Meta-analysis of quantified hand exercises to prevent PICC-related thrombosis |

Abstract Objective To evaluate the preventive effect of peripherally inserted central catheter (PICC)-related venous thrombosis by quantifying hand exercise. Methods CBM, CNKI, PubMed, CINAHL, Cochrane Library and Wanfang database were searched by computer, and the retrieval time was from the time of establishing database to October 31, 2019. All studies that quantify the prevention of PICC-related thrombosis in hand exercises had been collected. Two researchers independently carried out quality evaluation and data extraction. RevMan 5.3 software was used to conduct meta-analysis of the difference between quantified hand movement and regular fisting movement. Results A total of 12 articles that meet the standards were included. Meta-analysis results showed that the incidence of PICC-related venous thrombosis in the observation group was lower than that in the control group, and the difference was statistically significant (OR [95%CI] = 0.17 [0.10, 0.28], P < 0.000 01); the maximum blood flow velocity of the vein in the observation group was higher than that of the control group, and the difference was statistically significant (SMD [95%CI] = 1.00 [0.68, 1.31], P < 0.000 01); the average blood flow velocity per unit time in the observation group was greater than that of the control group, and the difference was statistically significant (SMD [95%CI] = 0.98 [0.79, 1.18], P < 0.000 01); the phlebitis infection rate of the observation group was lower than that of the control group, and the differences were statistically significant (OR [95%CI] = 0.36 [0.17, 0.76], P = 0.008). Conclusion Quantitative hand exercises can effectively reduce the occurrence of PICC-related thrombosis, improve venous blood flow velocity, and reduce the occurrence of catheter-related infections. However, in the future, large-sample, high-quality randomized controlled trials are still needed to further evaluate the effect of quantitative fisting exercises in PICC patients.
