SHUAI Bo1 CHEN Shenghao2 SHEN Lin1 YI Jianhua2 MA Chen1 YANG Yanping1 ZHAO Ying3 CAI Xun3

Objective To investigate the influence of renin angiotensin (RAS) biaxial on patients with ischemic necrosis of the femoral head (INFH). Methods From October 2015 to June 2017, 90 patients underwent femoral head replacement in the Department of Orthopedics in Union Hospital Affiliated to Tongji Medical College of Huazhong University of Science and Technology and Suizhou Central Hospital in Hubei Province were selected. Among them, 45 patients with INFH were served as INFH group, and 45 patients with postoperative traumatic femoral neck fracture were served as control group. The expressions of angiotensin converting enzyme (ACE), angiotensin Ⅱ (Ang Ⅱ), angiotensin Ⅱ-1 receptor (AT1R), ACE2, Ang (1-7), Mas receptor (Mas-R) in the circulating blood of two groups were detected before operation, and the expressions of ACE, AngⅡ, AT1R, ACE2, Mas-R, receptor activator of NF-κB ligand (RANKL) and osteoprotegerin (OPG) mRNA in local bone tissue of femoral head were detected after operation, and immunohistochemical analysis was performed. Results There were no significant differences in the expressions of ACE, AngⅡ, AT1R, ACE2, Ang(1-7) and Mas-R between two groups (P > 0.05). The mRNA expression levels of ACE2, Mas-R and OPG in local bone tissue of femoral head in INFH group were lower than those in control group (P < 0.05), while the mRNA expression levels of ACE, AngⅡ, AT1R, RANKL and RANKL mRNA/OPG mRNA were higher in INFH group than those in control group (P < 0.05). Compared with control group, HE staining of bone tissue in INFH group showed hyperplasia of adipocytes, thinning of bone trabeculae, pyknosis and deep staining of bone nucleus, and obvious increase of empty bone lacunae; Masson staining of bone tissue in INFH group showed that dark blue became lighter and more disordered, showing the decrease of collagen structure and obvious hyperplasia of fibrous structure. The results of immunohistochemistry showed that the expressions of ACE and AT1R were higher in INFH group than those in control group, while the expressions of ACE2 and Mas-R were lower than those of control group. There were significant differences in the percentage of positive expression area of ACE, AT1R, ACE2 and Mas-R between two groups (P < 0.05). Conclusion Some components of RAS biaxial in local bone tissue of INFH patients are abnormally expressed, which may be involved in the process of local bone turnover and osteonecrosis, providing new ideas for the follow-up study and diagnosis and treatment.

37 The role of microglia in the development of white matter damage
XU Manman CHAO Bin CUI Hanming PAN Juhua ZHANG Qiuyan HUANG Shijing
Microglia is widely distributed in the central nervous system and is the inherent immune cells. The nerve inflammation mediated by microglia is an important factor that cause white matter damage. More and more evidences show that the activation pathways of microglia in different white matter damage diseases are different, which is the basis of their complex role in the central nervous system. The typical manifestation of white matter damage is demyelination, which is a common pathological stage in multiple diseases including multiple sclerosis, Alzheimer′s disease, vascular cognitive impairment and dementia. In this article, the medical mechanism for microglia playing biological role through different activation pathways under various disease types and different brain regions is discussed. In addition, microglia can play a biphasic role in brain white matter damage, such as proinflammatory and immunomodulatory effects, therefore further research on these microglial subtypes may lead to new treatments that promote the repair of white matter damage.

45 Research progress of circular RNA in nervous system diseases
GAO Yongchao WANG Shuang WANG Jing TAI Xiantao
Circular RNA (circRNA) is a long-chain non-coding RNA molecule, which is widely found in eukaryotic cells. It is highly conserved in expression and has the functions of sponge adsorption, protein binding and protein translation. CircRNA can be involved in the regulation of a variety of diseases, such as tumors, cardiovascular diseases and nervous system diseases. Recent studies have found that circRNA is highly expressed in the nervous system and has tissue specificity, especially in the brain tissue. It is involved in the development and signal transmission of the nervous system and regulates the complex functions and various neural activities of the brain. CircRNA is closely associated with the development of nervous system diseases, and it has been found to be involved in cerebral palsy, alzheimer′s disease, and stroke. CircRNA is stable in structure, has a long half-life in exosomes, and is not easily degraded. Therefore, circRNA is considered as an ideal biomarker for monitoring a variety of nervous system diseases. This paper reviews the biological functions of circRNA and its application in nervous system diseases.

52 Application progress of “all manic and all belong to fire” in the treatment of hyperthyroidism
LI Chaolin1 JIANG Deyou2
“All manic and all belong to fire” comes from the nineteenth items of pathogenesis in Huangdi Neijing, which is still an important theoretical basis for the analysis of pathogenesis. According to clinical characteristics of hyperthyroidism, the pathogenesis is closely related to fire pathogen. Fire pathogen is the disease in the early stage of hyperthyroidism, and it damages yin for a long time. It is the syndrome of deficiency and excess, and fire evil is involved in the whole pathogenesis. This paper analyzes and summarizes the methods of treating hyperthyroidism by many doctors in recent years. All of them take the theory of “all manic and all belong to fire” as the pathogenesis theory to guide the treatment of hyperthyroidism. Whether it is classified and treated by stages, or by the treatment of prescriptions, all of them are based on the principle of clearing away heat and reducing firenourishing yin and reducing fire, which provides a reference for the future clinical treatment of hyperthyroidism by fire.

