4 Study on the protective effect and mechanism of Valsartan on acute liver failure in rats

Objective To investigate the protective effect and mechanism of Valsartan on acute liver failure in rats. Methods A total of sixty healthy male SD rats were divided into normal control group, liver failure group, and Valsartan group by random number table method, with 20 rats in each group. Normal control group was intraperitoneally injected with normal saline, liver failure group was intraperitoneally injected with D-galactosamine (D-GalN) / lipopolysaccharide (LPS) for modeling; Valsartan group was intraperitoneally injected with Valsartan (25 mg/kg) 30 min before modeling. The survival of rats in each group was observed. The levels of angiotensinⅡ (AngⅡ) and angiotensin 1-7 (Ang 1-7), and the radio of AngⅡ/Ang 1-7 in serum and liver tissues, and alanine aminotransferase (ALT), aspartate aminotransferase (AST), tumor necrosis factor α (TNF-α) and interleukin-6 (IL-6) in serum of rats were detected ten hours after model building. The level of inflammation andnecrosis in liver tissues was observed. The expression levels of glucose regulatory protein 78 (GRP78) and CCAAT/enhancer binding protein homologous protein (CHOP) in liver tissues were detected. Results The survival rate in Valsartan group was higher than that in liver failure group, and the difference was highly statistically significant (P < 0.01). The levels of AngⅡ and the radio of AngⅡ/Ang 1-7 in serum and liver tissues in Valsartan group were lower than those in liver failure group, and Ang 1-7 in Valsartan group were higher than that in liver failure group, with high statistical significance (all P < 0.01). Serum levels of ALT, AST, TNF-α and IL-6 in Valsartan group were lower than those in liver failure group, and the differences were highly statistically significant (all P < 0.01). In the liver failure group, the structure of liver lobules of part of liver tissues was disordered and the degeneration and necrosis of liver cells were serious. The degeneration and necrosis of liver cells in Valsartan group was less than that in liver failure group. The expression levels of GRP78 and CHOP in liver tissues of Valsartan group were lower than those of liver failure group, and the differences were highly statistically significant (all P < 0.01). Conclusion Valsartan can significantly reduce liver inflammation and liver cell apoptosis in the process of acute liver failure by blocking renin-angiotensin system, and its protective effect may be related to the inhibition of endoplasmic reticulum stress.

9 Effect of allergic reaction induced by echinococcosis granulosa on intestinal mucosal barrier function

Objective To investigate the effect of allergic reaction induced by echinococcosis granulosa on intestinal mucosal barrier function. Methods Forty-five female BALB/c mice aged six to eight weeks with SPF grade and body weight of 18 to 22 g were divided into blank control group, echinococcosis granulosa model group and echinococcosis granulosa sensitization group by random number table method, with 15 mice in each group. Echinococcus granulosus microcapsules were intraperitoneally injected into mice of echinococcosis granulosa model group and echinococcosis granulosa sensitization group. Each mouse was inoculated with about 50 microcapsules to establish the hydatid disease mouse model. After feeding for three months, the echinococcosis granulosa sensitization group was injected with crude hydatid cyst fluid to induce allergic reaction, while the blank control group and the echinococcosis granulosa model group were injected with the same amount of normal saline. The contents of interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α) and phosphorylated nuclear factor of kappa B-p65 (p-NF-κBp65), diamine oxidase (DAO) and D-lactic acid in plasma and the apoptosis rate of intestinal mucosal epithelial cells were detected in intestinal tissue of mice in each group. Pathological sections were performed to evaluate the barrier function. Results Compared with echinococcosis granulosa model group, the contents of TNF-α, IL-6, p-NF-κBp65, and the contents of DAO and D-lactic acid in plasma were increased in echinococcosis granulosa sensitization group (P < 0.05). Compared with echinococcosis granulosa model group, intestinal mucosa of echinococcosis granulosa sensitization group was significantly atrophied, part of intestinal villi became shorter or broke off, more acute and chronic inflammatory cells were infiltrated in the interstitium, and intestinal mucosa epithelial cells apoptosis increased. Conclusion Allergic reaction induced by echinococcosis granulosa can cause intestinal mucosal injury and lead to intestinal mucosal barrier dysfunction.

14 Study on the fingerprint and antioxidant activity of Xwak Granules

Objective To establish the high performance liquid chromatography and thin layer chromatography fingerprints of Xwak Granules, simultaneously determine the contents of five index components, and preliminarily investriage the antioxidant activity of Xwak Granules. Methods High performance liquid chromatography was performed on InertSustain C18 column. The mobile phase was acetonitrile-0.2% phosphoric acid, and the column temperature was 30℃. The segmented detection wavelengths were 245, 260, 300, 320, 350 and 360 nm, the flow rate was 1 mL/min, and the injection volume was 10 μL. The similarity analysis was carried out by the “similarity evaluation system for traditional Chinese medicine chromatographic fingerprint (2004A edition)”. A method for simultaneous determination of five characteristic compounds was established. The antioxidant activity was investigated by thin layer chromatography-bioautoradiography method. Results The similarity of the fingerprints of ten different batches of Xwak Granules was higher than 0.811, 36 common peaks were identified. Five characteristic peaks were identified as gallic acid, 3,5-O-dicaffeoylquinic acid, isoquercitrin, 4,5-O-dicaffeoylquinic acid and rupestonic acid. The linear coefficients of Xwak Granules content determination method were more than 0.9981. The relative standard deviations of precision, reproducibility, stability and recovery of characteristic peaks were all lower than 3%. Meanwhile, the contents of these five index components were determined. Xwak Granules had certain scavenging effects on 2,2’-amino-bis (3-ethyl-benzothiazoline-6-sulfonic acid) (ABTS) and 1, 1-diphenyl-2-picrylhydrazyl (DPPH). Conclusion The established fingerprint method is accurate, efficient and reproducible. It can be used for the quality control of Xwak Granules and the content of five components can be determined simultaneously. Xwak Granules contain many components which can scavenging ABTS and DPPH free radicals and have certain antioxidant activity.

19 Discussion on active ingredients and targets of Shenfu Decoction in treating myocardial infarction based on network pharmacology and molecular docking technology

Objective To explore the potential components and mechanism of Shenfu Decoction in the treatment of myocardial infarction based on network pharmacology and molecular docking. Methods Traditional Chinese medicine systems pharmacology database and analysis platform was used to search the active components and targets of Shenfu Decoction. GeneCards database was used to search the corresponding targets of myocardial infarction. The intersection of active components of Shenfu Decoction and myocardial infarction target were selected to obtain the potential target of Shenfu Decoction in treating myocardial infarction. Protein gene interaction data were obtained by String database, and protein-protein interaction network was constructed by importing into Cytoscape software. Gene ontology function and Kyoto Encyclopedia of Genes and Genomes pathway enrichment analysis of the same target proteins of drugs and diseases were carried out by David database. Molecular docking was performed by using DockThor and Pymol software. Results Shenfu Decoction was mainly used to treat myocardial infarction through 22 active components and 22 signaling pathways acting on 78 targets (26 core targets). Target pathway enrichment results showed that Shenfu Decoction mainly acted on PI3K-Akt, MAPK, apoptosis, NF-κB, and VEGF signaling pathways, etc. The results of molecular docking showed that kaempferol, β-sitosterol, stigmasterol and frutinone A had good binding ability with AKT1, TNF, MAPK8, STAT3, JUN, IL-1β and RELA. Conclusion Shenfu Decoction may play an anti-myocardial infarction by inhibiting apoptosis, promoting angiogenesis and reducing inflammation.

25 Research on knowledge-capacity model of cadre’s public health emergency management

Objective To discuss the knowledge-capacity model of cadres’ public health emergency management. Methods A sample of cadres from agencies affiliated to National Health Commission and the public health management departments of six provinces (province-level municipalities) in China were used in this study. By proportion sampling according to administrative level and post category of cadres to obtain data, this study aims to construct a reasonable and operable education and training curriculum model for Cadre’s Public Health Emergency Management. Results The Knowledge-capacity model of cadre’s public health emergency management was established. Knowledge-capacity training needs of cadres with different administrative levels and in different job categories were analyzed. A quantitative administrative level-oriented and job category-oriented training curriculum was designed. Conclusion Emergency response, monitoring and early warning, and preparedness are the common short-boards that cadres with different administrative levels and in different job categories need to make up. At the same time, when set up relevant courses, we should pay attention to need varies of cadres with different administrative levels and in different job categories. This study provides a quantitative education and training curriculum for knowledge and capacity model of cadre’s public health emergency management, which can be used as an applicable and targeted training option.

