Beijing expert consensus on diagnosis and treatment of Hashimoto thyroiditis with integration of traditional and western medicine (2021, Beijing)

Hashimoto thyroiditis is a common clinical autoimmune thyroid disease, and its pathogenesis is still unclear. Modern medicine focuses on follow-up, such as concurrent thyrotoxicosis and hypothyroidism, and carries out antithyroid drugs or hormone supplement treatment. There are many clinical studies on Hashimoto’s thyroiditis in traditional Chinese medicine, but there are many controversial points in the diagnosis and treatment of integration of traditional and western medicine. Based on this, the Beijing expert consensus on the diagnosis and treatment of Hashimoto thyroiditis with integration of traditional and western medicine is formulated to clarify the corresponding views and sort out the diagnosis and treatment scheme of integrated of traditional western medicine.

Effects of Xuanfei Jianpi Formula on lung tissue pathology, airway goblet cells and Mucin5AC in rats with chronic obstructive pulmonary disease

Objective To observe the effects of Xuanfei Jianpi Formula on the lung tissue pathology, airway goblet cells and Mucin5AC (MUC5AC) in rats with chronic obstructive pulmonary disease (COPD). Methods Forty Wistar male rats aged 10 to 12 weeks and weighing (230±20) g were divided into blank group, model group, traditional Chinese medicine group, and western medicine group according to random number table method, with ten rats in each group. Except the normal feeding of the blank group, the other groups were treated with lipopolysaccharide injection and smoked cigarettes to prepare COPD rat models. After the modeling, the traditional Chinese medicine group was given Xuanfei Jianpi Formula (1 ml/ 100 g), the western medicine group was given Mucosolvan (1 ml/100 g), and the blank and the model groups were given 0.9% normal saline for 14 consecutive days. Hematoxylin and eosin (HE) staining was performed to observe lung tissue pathology, and the total number and classification of white blood cells in bronchoalveolar lavage fluid (BALF) were detected. Goblet cells were observed by Alcian blue periodic acid-Schiff (AB-PAS) staining, and MUC5AC was detected by immunohistochemistry. Results The lung structure of blank group was complete, no inflammatory cell infiltration was observed, the lung structure destruction and inflammatory reaction were obvious in the model group, and the lung structure and inflammatory reaction were improved in the traditional Chinese medicine group compared with the model group. The total number of white blood cells, neutrophils and lymphocytes in BALF in model group were higher than those in blank group, and the differences were statistically significant (P<0.05). The total number of white blood cells, neutrophils and lymphocytes in BALF in traditional Chinese medicine group were lower than those in model group, and the differences were statistically significant (P<0.05). There were a few goblet cells occasionally in blank group, and the goblet cell proliferation in model group was higher than that in blank group. The goblet cell proliferation in the traditional Chinese medicine group and the western medicine group was lower than that in model group. The relative coloring area of model group was higher than that of the blank group, and the difference was statistically significant (P<0.05). The relative coloring area of the traditional Chinese medicine group was lower than that of model group, and the difference was statistically significant (P<0.05). The expression of MUC5AC protein in lung tissue of model group was higher than that of the blank group, and the difference was statistically significant (P<0.05). The protein expression of MUC5AC in the traditional Chinese medicine group group was lower than that in the model group, and the difference was statistically significant (P<0.05). Conclusion Xuanfei Jianpi Formula may down-regulate the expression of MUC5AC and reduce airway mucus hypersecretion by inhibiting goblet cell metaplasia. At the same time, it can reduce the pathological damage of lung tissue.

Visual analysis of acupuncture treat migraine based on CiteSpace

Objective Visualized analysis of the literature on acupuncture and moxiacupuncture for migraine was conducted based on CiteSpace application to provide directions for subsequent research. Methods Related Chinese literatures in CNKI, Wanfang, VIP, and China biomedical literature database from December 2002 to December 2021 was selected as the object. NoteExpressV was used for screening and exported in RefWorks-CiteSpace, excel 2016 was used for statistical analysis, CiteSpace 5.8.R1 was used for visual analysis of author, institution, key words, and research hotspots and trends. Results A total of 6 935 literatures were retrieved, and 1 855 were included after 5 080 were excluded. Overall, the number of published articles was increased. Representative authors in this field were Liang Fengsheng, Fulei, Wang Linpeng, etc.; the main research sites were Chengdu University of Chinese Medicine, Beijing University of Chinese Medicine and Tianjin University of Chinese Medicine. Research currently focused on “meta-analysis”, “5-hydroxytryptamine”, “quality of life”, and “migraine without aura”. Conclusion At present, the focus tends to diversify, but in-depth longitudinal researches need to be strengthened. The direction may incline to the pathogenesis of migraine, the quality of life and psychological state of patients.

Effect on proliferation and apoptosis of Oridonin on MG63 osteosarcoma cells and its possible mechanism

Objective To study the effect of oridonin (ORI) on proliferation and apoptosis of human osteosarcoma MG63 cells and the possible mechanism. Methods The experimental groups were treated with different concentrations of ORI (10, 20, 30, 40 and 50 μmol/L), and the control group was treated with the same amount of DMSO. Crystal violet staining was used to investigate the proliferation of MG63 cells. Western blot was used to detect the proteins of proliferation, apoptosis, and β-catenin. Flow cytometry was used to assess apoptosis of MG63 cells. T-cell factor/lymphoid enhancing factor(TCF4/ LEF) luciferase reporter plasmid was used to detect the effect on Wnt signal transduction. SqRT-PCR was used to analyze the expression of β-catenin in MG63 cells. Results There was no significant difference in crystal violet staining and quantitization between 10 μmol/L ORI and control group (P>0.05), but 20 to 50 μmol/L ORI group was lower than control group, and the difference was statistically significant (P<0.05). 10 μmol/L ORI group was higher than 20-50 μmol/L ORI group, 20 μmol/L ORI was higher than 30-50 μmol/L ORI, 30 μmol/L ORI was higher than 40 and 50 μmol/L ORI, 40 μmol/L ORI was higher than 50 μmol/L ORI, and the differences were statistically significant (P<0.05). The expression level of PCNA in experimental group was lower than that in control group, and the difference was statistically significant (P<0.05). Expression levels of caspase-3, Cleaved caspase-3, and Bad were higher in the experimental group than in the control group, with statistical significance (P<0.05). The expression level of Bcl-2 in all experimental groups was higher than that in control group, 20 and 30 μmol/L were lower than 10 μmol/L, 30 μmol/L was lower than 20 μmol/L, the differences were statistically significant (P<0.05). The expression of β-catenin in experimental group was lower than that in control group, 20 and 30 μmol/LORI was lower than 10 μmol/LORI, 30 μmol/LORI was lower than 20 μmol/L LORI, the differences were statistically significant (P<0.05). Compared with the control group, the apoptotic cells increased significantly in the experimental group, and decreased at 30 μmol/L, but the cell necrosis increased significantly. The transcriptional activity of TCF4/LEF reporter plasmids in all experimental groups was lower than that in control group, 20 and 30 μmol/L ORI were lower than 10 μmol/L ORI, 30 μmol/L ORI was lower than 20 μmol/L ORI, the differences were statistically significant (P<0.05). The expression of β-catenin in experimental group was lower than that in control group, 20 and 30 μmol/L ORI was lower than 10 μmol/L ORI, 30 μmol/L ORI was lower than 20 μmol/L ORI, the difference was statistically significant (P<0.05). Conclusion ORI inhibited proliferation and induced apoptosis in MG63 cells, which might be partly related to the inhibition of Wnt signal in this cancer cell line.

