcing on bilateral tàichōng once a day for 14 days. Model group and blank control group received the same grasping and fixation stimulation at the same points of reinducing method group and reinforcing method group. Hippocampus samples were collected on the 15th day, and the morphology of hippocampus was observed by HE staining, MDA content was detected by thiobarbituric acid method and GSH-Px content was detected by colorimetry. Results HE staining results showed that the cells in the blank group had complete structure, regular morphology, normal intercellular space, most nuclei were deeply stained, nucleoli were clear, and no denatured necrosis was observed. Compared with the blank group, the staining in the model group was significantly lighter, and nuclear pyknosis, cell lysis and fragmentation, vacuolar degeneration and apoptotic cells were observed. Compared with the model group, the cell contour was clear, the cell staining was deep, the nucleoli were clear, and the cell structure was basically normal in both the reducing method group and reinforcing method group. MDA content in model group was higher than that in blank group, while GSH-Px content in model group was lower than that in blank group, and the differences were highly statistically significant (P < 0.01). MDA contents in reducing method group and reinforcing method group were lower than those in model group, the contents of GSH-Px in the reducing method group and reinforcing method group were higher than those in the model group, and the differences were statistically significant (P < 0.05 or P < 0.01). Conclusion Reinforcing-reducing method by twirling of acupuncture can regulate the expression level of MDA and GSH-Px in the hippocampus of spontaneously hypertensive rats, inhibit the oxidative reaction, and reduce the damage of oxygen radicals to the hippocampus of spontaneously hypertensive rats, which may be one of the protective mechanisms of reinforcing-reducing method by twirling of acupuncture on hypertensive hippocampal injury.

36 Application and evaluation of experimental animal model of hypertension
WEN Xin ZHANG Qi▲
Hypertension is a major risk factor for cardiovascular diseases, and the prevalence of hypertension is high, and the patients are getting younger in recent years. Nowadays, medical science is not satisfied with the treatment of hypertension and the main treatment is to control blood pressure and prolong survival time. The establishment of hypertension experimental animal models is of great significance for the study of the pathogenesis of hypertension, clinical treatment, therapeutic targets and drug development. At present, animal models are mostly used in small animals such as rats and mice, while the application of large animal models are limited under the influence of experimental cost, laboratory environment and experimental technology. In order to select suitable animal models for the study of disease in the future, this paper briefly introduces hypertension animal models such as spontaneously hypertensive rat, salt-sensitive hypertensive rat and 2-kidney-1-clip hypertensive rat.

40 Research progress on the mechanism of depression and anxiety of antihyperglycemics
ZHOU Zijing YAN Shuang
Depression is one of the risk factors for type 2 diabetes mellitus (T2DM), and the incidence of depression in T2DM patients is significantly higher than that in non-T2DM population. Depression is often accompanied by severe anxiety symptoms. There are few systematic studies on the relationship between T2DM and depression and anxiety state. There are overlapping biological effects in the pathogenesis and treatment mechanism of both. Existing studies have found that Metformin, Thiazolidinedione, GLP-1 receptor agonists, DPP-4 inhibitors, insulin and other antihyperglycemics may relieve depression and anxiety state through neurobiochemical, neuroendocrine, neuroimmune and other mechanisms. Therefore, it is of great clinical significance to explore the biological mechanism of antihyperglycemics in relieving depression and anxiety state.

45 Research progress on the relationship between gastric microecology, gastric diseases and gastric mucosal immunity
QIN Heping1 YI Ping2
Stomach microecology is closely related to gastric diseases and gastric mucosal immunity. In addition to Helicobacter pylori colonizing the gastric, there are other bacteria and fungi such as Firmicutes, Proteobacteria, Bacteroides, Fusobacteria and Actinomycetes. There are many factors influencing the gastric microecology, such as microorganisms, drugs and proton pump inhibitors, which lead to highly dynamic changes in the gastric microecology. In patients with common diseases such as functional dyspepsia, chronic gastritis, gastric ulcer and gastric cancer, the diversity of gastric flora is reduced, and the abundance of pathogenic bacteria increases accordingly, and gastric microecological imbalance occurs. When the gastric microecology is out of balance, it mainly induces gastric mucosal immunity through intrinsic lymphoid cells, causing gastric mucosal damage.

