4 Thinking of innovative development in the new era and practice of medical science and technology under the century leadership of the Chinese Communist Party#br#

Medical science and technology is not only one of the key elements in public health improvement, but also a fundamental guarantee of the governance of the Chinese Communist Party and government. In the past century, under the leadership of the Chinese Communist Party and government, through the predecessors’ exploration and deep cultivation, Chinese medical science and technology has made a lot of progress. This paper reviews the development of medical science and technology innovation in the past 100 years from the aspects of top-level design, medical achievements, medical devices, and systematically combs the relevant plans, products, and technologies. Based on the urgent demand of “comprehensively promoting the construction of healthy China” and the increasing health demand of the people, some thoughts on the innovative development of medical science and technology in China in the new era are put forward.

8 Discussion on Professor Liu Daxin’s medication rules for idiopathic tinnitus based on data mining#br#

Objective To explore professor Liu Daxin’s medication rules for the treatment of idiopathic tinnitus by using data mining technology, and to provide guidance for clinical medication. Methods A total of 538 patients with idiopathic tinnitus who visited Professor Liu Daxin’s clinic in Department of Otolaryngology, Dongfang Hospital, Beijing University of Chinese Medicine Dongfang Hospital from January 2017 to August 2020 were collected, and traditional Chinese medicine prescriptions were extracted to establish a database. SPSS Statistics 25.0 and SPSS Modeler 18.0 statistical software were used to analyze idiopathic tinnitus medication frequency, four nature of drugs and five flavour, channel tropism rules and cluster analysis, and summarized Professor Liu Daxin’s clinical medication rule for idiopathic tinnitus. Results A total of 538 prescriptions were included, with a total of 110 medicines. The frequency of use was 7678 times. The frequency of use was ≥80 and the top 10 drugs were Poria, Radix Bupleuri, Radix Glycyrrhizae, Radix Paeoniae Alba, Rhizoma Atractylodis Macrocephalae, Radix Polygalae, Fructus Liquidambaris, Rhizoma Ligustici Chuanxiong, Radix Cyathulae, and Radix Codonopsis. The proportions of the four nature of drugs in descending order were warm, flat, slightly cold, cold, lukewarm, cool, hot and hot. The proportions of the five flavors of traditional Chinese medicine from high to low were pungent, sweet, bitter, sour, slightly bitter, light, astringent, salty, and slightly sweet. The liver accounted for the highest proportion of traditional Chinese medicine, followed by lung and spleen. The high-frequency drug combination was Radix Glycyrrhizae+Poria→Radix Bupleuri; Rhizoma Atractylodis Macrocephalae + Radix Paeoniae Alba + Poria→Radix Bupleuri; Radix Polygalae + Radix Paeoniae Alba + Radix Bupleuri→Poria. Chinese medicine could be grouped into five categories, mainly for soothing the liver and regulating qi, promoting blood circulation to clear orifice, invigorating the spleen, invigorating dampness, invigorating the kidney and calming the nerves. Conclusion Professor Liu Daxin’s clinical treatment of tinnitus focuses on soothing the liver and relieving depression, promoting qi and blood circulation, strengthening the spleen and dampness, soothing the nerves and replenishing the kidneys.

15 Effects of Chinese medicine barbary wolfberry fruit and danshen root for tonifying deficiency and promoting blood circulation on the expression of TNF-α and IL-1β in the retina of rd10 mice#br#

Objective To observe the effects of Chinese medicine barbary wolfberry fruit and danshen root for tonifying deficiency and promoting blood circulation on the expression of tumor necrosis factor-α (TNF-α) and interleukin-1β (IL-1β) in the retina of rd10 mice. Methods Forty four-week-old rd10 (Pde6b, -/-) mice with the body weight of 18-20 g were divided into model group, control group, barbary wolfberry fruit group, danshen root group, and barbary wolfberry fruit with danshen root group according to random number table method, with eight mice in each group. Physiological saline, vitamin A aqueous solution, Barbary Wolfberry Fruit Chinese medicine Formula Granules aqueous solution, Danshen Root Chinese medicine Formula Granules aqueous solution, Barbary Wolfberry Fruit and Danshen Root Chinese medicine Formula Granules aqueous solution were given 20 ml/(kg·d) by gavage, respectively. Eight C57 wild-type (Pde6b, +/+) mice were given the same amount of normal saline by gavage. The samples were taken after 28 days of gavage. The expression levels of TNF-α and IL-1β in the retina of mice in each group were detected by real-time fluorescence quantitative PCR and Western blot. Results Compared with blank group, the protein and mRNA relative expression levels of TNF-α and IL-1β in model group were increased (P < 0.05). Compared with model group, the protein and mRNA relative expression levels of TNF-α and IL-1β in barbary wolfberry fruit group, danshen root group, and barbary wolfberry fruit with danshen root group were decreased (P < 0.05). Compared with barbary wolfberry fruit group, the relative expression levels of TNF-α and IL-1β protein and IL-1β mRNA in barbary wolfberry fruit with danshen root group were decreased (P < 0.05). Compared with danshen root group, the protein and mRNA relative expression levels of TNF-α and IL-1β in barbary wolfberry fruit with danshen root group were decreased (P < 0.05). Conclusion Chinese medicine for tonifying deficiency and promoting blood circulation, Chinese medicine barbary wolfberry fruit and danshen root can reduce the expression levels of TNF-α and IL-1β in the retina of rd10 mice, and protect the ultrastructure of the retina.

19 Application of homogenization in quality uniformity control of Epimedium Herb Decoction Pieces#br#

Objective To improve the quality uniformity of Epimedium Herb Decoction Pieces. Methods The contents of epimedin C and icariine in Epimedium Herb Decoction Pieces were used as indexes. Homogenization software was used to homogenize seven batches of decoction pieces. The changes of relative standard deviation (RSD) and fingerprint similarity of different batches of index component content before and after homogenization were compared to verify the homogenization effect. Results After homogenization, RSD of epimedin C and icariine content in all batches of decoction pieces decreased from 52.49% and 26.07% to 6.13% and 2.63%, respectively. The similarity of fingerprints was above 0.960. Before homogenization, RSD and fingerprint similarity of each index component content were significantly different, but decreased significantly after homogenization. Conclusion The changes of RSD of index component content and similarity degree of fingerprint indicate that homogenization of different batches of decoction pieces can significantly improve quality uniformity. This study evaluate the homogenization results from the perspective of index component content and fingerprint similarity, and achieve good homogenization effect, which can be applied to the feeding process of industrial production of Traditional Chinese medicine preparations, and can provide reference for the homogenization study of epimedium herb and other decoction pieces.