30 Molecular Study of plasmid-mediated NDM-1-producing Enterobacteriaceae

Objective To analyze the homology of 29 New Delhi Meta-β-lactamase 1 (NDM-1)-producing Enterobacteriaceae strains, and to understand the prevalence of NDM-1 in Zhejiang Province, and the surrounding environment of blaNDM-1 gene was elucidated by sequence analysis. Methods The drug resistance genes of 29 NDM-1-producing Enterobacteriaceae strains isolated from Zhejiang Hospital were amplified by PCR; the plasmid types were analyzed based on PCR-based plasmid replicon typing (PBRT), and the surrounding sequence of blaNDM-1 gene was amplified by PCR to analyze the similarities and differences of the surrounding sequence. Results Among the 29 blaNDM-1-carrying strains, 86.9% (26/29) were associated with extended spectrum beta-lactamases or other carbapenemase resistance genes; PFGE results showed that six strains of Enterobacter aerogenes, five strains of Enterobacter cloacae, two strains of Escherichia coli and two strains of Klebsiella oxytoca had homology, respectively. Multilocus sequence typing found that three strains of Escherichia coli were ST167 and five strains of Enterobacter cloacae were ST114. The conjugation experiments showed that 27 strains were successfully conjugated; 16 strains were found to be IncX3 type by PBRT plasmid typing, and 16 strains were found to carry blaNDM-1-bleMBL-trpF gene fragment by analyzing the surrounding environment of blaNDM-1. Conclusion NDM-1 has become an important cause of carbapenem antibiotic resistance in Zhejiang Province. IncX3 plasmids are widely spread among different bacterial species, and blaNDM-1-bleMBL-trpF gene fragments are widely disseminated in NDM-1-producing strains.

34 Analysis of KRAS mutation in patients with a single-center non-small cell lung cancer

Objective To analyze the mutation of KRAS gene in patients with single-center non-small cell lung cancer (NSCLC). Methods Tumor tissue or peripheral blood samples were collected from 210 NSCLC patients diagnosed by pathology in Guangdong Nongken Central Hospital from December 2016 to October 2020. High-throughput sequencing technology was used to detect the mutation rate of KRAS gene exon, the distribution characteristics of mutation sites and the correlation with patient characteristics. Results KRAS mutations rate of 210 NSCLC patients was 13.8% (29 cases). Among these KRAS-mutated cases, codon 12 mutation was observed in 58.6% (17/29), codon G13 mutation in 10.3% (3/29), Q61 mutation in 27.6% (8/29) individuals. Most (24.1%, 7/29) of the cases with Q61 mutations were Q61L. KRAS mutations were associated with the gender (P < 0.05), but had no correlation with smoking history and sample types (P > 0.05). Smoking history was correlated with codons G12/13 mutations (P < 0.05), but had no correlation with Q61L mutation (P > 0.05). KRAS mutations were associated with the gender either in tissue samples or in plasma samples (P < 0.05), but had no correlation with smoking history (P > 0.05). Conclusion The mutation frequency of KRAS in NSCLC patients is similar to the frequency reported in East Asian, but the frequency of KRAS codon Q61L mutation is significantly higher than that reported in the literature, suggesting that KRAS Q61L may be an important driver mutation in NSCLC patients from the single center in the current study.

38 Expression of FOXJ1 and SATB1 in triple negative breast cancer and its clinical significance

Objective To investigate the relationship between the expression of forkhead box J1 (FOXJ1) and special AT rich sequence binding protein 1 (SATB1) and clinicopathological parameters and prognosis of triple negative breast cancer. Methods A total of 98 patients with triple negative breast cancer admitted to the Third Hospital of Nanchang from February 2014 to March 2015 were selected as study subjects. Cancer tissues and paracancerous tissues were collected for use. The expression of FOXJ1 and SATB1 and their relationship with clinicopathological parameters were analyzed. The difference of five-year overall survival rate among patients with different FOXJ1, SATB1 expression were analyzed by Kaplan-Meier survival curve. Results The positive rate of FOXJ1 in triple negative breast cancer tissues was lower than that in paracancerous tissues (P < 0.01); and the positive rate of SATB1 was higher than that of paracancerous tissues (P < 0.01). In triple negative breast cancer tissues, different FOXJ1 expression was correlated with TNM stage and tumor differentiation degree (P < 0.05 or P < 0.01); different SATB1 expression was associated with the depth of tumor invasion, TNM stage, and whether lymph node metastasis (P < 0.05 or P < 0.01). The expression of FOXJ1 was negatively correlated with the expression of SATB1 in triple negative breast cancer (r = -0.625, P < 0.05). The five-year overall survival rate of FOXJ1 positive patients was higher than that of negative patients, while the five-year overall survival rate of SATB1 positive patients was lower than that of negative patients (all P < 0.01). Conclusion The low expression of FOXJ1 and high expression of SATB1 in triple-negative breast cancer tissues are both involved in the occurrence and development of triple negative breast cancer, and are correlated with the prognosis of patients with triple negative breast cancer. Detection the expression of FOXJ1 and SATB1 are helpful for the prognosis evaluation of patients.

42 Correlation analysis between glucocorticoid receptor gene NR3C1 rs7701443, rs2963156 polymorphisms and cerebral small vessel disease

Objective To explore the correlation between the glucocorticoid receptor gene (NR3C1) rs7701443 and rs2963156 polymorphisms and cerebral small vessel disease (CSVD). Methods Using the first-generation sequencing method, patients from the outpatient and inpatient of the Emergency General Hospital from March 2013 to May 2018 were selected and divided into the CSVD group (200 patients) and the control group (100 patients) based on the presence or absence of CSVD diagnosed by cranial MRI. The distribution of NR3C1 rs7701443 and rs2963156 polymorphisms were analyzed. Results There was no statistically significant difference in the frequency of rs7701443 between the two groups (P > 0.05); after excluding the influence of CSVD risk factors such as hypertension, coronary heart disease and diabetes, the difference in the frequency of genotypes between the two groups was not statistically significant (P > 0.05). There was no statistically significant difference in the frequency of rs2963156 between the two groups (P > 0.05); after excluding the influence of CSVD risk factors such as hypertension, coronary heart disease, diabetes, etc., the difference in the frequency of genotypes between the two groups was not statistically significant (P > 0.05). Conclusion The polymorphisms of rs7701443 and rs2963156 of NR3C1 may not be related to the onset of CSVD.

46 HS-SPME-GC-MS combined analysis of volatile components in Guangdong Eryngium foetidum L.

Objective To analyze the volatile components of Guangdong Eryngium foetidum L. by headspace-solid-phase microextraction-gas chomatogragh-mass spectrometer (HS-SPME). Methods The volatile components of Guangdong Eryngium foetidum L. were analyzed by HS-SPME and Gas shomatogragh-mass spectrometer. The type of extraction head, equilibrium temperature, equilibrium time and extraction time were investigated by single factor experiments to determine the best conditions of SPME, and the relative content of each component was determined by area normalization method. Results Finally, the optimal conditions were determined as follows: polydimethylsiloxane extraction head, sample volume of 0.5 g, equilibrium at 170℃ for 30 minutes, extraction for 30 minutes, and desorption time for three minutes. Sixty samples of coriander powder were isolated and 55 of them were identified. Conclusion The headspace-solid-phase microextraction method was successfully used to analyze the volatile components of Guangdong Eryngium foetidum L..

52 Research progress of spinal cord injury related signaling pathways in the repair of spinal cord injury

Spinal cord injury (SCI) is very common in clinical work, and its incidence has been increasing year by year in recent years. Due to the non-renderability of neurons after injury, the effect of active treatment for SCI patients is still very limited even through various means. This causes physical, psychological and emotional harm, as well as a significant economic burden on the family and society. With the in-depth study of various signaling pathways, it has been found that they all play an important regulatory role in the process of nerve injury and repair, especially play an important role in the proliferation, differentiation and metabolism of neural stem cells. This provides a new direction for SCI repair. Among them, the molecular mechanisms of Wnt, Notch, TGF-β/Smads and mTOR signaling pathways in SCI and the repair process are currently the focus of research. In this paper, the research achievements and progress of SCI related signaling pathways in SCI repair process are reviewed.

