Expert consensus on the clinical diagnosis and treatment of ductoscopy (2022)
DOI: 10.20047/j.issn1673-7210.2023.11.01
Ductoscopy is the top test method for patients with nipple discharge. It has the characteristics of convenient operation, small trauma, direct vision in the duct, real-time imaging recording, and has become the main diagnostic and adjuvant therapy tools for breast intraductal lesions. However, there is no clinical consensus on the application, diagnosis, and treatment of ductoscopy, which limits its clinical popularization. Breast Disease Branch of Chinese Association for Research and Advancement of Chinese Medicine reached consensus on indications, contraindications, equipment and personnel configuration, preparation before examination, operation technique and considerations, manifestations under ductoscopy, endoscopic adjuvant therapy, and complications and management, etc. That is to provide a clinical reference for the optimization and standardization of the technical operation specification of ductoscopy. Looking forward to improving the level of ductoscopy treatment in China.

WEI Naxin SUN Hongyan
DOI: 10.20047/j.issn1673-7210.2023.11.02
Objective To excavate and analyze the literature on the treatment of primary Sjogren’s syndrome (pSS) by traditional Chinese medicine, and to summarize the syndrome characteristics and medication rules of pSS. Methods The literature related to traditional Chinese medicine treatment of pSS from January 2011 to December 2021 was retrieved from CNKI and Wanfang data, and the traditional Chinese medicine syndrome elements, tongue and pulse, prescriptions, and so on were counted successively by Excel. SPSS 25.0, SPSS Modeler and Cytoscape were used for cluster analysis, association analysis, and complex network analysis respectively. Results A total of 145 literatures were included, and the syndrome types were mainly Qi and Yin deficiency. The tongue pulse was mainly fine pulse, red tongue, number pulse, and moss stripping. In addition to the self-designed formula, Yiwei decoction was often used; there were 197 kinds of traditional Chinese medicines involved, mainly ophiopogon ophiopogon, licorice and radix rehmanniae. The medicinal tastes were mainly sweet, bitter and pungent, while the medicinal properties were mainly cold and flat, which were mainly attributed to the three classics of lung, liver, and stomach. Seven core drug groups were obtained by systematic cluster analysis. A total of 14 groups of association rules were obtained by association analysis. Complex network analysis showed that the core drug combinations were ophiopogon ophiopogon, Radix rehmanniae, Radix ginseng and licorice. Conclusion pSS syndrome type is based on “Qi and Yin deficiency, stasis, toxicity, phlegm, and dampness as the standard”, and the treatment is based on “tonifying liver and kidney, tonifying lung and tonifying stomach”, supplemented by activating blood circulation and removing blood stasis, clearing heat and cooling blood, regulating qi, and so on.

DOI: 10.20047/j.issn1673-7210.2023.11.03
Objective To investigate the expression of PRL-3 gene in glioblastoma and its effect on the sensitivity of Temozolomide in glioblastoma. Methods The expression of PRL-3 gene in glioblastoma and normal tissues was analyzed by the TCGA database, and postoperative tumor tissues of 15 patients with glioblastoma diagnosed and treated by the First Affiliated Hospital of Air Force Military Medical University (ten primary cases, five recurrent cases) were further selected, the expression of PRL-3 positive cells in primary and recurrent glioblastoma tissues was detected by immunohistochemistry. Human glioblastoma LN229 and temozolomid-resistant LN229/TMZ-R cell lines were selected to detect PRL-3 mRNA and protein expression by qRT-PCR and Western blot. LN229, SHG44, and U87 cell lines which was insensitive to Temozolomide were selected, and normal group, control group, no-load liposome group, PRL-3 overexpression group, and PRL-3 downregulation group were set, respectively. The PRL-3 mRNA expression of control group, no-load liposome group, PRL-3 overexpression group, and PRL-3 downregulation group were compared to verify the success of PRL-3 overexpression and downregulation. Then the normal group, PRL-3 overexpression group, and PRL-3 downregulation group were treated with Temozolomide(LN229 and SHG44 cell lines were treated with 100 μmol/L Temozolomide, U87 cell line were treated with 500 μmol/L Temozolomide), while the normal group was not treated, the cell activity was evaluated by CCK-8 method after 72 h. Results Database analysis showed that PRL-3 expression in primary glioblastoma was higher than that in normal tissues and increased with the increase of tumor grade (P<0.05). The immunohistochemical scores of PRL-3 positive cells in recurrent glioblastoma tissues was higher than that in primary glioblastoma tissues (P<0.05). The mRNA expression of PRL-3 in LN229/TMZ-R cell line was higher than that in LN229 cell line (P<0.05), and PRL-3 protein was highly expressed in LN229/TMZ-R cell line. The PRL-3 mRNA in PRL-3 overexpression group was higher than that in normal group, and the PRL-3 downregulation group was lower than that in normal group (P<0.05). In LN229, SHG44, and U87 cell lines, the cell activity of control group was lower than that of normal group, the cell activity of PRL-3 overexpression group was higher than that of control group, and the cell activity of PRL-3 downregulation group was lower than that of control group (P<0.05). Conclusion The low expression of PRL-3 gene can enhance the sensitivity of glioblastoma to Temozolomide treatment, which is expected to be a beneficial target for synergistic chemotherapy with Temozolomide.