23 Intervention of cryptotanshinone on depression induced by chronic corticosterone injection in mice#br#

Objective To observe the intervention of cryptotanshinone on depression induced by chronic corticosterone (CORT) injection in mice. Methods Sixty male SPF KM mice were selected. According to random number table method, they were divided into cryptotanshinone low-dose group (10 mg/kg), cryptotanshinone medium-dose group (20 mg/kg), cryptotanshinone high-dose group (40 mg/kg), Fluoxetine group (20 mg/kg), normal control group (equal amount of normal saline), and model group (equal amount of normal saline), with ten mice in each group. Except for normal control group, the depression model was established by continuous injection of CORT (20 mg/kg) in other groups, and the model was established at the same time. After 21 days of continuous administration, the effects of cryptotanshinone on body weight gain, sugar water consumption ratio of mice were observed. Behavioral evaluation was conducted by forced swimming test, open field test and water maze test. Serum levels of CORT, adrenocorticotropic hormone (ACTH), and brain-derived neurotrophic factor (BDNF) in hippocampus and prefrontal cortex were determined. Results In model group, the weight gain, sugar water consumption ratio, and BDNF content were lower than those in normal control group, the immobility time, central lattice residence time, and escape latency were longer than those of the normal control group, the number of horizontal crossing lattice, upright times, and modification times were less than those of normal control group, the stay time of the target quadrant and the movement distance around the platform were shorter than those of the normal control group, the contents of CORT and ACTH were higher than those of normal control group, and the differences were statistically significant (P < 0.05 or P < 0.01). In cryptotanshinone dose groups, the weight gain was higher than that of the model group, the central lattice residence time was shorter than that of model group, the number of horizontal crossing lattice and modification times were more than those of model group, and the levels of CORT and ACTH were lower than that of model group, the differences were statistically significant (P < 0.05 or P < 0.01). In cryptotanshinone medium-dose and high-dose groups, the sugar water consumption ratio was higher than that of model group, the immobility time was shorter than the model group, the center lattice residence time was shorter than the cryptotanshinone low-dose group, and the upright times were more than the model group and the cryptotanshinone low-dose group, the number of horizontal crossing lattice and the modifications times were more than those of cryptotanshinone low-dose group, the escape latency was shorter than that of the model group, the stay time of the target quadrant and the movement distance around the platform were longer than those of the model group, levels of CORT and ACTH were lower than those of cryptotanshinone low-dose group, and the content of BDNF was higher than that of the model group and the cryptotanshinone low-dose group, with statistical significances (P < 0.05 or P < 0.01). The escape latency in the cryptotanshinone high-dose group was shorter than that in the cryptotanshinone low and medium dose groups, the level of CORT was lower than that in the cryptotanshinone medium-dose group, and the content of BDNF in hippocampus was higher than that in the cryptotanshinone medium-dose group, with statistical significances (P < 0.05 or P < 0.01). Conclusion Cryptotanshinone can improve the depression-like behavior of CORT-induced mice, and the effect is more obvious in the medium-dose and high-dose groups. The mechanism may be related to the regulation of hypothalamic-pituitary-adrenal axis function and the up-regulation of BDNF expression level in brain.

28 Effect of adipose-derived mesenchymal stem cell exosomes on myocardial ischemia / reperfusion injury in rats and the effect of Wnt3a/β-catenin signaling pathway#br#

Objective To study the protective effect of adipose-derived mesenchymal stem cell exosomes (ADMSC-exs) on myocardial ischemia / reperfusion (I/R) injury in rats and the influence on the Wnt3a/β-catenin signaling pathway. Methods A total of 40 adult rats were randomly divided into four groups: S group (sham operation group), only thoracotomy without ligation of the coronary artery left anterior descending (LAD); I/R group, reperfusion after ligation of LAD 30 min; in the Ex group, LAD was ligated for 30 minutes and then reperfused, and ADMSC-exs was injected into the tail vein within five minutes of reperfusion; in the Ex+X group, LAD was ligated for 30 minutes and then reperfused, and ADMSC-exs and XAV939 were injected from the tail vein within five minutes of reperfusion. Ten rats in each group. The levels of creatine kinase-MB (CK-MB), lactate dehydrogenase (LDH) and cardiac troponin I (cTnI), and myocardial cell apoptosis index (AI) in each group of myocardial ischemic area were detected by TUNEL method; Western blot was used to detect B lymphoma-2 gene (Bcl-2), Bcl-2 related X protein (Bax), caspase-3, Wnt3a, and β-catenin. Results The levels of LDH, CK-MB and cTnI in the I/R group were higher than those in the S group, and the differences were statistically significant (P < 0.05). The levels of myocardial enzymes in the Ex group was lower than those in the I/R group, and the differences were statistically significant (P < 0.05). When the Wnt3a/β-catenin pathway inhibitor XAV939 was injected into the tail vein, the levels of myocardial enzymes in the Ex+X group were higher than those in the Ex group, and the differences were statistically significant (P < 0.05); compared with the I/R group, there was no statistical difference between two groups (P > 0.05). The AI of S group was lower than that of I/R group, and the difference was statistically significant (P < 0.05). After ADMSC-exs treatment, AI in Ex group was lower than that in I/R group, and the difference was statistically significant (P < 0.05). The Bcl2/Bax ratio of I/R group lower higher than that of S group, and the difference was statistically significant (P < 0.05). The Bcl2/Bax ratio of Ex group was higher than that of I/R group, and the difference was statistically significant (P < 0.05). The level of caspase-3 in Ex group was lower than that in I/R group, and the difference was statistically significant (P < 0.05). The level of caspase-3 in Ex+X group was significantly higher than that in Ex group, and the difference was statistically significant (P < 0.05). The levels of Bcl2/Bax, Wnt3a, and β-catenin in the I/R group were lower than those in the S group, while the caspase-3 level was higher than that in the S group, and the differences were statistically significant (P < 0.05). The levels of Bcl2/Bax, Wnt3a and β-catenin in the Ex group were higher than those in the I/R group, while the level of caspase-3 was lower than that in the S group, and the differences were statistically significant (P < 0.05). The levels of Bcl2/Bax, Wnt3a, and β-catenin in the Ex+X group were lower than those in the S group, while the caspase-3 level was higher than that in the Ex group, and the differences were statistically significant (P < 0.05). Conclusion ADMSC-exs can protect cardiomyocytes after I/R by up-regulating the expression of Bcl-2 and down-regulating the expression of Bax and caspase-3. Its mechanism may be related to the Wnt3a/β-catenin signaling pathway.

32 Immune infiltration mechanism of differential genes in thyroid-associated ophthalmopathy based on GEO database#br#

Objective GEO database was used to conduct differential analysis, enrichment analysis, and immune infiltration analysis of gene data of thyroid-associated ophthalmopathy, providing theoretical basis for in-depth understanding of the immune related mechanism of thyroid-associated ophthalmopathy. Methods “Graves disease” was retrieved from GEO database, and orbital expression genes before thyroid-associated ophthalmopathy were selected as experimental group, and normal anterior orbital expression genes were used as control group. PPI analysis of differential genes was performed using STRING database to screen core genes; the R software was used for GO and KEGG enrichment analysis of differential genes; meanwhile, CIBERSORT deconvolution method was used to analyze the infiltration of 22 kinds of immune cells in thyroid-associpated ophthalmopathy. Results According to GEO database, 315 differential genes were obtained, including 220 down-regulated genes and 95 up-regulated genes, 56 core genes were screened by protein-protein interaction (PPI) analysis, and the top five core genes were FOS, JUN, CXCL8, ITGAM, and CXCL12, respectively; GO enrichment analysis showed 460 biological processes, six cellular components, and 36 molecular functions, it mainly involved cell chemotaxis, leukocyte chemotaxis, granulocyte chemotaxis, leukocyte migration, monocyte migration, and other biological processes; KEGG enrichment analysis showed 20 signaling pathways, mainly involved cytokine-cytokine receptor interaction, PI3K-Akt signaling pathway, interleukin-17 signaling pathway, NF-κB signaling pathway, Toll-like receptor signaling pathway, osteoclast differentiation, and other signaling pathways; the results of immune infiltration showed that monocytes, plasma cells and neutrophils were significantly increased in the experimental group, while M2 macrophages and inactive resting mast cells were significantly decreased. Immunocell correlation analysis showed a significant positive correlation between plasma cells and activated mast cells (r = 0.64), and there was a significant positive correlation between monocytes, inactive natural killer cells and neutrophils (r = 0.61, 0.62), regulatory T cells were significant positively correlated with CD8+T cells (r = 0.60), there was a significant negative correlation between inactive resting mast cells, plasma cells and activated mast cells (r = -0.71, -0.68), there was a significant negative correlation between monocytes, M2 macrophages and inactive resting mast cells (r = -0.71, -0.66), there was a significant negative correlation between inactive and activated natural killer cells (r = -0.71), the difference analysis of immune infiltration between experimental group and control group indicated that monocytes and neutrophils in experimental group were significantly increased (P < 0.05), M2 macrophages and non-activated resting mast cells were significantly decreased (P < 0.05). Conclusion There are various kinds of immune cell infiltration in thyroid-associated ophthalmopathy, immune response and inflammatory reaction throughout the disease.