56 Research progress of acid sphingomyelinase and hepatorenal diseases

In this paper, the role of acid sphingomyelinase in the occurrence and development of liver related diseases such as hepatocyte apoptosis, fatty liver disease, liver fibrosis, liver cirrhosis and liver tumor, as well as its relationship with the occurrence and development of kidney related diseases such as diabetic nephropathy, glomerular disease and cadmium-induced renal damage were reviewed. It was found that acid sphingomyelinase can regulate the metabolic process of sphingolipids by activating a large amount production of ceramides, and thus plays an important role in the development of liver and kidney related diseases. Inhibition of the activity and expression of acid sphingomyelinase is of great importance for further research on the pathogenesis of diseases, prevention and control of liver and kidney related diseases, and slowing down the progression of diseases.

61 Research progress of continuous nursing care of stroke patients with dysphagia

To carry out effective continuous nursing for stroke patients with dysphagia, so that they can continue to obtain professional and individual nursing services. It can effectively improve patients’ life satisfaction and quality of life, improve the clinical outcome of them, and reduce the readmission rate. This paper reviews the research progress of continuous nursing care for stroke patients with dysphagia from the aspects of the concept and development of continuous care, related outcomes of stroke patients with dysphagia, content of continuous nursing care intervention for stroke patients with dysphagia, domestic application effects, challenges facing and so on. It is expected to provide reference for clinical nursing intervention and improve the public’s understanding of the continuous nursing care of stroke patients with dysphagia.

65 Construction and practice of traditional Chinese medicine chemistry on “golden course” oriented by cultivating innovative thinking and social service ability

The course of traditional Chinese medicine chemistry is a compulsory course for higher education majors and colleges of Chinese pharmacy, and it is also a traditional advantage course of Heilongjiang University of Chinese Medicine. According to the current goals and requirements of “golden course” construction of the Ministry of Education, the curriculum team has taken the lead in putting forward the goal of cultivating traditional Chinese medicine chemistry in Heilongjiang University of Chinese Medicine, which is oriented by cultivating students’ innovative thinking and social service capabilities in recent years. After several generations of nearly 50 years of construction and development, the curriculum construction concept of “centering on talent training objectives, integrating innovative teaching contents, highlighting the student main body status, teaching students in accordance with their aptitude, rationally and effectively using modern teaching methods and educational concepts, and establishing diversified, scientific and effective examination and evaluation mechanisms” has been gradually formed and improved. After years of continuous summary and optimization of a large number of classroom teaching practices, a suitable construction method and train of thought for the “gold course” of traditional Chinese medicine chemistry has been explored. The construction of this course can provide some reference for the construction of other courses related to Chinese pharmacy.

The Diagnostics Teaching and Research Section of Bengbu Medical College makes full use of the convenient features of Internet resources, organically integrates various free resources, and combines the characteristics of diagnostics disciplines to construct a new type of medical diagnostics network teaching method interspersed with “teaching”, “learning” and “evaluation”. And through the real-time sharing of teaching resources, the immediacy of interactive communication, the intelligence of daily homework, the convenience of review and testing, and the rationalization of evaluation mechanisms, this article will further explore diagnostics teaching under the integration of mobile Internet resources.

74 Meta-analysis on the efficacy and safety of Fluoxetine in the treatment of post-stroke depression

Objective To systematically evaluate the efficacy and safety of Fluoxetine in the treatment of post-stroke depression (PSD). Methods Literatures related to Fluoxetine in the treatment of PSD from PubMed, The Cochrane Library, CBM, CNKI, Wanfang Data and VIP were searched by computer. The retrieval time limit is from inception to September 2020. The literatures were selected according to the inclusion and exclusion criterias, the data was extracted and the literature quality was evaluated. RevMan 5.3 software was used for meta-analysis. Results A total of 763 patients were included in nine clinical randomized controlled studies. Meta-analysis results showed that the score of Hamilton depression scale in the Fluoxetine group was lower than that in the control group (SMD = -4.11, 95%CI [-5.31, -2.91], P < 0.000 01). The score of national institute of health stroke scale in the Fluoxetine group was lower than that in the control group (SMD = -1.50, 95%CI [-2.41, -0.59], P = 0.001). There was no significant difference in the incidence of adverse reactions between Fluoxetine group and control group (RR = 2.11, 95%CI [0.05, 92.23], P = 0.70). Conclusion Fluoxetine can improve the depressive symptoms and neurological deficits in PSD patients without increasing the adverse reactions. However, whether this drug can be widely used in clinic still needs to carry out more high-quality studies to further prove the clinical efficacy.

78 Analysis of detection rate and the affected population characteristics of esophageal related diseases under painless and common electronic gastroscope

Objective To investigate the difference of detection rate of esophageal related diseases between painless and common electronic gastroscope and the gender and age characteristics of the affected population. Methods The data of 8643 adult electronic gastroscopes in the Endoscopy Center of Xiyuan Hospital, China Academy of Chinese Medical Sciences from November 2014 to November 2019 were retrospectively analyzed, they were grouped in different ways. According to the examination methods, they were divided into the painless group (4012 cases) and the ordinary group (4631 cases), and the detection of esophageal related diseases in the two groups were compared. According to gender, they were divided into male group (3610 cases) and female group (5033 cases). According to age, the patients were divided into young group (18-44 years old, 2916 cases), middle ege group (45-59 years old, 3445 cases) and old group (≥60 years old, 2282 cases). The characteristics of the patients were analyzed. Results The detection rates of oropharyngeal lesions, short esophagus, stomach esophagus mucosa ectopic, papillary tumor in painless group were higher than those in ordinary group, the detection rates of candida esophagitis, hiatal hernia, esophageal cancer, cardiac relaxation loss, cardiac relaxation, poor cardiac dilatation and cardia inflammatory change were lower than those ordinary group, the differences were statistically significant (all P < 0.05). The detection rates of oropharyngeal lesions, reflux esophagitis, candida esophagitis, hiatal hernia, stomach esophagus mucosa ectopic, early esophageal cancer, esophageal cancer and cardiac relaxation in the male group were higher than those in the female group, and the detection rate of short esophagus in the female group was higher than that in the male group, with statistical significance (all P < 0.05). The detection rates of oropharyngeal lesions, reflux esophagitis, candida esophagitis, papillary tumor and cardiac relaxation in the young and old groups were all lower than those in the middle age group, with statistical significance (all P < 0.017). The detection rates of short esophagus, hiatal hernia, stomach esophagus mucosa ectopic, early esophageal cancer, poor cardiac dilatation, and cardia inflammatory in the young group were lower than those in the middle age group and the old group, the detection rates of stomach esophagus mucosa ectopic, cardia inflammatory in the young group were lower than those in the middle age group, and higher than those in the old group. And the detection rates of ecotopia of short esophagus, stomach esophagus mucosa ectopic and cardia inflammatory in the old group were lower than those in the middle age group, with statistical significance (all P < 0.017). The detection rates of esophageal cancer and cardiac relaxation loss in the young group were lower than those in the old group, and the differences were statistically significant (all P < 0.017). Conclusion The two methods of gastroscopy have their own advantages and disadvantages in the detection of esophageal related diseases, and the endoscopy method that conforms to the principle of cost-valence should be selected from the perspective of safety and economics according to the disease characteristics, gender and age characteristics of the patients.