Study on the skin penetration process and mechanism of acupoint application of evodia rutaecarpa mixed with vinegar in the treatment of para-chlorophenylalanine insomnia rats

Objective To explore skin penetration process and mechanism of acupoint application of evodia rutaecarpa mixed with vinegar in the treatment of para-chlorophenylalanine (PACA) insomnia rats. Methods One hundred and sixty-two SPF grade 7-week-old healthy SD male rats were divided into normal group, model group, positive control group (Diazepam 1 mg/kg, 10 ml/kg), small, medium, and large dose groups (doses of 2, 4, 8 g/kg; each group was divided into yǒngquán point group and shénquē point group). There were 10 rats in normal group, model group, and positive control group (Diazepam), and 22 rats in other groups. Ten rats were used to observe the content of the neurotransmitter γ-aminobutyric acid (GABA) in brain tissue and the expression of GABAA1 mRNA, and the other 12 rats were used to observe the penetration process of the acupoint application of evodia rutaecarpa mixed with vinegar in body skin. Except normal group, the other groups were modeled by PCPA method. The rats in the administration group were affixed and administered for 4 h a day, and after 7 d of continuous administration, brain tissue samples were obtained, and the content of the neurotransmitter GABA was detected by enzyme linked immunosorbent assay, and the expression level of GABAA1 mRNA was detected by real-time PCR. After 7 days to 1 h (T1), 4 h after drug (T2), after 8 h (T3), and 8 h after application and 1 h after removal of the drug (T4), the skin of yǒngquán point and shénquē point of rats was frozen and sliced, the penetration process of acupoint application of evodia rutaecarpa mixed with vinegar was observed by confocal laser scanning micrography. Results Compared with model group, GABA content in hypothalamus, brainstem, and hippocampus of rats in large, medium and small dose groups of acupoint application of evodia rutaecarpa mixed with vinegar were increased, and the differences were statistically significant (P<0.05); GABAA1 mRNA expression of hypothalamus, brainstem, and hippocampus of rats in middle and high dose groups of acupoint application of evodia rutaecarpa mixed with vinegar were increased, and the differences were statistically significant (P<0.05). The average fluorescence intensity of two acupoint groups with large, medium, and small dose of acupoint application of evodia rutaecarpa mixed with vinegar was compared at different time points, and the differences were statistically significant (P<0.05); there were statistically significant differences in the average fluorescence intensity on T1-T4 in large, medium, and small dose groups of two acupoints of acupoint application of evodia rutaecarpa mixed with vinegar (P<0.05). Conclusion Acupoint application of evodia rutaecarpa mixed with vinegar can improve insomnia quality, and the mechanism may be related to the up-regulation of GABA and GABAA1 mRNA expression in brain tissue. It showed good percutaneous permeability in acupoint application of evodia rutaecarpa mixed with vinegar.

Research hotspots and trend analysis of cold-damp paralysis evidence based on CiteSpace and VOSviewer knowledge spectrum map

Objective To analyze the research hotspots and development trends of cold-damp paralysis evidence by visualization software CiteSpace and VOSviewer. Methods Relevant literatures from January 2001 to March 2022 were retrieved from China National Knowledge Infrastructure, and authors, institutions, and keywords were visualized and analyzed using CiteSpace and VOSviewer software. Results A total of 1 106 articles were included. The number of publications showed a gradual increase; the authors with more publications were represented by Jiang Quan, Wu Yang; the institution with the most publications was Henan University of Chinese Medicine; the keyword analysis suggested that the research content included cold-damp paralysis Chinese medicine evidence type research, clinical disease, and Chinese medicine treatment, etc., and 11 main clustering tags were formed. Conclusion The knowledge spectrum map visualization method analyzes the development status of the field of cold-damp paralysis evidence and reveals its research hotspots and development trends, in order to provide new value references for related research.

Effect of Qianlie Xiaozheng Decoction on epithelial-mesenchymal transformation of PC-3 cells in prostate cancer

Objective To investigate the effects of Qianlie Xiaozheng Decoction on invasion, migration, and epithelial-mesenchymal transformation of PC-3 cells in prostate cancer. Methods PC-3 cells of prostate cancer were selected as the research object, and the blank group, positive group (PC-SPESⅡ62.3 μl/100 ml), and Qianlie Xiaozheng Decoction low (62.5 μl/100 ml), medium (125.0 μl/100 ml), and high (250.0 μl/100 ml) dose groups were set. The invasion and migration of five groups of cells were observed by invasion and scratch test. Western blot and PCR were used to determine the expression of epithelial-mesenchymal transformation related proteins and genes. Results The invasion cell count and migration rate of Qianlie Xiaozheng Decoction medium and high dose groups were lower than those of the blank group and Qianlie Xiaozheng Decoction low dose group, and the invasion cell count of Qianlie Xiaozheng Decoction high dose group was lower than that of Qianlie Xiaozheng Decoction medium dose group, the differences were statistically significant (P<0.05 or P<0.01). The expressions of N-cadherin protein in Qianlie Xiaozheng Decoction low, medium, and high dose groups were lower than those in the blank group, the expressions of SLUG protein in Qianlie Xiaozheng Decoction low and high dose groups were lower than those in the blank group, the expression of SLUG protein in Qianlie Xiaozheng Decoction high dose group was lower than that in the Qianlie Xiaozheng Decoction medium dose group, the expression of E-cadherin protein in Qianlie Xiaozheng Decoction high dose group was higher than those in the blank group, and Qianlie Xiaozheng Decoction low and medium dose groups, the differences were highly statistically significant (P<0.01). The expressions of SLUG, TWIST, and N-cadherin genes in Qianlie Xiaozheng Decoction low and medium dose groups were lower than those in the blank group, the expression of E-cadherin gene in Qianlie Xiaozheng Decoction high dose group was higher than that in the blank group and Qianlie Xiaozheng Decoction low and medium dose groups, the differences were statistically significant (P<0.05 or P<0.01). Conclusion Qianlie Xiaozheng Decoction may inhibit the invasion and migration of tumor cells by regulating the epithelial-mesenchymal transformation of PC-3 cells and delay the progression of metastatic prostate cancer.

Advances in biomechanics research in sports-related osteoarthritis

Exercise is an effective means of improving overall physical fitness, and different exercise intensities and styles can affect the knee joint to varying degrees. Reasonable and moderate exercise has a shaping and protective effect on joint cartilage and is very important for maintaining joint stability and normal physiological function, while unreasonable, high-intensity exercise can cause biomechanical abnormalities, resulting in acute or chronic joint injury. Normal necessary biomechanical loading maintains the stability of the intra-articular environment, while abnormal biomechanical loading leads to apoptosis of the associated chondrocytes and degradation of the extracellular matrix, while long-term, sustained abnormal biomechanical loading leads to enhanced local inflammatory and immune response, damage and destruction of cartilage, and aggravates degenerative joint disease, ultimately inducing osteoarthritis. This article provides a review of the relationship between biomechanics and sports-related osteoarthritis.

Research progress on the relationship between human immunodeficiency virus infection and vaginal microorganisms

Heterosexual transmission of human immunodeficiency virus (HIV) through the genital mucosa is the most common route of infection in women, but the immunobiological factors contributing to HIV susceptibility remain unclear. Vaginal microorganisms play a crucial role in HIV infection, and a better understanding of the relationship between vaginal microorganisms and HIV infection is essential to effectively control the epidemic. Sexually transmitted infections and bacterial vaginosis caused by Neisseria gonorrhoeae, Trichomonas vaginalis, Chlamydia trachomatis, and others can cause confusion in the vaginal microorganisms, which increases HIV susceptibility, and human papillomavirus infection rates among HIV-infected women are significantly higher. This paper reviews the current relationship between HIV infection and vaginal microorganisms in women and reviews recent findings on vaginal microorganisms and HIV susceptibility.

Prostate cancer is one of the common malignant tumors in the genitourinary system, which seriously threatens the health of patients. There are various treatment methods for prostate cancer, but the effect of various treatments is not ideal, and eventually it will develop into castration-resistant prostate cancer. Notch signaling pathway is currently one of the latest hotspots in cancer research. Notch signaling pathway is closely related to the occurrence, progression, and metastasis of various malignant tumors. This article reviews the role of Notch signaling pathway in prostate cancer (angiogenesis, cancer cell proliferation, and metastasis).

Research progress on the manifestations and intervention strategies of post intensive care syndrome-family

The occurrence of post intensive care syndrome-family (PICS-F) will bring short and long adverse effects on the health of patients and family members, and seriously harm the physical and mental health of them. It will not only lead to prolonged hospital stay, increased costs, increased complications, and worsening of the disease, which will affect the prognosis of the patient, but also lead to decreased quality of life of the family members and limited daily function. This paper reviews PubMed, CNKI, and Wanfang Data on PICS-F, summarizes the concept, manifestation, evaluation tools, and intervention measures of PICS-F, aiming to raise the public, especially the clinical staff’s attention to this disease, and provide reference for clinical improvement of the quality of family care.