38 Research progress of inflammatory biomarkers of diabetic nephropathy#br#

Diabetic nephropathy (DN) is a renal microvascular disease caused by long-term diabetes mellitus (DM), which is also the main cause of end-stage kidney disease. The pathogenesis of DN involves a wide range. Recent studies have shown that inflammation plays a key role in the pathogenesis of DN. Increased expression of cell adhesion molecules, growth factors, chemokines, and pro-inflammatory cytokines is found in the renal tissues of DM patients, which is closely related to renal function injury. Eight inflammatory factors closely related to the pathogenesis of DN, including high sensitivity C-reactive protein, ratio of neutrophil count to lymphocyte count, pentraxin 3, tumor necrosis factor-α and its receptor, interleukins, Nod-like receptor protein 3 inflammasome, adhesion molecule and monocyte chemoattractant protein-1 are discussed in this paper, aiming to provide a new idea for monitoring the renal function changes and early treatment of DM patients.

42 Research progress of traditional Chinese medicine in the treatment of rheumatoid arthritis via JAK-STAT signaling pathway#br#

Rheumatoid arthritis is a chronic autoimmune disease mainly characterized by inflammatory synovitis, with high morbidity, it seriously affects the quality of life of patients. JAK-STAT signaling pathway plays an important role in the production and activation of synovial cytokines in rheumatoid arthritis joints. Traditional Chinese medicine has unique advantages in treating rheumatoid arthritis. In this paper, the mechanism of traditional Chinese medicine in the treatment of rheumatoid arthritis via JAK-STAT signaling pathway in recent years is briefly reviewed, in order to broaden the thinking for clinical and experimental studies.

46 Research progress on radioresistance mechanism of cervical cancer#br#

Cervical cancer is the most common malignant tumor of female reproductive system, middle and advanced stage and postoperative high-risk factors of early cervical cancer need supplementary treatment, mainly radiotherapy. Improving the sensitivity of cervical cancer to radiation has always been an urgent problem for gynecological oncologists. Recent studies have found that radioresistance of cervical cancer is closely related to cancer stem cell, DNA damage repair, and tumor microenvironment. The mechanisms of radioresistance induced by cancer stem cell include epithelial-mesenchymal transformation, hypoxia, oxidative regulation, intracellular phagocytosis, and inflammatory reaction, etc. Factors related to DNA damage repair pathway, such as ataxic telangiectasia-mutant gene, ATM-Rad3-related protein kinase, and genes interacting with the pathway, may be targets for improving radiotherapy sensitivity. Some factors in tumor microenvironment play a role in radioresistance of cervical cancer. The above factors are interrelated and interact with each other, and their specific mechanism of action affects the sensitivity and efficacy of radiotherapy for cervical cancer. This paper reviews the research progress on the mechanism of radioresistance of cervical cancer.

50 Research progress on polymorphism of drug-related genes in essential hypertension

Essential hypertension is the most common chronic cardiovascular disease, controlling blood pressure through drugs is the main means to treat hypertension and reduce its complications. However, individual differences of antihypertensive drugs are very common in clinical practice, which is mainly related to the variation of metabolic enzymes, receptors, and other drug targets of antihypertensive drugs. The purpose of this article is to review the relationship and mechanism between gene polymorphisms associated with seven antihypertensive drugs and individual differences in hypertension drugs, and formulat different treatment regimens according to individual genetic variation of metabolic enzymes, receptors and other drug targets associated with antihypertensive drugs, so as to implement individualized and precise treatment.

54 Research progress of lncRNA PVT1 in orthopedic diseases#br#

In today’s aging society, the incidence rate of orthopedic diseases related to the elderly is increasing, which seriously threatens human health. Long non-coding RNA (lncRNA) belongs to non-coding RNA, which plays an important role in the occurrence and development of many diseases. Plasmacytoma variant translocation gene 1 (PVT1) is one of the lncRNAs, which plays an important role in orthopedic diseases. It is expected to become a therapeutic target for osteopathic diseases such as osteoarthritis, rheumatoid arthritis and asteosarcoma, as well as a biomarker for diagnosis, prognosis, and drug resistance. This article reviews the research progress of PVT1 in orthopedic diseases at home and abroad, so as to provide theoretical basis for basic research and clinical practice of orthopedics.

58 Investigation on death anxiety of nurses in hepatobiliary surgery and analysis of influencing factors#br#

Objective To investigate the death anxiety of nurses in hepatobiliary surgery and influencing factors, and to provide the basis for the hospital to formulate countermeasures to relieve the death anxiety of nurses. Methods Convenience sampling was used to conduct death anxiety surveys on 382 nurses in the Department of Hepatobiliary Surgery of four Class-A hospitals in Nanjing from July to December 2020 by using self-compiled general information questionnaire, death anxiety scale, and empathy. Descriptive analysis, single factor analysis, correlation analysis, and multiple linear hierarchical regression analysis were used to discuss related influencing factors. Results The score of death anxiety of hepatobiliary surgery nurses was (7.46±1.89) points, among which the score of emotional dimension was the highest, followed by stress and pain, time awareness, and cognition. There were statistically significant differences in the death anxiety scores of nurses with different working years, participation in death education, death of friends or relatives in the past five years, had suffered major diseases (P < 0.05). The empathy score of nurses in hepatobiliary surgery was (57.65±10.30) points. The death anxiety of nurses was negatively correlated with empathy (r = -0.643, P < 0.05). Multivariate linear hierarchical regression analysis showed that different working years, participation death education, death of friends or relatives in the past five years, major illnesses, and empathy were independent factors influencing the death anxiety of hepatobiliary surgery nurses (P < 0.05), which could explain 52.1% of the total variation. Conclusion The death anxiety level of hepatobiliary surgery nurses is relatively high. Hospital administrators should intervene in the death anxiety level of nurses in terms of death education and empathy, so as to improve the mental health of nurses and promote the development of high-quality nursing services.

Objective To retrospectively investigate and analyze the current status of anticoagulant prevention in hospitalized patients with gastrointestinal tumors, and to explore the role that clinical pharmacists played in the prevention of venous thromboembolism (VTE) in hospitalized patients with gastrointestinal tumors. Methods A random number table was used to select 120 patients with gastrointestinal tumors hospitalized in a tertiary hospital in Shanghai from July to December 2019, the risk of venous thrombosis was evaluated, and the use of anticoagulant drugs in the patients were retrospectively investigated and analyzed. Results The Caprini score scale was used in 46 cases for surgical patients; the Khorana score scale was used for cancer chemotherapy patients, a total of 41 cases; and the Padua score scale was used for medical inpatients, a total of 33 cases. Among 120 patients, Nadroparin or Rivaroxaban for preventive anticoagulation was chosen by 19 cases. According to the Caprini score, 45 patients with high-risk tumors ≥3 points were obtained, of which 17 were given preventive anticoagulation; 41 patients with Khorana score ≥1 point were medium-high-risk patients, and there were no patients with preventive anticoagulation; there were 13 high-risk patients with Padua score ≥4 points, and 1 case of preventive anticoagulation. Conclusion The overall rate of anticoagulation prevention for patients with gastrointestinal tumors in the hospital is relatively low. Clinical pharmacists should strengthen the education of tumor-related VTE in clinical departments, improve clinicians’ awareness of preventing anticoagulation, reduce the incidence of VTE in patients with malignant gastrointestinal tumors and improve their survival rate.

Objective To explore the participation of undergraduate dentistry students in scientific research projects for undergraduate students, and to analyze the factors that affect their participation and their cognition of scientific research training, and to provide possible reform directions for the scientific research training module of undergraduates. Methods From January to March 2021, a questionnaire was used to randomly sample and investigate the participation of undergraduates from the 2016 to 2018 grades of the West China Hospital of Stomatology. Results 81.09% (373 students) of undergraduate had participated in scientific research projects, and more than half of the students believed that this activity had deepened their understanding of scientific research. The main motivation for undergraduates to participate in scientific research projects (64.34%, 240 students) was scholarship awards or research guarantees, and the main obstacles were implementation details, thesis writing, and teacher guidance. Regarding the necessity of cultivating scientific research ability at the undergraduate level, 39.35% (181 students) of undergraduate thought it was necessary, 38.48%(177 students) thought it was related to career planning. 50%(230 students) of undergraduate thought that they might participate in research-related work after graduation. Conclusion Undergraduate have a high degree of participation in scientific research training projects, and their participation motivation and scientific research cognition need to be adjusted and guided. Cultivating correct scientific research cognition, improving the allocation of resources for undergraduate scientific research training, and establishing a complete scientific research skills evaluation system may be the direction of reform for medical undergraduate scientific research skills training.