82 Efficacy of intranasal high flow oxygen therapy for type Ⅰ respiratory failure and type Ⅱ respiratory failure with no obvious carbon dioxide retention

Objective To observe the curative effect of intranasal high flow oxygen therapy for type Ⅰ failure and Ⅱ respiratory failure with no obvious carbon dioxide retention. Methods A total of 60 patients with type Ⅰ failure and Ⅱ respiratory failure with no obvious carbon dioxide retention were selected as the research subjects from July 2019 to March 2020 in the Department of Respiratory Medicine of the Changshu NO.2 People’s Hospital, Jiangsu Province, according to numerical odd-even method, they were divided into control group and observation group, with 30 cases in each group. The control group was treated with non-invasive positive pressure ventilation, and the observation group was treated with transnasal high-flow oxygen therapy. The effective rate of treatment was compared between the two groups, blood gas analysis indexes, hemodynamic indexes, comfort score and Borg score were recorded before and after treatment, and the incidence of adverse events during treatment was observed between the two groups. Results The total effective rate of observation group was higher than control group, and the difference was statistically significant (P < 0.05). After treatment, the levels of oxygen saturation (SpO2) and oxygen partial pressure (SaO2) in the two groups were significantly higher than before treatment, the level of carbon dioxide partial pressure (PaCO2) was lower than before treatment, and after treatment, the levels of PaO2 and SaO2 in the observation group were higher than the control group, while the levels of PaCO2 was lower than the control group, with statistical significance (P < 0.05). After treatment, heart rate (HR), respiratory rate (RR) and pulmonary artery pressure (PAP) in the two groups were lower than before theatment, and the observation group was lower than the control group, the differences were statistically significant (P < 0.05). After treatment, the comfort scores of the two groups were higher than before treatment, the Borg scores were lower than before treatment, and the comfort score of the observation group was higher than the control group, and the Borg score was lower than the control group, and the differences were statistically significant (P < 0.05). The incidence of adverse events in observation group was lower than that in control group, and the difference was statistically significant (P < 0.05). Conclusion Transnasal high-flow oxygen therapy can significantly improve the clinical symptoms and oxygen status for typeⅠfailure and Ⅱ respiratory failure with no obvious carbon dioxide retention, and improve the comfort level and reduce the occurrence of adverse events.

86 Relationship between intestinal flora disorder and Th17/Treg balance and related cytokines in patients with ulcerative colitis

Objective To investigate the relationship between intestinal flora disorder and helper T cell 17 (Th17)/regulatory T cell (Treg) balance and related cytokines in patients with ulcerative colitis (UC). Methods From July 2018 to July 2020, 30 patients with active UC (AUC) admitted to Shuguang Hospital, Shanghai University of Traditional Chinese Medicine (hereinafter referred to as “our hospital”) were selected as the AUC group, and 30 patients with remission UC (RUC) in our hospital were selected as the RUC group with a ratio of 1∶1. Another 30 healthy subjects who came to our hospital for physical examination during the same period were selected as the control group. Real-time quantitative polymerase chain reaction was used to detect the number of intestinal flora in the three groups, the percentage of Th17 and Treg cells in peripheral blood was determined by flow cytometry, and the expression of inflammatory factors in colonic mucosa was determined by immunohistochemistry. Pearson correlation was used to analyze the correlation between the expression levels of Th17, Treg and inflammatory factors and the number of intestinal flora. Results The number of Enterococcus and Enterobacter, the proportion of Th17 cells in peripheral blood and the expression of interleukin (IL)-2, IL-6 and IL-17 in mucosa tissues in AUC group were significantly higher than those in RUC group and control group, and RUC group was higher than control group (P < 0.05); the number of Bacteroides, Lactobacillus and Bifidobacterium, the proportion of Treg cells in peripheral blood, the Th17/Treg ratio and the expression of IL-10 in mucosal tissues in AUC group were significantly lower than those in RUC group and control group, and RUC group was lower than control group (P < 0.05). Pearson correlation analysis showed that Th17, IL-2, IL-6 and IL-17 were positively correlated with Enterococcus and Enterobacter, it was negatively correlated with Bacteroidetes, Lactobacillus and Bifidobacterium. Treg and IL-10 were negatively correlated with Enterococcus and Enterobacterium, and positively correlated with Bacteroidetes, Lactobacillus and Bifidobacterium (all P < 0.05). Conclusion The disturbance of intestinal flora may affect the immune balance of Th17/Treg through various mechanisms, regulate the secretion of inflammatory mediators, and participate in the occurrence and development of UC.

91 Risk factors of diagnostic delay of pulmonary thromboembolism in hospitalized patients at department of respiratory medicine

Objective To explore the risk factors of diagnostic delay of pulmonary thromboembolism (PTE) in hospitalized patients at department of respiratory medicine. Methods Clinical data of 98 inpatients with PTE initially diagnosed in the Department of Respiratory Medicine, Beijing Tongren Hospital from December 2011 to December 2019 were collected, and they were divided into delayed diagnosis group (39 cases) and non-delayed diagnosis group (59 cases) according to the median time of PTE diagnosis. The difference of clinical data between the two groups was analyzed, and the risk factors of delayed PTE diagnosis were analyzed by logistic regression. Results The rates of basic pulmonary disease, chronic obstructive pulmonary disease (COPD) and asthma in the delayed diagnosis group were higher than those in the non-delayed diagnosis group, and the differences were all statistically significant (all P < 0.05). The proportion of patients with surgical history in the last one month was lower than that in the non-delayed diagnosis group, and the difference was statistically significant (P < 0.05). The incidence of hemoptysis, syncopal and lower limb edema, D-dimer value, simplified wells score and revised Geneva score in the delayed diagnosis group were lower than those in the non-delayed diagnosis group, with statistical significance (all P < 0.05). The incidence of dyspnea and fever were higher than that in the non-delayed diagnosis group, and the differences were all statistically significant (all P < 0.05). Pulmonary underlying disease (OR = 13.566, 95%CI: 2.410, 76.373) and COPD (OR = 7.030, 95%CI: 1.265, 39.065) were risk factors for delayed diagnosis of PTE (P < 0.05). Conclusion There is a high prevalence in diagnostic delay of PTE in hospitalized patients at department of respiratory medicine. COPD is independently and significantly related to the diagnostic delay of PTE.

95 Comparison on effect of different treatment measures in prehospital emergency of supraventricular tachycardia

Objective To compare the application effect of different treatment measures in prehospital emergency of supraventricular tachycardia (SVT). Methods The data of 150 patients with SVT who received prehospital emergency in the Department of Emergency, Beijing Emergency Center from March 2019 to March 2020 were retrospectively analyzed, according to different treatment measures, they were divided into traditional physical group (39 cases), drug intervention group (56 cases), synchronous cardioversion group (14 cases) and modified Valsalva maneuver group (14 cases). The effect of cardioversion in prehospital emergency and the incidence of adverse reactions were compared among the four groups. Results The success rates of one-time cardioversion and total cardioversion in drug intervention group, synchronous cardioversion group and modified Valsalva maneuver group were higher than those in traditional physical group, and the differences were statistically all significant (all P < 0.008); there was no statistical significant difference in the success rate of one-time cardioversion and the total cardioversion among drug intervention group, synchronous cardioversion group and modified Valsalva maneuver group (P > 0.008); the total incidence of adverse reactions in traditional physical group and modified Valsalva maneuver group were lower than those in drug intervention group and synchronous cardioversion group, and the differences were statistically significant (P < 0.008); there was no statistical significant difference in the total incidence of adverse reactions between the modified Valsalva group and the traditional physical group (P > 0.008). Conclusion Compared with the traditional physical intervention, the effects of drug intervention, synchronous cardioversion and modified Valsalva maneuver cardioversion in prehospital emergency of SVT are better, and compared with drug intervention and synchronous cardioversion, the modified Valsalva maneuver has higher safety.