48 Analysis of the intention and influencing factors of doctors in tertiary hospitals to refer down patients in the chronic recovery period
WANG Xia1,2 DAI Tao2▲ HUANG Ju2
Objective To analyze clinicians’ behavioral intentions and influencing factors of refer down to patients in the chronic recovery period. Methods From October 2018 to October 2019, the Questionnaire for Clinicians’ Referral Intentions in Tertiary Medical Institutions designed based on the theory of planned behavior was used to evaluate the results of oxght tertiary general hospitals in Xuzhou of Jiangsu Province, Hangzhou of Zhejiang Province, and Jining of Shandong Province. Clinicians conducted a cross-sectional survey and recovered 442 valid questionnaires. Pearson correlation was used to analyze the relationship between the doctor’s attitude toward referral patients, subjective norms, and perceptual behavior control influencing factors and the doctor’s refer down intention. Finally, a structural equation model was established to analyze the degree of influence of various factors on the doctor’s refer down intention and the influence path. Results 76.5% of the surveyed doctors had the intention to refer down patients in the chronic recovery period, among which, the intention of the senior professional title clinician was the highest, reaching 92.7%. There was a statistically significant difference in the referral intention of clinicians with different professional titles (P < 0.01). Attitudes, subjective behavioral norms, and perceived behavioral control had a direct impact on behavioral intentions, explaining 52%, 42%, and 23% of the differences in behavioral intentions. Attitudes had the greatest influence on intentions. Conclusion Attitude and subjective norms are the most important factors influencing the downward transfer intentions of clinicians in tertiary hospitals. Attitude has the greatest impact, followed by the social pressure that doctors feel from hospital deans, department directors, and colleagues, and patients and their families. The degree of support has little effect on the doctor’s transfer. In addition, the higher the degree of completeness of the transfer standard, the more convenient the medical insurance approval system, and the higher the proportion of doctors with high professional titles, the more favorable it is to promote the refer down of doctors to patients.

ZHENG Gongchi1,2 ZHAO Jun2 LI Xi2 FENG Yikai1,2 CHEN Junjia1,2 WANG Fang2
Objective To forecast the demand for nursing bed resources and the gap between supply and demand in Shunyi District, Beijing during the “14th Five-Year Plan” period, and to put forward policy recommendations for resource allocation. Methods The questionnaire survey was used to obtain data on the resident elderly population in Shunyi District from January 2015 to December 2019 and data on the allocation of nursing beds in Shunyi District in December 2019. The time series analysis method was used to predict the number of resident elderly population from 2020 to 2025. The demands and gaps of nursing beds were calculated by the elderly population, the proportion of institutionalized elderly, and the proportion of nursing beds. Results From 2020 to 2025, there will be a gap of 742-3465 nursing beds in the district, with a supply-demand ratio of 0.36-0.73. At the same time, the supply-demand ratio was generally less than one in all streets and towns, and the supply of nursing beds is difficult to meet service demand. Conclusion During the “14th Five-Year Plan” period, Shunyi District may have problems such as insufficient total nursing bed resources and uneven regional distribution. It is recommended to increase the total number of nursing bed allocations. A certain amount of resources will be given to streets and towns with large gaps in nursing beds and relatively low supply and demand. In addition, institutions should improve their elderly care service capabilities and promptly introduce regional policies and regulations related to nursing beds.

