CAI Liuyan JI Qianqian LI Hengjie
Objective To analyze the relationship between serum high-sensitivity C-reactive protein (hs-CRP), interleukin-15 (IL-15), and Helicobacter pylori (Hp) infection in elderly patietns with acute cholecystitis, and their value in predicting Hp infection. Methods The clinical data of 100 elderly patients with acute cholecystitis admitted to Zhejiang Provincial People’s Hospital from April 2017 to May 2019 were analyzed. According to Hp infection, they were divided into infected group and non-infected group. Serum hs-CRP and IL-15 levels were compared between two groups; the metaplasia of gallbladder mucosa and gastric epithelium was observed; the correlation of hs-CRP, IL-15, and Hp positive rate and the efficacy of hs-CRP and IL-15 in predicting Hp infection in elderly patients with acute cholecystitis were analyzed. Results Fifty-three elderly patients with acute cholecystitis were infected with Hp. Serum hs-CRP and IL-15 levels in infected group were higher than those in non-infected group (P < 0.05). The metaplasia rate of gallbladder mucosa and gastric epithelium in infected group was higher than that in non-infected group (P < 0.05). Hp positive rate was positively correlated with hs-CRP and IL-15 (r = 0.694, 0.677, P < 0.05); there was positive correlation between hs-CRP and IL-15 (r = 0.686, P < 0.05). When hs-CRP and IL-15 were combined to predict Hp infection in acute cholecystitis, the area under the curve was 0.870, the sensitivity and specificity were 75.47% and 95.74%, respectively. Conclusion Hp infection in elderly patients with acute cholecystitis can aggravate the local inflammatory response of cholecystitis, and induce release of hs-CRP and IL-15; hs-CRP and IL-15 are highly correlated with Hp infection, which can be used as a basis for preliminary screening of early Hp infection.

ZHUO Yuanyuan1 WANG Xinru2 HUANG Congcong2 PI Min1 YUAN Weiqu1 LI Kailin2 YU Haibo1
Objective To compare the recurrence risk of cerebral infarction and cerebral hemorrhage within one year and to analyze the clinical risk factors. Methods Eight hundred and sixteen stroke patients (592 cases of cerebral infarction and 224 cases of cerebral hemorrhage) hospitalized in the Department of Acupuncture and Moxibustion, Shenzhen Traditional Chinese Medicine Hospital from September 2017 to June 2019 were included. The demographic characteristics and control of risk factor of stroke were collected and followed up for one year. The recurrence risk of cerebral infarction and cerebral hemorrhage were compared by Kaplan-Meier method. The risk factors of cerebral infarction and cerebral hemorrhage recurrence were analyzed by single factor analysis, and then the Cox proportional risk regression model related to recurrence was established by stepwise regression method. Results After correction of age and gender, comparison of K-M curves between two groups showed statistically significant difference (P < 0.05). For cerebral infarction, the risk of recurrence was 1.872 times higher in patients with family history of stroke than patients without family history of stroke (95%CI: 1.063-3.311, P = 0.031), and 1.464 times higher in patients with poor blood pressure control than in patients with good blood pressure control (95%CI: 1.058-2.018, P = 0.020). For intracerebral hemorrhage, patients with family history of stroke had 4.685 times higher risk of recurrence than those without family history of stroke (95%CI: 1.372-16.003, P = 0.014), and patients with poor blood glucose control had 2.961 times higher risk of recurrence than those with good blood glucose control (95%CI: 1.459-6.010, P = 0.003), the risk of recurrence in patients who still drank alcohol was 2.719 times that of those who did not drink alcohol (95%CI: 1.223-6.046, P = 0.014). Conclusion The recurrence rate of cerebral infarction is higher than that of cerebral hemorrhage in stroke patients with one-year onset. Poor blood pressure control is an independent risk factor for recurrence of cerebral infarction, while poor glycemic control and alcohol consumption behavior are independent risk factors for recurrence of cerebral hemorrhage, and interventions targeting these factors are needed to reduce the risk of stroke recurrence.

