DOI: 10.20047/j.issn1673-7210.2023.24.07
Pancreatic cancer (PC) is an aggressive, low early diagnosis rate, poor prognosis of malignant digestive system tumors. Experimental studies have found that epithelial mesenchymal transformation (EMT) is closely related to the pathogenesis, metastasis, and drug resistance of pancreatic cancer, which involves multiple signaling pathways and transcriptional regulatory factors. The active components of traditional Chinese medicine are closely related to EMT-related pathways and factors, thus effectively inhibiting the invasion process of tumor cells. This paper reviews the regulatory mechanism and function of EMT in pancreatic cancer and the progress of traditional Chinese medicine in the treatment of pancreatic cancer through the intervention of EMT, in order to provide a theoretical basis for the research and treatment of pancreatic cancer.

DOI: 10.20047/j.issn1673-7210.2023.24.08
Lymphocyte function-associated antigen 1(LFA-1) and intercellular adhesion molecule (ICAM-1) are homologous ligands, the interaction of LFA-1/ICAM-1 is crucial for the proliferation and infiltration of immune cells. The pathogenesis of ophthalmoxerosis has afferent arm and efferent arm two pathways. The afferent arm refers to the transfer of antigen presenting cells to lymph nodes after being stimulated by the ocular surface; the efferent arm refers to the transfer of activated lymphocytes from lymph nodes to the ocular surface. LFA-1/ICAM-1 plays an important role in both the afferent and efferent arms, and is important for the diagnosis and treatment of diseases related to immune homeostasis destruction. The drugs of Lifitegrast, corticosteroids, and cyclosporine A used to ophthalmoxerosis from the perspective of immunopathology have achieved clinical results. This article reviews the pathogenesis of LFA-1/ICAM-1 in ophthalmoxerosisand the research progress of immunosuppressive drugs.

DOI: 10.20047/j.issn1673-7210.2023.24.09
Alopecia areata is a non-cicatricial inflammatory alopecia. Studies have shown that psychological stress is closely related to the occurrence and development of alopecia areata. There are similarities and commonalities between the theory of emotional pathogenicity in traditional Chinese medicine and psychological stress in modern medicine. The internal injury of seven emotions and the disorder of qi and blood are important factors of alopecia areata. Based on the theory of emotional pathogenicity in traditional Chinese medicine, this paper discusses the effect of psychological stress on the mechanism of alopecia areata from three aspects: neuro-endocrino-immune network disorder, oxidative stress and inflammatory response, scalp microcirculation disorder, and emphasizes the importance of individualized psychological intervention in the treatment of alopecia areata, with a view to providing new ideas for clinical diagnosis and treatment of alopecia areata.

44 Research progress on the production of antimicrobial peptide LL37 and its effects on inflammatory skin diseases
DUAN Meng LEI Tiechi
DOI: 10.20047/j.issn1673-7210.2023.24.10
Antimicrobial peptide LL37 is the only human Cathelidin family antimicrobial peptide found in clinical practice. It can resist the invasion of pathogens, and also play a role in immune regulation and immunochemotatic to trigger or exacerbate the inflammatory process. LL37, which is highly expressed in patients with acne rosacea and psoriasis, can induce mast cells and other inflammatory cells to secrete a variety of proinflammatory cytokine and chemokines to participate in the formation of skin lesions. However, the expression of LL37 in patients with atopic dermatitis is reduced, which damages the skin barrier function and makes them more susceptible to bacterial infection. Therefore, LL37 as a target is expected to become a new idea and method for the treatment of inflammatory skin diseases such as acne rosacea. This article reviews the expression process and regulatory mechanism of LL37, and its role and related mechanisms in patients with acne rosacea, psoriasis, and atopic dermatitis.

48 Research progress on renal injury induced by calcineurin inhibitor
SUN Xuan1 ZHANG Kunying2
DOI: 10.20047/j.issn1673-7210.2023.21.11
Calcineurin inhibitor (CNI) is a kind of widely used immunosuppressive drug. Ciclosporin A and Tacrolimus are commonly used in clinic. CNI exerts immunosuppressive effects by inhibiting T cell activation, inducing T cell apoptosis, and inhibiting the expression of various immune factors, and has significant therapeutic effects on autoimmune diseases such as systemic lupus erythematosus and membranous nephropathy. Meanwhile, CNI also has toxic side effects that can cause kidney damage. CNI can damage the glomerulus, renal microvasculature, renal tubules, and renal interstitium to varying degrees by activating the renin-angiotension-aldosterone system, destroying the balance of local vasoactive substances in the kidney, promoting oxidative stress, and other ways. Urinary retinol binding protein and other biomarkers may have some value in early detection of renal injury caused by CNI. Calcium channel blocker is considered as an effective means to alleviate CNI induced renal injury. This article aims to provide theoretical basis and prevention and treatment ideas for renal injury caused by CNI, and promote the rational and safe use of CNI in clinical practice.

