7 Exploring Professor Huang Li’s experience in treating different types of essential hypertension based on data mining

Objective To analyze the medical case prescriptions of Professor Huang Li for the treatment of essential hypertension, and to summarize the relevant medication rules of Professor Huang Li for the treatment of essential hypertension. Methods The 597 prescriptions written by Professor Huang Li at China-Japan Friendship Hospital from January 2014 to December 2019 were collected, and frequency analysis, cluster analysis, and association rule analysis were performed to observe the distribution of their patient evidence types and core drug combinations. Results The 597 prescriptions included in the analysis were counted according to the evidence of liver-yang hyperactivity, qi deficiency and blood stasis, wind-phlegm upheaval, yin deficiency and fire stasis, and blood stasis, among which 365 patients (60.93%) with liver-yang hyperactivity syndrome, 79 patients (13.19%) with wind-phlegm upheaval syndrome, 72 patients (12.02%) with yin deficiency and fire stasis syndrome, 47 patients (7.85%) with qi deficiency and blood stasis syndrome, 34 patients (5.68%) with blood stasis syndrome. Cluster analysis and association rule analysis showed that the core Chinese medicine combination of liver-yang hyperactivity syndrome was Puerariae Lobatae Radix-Uncariae Ramulus cum Uncis-Chuanxiong Rhizoma-Gastrodiae Rhizoma. The core Chinese medicine combination of wind-phlegm syndrome was Atractylodis- Macrocephalae Rhizoma-Rhodiola rosea-Semen Platycladi. The core Chinese medicine combination of yin deficiency and fire was Taxilli Herba-Paeoniae Radix Rubra. The core Chinese medicine combination of qi deficiency and blood stasis syndrome was Chuanxiong Rhizoma - Notoginseng Radix. The core Chinese medicine combination of stasis block syndrome was Macrocephalae Rhizoma-Croci Stigma-Astragali Radix-Notoginseng Radix. Cluster analysis showed that the syndrome of liver-yang hyperactivity, qi deficiency and blood stasis, wind-phlegm upheaval, yin deficiency and fire stasis and blood stasis were clustered into 5, 5, 4, 5, and 4 categories respectively. Conclusion Professor Huang Li’s clinical treatment of primary hypertension commonly used to pacify the liver and calming the wind, invigorating blood stasis and calming the mind, paying attention to the important role of the liver in the pathogenesis of hypertension and pathological factors such as blood stasis, and using targeted drug combinations.

13 Study on the apoptosis of human hepatoma cell line HepG2 induced by water extract from guiqi and tainong mango peels and its mechanism

Objective To study the apoptotic effect and mechanism of water extract of guiqi and tainong mango peels on human hepatoma cell line HepG2. Methods Human hepatoma cell line HepG2 without being treated with water extract from mango peels was taken as control group, and human hepatoma cell line HepG2 with mango peel water extract added 4, 8, 12, 16 μg/μl, and 20 μg/μl was taken as experimental group. CCK-8 method was used to detect the inhibitory effect of water extract from guiqi and tainong mango peels on the proliferation of human hepatoma cell line HepG2, and the optimal concentration of drug action was determined; flow cytometry was used to determine the apoptosis rate of water extract from guiqi and tainong mango peels; the contents of Caspase 6 and Caspase 9 were determined by enzyme-linked immunosorbent assay. Results The optimal concentration of water extract from Guiqi mango peel on human hepatoma cell line HepG2 for 24 h was 16 μg/μl; the optimal concentration of water extract from Tainon mango peel on human hepatoma cell line HepG2 for 24 h was 8 μg/μl. The apoptosis rate of water extract from guiqi mango peel group and water extract from tainong mango peel group was higher than that of control group (P<0.05). The content of Caspase 6 of water extract from guiqi mango peel group was lower than that in control group; the contents of Caspase 9 of water extract from guiqi mango peel group and water extract from guiqi mango peel group were higher than those in control group (P<0.05). Conclusion Water extract from guiqi and tainong mango peels can promote apoptosis of human hepatoma cell line HepG2, and the mechanism may be related to the decrease of Caspase 6 and the increase of Caspase 9.

17 Data mining of acupoint spectrum and stimulation parameters in electro-acupuncture treatment of knee osteoarthritis

Objective To study the acupoint spectrum and common stimulation parameters of electro-acupuncture treatment of knee osteoarthritis. Methods Clinical literatures on electro-acupuncture treatment of knee osteoarthritis collected from China National Knowledge Infrastructure, Wanfang Data, and PubMed from inception to December 2021 were searched to establish a database of electro-acupuncture point selection and stimulation parameters for knee osteoarthritis. Descriptive statistics, cluster analysis, and association rule analysis were conducted on the data. Results According to the analysis of 51 prescriptions in 51 literatures included, the acupoints with high frequency of use were successively dúbí, nèixīyǎn, xuèhǎi, liángqiū, and ashi point. The main acupoints meridians were the stomach meridian of foot-yangming, extra ordinary point, and spleen meridian of foot-taiyin. The two most commonly connected acupoint pairs of electro-acupuncture were nèixīyǎn-dúbí and liángqiū-xuèhǎi. Cluster analysis results in eight cluster clusters. The highest degree of support in association rules was the combination of dúbí and nèixīyǎn. The most commonly used stimulation parameters were: rarefaction wave, dilatational wave, 2 Hz, 2/100 Hz, intensity tolerance, 30 min/time, once every other day, and total treatment time of four weeks. Conclusion Electro-acupuncture treatment of knee osteoarthritis is mainly based on local acupoint selection, supplemented by syndrome differentiation along channel, and combined treatment of muscles and bones. Attention should be paid to the selection of electro-acupuncture points and waveforms. The treatment interval should not be too long, and the retention time of needles can be extended appropriately within the safe range, so as to provide reference for clinical use of acupoint and stimulation parameter selection of electro-acupuncture in the treatment of knee osteoarthritis.

23 Effect of Schisandrin B on acute myocardial ischemic injury in rats based on thioredoxin interaction protein /thioredoxin/apoptosis signal-regulating kinase 1 pathway

Objective To study the effect of Schisandrin B (Sch B) on the expression of thioredoxin interaction protein (TXNIP), thioredoxin (TRX), and apoptosis signal-regulating kinase 1 (ASK1) in acute myocardial ischemic injury of rats. Methods Sixty SPF male SD rats (body weight 210-250 g) were selected and divided into control group, model group, and SchB low-dose, medium-dose, and high-dose groups (20, 40, 80 mg/kg) by random number table method, with 12 rats in each group. SchB low-dose, medium-dose, and high-dose groups were intragastrically given the corresponding drug (10 ml/kg), while control group and model group were intragastrically given 1% sodium carboxymethylcellulose, once a day, for consecutive 14 days. Two hours after the last administration, myocardial ischemia model was establishedby ligation of the left anterior descending branch of coronary artery in the model group and SchB low-dose, medium-dose, high-dose groups, while the control group only underwent thoracotomy without ligation. Mean arterial pressure (MAP), left ventricular systolic pressure (LVSP), left ventricular end-diastolic pressure (LVEDP), maximum rate of rise or fall of left ventricular pressure (±dp/dtmax), and left ventricular developmental pressure (LVDP) were measured at 3 h of ischemia. Then the rats were killed and the heart tissue was taken. The contents of lipid peroxides (LPO), glutathione (GSH), and the activities of TRX reductase (TrxR) of heart tissues in five groups were determined by enzyme-labeled instrument. TTC staining was used to measure myocardial infarction size. Myocardial cell apoptosis was observed by TUNEL staining. TXNIP, TRX, cysteine aspartic acid specific protease-3 (caspase-3), ASK1, and p-ASK1 levels were detected by Western blot. Results The MAP, LVDP, ±dp/dtmax, expression of GSH, TrxR, and TRX in model group were lower than those in control group, while the LVEDP, LPO, myocardial apoptosis index (AI), and expressions of TXNIP, caspase-3, and p-ASK1/ASK1 in model group were higher than those in control group (P<0.05). The expression of GSH, TrxR, and TRX in SchB low-dose, medium-dose, and high-dose groups were higher than those in model group, while LPO and myocardial infarction area were lower than those in model group (P<0.05). MAP, LVDP, ±dp/dtmax in SchB medium-dose and high-dose groups were higher than those in model group, while the LVEDP, myocardial AI, and expressions of TXNIP, caspase-3, and p-ASK1/ ASK1 in SchB medium-dose and high-dose groups were lower than those in model group (P<0.05). The MAP, LVDP, ±dp/dtmax, and expression of TRX in SchB high-dose group were higher than those in SchB low-dose group, and the LVEDP, myocardial infarction area, myocardial AI, and expressions of TXNIP, caspase-3, p-ASK1/ASK1 in SchB high-dose group were lower than those in SchB low-dose group (P<0.05). Conclusion SchB may play a myocardial protective role by reducing oxidative stress injury and inhibiting myocardial apoptosis through TXNIP/TRX/ASK1 pathway.