Research progress on the analgesic mechanism of buccal needle and its application to postoperative pain

With the development of modern clinical comfort diagnosis and treatment, the concept of postoperative analgesia has changed from routine analgesia to advanced analgesia, and then to preventive analgesia. Preventive analgesia is constantly updated. A variety of analgesic methods are applied in combination with various analgesic drugs to achieve rapid postoperative recovery. Buccal acupuncture therapy has specific advantages in the treatment of different pain diseases. Buccal acupuncture stimulation can promote cerebrospinal fluid β-endorphin content increase, control the content of cholecystokinin octapeptide to the normal level, and promote the release of central 5-hydroxytryptamine to achieve analgesia. The combined application of buccal acupuncture perioperative period can reduce the use of anesthetics, improve the analgesic effect after operation, while have obvious preventive and therapeutic effects on postoperative complications. Buccal acupuncture is used as an adjunctive therapy for postoperative pain, which has a definite effect and is worthy of clinical promotion.

Current situation and thinking professional postgraduates training of traditional Chinese medicine under the background of dual-track integration

“Dual-track integration” is a postgraduate training mode implemented under the background of the “healthy China” strategy, which shortens the training cycle and improves the efficiency of talent training. It is a major innovation and breakthrough in the reform of the clinical medical talent training mechanism. However, there are still some deficiencies in the current professional postgraduates training modes of traditional Chinese medicine, which need to be continuously improved and perfected. By comparing the similarities and differences of the training modes of medical specialties at home and abroad, this paper analyzed the problems existing professional postgraduates training modes of traditional Chinese medicine, and puts forward some considerations and recommendations, so as to provide some references for improving and perfecting the training mode of traditional Chinese medicine professional degree postgraduates.

Chinese medicine university as a higher education and teaching institution for the inheritance and development of traditional Chinese medicine, colleges and universities of traditional Chinese medicine shoulder the mission and responsibility of building morality, cultivating people and inheriting and innovating traditional Chinese medicine culture. The construction of campus culture with distinctive characteristics can complement and sublimate the cultivation of talents and the inheritance of innovative culture in Colleges and universities. This paper makes a profound analysis of three main problems in the current assessment and evaluation of traditional Chinese medicine personnel training, one is the lack of stage assessment in the training process, the other is the incomplete evaluation system of some practical teaching links, and the third is the lack of feedback after assessment and evaluation. Aiming at the existing problems in medical specialties in medical colleges and schools (undergraduate) proficiency test as an example, the paper discusses evaluation personnel training mode reform train of thought of Chinese medicine and effective path, renews the idea of education evaluation, evaluats mode innovation, strengthens the education loop, assisted with the development of Chinese medicine thinking orientation of talent training mode reform and innovation of traditional Chinese medicine.

Objective To explore the application effect of narrative education in the clinical teaching of medical students in optometry. Methods Sixty-one optometry students who practiced in the ophthalmology department of Kunhua Hospital Affiliated to Kunming University of Science and Technology from June 2018 to July 2021 were selected as the research objects. According to the method of random number table, they were divided into control group and observation group. A total of 30 subjects in the control group were given routine education, 31 subjects in the observation group were given narrative education, and both groups were trained for six months. The assessment results and learning satisfaction of the two groups were compared. Results The results of theoretical examination, practical operation and case analysis in the observation group were higher than those in the control group (P<0.05). The scores of active initiative, comprehensive ability, and clinical thinking of medical students in observation group were higher than those in control group (P<0.05), the total satisfaction rate of the observation group was higher than that of the control group (P<0.05). Conclusion Narrative education has a good effect on the clinical teaching of medical students with optometry, which can significantly improve their performance in the examination, and has a significant effect on improving the teaching quality and learning satisfaction of medical students.

Discussion of the application status of hydrotherapy in the field of rehabilitation in China based on CiteSpace

Objective To analyze the application status of hydrotherapy in the field of rehabilitation in China with CiteSpace software. Methods Literatures related to the application of hydrotherapy in the field of rehabilitation in CNKI were retrieved from the inception to February 2022, and Citespace 5.7.R2 software was used for measurement and visualization analysis to understand the research status and hot spots in this field. Results A total of 512 literatures were included, and a total of 504 authors and 375 institutions were retrieved. Since 2005, the number of hydrotherapy in the field of rehabilitation has increased significantly, and the author with the most literatres was Cong Fang. The Beijing area had a high degree of attention to hydrotherapy, and the top three institutions in terms of publication volume were Beijing Bo’ai Hospital, Capital Medical University School of Rehabilitation, and Guangdong Industrial Injury Rehabilitation Hospital. The top two keywords were neonates and cerebral palsy, the cluster analysis resulted in ten clusters, and the application of hydrotherapy in the field of neonatal rehabilitation was a research hotspot at present. Conclusion The application of hydrotherapy in the field of rehabilitation in China is mainly clinical research, and the forms and methods of intervention are diverse. It can learn from foreign advanced technology to promote the further development of hydrotherapy rehabilitation in China.

Clinical effect of herpes zoster treated with topical Scorzonera Herpes Ointment

Objective To observe the clinical effect of herpes zoster (HZ) treated with topical Scorzonera Herpes Ointment. Methods Sixty patients with HZ who met the inclusion criteria and attended the No.988 Hospital of Logistic Support Forces of the Chinese People’s Liberation Army from September 2020 to October 2021 were selected and divided into treatment group and control group according to the random number table method, with 30 cases in each group. The treatment group was given topical Scorzonera Herpes Ointment and the control group was given topical Recombinant Human Interferon α2b Gel, while both were given oral acyclovir 0.4 g/time, 5 time/day. The efficacy was evaluated after 10 d of treatment, and were observed and compared the efficacy scores of comprehensive symptoms and signs and visual analogue scores (VAS) of pain in the two groups on days 3, 7, and 10 of HZ were observed and compared. The time of erythema reduction, cessation of blistering, crusting, and debridement were observed during the treatment in the two groups. The incidence of postherpetic neuralgia (PHN) was compared between the two groups at a month of treatment. Results The clinical efficacy of the treatment group was better than that of the control group, and the difference was statistically significant (P<0.05). The differences were statistically significant when comparing the intergroup, time, and interaction effects of total HZ symptom and sign ratings between the two groups (P<0.05). Among them, the total score of HZ symptoms and signs on day 3 was lower than that before treatment, the total score of HZ symptoms and signs on day 7 was lower than that before treatment and on day 3, while the total score of HZ symptoms and signs on day 10 was lower than that at other time points in both groups, and the differences were statistically significant (P<0.05). Except before the treatment, the total score of HZ symptoms and signs in the treatment group at the same time point was lower than that in the control group, and the difference was statistically significant (P<0.05). The erythema reduction time, blister stopping time, crusting time, and debridement time in the treatment group were shorter than those in the control group, and the differences were statistically significant (P<0.05). There were a statistically significant differences between the two groups in terms of VAS scores between groups, time, and interaction (P<0.05). In both groups, VAS scores on day 3 were lower than before treatment, VAS scores on day 7 were lower than before treatment and on day 3, and VAS scores on day 10 were lower than at other time points, with statistically significant differences (P<0.05). Except before the treatment, the treatment group had lower VAS scores than the control group at the same time point, and the difference was statistically significant (P<0.05). The incidence of PHN in the treatment group was lower than that in the control group, and the difference was statistically significant (P<0.05). When comparing the incidence of adverse reactions between the two groups, and the difference was not statistically significant (P>0.05). Conclusion The effect of topical treatment of HZ with opium herpes cream is remarkable, which can more effectively reduce skin lesions, shorten the course of disease and relieve pain. It is especially effective in the prevention of PHN, with no adverse reactions in clinical application, safe and effective, and worthy of clinical promotion.