99 Application effect of Dexmedetomidine in pediatric liver surgery

Objective To investigate the application effect of dexmedetomidine in pediatric liver surgery. Methods A total of 60 children who underwent liver lumpectomy at Hu’nan Children’s Hospital from July 2018 to June 2020 were selected. According to random number table method, they were divided into group D and group P, with 30 patients in each group. Group D was treated with Dexmedetomidine, and group P was treated with Propofol. The level of hemodynamic indexes, regional cerebral oxygen saturation (rSCO2) and serum contents of brain-derived neurotrophic factor (BDNF), S-100β, interleukin-10 (IL-10), tumor necrosis factor-α (TNF-α) and superoxide dismutase (SOD) were compared between the two groups. Results There were statistically significant differences in heart rate (HR) between the two groups and mean arterial pressure (MAP) of group D at each time point (all P < 0.05). The differences of the MAP at T1, T2, T3, T4 time points in group P compared with T0, with the statistically significant (all P < 0.05). The HR of group D at T1, T2, T3, T4 time points were lower than those of group P at the same time point, the MAP of group D at T2, T3, T4 time points were lower than those of group P at the same time point, SpO2 in group D were higher than those in group P at T2, T3 time points, and the differences were statistically significant (all P < 0.05). LrSCO2 in group D at time point T1 was higher than that at T0, and at time point T3 was lower than that at T1, LrSCO2 in group P at time points T2 and T3 were lower than those at T0 and T1, and at time point T4 was higher than that at T3, the differences were statistically significant (all P < 0.05). RrSCO2 in group D at time point T1 was higher than that at T0, and at time point T2, T3 were lower than those at time point T1, RrSCO2 in group P at time points T2 and T3 were lower than those at T0 and T1, and at time point T4 was higher than that at T2, T3, the differences were statistically significant(all P < 0.05). The LrSCO2 and RrSCO2 of group D at time points T1, T2 and T3 were higher than those of group P at the same time points, and the differences were statistically significant (all P < 0.05). There were statistically significant differences in BDNF levels between the two groups at each time point (all P < 0.05). There were no significant differences in S-100β level at T2 and T3, T3 and T4 time points in group D (P > 0.05), but there were significant differences at other time points in group D (P < 0.05). The levels of S-100β in group P at time points T2, T3, T4 were higher than those at time point T0, T1, and at time point T4 was higher than that at time points T2, T3, with statistical significance (all P < 0.05). The levels of BDNF at T2, T3, T4 in group D were higher than those in group P at the same time point, and the levels of S-100β were lower than those in group P at the same time points, with statistical significance (all P < 0.05). There were significant differences in the time points of IL-10, TNF-α and SOD between the two groups (all P < 0.05). The levels of IL-10 at time points T2, T3, T4 in group D were lower than those in group P at the same time points, the levels of TNF-α at time points T1, T2, T3 in group D were lower than those in group P at the same time points, and the levels of SOD at T3, T4 in group D were lower than those in group P at the same time points, with statistical significance (all P < 0.05). Conclusion The application of Dexmedetomidine in pediatric liver surgery can stabilize hemodynamics, improve local cerebral oxygen saturation, and reduce inflammatory response and oxidative stress response.

104 Effects of ultrasound-guided bilateral TAP block on hemodynamics, inflammatory response and postoperative analgesia in patients undergoing upper abdominal surgery

Objective To investigate the effect of ultrasound-guided bilateral transverse abdominalmuscle (TAP) block on hemodynamics, inflammatory response and postoperative analgesia in patients undergoing upper abdominal surgery. Methods Sixty patients who underwent upper abdominal surgery admitted to Nanjing Gaochun People’s Hospital of Nanjing from September 2017 to October 2019 were selected. They were divided into observation group and control group by random number table method, with 30 cases in each group. Under general anesthesia, the observation group was injected with 0.25% Ropivacaine 20 mL bilateral in TAP under the guidance of ultrasound, while the control group was injected with the same amount of normal saline. Upper abdominal surgery was performed in both groups 30 min after injection. Clinical indicators were compared between the two groups. The hemodynamic indexes of the two groups were compared at each time point during the surgery. Resting and moving visual analogue scores (VAS) before surgery and 4, 12, 24 h after surgery were compared between the two groups. The levels of inflammatory factors before surgery and 1, 3 d after surgery were compared between the two groups. The incidence of adverse reactions was compared between the two groups. Results The changes of blood pressure and heart rate (HR) in the observation group were lower than those in the control group, and the postoperative exhaust time and the time to first get ont of bed in the observation group were shorter than those in the control group, with statistical significance (all P < 0.05). HR and mean arterial pressure (MAP) at T1 and T2 were lower than T0 in the two groups, and HR at T2 was lower than T1 and MAP was higher than T1 in the two groups. HR at T1 and T2 in the observation group was lower than the control group at the same time point, and MAP was higher than the control group at the same time points, with statistical significance (all P < 0.05). The resting and moving VAS 12, 24 h after surgery in both groups were lower than 4 h after surgery, and 24 h after surgery was lower than 12 h after surgery. The resting and moving VAS of the observation group at 12, 24 h after surgery were lower than those of the control group at the same time points, and the differences were statistically significant (all P < 0.05). The levels of interleukin-6 (IL-6), C-reactive protein (CRP) and tumor necrosis factor-α (TNF-α) in both groups at 1, 3 d after surgery were higher than those before surgery, and the levels of IL-6, CRP and TNF-α in three days after surgery were lower than those on one day after surgery. The levels of IL-6, CRP and TNF-α in observation group at 1, 3 d after surgery were higher than those in control group at the same time points, and the differences were statistically significant (all P < 0.05). There was no significant difference in the total incidence of adverse reactions between the two groups (P > 0.05). Conclusion Ultrasound guided bilateral TAP block can effectively meet the postoperative analgesia needs of upper abdominal patients, reduce the pain of patients, change hemodynamics, and reduce the release of inflammatory factors, which is relatively safe and reliable.

109 Analysis on prescription rules of enema prescription in the treatment of ulcerative colitis based on data mining

Objective To explore the prescription rules of traditional Chinese medicine enema in the treatment of ulcerative colitis (UC). Methods Clinical research literatures of traditional Chinese medicine enema in the treatment of UC published from the establishment of the database to October 2020 were searched from CNKI and Wanfang database by computer. Traditional Chinese medicine inheritance assistant system software (V2.5) was used to mine the data of meridian tropism and frequency of drug flavor in literature prescriptions. Association rules, complex system entropy clustering and other methods were used to analyze the rule of prescription. Results A total of 322 relevant prescriptions were included, involving 170 Chinese herbs, among which, cold and warm medicinal herbs, bitter and sweet medicinal herbs were accounted for a larger proportion. In the meridian tropism, liver, stomach, large intestinal, spleen meridians were the ones with a higher frequency of meridian. Twenty-four groups of high-frequency drug combinations (frequency greater than 50 times), 10 drug association rules and 7 potential new prescriptions were excavated. Conclusion Traditional Chinese medicine enema treatment of UC includes heat-clearing and damp-drying, and regulating qi and blood. Treating from liver and taking care of the spleen and stomach is the important ideas for treating UC.

Objective To observe the effect of Wuling Powder combined with Wupi Decoction for internal and external use combined with free hand massage and external application of traditional Chinese medicine in the treatment of lower limb lymphedema after radical operation of gynecological cancer. Methods From May 2015 to May 2018, 85 patients with lymphedema after radical surgery for gynecological malignant tumors (cervical cancer, endometrial cancer, ovarian cancer, etc.) treated with Affiliated Hospital of Shaanxi University of Chinese Medicine were selected and divided into control group (42 cases) and treatment group (43 cases) according to random number table method. Control group was treated with free hand massage and external application of traditional Chinese medicine, while treatment group was treated with Wuling Powder plus Wupi Decoction and rectal drip. Three weeks was a course of treatment. Two groups were treated for two courses, and the curative effect was evaluated after two courses. The curative effect, limb circumference and one year follow-up recurrence of two groups were compared. Results Clinical effect of treatment group was better than that of control group, and the difference was statistically significant (P < 0.05). After treatment, the circumferences of the middle leg, ankle and middle thigh of affected limb in two groups were shorter than those before treatment, and treatment group was shorter than that control group, the differences were statistically significant (P < 0.05). One year follow-up recurrence rate of treatment group was lower than that of control group, the difference was statistically significant (P < 0.05). Conclusion Wuling Powder combined with Wupi Decoction and rectal drip combined with free hand massage, external application of traditional Chinese medicine in the treatment of lower limb lymphedema after gynecological cancer radical operation has definite effect.