28 Study on anti-anxiety effect of Hemerocallis citrina Borani and its mechanism

Objective To determine the anti-anxiety effect of Hemerocallis citrina Borani(XCH) and its mechanism. Methods Sixty male ICR mice were divided into 6 groups by random number table method, including hemerocallis low-dose group (1 g/kg), medium-dose group (4 g/kg), high-dose group (8 g/kg), blank group (normal saline), model group and diazepam positive control group (2 mg/kg), with 10 mice in each group. Except the blank group, the other groups were prepared by chronic restraint stress. Mice of each group underwent elevated cross maze, open field and light and dark box experiments. The levels of dopamine (DA), 5-hydroxytryptamine (5-HT), gamma-aminobutyric acid (GABA) and glutamic acid (Glu) in the brain of mice were determined by enzyme-linked immunosorbent assay. Results Compared with blank group, the number of times of entering the central area decreased (P<0.05), the time of staying in the central area shortened (P<0.01), and the percentage of open arm time (OT%) and the percentage of open arm times (OE%) decreased in model group (P<0.05); the times of entering the open box were reduced, and the activity time of the open box was shortened (P<0.05). Compared with model group, the number of times of entering the central region in medium and high dose groups increased (P<0.01); the residence time in the central region of low and medium dose groups was prolonged (P<0.05). OT% of low, medium and high dose groups was increased (P<0.05 or P<0.01); the OE% of medium and high dose groups was increased (P<0.05 or P<0.01); the number of mice entering the open chamber increased in the medium dose group (P<0.05); the open chamber activity time of mice in medium and high dose groups was prolonged (P<0.01). Compared with blank group, the content of GABA in brain tissue of model group was significantly decreased, while the contents of GLU, 5-HT and DA were significantly increased (P<0.05). Compared with the model group, the GABA level in the brain tissues of the low, medium and high dose groups was increased (P<0.05 or P<0.01). The 5-HT and DA level in the brain tissues of the medium dose group was reduced (P<0.05); the GLU and DA level in the brain tissues of the high dose group was decreased (P<0.05). Conclusion XCH shows obvious anti-anxiety effects, which mechanism may be related to increasing the level of GABA and decreasing the level of DA and 5-HT in the brain tissues.

32 Visual analysis of the relationship between rheumatoid arthritis and intestinal flora based on CiteSpace

Objective To visual analysis the literature related to rheumatoid arthritis and intestinal flora based on CiteSpace. Methods The Web of Science core collection database was searched, and the literature related to enteric flora in the field of rheumatoid arthritis included since the establishment of the database to August 2022 was selected, and the CiteSpace 6.1.R2 software was used to visual analysis the annual number of articles, authors, countries, institutions, and keywords of the literature. Results A total of 822 papers were included in this study, and the number of publications, and citation frequency showed an increasing trend. Research scholars were represented by LIU Y, LI X, and SCHER J. Among the research institutions, New York University published the most papers, and the top three countries in terms of publications were the United States, China, and Italy. Research hotspots were mainly focused on intestinal mucosal immunity, inflammatory bowel disease, the molecular mechanism of action, and probiotic agents. Conclusion The number of publications in the field of rheumatoid arthritis and intestinal flora is increasing year by year, with many research hotspots and a wide coverage area, but the research directions are relatively independent among institutions, and there is less cooperation; 16sRNA sequencing, probiotic therapy, and exploration of regulatory T cell mechanisms are the future research trends in this field.

37 Research progress in magnetic resonance imaging of chronic migraine

Chronic migraine is a disabling neurological disease that causes severe living burden on patients. In recent years,to elucidate the pathogenesis of chronic migraine, more and more studies have been conducted on the pathogenesis. Structural magnetic resonance imaging and functional magnetic resonance imaging with its non-invasive advantages accepted by many researchers, in the disease research has made a breakthrough. Related studies found that chronic migraine patients in the brain pain-related brain abnormalities, including white matter microstructure destruction, cortical layer thickness thinning, gray matter, brain volume, and functional network changes, these may be related to the occurrence and chronicity of migraine. This paper observes the relevant studies on structural magnetic resonance imaging  and functional magnetic resonance imaging  of chronic migraine in recent 5 years, and summarizes the key contents to provide scientific basis for the elaboration of the mechanism, clinical diagnosis, and efficacy evaluation of the disease.

42 Advances in the study of exosomes in the development and diagnosis of hepatocellular liver cancer

Globally, hepatocellular carcinoma (HCC) is the fourth most common type of cancer. Most patients present with intrahepatic or distant organ metastases at the time of diagnosis, and their prognosis is poor. The early onset of HCC is insidious, and early diagnosis has long been one of the major research areas in the treatment of liver cancer. As the role of lncRNA and miRNA in HCC has been gradually clarified, the role of exosomes in tumor progression, and diagnosis has gradually received attention from researchers. In recent years, it has been found that exosomes have an important role in the development of HCC and can be applied in the diagnosis and treatment of HCC. This paper reviews the role of exosomes in the progression of HCC, and their development as tumour markers for the diagnosis of HCC.

45 Analysis of the correlation between tumor and blood lipids and research progress of lipid-lowering drugs in tumor

After reviewing a large number of domestic and foreign literatures, triglycerides, total cholesterol, and low density lipoprotein cholesterol are found to be positively correlated with the occurrence and development risks of many tumors, and negatively correlated with the occurrence and development risks of a few tumors. Different studies have revealed inconsistent results of the correlation between the occurrence and development risk of tumor and triglyceride. However, high density lipoprotein cholesterol is negatively correlated with the occurrence and development risk of tumor. There have been basic and applied studies on statins, beta, cholesterol absorption inhibitors, antioxidant-probucol, pre-protein converting enzyme subtilisin inhibitor, nicotinic acid, and other chemical and traditional Chinese medicine lipid-lowering drugs in tumor treatment, and preliminary study on their target and mechanism. This paper found that the occurrence and development of tumors are closely related to the level of blood lipids, and the mechanism of the anti-tumor effect of lipid-lowering drugs needs further study.

50 Research progress of “internet+” whole-course management mode of chronic obstructive pulmonary disease

Chronic obstructive pulmonary disease (COPD) is a chronic disease with a large number of diseases but can be prevented and treated. Therefore, seeking effective management mode is the basis of comprehensive prevention and treatment of COPD. The whole-course management mode breaks through the shackles of traditional management mode because of its continuity, universality, and economy, and becomes the key to improve patients’ treatment compliance and continuity of medical services. The use of the internet is becoming an important help for its further development. This paper introduces the characteristics of “internet+” COPD whole-course management mode, and analyzes its necessity, the research status of this mode is mainly expounded from before diagnosis, during diagnosis, and after diagnosis, in order to provide new ideas and reference experience for COPD management and related research.