Clinical application of total endoscopic Ivor-Lewis surgery in the treatment of middle and lower thoracic esophageal cancer

Objective To observe the clinical application effects of total endoscopic Ivor-Lewis surgery in the treatment of middle and lower thoracic esophageal cancer. Methods A total of 165 patients with middle and lower thoracic esophageal cancer were treated in Department of Cardiothoracic Surgery, Jinhua Hospital Affiliated to Zhejiang University School of Medicine from June 2020 to January 2022, and they were divided into group A, group B, and group C by random number table method, with 55 cases in each group. Group A carried out open Ivor-Lewis surgery, group B was treated with total endoscopic Ivor-Lewis surgery, and group C performed total endoscopic Mckeown surgery. The surgical indicators, lymph node dissection, and metastasis were compared among the three groups. The quality of life quotient (QLQ-C30) of patients in the three groups were observed before surgery (T0) and at one month (T1), three months (T2), and six months (T3) after discharge. The postoperative complications and recurrence at six months after surgery were statistically analyzed in the three groups. Results The intraoperative blood loss and postoperative drainage volume in group B and group C were less than those in group A, and the postoperative catheterization time and hospital stay were shorter than those in group A (P<0.05), and the postoperative drainage volume in group B was less and the postoperative catheterization time and hospital stay were shorter than those in group C (P<0.05). There were no statistically significant differences in the number of thoracic and abdominal lymph node dissection and metastasis among the three groups (P>0.05). During T1-T3, the functional field scores of group B were higher than that of group C and group A, the symptom field scores were lower than those of group C and group A, and the health status score was higher than that of group C and group A (P<0.05). In group B, functional domain scores during T2-T3 were higher than T0, and in group C, functional domain scores at T3 was higher than T0 (P<0.05). Symptom domain scores during T1-T3 of the three groups were lower than T0, and total health status score was higher than T0 . There were no statistically significant differences in the incidence of postoperative complications and postoperative recurrence rate among the three groups (P>0.05). Conclusion Total endoscopic Ivor-Lewis surgery is safe and effective in the treatment of middle and lower thoracic esophageal cancer, and it can enhance postoperative quality of life.

Relationship between monocyte/high-density lipoprotein ratio and coronary artery stenosis in patients with coronary heart disease

Objective To analyze the relationship between monocyte/high-density lipoprotein cholesterol ratio (MHR) and coronary artery stenosis in patients with coronary artery heart disease (CHD). Methods In this study, 127 CHD patients admitted to the Third Hospital of Changsha from February 2019 to December 2021 were divided into CHD group (97 cases) and non-CHD group (30 cases) according to the results of coronary angiography. The differences of clinical data, laboratory biochemical indexes, and MHR levels between the two groups were compared, and the correlation between serum MHR level and other indexes in CHD group was analyzed. According to the Gensini score of CHD group, the patients were divided into mild group (31 cases) (< 20 points), moderate group (35 cases) (20-43 points), and severe group (31 cases) (>43 points). The difference of MHR among the three groups was compared, the correlation between the MHR level and Gensini score in the CHD group was analyzed, and the diagnostic efficacy of MHR in predicting coronary artery stenosis in CHD patients was evaluated by receiver operator characteristic curve. Results The male proportion, smoking history proportion, type 2 diabetes mellitus proportion, hypertension history proportion, fasting blood glucose, low-density lipoprotein cholesterol, uric acid, hypersensitive C-reactive protein, white blood cell, myeloperoxidase, and MHR in CHD group were all higher than those in non-CHD group, and the differences were statistically significant (P<0.05). Correlation analysis showed that MHR in CHD group was positively correlated with low-density lipoprotein cholesterol, uric acid, hypersensitive C-reactive protein, myeloperoxidase, and leukocyte (r>0, P<0.05). The MHR of the severe group was higher than that of the mild and moderate groups, and the MHR of the moderate group was higher than that of the mild group, the differences were statistically significant(P<0.05). Correlation analysis showed that MHR in CHD group was positively correlated with Gensini score (r>0, P<0.05). MHR predicted coronary artery stenosis in CHD patients with the area under the receiver operator characteristic curve of 0.929 (95%CI: 0.885-0.974, P<0.05), truncation value of 0.42, sensitivity of 78.0%, specificity of 90.0%. Conclusion MHR can be used as an inflammatory marker for early diagnosis and evaluation the severity of coronary artery disease.

Effects of dural puncture epidural block on the dosage of Ropivacaine, stress response, and pregnancy outcome in painless delivery

Objective To investigate the effects of dural puncture epidural block on the dosage of Ropivacaine, stress response, and pregnancy outcome in painless delivery. Methods A total of 204 pregnant women who underwent painless delivery in Xuzhou Maternal and Child Health Hospital Affiliated to Xuzhou Medical University from May to December 2021 were selected as the research objects. They were divided into dural puncture group (dural puncture epidural block) and control group (routine epidural block), with 102 pregnant women in each group. The levels of blood glucose, serum cortisol, and norepinephrine in two groups were measured before analgesia and 30 min analgesia were compared; and the visual analogue scale (VAS) score in two groups before analgesia, 30 min analgesia, 60 min analgesia and delivery were compared; the onset time of analgesia, the consumption of Ropivacaine, the first patient controlled analgesia (PCA) time in two groups were compared; the occurrence of adverse reactions was recorded of pregnant women in two groups; the Apgar scores 1 and 5 min after birth and the occurrence of adverse reactions were compared between two groups. Results The levels of blood glucose, cortisol, and norepinephrine 30 min analgesia in two groups were higher than those before analgesia, and the differences were statistically significant (P<0.05); the levels of blood glucose, cortisol, and norepinephrine in dural puncture group were lower than those in control group, and the differences were statistically significant (P<0.05). There was statistical significance between two groups at different time points (P<0.05). The VAS scores of dural puncture group were lower than those of control group at 30 min analgesia, 60 min analgesia and delivery, and the differences were statistically significant (P<0.05). The dosage of Ropivacaine in dural puncture group was lower than that in control group, the onset time of analgesia was shorter than that in control group, and the duration of first PCA was longer than that in control group, and the differences were statistically significant (P<0.05). There was no significant difference in the incidence of adverse reactions between two groups (P>0.05). There were no significant differences in Apgar scores 1 and 5 min between two groups (P>0.05). There were no adverse outcomes such as respiratory depression and nerve injury in two groups. Conclusion Dural puncture epidural block has quick effect, reduces stress response of pregnant women, and can reduce the dosage of Ropivacaine, which has no adverse effect on maternal and neonatal outcomes.

Objective To review the literatures on the diagnosis and treatment of femoral head necrosis with traditional Chinese medicine in the past 40 years, and to explore the research status and future development trend in this field. Methods From China National Knowledge Infrastructure and Wanfang Data, literature related to traditional Chinese medicine diagnosis and treatment of femoral head necrosis from inception to March 2021 were retrieved. CiteSpace and VOSviewer software were used for visual analysis of literature funds, authors, journals, institutions and keywords. Results A total of 3 148 literatures were included, and the number of literatures increased with fluctuations. The literatures were supported by 770 grants, and the journal of Traditional Chinese Medicine Bonesetting was the most published. Chen Weiheng was the author of the most published literatures, and Guangzhou University of Chinese Medicine was the most published institution. Clinical observation was the key word of high frequency, mechanism discussion, and treatment were the focus of research. Conclusion The research focuses on the mechanism of drug action and the improvement of clinical efficacy. Network pharmacology, signaling pathway, and clinical observation are the current research trends, and the cooperation between research teams and institutions should be strengthened.

