Abstract  Objective To investigate the correlation of CD4+ T cell count and hepatitis B virus (HBV) replication, neutrophil- to-lymphocyte ratio (NLR) in human immunodeficiency virus (HIV)/HBV co-infected patients. Methods The clinical data of 59 patients with HBV simplex infection (HBV group) and 76 patients with HIV/HBV co-infection (HIV/HBV group) admitted to the Affiliated Hospital of Yangzhou University from February 2018 to January 2022 were retrospectively analyzed. According to CD4+T cell count, the HIV/HBV group was divided into group A (CD4+T cell count>200 /μl, 31 cases) and group B (CD4+T cell count≤200 /μl, 45 cases). The CD4+T cell count, HBV DNA level, and NLR were compared between HBV group and HIV/HBV group, and the HBV DNA level and NLR were further compared between group A and group B, and the correlation between the CD4+T cell count and HBV DNA level, NLR was analyzed in HIV/HBV co-infected patients. Results CD4+T cell count and HBV DNA level in HIV/HBV group were lower than those in HBV group, while NLR was higher than that in HBV group, the differences were statistically significant (P<0.05). The HBV DNA level and NLR in group A were lower than those in group B, the differences were statistically significant (P<0.05). CD4+T cell count was negatively correlated with HBV DNA level and NLR in HIV/HBV group (r<0, P<0.05). Conclusion In HIV/HBV co-infected patients, CD4+T cell count is decreased and HBV DNA replication is more active, which has important significance for clinical drug guidance.