55 Effects of circRNAs on maternal reproductive organs and embryo development during pregnancy

circularRNA (circRNAs) play an important regulatory role in pregnancy, this regulation is mainly manifested in the regulation of ovarian and uterus and other reproductive organs, and embryonic development process. At present, there are many studies on the important regulatory role of circRNAs in cancer, embryogenesis and differentiation during pregnancy have similar molecular mechanisms with cancer. However, there are few studies on the regulatory role of circRNAs during pregnancy. This paper reviewed the latest literature on the regulatory role of circRNAs in pregnancy, to investigate how circRNAs regulates oocyte development, embryo implantation, and embryo development. Exploring the application of circRNAs in pregnancy is helpful to search for new molecular markers of disease, and also broaden the thinking of infertility diagnosis and treatment.

59 Research progress in the treatment of postherpetic neuralgia with Chinese and western medicine

Postherpetic neuralgia is one of the most common complications of herpes zoster and often presents as moderate to severe pain, long-term pain can induce other organ lesions, bring great physical and psychological harm to patients. At present, there are various therapeutic methods for postherpetic neuralgia, and drug therapy is still one of the main therapeutic methods. Western medicine mainly includes calcium channel modulators, antidepressants, non-steroidal anti-inflammatory drugs, topical patches, opioids, and other types of drugs; traditional Chinese medicine believes that the occurrence of postherpetic neuralgia is caused by the dysfunction of viscera, and the treatment should start from harmonizing qi and blood and restoring the function of liver and spleen, the treatment aims to relieve pain, improve sleep, and improve quality of life. This paper summarizes the domestic and foreign literature on the treatment of postherpetic neuralgia and introduces the current situation and progress of Chinese and western medicine in the treatment of postherpetic neuralgia.

63 Research progress of traditional Chinese medicine treatment of urticaria based on interleukin-related effect mechanism

In recent years, clinical studies have found that traditional Chinese medicine can relieve pruritus caused by a variety of diseases by regulating the level of serum interleukin (IL), among which urticaria is the main target. In this paper, literatures on the treatment of urticaria by traditional Chinese medicine based on IL-related effect mechanism in recent five years are reviewed, and combine with modern medicine, helper T cell (Th)1/Th2 drift, IL-31, IL-33, Th17/regulatory T cell ratio, IL-23/IL-17 inflammatory axis, Th9, IL-9, and other aspects are reviewed, in order to provide references for clinical and scientific research directions.

67 Reflection on the teaching of formulaology for Chinese medicine program in the new era

In the new era of vigorous development of Chinese medicine in China, higher education of Chinese medicine should show a new style of keeping pace with the times. Formulaology is the core course for undergraduates majoring in traditional Chinese medicine. Compared with traditional Chinese medicine speciality, Chinese medicine program is to train professional talents of traditional Chinese medicine with both ability and morality, rather than doctors of traditional Chinese medicine who send prescriptions for clinical use. Therefore, the course of formulaology for Chinese medicine program should have different teaching characteristics from that of Chinese medicine specialty. At present, the students majoring in Chinese medicine are not interested in the course of Chinese medicine, the teaching content is deviated from the professional training direction, the foundation of students in Chinese medicine is weak and their thinking of Chinese medicine is deficient. It is necessary to improve the teaching quality of formulaology for Chinese medicine program by encouraging students to participate in the classroom, keeping close to the professional training objectives, strengthening the training of Chinese medicine thinking and integrating the ideological and political aspects of the course, stimulating interest of students in learning, establishing self-confidence in Chinese medicine culture, and making students become excellent talents with the characteristics of the new era to inherit and carry forward the cause of Chinese medicine.

71 Exploration on the construction path for a “golden course” of the neurology combined with online and offline teaching mode based on standard of “profoundness-innovation-challenging”

Building a first-class medical undergraduate course based on the goal of “rejuvenating the country through education” is one of the goals of the 2.0 Excellent Doctor Education and Training Program, and it is also the core element of first-class talent training. “Golden courses” refer to courses with high value like gold. By appropriately increasing the difficulty of major courses and the degree of academic challenge, expanding the optionality of courses and the depth of courses, the quality of teaching can be improved. Starting from the neurology course, this paper analyzes the current situation of university course teaching from the teacher and the student level respectively, and expounds the necessity of building “golden course” from the background of the times, training clinical thinking ability of traditional Chinese medicine undergraduates and training quality teachers. According to the standard of “profoundness-innovation-challenging”, from the perspective of“teaching attitude, teaching content, teaching method, teaching design, teaching form, and teaching evaluation”, it discusses the construction method of the online and offline hybrid “golden course” in neurology of Guangxi University of Chinese Medicine, so as to improve medical students’ solutions clinical issues and innovative thinking skills.

Based on the core values of traditional Chinese medicine culture “benevolence, harmony, essence, and sincerity”, this paper expounds the connotation value and core thinking of the core values of traditional Chinese medicine culture integrated into the four levels of medical humanistic knowledge, medical humanistic behavior, medical humanistic spirit, and medical ethics in medical humanistic quality. This paper also explores the cultivation mode of integrating the core values of traditional Chinese medicine culture into the humanistic quality of medical students by building the humanistic environment of traditional Chinese medicine, optimizing the construction of medical humanistic courses, practicing the humanistic spirit of medicine, and forging the cultural quality of teachers of traditional Chinese medicine.

Good teaching design is the fundamental guarantee of classroom teaching effect, which is also the premise and foundation for giving full play to the function of classroom education. It is necessary to fully reflect the dominant position of students in the process of teaching. BOPPPS teaching model can better achieve the teaching design concept of “student-centered”. The realization of information-based teaching in theory courses is the inevitable trend of the development of the times. As a means of modern information technology, rain classroom provides a new method for classroom teaching innovation. Combined with the characteristics of the course, the author takes the chapter of visceral manifestation of lung in “basic theory of traditional chinese medicine” as an example, and deeply studies and discusses how to organically integrate the two methods in specific teaching applications, so as to provide a reference for the teaching practice reform of the basic theory of traditional Chinese medicine.

84 Comparison of the effect of laparoscopic totally extraperitoneal herniorrhaphy and transgastric abdominal pre-peritoneal herniorrhaphy in the treatment of unilateral primary inguinal hernia in adults

Objective To compare the clinical effect of laparoscopic totally extraperitoneal herniorrhaphy (TEP) and transgastric abdominal pre-peritoneal herniorrhaphy (TAPP) in the treatment of unilateral primary inguinal hernia in adults. Methods A total of 82 patients with unilateral primary inguinal hernia admitted to Taicang Hospital of Traditional Chinese Medicine in Jiangsu Province from May 2019 to June 2021 were selected as the research objects. They were divided into TEP group (receiving TEP treatment) and TAPP group (receiving TAPP treatment) by random number table method, with 41 cases in each group. Operation-related indexes, hospitalization costs, blood gas changes (the difference of partial pressure of carbon dioxide in arterial blood [ΔPaCO2] before and after surgery, the difference of the intraoperative maximum of transcutaneous monitoring of partial pressure of end-tidal carbon dioxide [TcCO2] and the preoperative TcCO2 [ΔTcCO2], the time to reach the maximum TcCO2 [Tmax]), postoperative pain, postoperative complications, and recurrence six months after surgery were compared between the two groups. Results Operating time and Tmax in TEP group were shorter than those in TAPP group, hospitalization cost was lower than that in TAPP group, ΔPaCO2 and ΔTcCO2 were higher than those in TAPP group, and the differences were statistically significant (P<0.05). There were no significant differences in intraoperative blood loss, length of hospital stay, duration of pain, 24 h postoperative visual analog scale score, total incidence of postoperative complications, and recurrence rate at six months after surgery between the two groups (P>0.05). Conclusion TEP is superior to TAPP in terms of surgical time and hospitalization cost, and TAPP has advantages in reducing carbon dioxide in vivo during surgery. The two surgical methods are similar in the treatment of postoperative pain in adults with unilateral primary inguinal hernia, and both are safe and effective.

