Abstract Antimicrobial peptide LL37 is the only human Cathelidin family antimicrobial peptide found in clinical practice. It can resist the invasion of pathogens, and also play a role in immune regulation and immunochemotatic to trigger or exacerbate the inflammatory process. LL37, which is highly expressed in patients with acne rosacea and psoriasis, can induce mast cells and other inflammatory cells to secrete a variety of proinflammatory cytokine and chemokines to participate in the formation of skin lesions. However, the expression of LL37 in patients with atopic dermatitis is reduced, which damages the skin barrier function and makes them more susceptible to bacterial infection. Therefore, LL37 as a target is expected to become a new idea and method for the treatment of inflammatory skin diseases such as acne rosacea. This article reviews the expression process and regulatory mechanism of LL37, and its role and related mechanisms in patients with acne rosacea, psoriasis, and atopic dermatitis.