Reaserch on prescription rule of external treatment of Chinese medicine for radiopharyngitis of head and neck malignancy based on data mining

Objective To explore the prescription rule of external treatment of Chinese medicine for radiopharyngitis of head and neck malignancy based on data mining. Methods The external treatment literatures of Chinese medicine for radiopharyngitis of head and neck malignancy from the inception to November 2021 on CNKI, Wanfang, CBM and VIP Data were retrieved. SPSS Modeler 18.0 and SPSS Statistics 25.0 software were used for statistical analysis by frequency of medication, nature, flavour and channel tropism, drug association rules and clustering method. Results Sixty-five literatures, 66 prescriptions, 125 traditional Chinese medicines were finally screened, and the cumulative use frequency was 474 times. The Chinese medicine used more than 30% frequently were Glycyrrhizae Radix Et Rhizoma, Lonicerae Japonicae Flos, Rehmanniae Radix, Ophiopogonis Radix and Menthae Haplocalycis Herba. The four nature of drugs were mainly cold, slightly cold, and flat. The five flavors were mostly bitter, sweet, and pungent. The channel tropism were mainly lung, liver, stomach, and heart channels. Most of the effects were heat clearing and deficiency tonifying. Twenty-five association rules were obtained from the medication rule. Complex network analysis found that Glycyrrhizae Radix Et Rhizoma, Lonicerae Japonicae Flos, Rehmanniae Radix, Ophiopogonis Radix and Scrophulariae Radix had strong links. Six clusters were obtained by cluster analysis. Conclusion The external treatment of radiopharyngitis with Chinese medicine is mostly by stages. Removing pathogenic factors and clearing away heat in the early stage, nourishing yin and promoting fluid production in the middle and late stages. Simultaneous treatment of sanjiao, synchronization of strengthening vital qi to eliminate pathogenic evil. The medication rule can provide practical reference for clinical practice.

Objective To explore the medication rules of Professor Wei Wei in treating diarrhea predominant irritable bowel syndrome (IBS-D) based on data mining. Methods The data of IBS-D patients treated by Professor Wei Wei in Wangjing Hospital of China Academy of Chinese Medical Sciences from January 2016 to January 2021 were collected. Excel 2019, SPSS 20.0 and SPSS Modeler 18.0 were used to analyze the frequency, nature, flavour and channel tropism of prescription drugs, strong related drugs and core components. Results Two hundred and thirty-seven prescriptions were collected, involving 91 kinds of traditional Chinese medicine, frequency of traditional Chinese medicine was 3 014 times in total. The most frequently used drugs were tonifying deficiency drug and digestant drug. The top five drug use frequencies were Pseudostellariae Radix, Myristicae Semen, Psoraleae Fructus, Setariae Fructus Germinatus, Hordei Fructus Germinatus. The drug nature was mainly warm; the drug flavours were mainly pungent, bitter and sweet; spleen and stomach channels were the main channel tropism. Combination of drugs with correlation frequency more than 100 times was 14 groups. Four groups of core medicine combinations were obtained according to the results of cluster analysis. Conclusion Professor Wei Wei mostly uses tonifying dificiency drug and digestant drug in the treatment of IBS-D. The main treatment principle is to strengthen the spleen and warm the kidney, regulating stomach and benefiting qi, help the spleen and stomach transport.

Professor Zhou Yongming’s experience in differentiating and treating cancer-related anemia

Summarize Professor Zhou Yongming’s clinical experience in the differentiation and treatment of cancer-related anemia (CRA). Professor Zhou believes that CRA belongs to the category of “consumptive disease”, “blood deficiency” and “loss of flourishing” in Chinese medicine. The pathology is characterized by asthenia in origin and asthenia in superficiality and intermingled deficiency and excess. It is argued that CRA roots from spleen and kidney deficiency, exhibits the internal accumulation of phlegm, dampness, blood stasis and toxin, and the change of internal invasion of fire-toxicity. The use of enforcing spleen and nourishing kidney, eliminating evil and detoxification can achieve good therapeutic effect. Professor Zhou suggests that CRA should be treated as early as possible, and the treatment should follow the principle of individualization in Chinese medicine, grasp the key point of eliminating pathogen and strengthening vital qi, and be flexible to take corresponding measures according to the different stages of chemotherapy.

Bayesian network meta-analysis of the efficacy of external treatment of traditional Chinese medicine in the treatment of post-stroke constipation

Objective To evaluate the effect of external treatment of traditional Chinese medicine in the treatment of post-stroke constipation. Methods Computer search was conducted on China National Knowledge Infrastructure, Wanfang Data Knowledge Service Platform, VIP Data, Chinese Biomedical Literature Database, The Cochrane Library, PubMed, and Embase from the inception to August, 2021. Randomized controlled literatures on the efficacy of external treatment of traditional Chinese medicine in the treatment of post-stroke constipation patients were included. The Cochrane collaborative network bias risk assessment tool was used for quality assessment, and R software gemtc package and JAGS software were used for Bayesian network meta-analysis. Results Among the 1 605 literatures retrieved, 27 randomized controlled trial studies were included. The results of network meta-analysis showed that compared with conventional treatment, moxibustion + conventional treatment (OR=3.82, 95%CI [1.56, 10.13], P<0.05), electroacupuncture + conventional treatment (OR=2.77, 95%CI [1.45, 5.79], P<0.05), auricular point sticking + conventional treatment (OR=4.72, 95%CI [2.35, 10.14], P<0.05), common acupuncture + conventional treatment (OR=4.35, 95%CI [2.68, 7.21], P<0.05), acupoint massage + conventional treatment (OR=4.48, 95%CI [1.25, 16.10], P<0.05), acupoint application + conventional treatment (OR=4.82, 95%CI [1.31, 23.26], P<0.05) could significantly improve the overall patient response rate; there was no statistical difference between the external treatment of traditional Chinese medicine methods (P>0.05). The results of network meta sorting showed that auricular point sticking + conventional treatment (0.685) > acupoint application + conventional treatment (0.669) > common acupuncture + conventional treatment (0.633) > acupoint massage + conventional treatment (0.621) > moxibustion + conventional treatment (0.538) > electroacupuncture + conventional treatment (0.350) > conventional treatment (0.004). Conclusion Compared with conventional treatment, external treatment of traditional Chinese medicine can effectively improve post-stroke constipation. The top three curative effects are auricular point sticking, acupoint application, and ordinary acupuncture. Due to the limited sample size of the literature, large samples and high-quality literature are still needed to be verified in the future.

Polycystic ovary syndrome is a common and complex endocrine disease in women, which seriously affects the physical and mental health of patients. Phlegm and stasis is an important factor leading to the long course and refractory of polycystic ovary syndrome, phlegm and stasis polycystic ovary syndrome is a common clinical syndrome type at present. Author combines with Professor Wu Zhaoli’s practice and perception of clinical evidence, from the perspective of phlegm and stasis, zang-fu viscera, meridians and qi and blood syndrome differentiation, with the adjustment of the function of zang-fu viscera, and the treatment of qi and blood with phlegm and stasis as the whole treatment idea, which can achieve better clinical effect, and provide feasible clinical thoughts and methods for the clinical treatment of polycystic ovary syndrome.

Professor Sun Liyun’s experience in the treatment of psoriasis with blood stasis syndrome with the method of coordinating yin and yang of Yanjing Zhao’s Academic School

Psoriasis with blood stasis syndrome is often repeated disease and difficult to heal, which has a great impact on the quality of life of patients. On the basis of inheriting the thought of “treatment based on blood” in the treatment of psoriasis by Yanjing Zhao’s Academic School, Professor Sun Liyun believes that psoriasis with blood stasis syndrome is a syndrome of blood heat and dryness, dampness, blood stasis, toxin, and deficiency caused by the imbalance of yin and yang. Compound Qinjiao Pills and Siteng Decoction are the classic prescriptions of Professor Zhao Bingnan of Yanjing Zhao’s Academic School. They have the effects of dispelling wind and eliminating dampness, clearing heat and removing toxicity, promoting blood circulation for removing obstruction in collaterals, nourishing blood and yin, invigorating qi and raising yang, and communicating yin and yang. On this basis, Professor Sun Liyun adds and reduces the treatment of psoriasis with blood stasis syndrome, with Caulis Lonicerae, Caulis Trachelospermi, Caulis Sargentodoxae, Caulis Sinomeniito, etc., to strengthen blood stasis and collagalysis, regulate qi and blood yin and yang; with Liujinu, Radix Cynanchi Paniculati, Radix Clematidis, etc., to strengthen the effect of dehumidification and collagalysis; with Herba Solani, Radix Angelicae Dahuricae, Herba Hedyotis to strengthen the work of detoxification, with Radix Curcumae, raw oysters to soothe the mind and relieve depression; with Herba Leonuri, Herba Lycopi, Radix Salviae Miltiorrhizae, Rhizoma Corydalis, Cortex Moutan Radicis, Herba Agrimoniae, Cortex Fraxini, Cortex Ailanthi to regulate the blood. This paper summarizes the typical medical cases.