Objective To investigate the association between tumor necrosis factor-α (TNF-α), high mobility group box-1 protein (HMGB1), and high sensitivity C-reactive protein (hs-CRP) with trauma-induced coagulopathy (TIC) in patients. Methods A total of 120 patients with acute trauma admitted to Fuling Hospital Affiliated to Chongqing University from July 2020 to July 2022 were selected and divided into TIC group (39 cases) and non-TIC group (81 cases) according to the incidence of TIC. The difference of clinical data between the two groups was compared. Multivariate logistic regression model was used to analysis the relationship between TNF-α, HMGB1, and hs-CRP and the occurrence of TIC. Receiver operating characteristic (ROC) curve was drawn to evaluate the predictive value of TNF-α, HMGB1, and hs-CRP for the occurrence of TIC. Results The injury severity score (ISS), shock index (SI), and levels of TNF-α, HMGB1, hs-CRP in TIC group were higher than those in non-TIC group (P<0.05). Multivariate analysis showed that SI (OR=1.545, 95%CI: 1.100-2.170), TNF-α (OR=1.021, 95%CI: 1.008-1.034), HMGB1 (OR=1.068, 95%CI: 1.025-1.113), hs-CRP (OR=1.047, 95%CI: 1.012-1.083) were the influencing factors of TIC occurrence (P<0.05). ROC curve analysis showed that the area under the curve of TNF-α, HMGB1, and hs-CRP combined to predict TIC occurrence was 0.900, the sensitivity was 0.923, and the specificity was 0.753. Conclusion TNF-α, HMGB1, and hs-CRP are factors affecting the occurrence of TIC in patients, and can be used as serum biomarkers for early screening of TIC.

92 Effect of supra arcuate-quadrat lumborum block on postoperative recovery quality in patients with laparoscopic cholecystectomy

Objective To observe the effect of supra arcuate-quadratus lumborum block (SA-QLB) on postoperative recovery quality of patients with laparoscopic cholecystectomy. Methods A total of 60 patients undergoing laparoscopic cholecystectomy under elective general anesthesia admitted to Dongfang Hospital Affiliated to Xiamen University from January to April 2022 were selected, they were divided into control group (group C) and SA-QLB group, with 30 cases in each group. SA-QLB group underwent bilateral SA-QLB under ultrasound guidance before anesthesia induction, and group C did not have nerve block. Both groups were treated with combined intravenous-inhalational anesthesia, and patient-controlled analgesia was used after operation. The intraoperative dosage of Propofol and Remifentanil, anesthesia time, and operation time were compared between two groups; visual analogue scale (VAS) scores at rest and cough in upper abdomen and shoulder were compared between two groups at 2, 6, 12, 24 h, and 48 h after operation; the number of effective analgesic pump compressions, the rate of relief analgesia, the incidence of nausea and vomiting, the incidence of urinary retention, the first time to get out of bed, the first time to exhaust gas were compared between two groups within 48 h after operation; and the scores of the 40-item quality of recovery scale (QoR-40) before and after operation were compared between two groups. Results At 2, 6, 12, 24 h, and 48 h after operation, VAS scores of rest and cough in upper abdomen and shoulder in SA-QLB group were lower than those in group C (P<0.05). The dosages of Propofol and Remifentanil in SA-QLB group were lower than those in group C; the number of effective analgesic pump compressions, the rate of relief analgesia, the incidence of nausea and vomiting were lower than those of group C, and the first time to get out of bed, the first time to exhaust gas were shorter than those of group C (P<0.05). There was no significant difference in the incidence of urinary retention between two groups (P>0.05). The grading scores and total scores of QoR-40 in SA-QLB group were higher than those in group C at 24 and 48 h after operation (P<0.05). Conclusion SA-QLB can improve the postoperative recovery quality of patients undergoing laparoscopic cholecystectomy.

96 Application of sedation and analgesia nursing based on eCASH concept in patients undergoing abdominal surgery in ICU

Objective To explore the clinical effect of sedation and analgesia nursing based on eCASH concept in patients undergoing abdominal surgery in intensive care unit (ICU). Methods From February 2020 to February 2022, 100 patients admitted to ICU of Southern Theater Command General Hospital of Chinese People’s Liberation Army were selected as the study objects. According to the random number table method, they were divided into observation group and control group, with 50 cases in each group. The observation group received sedation and analgesic nursing management under eCASH concept, while control group received sedation and analgesic nursing management under traditional concept. The effects of sedation and analgesia, incidence of adverse events during ICU stay, and Glasgow prognostic score (GOS) at discharge were compared between two groups. Results The sedation standard time of observation group was longer than that of control group, the waking time, ICU stay time, and total hospital stay of observation group were shorter than those of control group, and RASS of observation group was lower than that of control group (P<0.05). The total incidence of adverse events in observation group was lower than that in control group (P<0.05). There was no significant difference in GOS at discharge between two groups (P>0.05). Conclusion The sedative and analgesic nursing of eCASH concept has a good effect in ICU abdominal surgery patients, which can improve the analgesic and sedative effects, shorten the length of stay, reduce the incidence of adverse reactions, and improve the hemodynamic indexes.

100 Discussion on the rule of acupoint selection for acupuncture treatment of enuresis in children based on data mining

Objective To explore the acupoint selection rule of acupuncture treatment of enuresis in children based on data mining. Methods Clinical research literatures on acupuncture treatment of enuresis in children were retrieved from China National Knowledge Infrastructure, Wanfang Data, and VIP from the inception to September 2022, and the acupoints included in the prescription of the literatures and their use frequency, frequency, channel tropism, distribution position, and specific acupoints were counted. IBM SPSS Modeler 18.0 and IBM SPSS Statistics 19.0 were used respectively to conduct association rules and cluster analysis on acupoint prescriptions. Results A total of 75 literatures were included, including 75 acupuncture prescriptions and 62 acupoints, and the total frequency of acupoints was 464 times. The high frequency acupoints were successively guānyuán, sānyīnjiāo, zhōngjí, shènshù, and zúsānlǐ. The conception vessel and the bladder meridian of foot-taiyang were the channels used more frequently. The acupoints were mainly distributed in the chest, abdomen, lumbosacral back. The front-mu point was the specific point used most frequently. Guānyuán-zhōngjí-sānyīnjiāo was the common acupoint group for acupuncture treatment of enuresis in children. Cluster analysis showed that five clusters were commonly used acupoint groups. Conclusion Acupuncture treatment of children enuresis points pay attention to children’s physiology and pathology, mainly to supplement, on the basis of disease differentiation of syndrome and points to strengthen the curative effect.

105 Clinical effect of Huayu Tongluo Moxibustion in the treatment of patients with mild to moderate post-stroke depression

Objective To investigate the clinical effect of Huayu Tongluo Moxibustion in the patients of mild to moderate post-stroke depression. Methods A total of 50 patients with mild to moderate post-stroke depression in the First Affiliated Hospital of Anhui University of Chinese Medicine from September 2020 to September 2021 were selected and they were divided into the control group (25 cases, treated with acupuncture) and the observation group (25 cases, treated with Huayu Tongluo Moxibustion and acupuncture) according to the random number table method, each group was treated for 2 weeks. The scores of the Hamilton depression rating scale (HAMD), Montgomery asperger depression rating scale (MADRS), activity of daily living (ADL) scale, the level of homocysteine (Hcy) and the clinical efficacy of the two groups were compared before and after treatment. Results Before treatment, there were no significant differences in HAMD, MADRS, ADL scores, and serum Hcy levels between the two groups (P>0.05). After treatment, HAMD and MADRS scores of the two groups were lower than before treatment, ADL score was higher than before treatment, and HAMD and MADRS scores of the observation group were lower than control group, ADL score was higher than control group (P<0.05). After treatment, the serum Hcy level of the two groups was lower than before treatment, and the observation group was lower than the control group (P<0.05). The clinical efficacy of the observation group was better than that of the control group, and the difference was statistically significant (P<0.05). Conclusion The clinical symptoms in the patients of mild to moderate post-stroke depression treated with Huayu Tongluo Moxibustion is improved significantly. The efficacy is good. It is worthy of promotion and application.