73 Analysis of the level of perception of autonomy and its influencing factors in patients with coronary artery heart disease
YANG Xiaolan SONG Minmin XIE Hanhua▲
Objective To analyze of the level of perception of autonomy and its influencing factors in patients with coronary artery heart disease (CHD). Methods A total of 213 patients with CHD admitted to Suzhou Jiulong Hospital Affiliated to Shanghai Jiao Tong University School of Medicine from April 2017 to May 2020 were selected. Simplified Chinese version of perception of autonomy scale (PEAS-SCV) was used to evaluate the level of patients’ perception of autonomy. Baseline data of patients were collected and recorded. The PEAS-SCV scores of patients with different baseline data characteristics were compared to analyze the related factors that might affect their perception of autonomy level. Results The mean PEAS-SCV score of CHD patients was 53.00(52.00,56.00) points. The PEAS-SCV scores of patients with depression, low life satisfaction and low self-efficacy were lower than those of patients without depression, normal life satisfaction and normal self-efficacy, and the differences were statistically significant (all P < 0.05). The results of multiple linear regression analysis showed that depression (X1), low life satisfaction (X2) and low self-efficacy (X3) were the influencing factors of low autonomy perception level in CHD patients. The equation Y=35.179+5.493X1+11.326X2+10.188X3 was obtained (R2 = 0.384, P < 0.05). Conclusion The perception of autonomy of CHD patients is generally at a medium level, which may be related to the patients’ depression emotion, life satisfaction, self-efficacy and other factors. With the aggravation of depression emotion, the decrease of life satisfaction, and the decrease of self-efficacy, the autonomy perception level of patients decreases, which should be paid clinical attention to.

130 Discussion on the treatment of idiopathic pulmonary fibrosis accompanied by gastroesophageal reflux disease based on “acrid opening and bitter downbearing” theory
ZHOU Hongwei1 YU Huiyong2 WANG Chengxiang2
Idiopathic pulmonary fibrosis (IPF) is a common respiratory disease, with dyspnoea after progressive exercise and dry cough as the main symptoms, often accompanied by gastroesophageal reflux disease (GERD). The pathogenes between IPF and GERD is not clear. So far, Western medicine has no good means of treatment. The main pathogenesis of traditional Chinese medicine of IPF combined with GERD is characterized by the disease of lung, spleen, stomach and kidney, the internal obstruction of dampness-heat and phlegm-stasis, the disorder of qi activity of triple energizer, and the imbalance of yin and yang. The method of acrid opening and bitter downbearing is widely used to treat the diseases of complexity of cold and heat, disorder of ascending and descending, and combination of pathogenic factors. In view of the pathogenesis of IPF combined with GERD, the method of acrid opening and bitter downbearing can be discussed and treat this disease from the point of view of appling herbs of heat and cold nature for balance of yin and yang, ascending and descending for restoring of qi activity, dispersing damp-heat for dispelling of pathogenic factors.

134 Application of national medicine master Wang Lie’s idea of preventive treatment of disease in pediatric pulmonary diseases
GUO Tingting1 SUN Liping2 WANG Lie2
The traditional idea of “preventive treatment of disease” in traditional Chinese medicine is mainly reflected in four aspects: prevention before disease, prevention after disease, prevention and cure after change, prevention and cure after stagnation. Professor Wang Lie, a master of traditional Chinese medicine, guides the treatment of pediatric pulmonary diseases, such as asthma, wheezing cough, infantile cold caused by food accumulation (repeated respiratory tract infection), nasal asthma, and so on, and creatively put forward the theories of “asthma seedling stage”“three-stage treatment”“asthma on chronic cough”“nasal asthma”, and so on. In the specific treatment, Professor Wang Lie proposes early intervention, fine treatment and prevention, focusing on the treatment of asthma seedling stage; he proposes that repeated respiratory tract infection is mostly caused by food accumulation and lung heat, treating spleen disease to prevent lung disease, cutting off the occurrence and progress of lung disease; he proposes that rhinorrhea and asthma have the same origin, treating both rhinorrhea and asthma, seeking the root of the disease, so as to prevent the disease from rebirth. Professor Wang Lie’s idea of “preventive treatment of disease” runs through the whole process in the treatment of children’s pulmonary diseases, and the curative effect is remarkable, which is worthy of clinical reference and promotion.