72 Analysis of the current status and influencing factors of the care burden of donors for pediatric liver transplantation#br#

Objective To understand the current situation of donor care burden in pediatric liver transplantation and to analyze the influencing factors. Methods The convenience sampling method was adopted to select 110 pediatric liver transplant donors admitted to hospital from January 2018 to March 2021 in the organ transplantation department of a tertiary hospital in Guangxi as the research objects. The general information questionnaire, caregiver burden scale, social support rating scale for investigation were used to survey. Results The total score of care burden for children liver transplantation donors was (37.16±12.92) points, of which the personal burden dimension was (18.81±6.52) points, and the responsibility burden dimension was (26.41±6.53) points, which was at a moderate level of care burden. There were statistically significant differences in the scores of the care burden of liver transplantation donors for children of different ages, postoperative pediatric intensive care unit (PICU) hospitalization days, complications, only child, postoperative recovery, hospital stay and donor education level, daily nursing time, family average monthly income, physical condition, and degree of social support (P < 0.05). Multiple linear regression analysis showed that the number of postoperative PICU hospitalization days, recovery, family average monthly income and physical condition of the donor’s family were independent risk factors for the burden of care for the donor of pediatric liver transplantation (P < 0.05). Conclusion The burden of care for pediatric liver transplantation donors is heavier and the degree of care preparation is low. Medical staff should adopt individualized intervention measures to reduce the burden of donor care and provide a basis for clinical nursing intervention.

76 Detection of CRP and Lp(a) levels and correlation with myocardial injury during PCI perioperative period in patients with coronary heart disease and angina pectoris #br#

Objective To observe the levels of C-reactive protein (CRP) and lipoprotein (a) [Lp(a)] in the percutaneous coronary intervention (PCI) perioperative period in patients with coronary heart disease and angina pectoris and their correlation with myocardial injury. Methods The clinical data of 80 patients with coronary heart disease and angina pectoris diagnosed and treated in Chaozhou Central Hospital in Guangdong Province (“our hospital” for short) from March 2020 to March 2021 were retrospectively analyzed. According to the type of angina pectoris, they were divided into stable group (stable angina pectoris, 38 cases), and unstable group (unstable angina, 42 cases). In addition, 50 healthy people who received physical examination in our hospital during the same period were selected as the control group. The immunoscattering turbidimetric method was used to detect the CRP level, the cardiac marker detector was used to detect the cardiac troponin T (cTnT) level, and the Beckman DXC600 detector was used to detect the Lp(a) level. According to the postoperative value of cTnT, they were divided into normal group (cTnT<0.014 μg/L, 32 cases) and elevated group (cTnT>0.014 μg/L, 48 cases), and the levels of CRP and Lp(a) were compared between the two groups. At the same time, multivariate logistic regression was selected to analyze the correlation between CRP, Lp(a) levels and myocardial injury. Results The CRP and Lp(a) levels of the stable group and the unstable group were higher than those of the control group, while the unstable group was higher than the stable group, and the differences were statistically significant (P < 0.05). The levels of CRP and Lp(a) in the elevated group were higher than those in the normal group, and the differences were statistically significant (P < 0.05). High levels of CRP and Lp(a) were independent risk factors for the increase of cTnT in patients with coronary heart disease and angina pectoris during PCI perioperative period (OR > 1, P < 0.05). Conclusion The levels of CRP and Lp(a) in patients with coronary heart disease and angina pectoris increased significantly during PCI perioperative period, and the increased levels of CRP and Lp(a) are independent risk factors for myocardial injury after PCI.

80 Efficacy and safety of Olanzapine compared with Risperidone in the treatment of mental and behavioral symptoms of Alzheimer’s disease:a meta-analysis

Objective To systematically evaluate the efficacy and safety of Olanzapine compared with Risperidone in the treatment of mental and behavioral symptoms of Alzheimer’s disease, in order to provide evidence-based reference for rational clinical drug use. Methods PubMed, Embase, Cochrane Library, CNKI, Wanfang Database, VIP Database and Chinese Biomedical Literature Database were searched by computer. Randomized controlled trials (RCT) of Olanzapine (experimental group) compared with Risperidone (control group) in the treatment of mental and behavioral symptoms of Alzheimer’s disease from inception to published in August 2020 of each database were collected. Literature was screened, data were extracted, literature quality was evaluated using the Cochrane handbook for systematic reviews of bias risk assessment tool, and meta-analysis was performed using Rev Man 5.3 software. Results A total of 13 studies were included, with a total of 1448 patients. Meta-analysis showed that the total effective rate of experimental group (RR = 1.14, 95%CI [1.06, 1.23], P = 0.0008) was significantly higher than that of control group. After two weeks of treatment (WMD = -2.67, 95%CI [-3.79,-1.55], P < 0.000 01) and four weeks later (WMD = -2.83, 95%CI [-4.02,-1.65], P < 0.000 01) the scores of rating scale of the behavioral pathology in Alzheimer’s disease in experimental group were significantly lower than those in control group. After eight weeks of treatment, there were no significant differences between the two groups in the scores of rating scale of the behavioral pathology in Alzheimer’s disease (WMD = -1.40, 95%CI [-4.34, 1.54], P = 0.35) and the scores of mini mental state examination (WMD = 1.01, 95%CI [-1.73, 3.76], P = 0.47). The incidence of extrapyramidal reaction in experimental group (RR = 0.52, 95%CI [0.34, 0.80], P = 0.003) and the incidence of insomnia in experimental group (RR = 0.49, 95%CI [0.32, 0.76], P = 0.002) were significantly lower than those in control group. There were no significant differences in the incidence of headache, fatigue, blurred vision, irritation, liver function, increased body mass and lethargy between the two groups (P > 0.05). Conclusion Olanzapine is more effective than Risperidone in the treatment of mental and behavioral symptoms of Alzheimer’s disease, and the incidence of extrapyramidal reaction and insomnia is lower than Risperidone.

88 Clinical analysis of anti-CASPR2 antibody associated autoimmune encephalitis#br#

Objective To investigate the clinical features and treatment of anti-contactin associated protein-like 2(CASPR2) antibody associated autoimmune encephalitis. Methods Clinical data of five cases of anti-CASPR2 antibody associated autoimmune encephalitis diagnosed in the Third People’s Hospital of Zhengzhou and the First Affiliated Hospital of Henan University of Chinese Medicine from March 2017 to December 2020 were retrospectively analyzed. Results Among the five patients, there were three male patients, one patient had a history of fever before onset, two patients showed abnormal mental behavior, two patients showed epilepsy as the first symptom, and one patient showed memory loss. One patient had insomnia based on nerve pain, one patient had insomnia combined with cerebellar symptoms, and two patient had autonomic nerve dysfunction. Serum anti-CASPR2 antibody was positive in five patients, and cerebrospinal fluid anti-CASPR2 antibody was positive in one patient. All patients received immunotherapy, and the treatment was effective. Conclusion Anti-CASPR2 antibody associated autoimmune encephalitis may involve the central nervous system, the peripheral nervous system, the autonomic nervous system and other systems, leading to variable clinical syndrome, and immunotherapy is effective.

Objective To analyze the clinical effect of Yishen Zhuyu Decoction combined with Leuprorelin Acetate Microspheres in the treatment of endometriosis (EMT) of kidney deficiency and phlegm stasis. Methods From June 2018 to June 2020, 80 patients with EMT treated in Yangjiang Hospital of Traditional Chinese Medicine Affiliated to Guangzhou University of Chinese Medicine were selected as the research objects, according to random number table method, they were divided into two groups, with 40 cases in each group. The control group was treated with Leuprorelin Acetate Microspheres, and the study group was treated with Yishen Zhuyu Decoction on the basis of control group. The clinical efficacy, the levels of matrix metalloproteinase-9 (MMP-9) and tissue inhibitor of metalloproteinase-1 (TIMP-1) before and after treatment and the occurrence of adverse reactions in two groups were observed and recorded. Results The clinical effect of study group was better than that of control group (P < 0.05). After treatment, MMP-9 levels between two groups were lower than those before treatment, and TIMP-1 levels between two groups were higher than those before treatment (P < 0.05); MMP-9 level in study group was lower than that in control group, and TIMP-1 level was higher than that in control group (P < 0.05). The total incidence of adverse reactions in study group was lower than that in control group (P < 0.05). Conclusion The application of Yishen Zhuyu Decoction combined with Leuprorelin Acetate Microspheres in EMT is helpful to improve the therapeutic effect, regulate the levels of TIMP-1 and MMP-9 and reduce adverse reactions.