109 Influence of supplementing chemotherapy with Yiqi Fuzheng Xiaoji Formula on the quality of life and side effects in advanced lung cancer patients with syndrome of qi stagnation and blood stasis

Objective To investigate the influence of supplementing chemotherapy with Yiqi Fuzheng Xiaoji Formula on the quality of life and side effects in advanced lung cancer patients with syndrome of qi stagnation and blood stasis. Methods A total of 120 advanced lung cancer patients with syndrome of qi stagnation and blood stasis admitted to Hubei Hospital of Traditional Chinese Medicine from May 2020 to December 2021 were selected and divided into chemotherapy group and combination group by random number table method, with 60 cases in each group. The chemotherapy group received chemotherapy alone (Cisplatin + Gemcitabine), and the combination group received supplementing chemotherapy with Yiqi Fuzheng Xiaoji Formula. Both groups were treated continuously for two months. The short-term efficacy, side effects, and quality of life before and after treatment were observed. Results The short-term clinical efficacy of the combination group was better than that of the chemotherapy group, and the objective remission rate and disease control rate were higher than those of the chemotherapy group, the differences were statistically significant (P<0.05). The degree of leukopenia, thrombocytopenia, liver and kidney function injury, nausea, vomiting, peripheral neurotoxicity in combination group were less than those in chemotherapy group, and the differences were statistically significant (P<0.05). After treatment, scores of physical function, role function, emotional function, cognitive function, and social function in both groups were higher than those before treatment, the scores of physical function, role function, emotional function, and social function in combination group were higher than those in chemotherapy group, and the differences were statistically significant (P<0.05). Conclusion Supplementing chemotherapy with Yiqi Fuzheng Xiaoji Formula can reduce the side effects of drugs, enhance the anti-tumor effect, and improve the quality of life of advanced lung cancer patients with syndrome of qi stagnation and blood stasis, which has high clinical value.

Tang Rongchuan’s Three-Character Classic for Dysentery Syndrome proposes that “disease in the spleen, cure liver and lung”. He believes that although dysentery syndrome mainly damages the spleen and stomach, attention should be paid to regulating the liver and lung in order to make the “spleen meridian autonomous”, Professor Li Qiangou mainly treats ulcerative colitis from the spleen, liver, and lung to achieve the purpose of anti-ulcer and cure dysentery based on many years of clinical experience. The diseases of spleen, liver, and lung can be caused by improper diet, lung and intestinal disorders, anxiety and irritation, which can lead to the adverse conduction of large intestine, thereby causing disease. It is the disease, which has a long course and consumes vital qi. Professor Li takes the pathogenesis of spleen and stomach weakness, liver failing in conveyance and dispersion, and lung failing in dispersing as its clinical symptoms, and the fundamental method of treatment is to nourish the spleen and stomach, dredge the liver and regulate the lung. Combined with the specific situation of the patient and extraintestinal manifestations in order to add or subtract according to the syndrome. The clinical effect is significant.

Functional constipation is a common and frequent intestinal disease. With the aging of the society, changes in the pace of life and dietary habits, it has become an important factor affecting the quality of life. Traditional Chinese medicine has had a systematic understanding of the etiology and pathogenesis of functional constipation for a very long time, and doctors of all dynasties have a clear vein of syndrome differentiation and treatment of constipation, which is constantly developed and improved. Therefore, traditional Chinese medicine has obvious advantages in the treatment of constipation, and the long-term effect is satisfactory. However, since modern times, great changes have taken place in residents’ dietary structure, eating habits, living, and working environment, and the traditional thought of constipation syndrome differentiation has exposed certain deficiencies. Only by breaking the rules and keeping pace with the times can traditional Chinese medicine be better developed. After years of clinical practice and theoretical research, Professor An Ayue proposes that type of blood stasis due to qi deficiency has become the main syndrome differentiation type in constipation, so the clinical method should be used to supplement qi, nourish blood, and promote blood circulation as the treatment, and the various elements of pathogenesis should be identified and balanced. The empirical formula “Yiqi Yangxue Tongbian Prescription” is created, which has achieved good clinical effects.

122 Clinical effect of modified Huangqi Guizhi Wuwu Decoction in the treatment of peripheral neurotoxicity induced by Oxaliplatin

Objective To observe clinical effect of modified Huangqi Guizhi Wuwu Decoction in the treatment of peripheral neurotoxicity induced by oxaliplatin. Methods According to the random number table method, 46  peripheral neurotoxicity induced by Oxaliplatin with colorectal cancer patients admitted to the Department of Oncology, Yancheng Traditional Chinese Medicine Hospital Affiliated to Nanjing University of Chinese Medicine from January 2019 to March 2022 were divided into control and treatment group, with 23 cases in each group. The control group was treated with chemotherapy alone, and the treatment group was treated with Huangqi Guizhi Wuwu Decoction Granules based on the control group, for a total of 6 months. Chemotherapy-induced peripheral neurotoxicity 20 (CIPN) 20 score, Karnofsky performance state (KPS) score, general adverse event terminology standard 4.0 (blood toxicity, digestive tract reaction, liver, and kidney function) were observed in two groups before and after treatment. Results After treatment, CIPN20 score was higher than before treatment, and KPS score was lower than before treatment (P<0.05), and CIPN20 score in the treatment group was lower than the control group, and KPS score was higher than those in control group (P<0.05). The elassificatim of leukocyte, hemoglobin, and platele in the observation group was better than that in the control group, but there was no significant difference in the incidence of nausea, vomiting, diarrhea, elevated alanine aminotransferase, and oxalacetic aminotransferase between the two groups after treatment (P>0.05). Conclusion Modified Huangqi Guizhi Wuwu Decoction can effectively treat the peripheral neurotoxicity induced by oxaliplatin, improve the patient’s quality of life, and reduce myelosuppression, which is safe and reliable.

126 Effect of Cheng Jiazheng’s empirical formula “Cheqian Lingzhu Decoction” on children with diarrhea of damp heat type

Objective To investigate the clinical effect of Cheqian Lingzhu Decoction, an experienced prescription of Professor Cheng Jiazheng, a famous old Chinese medicine in Shanghai, on children with diarrhea of damp heat type. Methods A total of 160 children with diarrhea of damp heat admitted to Shuguang Hospital, Shanghai University of Traditional Chinese Medicine from May 2021 to January 2022, they were divided into test group and control group, with 80 cases in each group. On the premise of basic treatment, the test group was given Cheqian Lingzhu Decoction Granules orally; the control group was given a shaped placebo of Cheqian Lingzhu Decoction on the premise of basic treatment. The total score of main symptoms, total score of concomitant symptoms, quality of life assessment, clinical efficiency, and safety were evaluated. Results After treatment, the main symptom score, concomitant symptom score, and total symptom score were decreased in both groups, and the treatment group was lower than the control group, the differences were statistically significant (P<0.05). After treatment, the quality of life scores of the two groups were lower than those before treatment, the differences were statistically significant (P<0.05), but there was no statistically significant difference between the two groups (P>0.05). The syndrome efficacy and disease efficacy of experimental group were better than those of control group, and the differences were statistically significant (P<0.05). There were no adverse reactions in the two groups. Conclusion Cheqian Lingzhu Decoction can obviously improve the symptoms and signs of children with damp heat diarrhea, improve the clinical effect, and help to quickly relieve the discomfort symptoms of children with diarrhea of damp heat type.

Cough variant asthma is the leading cause of chronic cough in children in China, and its incidence is increasing year by year, causing great disturbance to children’s growth and development as well as social activities. This article summarizes Professor Wang Youpeng’s treatment thoughts and medication experience in the treatment of cough variant asthma in children in cold areas. Professor Wang believes that cough variant asthma in children in cold areas can be treated from dampness pathogen. There are three main ideas: one is to expel wind, expel dampness pathogen, and promote lung qi, the second is to activate the spleen, which cut off the source of phlegm, and the third is to use the method of elimination of pathogens through purgation and dieresis, and spread the qi mechanism to decompose dampness and heat. In clinical treatment, the method of elimination of pathogens through purgation and dieresis is often used to reconcile bias, guide the evil according to the situation to find a way out, remove the dampness, there is no phlegm, and the heat is self isolated. This article in order to broaden new ideas for the clinical traditional Chinese medicine diagnosis and treatment of cough variant asthma.