Research progress in pulmonary sarcoidosis models
HU Zhuannan ZHANG Shaoyan HUANG Xing LI Cui WU Xianwei ZHOU Wei QIU Lei LU Zhenhui
DOI: 10.20047/j.issn1673-7210.2023.11.08
Sarcoidosis is a granulomatous inflammatory disease of unknown etiology that can involve multiple organs throughout the body, with the lungs being the most common. Its pathogenesis is related to excessive immune, microbial infection, environmental, and genetic factors, and there is still no comprehensive or widely accepted disease model. The studies for sarcoidosis have used Propionibacterium acnes, Mycobacterium tuberculosis, and related derivatives, nanoparticles, ApoE and Tsc2 knockout mice to establish animal models. Cellular models use peripheral blood monocytes or macrophages to replicate the immune environment of the disease, while mathematical models based on clinical data provide more options for basic research of sarcoidosis. This paper summarizes the advantages and limitations of pulmonary sarcoidosis models, and then develops appropriate models to explore the pathogenesis of pulmonary sarcoidosis, find new targets for disease treatment and drug evaluation.

Research progress of combined application of negative pressure drainage after total knee replacement
DING Chuan1 TANG Hongjuan2 MA Xianming3 ZHOU Yi3 XIANG Bingyan3 DENG Jiang3▲
DOI: 10.20047/j.issn1673-7210.2023.11.10
Total knee arthroplasty (TKA) is currently the gold standard for the treatment of end-stage knee osteoarthritis. And after decades of development and innovation of TKA, the combined application of closed negative pressure drainage(CSD) technology has been widely used as a conventional drainage solution after TKA. It is believed to significantly reduce the formation and relief of hematoma after TKA. Pain, accelerate wound healing, promote the rehabilitation of affected limbs, and promote the implementation of the concept of rapid postoperative recovery plans, but at the same time it may increase the blood loss after TKA, which will lead to an increase in the patient’s blood transfusion rate and longer hospital stays. There is still controversy about the combined application of CSD technology after TKA. This article will review the combined application methods and research progress of CSD technology after TKA. The main methods include CSD technology combined with clamp technology, normal pressure to negative pressure program, negative pressure wound therapy, controller pressure adjustment program, CSD technology combined with autologous blood transfusion and intra-articular tranexamic acid injection, etc., in order to provide reference for clinicians.