97 Meta-analysis of the relationship between vasculogenic mimicry and poor prognosis in patients with colorectal cancer#br#

Objective To investigate the relationship between vasculogenic mimicry (VM) and poor prognosis in patients with colorectal cancer. Methods The cohort studies on the relationship between VM and poor prognosis of colorectal cancer published in PubMed, Embase, Cochrane Library, Web of Science, CBM, CNKI, VIP, and Wanfang Data from inception to April 2021 were selected by computer. Then meta-analysis was performed by Stata 14.0 software. The pooled hazard ratio and its 95%CI were calculated. The subgroup analysis was performed. Results Five studies were included, including 769 patients and 203 VM positive patients. Meta-analysis showed a significant correlation between positive VM and poor prognosis (HR = 2.433, 95%CI: 1.844-3.023, P < 0.001). Subgroup analysis showed that sample size, extraction method of patients’ prognosis, and comparability adjustment between groups had no significant influence on the correlation between VM positivity and patients’ prognosis (I2<50%, P > 0.1). Conclusion VM positive is significantly correlated with poor prognosis of colorectal cancer, and VM can be a potential direction for clinical treatment of colorectal cancer.

101 Expression and clinical significance of screen lncRNAs in cervical squamous cell carcinoma based on high-throughput sequencing#br#

Objective To investigate the expression and clinical significance of long non-coding RNA (TOPORS-AS1, TMPO-AS1, LINC00925, MIR155HG, and NR2F2-AS1) in cervical squamous cell carcinoma (CSCC) based on high-throughput sequencing. Methods Cervical tissues of 80 Uyghur patients admitted to the First Affiliated Hospital of Xinjiang Medical University from June to December 2019 were collected, including cervicitis (20 cases), high-grade squamous intraepithelial lesion (HSIL, 20 cases) and CSCC (40 cases). The relative expression levels of long non-coding RNA in each group were detected, the differences of long non-coding RNA in each group and the relationship between MIR155HG and clinicopathological features of CSCC patients were compared. Results The expressions of TMPO-AS1, LINC00925, MIR155HG, and NR2F2-AS1 in CSCC group were higher than those in cervicitis group, and the differences were statistically significant (P < 0.05); the expressions of TOPORS-AS1, TMPO-AS1, LINC00925, MIR155HG, and NR2F2-AS1 in CSCC group were higher than those in HSIL group, and the differences were statistically significant (P < 0.05). The expression of MIR155HG in CSCC patients with mass diameter > 4 cm was higher than that in patients with mass diameter ≤4 cm, and the expression of MIR155HG in lymph node metastasis was higher than that in patients without lymph node metastasis, and the differences were statistically significant (P < 0.05). Conclusion TOPORS-AS1, TMPO-AS1, LINC00925, MIR155HG, and NR2F2-AS1 are highly expressed in cervical cancer tissues, and the expression of MIR155HG is correlated with mass diameter and lymph node metastasis status.

105 Research on the advantages of 3D printing template compared with simple CT-guided 125I seeds implantation in recurrent and metastatic cancer of head and neck#br#

Objective To investigate the advantages of 3D printing template compared with simple CT-guided 125I seeds implantation in recurrent and metastatic cancer of head and neck. Methods A total of 99 patients (99 lesions) with recurrent and metastatic cancer of head and neck treated with 125I seeds in Hebei General Hospital from April 2015 to February 2019 were selected, according to whether 3D printing template was applied, they were divided into 3D printing template group (group A, 49 cases [49 lesions]) and simple CT guided group (group B, 50 cases [50 lesions]). The quality and efficacy of 125I seeds implantation were evaluated between two groups; CT scan times, operation time and single seed implantation time were compared between two groups; the target volume, seeds number and dosimetric parameters (D90, V90, V100, V150) before and after operation were compared between two groups; the incidence of complications in two groups were observed. Results There was no significant difference in the quality of 125I seeds implantation between two groups (P > 0.05). Two months after operation, there were no significant differences in local control rate and effective rate between two groups (P > 0.05). Three months after operation, there was no significant difference in local control rate between two groups (P > 0.05); the effective rate of group A was higher than that of group B, and the difference was statistically significant (P < 0.05). The operation time and single seed implantation time of group A were shorter than those of group B, and CT scan times were less than those of group B, and the differences were statistically significant (P < 0.05). There were no significant differences in target volume, seeds number, and dosimetric parameters in group A before and after operation (P > 0.05). There were no significant differences in target volume, seeds number, D90, and V90 in group B before and after operation (P > 0.05); after operation, V100 in group B was lower than that before operation, and V150 was higher than that before operation, and the differences were statistically significant (P < 0.05). After operation, there were no significant differences in target volume, seeds number, D90, V90, and V150 between two groups (P > 0.05); V100 in group A was higher than that in group B, and the differences were statistically significant (P < 0.05). The incidence of puncture pain in group A was lower than that in group B, and the difference was statistically significant (P < 0.05); there was no significant difference in the incidence of hoarseness between two groups (P > 0.05). Conclusion Compared with simple CT guidance, 3D printing template can shorten the operation time, reduce the number of scans, reduce the incidence of complications, and improve the efficacy to a certain extent.

110 Real-world study of the efficacy and safety of Carrelizumab combined with chemotherapy in the treatment of advanced non-small cell lung cancer#br#

Objective To explore the real-world study of the efficacy and safety of Carrelizumab in the treatment of advanced non-small cell lung cancer. Methods A total of 62 patients with advanced non-small cell lung cancer treated with Carrelizumab from March 2019 to March 2020 who admitted to the Taizhou People’s Hospital of Jiangsu Province were selected, and according to the wishes of patients, they were divided into single-drug group (Carrelizumab, 41 cases) and combination group (Carrelizumab combined with chemotherapy regimen, 21 cases). After three cycles of treatment, the clinical efficacy of two groups were compared, adverse drug reactions of two groups were observed, and progression-free survival and median survival time of two groups was observed after 12 months of follow up. Results The total effective rate of combination group was higher than that of single-drug group (P < 0.05). There was no statistically significant difference in the total incidence of adverse reactions between two groups (P > 0.05). Progression-free survival rate and median survival time between two groups were statistically significant (P < 0.05). Conclusion Carrelizumab has a good effect in the treatment of advanced non-small cell lung cancer in real-world study, and the combined treatment effect is better.

113 Correlation between hope level and attribution mode of patients with advanced breast cancer after breast-conserving chemotherapy#br#

Objective To explore the relationship between the level of hope and the attribution mode of patients with advanced breast cancer after breast-conserving chemotherapy. Methods A total of 113 patients with advanced breast cancer after breast-conserving chemotherapy in the West China Hospital from January 2019 to January 2020 were prospectively selected as the research objects. Before chemotherapy, one month after chemotherapy, and three months after chemotherapy, hope level (assessed by Herth hope level scale) and attribution method (assessed by attributional style questionnaire [ASQ]) of patients were evaluated, the changes of hope level and attribution mode in patients were observed, and the relationship between hope level and attribution mode were analyzed. Results After one and three months of chemotherapy, the scores and total scores of the Herth hope level scale were higher than those before chemotherapy, while after three months of chemotherapy were higher than after one month of chemotherapy, and the differences were statistically significant (P < 0.05). After one and three months of chemotherapy, the positive event scores of patients with advanced breast cancer after breast-conserving chemotherapy on the ASQ were higher than those before chemotherapy, while the negative event scores were lower than those before chemotherapy, and the differences were statistically significant (P < 0.05). And after three months of chemotherapy, the positive event score on the ASQ was higher than that of chemotherapy for one month, while the negative event score was lower than that of chemotherapy for one month, and the differences were statistically significant (P < 0.05). Correlation analysis results showed that the scores and total scores of the various dimensions of the herth hope level scale were positively correlated with the positive event scores in the ASQ (r > 0, P < 0.05), while it was negatively correlated with the negative event score in the ASQ of attribution (r < 0, P < 0.05). Conclusion For patients with advanced breast cancer after breast-conserving chemotherapy, the level of hope is gradually increasing, and the attribution method gradually tends to be positive. There is a clear correlation between the level of hope and the attribution mode of patient.