YU Yuncui1▲ DU Saifei2▲ ZHANG Li3 WEI Ting2 JIA Lulu1 MA Jinjing4 JIN Yan2 WANG Xiaoling4
Objective To understand the current status and problems of the clinical application of microecological preparations in the treatment of children with functional dyspepsia (FD) in China, and to provide the basis for the rational application of microecological preparations in the treatment of children with FD. Methods A retrospective selection of five Grade A children′s hospitals across the country (Beijing Children′s Hospital of Capital Medical University, Harbin Children′s Hospital, He′nan Children′s Hospital, Hu′nan Children′s Hospital, Jiangxi Provincial Children′s Hospital) would use microecological preparations to treat children from January to March 2019, while all outpatient/emergency prescriptions for FD were extracted. At the same time, the relevant guidelines/consensus of probiotics and FD at home and abroad were sorted out, and the treatment of children with FD by probiotics were investigated and analyzed. Results A total of 14 099 FD prescriptions for the treatment of children with microecological preparations were drawn, of which 56.43% were males (7956/14 099) and females accounted for 43.57% (6143/14 099). The proportion of children aged one to three years was the highest, reaching 28.58%. A total of 17 kinds of microecological preparations were involved in the prescriptions. Among them, the prescription number of Clostridium Butyricum Duojun San was the highest, reaching 64.65% (9115/14 099). The proportion of prescriptions with combined drugs accounted for 59.69% (8415/14 099). The combined drugs mainly included Chinese patent medicines for strengthening the spleen and eliminating food, Chinese patent medicines for clearing away heat and detoxification, and antibacterial drugs. The incidence of off-label medication prescriptions was 27.68% (3903/14 099), and there were mainly cases of over-indications, over-population, over-usage and dosage. A total of 15 domestic and foreign microecological preparations and FD-related clinical guidelines/consensus have been sorted out. In terms of effectiveness, only two domestic expert consensus suggested that probiotics can be used as adjuvant drugs for the treatment of FD. The recommended varieties mainly include double Fidobacterium, Lactobacillus, etc. In terms of safety, the seven guidelines and consensus mention and generally affirm the safety of probiotics, however none of them separately mentions the safety of probiotics in treating children with FD. Conclusion The application of microecological preparations to treat children with FD is common clinically, but there are still cases of off-label use of drugs, and there is little evidence-based medicine. Clinicians should standardize their prescription behavior. At the same time, a large number of multi-center prospective studies based on pediatric groups are still needed in the future to provide more support for the rational application of probiotics in the treatment of children with FD.

115 Correlation study between renal injury and humoral immunity in patients with rheumatoid arthritis, systemic lupus erythematosus and primary Sjogren′s syndrome
GUO Huijuan1 TIAN Bingchao2 TANG Jinyang3 LI Qi1
Objective To explore the correlation between renal injury and humoral immune in patients with rheumatoid arthritis (RA), systemic lupus erythematosus (SLE) and primary Sjogren′s syndrome (pSS). Methods Eighty-three cases of RA as RA group, 40 cases of SLE as SLE group and 50 cases of pSS as pSS group treated in Department of Rheumatology, Xiyuan Hospital, Chinese Academy of Traditional Chinese Medicine from March 2018 to November 2018. Another 50 healthy persons in the same period were selected as control group. Serum immunoglobulin (IgG, IgA), urinary β2-microglobulin (β2-MG), α1-microglobulin (α1-MG), microalbumin (mALB), creatinine (Cr) and ACR were detected. Pearson analysis was used to analyze the correlation between renal injury index and humoral immunity. Results The levels of serum IgG, IgA, urinary β2-MG and α1-MG in pSS group, SLE group and RA group were higher than those in control group (P < 0.05). Urine Cr of pSS group and RA group was lower than that of control group, and ACR was higher than that of control group (P < 0.05). There was no significant difference in urine mALB level among four groups (P > 0.05). Serum IgA, β2-MG and IgG were positively correlated in RA group (P < 0.05). Serum IgA, mALB, α1-MG, β2-MG were positively correlated with IgG in SLE group (P < 0.05). There was a positive correlation between serum IgA, β2-MG and IgG in pSS group (P < 0.05). Conclusion The immunological indexes of RA, SLE and pSS patients are significantly increased, which indicated that humoral immunity might be activated in different degrees, leading to immune dysfunction. The increase of renal injury index indicates that RA, SLE and pSS may have different degrees of renal injury. The positive correlation between renal injury index and immunologic index indicates that serum immunoglobulin (IgG, IGA) may participate in the pathological process of renal injury, which can provide certain reference value for clinical diagnosis and treatment of patients.