Research progress of impulse oscillometry in the diagnosis and treatment of bronchial asthma in children
JIN Lingling1 YANG Lei2
DOI: 10.20047/j.issn1673-7210.2023.11.11
Bronchial asthma is a common chronic inflammatory disease of airway in children’s respiratory system, and its incidence is increasing year by year. Therefore, early diagnosis and control of disease progression are particularly important. At present, the diagnosis of bronchial asthma in children is mainly based on symptoms, signs, and pulmonary function examination. However, the routine pulmonary function examination requires strenuous breathing, and its detection results are not reliable for young children. Therefore, a new detection method is needed for young children. The emergence of impulse oscillometry (IOS) makes up for the deficiencies of conventional lung function detection methods. As a method to evaluate lung function, it is simple to operate and easy to complete, providing effective information for the accurate diagnosis and treatment of bronchial asthma in children. The clinical application of IOS is not only reflected in the diagnosis of bronchial asthma, but also has great value in the assessment of airway reactivity, efficacy monitoring, and prognosis assessment. This paper reviews the basic principles of IOS and its clinical application in children with bronchial asthma, in order to provide new ideas for the diagnosis and treatment of this disease.

Research progress in the prevention and treatment of glucocorticoid-induced osteoporosis by traditional Chinese medicine
ZHANG Xinxin1 YANG Fengwen2 LI Liqiang2
DOI: 10.20047/j.issn1673-7210.2023.11.12
Glucocorticoids are commonly used in the treatment of nephrotic syndrome, but long-term and massive use of glucocorticoids will lead to a variety of complications, among which osteoporosis is one of the most common complications. How to take effective prevention and treatment measures to avoid or reduce hormone-induced osteoporosis is very critical. According to traditional Chinese medicine, kidney dominating bone and bone produces bone marrow, the deficiency of kidney essence, and yin-yang disharmony of kidney are the key factors of the disease. Traditional Chinese medicine can treat glucocorticoid-induced osteoporosis based on syndrome differentiation by nourishing yin and clearing heat, warmly invigorating spleen and kidney, and coordinating yin and yang. In recent years, many clinical studies have shown that traditional Chinese medicine has achieved remarkable effects in the prevention and treatment of glucocorticoid-induced osteoporosis, and some progress has been made in the experimental research to explore its related mechanism, but it is still in the stage of exploration. This article reviews the progress in the prevention and treatment of glucocorticoid-induced osteoporosis by traditional Chinese medicine in recent years, in order to provide scientific theoretical basis for clinical treatment.

Objective To analyze the correlation between Alberta stroke program early CT score (ASPECTS) based on RAPID software and core infarction volume in patients with anterior circulation acute ischemic stroke with large vessel occlusion (AIS-LVO) over time window. Methods Ninety-eight patients with anterior circulation AIS-LVO over time window admitted to Yongchuan Hospital Affiliated to Chongqing Medical University from January 2019 to December 2021 were selected and further divided into patients with definite onset time (51 cases) and patients with unclear onset time (47 cases) according to the onset time. Clinical and imaging data of these patients were collected. ASPECTS was calculated by RAPID software. Spearman correlation coefficient was used to analyze the correlation between ASPECTS and core infarction volume, and receiver operating characteristic (ROC) curve was used to analyze the predictive value of ASPECTS for core infarction volume ≤70 ml. Results There was a negative correlation between ASPECTS and core infarction volume in patients with anterior circulation AIS-LVO over time window and patients with definite onset time among them (rs<0, P<0.05), but no correlation with patients with unclear onset time (P>0.05). ROC curve results showed that ASPECTS had predictive value for core infarction volume ≤70 ml in patients with anterior circulation AIS-LVO over time window and patients with definite onset time among them (P<0.05), and cut-off values were seven points. Conclusion The ASPECTS based on RAPID software of patient with anterior circulation AIS-LVO over time window are correlated with their core infarct volume, which can help to determine the further treatment of patients.