125 Treatment of sleep-wake disorder from gallbladder
ZHANG Ming1 LI Guixia2 ZHANG Jinhua2 WANG Weidong2
The most common abnormal manifestations of sleep-wake disorder are insomnia and drowsiness. The recognition and treatment are usually based on zangfu dysfunction, in which few of them is from gallbladder. Inspired by the theory of “all the eleven zang depend on gallbladder”, here is a tentative discussion on the relationship between gallbladder and sleep-wake disorder and a systematic review of ideas about traditional Chinese medicine treatment of sleep-wake disorder from gallbladder. From the perspective of gallbladder being the pivot, gallbladder managing decision and gallbladder liking tranquility, it is summed up that gallbladder dysfunction leads to sleep-wake disorder in three ways: disharmony between yin and yang because of dysfunction of shaoyang pivot; emotional disorder because of deficiency of gallbladder and decision dysfunction; dredging failure because of impurity of gallbladder invaded by evil. This article ulteriorly enriches the theoretical and clinical study of traditional Chinese medicine in sleep-wake disorder.

142 Professor Chen Yiping’s experience in the treatment of IgA nephropathy with the “woxuan sanjiao”
ZHANG Quan CHEN Yiping ZHANG Xianwen SHENG Lingli WANG Lin▲
The treatment of IgA nephropathy is a research problem in the world. Professor Chen Yiping, a famous traditional Chinese medicine in Shanghai and the founder of the nephrology discipline of integrated traditional Chinese and western medicine in China, believes that the occurrence and development of IgA nephropathy are closely related to the sanjiao, and puts forward the treatment method of “woxuan sanjiao, divide and rule”. The secret of its treatment lies in dredging the qi and blood of the sanjiao, and the clinical syndrome attaches importance to the methods of reconciling shaoyang and benefiting qi for activating blood circulation. At the same time, in the different stages of IgA nephropathy, he proposed to apply reasonable methods such as dispersing the lung and relieving throat disorder, invigorating spleen for eliminating dampness, clearing heat and promoting diuresis to focus therapeutic goal on the different parts of the upper, middle and lower sanjiao, and eliminate the diffuse evil poison of the sanjiao. Professor Chen has accumulated rich clinical experience and outstanding curative effect by creating prescription and selecting drugs, combining Chinese and western ingredients, and paste prescription in sequence, and regulating the sanjiao and quenching “the kidney wind”.

146 Professor Sun Zhanxue’s experience in the treatment of lichen planus from stagnant fire
WANG Shiwen1 SUN Zhanxue2 FAN Ziyi1 ZHANG Huili1 CHENG Jing1
Lichen planus is a chronic inflammatory skin disease characterized by polygonal purplish red flat papules of skin damage and easily involving mucous membranes. Professor Sun Zhanxue believes that the occurrence of lichen planus and the characteristics of lingering and difficult to heal are closely related to stagnant fire. In clinic, relieving depression and dispelling fire is the treatment method with definite curative effect. Based on the pathogenesis evolution characteristics of “stagnant fire in sanjiao caused by stagnation of various evils”, “stagnant fire for a long time leads to blood stasis and yin injury”, “overuse of cold medicine will hinder the positive”, combined with classical and modern theories, this paper systematically expounds the treatment of lichen planus from stagnant fire. In terms of treatment, Professor Sun advocates evacuating the stagnant fire of sanjiao from the three levels of “cool the upper, smooth the middle, and purge the lower”, and stresses that the drugs with cold, moisturizing, purging and nourishing should be used to promote blood circulation and nourish yin, and the wind medicine with pungent flavor should be used to invigorate depression and to strengthen the body, and to avoid excessive use of bitter cold, at the same time, pay attention to syndrome differentiation according to the characteristics of the disease.