Objective To explore the application value of 18F-fluorodeoxyglucose positron emission tomography/computed tomography (18F-FDG PET/CT) in the differrentiation of benign and malignant pulmonary ground glass nodules (GGN). Methods Sixty patients (63 GGN [diameter ≥5 mm]) with complete 18F-FDG PET/CT image data and pathologically confirmed by surgery or puncture biopsy were selected from Chongqing Huajianyoufang Hospital from February 2016 to April 2021. The morphological manifestations of the lesions were analyzed, and maximum standard uptake value (SUVmax) of lesions were calculated. Results Among the 63 GGN, 38 were mixed GGN (7 benign and 31 malignant), and 25 were pure GGN (12 benign and 13 malignant). There were 40 adenocarcinoma, 2 squamous cell carcinoma, and 2 adenosquamous cell carcinoma in malignant GGN. There were 12 inflammation, 6 tuberculosis, and 1 atypical hyperplasia of alveolar epithelium in benign GGN. There were significant differences in the size, margin, and density of benign and malignant GGN lesions (P<0.05); there was no significant difference in GGN location between benign and malignant (P>0.05). SUVmax≥1.0 in benign and malignant mGGN was statistically significant (P<0.05); there was no statistical significance in SUVmax≥1.0 between benign and malignant pGGN (P>0.05). Conclusion 18F-FDG PET/CT can provide imaging and metabolic information in the differential diagnosis of benign and malignant pulmonary GGN, which has important clinical value.

138 Diagnostic value of transvaginal three-dimensional ultrasound combined with POP-Q score in the diagnosis of occult stress urinary incontinence before pelvic floor reconstruction for pelvic organ prolapse

Objective To investigate the diagnostic value of transvaginal three-dimensional ultrasound combined with pelvic organ prolapse quantitative staging (POP-Q) score in the diagnosis of occult stress urinary incontinence (OSUI) before pelvic floor reconstruction for pelvic organ prolapse (POP). Methods A retrospective analysis of the clinical data of 72 patients with pelvic floor reconstruction for POP who were admitted to Rugao Traditional Chinese Medicine Hospital in Jiangsu Province from August 2020 to March 2022. According to urodynamics, they were divided into combined with OSUI group (combined group, 38 cases) and uncombined OSUI group (uncombined group, 34 cases). Both groups underwent transvaginal three-dimensional ultrasonography and POP-Q score before operation. The diagnostic value of transvaginal three-dimensional ultrasound-related parameters and POP-Q score in the diagnosis of OSUI before pelvic floor reconstruction for POP were analyzed. Results The rest area of the levator hiatus (R-ALH), Valsalva area of the levator hiatus (V-ALH), urethral rotation angle (UR), and bladder neck mobility (BND) in combined group were higher than those in the uncombined group (P<0.05). There were significant differences in cue points at the forearm of the vagina 3 cm from the hymen (Aa) and the furthest part of the forearm of the vagina from the hymen (Ba) between two groups (P<0.05). The AUC value of R-ALH, V-ALH, UR, BND, combined with cue points at Aa and Ba in the diagnosis of OSUI before pelvic floor reconstruction for POP was higher than that of each individual test (P<0.05). Conclusion The R-ALH, V-ALH, UR, BND, and cue points at Aa and Ba have certain diagnostic value for OSUI before pelvic floor reconstruction for POP.

Objective To investigate the distribution of pathogenic bacteria of urinary tract infection in Xiyuan Hospital, China Academy of Chinese Medical Sciences and analyze the drug resistance of different pathogenic bacteria. Methods Urine culture samples were collected from patients with urinary tract infection in Xiyuan Hospital, China Academy of Chinese Medical Sciences from January 2019 to December 2021. The isolated strains were identified by MALDI-TOF MS, and the BD PheniX-100 automatic bacterial identification drug sensitivity system was used to determine the minimum inhibitory concentration. WHONET 5.6 was used for drug susceptibility data statistics, and SPSS 25.0 was used for data analysis. Results A total of 1 191 strains of urinary tract infection pathogens were obtained. The main Gram-negative bacteria were Escherichia coli and Klebsiella pneumoniae. The main Gram-positive bacteria were Enterococcus faecalis and Streptococcus agalactiae. There was no significant difference in the sensitivity rate of extended-spectrum β-lactamases (ESBLs)-producing and non-ESBLs-producing Escherichia coli to gentamicin (P>0.05). The sensitivity rate of ESBL-producing Escherichia coli to other commonly used drugs was lower than that of non-ESBL-producing Escherichia coli (P<0.05). Conclusion The detection rate of ESBLs and drug-resistant strains in Escherichia coli and Klebsiella pneumoniae is high. Enterococcus faecalis is also resistant to commonly used drugs. We should strengthen the monitoring of bacterial resistance in hospitals and use antibiotics rationally.

145 Correlation of CD4+ T cell count and hepatitis B virus replication, neutrophil- to-lymphocyte ratio in human immunodeficiency virus/hepatitis B virus co-infected patients

Objective To investigate the correlation of CD4+ T cell count and hepatitis B virus (HBV) replication, neutrophil- to-lymphocyte ratio (NLR) in human immunodeficiency virus (HIV)/HBV co-infected patients. Methods The clinical data of 59 patients with HBV simplex infection (HBV group) and 76 patients with HIV/HBV co-infection (HIV/HBV group) admitted to the Affiliated Hospital of Yangzhou University from February 2018 to January 2022 were retrospectively analyzed. According to CD4+T cell count, the HIV/HBV group was divided into group A (CD4+T cell count>200 /μl, 31 cases) and group B (CD4+T cell count≤200 /μl, 45 cases). The CD4+T cell count, HBV DNA level, and NLR were compared between HBV group and HIV/HBV group, and the HBV DNA level and NLR were further compared between group A and group B, and the correlation between the CD4+T cell count and HBV DNA level, NLR was analyzed in HIV/HBV co-infected patients. Results CD4+T cell count and HBV DNA level in HIV/HBV group were lower than those in HBV group, while NLR was higher than that in HBV group, the differences were statistically significant (P<0.05). The HBV DNA level and NLR in group A were lower than those in group B, the differences were statistically significant (P<0.05). CD4+T cell count was negatively correlated with HBV DNA level and NLR in HIV/HBV group (r<0, P<0.05). Conclusion In HIV/HBV co-infected patients, CD4+T cell count is decreased and HBV DNA replication is more active, which has important significance for clinical drug guidance.

149 Distribution and drug resistance of pathogenic bacteria in children with acute abdomen complicated with severe infection

Objective To explore the distribution and drug resistance of pathogenic bacteria in children with acute abdomen complicated with severe infection. Methods A total of 100 children with acute abdomen complicated with severe infection admitted to Shuyang Hospital Affiliated to Xuzhou Medical University from January 2020 and January 2022 were selected as the research objects. The peritoneal effusion within 24 hours after admission was collected and the peritoneal effusion samples were isolated and cultured according to the national clinical examination operation procedures. The full-automatic bacterial identification/drug sensitivity system was used to monitor the species of infectious pathogens, and the K-B disk diffusion method was used to complete the drug sensitivity test analysis with reference to the Chinese medicine sensitivity test standard of the American Society for Clinical Laboratory Standards Institute. Results A total of 113 pathogenic bacteria were isolated from 100 cases of acute abdomen complicated with severe infection, including 25 Gram-positive bacteria (22.12%), 86 Gram-negative bacteria (76.11%), and 2 fungi (1.78%). The resistance of Escherichia coli, Klebsiella pneumoniae, and Pseudomonas aeruginosa to Gentamicin, Ceftazidime, Ampicillin, Piperacillin / Tazobactam were all >50%. Sensitive to Imipenem and Aztreonam; there was no significant difference in the drug resistance rates of Escherichia coli, Klebsiella pneumoniae and Pseudomonas aeruginosa in different years (P>0.05). Conclusion The children with acute abdomen complicated with severe infection are mainly infected by gram-negative bacteria such as Escherichia coli, Klebsiella pneumoniae, and Pseudomonas aeruginosa, but they have not shown obvious changes in drug resistance in the past two years. Escherichia coli, Klebsiella pneumoniae, and Pseudomonas aeruginosa still have high sensitivity to Imipenem, Aztreonam and other antibiotics.