Professor Wang Weidong’s experience in treating insomnia based on “unblocking and removing stagnation”
HE Shiqing1 LI Guixia2 YUAN Guoxing3 CHANG Jingjing2 WANG Weidong2
DOI: 10.20047/j.issn1673-7210.2023.11.29
Insomnia is relatively common in clinical practice. Through long-term clinical practice, Professor Wang Weidong put forward the theory of treating insomnia based on “unblocking and removing stagnation” with good effects. Insomnia or new, or lingering for a long time, should be responsible for “stagnation”, affect nutrient qi, defense qi, qi and blood operation, make the body in a state of stagnation, “unblocking and removing” throughout the treatment process of the disease, pay attention to syndrome differentiation, take care of the spleen and stomach, dispersing liver and regulating qi, nourishing blood for tranquillization, promoting blood circulation for removing blood stasis, and other methods to help sleep, make the body in the“unblocking and removing” state, achieve independent sleep, conscious sleep, natural sleep. An experimental case is attached, in order to provide new ideas and methods for the treatment of insomnia in clinic.

Discussion on Professor Song Yanli’s experience of Qigui Runzao Prescription in treating atopic dermatitis with type of wind and dryness due to blood deficiency based on “treating blood before treating wind”
CUI Han ZHANG Tianbo QU Yun LIU Xin WANG Jingjing YAO Chunhai SONG Yanli
DOI: 10.20047/j.issn1673-7210.2023.11.30
This article introduces Professor Song Yanli’s experience in treating atopic dermatitis from the perspective of “treating blood before treating wind”. Professor Song believed that the traditional Chinese medicine pathogenesis of atopic dermatitis includes blood deficiency, blood dryness, blood stasis, blood heat generating wind, etc. Clinical method of treatment is based on benefiting qi for promoting production of blood, nourishing yin and moistening dryness, promoting blood circulation for removing blood stasis, and clearing heat and cooling blood, and the empirical prescription Qigui Runzao Prescription is used to treat this disease, and the effect is obvious. Therefore, this paper briefly expounds the theoretical connotation of “treating blood before treating wind”, and based on this, discusses the related etiology and pathogenesis of atopic dermatitis, summarizes the deficiency and excess changes of “blood” in this disease, and then leads to Professor Song’s clinical insights on the application of Qigui Runzao Prescription in the treatment of this disease, focusing on the discussion of prescription ideas and drug characteristics, and gives a trial case for clinical reference.

This paper summarizes the experience of Professor Wang Guiling in the treatment of this disease by combination of acupuncture and medicine, and expounds the etiology, pathogenesis and therapeutic principle and method. Professor Wang bases on the theory of “five viscera storing spirit” of Nei Jing, believes the pathogenesis is “five viscera dysfunction, yang doesn’t enter yin, spirit mustn’t store”. About treatment, the pathogenesis should be guarded, focusing on syndrome differentiation of viscera, starting from the five viscera, focusing on regulating the mind and spirit, advocating the combination of acupuncture and medicine, the harmonization of viscera and meridians, and determine treatment on the basis of pattern types. The prescription of traditional Chinese medicine is based on classics. Acupuncture and moxibustion prescription focus on regulating the mind and spirit, flexibly match acupoints according to syndrome differentiation, and cooperate with fire needle or bloodletting to reinforce deficiency and reduce excess to improve the curative effect. And attach the verification case for analysis.

DOI: 10.20047/j.issn1673-7210.2023.11.33
Fu Shan is a famous doctor in the late Ming and early Qing Dynasties. In his works of Fu Qing Zhu Andrology and Fu Qing Zhu Obstetrics and Gynecology, he uses the method of restoring coordination between heart and kidney to treat a variety of diseases, such as insomnia, male spermatorrhea, menstrual diseases, infertility, pregnancy diseases, postpartum diseases of female and so on. Although they are both restoring coordination between heart and kidney, it involves many aspects such as yin, yang, qi, blood, essence and vitality, and the severity and urgency of the disease are different. They either nourish the heart blood and kidney yin, or nourish the heart yin and kidney essence, or nourish the monarch fire to help the ministerial fire, or restore coordination between heart and kidney to regulate the liver, spleen, uterus other viscera disease, or restore coordination between heart and kidney by the liver’s relieving or the rise and fall of the spleen and stomach in the middle energizer, or urgently replenish heart and kidney yin and yang to save the loss of essence and blood. The prescriptions are the same and different. It is flexible, adaptable to clinic, simple and practical, and worthy of in-depth research and mining.