Application of volume rotary intensity modulated radiotherapy and static intensity modulated radiotherapy in nasopharyngeal carcinoma and their effects on thyroid function

Objective To compare the application of volume rotary intensity modulated radiotherapy (VMAT) and static intensity modulated radiotherapy (s-IMRT) in nasopharyngeal carcinoma (NPC) and their effects on thyroid function. Methods A total of 62 NPC patients admitted to Xinjiang Uygur Autonomous Region People’s Hospital from June 2015 to June 2019 were prospectively selected and divided into VMAT group and s-IMTR group by random number table method, with 31 cases in each group. The VMAT group received VMAT, and the s-IMRT group received s-IMRT. The dose parameters of plan gross tumor volume (PGTV), plan gross tumor volume of the cervical lymph node (PGTVnd), plan tumor volume (PTV1, 2), and organs at risk, and radiotherapy efficiency (machine hop number, beam out time) were compared between the two groups. The changes of thyroid function [free triiodothyronine (FT3) and serum thyroid-stimulating hormone (TSH)] were compared between the two groups before and 3, 6 months after radiotherapy. The effects of radiotherapy on oral mucosa and normal tissue of oral salivary glands in two groups were recorded. Results The conformance indexes of PGTV, PGTVnd, PTV1, and PTV2 in VMAT group were higher than those in s-IMRT group, the mean doses of PTVnd, PTV1, and PTV2 were lower than those of s-IMRT group (P<0.05). The mean doses of bilateral parotid gland, lens, and optic nerve, the V30 of bilateral parotid gland, the maximum doses of bilateral lens, and the V40 of the left temporomandibular joint in VMAT group were all lower than those in s-IMRT group (P<0.05). The machine hop number of VMAT group was lower than that of s-IMRT group, and the beam out time was shorter than that of s-IMRT group (P<0.05). In both groups, serum FT3 levels at 3, 6 months after radiotherapy was lower than those before radiotherapy, and TSH level was higher han those before radiotherapy, and serum FT3 level at six months after radiotherapy was lower than that at three months after radiotherapy, and TSH level was higher than that at three months after radiotherapy (P<0.05). Serum FT3 levels at 3, 6 months after radiotherapy in VMAT group were higher than those in s-IMRT group at the same time, and serum TSH levels were lower than those in s-IMRT group at the same time (P<0.05). The incidence of oral mucosal injury in VMAT group was lower than that in s-IMRT group (P<0.05). Conclusion Comparing with s-IMRT, VMAT in NPC patients can help regulate the dose of target area, control the dose of organs at risk, improve the efficiency of treatment, and reduce the influence on thyroid function and normal tissues of oral mucosa.

Imaging manifestations of focal nodular hyperplasia of the liver

Objective To investigate the CT and MRI imaging manifestations of focal nodal hyperplasia (FNH) of the liver. Methods The clinical and imaging data of 27 patients with FNH confirmed by surgery and pathology in the First People’s Hospital of Zhaoqing and the Affiliated Hospital of Zhaoqing Medical College from February 2015 to October 2020 were analyzed, and their imaging manifestations were analyzed. Results CT and MRI of typical FNH patients showed central fiber scar of the lesion with high signal intensity on T2WI. In contrast-enhanced arterial phase, the lesion was significantly enhanced except fibrous scar, while in delayed phase, contrast agent filling was found in fibrous scar. Atypical FNH showed various imaging manifestations, CT scan showed low density, large blood supply arteries in or around the enhanced artery lesions, rich “radial” vessels of different thickness in portal phase and delayed phase lesions, and the strengthening mode was “central flowering”. MRI showed low signal on T1WI and high signal on T2WI. The enhancement mode of contrast-enhanced scan was basically the same as that of CT. Conclusion The CT and MRI findings of typical FNH have certain characteristics and are not difficult to diagnose clinically; however, atypical FNH can be steatosis and has no “central fibrous scar”, so it is still difficult to accurately diagnose atypical FNH in clinical work. The “central flowering” enhancement in CT and MRI may help to improve the diagnostic rate of atypical FNH.

Feasibility of external multi-leaf collimator for intensity-modulated radiation therapy of cervical cancer

Objective To study the differences of physical parameters between external multi-leaf collimator (MLC) and internal MLC in intensity-modulated radiotherapy (IMRT) for cervical cancer, and to provide necessary reference for the rational application of IMRT technology in clinical practice. Methods A total of 29 postoperative patients with cervical cancer who received radiotherapy in Cangzhou People’s Hospital of Hebei Province from June 2020 to June 2021 were selected. Two IMRT plans were designed on the Pinnacle3 9.11 planning system using external MLC and internal MLC, and were divided into external group and internal group. The target area and organ at risk dose (D2, D50, D95, D98, Dmin, Dmax, and Dmean), volume parameters (V20%, V30%, V40%, V45%, V95%, V100%, V105%, and V110%), treatment plan validation pass rate, machine units (MU), and treatment time of two groups were compared. Results The conformal index of planned target volume (PTV) in external group was higher than that in internal group, and the homogeneity index was lower than that in internal group (P<0.05). The D95, D98, Dmin, V95%, and V100% of target PTV in external group were higher than those in internal group, while Dmax, D2, D50, Dmean, V105%, and V110% were lower than those in internal group (P<0.05). D2, Dmean, V30%, V40%, V45% of rectum, Dmean, V30%, V40% of bladder, D2 of small intestine, Dmean, D98 of left femoral head and D98 of right femoral head in external group were lower than those in internal group (P<0.05); V20% of small intestine was higher than that in external group (P<0.05). The MU and pass rate of external group were lower than those in internal group, the treatment time was longer than that in internal group (P<0.05). Conclusion The dose parameters of the external collimator plan are better than those of the internal collimator plan, but the verification pass rate is low and the treatment time is long. Both plans can meet the clinical needs. It is feasible to conduct postoperative radiotherapy for cervical cancer.

Value of ultrasound-guided fine needle aspiration cytology combined with contrast-enhanced ultrasound in the qualitative diagnosis of thyroid nodules

Objective To investigate the value of ultrasound-guided thyroid fine needle aspiration cytology (US-FNAB) and contrast-enhanced ultrasound (CEUS) in the qualitative diagnosis of thyroid imaging reporting and data system (TI-RADS) class 4 thyroid nodules. Methods The clinical data of 298 patients with TI-RADS class 4 thyroid nodules admitted to Ninth Hospital of Xi’an from January 2019 to December 2021 were retrospectively analyzed, a total of 326 thyroid nodules were confirmed by postoperative pathological examination, included 182 malignant nodules and 144 benign nodules. All patients underwent CEUS examination, US-FNAB examination, and combined US-FNAB and CEUS examination. Postoperative pathological examination as the gold standard, the benign and malignant detection of US-FNAB, CEUS, and their combined examination, the diagnostic value of US-FNAB, CEUS, and their combined examination were compared between the two groups, and the characteristics of CEUS perfusion and US-FNAB needle sensation of benign and malignant nodules were recorded. Results CEUS detected 180 cases of malignant TI-RADS class 4 thyroid nodules and 146 cases of benign TI-RADS class 4 thyroid nodules; US-FNAB detected 181 cases of malignant and 145 cases of benign; 208 cases of malignant and 118 cases of benign by jointly detected. The results of CEUS, US-FNA, and combined examination were consistent with those of postoperative pathological examination. The perfusion of CEUS in malignant showed low enhancement, while that in benign showed high enhancement or equal enhancement. In the malignant, the proportion of needle sensation in the US-FNAB was 81.77%, which was higher than 19.31% in the benign group (P<0.05). Conclusion CEUS, US-FNA, and combined examination can effectively determine the benign and malignant of TI-RADS class 4 thyroid nodules, CEUS-enhanced features and US-FNAB acupuncture can provide the basis, and the combined examination of US-FNAB and CEUS have high sensitivity in the identification of benign and malignant, and have high diagnostic value.