152 Application of metagenome next-generation sequencing in the patients with anergic pulmonary infection after initial treatment

Objective To explore the application of metagenome next-generation sequencing (mNGS) in the patients with anergic pulmonary infection after initial treatment. Methods Fifty patients with anergic pulmonary infection after initial treatment admitted to the Department of Respiratory and Critical Care Medicine, the Central Hospital of Songjiang, Shanghai from January 2021 to February 2022 were retrospectively analyzed. Conventional microbial culture of sputum and bronchoalveolar lavage fluid (BALF) were performed, and mNGS of BALF were detected. The distribution of pathogenic microorganisms was observed, and the differences between the traditional detection method and the mNGS detection method in the microbial detection of anergic pulmonary infection after initial treatment were compared. Results Among the 50 patients with anergic pulmonary infection after initial treatment, two cases were positive for sputum culture, four case were positive for BALF culture, and 44 cases were positive for BALF mNGS. The positive rate of microbial detection of BALF mNGS was higher than those of conventional sputum culture and BALF culture, and the differences were statistically significant (P<0.05). There was no significant difference in the positive rate of microbial detection between conventional sputum culture and BALF culture (P>0.05). Twenty-seven patients improved after adjusting antibiotics according to the results of mNGS detection. The most common microorganisms detected in mNGS were bacterial infections, mainly Streptococcus pneumoniae, Haemophilus influenzae, Klebsiella pneumoniae, and Pseudomonas aeruginosa. Conclusion The application of mNGS in the patients with anergic pulmonary infection after initial treatment has a higher positive rate of microbial detection, which can provide a strong reference for clinical adjustment of treatment plan.

Objective To use the application of the PASS prescription review and prescribing audit system as an entry point, multidisciplinary collaboration for continuous improvement, and in order to improve the level of rational drug use. Methods The problems of rational medication management in Guangwai Hospital, Xicheng District, Beijing were summarized through multidisciplinary team interviews, and the PDCA cycle management method was used to improve antibiotics, non-antibiotic western medicine, and proprietary Chinese medicine for the problems and the top three unreasonable medical prescriptions; the improvement effect was evaluated by the monitoring indexes of the hospital’s emergency and outpatient prescriptions in the prescription review system and prescription review system in 2021. Results Before and after the implementation of PDCA cycle management in 2021, the prescription review pharmacist intervention rate, the physician prescription revision rate after pharmacist intervention, and the prescription qualification rate of the prescribing review system gradually increased in the 1st to 4th quarters; the percentage of false-positive prescriptions in the prescription review system gradually decreased; the percentage of problematic prescriptions and the physician revision rate increased in the 2nd quarter compared with the 1st quarter of the prescribing review system, and then gradually decreased in the 3rd to 4th quarters compared with the 1st quarter. Conclusion The application of PDCA cycle management method can effectively improve the level of rational drug use in this hospital.

160 Text analysis of hospital informatization policy based on the perspective of policy tools

Objective To analysis of hospital informatics policy texts based on a policy tool perspective. Methods Hospital informatization policies were retrieved from May 20, 2021 to August 31, 2021 on the Chinese government website, the Health Care Commission website, the Ministry of Industry and Information Technology website, the State Administration of Traditional Chinese Medicine website, and Baidu, and 128 policies were coded and categorized based on policy tools, and a three-pair two-dimensional analysis framework of policy tool-policy goal (X-Y), policy goal-policy release year (Y-Z), and policy tool-policy release year (X-Z) was constructed for quantitative analysis. Results The use of policy tools was relatively uneven, with supply-based accounting for 29.91%, demand-based accounting for 11.73%, environment-based accounting for 33.45%, and information transfer accounting for 24.91%; the structure of policy objectives was unreasonable, with infrastructure accounting for 26.52%, information content accounting for 51.62%, information security accounting for 7.86%, and information linkage accounting for 14.00%. Conclusion The ratio of each policy tool should be adjusted, the structure of hospital informatization policy system should be optimized, and the synergy between hospital informatization policy subjects and objects should be strengthened.

165 Value of prehospital sequential assessment pathway form in the treatment of patients with craniocerebral injury

Objective To investigate the value of the prehospital sequential assessment pathway form in the treatment of patients with craniocerebral injury. Methods Eighty-two patients with craniocerebral injuries treated in emergency resuscitation at Lianyungang Second People’s Hospital from May 2019 to April 2021 were selected. The random number table method was used to divide the patients into control group and observation group, with 41 cases in each group. The control group was implemented the traditional pre-hospital emergency procedure, and the observation group was added the pre-hospital sequential assessment pathway form on the basis of the control group. The differences between the two groups in terms of overall time to rescue, hospital response time, resuscitation effect, and complications were observed and compared. Results The overall rescue time and hospital response time of the observation group were shorter than those of the control group, and the differences were statistically significant (P<0.05). The effect of first aid in the observation group was better than that in the control group, and the difference was statistically significant (P<0.05). The overall incidence of complications in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05). Conclusion The pre-hospital sequential assessment pathway form for pre-hospital emergency care of patients with craniocerebral injury can significantly shorten the hospital response, and overall treatment time, improve the cure rate, and reduce the disability and complications, which is worthy of reference.

170 Effect of evidence-based concept of continuity of care on recurrence of major adverse cardiovascular events after intervention in elderly patients with cardiovascular disease

Objective To investigate the impact of constructing continuity of care based on evidence-based concepts on recurrent major adverse cardiovascular events (MACE) after intervention in elderly patients with cardiovascular disease. Methods One hundred and ninety-eight elderly post-interventional patients with cardiovascular disease attending the Qinhuai Medical District of the Eastern Theater General Hospital from April 2018 to November 2020 were selected and divided into control group (given conventional nursing interventions) and study group (given extended nursing care based on evidence-based concepts on the basis of the control group) using the random number table method, with 99 cases in each group. Both groups were intervened for six months. The score of health survey short form (SF-36), depression self-rating scale (SDS), and anxiety self-rating scale (SAS) were compared between the two groups before and after the intervention to observe the occurrence of MACE within six months of the intervention. Results After six months of intervention, the SF-36 scores of both groups were higher than before the intervention, while the study group was higher than the control group, with statistically significant differences (P<0.05). After six months of intervention, the SDS and SAS scores of both groups were lower than before the intervention, while those of the study group were lower than those of the control group, and the differences were statistically significant (P<0.05). The incidence of MACE in the study group was lower than that in the control group, and the difference was statistically significant (P<0.05). Conclusion Evidence-based continuity of care improves quality of life, improves depression and anxiety, and reduces the risk of recurrent MACE in elderly patients after cardiovascular disease interventions.

