Abstract Objective To systematically assess the association of APOE e4 allele with mental and behavioral symptoms of Alzheimer’s disease. Methods PubMed, Web of Science, Cochrane, Embase, and CNKI databases were searched, clinical study of screening mental behavioral symptoms and APOE e4 allele in Alzheimer’s disease. Self-built database until September 2020. Two researchers independently completed literature retrieval, quality evaluation, and data extraction, and Stata 15 software was used for data analysis. Results A total of 4 937 patients were included in 13 literatures. Agitation (Z = 0.31, P = 0.760), depression (Z = 1.71, P = 0.086), hallucinations (Z = 0.75, P = 0.456), delusions (Z = 1.80, P = 0.072), indifference (Z = 1.52, P = 0.128), nocturnal behavior abnormalities (Z = 1.70, P = 0.09) were not correlated with APOE e4 allele. Anxiety (Z = 4.38, P = 0.000) and abnormal behavior movement (Z = 2.16, P = 0.031) were correlated with APOE e4 allele. Conclusion The APOE e4 allele may be a risk gene for anxiety and abnormal behavior movement in psychobehavioral symptoms.