Obstructive sleep apnea syndrome and chronic kidney disease have a two-way effect, which can induce each other. According to many years of clinical experience, Professor Li Guangxi believes that the two diseases can be classified as metabolic disorders of qi and body fluid. If yang does not change into qi, then water dampness (phlegm) condenses, and water (phlegm) stops, then yang qi is blocked. That is, yang qi deficiency of lung, spleen and kidney, phlegm and water dampness are the main pathogenesis characteristics, traditional Chinese medicine should be based on warming yang, benefiting qi and resolving phlegm. Among them, warming yang includes warming and tonifying spleen yang, strengthening and protecting kidney yang. Benefiting qi is divided into promoting lung qi, tonifying spleen qi, and absorbing kidney qi. Resolving phlegm is actually eliminating lung phlegm, drying and resolving spleen drink, and dividing kidney water. According to the characteristics of the disease and the characteristics of pathogenesis, it can be used to achieve good results. This article analyzes the etiology of non-alcoholic fatty liver disease through the constitutional medical thought in the Neijing, and it is found that constitution plays a very important role in both innate and acquired factors. The innate factors are based on genetic susceptibility, genetic mutations and chromosomal abnormalities. The acquired factors include geographic environment, diet and nutrition, mental status, age differences, gender factors, social factors, work and rest conditions, disease influence, acupuncture and medicine effects, etc. In terms of pathogenesis, constitution not only determines the onset of non-alcoholic fatty liver disease, but also affects the development and prognosis after the onset. This paper discusses the role of constitution in the etiology and pathogenesis of nonalcoholic fatty liver disease, which has a certain theoretical and clinical guiding value to enrich and expand the prevention and treatment of nonalcoholic fatty liver disease.

Conventional methods, such as conventional surgery, radiochemotherapy and molecular targeted therapy, still have great limitations. Through years of clinical observation and practice, Professor Lao Changhui concludes that the key pathogenesis of occurrence and metastasis of lung adenocarcinoma is “cold-dampness due to kidney deficiency”, and around this pathogenesis, he puts forward the basic treatment of “cultivating yuan, tonifying kidney, dispelling yin and cold” throughout the treatment process. With the intervention of course of disease and treatment, cancerous accumulation may turn toxic and heat, leading to the changes of pathogenesis of deficiency of both qi and yin and deficiency of both yin and yang. Therefore, “cultivating yuan, tonifying kidney, dispelling yin and cold” should be used throughout the treatment process. At the same time, according to the changes of course of disease and syndrome, it should be added and subtracted with syndrome. On the premise of taking care of healthy qi, it can be supplemented with less specialized anti-cancer drugs. This reflects the advantages of traditional Chinese medicine thinking of “people oriented, syndrome differentiation and treatment”.

129 Analysis on the characteristics of menstrual period drug use in the treatment of infertility by Professor Xia Guicheng

Objective To understand Professor Xia Guicheng’s prescription drug use of traditional Chinese medicine decoction pieces in menstrual period in the treatment of infertility, and to explore and study the characteristics and rules of its drug use. Methods A total of 984 prescriptions of traditional Chinese medicine decoction pieces in menstrual period for infertility treated by Professor Xia Guicheng in Affiliated Hospital of Nanjing University of Chinese medicine from September 2016 to September 2017 were selected to conduct data sorting, and high-frequency drugs, high-frequency drug pairs, menstrual return and basic prescriptions in the prescriptions were analyzed to explore their characteristics of drug use. Results The single traditional Chinese medicine with the frequency of more than 10% included 20 herbs, such as Paeoniae Radix Rubra, Lycopi Herba, stir-baking with bran Atractylodis rhizoma, Salviae Miltiorrhizae; processing with vinegar Cyperi Rhizoma, Leonuri Herba and Poria. Among them, drugs for promoting blood circulation for removing blood stasis were used most frequently, followed by regulating qi-flowing drugs, heat-clearing drugs, eliminating dampness drugs and tonifying deficiency drugs. Twenty-six high-frequency drug pairs included Paeoniae Radix Rubra and Lycopi Herba, Paeoniae Radix Rubra and stir-baking with bran Atractylodis rhizoma, Paeoniae Radix Rubra and Salviae Miltiorrhizae, etc. Drugs belonging to the liver channel were used most. The basic formula was stir-baking with bran Atractylodis rhizoma, Paeoniae Radix Rubra, Dipsaci Radix, Salviae Miltiorrhizae, Poria, Cinnamomi Cortex, processing with vinegar Cyperi Rhizoma, Leonuri Herba, Lycopi Herba. Conclusion Professor Xia Guicheng often uses drugs such as promoting blood circulation for removing blood stasis, regulating qi-flowing, eliminating dampness and tonifying deficiency to treat infertility during menstrual period. Menstrual period treatment focuses on promoting blood circulation for regulating menstruation, tonifying the kidney and warming Yang, and activating qi flowing and eliminating dampness.

133 Effect of electric nape acupuncture combined with local acupuncture on central retinal artery occlusion

Objective To observe the effect of electric nape acupuncture combined with local acupuncture on central retinal artery occlusion (CRAO). Methods A total of 40 cases (eyes) of CRAO patients admitted to Second Affiliated Hospital of Heilongjiang University of Chinese Medicine from February 2018 to November 2020 were divided into control group and treatment group by random number table method, with 20 cases (eyes) in each group. The control group was given oral medicine, local acupuncture, symptomatic basic treatment; The treatment group was combined with electric nape acupuncture therapy on the basis of the control group, and the two groups were treated for eight weeks. The visual acuity, visual field and blood flow of the central retinal artery were evaluated before and eight weeks after treatment in both groups. Results Before treatment, there was no significant difference in the best corrected visual acuity between the two groups (P > 0.05). After treatment, the best corrected visual acuity in the two groups was higher than before treatment, and the treatment group was higher than the control group, with statistical significance (P < 0.05). Before treatment, there was no significant difference in visual field defect between the two groups (P > 0.05). After treatment, the visual field defect in the two groups was lower than before treatment, and the treatment group was lower than the control group, with statistical significance (P < 0.05). Before treatment, there were no significant differences in peak systolic flow velocity and end-diastolic flow velocity between the two groups (P > 0.05). After treatment, the peak systolic flow velocity and end-diastolic flow velocity in the two groups were higher than before treatment, and the treatment group was higher than the control group, with statistical significance (P < 0.05). Conclusion Electric nape acupuncture combined with local acupuncture can improve the visual acuity of patients with CRAO.

137 Predictive value of four-dimensional Doppler ultrasound in early pregnancy of fetal growth restriction

Objective To explore the predictive value of four-dimensional Doppler ultrasound in early pregnancy of fetal growth restriction (FGR). Methods A total of 367 normal single pregnant women who delivered in Tangshan Maternal and Child Health Hospital from March 2019 to March 2020 were selected as the research subjects. During nuchal translucency examination, placental energy Doppler volume data were collected by four-dimensional energy Doppler ultrasonography, and vascularization index (VI), vascularization-blood flow index (VFI) and blood flow index (FI) were calculated by GE 4D View software. All pregnant women were followed up by telephone until birth, according to the diagnosis results, they were devided into FGR group (59 cases) and control group (308 cases). Placental volume (PV), placental quotient (PQ), left uterine artery pulse index (PI), right uterine artery PI, umbilical artery systolic/diastolic velocity ratio (S/D), PI of middle cerebral artery and four-dimensional energy Doppler ultrasound parameters VI, VFI, FI were compared between two groups, and the pregnancy prognosis of two groups were observed. Results PV, PQ and PI of middle cerebral artery in FGR group were lower than those in control group, while left uterine artery PI, right uterine artery PI and S/D of umbilical artery in FGR group were higher than those in control group (all P < 0.01). VI, FI and VFI of FGR group were lower than those of control group (all P < 0.01). Apgar score of FGR group was lower than that of control group (P < 0.01); there were no significant differences in the incidence of fetal distress and neonatal asphyxia between two groups (P > 0.05). Conclusion Four-dimensional energy Doppler ultrasound detection in early pregnancy is of great value for the early prediction of FGR. VI and VFI indexes are sensitive indicators for the evaluation of FGR in early pregnancy, and have high clinical diagnostic value.

142 Predictive value of buffer coefficient in malignant middle cerebral artery infarction

Objective To investigate the predictive value of buffer coefficient in malignant middle cerebral artery infarction (MMI). Methods Clinical data of 131 patients with internal carotid or middle cerebral artery infarction treated to Yongchuan Hospital Affiliated to Chongqing Medical University from January 2017 to August 2020 were retrospectively analyzed. According to whether they had progressed of MMI, the patients were divided into malignant group (53 cases) and non-malignant group (78 cases). The clinical and imaging data of two groups were analyzed. Results Cerebral infarction volume and buffer coefficient in malignant group were larger than those in non-malignant group, while buffer volume was smaller than that in non-malignant group, and the differences were statistically significant(P < 0.05). Cerebral infarction volume and buffer coefficient were risk factors for MMI (OR > 1,P < 0.05), while buffer volume was a protective factor (OR < 1,P < 0.05). The area under curve predicted by buffer coefficient was 0.965, the cut-off value was 2.25, the sensitivity was 87%, and the specificity was 95%. Conclusion Buffer coefficient is better than the cerebral infarction volume or buffer volume to predict the occurrence of MMI, which has certain reference value for the judgment of the condition of middle cerebral artery infarction patients and the choice of treatment plan.