174 Effect of rehabilitation training on mobility and rehabilitation outcome of patients after arthroscopic surgery for meniscal injury

Objective To investigate the analysis of the effect of intensive quadriceps early rehabilitation training on the mobility and rehabilitation of patients after arthroscopic surgery for meniscal injury. Methods One hundred and eight patients with arthroscopic surgery for meniscus injury taken from January 2019 to December 2021 from the Department of Joint Orthopedics of Binhu Hospital in Hefei were used as the study subjects, and were divided into control and observation groups according to the random number table method, with 54 cases in each group. The control group was adopted conventional functional rehabilitation training, while the observation group was applied intensive quadriceps early rehabilitation training. Postoperative pain, mobility and knee scores were compared between the two groups. Results After treatment, the motor, cognitive, and total scores of both groups were higher than before treatment, while those of the observation group were higher than those of the control group, and the differences were statistically significant (P<0.05). The differences were statistically significant when comparing the vasual analog score (VAS) of two groups at different times, between after discharge from hospital groups and by interaction (P<0.05). Among them, the VAS was lower than that before surgery at 5 d and 10 d after surgery and 1 month in both groups, while those in the observation group were lower than those in the control group, and the differences were statistically significant (P<0.05). After treatment, the scores of stable type, walking assistance, walking distance, extension impairment, and flexion impairment were lower in both groups than before treatment, while those in the observation group were lower than those in the control group, and the differences were statistically significant (P<0.05). Conclusion Intensive early rehabilitation of the quadriceps muscle in patients after arthroscopic surgery for meniscal injuries can effectively reduce pain, improve mobility, enhance knee function, and facilitate rapid prognosis.

178 Practical study on the clinical effect of postoperative compound heat preservation scheme in patients undergoing abdominal surgery

Objective To explore the practical study on the clinical effect of postoperative compound heat preservation scheme in patients undergoing abdominal surgery. Methods A total of 90 patients who received abdominal surgery in Peking University Third Hospital from January 2021 to January 2022 were selected as subjects, they were divided into routine group and intervention group by random number table method, with 45 cases in each group. After returning to the common ward, the conventional group was given routine insulation intervention, and the intervention group was given conrentiond insulation intervention. The changes of blood pressure and heart rate in the two groups were observed, and the occurrence of postoperative complications such as hypothermia and shivering were observed. Results The overall analysis results showed that there were statistically significant differences in heart rate time, intergroup and interaction between the two groups (P<0.05), and the comparison of the interaction between the two groups of systolic blood pressure groups and the difference was statistically significant (P<0.05). Further pairwise comparison showed that the heart rate and systolic blood pressure of the conventional group had significant changes immediately after surgery, and 15, 30 min after surgery, the differences were statistically significant (P<0.05); there were no significant changes in the intervention group immediately after surgery, and 15, 30 min after surgery, the differences were not statistically significant (P>0.05); comparison between groups: the intervention group had lower heart rate, and 15, 30 min after operation than the conventional group, and the differences were statistically significant (P<0.05). The rewarming time of the intervention group was shorter than that of the conventional group, and the total incidence of postoperative complications was also lower, and the differences were statistically significant (P<0.05). Conclusion Combined thermal insulation intervention in postoperative care of patients with abdominal surgery can better ensure that the patient’s body temperature is maintained in a good state after returning to the ward, reduce stress response, and effectively prevent the occurrence of hypothermia and related complications.

182 Application of multidisciplinary comprehensive nursing interventions based on quantitative assessment strategies in patients with severe preeclampsia

Objective To explore the application of multidisciplinary comprehensive nursing interventions based on quantitative assessment strategies in patients with severe preeclampsia. Methods A total of 101 patients with severe preeclampsia admitted to Nanjing Hospital Affiliated to Nanjing Medical University from June 2019 to June 2022 were included. The patients were divided into control group (51 cases) and observation group (50 cases) by random number table method. The control group received multidisciplinary nursing intervention led by obstetrics, in the observation group, multidisciplinary comprehensive nursing intervention based on quantitative evaluation strategy was implemented. The time from admission to delivery was compared between the two groups. The psychological status (assessed by the self-rating anxiety scale [SAS]) of the two groups before intervention and one day before delivery was observed, and the occurrence of complications, delivery methods, and adverse pregnancy outcomes in the two groups were observed. Results The time from admission to delivery in the observation group was longer than that in the control group, and the difference was statistically significant (P<0.05); the SAS score one day before delivery of the two groups were lower than those before intervention, and the observation group was lower than the control group, and the differences were statistically significant (P<0.05). The complication rate of observation group was lower than control group, and the difference was statistically significant (P<0.05); there was no significant difference in the delivery mode and incidence of adverse pregnancy outcome between the two groups (P>0.05). Conclusion Multidisciplinary nursing interventions based on quantitative assessment strategies can extend the time from hospital admission to delivery, improve the psychological status of patients with severe preeclampsia, and reduce the risk of complications and adverse pregnancy outcomes to some extent.

186 Effect of evidence-based preventive nursing on patients with primary tumor immunotherapy

Objective To explore the effect of evidence-based preventive nursing on primary tumor immunotherapy patients. Methods A total of 83 patients with primary tumor who received immunotherapy in Huaibei People’s Hospital of Anhui Province from January 2019 to June 2022 were selected, the patients were divided into routine group (41 cases) and intervention group (42 cases) by random number table method. The routine group received routine nursing intervention, and the intervention group received evidence-based preventive nursing on the basis of the routine group. The compliance, nutritional status, and the occurrence of adverse reactions between the two groups were observed and compared. Results After intervention, the compliance of intervention group was higher than that of conventional group, and the difference was statistically significant (P<0.05); the scores of hemoglobin, serum albumin, and subjective comprehensive nutrition assessment in the two groups were higher than those of before intervention, and the intervention group was higher than the routine group, and the differences were statistically significant (P<0.05); the incidence of adverse reactions in the intervention group was lower than that in the routine group, and the difference was statistically significant (P<0.05). Conclusion Evidence-based preventive nursing can improve the compliance and nutritional status of patients with primary liver cancer undergoing immunotherapy, and improve the safety of immunotherapy.

190 Effect of preventive nursing on postoperative recovery, surgical site infection and satisfaction in patients with percutaneous nephrolithotomy

Objective To explore the effect of preventive nursing on postoperative recovery, surgical site infection, and satisfaction in patients with percutaneous nephrolithotomy (PCNL). Methods A total of 98 patients with PCNL admitted to the Department of Urology in Fuyang People’s Hospital of Anhui Province from January 2020 to January 2022 were selected, and they were divided into observation group and control group, with 49 cases in each group. Control group received routine nursing, observation group received preventive nursing. The postoperative recovery, surgical site infection rate, and nursing satisfaction rate were compared. Results The surgical site infection rate in observation group was lower than that in control group (P<0.05); operation time, hospital stay, and bed time of observation group were shorter than those of control group (P<0.05); nursing satisfaction of observation group was better than that of control group (P<0.05). Conclusion Preventive nursing intervention can effectively promote the postoperative recovery of patients with PCNL, reduce the surgical site infection, patient satisfaction is high, and has a high value.

194 Multi-dimensional intervention strategy under the concept of pre-rehabilitation in total hip arthroplasty on postoperative functional rehabilitation

Objective To explore the effect of multi-dimensional intervention strategy under the concept of pre-rehabilitation in total hip arthroplasty on postoperative functional rehabilitation. Methods A total of 90 patients who underwent total hip arthroplasty in the First Affiliated Hospital of Anhui University of Science and Technology  from February 2019 to February 2022 were selected, and they were divided into control group and observation group according to random number table method, with 45 cases in each group. The control group was given routine intervention, and the observation group was given multi-dimensional intervention strategy the concept of routine intervention combined with pre-rehabilitation. Both groups were given intervention for three months. Harris (Harris) score, barthel index (BI) score, visual analog scale (VAS) score, self-rating anxiety scale (SAS), and self-rating depression scale (SDS) score, and the incidence of complications were compared before intervention and after three months intervention. Results After three months of intervention, VAS, SAS, and SDS scores of both groups were lower than those before intervention, and observation group were lower than control group (P<0.05). The Harris score and BI score of the two groups were higher than those before intervention, and the observation group was higher than the control group (P<0.05). The complication rate of observation group was lower than that of control group (P<0.05). Conclusion The introduction of multi-dimensional intervention strategy under the concept of pre-rehabilitation for patients undergoing total hip arthroplasty is beneficial to reduce postoperative pain, improve hip function, and daily activity ability, improve patients’ negative emotions and reduce complications.