117 Effect of early individualized enteral nutrition in patients with swallowing dysfunction after stroke#br#

Objective To explore the effect of early individualized enteral nutrition on patients with swallowing dysfunction after stroke. Methods A total of 125 patients with stroke and swallowing dysfunction who received nasogastric tube feeding in Shanghai Fourth Rehabilitation Hospital from January 2018 to July 2020 were prospectively selected. They were divided into group A (63 cases, received individualized enteral nutrition support) and group B (62 cases, received home-made uniform diet support) according to random number table method. Nutritional status, complications and rehabilitation effect were compared between the two groups after 30 days of treatment. Results After 30 days of treatment, there were no significantly differences in hemoglobin, albumin, and prealbumin in group A compared with before treatment (P > 0.05), while hemoglobin, albumin, and prealbumin in group B were lower than those before treatment and the group A were higher than those in group B, the differences were statistically significant (P < 0.05). The incidence of pulmonary infection and gastrointestinal complications (gastric retention, constipation, and diarrhea) in group A were lower than those in group B, and the differences were statistically significant (P < 0.05). After 30 days of treatment, simplified Fugl-Meyer motor function score, mini-mental state test and modified Barthel index in two groups were higher than before, and group A was higher than group B, and the differences were statistically significant (P < 0.05). Conclusion The early individualized enteral nutrition treatment according to the actual condition of the stroke patients with swallowing dysfunction, can effectively improve the patient’s nutritional status, promote recovery, and reduce lung infection and gastrointestinal complications.

Methods A retrospective selection of 109 patients with chronic hepatitis B and liver cirrhosis admitted to Jilin Province Faw General  Hospital from December 2018 to December 2020, they were divided into Entecavir (ETV) group (55 cases) and Tenofovir Disoproxil Fumarate (TDF) group (54 ceses) according to their treatment methods. Blood calcium (Ca), blood phosphorus (P), estimated glomerular filtration rate (eGFR) and bone mineral density (BMD) were collected before treatment (T0), 24 weeks after treatment (T1), 48 weeks after treatment (T2), and 96 weeks after treatment (T3), and the clinical significance of changes in Ca, P, eGFR and BMD were analyzed and compared. Results The time point difference of Ca between the two groups was statistically significant (P < 0.05). There was no significant difference in P at time point, between groups and interaction (P > 0.05). The time point and interaction of eGFR showed statistically significant differences (P < 0.05), but there was no statistically significant difference between groups (P < 0.05). Further pairwise comparison, intra-group comparison: Ca decreased at T3 compared with T0, and eGFR decreased at T1, T2, and T3 in TDF group compared with T0, with statistical significances (P < 0.05). The time comparison of femoral neck bone mineral density and lumbar vertebra bone mineral density between the two groups had statistical significances (P < 0.05). Further pairwise comparison showed that lumbar bone mineral density in TDF group was lower than that in ETV group at T2 and T3, with statistical significances (P < 0.05).  Conclusion ETV and TDF in the treatment of patients with hepatitis B and cirrhosis of the liver, the two drugs is not obvious effect on Ca, P, patients with the extension of treatment time of TDF affect the eGFR, TDF bone mineral density of patients treated by TDF is higher than ETV, and both the mean bone density decrease, as the treatment time extended effect is not obvious, and provide the basis for clinical doctor standard antiviral treatment for a long time.

125 Analysis of risk factors and drug resistance in chronic obstructive pulmonary disease complicated with Klebsiella pneumoniae infection#br#

Objective To explore the risk factors of chronic obstructive pulmonary disease (COPD) complicated with Klebsiella pneumoniae infection, and to analyze the drug resistance of Klebsiella pneumoniae. Methods The clinical data of 198 COPD patients admitted to Wuxi Ninth Hospital, Soochow University from March 2016 to March 2021 were retrospectively analyzed. According to the presence or absence of Klebsiella pneumoniae infection, they were divided into the infected group (24 cases) and the uninfected group (174 cases). The general data two of groups were collected, multiple logistic regression was used to analyze the risk factors of COPD complicated with Klebsiella pneumoniae infection, and K-B disk diffusion method was used to detect the resistance of Klebsiella pneumoniae to Ceftazidime and other antibiotics. Results The proportion of hospitalization days ≥14 days and diabetes in the infected group were higher than those in the uninfected group, and the differences were statistically significant (P < 0.05). The proportion of patients with mechanically ventilated in the infected group was lower than that in the uninfected group, and the difference was statistically significant (P < 0.05). Hospitalization days ≥14 days, diabetes mellitus, and no mechanical ventilation were all risk factors for COPD and Klebsiella pneumoniae infection (P < 0.05). There was no resistance of Klebsiella pneumoniae to Meropenem and Imipenem. The resistance of Klebsiella pneumoniae to Cefepime, Tobramycin and Ciprofloxacin were all >40%. The resistance of Klebsiella pneumoniae to Ceftazidime, Levofloxacin, Ampicillin and Sulbactam, Piperacillin, and Tazobactam was 10%-40%. Conclusion COPD patients with long hospital stays, diabetes melitus, and no mechanical ventilation have a higher risk of Klebsiella pneumoniae infection. Klebsiella pneumoniae has multidrug resistance, antibiotics should be used clinically, and reasonable measures should be taken to avoid the occurrence of Klebsiella pneumoniae infection.

129 Evaluation value of serum HMGB1 combined with EVLWI in the prognosis of elderly patients with severe pneumonia complicated with acute respiratory distress syndrome#br#

Objective To investigate the predictive value of serum highy mobility group protein B1 (HMGB1) combined with extravascular lung water index (EVLWI) on the prognosis of elderly patients with severe pneumonia complicated with acute respiratory distress syndrome (ARDS). Methods The clinical data of 125 elderly patients with severe pneumonia complicated with ARDS in Jinan People’s Hospital from January 2018 to June 2020 were analyzed retrospectively. They were divided into death group (54 cases) and survival group (71 cases) according to the survival condition. The clinical data, HGMB1, and EVLWI of the two groups were compared. The prognostic factors of elderly patients with severe pneumonia complicated with ARDS were analyzed by logistic regression. Receiver operating characteristic curve was used to analyze the prognostic value of HMGB1 combined with EVLWI in elderly patients with severe pneumonia complicated with ARDS. Results The levels of serum HMGB1, EVLWI, and sepsis-related organ failure assessment score in the death group were higher than those in the survival group, oxygenation index was lower than that of the survival group, and the differences were statistically significant (P < 0.05). Multivariate analysis showed that high HMGB1 level and high EVLWI index were independent risk factors for prognosis of elderly patients with severe pneumonia complicated with ARDS (OR > 1, P < 0.05). HMGB1 and EVLWI alone and in combination had certain diagnostic value for the prognosis of elderly patients with severe pneumonia complicated with ARDS (AUC > 0.5, P < 0.05). The area under the curve of HMGB1 combined with EVLWI in predicting the prognosis of elderly patients with severe pneumonia complicated with ARDS was 0.904, which was higher than those of HMGB1 and EVLWI alone (P < 0.05). Conclusion High levels of HMGB1 and EVLWI are associated with poor prognosis in elderly patients with severe pneumonia complicated with ARDS, and combination of HMGB1 and EVLWI can improve the prognostic efficacy of patients.