Objective To investigate the related factors of asthenic syndrome in patients with malignant tumors, and construct a risk assessment model to predict its efficacy. Methods A total of 327 patients with malignant tumors who were hospitalized in the oncology department of a top-three hospital in Guangxi from May to August 2020 were selected. General data and laboratory indicators of the patients were collected, and grip strength was measured. The risk assessment model was determined by binary logistic regression analysis, and the model effect was tested by subject working characteristic curve. Results The incidence of asthenic syndrome in malignant tumor patients was 28.4%. There were significant differences in gender, age, grip strength, arm circumference, calf circumference, body mass index, erythrocyte, hemoglobin, total protein, albumin and prealbumin between the two groups (P<0.05). Logistic regression results showed that gender, grip strength, arm circumference and albumin were the influencing factors of asthenic syndrome in malignant tumor patients (OR<1, P<0.05). H-L goodness of fit test was used to test the FMS risk prediction model (P>0.05), and the model fit was good. When the area under ROC curve was 0.852 (95%CI: 0.807-0.897, P<0.05), the sensitivity was 0.849, and the specificity was 0.722, the Youden index was the largest (0.572), and the prediction model score was 149 points. Conclusion The incidence of asthenic syndrome is high in patients with malignant tumors, and female patients and lower arm circumference, grip strength and albumin are risk factors for asthenic syndrome in patients with malignant tumors. 149 points is the optimal threshold value for the asthenic syndrome risk assessment model in patients with malignant tumors. The constructed asthenic syndrome risk assessment model has good diagnostic efficacy and can be verified in the next large sample.

Objective To explore the relationship between self-perceived burden and psychological vulnerability in elderly patients with chronic renal failure sustained hemodialysis. Methods A total of 102 elderly patients with chronic renal failure who underwent maintenance hemodialysis treatment in the Renal Disease Intensive Ward of Jiangsu People’s Hospital from September 2019 to May 2022 were selected as the study objects. General data, scores of mental vulnerability questionnaire(MVQ), Connor-Dacidson psychological resilience scale (CD-RISC), and self-perceived burden scale (SPBS) were collected. The correlation of SPBS score with MVQ score and CD-RISC score were analyzed. According to the scores of SPBS, patients were divided into group A (SPBS score<30 points) and group B (SPBS score≥30 points), and the influencing factors of patients’ self-perceived burden were analyzed. Results Among 102 elderly patients with chronic renal failure, MVQ score was (56.88±12.25) points, CD-RISC score was (35.73±9.25) points, SPBS score was (27.25±8.76) points, of which 54 cases (52.94%) in group A and 48 cases (47.06%) in group B. Pearson correlation analysis showed that SPBS score of elderly patients with chronic renal failure maintenance hemodialysis was positively correlated with MVQ score (r>0, P<0.05), and negatively correlated with CD-RISC score (r<0, P<0.05). The dialysis time of group B was longer than that of group A, the score of age, medical expense and MVQ was higher than that of group A, and the score of CD-RISC was lower than that of group A, with statistical significances (P<0.05). Logistic regression analysis showed that dialysis time and MVQ score were risk factors for patients’ self-perceived burden (OR>1, P<0.05), and CD-RISC score were protective factors for patients’ self-perceived burden (OR<1, P<0.05). Conclusion Senile chronic renal failure patients with persistent hemodialysis generally have different degrees of self-perceived burden, and its degree is closely related to dialysis time and psychological vulnerability of patients.