Drug resistance genotyping and drug susceptibility analysis of clinically isolated methicillin-resistant Staphylococcus aureus

Objective To analyze the drug resistance genotyping and drug susceptibility of methicillin-resistant staphylococcus aureus (MRSA) isolated from clinical practice. Methods A total of 254 strains of staphylococcus aureus (SA) isolated from various clinical specimens of inpatients and outpatients in Nanjing Gaochun People’s Hospital from March 2018 to July 2020 were selected. The mecA gene was detected by ordinary polymerase chain reaction (PCR). The MRSA isolates were genotyped using multiplex PCR technology for the staphylococcal chromosome mec box (SCCmec).Antimicrobial susceptibility test of SCCmec type Ⅱ and SCCmec type Ⅲ by kirby-Bauer (K-B) method. Results The results of Cefoxitin K-B showed that a total of 156 MRSA positive strains were detected in 254 SA strains, and the MRSA positive rate was 61.42%. The PCR amplification result of mecA gene showed that a total of 150 MRSA confirmed positive strains were detected out of 254 strains, and the detection rate was 59.06%. The results of multiplex PCR showed that among the 150 MRSA strains, 39 were SCCmec type Ⅱ, 86 were SCCmec type Ⅲ, two were SCCmec Ⅳa, two were SCCmec Ⅳb, and 21 were untyped. The resistance of SCCmec Ⅱ and SCCmec Ⅲ to Oxacillin, Ampicillin/Sulbactam, Cefoxitin, Meropenem, Penicillin G, and Cefazolin was 100.00%. SCCmecⅢ was resistant to Ciprofloxacin. The resistance of Erythromycin, Clindamycin, Chloramphenicol, Gentamicin, Tetracycline, and Rifampicin was higher than that of SCCmecⅡ (P<0.05). Conclusion Using multiplex PCR technology to detect 150 strains of clinically isolated MRSA are mainly SCCmec type Ⅲ, followed by SCCmec type Ⅱ, and the two SCCmec genotypes have a diverse spectrum of drug-resistant bacteria.

Research status analysis and promotion strategy discussion of medical staff in two team-affiliated hospitals

Objective To analyze the scientific research status of medical staff in team-affiliated hospital, so as to provide countermeasures and suggestions for improving scientific research level. Methods A total of 98 medical staff from two team-affiliated hospitals from October to November, 2021 were selected as investigation objects. Self-designed questionnaire survey was used to analyze the self-evaluation score of scientific research, the reasons for the development of scientific research, the existing difficulties and obstacles, and explore the influencing factors restricting the development of scientific research activities; at the same time, statistics of published scientific research papers, patent applications for inventions, projects undertaken, and scientific research achievements from 2017 to 2021 were made. Results There were significant differences in literature retrieval and statistical analysis scores of medical staff with different gender, age, education background, occupation, and title (P<0.05). Age, education background, and occupation were the main factors affecting self-evaluation total scores (P<0.05). Improving ability, promoting professional title, and reflecting professional value were the main reasons for scientific research, while lack of awareness, lack of research motivation and limited research platform were the obstacles to scientific research. From 2017 to 2021, 89 Chinese papers were published, with core journals accounting for 74.16%. Conclusion Measures such as perfecting system mechanism, building research platform, strengthening team building, and scientific planning and training are helpful to improve the scientific research level of medical staff in team-affiliated hospitals.

Analysis of influencing factors of inpatient service utilization in patients with mixed hemorrhoids

Objective To study the inpatient service utilization of mixed hemorrhoids and explore the influencing factors of mixed hemorrhoids inpatient service utilization. Methods The research data was based on the medical insurance database of inpatients in secondary and above public hospitals (31 tertiary hospitals and 26 secondary hospitals) in A city from January 2017 to December 2020. A total of 4 417 patients who were mainly diagnosed as mixed hemorrhoids and underwent surgical treatment were involved. The basic information of inpatients (age, comorbid diseases, and type of surgery, etc.) and the inpatient services utilization (average hospitalization expenses per case and average hospitalization days per time) were recorded, and quantile regression method was used to analyze the influencing factors of utilization of inpatient services. Results The average hospitalization expenses per case of patients with different age, hospital level, hospital category, medical insurance type, number of comorbidities, type of surgery, year, and average hospitalization days per time were compared, and the differences were statistically significant (P<0.05). The average hospitalization days per time of patients with different age, hospital level, hospital category, medical insurance type, number of comorbidities, type of surgery were compared, and the differences were statistically significant (P<0.05). Hospital level, hospital category, medical insurance type, number of comorbidity, type of surgery, year (2018), and average hospitalization expenses per case were the influencing factors of average hospitalization cost (P<0.05). Hospital category, medical insurance type, number of comorbidities, operation type (extirpation and endoligation, procedure for prolapse and hemorrhoids, and other operations), and year were the influencing factors of average hospitalization days per time (P<0.05). Conclusion Patients with mixed hemorrhoids have a high degree of inpatient services utilization. It is recommended to promote the clinical pathway management of inpatient treatment, optimize the diagnosis and treatment process, and further reduce the hospitalization burden of patients.

Effect of evidence-based nursing for digestive endoscopic diagnosis and treatment of gastrointestinal polyps patients

Objective To observe the application effect of evidence-based nursing for digestive endoscopic diagnosis and treatment of gastrointestinal polyps patients. Methods A total of 120 patients treated with digestive endoscopy from January 2017 to December 2020 in the Second Pancreaticobiliary Disease Ward of Jiangsu Provincial People’s Hospital were selected, according to different nursing methods, the patients were divided into routine group and evidence-based group, with 60 cases in each group. The conventional group was treated with routine continuation nursing, and the evidence-based group was treated with evidence-based nursing. The quality of intestinal preparation, gastrointestinal function, and incidence of complications after diagnosis and treatment were compared between the two groups. Results The time of anal exhaust, anal defecation and bowel sound recovery in the evidence-based group were shorter than those in the conventional group, and the differences were statistically significant (P<0.05). Gastrointestinal preparation quality in the evidence-based group was better than that in the conventional group, and the difference was statistically significant (P<0.05). The incidence of complications in the evidence-based group was lower than that in the conventional group, and the difference was statistically significant (P<0.05). Conclusion During the diagnosis and treatment of gastrointestinal polyps by digestive endoscopy, evidence-based nursing can promote the rehabilitation of gastrointestinal function, improve the quality of intestinal preparation, and reduce post-diagnosis complications.

Application of touch combined with acupoint massage in the nursing of neonatal pathologic jaundice

Objective To study the application of touching combined with acupoint massage in the nursing of neonatal pathologic jaundice. Methods A total of 164 children with neonatal pathologic jaundice hospitalized in the First Medical Center of the General Hospital of the Chinese People’s Liberation Army from August 2019 to March 2021 were selected. They were divided into control group and observation group according to random number table method, with 82 cases in each group. The control group was given routine touch nursing, and the observation group was combined with acupoint massage on the basis of the control group. The changes of percutaneous bilirubin levels on the admission the 3rd, 5th and 7th after admission, defecation times in the first five days, yellowing time of fetal stool, jaundice regression time, and day 1-5 sleeping duration were compared between the two groups. Results The overall analysis showed that percutaneous bilirubin levels there were significant differences among groups, time, and interaction (P<0.05); further pairwise comparison, intra-group comparison: the two groups were compared at each time point, and the differences were statistically significant (P<0.05); comparison between groups: the percutaneous bilirubin level of the observation group was lower than that of the control group on the 3rd, 5th and 7th days after nursing intervention, the differences were statistically significant (P<0.05). The number of defecation in the first 5 days in the observation group was more than that in the control group, and the time of fetal stool yellowing and yellowing regression was shorter than that in the control group, the differences were statistically significant (P<0.05). The overall analysis showed that there were statistically significant differences in daily sleep duration, time, and interaction between the two groups (P<0.05). Further pairwise comparison, intra-group comparison: the two groups were compared at each time point, and the differences were statistically significant (P<0.05); comparison between groups: the observation group was higher than the control group at each time points after nursing intervention, the differences were statistically significant (P<0.05). Conclusion Massage combined with acupoint massage in the treatment of neonatal jaundice can effectively reduce the level of bilirubin in neonates, shorten the time of fetal stool yellowing and jaundice regression, prolong the sleeping time, improve the quality of nursing, has a certain clinical application value.