128 Professor Chen Xianhai′s experience of differentiation and treatment of pulmonary diseases based on perpetual-motion thinking
MA Jialu1 CHEN Xianhai2
Professor Chen Xianhai, under the influence of medical classics such as Shanghan Lun, combined with his own clinical experience for many years, believes that, in Shanghan Lun, perpetual-motion thinking plays an important guiding role in the treatment of pulmonary diseases and runs through each stage of the disease, pulse, syndrome and treatment. Clinical thinking such as “the differentiating and treating hierarchically” “truncated therapy” and “strengthening body resistance” based on perpetual-motion thinking can often guide the clinical treatment. Perpetual-motion thinking has unique advantages in reducing symptoms, and the clinical results are quite good. This has brought new thinking to the prevention and cure of pulmonary diseases, and is worthy of learning and reference.

131 Mechanism of kidney-reinforcing mass-disintegrating and removing blood stasis method in the treatment of IgA nephropathy
AN Mi1 TIAN Yun2
IgA nephropathy (IgAN) is an immune-mediated primary glomerular disease with a high incidence and no unified treatment plan. By discussing the theoretical basis of IgAN in traditional Chinese medicine and combining it with the microscopic pathological changes of the kidneys, it is clarified that the basic pathogenesis of IgAN is qi-deficiency and stasis of the collaterals in kidney and that the treatment of IgAN is kidney-reinforcing mass-disintegrating and removing blood stasis. With the studies of the pathogenesis of Western medicine of IgAN and the modern laboratory researches of kidney-reinforcing mass-disintegrating and removing blood stasis, it is found that this method can relieve the clinical symptoms of IgAN, prevent and cure the pathological changes of the kidney to delay the progression of the disease, which approves the efficacy of traditional Chinese medicine in the treatment of this disease and provides a reference for clinical treatment.

139 Director Wang Junling’s experience in the treatment of early threatened abortion combined with subchorionic hematoma with Jiaoai Decoction
LIU Yulei NING Yan TENG Hui YU Donghui WANG Junling
According to Director Wang Junling, the main pathogenesis of fetal leakage and threatened abortion is the loss of spleen and kidney, qi and blood, chong and conception channels, of which deficiency of kidney qi is the main cause. Subchorionic hematoma belongs to blood accumulation of menstrual separation and belongs to the category of “static blood”. The main pathogenesis of early threatened abortion combined with subchorionic hematoma is kidney deficiency and blood stasis, kidney deficiency is the foundation and blood stasis is the standard. The main treatment is to tonify kidney qi, and to activate blood circulation to stop bleeding. It is emphasized that blood stasis does not go away and blood does not return to meridians. Only proper blood circulation can achieve the purpose of hemostasis. In clinical practice, Jiaoai Decoction is commonly used to activate blood circulation and stop bleeding, Shoutai Pills to tonifying kidney for preventing miscarriage, and at the same time, attention should be paid to the observation of the condition of the disease and treat it according to the syndrome, so as to achieve the goal of “treating the disease and stabilizing the fetus simultaneously”.

LI Xin′ai1 QI Shuo2 CHEN Xiaoheng2 LI Zhe2 LI Huilong2 DING zhiguo2 YANG Yapeng2
Nodular goiter in traditional Chinese medicine is called “gall”. It is traditionally believed that the pathogenesis of this disease is based on qi stagnation, syndrome of intermingled phlegm and blood stasis is main pathogenesis. For the disease syndrome differentiation, experts and scholars in different areas have different opinions. However, this disease has its own law of occurrence, development and evolution. On the basis of deficiency of vital qi and dysfunction of Zang Fu organs, qi stagnation, phlegm coagulation and blood stasis occur. Thyroid diseases often involve multiple systems in the whole body, that is, the disease is located in the thyroid gland, involving multiple systems, and the thyroid gland is integrated with the whole body, that is, “dimple corresponding”. This paper will start from the pathogenesis, clinical manifestations, treatment prescriptions, expounds the academic thought of “dimple corresponding” theory in the treatment of nodular goiter.

JIANG Sijing1 WANG Jianchun2 CHEN Ling2 ZHOU Yuteng2 CAI Bingqin2
With the development of social economy and the popularization of thyroid ultrasound during the national health checkup, thyroid malignant tumors have become the fastest growing malignant tumors in the world. The diagnosis and treatment of thyroid malignant tumors are also receiving more and more attention. Generally speaking, surgical operations treatment is the clinically recognized preferred treatment method. The treatment feature of removing cancerous lesions directly is also one of the traditional Chinese medicine “eliminate evils” methods, which can clearly help patients with thyroid malignant tumors to provide a higher survival rate. The principle of dialectical treatment of traditional Chinese medicine believes that each patient with thyroid malignant tumor should be provided with a more comprehensive and individualized treatment plan. Professor Cai Bingqin adheres to the theory of traditional Chinese medicine treatment, combining “eliminating pathogen and strengthening vital qi” “strengthening vital qi to eliminate pathogenic factor” “conditioning qi movement” “medicine channel tropism” “balance of Yin and Yang” and other classic Chinese medicine principles in the perioperative period of thyroid cancer has achieved good clinical efficacy in the treatment of traditional Chinese medicine.