DOI: 10.20047/j.issn1673-7210.2023.23.32
Irritable bowel syndrome (IBS) is a common gastrointestinal dysfunction, of which diarrhea type IBS(IBS-D) is the most common. Although the pathogenesis of IBS-D is related to such as spleen deficiency, dampness, and liver depression, Shi Suhua believes that chronic diarrhea will damage the kidney, no matter how the pathogenesis evolves, it will eventually develop into spleen and kidney yang deficiency. “The brain” regulates and affects gastrointestinal function, and brain-gut dysfunction runs through IBS-D. On the basis of inheriting Professor Li Zhigang’s “Tongdu Qishen” method of acupuncture treatment of mental illness, Shi Suhua proposed to treat IBS-D with tiaoshen anchang moxibustion. The moxibustion has the characteristics of large amount and special effect, ginger and moxibustion are used together, and the Ren and Du meridians are adjusted at the same time. With the help of the efficacy of meridians and acupoints and the warming effect of moxibustion and ginger, the spleen and kidney yang qi are warmed to the maximum extent, and the brain and gut function are reconciled. This article will combine’s Shi Suhua’s understanding of the etiology and pathogenesis of IBS-D, introduce the treatment of tiaoshen anchang moxibustion, and list typical cases for reference.

140 Professor Zhang Yu’s experience in treating nephrotic proteinuria based on “strengthening essence and reducing toxicity”
ZHANG Qi ZHANG Yu
DOI: 10.20047/j.issn1673-7210.2023.24.33
Proteinuria is an important observation index for clinical treatment of chronic kidney disease patients. It can not only reflect kidney damage, but also has toxic effects on the kidney. It is the strongest predictor of end-stage kidney disease. Therefore, how to eliminate proteinuria is very important for patients with chronic kidney disease. Professor Zhang Yu believes that the production of proteinuria is based on the deficiency of spleen and kidney, the disadvantage of lung qi, and is also induced by the toxin of rheumatism and blood stasis. Therefore, this article explores and summarizes the six methods of tonifying the kidney and nourishing the yuan, strengthening the spleen and nourishing the earth, promoting lung and clearing diuresis, dispelling wind and detoxifying, resolving dampness and turbidity, and removing blood stasis and detoxifying from the perspective of “strengthening essence and reducing toxicity”. All methods should be carried out simultaneously and used alternately to achieve the goal of “strengthening the essence and supporting the root, eliminating pathogenic factors and preventing the change”, and provide a broader idea for clinical prescription of nephrotic proteinuria.

Bayu Wei’s osteopathy school in the treatment of distal radius fractures, which advocates “conservative treatment without surgery, minimally invasive treatment without opening”. The conservative treatment is based on the principle of “viewing the skin and feeling the bone, observing the shape and dividing the age, apply their respective methods”. The core reduction technology is “traction and pulls on the radius, abduction and supination of the wrist, compression of the fracture end and turning to the opposite side, separation of the radius and ulna”. Internal medication, two-stage syndrome differentiation, tonifying the kidney and promoting blood circulation throughout the entire process. Apply “Yizhu Powder” externally to the affected area, use a new limited contact pressure splint to fix the broken end to reduce swelling, and cooperate with elbow flexion suspension to reduce displacement. Combining muscle management with martial arts to accelerate the recovery of injured limbs. This diagnosis and treatment scheme improves the effect of conservative management for distal radius fracture, and enriches the theory of traditional Chinese medicine for fracture treatment.

Against the backdrop of the construction of a healthy China, Sichuan Province firmly implements the decisions and deployments of the Party Central Committee, prioritizes the development of ensuring the health of the entire Sichuan people, and implements the great health and great health strategy throughout the province. The strategic implementation focuses on the western ethnic areas, with the main measures being to increase support for the construction of grassroots medical and health talent teams in ethnic areas, and to implement the targeted medical student training project in ethnic areas. The training of Yi nationality targeted medical students is an important component of this project, and the training task is undertaken by medical colleges within the province. Chengdu Medical College, as one of the colleges and universities undertaking the training task, has conducted targeted training for fresh graduates of rural medicine majors from Liangshan, Ganzi, and Aba Health Schools since 2012 to 2020. During the training, it is found that there are mainly problems such as lack of learning motivation for Yi nationality targeted medical students, poor connection between theoretical courses and clinical practice, and difficulty in improving academic qualifications. Based on this, future training should focus on the following three aspects: firstly, combining the characteristics of Yi nationality targeted medical students with individualized teaching; secondly, efforts to improve the professional skills of Yi nationality targeted medical students; thirdly, expanding the education improvement path for Yi nationality targeted medical students.