133 Application of intracavity radiofrequency closure in the treatment of varicose of great saphenous vein#br#

Objective To evaluate the value of intracavity radiofrequency closure in the treatment of varicose of great saphenous vein. Methods A total of 126 patients with varicose of great saphenous vein admitted to Electric Power Teaching Hospital of Capital Medical University from June 2019 to June 2020 were selected. They were divided into control group and observation group (63 cases in each group) by random number table method. The control group received traditional surgery, and the observation group received intracavity radiofrequency closure. The therapeutic effect, perioperative indicators, and hemodynamic indexes 12 hours after surgery were compared between the two groups. The levels of inflammatory factors were compared between the two groups before surgery and one week after surgery, and the venous clinical severity score were compared between the two groups before surgery and six months after surgery. The two groups were followed up for one year to observe the incidence of adverse reactions. Results The amount of intraoperative blood loss in the observation group was lower than that in the control group, and the surgery time, postoperative time out of bed, and hospital stay were shorter than those in the control group, with statistical significances (P < 0.05). There were no significant differences in percutaneous arterial oxygen saturation, heart rate, and mean arterial pressure between the two groups at 12 hours after surgery (P > 0.05). Serum levels of tumor necrosis factor-α, interleukin-6, and matrix metalloproteinase-13 in two groups one week after operation were lower than those before surgery, and the observation group was lower than the control group, the differences were statistically significant (P < 0.05). The venous clinical severity scores at six months after surgery was lower than that before surgery, and the observation group was lower than the control group, the differences were statistically significant (P < 0.05). There was no significant difference in treatment effect between the two groups (P > 0.05). There was no significant difference in the incidence of deep vein thrombosis, subcutaneous hematoma, phlebitis, saphenous nerve injury, pigmentation, and varicose recurrence between the two groups (P > 0.05). The incidences of wound infection/poor healing and thigh hematoma in the observation group were lower than those in the control group, and the differences were statistically significant (P < 0.05). Conclusion Intracavity radiofrequency closure for the treatment of varicose of great saphenous vein can effectively relieve the symptoms of lower limb veins and reduce the inflammatory state of the body, with less surgical trauma, less intraoperative bleeding, less complications, precise clinical efficacy and high safety.

Professor Cao Zhiqun has concluded that the occurrence of refractory gastroesophageal reflux disease is closely related to stagnation fire in his long-term clinical practice. The basis of refractory gastroesophageal reflux disease is qi depression which causes the stomach to lose its balance. It is considered that the treatment should follow the principle of release of fire stagnation, to deal with the cause of stagnation fire, opening the stagnant blockage is the first step. In response to the results of the heat of stagnation, the method of clearing heat is added to the method of dispersing and clearing heat, while taking care of nourishing yin fluid. Methods A total of 82 CAG patients with syndrome of disharmony of liver and stomach admitted to Jiangyin Hospital of Traditional Chinese Medicine Hospital, Jiangsu Province from December 2017 to December 2020 were selected as the research subjects, according to random number table method, the patients were divided into control group and study group, with 41 cases in each group. The control group was treated with conventional Western medicine, and the study group was treated with the method of soothing liver and regulating qi on the basis of the control group. Both groups were treated for three months. The traditional Chinese medicine syndrome score and the condition of NBI-ME endoscopy grading before and after treatment were observed in the two groups, the clinical efficacy and incidence of adverse reactions were compared between the two groups. Results After treatment, the clinical efficacy of the study group was better than that in the control group (P < 0.05). The traditional Chinese medicine syndrome scores in both groups were lower than those before treatment, which in the study group were lower than that in the control group (P < 0.05). After treatment, the condition of NBI-ME endoscopy grading in the two groups were better than those before treatment, which in the study group was better than that in the control group (P < 0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P > 0.05). Conclusion The effect of soothing liver and regulating qi in the treatment of CAG with syndrome of disharmony of liver and stomach is significant, which has certain reverse effect, safe, and reliable.

158 Effect of acupuncture combined with Wenpi Tongluo Kaiqiao Decoction in the treatment of vascular dementia with syndrome of phlegm turbidity obstructing the orifices#br#

Objective To explore the effect of acupuncture combined with Wenpi Tongluo Kaiqiao Decoction in the treatment of vascular dementia (VD) with syndrome of phlegm turbidity obstructing the orifices. Methods A total of 113 VD patients with syndrome of phlegm turbidity obstructing the orifices admitted to Fuxin Mine General Hospital of Liaoning Health Industry Group from January 2018 to March 2021 were selected as the research subjects. According to the random number table method, the patients were divided into control group (56 cases) and experimental group (57 cases). The control group was treated with conventional Western medicine and acupuncture, and the experimental group was treated with Wenpi Tongluo Kaiqiao Decoction on the basis of the control group, both groups were treated for 12 weeks. The clinical efficacy, traditional Chinese medicine syndrome score, mini-mental state examination (MMSE) scale, Hasegawa dementia scale (HDS), activities of daily living (ADL) scale, interleukin-6 (IL-6), vascular endothelial growth factor (VEGF), and tumor necrosis factor-α (TNF-α) in the two groups were compared, the incidence of adverse reactions were observed. Results After treatment, the total effective rate of the experimental group was higher than that in the control group (P < 0.05). After treatment, the headache with binding sensation score, hypophrenia score, anorexia and abdominal distension score, silence score, slow movement score, and vomiting score in the two groups were decreased, which in the experimental group were lower than those in the control group (P < 0.05). After treatment, the MMSE, HDS, and ADL scores in the two groups were increased, which in the experimental group were higher than those in the control group (P < 0.05). After treatment, the levels of serum IL-6 and TNF-α in the two groups were decreased, and which in the experimental group were lower than those in the control group (P < 0.05), the VEGF levels in the two groups were increased, which in the experimental group was higher than that in the control group (P < 0.05). There was no significant difference in the incidence of adverse reactions in the two groups (P > 0.05). Conclusion Acupuncture combined with Wenpi Tongluo Kaiqiao Decoction in the treatment of VD patients with syndrome of phlegm turbidity obstructing the orifices can rapidly improve the clinical symptoms, has definite curative effect, can reduce inflammatory factors, improve the ability of life, cognitive function and neurological function.

Objective To investigate the intervention effect of tetrandrine on radiation pneumonia and its influence on transforming growth factor-β1 (TGF-β1) and interleukin-6 (IL-6). Methods A total of 75 radiation pneumonia patients admitted to Nanjing Chest Hospital from June 2017 to December 2020 were selected as the research subjects. They were divided into the control group (37 cases) and the test group (38 cases) by random number table method, and the control group was given conventional treatment, the experimental group was given oral tetrandrine on the basis of conventional treatment, both groups were treated for two weeks. Vital capacity (VC), forced expiratory volume in the first second (FEV1), TGF-β1 and IL-6 levels were compared before and after treatment. Results After treatment, the total effective rate of experimental group was higher than that in the control group (P < 0.05). After treatment, VC and FEV1 in the two groups were higher than those before treatment, TGF-β1 and IL-6 in the two groups were lower than those before treatment, and VC and FEV1 in the experimental group were higher than those in the control group, TGF-β1 and IL-6 in the experimental group were lower than those in the control group (P < 0.05). There were no obvious drug-related adverse reactions in the both groups. Conclusion Tetrandrine can improve the symptoms of radiation pneumonia and reduce the inflammation reation caused by radiotherapy.

167 Effect of Banxia Xiexin Decoction combined with acupuncture in the treatment of chronic atrophic gastritis with syndrome of cold and heat complex#br#

Objective To explore the effect of Banxia Xiexin Decoction combined with acupuncture in the treatment of chronic atrophic gastritis (CAG) with syndrome of cold and heat complex. Methods A total of 78 CAG patients with syndrome of cold and heat complex admitted to Suzhou Hospital of Integrated Traditional Chinese and Western Medicine, Suzhou Hospital of Traditional Chinese Medicine, Jiangsu Province from July 2020 to March 2021 were selected as the research subjects. According to random number table method, they were divided into control group (39 cases) and observation group (39 cases). The control group was treated with conventional Western medicine combined with acupuncture, and the observation group was treated with Banxia Xiexin Decoction on the basis of the control group, and both groups were treated for four weeks. The clinical efficacy, pepsinogenⅠ(PGⅠ) / pepsinogenⅡ (PGⅡ), interleukin-17 (IL-17), traditional Chinese medicine syndrome score and safety were observed between the two groups. Results After treatment, the total effective rate in the observation group was higher than that in the control group (P < 0.05). The PGⅠ/PGⅡ in the two groups was higher than that before treatment, which in the observation group was higher than that in the control group (P < 0.05). The serum IL-17 levels in the two groups were lower than that before treatment, which in the observation group was lower than that in the control group (P < 0.05). The traditional Chinese medicine syndrome symptom scores in the two groups were lower than that before treatment, which in the observation group was lower than that in the control group (P < 0.05), there was no significant difference in the incidence of adverse reactions between the two groups (P > 0.05). Conclusion Banxia Xiexin Decoction combined with acupuncture in the treatment of CAG patients with syndrome of cold and heat complex have a definite effect, which can reduce the body’s inflammation, and regulate PGⅠ/ PGⅡ balance, safe, and reliable.