146 Application value of dual-source coronary CTA in the diagnosis of different grades of coronary stenosis in T2DM patients with coronary heart disease

Objective To research clinical application of dual-source coronary CT angiography (CTA) in the diagnosis of different grades of coronary stenosis in type 2 diabetes mellitus (T2DM) patients with coronary heart disease (CHD). Methods From April 2017 to April 2020, 96 patients with T2DM and CHD in General Hospital of Huainan Eastern Hospital Group were selected as the research objects. The result of dual-source coronary CTA and CAG in the diagnosis of different grades of coronary stenosis in patients with T2DM and CHD were compared, and the diagnostic efficiency of dual-source coronary CTA in the diagnosis of different grades of coronary stenosis in patients with T2DM and CHD were analyzed. Results There was no significant difference in between the two methods in the different grades of coronary stenosis (P > 0.05). The AUC of dual-source coronary CTA in the diagnosis of slight, mild, moderate and severe coronary stenosis were 0.823, 0.801, 0.794, 0.928; and the sensitivity were 80.00%, 77.78%, 83.33%, 95.24%, respectively; and the specificity were 99.37%, 98.41%, 98.37% and 99.38%, respectively. Conclusion Dual-source coronary CTA can accurately grade the degree of coronary stenosis in patients with T2DM and CHD. It has high diagnostic efficiency and is a safe, efficient and non-invasive examination method.

155 Application effect of optimizing the pre-hospital transport mode of critically ill newborns based on the information sharing system

Objective To explore the application effect of the pre-hospital transport model based on the optimization of the information sharing system in critically ill newborns. Methods The data of 94 critically ill newborns admitted to Hebei Children’s Hospital from February 2019 to February 2020 who implemented prehospital transfer based on the optimization of the information sharing system were retrospectively analyzed and recorded as group A, the data of 88 critically ill newborns undergoing routine transportation were retrospectively analyzed and recorded as group B. The time spent in the transfer process and the hospital stay after transfer, the severity of disease scores before transfer, during transfer and after transfer, the outcome and the occurrence of adverse events of newborns of the two groups were compared. Results The time spent in the transfer process and hospital stay after transfer in group A were shorter than those in group B (all P < 0.05). There were statistically significant differences in the severity of neonatal disease between the two groups, time and interaction (all P < 0.05). There was no significant difference in disease severity scores before, during and after transport in group A (P > 0.05). The disease severity score of group B in transport was lower than that before transport (P < 0.05), and that after transport was lower than that in transport, before transport (P < 0.05). The disease severity score of group A during and after transport was higher than that in group B (P < 0.05). The outcome of group A was better than that of group B (P < 0.05). The incidence of adverse events in group A was lower than that in group B (P < 0.05). Conclusion Optimizing the pre-hospital transport mode of critically ill newborns based on the information sharing system can shorten the time spent in the transport process and hospital stay after transfer, reduce the severity of neonatal disease, help the outcome of the disease and reduce the incidence of adverse events.

163 Effect of comprehensive nursing intervention based on the concept of MDT writing on the rehabilitation of patients with knee replacement

Objective To explore the comprehensive nursing intervention based on the concept of multidisciplinary temwork (MDT) on the rehabilitation of patients with knee replacement and clinical controlled research. Methods From January 2019 to April 2020, a total of 94 patients underwent artificial total knee arthroplasty (TKA) were selected in the Joint Surgery Department of the First People’s Hospital of Lianyungang City, Jiangsu Province. According to the random number method, they were divided into group A (comprehensive nursing after operation, 47 cases) and group B (MDT nursing on the basis of group A, 47 cases). The postoperative rehabilitation indicators, preoperative, 1, 7 d and 14 d postoperatively, American hospital of special surgery (HSS) score and visual analog scale (VAS), knee joint recovery index, postoperative quality of life score, and nursing satisfaction of two groups were observed. Results The number of days of hospitalization and the time of getting out of bed for the first time in group B were shorter than those in group A, while the hospitalization expenses were lower than those in group A, and the differences were statistically significant (P < 0.05). There were statistically significant differences in HSS scoring time, between groups and the interaction between the two groups (P < 0.05). Further pairwise comparisons showed that the HSS scores of the two groups were higher on 1, 7 d and 14 d after operation than before operation, and on 7 d and 14 d were higher than that on 1 d after operation, and the differences were statistically significant (P < 0.05). The HSS scores of group B were lower than those of group A at 7 d and 14 d after operation, and the differences were statistically significant (P < 0.05). There were statistically significant differences in the VAS time, between groups and the interaction between the two groups (P < 0.05). Further comparisons between the two groups showed that the VAS of the two groups were higher on the 7 d and 14 d after the operation than before and 1 d after the operation, and the differences were statistically significant (P < 0.05). The VAS of group B were higher than those of group A at 7 d and 14 d after operation, and the differences were statistically significant (P < 0.05). In the knee joint recovery index of group B, while the time required for knee flexion >90° was shorter than that of group A, and the differences were statistically significant (P < 0.05). The maximum flexion, range of motion and knee flexion in group B were greater than those in group A, and the differences were statistically significant (P < 0.05). The physiological function, physical pain, energy, social function, emotional title, mental health, health status and daily activities scores of group B were higher than those of group A, and the differences were statistically significant (P < 0.05). The nursing satisfaction of group B was high in group A, and the difference was statistically significant (P < 0.05). Conclusion The comprehensive intervention based on the MDT collaboration concept can improve the knee joint function of patients with postoperative OA, reduce pain especially at 7 and 14 d after surgery, effectively improve the quality of life and increase nursing satisfaction.

168 Meta-analysis of quantified hand exercises to prevent PICC-related thrombosis

Objective To evaluate the preventive effect of peripherally inserted central catheter (PICC)-related venous thrombosis by quantifying hand exercise. Methods CBM, CNKI, PubMed, CINAHL, Cochrane Library and Wanfang database were searched by computer, and the retrieval time was from the time of establishing database to October 31, 2019. All studies that quantify the prevention of PICC-related thrombosis in hand exercises had been collected. Two researchers independently carried out quality evaluation and data extraction. RevMan 5.3 software was used to conduct meta-analysis of the difference between quantified hand movement and regular fisting movement. Results A total of 12 articles that meet the standards were included. Meta-analysis results showed that the incidence of PICC-related venous thrombosis in the observation group was lower than that in the control group, and the difference was statistically significant (OR [95%CI] = 0.17 [0.10, 0.28], P < 0.000 01); the maximum blood flow velocity of the vein in the observation group was higher than that of the control group, and the difference was statistically significant (SMD [95%CI] = 1.00 [0.68, 1.31], P < 0.000 01); the average blood flow velocity per unit time in the observation group was greater than that of the control group, and the difference was statistically significant (SMD [95%CI] = 0.98 [0.79, 1.18], P < 0.000 01); the phlebitis infection rate of the observation group was lower than that of the control group, and the differences were statistically significant (OR [95%CI] = 0.36 [0.17, 0.76], P = 0.008). Conclusion Quantitative hand exercises can effectively reduce the occurrence of PICC-related thrombosis, improve venous blood flow velocity, and reduce the occurrence of catheter-related infections. However, in the future, large-sample, high-quality randomized controlled trials are still needed to further evaluate the effect of quantitative fisting exercises in PICC patients.