Application effect of self-management support strategy in the nursing of patients with heart failure with reduced ejection fraction

Objective To explore the effect of self-management support strategy in the nursing of heart failure with ejection fraction reduction. Methods A total of 120 patients with heart failure with reduced ejection fraction admitted to Shijiazhuang Hospital of Traditional Chinese Medicine from January 2020 to December 2021 were selected as the research subjects. The patients were divided into control group and observation group by random number table method, with 60 cases in each group. The control group received routine nursing, and the observation group received self-management support strategy based on the control group. Both groups were treated for one month, and the condition control and self-management ability of the two groups were observed. Results The readmission rate of the observation group was lower than that of the control group, and the average length of hospitalization was shorter than that of the control group, the differences were statistically significant (P < 0.05). After intervention, the self-management scores of both groups were higher than before intervention, and the observation group were higher than those of the control group, the differences were statistically significant (P < 0.05). The score of health survey scale in both groups were higher than that before nursing, and those of the observation group was higher than those of the control group, the differences were statistically significant (P < 0.05). Conclusion Self- management support strategy for heart failure patients with reduced ejection fraction can effectively improve the condition of patients, improve their self-management ability, and improve their quality of life.

Application effect of green channel combined with information-based triage system in the early treatment of pediatric emergency patients

Objective To explore the effect of green channel combined with information-based triage system in the early treatment of children in pediatric emergencies. Methods A total of 360 emergency children attending Hebei Provincial Children’s Hospital from March 2020 to June 2021 were selected and categorized into control group (180 cases) and experimental group (180 cases) according to a computerized randomized grouping model. The control group was given the general triage method, while the experimental group was given the green channel+information triage system intervention. The implementation effectiveness of the two groups was compared. Results The satisfaction scores of order, work efficiency, service attitude, triage time, and correct triage rate of family members in the experimental group were higher than those in the control group, and the differences were statistically significant (P<0.05). The waiting time of children with grade 3 and 4 in the experimental group were shorter than that in the control group, and the differences were statistically significant (P<0.05). The success rate of resuscitation in the experimental group was higher than that in the control group, and the difference was statistically significant (P<0.05). Conclusion The application of the green channel combined with the information-based triage system effectively shortens the waiting time of emergency children, improves the success rate of resuscitation and the satisfaction of the children’s families, and facilitates the early improvement of the children's condition, which is worth adopting.

Effect of bowel preadaptation on the quality of early recovery after surgery for portal hypertension

Objective To investigate the impact of bowel preadaptation on the quality of early recovery after surgery for portal hypertension. Methods Ninety patients diagnosed with portal hypertension undergoing surgery from April 2020 to September 2021 in the Department of Hepatobiliary and Pancreatic Surgery, Huai’an First Hospital Affiliated to Nanjing Medical University were selected for the study and they were divided into experimental group and control group by random number table method,with 45 cases in each group. The experimental group received intestinal preadaptation, while the control group was prepared according to the conventional preoperative nutrition, and the two groups were compared in terms of postoperative intestinal function recovery and postoperative recovery quality. Results The rates of postoperative abdominal distension, abdominal pain, nausea, and vomiting in the experimental group were lower than those in the control group, and the differences were statistically significant (P<0.05). The time to the first feeding of fluid and the time to the resumption of anal venting in the experimental group were shorter than in the control group, and the differences were statistically significant (P<0.05). On postoperative 5 d, albumin and serum prealbumin were higher in both groups than on postoperative 1 d, while those in the experimental group were higher than those in the control group, with statistically significant differences (P<0.05). The differences were statistically significant when comparing QoR-15 scores, leukocytes, and C-reactive protein levels between groups, time, and interaction between the two groups (P<0.05). Among them, the level of QoR-15 score was higher in both groups at 7 d postoperatively than at 1 and 3 d postoperatively, while the levels of leukocytes and C-reactive protein were higher than at 1 d postoperatively and lower than at 3 d postoperatively; the indexes were higher in both groups at 3 d postoperatively than at 1 d postoperatively, and the differences were statistically significant (P<0.05). The QoR-15 score of the experimental group was higher than that of the control group at 7 d postoperatively, while the leukocytes and C-reactive protein of the experimental group were lower than those of the control group, with statistically significant differences (P<0.05). Conclusion Bowel preadaptation in patients with portal hypertension can reduce the occurrence of postoperative gastrointestinal dysfunction in patients, improve nutritional status, and enhance the quality of early recovery.

Effect of patient-friend mutual-aid nursing mode on the mental state and compliance behavior of patients undergoing excision of laryngeal tumor

Objective To analyze the effect of patient-friend mutual-aid nursing mode on the mental state and compliance behavior of patients undergoing excision of laryngeal tumor. Methods A total of 54 patients with laryngeal mass resection admitted to the Department of Otolaryngology in Cangzhou People’s Hospital of Hebei Province from March 2016 to August 2019 were collected. According to the random number table method, the patients were divided into control group and observation group, with 27 cases in each group. The control group received routine nursing, the observation group on the basis of the control group to give patients mutual nursing mode. After three months of nursing, the scores of social support scale (SSRS), mental state emotion regulation self-efficacy scale (RESE) and compliance behavior were compared between the two groups. Results After three months of nursing, the social support level of the two groups were higher than that before nursing, and the SSRS score of the observation group was higher than that of the control group (P<0.05). RESE scores in both groups were higher than before nursing, and observation group was higher than control group (P<0.05). The compliance rate of observation group was higher than that of control group (P<0.05). Conclusion The application effect of patient mutual nursing mode in laryngectomy patients is significant, which helps to enhance their level of social support, improve their psychological state, and improve their compliance behavior.

Application of protective sleep nursing combined with hydrotherapy in children with neonatal jaundice

Objective To investigate the effect of using protective sleep care combined with hydrotherapy intervention in children with neonatal jaundice (NG). Methods One hundred cases of NG children from January 2019 to January 2022 in Drum Tower Hospital Affiliated to Nanjing University Medical School, were selected and divided into two groups using the lottery method, with 50 cases in each group. Both groups were given conventional interventions, with the control group adding protective sleep care and the observation group adding protective sleep care combined with hydrotherapy intervention for a week. The time to resolution of jaundice, bilirubin levels (total bilirubin, direct bilirubin), sleep quality, and adverse events were compared between the two groups. Results The jaundice regression time in the observation group was shorter than that in the control group, and the difference was statistically significant (P<0.05). At a week of intervention, serum total bilirubin and direct bilirubin in both groups were lower than before the intervention, while those in the observation group were lower than those in the control group, and the differences were statistically significant (P<0.05). At a week of intervention, the number of nocturnal awakenings in the observation group was lower than that in the control group, while the duration of nocturnal sleep, total sleep time were longer than those in the control group, and time before going to sleep was shorter than that in the control group, with statistically significant differences (P<0.05). The difference in the incidence of adverse events between the two groups was not statistically significant (P>0.05). Conclusion Protective sleep care combined with hydrotherapy intervention shortens the time to resolution of jaundice, reduces serum bilirubin levels, and improves sleep quality in children with NG.

Application of stratified intervention under risk assessment strategy in nursing of patients after hip arthroplasty

Objective To investigate the impact of a stratified intervention under a risk assessment strategy on the outcome of patient care after hip arthroplasty. Methods Two hundred patients with femoral head necrosis carried hip arthroplasty in the Department of Orthopedics, Nanjing First Hospital from April 2020 to January 2022 were selected and divided into observation and control groups using computer-generated random numbers, with 100 cases in each group. The control group adopted conventional nursing interventions while the observation group adopted stratified interventions under conventional nursing combined with risk assessment strategies, and both groups were intervened until the time of discharge. The degree of pain at discharge (assessed using the numerical pain score [NRS]), hip function (assessed using the Harris hip function rating scale) before the intervention and at discharge, activities of daily living (assessed using the modified Barthel index [MBI]), and postoperative complications were compared between the two groups. Results The pain level at discharge was lower in the observation group than that in the control group, and the difference was statistically significant (P<0.05). At discharge, the Harris hip function and MBI scores of both groups were higher than before the intervention, while those in the observation group were higher than those in the control group, with statistically significant differences (P<0.05). The overall incidence of postoperative complications in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05). Conclusion Stratified interventions under the risk assessment strategy can reduce the degree of pain, accelerate the recovery of hip function, and enhance patients' activities of daily living after hip arthroplasty with fewer postoperative complications.