Objective To explore the clinical value of structured nursing based on internet electronic medical record for elderly patients with rheumatoid arthritis. Methods A total of 90 elderly patients with rheumatoid arthritis admitted to Huaian First People’s Hospital of Jiangsu Province from March 2018 to May 2020 were selected, according to the order of admission, patients were included in the study group and the control group, with 45 cases in each group. The control group was given routine nursing intervention, and the study group was given structured nursing intervention based on internet electronic medical record, the pain score, compliance score, and quality of life score of the two groups after nursing intervention were compared. Results The pain score of the study group was lower than that of the control group, and the difference was statistically significant (P < 0.05); the scores of treatment compliance and quality of life in the study group were higher than those in the control group, and the differences were statistically significant (P < 0.05). Conclusion Elderly patients with rheumatoid arthritis received structured nursing intervention based on the internet electronic medical record, has the effect of promoting curative effect and relieving pain, and then indirectly improve the quality of life.

Objective To observe the nursing effect of fast track surgery (FTS) concept in perioperative period of laparoscopic total hysterectomy. Methods A total of 79 patients undergoing total hysterectomy admitted to Affiliated Hospital of Jiangnan University from July 2017 to January 2020 were selected, according to random number table method, they were divided into study group (n = 39) and control group (n = 40). The control group received routine perioperative nursing, and the study group received perioperative FTS idea-guided nursing, the time of first exhaust, time of gett out of bed, hospitalization time, hospitalization cost, and incidence of complications of venous thrombosis, infection and bleeding of lower limbs were compared between the two groups after receiving different nursing modes. Results The first exhaust time, getting out of bed, and hospitalization time in the study group were shorter than those in the control group, and the hospitalization cost was lower than that in the control group, the differences were statistically significant (P < 0.05); the total incidence of complications of lower limb venous thrombosis, infection, and bleeding in study group was lower than that in control group, and the difference was statistically significant (P < 0.05). Conclusion Perioperative nursing measures guided by the concept of FTS rapid rehabilitation surgery for patients undergoing laparoscopic total hysterectomy can shorten the time of first exhaust, the time of getting out of bed and the length of hospital stay, reduce the hospitalization cost and control the incidence of various complications, which is worthy of widespread clinical application.

177 Application of train the trainer model in neurosurgery health education#br#

Objective To explore the application effect of trainer the training (TTT) mode in the health education of neurosurgery patients. Methods From July 2018 to August 2019, 48 nursing staff and 880 hospitalized patients in the Department of Neurosurgery, the Third Medical Center of PLA General Hospital were selected as the research objects. From July 2018 to December 2018, 48 nursing staff were trained by conventional health education, and during the same period, 440 patients (control group) were given nursing. Forty-eight nurses were trained in TTT mode from March to August 2019, and during the same period, 440 patients (study group) were given nursing. The theoretical knowledge and operation skill assessment results of nursing staffs in the department after training and patients’ satisfaction with nuring after TTT mode were compared. Results The scores of theoretical knowledge and operational skills of nursing staff after training were higher than those before training, the differences were statistically significant (P < 0.05). The satisfaction of the experimental group was higher than that of the control group, and the difference was statistically significant (P < 0.05). Conclusion TTT mode can effectively improve the theoretical level and practical ability of health education of neurosurgical nursing staff, and improve the satisfaction of patients.

181 Effects of SOLDQ-based graded nursing model on lung function, ADO index and treatment compliance of children with bronchial asthma#br#

Objective To explore the effect of Seattle obstructive lung disease questionnaire (SOLDQ)-based graded nursing mode on lung function, ADO index and treatment compliance of children with bronchial asthma. Methods A total of 100 children with bronchial asthma admitted to Tangdu Hospital of Air Force Medical University of PLA from January to December 2020 were selected. They were divided into control group and observation group by random number table method, with 50 cases in each group. The control group was given routine nursing mode, and the observation group was given graded nursing mode based on SOLDQ, and the lung function indexes, ADO index, and treatment compliance were compared. Results After intervention, the levels of vital capacity, forced vital capacity, and forced expiratory volume in one second in both groups were higher than before intervention, and the observation group was higher than the control group (P < 0.05). D and O in two groups were lower than before intervention, and observation group was lower than control group (P < 0.05). The treatment compliance of observation group was higher than that of control group (P < 0.05). Conclusion Under the guidance of SOLDQ model, the application effect of graded nursing in children with bronchial asthma is obvious, which can improve the lung function and ADO index of children, and improve the treatment compliance of children, which is worthy of widespread clinical promotion and application.

185 Effect of accelerated rehabilitation surgical nursing based on postoperative functional exercise on postoperative functional recovery and compliance of patients with patella fracture

Objective To explore the influence of accelerated rehabilitation surgical nursing based on postoperative functional exercise on postoperative functional recovery and compliance of patients with patella fracture. Metohds A total of 90 patients with patella fracture admitted to Dazhou Central Hospital, Sichuan Province from January 2019 to December 2020 were selected, and divided into control group and observation group by random number table method, 45 cases in each group. The control group received conventional nursing, and the observation group received accelerated rehabilitation surgical nursing based on postoperative functional exercise on the basis of the control group. The knee function score, including scores of pain, function and stability, as well as knee range of motion, compliance (medication, exercise, diet, and other items) and complications the two groups were compared. Results After three months of intervention, knee function score and range of motion of knee joint in both groups were higher than before, and observation group was higher than control group, the differences were statistically significant (P < 0.05). The compliance of observation group was higher than that of control group, and the difference was statistically significant (P < 0.05). The complication rate in the observation group was lower than that in the control group, and the difference was statistically significant (P < 0.05). Conclusion The accelerated rehabilitation surgical nursing based on postoperative functional exercise can promote the postoperative functional recovery of patients with patella fracture, improve compliance and reduce the rate of complications, which is worthy of extensive clinical promotion and application.

The “14th Five-Year Plan” period is a critical period for the development of modern hospital management system. New progress has been made in the new mechanism of hospital operation, which is oriented by patients’ needs, guided by high-quality development and guaranteed to be sustainable. High-quality development and modern hospital management system have made important arrangements for the development and planning of public hospitals. This paper puts forward three problems in view of the current situation of the development planning of public hospitals: firstly, the formulation of long-term hospital planning is still in the exploratory stage; second, the completeness and scientificity of the formulation of long-term hospital planning need to be improved; thirdly, there is still room for improvement in the implementation and implementation of the hospital’s long-term plan. At the same time, four aspects are proposed to explore: firstly, strengthen the top-level design, attach great importance to the preparation of hospital development planning; second, make the development of high quality the goal orientation of public hospital planning; thirdly, modern hospital management system should be fully reflected in the operation of public hospitals; fourth, adhere to the problem-oriented approach and promote the formulation of hospital development plan through innovation.

Human resource cost is a major component of the total cost of hospitals, and its gradual increase has posed many challenges to the operation and management of public hospitals. Therefore, it is necessary to control the cost of human resources,  improve the operation efficiency of hospitals and maintain the sustainable and healthy development of hospitals. Combining with the actual situation of a tertiary hospital in Beijing, this paper sorts out the composition of human resource cost of the hospital and summarizes the main practices in the implementation of human resource cost control, points out the misunderstandings in cost control, and also formulates corresponding countermeasures. It aims to promote the optimization and upgrading of human resource cost management system of public hospitals and then enhance its own competitive advantages.