173 Effect of health education based on the concept of health quotient on the healthy behavior ability and blood sugar control level of patients with type 2 diabetes

Objective To explore the impact of health education based on the concept of health quotient on the health behavior and blood sugar control level of patients with type 2 diabetes. Methods A total of 106 patients with type 2 diabetes who were treated in the Sixth People’s Hospital of Nantong from January 2017 to December 2018 were selected. According to the treatment time, they were divided into the treatment group (January to December 2018) with 54 cases and the control group (January to December 2017) with 52 cases. Excluding the falling cases, the treatment group completed 52 cases and the control group completed 48 cases. The control group was given regular health education, while the treatment group was given health education guided by the concept of health quotient. Follow-up for six months, the health behavior ability, blood sugar control level, health education satisfaction of the two groups were compared. Results Follow-up for six months, the total scores of diet management, exercise, blood glucose monitoring, foot care scores, and health behavior ability of the two groups were higher than those of the same group before education, while the experimental group was higher than the control group, and the differences were statistically significant (P < 0.05). Follow-up for six months, the levels of fasting blood glucose, 2 h postprandial blood glucose, glycosylated hemoglobin in the two groups were lower than before education, while the experimental group was lower than the control group, and the differences were statistically significant (P < 0.05). The health education satisfaction of the experimental group was significantly higher than that of the control group, and the difference was statistically significant (P < 0.05). Conclusion Health education based on the concept of health quotient helps to promote the development of healthy behavior ability of patients with type 2 diabetes, improve blood sugar control level, and enhance patient satisfaction with health education.

177 Effect of family participatory health education in breastfeeding primiparous women

Objective To explore the effect of family participation health education on the subjective well-being of primipara and breastfeeding efficacy. Methods From April 2017 to March 2018, a total of 108 cases of primiparous women who were expected to give birth in the Nanjing Lishui People’s Hospital were selected, while they were divided into observation group of 55 cases and control group of 53 cases by random number table method. The control group was given regular health education, and the observation group was combined with family-participatory health education on the basis of the control group. Followed up for three months, the subjective well-being, breastfeeding efficacy and breastfeeding behavior of the two groups were compared. Results Follow-up for three months, the total scores for health worry, energy, melancholy and happy mood, relaxation and tension, and subjective well-being of the observation group were significantly higher than those of the control group, and the differences were statistically significant (P < 0.05). After three months of follow-up, there was no statistically significant difference in the depression and happiness scores between the control group and the same group before education (P > 0.05). Other subjective well-being scores were higher than those of the same group before education, and the differences were statistically significant (P < 0.05). After three months of follow-up, the subjective well-being scores of the observation group were higher than those before education, and the differences were statistically significant (P < 0.05). After three months of follow-up, the total scores of breastfeeding skills, inner activity and breastfeeding self-efficacy scale in the two groups were higher than those before education, while those of the observation group were higher than those of  the control group, and the differences were statistically significant (P < 0.05). The breastfeeding behavior of primiparas in the two groups was significantly different, and the difference was statistically significant (P < 0.05). Conclusion Family participatory health education helps to enhance the subjective well-being of primiparas, promote the development of breastfeeding efficiency, and increase the rate of breastfeeding behavior.

181 Effect of sleep quality intervention based on Roy adaptation model in patients with cerebrovascular disease interventional therapy

Objective To analyze the application effect of sleep quality intervention based on Roy adaptation model in patients with interventional therapy for cerebrovascular disease. Methods A total of 120 patients with interventional treatment of cerebrovascular disease in Nanjing First Hospital, Nanjing Medical University from April 2018 to June 2019 were selected. Using random number table method, they were divided into 60 cases in the treatment group and 60 cases in the control group. The control group was given routine nursing intervention, and the treatment group was combined with sleep quality intervention based on Roy adaptation model, the mental health symptom self-rating scale (SCL-90) and sleep quality Pittsburgh sleep quality index (PSQI) between the two groups were compared. Results After eliminating the invalid ones, 58 cases in the treatment group and 54 cases in the control group actually completed the test. At discharge, SCL-90 scores of the two groups were lower than those before intervention, somatization, depression, anxiety, sleep and diet, SCL-90 average scores of the treatment group were lower than those of the control group, and the differences were statistically significant (P < 0.05); the PSQI scores of the two groups were lower than those before treatment, and the subjective sleep quality, sleep time, sleep efficiency, daytime dysfunction and PSQI total scores of the treatment group were lower than those of the control group, with statistical significance (P < 0.05). Conclusion Nursing intervention based on Roy adaptation model can help improve the mental health and sleep quality of patients with cerebrovascular disease interventional therapy.

Objective To observe the application effect of kaiqiao liyan massage combined with tongue pressure resistance feedback training in rehabilitation nursing of patients with dysphagia after stroke (DAS). Methods A total of 100 patients with DAS from January 2019 to January 2020 treated in Qingdao Haici Medical Group  Qingdao Traditional Chinese Medicine Hospital  were prospectively selected, the random number table method was used to divide them into control group and observation group, with 50 cases in each group. The control group was given routine rehabilitation nursing+tongue pressure resistance feedback training; the observation group was given routine rehabilitation nursing+tongue pressure resistance feedback training+resuscitation and throat massage, both were intervened continuously for four weeks. The swallowing function (evaluated by Rosenbek penetration-aspiration scale [PAS]), degree score of dysphagia (VGF) and quality of life (evaluated by swallowing quality of life questionnaire [SWAL-QOL]) were compared between the two groups before and after four weeks of intervention, the incidence of complications during the intervention was statistically analyzed. Results After four weeks of intervention, the PAS score in both groups was decreased than that before intervention, the VGF score was increased than that before treatment, and the PAS score in the observation group was lower than the control group, and the VGF score was higher than the control group, with statistical significant differences (P < 0.05); after four weeks of intervention, the SWAL-QOL score of both groups were increased than that before intervention, the observation group was higher than the control group, with statistical significant differences (P < 0.05). There were no significant difference in the total incidence of complications between two group (P > 0.05). Conclusion Patients with DAS receiving kaiqiao liyan massage combined with tongue pressure resistance feedback training during rehabilitation nursing can improve the benefits of rehabilitation nursing in patients, the swallowing function and quality of life of patients are significantly improved, which is worthy of application.

189 Nursing effect of continuous improvement based on communication feedback in high-flux hemodialysis of patients with uremia

Objective To explore the intervention effect of continuous improvement based on communication feedback in high-flux hemodialysis (HFHD) of patients with uremia during treatment. Methods A total of 124 patients with uremia who received HFHD treatment in the Sixth Medical Center of PLA General Hospital were selected from January 2018 to January 2020, the random number table method was used to divide them into control group and observation group, with 62 cases in each group. The control group was received routine intervention, the observation group was received continuous improvement intervention based on communication feedback, both were intervened for 6 months. The psychological state of the two groups was evaluated and compared by self rating anxiety scale (SAS) and self-rating depression scale (SDS) before and six months after intervention. After six months of intervention, the satisfaction of the two groups was investigated by self-made satisfaction questionnaire; the skin itching degree of the two groups was evaluated and compared; the complications of the two groups were recorded. Results At six months of intervention, the SAS and SDS scores of both groups were decreased than those before intervention, and the observation group were lower than the control group (P < 0.05); the scores of all items in the questionnaire of nursing satisfaction of the observation group were higher than those of the control group (P < 0.05); the degree of skin itching in observation group was less than that in control group (P < 0.05); the incidence of allergy and bleeding in the observation group was lower than the control group (P < 0.05). Conclusion The intervention effect of continuous improvement based on communication feedback in high flow hemodialysis of patients with uremia during treatment has certain effect, which is helpful to improve the mental state of patients and reduce the itching degree of skin, and it is easy to be accept by patients, with high degree of satisfaction.

193 Application of quality control circle in improving the accuracy of activities of daily living assessment

Objective To observe the application effect of quality control circle activities in improving the accuracy of daily living activities (ADL) scores. Methods From May 2018 to April 2019, the quality control circle was established in Beijing Luhe Hospital Affiliated to Capital Medical University, among 615 inpatients in surgical wards, 292 cases were before improvement (without quality control circle) and 323 cases were after improvement (with quality control circle), the accuracy of ADL scoring before and after quality control circle activities, the effect of quality control circle activities in improving the accuracy of ADL scoring were analyzed. Results The accuracy of ADL score after quality control circle (91.3%) was higher than that before improvement (60.6%), and the difference was statistically significant (P < 0.05). The accuracy of ADL scores were higher than before except for stool control, and the differences were statistically significant (P < 0.05). Conclusion Quality control circle activities can further improve the accuracy of ADL score, worthy of promotion and application.
